Next-generation sequencing (NGS) has been implemented in clinical oncology for diagnosis, prognosis, and therapeutic guidance. Among the various NGS applications in molecular oncology, we focused on the following topics: laboratory standards for targeted gene panels (somatic mutations) and therapeutic guidance based on NGS of lung cancer and rare cancers, namely sarcomas and cancers of unknown primary. Multiple quality control checkpoints should be addressed in the pre-analytical phase for good quality and interpretation of the NGS results. It includes tumor size and cellularity, tissue processing and decalcification, tumor fraction, tumor viability, fixatives, and staining. Communication between clinicians and laboratory support is also essential. In lung cancer, all patients with non-squamous non-small cell lung cancer should be tested with a NGS panel, and it should include not only genes with approved targeted therapies (ALK, BRAF, EGFR, MET, NTRK, RET, and ROS1) but also genes with potentially actionable genomic alterations (HER2 and KRAS). Since there is a lack of extensive knowledge regarding the use of NGS in rare tumors performing comprehensive genomic profiling, NGS panels to better manage the disease are recommended. Moreover, other patients with other incurable solid tumors may benefit from being included in biomarker-driven clinical trials. Multidisciplinary tumor boards with the participation of experts with the ability to integrate genomic profiling data are essential to tailor the best strategy for each patient. Considering that there are no national guidelines, this article aims to guide laboratory and clinical practice for the use of NGS in the context of lung cancer, rare tumors, and cancer of unknown primary in Portugal.

Neste texto, apresenta-se um estudo sobre o impacto que o ensino explícito e sistemático do processo de escrita e dos seus subprocessos: a planificação, a textualização, a revisão e a edição tem na qualidade de textos expositivos produzidos por alunos de 4.º ano de escolaridade, em Portugal. Foi desenvolvida uma intervenção no sentido de ensinar explicitamente estes subprocessos, aplicados aos textos expositivos. O desenho investigativo contemplou um diagnóstico da situação através da realização de um pré-teste; uma intervenção didática, visando implementar um conjunto de sequências didáticas em que a dimensão processual da escrita era objeto de ensino explícito; e, por último, a realização de um pós-teste para avaliar o impacto da intervenção na qualidade dos textos. Constata-se que, a partir de uma análise mista dos pré e pós-testes, houve uma progressão do grupo experimental, no que se refere à interiorização da dimensão processual da escrita e à qualidade dos textos. Angiosarcoma is a rare malignant vascular tumor. Pulmonary involvement is usually attributable to metastasis from other primary sites, primary pulmonary angiosarcoma therefore being quite uncommon. We report a case of angiosarcoma with pulmonary involvement, probably primary to the lung, which had gone untreated for more than two years. We describe this rare neoplasm and its growth, as well as the extensive local invasion and hematogenous metastasis at presentation. We also discuss its poor prognosis.

O cancro do pulmão é a causa mais comum de morte por doença oncológica. Os doentes com diagnóstico de neoplasia - qualquer que seja a localização - têm risco acrescido de desenvolver um segundo tumor. Pretendeu-se caracterizar uma população com neoplasia do pulmão e o diagnóstico síncrono ou metácrono de tumor primário com outra localização. Foi feito um estudo retrospectivo, referente ao período de 2000-2007, analisando-se os processos clínicos de doentes seguidos na Unidade de Pneumologia Oncológica do nosso hospital em que estavam referenciadas duas neoplasias. Dos doentes seguidos na Unidade (n=1046) no período referido, 4,2% (n=44) tinham evidência de duas neoplasias. A maioria (88,6%) dos doentes eram homens, com idade média elevada (70,1±10 anos). Oitenta e seis por cento (n=38) tinham hábitos tabágicos e 65,4% dos doentes com registo de história familiar de neoplasia (n=26) tinham história familiar relevante (n=17). A doença oncológica manifestou-se em primeiro lugar na próstata, cólon, cabeça e pescoço ou bexiga. O cancro do pulmão foi em geral a segunda neoplasia. O intervalo de tempo médio entre a primeira e a segunda neoplasia foi de 62,9±64,9 meses (max: 240, min: 0), sendo que geralmente a segunda neoplasia foi detectada em estádio avançado. A sobrevivência média foi de 8,6±8,24 meses (max: 32; min: 1), permanecendo vivos quatro doentes. Os presentes dados levam-nos a sugerir vigilância clínica prolongada em doentes com diagnóstico prévio de cancro. Sugerimos um algoritmo que possibilite vigiar melhor estes doentes e estudo prospectivo para verificar se existem marcadores moleculares nesta população.Lung cancer is the leading form of cancer death worldwide. Cancer patients are at a high risk of developing a second cancer. The present study attempts to determine the characteristics of a population with lung cancer diagnosed with another cancer. We analysed records of patients from the Department of Lung Oncology of our hospital from 2000 to 2007 who were identified as having two or more tumours. We found 4.2% (n=44) multiple cancers among the registered cases (n=1046), 88.6% males, (high) mean age 70.1±10 years old. About 86% (n=38) of the patients were smokers or ex -smokers. From the patients with record of family history, 65.4% (n=17) had relevant family history of cancer. The majority of the first malignancy diagnosed was prostate, colon, head and neck and bladder. Lung cancer was essentially the second malignancy. The mean time lag between the two diagnoses was 62.9±64.9 months (max. 240, min. 0), with the second cancer usually detected at an advanced stage. Resumo: Descreve-se o caso clínico de um doente do sexo masculino com uma massa mediastínica de crescimento lento. Após terem-se efectuado vários exames complementares, foi através de cirurgia que se confirmou o diagnóstico de fibrose idiopática do mediastino. A remoção parcial da massa permitiu uma estabilização transitória das suas dimensões. Cerca de quatro anos depois, verificou-se um crescimento marcado com repercurssões importantes, através da inevitável compressão de estruturas vitais, que conduziram a hipertensão pulmonar grave. A fibrose idiopática do mediastino é uma entidade extremamente rara, estando descritas associações a outras patologias, mais frequentemente às auto-imunes, mas apresenta sempre um curso fatal quando não é possível a cirurgia.Rev Port Pneumol 2005; XI (2): 155-163 Abstract: We herein report a case of a male patient, who presented a mediastinal mass that had a slow growth over the years. After surgery, that made the diagnosis of idiopathic mediastinal fibrosis, the growth stabilized. Four years latter it was observed a substantial increase which lead to the inevitable compression of vital structures which, in turn, led to pulmonary hypertension. Idiopathic mediastinal fibrosis is an extremely rare pathology. Cancer is the second most important cause of death in Portugal, following cardiovascular diseases (CVD), and shows a constant progressive increase in the proportional share of total deaths. In Portugal, as in most countries, the health care budget is under constant cost-containment pressures. In this context it is necessary to verify if enough resources have been allocated to the disease in terms of health care expenditure. The main objective of this study is to estimate the cost of cancer care in Portugal and to compare it to similar data in Europe and the United States of America (USA), to the cost of CVD. The secondary objective is to evaluate the cost of pharmaceuticals used in the treatment of cancer in Portugal, both in relation to total pharmaceutical expenditure and to other therapeutic areas.Three main sources of information were used: comprehensive literature review, primary and secondary data sources, and a modified Delphi Panel, which was used to fill in gaps in the information derived from the data sources and the literatura review. The burden of cancer was measured through the Disability-adjusted life-year (DALY) and, in order to determine the costs of cancer, detailed information on the costs of medical visits and of inpatient episodes based on Diagnosis Related Groups (DRG), in 2006, was used. To estimate the total cost of cancer, we used a combination of top down (breaking global expenditure data to specific levels) and bottom up methodology (based on the sum of different components).Based on 2006 data on direct medical care expenditures in Portugal, we found that 565 million euro were spent on cancer in comparison to 1 320 million on CVD representing 3.91% and 9.14% of total cost on health respectively. When we break down total expenditure on drugs by therapeutic area we find that CVD drugs represent about 21.6% of total drug costs in Portugal and cancer drugs represent about 5.6% of the total. Oncology drugs represent 32% of the total expenditure on cancer, while CVD drugs represent 54% of the total expenditure on CVD. In comparison, in terms of BoD in Portugal, 18.6% of DALY's were associated with CVD and 15.3% with cancer.Considering the burden of disease (BoD) of CVD and cancer in Portugal, we can state that the expenditure allocated to cancer is significantly lower than expected. Using the criterion of expenditure according to need, we observed that there is an imbalance of expense/BoD in oncology indicating that cancer seems to be underfunded in Portugal. Even considering that this shouldn't be the only criterion to determine the volume of expense in a certain therapeutic area, the differential observed in this study is sufficiently high to deserve attention from the decision-makers.

O erlotinib em monoterapia após falência de pelo menos um esquema de quimio-terapia prévia prolonga a sobrevivência e melhora a qualidade de vida, embora com modesta taxa de resposta. As mulheres, não fumadores, adenocarcinomas e asiáticos estão associados a melhor resposta. É neste grupo de doentes que mais frequentemente se encontra a mutação do EGFR. The Brazilian Institute of Geography and Statistics (IBGE) is, with INEGI in Mexico are the only ones in the world to claim the "geography". But is it a scientific guarantee for researchers who implement local census data? The large number of territorial categories of official statistics of present-day Brazil reveals a great wealth of data, provided they are not considered as a "turnkey" material for research.From a pre-statistical European model inherited from the colonial period to the rationality of the state, then to the liberal and post-modern context that spread in the globalized world, the IBGE categories reveal the paradoxes due to the superposition of different points of view on how to characterize the urban space. This translates into difficulties in understanding and using aggregated population data on official spatial and urban categories. The avatars of the current situation are symptomatic of the paradigm of complexity that increasingly influences the quantitative approach in the social sciences and more generally reflects a break between citizens, policies and the logic of experts. Based on an in-depth analysis of territorial categories produced by the IBGE, the article uses these sources to understand the criteria that prevail in the divisions of space and in the definition of the urban space by the institution in order to assess the interest and limitations of using these sources of information. This analysis should allow a better understanding and better use of the rich dataset spread by the national statistical services.

Niemann–Pick type C (NPC) is a lysosomal storage disorder (LSD) caused by pathogenic variants in either the NPC1 or NPC2 genes, which encode proteins involved in the lysosomal export of unesterified cholesterol. In patients of Western European descent, the p.Ile1061Thr variant in NPC1 is especially prevalent. However, mounting evidence has positioned p.Ala1035Val as the most common variant in Portugal and the second most prevalent variant worldwide. By analyzing 10 Portuguese NPC patients homozygous for p.Ala1035Val, we found an SNP in cis on position 858 (p.Ile858Val), which we hypothesize could have a disease-modifying effect. To address this query, we created variant-specific in vitro models of NPC by stably transducing NPC1−/− ARPE-19 cells with constructs encoding different fluorescently-tagged variants of NPC1, which we used, alongside patient-derived skin fibroblasts, to investigate lysosomal positioning and the trafficking routes elicited by p.Ile1061Thr and p.Ala1035Val (with and without the p.Ile858Val SNP in cis). Our results corroborate the previously described decrease in p.Ile1061Thr-NPC1 trafficking to the lysosome and suggest a similar, if not worse, scenario for the p.Ala1035Val variant, especially when in cis with p.Ile858Val. This is the first reported functional study addressing the impact of the p.Ala1035Val variant at the cellular level, paving the way for novel therapeutic options. [ABSTRACT FROM AUTHOR]

Introduction: A promising approach to optimizing recovery in youth football has been the use of machine learning (ML) models to predict recovery states and prevent mental fatigue. This research investigates the application of ML models in classifying male young football players aged under (U)15, U17, and U19 according to their recovery state. Weekly training load data were systematically monitored across three age groups throughout the initial month of the 2019–2020 competitive season, covering 18 training sessions and 120 observation instances. Outfield players were tracked using portable 18-Hz global positioning system (GPS) devices, while heart rate (HR) was measured using 1 Hz telemetry HR bands. The rating of perceived exertion (RPE 6–20) and total quality recovery (TQR 6–20) scores were employed to evaluate perceived exertion, internal training load, and recovery state, respectively. Data preprocessing involved handling missing values, normalization, and feature selection using correlation coefficients and a random forest (RF) classifier. Five ML algorithms [K-nearest neighbors (KNN), extreme gradient boosting (XGBoost), support vector machine (SVM), RF, and decision tree (DT)] were assessed for classification performance. The K-fold method was employed to cross-validate the ML outputs. Results: A high accuracy for this ML classification model (73–100%) was verified. The feature selection highlighted critical variables, and we implemented the ML algorithms considering a panel of 9 variables (U15, U19, body mass, accelerations, decelerations, training weeks, sprint distance, and RPE). These features were included according to their percentage of importance (3–18%). The results were cross-validated with good accuracy across 5-fold (79%). Conclusion: The five ML models, in combination with weekly data, demonstrated the efficacy of wearable device-collected features as an efficient combination in predicting football players' recovery states. Background/Objectives: This study examined the effects of an intervention on anthropometrics, body composition, physical fitness, and sleep quality in aged individuals, comparing a control group (N = 11) and an experimental group (N = 13) across two measurement points. Methods: A multicomponent training program of 8 months was adopted as the intervention group. A bioimpedance balance, functional fitness test, and Pittsburgh Sleep Quality Index measured body composition, functional fitness, and sleep quality. Results: Both groups showed minimal changes in body mass and hand grip strength. However, the experimental group experienced significant improvements in physical fitness, including a 26% increase in arm curl repetitions, an 18% reduction in 5 times sit-to-stand (5TSTS) completion time, and a 29% rise in 2-min step test (2MST) steps, indicating enhanced muscle endurance and cardiovascular fitness. Flexibility decreased significantly in the experimental group, while body fat percentage was reduced by 10%. Sleep quality improved by 47% in the experimental group but declined by 14% in the control group. Correlational analysis revealed that better sleep quality was linked to improved fitness performance and reduced body fat in the experimental group, with post-intervention results further confirming the connection between sleep and fat reduction. In the control group, improved sleep quality was associated with higher metabolic rates after 8 months. Conclusions: These findings suggest that the intervention positively impacted physical fitness and sleep quality, with potential benefits for overall health. [ABSTRACT FROM AUTHOR]

This paper presents the control and hardware design for a Hybrid Solid-State Transformer (HSST) applied to modern distribution systems. The HSST combines the advantages of conventional transformers, such as high efficiency and low cost, with those of Solid-State Transformers (SST), such as multifunctionality and fast dynamic response. Real-time simulation using a Typhoon HIL404 device is performed to validate the proposed multifunctional control strategy. The corresponding results validate the HSST's capability to provide, while only processing partial system power, a regulated output voltage with grid voltage harmonic suppression and, at the same time, current load reactive and harmonic compensation to ensure a high-power factor and improved power quality to the grid. Though the mechanisms are not fully understood, tryptophan (Trp) and physical exercise seem to regulate mechanical hypersensitivity in fibromyalgia. Here, we tested the impact of Trp supplementation and continuous low-intensity aerobic exercise on the modulation of mechanical hypersensitivity in a fibromyalgia-like model induced by acid saline in female rats. Twelve-month-old female Wistar rats were randomly divided into groups: [control (n = 6); acid saline (n = 6); acid saline + exercise (n = 6); acid saline + Trp (n = 6); and acid saline + exercise + Trp (n = 6)]. Hypersensitivity was caused using two intramuscular jabs of acid saline (20 μL; pH 4.0; right gastrocnemius), 3 days apart. The tryptophan-supplemented diet contained 7.6 g/hg of Trp. The three-week exercise consisted of progressive (30–45 min) treadmill running at 50 to 60% intensity, five times (Monday to Friday) per week. We found that acid saline induced contralateral mechanical hypersensitivity without changing the levels of Trp, serotonin (5-HT), and kynurenine (KYN) in the brain. Hypersensitivity was reduced by exercise (~150%), Trp (~67%), and its combination (~160%). The Trp supplementation increased the levels of Trp and KYN in the brain, and the activity of indoleamine 2,3-dioxygenase (IDO), and decreased the ratio 5-HT:KYN. Exercise did not impact the assessed metabolites. Combining the treatments reduced neither hypersensitivity nor the levels of serotonin and Trp in the brain. In conclusion, mechanical hypersensitivity induced by acid saline in a fibromyalgia-like model in female rats is modulated by Trp supplementation, which increases IDO activity and leads to improved Trp metabolism via the KYN pathway. In contrast, physical exercise does not affect mechanical hypersensitivity through brain Trp metabolism via either the KYN or serotonin pathways. ABSTRACT The goal of this paper is to discuss representations about grammar teaching of Portuguese L1 teachers. It draws on the exploratory study designed to identify the invariants and divergences in self-perceptions about L1 grammar teaching of trainees in different training situations: in-service teachers, pre-service teachers and bachelor’s students. The study focus on student’s and teacher’s acknowledgment of the Portuguese L1 Curriculum (Reis et al. 2009), which conveys a clear paradigm change in Portuguese language education, assuming that grammar teaching is based upon language awareness development. The results show an evident gap between students and teachers in what concerns the familiarity with the Portuguese L1 Curriculum and teaching strategies. However, some puzzling convergent points are the importance attributed to grammar and poor linguistics background. The results analysis draws attention to the relationship between teacher cognition and practice, highlighting fragilities in teacher education and potential conceptual changes that may lead to effective changes in classroom practices.

The increasing use of nonlinear loads in power systems introduces voltage and current components at non-fundamental frequencies, leading to harmonic distortion, which negatively impacts electrical and electronic devices. A common mitigation strategy involves identifying harmonic sources and installing filters nearby. However, due to the high cost of power quality (PQ) meters, comprehensive harmonic level monitoring across the entire power system is impractical. To address this, various methodologies for Harmonic State Estimation (HSE) have been developed, which estimate distortion levels on unmonitored system buses using data from a minimal set of monitored ones. Many HSE techniques rely on optimization algorithms with numerous tuning parameters, complicating their application. This paper proposes a novel methodology for fundamental frequency power flow and harmonic state estimation using the Jaya algorithm, which is characterized by fewer tuning parameters for easier adjustment. It also introduces a strategy to determine the minimal number of buses that need monitoring to achieve system observability. The methodology is validated on the IEEE-14 and IEEE-30 bus systems, demonstrating its effectiveness. The results of the proposed methodology are compared with those obtained using Evolutionary Strategies (ESs), highlighting its enhanced accuracy and computational efficiency. [ABSTRACT FROM AUTHOR]

Background: The study evaluated the effects of resistance exercise training and açaí supplementation on cardiac parameters in hypertensive animals. Methods: For this study, rats from the Wistar and SHR lines (spontaneously hypertensive rats) were used. The animals were divided into 5 groups: Wistar Control (C); Control Hypertensive (H); Trained Hypertensive (HT); Hypertensive and Supplemented with Açaí (HA); and Hypertensive Trained and Supplemented with Açaí (HAT). Resistance exercise training was carried out through climbing. The supplemented groups received 3 g of açaí/kg of body mass. The animals' systolic blood pressure (SBP), body mass, and physical test were measured at the beginning and end of the intervention. At the end, an echocardiographic analysis was performed. Histological analysis and oxidative stress of the LV were performed. Results: It was found that hypertensive animals showed an increase in SBP, and the treatments reduced this parameter. The trained groups achieved higher values of maximum carrying load. Hypertension increased the dimension of the left ventricular free wall in diastole and reduced ejection and shortening fractions. The trained groups showed improvement in ejection and shortening fractions. The H group increased the proportion of extracellular matrix and reduced the proportion of cells, with the HAT group attenuating this change. Cell diameter was greater in group H, and all treatments reduced this parameter. Hypertension increased the concentration of malondialdehyde and decreased catalase activity in LV. The treatments managed to mitigate this damage. Conclusions: It is concluded that the treatments managed to generate positive cardiovascular adaptations, and their combination enhanced these effects. The aim of this study was to analyze the match-to-match variation in high-intensity demands from one Portuguese professional football team according to playing positions. Twenty-three male outfield professional football players were observed during eighteen matches of the Portuguese Second League. Time–motion data were collected using Global Positioning System (GPS) technology. Match running performance was analyzed based on the following three playing positions: defenders (DF), midfielders (MF), and forwards (FW). Repeated measures ANOVA was utilized to compare match running performance within each position role, and seasonal running variation. Practical differences were assessed using the smallest worthwhile change (SWC), coefficient of variation (CV), and twice the coefficient of variation (2CV). Significant differences were found among playing positions in total distance covered (F = 15.45, p < 0.001, η2 = 0.33), average speed (F = 12.79, p < 0.001, η2 = 0.29), high-speed running (F = 16.93, p < 0.001, η2 = 0.36), sprinting (F = 13.49, p < 0.001, η2 = 0.31), accelerations (F = 4.69, p = 0.001, η2 = 0.132), and decelerations (F = 12.21, p < 0.001, η2 = 0.284). The match-to-match running performance encompassed TD (6.59%), AvS (8.67%), HSRr (37.83%), SPR (34.82%), ACC (26.92%), and DEC (27.85%). CV values for total distance covered ranged from 4.87–6.82%, with forwards and midfielders exhibiting the greatest and smallest variation, respectively. Midfielders demonstrated the highest match-to-match variation for all other analyzed variables (8.12–69.17%). All playing positions showed significant variation in high-demanding variables (26.94–37.83%). This study presents the initial analysis of match-to-match variation in high-intensity demands within a Portuguese professional football team. Thus, the position's specificity and context can provide a helpful strategy for evaluating match-to-match running performance, and for recommending individualized training exercises based on the peak and high-intensity demands for each player's role within the game. The aim of this study was to test a machine learning (ML) model to predict high-intensity actions and body impacts during youth football training. Sixty under-15, -17, and -19 sub-elite Portuguese football players were monitored over a 6-week period. External training load data were collected from the target variables of accelerations (ACCs), decelerations (DECs), and dynamic stress load (DSL) using an 18 Hz global positioning system (GPS). Additionally, we monitored the perceived exertion and biological characteristics using total quality recovery (TQR), rating of perceived exertion (RPE), session RPE (sRPE), chronological age, maturation offset (MO), and age at peak height velocity (APHV). The ML model was computed by a feature selection process with a linear regression forecast and bootstrap method. The predictive analysis revealed that the players' MO demonstrated varying degrees of effectiveness in predicting their DEC and ACC across different ranges of IQR. After predictive analysis, the following performance values were observed: DEC ( x ¯ predicted = 41, β = 3.24, intercept = 37.0), lower IQR (IQRpredicted = 36.6, β = 3.24, intercept = 37.0), and upper IQR (IQRpredicted = 46 decelerations, β = 3.24, intercept = 37.0). The player's MO also demonstrated the ability to predict their upper IQR (IQRpredicted = 51, β = 3.8, intercept = 40.62), lower IQR (IQRpredicted = 40, β = 3.8, intercept = 40.62), and ACC ( x ¯ predicted = 46 accelerations, β = 3.8, intercept = 40.62). The ML model showed poor performance in predicting the players' ACC and DEC using MO (MSE = 2.47–4.76; RMSE = 1.57–2.18: R2 = −0.78–0.02). Maturational concerns are prevalent in football performance and should be regularly checked, as the current ML model treated MO as the sole variable for ACC, DEC, and DSL. Applying ML models to assess automated tracking data can be an effective strategy, particularly in the context of forecasting peak ACC, DEC, and bodily effects in sub-elite youth football training. Utilizing techniques for reducing multivariate data is essential for comprehensively understanding the variations and relationships within both biomechanical and physiological datasets in the context of youth football training. Therefore, the objective of this study was to identify the primary factors influencing training sessions within a standard microcycle among young sub-elite football players. A total of 60 male Portuguese youth sub-elite footballers (15.19 ± 1.75 years) were continuous monitored across six weeks during the 2019–2020 in-season, comprising the training days from match day minus (MD-) 3, MD-2, and MD-1. The weekly training load was collected by an 18 Hz global positioning system (GPS), 1 Hz heart rate (HR) monitors, the perceived exertion (RPE) and the total quality recovery (TQR). A principal component approach (PCA) coupled with a Monte Carlo parallel analysis was applied to the training datasets. The training datasets were condensed into three to five principal components, explaining between 37.0% and 83.5% of the explained variance (proportion and cumulative) according to the training day (p < 0.001). Notably, the eigenvalue for this study ranged from 1.20% to 5.21% within the overall training data. The PCA analysis of the standard microcycle in youth sub-elite football identified that, across MD-3, MD-2, and MD-1, the first was dominated by the covered distances and sprinting variables, while the second component focused on HR measures and training impulse (TRIMP). For the weekly microcycle, the first component continued to emphasize distance and intensity variables, with the ACC and DEC being particularly influential, whereas the second and subsequent components included HR measures and perceived exertion. On the three training days analyzed, the first component primarily consisted of variables related to the distance covered, running speed, high metabolic load, sprinting, dynamic stress load, accelerations, and decelerations.
