Background: Helicobacter pylori infection is a major cause of gastric diseases in humans that its rapid, simple and low cost diagnosis is important. Diabetes mellitus increases the risk of Helicobacter pylori infection. Objective: The aim of this study was to compare diagnostic accuracy of serologic test with histology for Helicobacter Pylori in diabetic patients with dyspepsia. Methods: This analytical study was conducted on 183 diabetic patients with dyspepsia referred to Imam Hossain hospital of Shahroud in 2014. All patients underwent endoscopy. Standard biopsy was performed as gold standard which included six gastric and duodenal samples. Simultaneously, blood samples were taken for serologic test. The presence of Helicobacter pylori was examined using Giemsa staining in histological study. Anti-Helicobacter pylori IgG was measured using ELISA in serological study. Sensitivity, specificity, and positive and negative predictive values were calculated. Data were analyzed using Chi-square test. Findings: Of 183, 104 patients (57.1%) were positive for Helicobacter pylori in serological study and 151 patients (82.5%) were positive in histological study. The sensitivity, specificity, positive predictive value and negative predictive value of serologic test for diagnosis of Helicobacter pylori were 64.2%, 78.1%, 93.3%, and 31.6%, respectively. IgG cut point for diagnosis of Helicobacter pylori was set to be 1.4 (sensitivity: 92.4% and specificity: 75.3%). Conclusion: The serologic test can be used as screening test for Helicobacter pylori in diabetic patients with regards to relatively high sensitivity and specificity as well as its simplicity, rapidity, and low price.

The ‘pitchers’ of carnivorous pitcher plants are exquisite examples of convergent evolution. An open question is whether the living communities housed in pitchers also converge in structure or function. Using samples from more than 330 field-collected pitchers of eight species of Southeast Asian Nepenthes and six species of North American Sarracenia, we demonstrate that the pitcher microcosms, or miniature ecosystems with complex communities, are strikingly similar. Compared to communities from surrounding habitats, pitcher communities house fewer species. While communities associated with the two genera contain different microbial organisms and arthropods, the species are predominantly from the same phylogenetic clades. Microbiomes from both genera are enriched in degradation pathways and have high abundances of key degradation enzymes. Moreover, in a manipulative field experiment, Nepenthes pitchers placed in a North American bog assembled Sarracenia-like communities. An understanding of the convergent interactions in pitcher microcosms facilitates identification of selective pressures shaping the communities.

Calpain has been proposed to play a critical role in the development of epilepsy. Here we used conditional calpain-2 knock-out (C2CKO) mice in a C57/Bl6 background and a selective calpain-2 inhibitor to analyze the role of calpain-2 in epilepsy. Neurodegeneration was evident in various hippocampal subfields, in particular in mossy cells in the hilus of the dentate gyrus (DG) in C57/Bl6 mice 7 days after kainic acid (KA)-induced seizures. Calpain-2 activation was still observed in mossy cells 7 days after seizures. Calpain activation, astroglial and microglial activation, neurodegeneration, and cognitive impairment were absent in C2CKO mice and in C57/Bl6 mice treated with a selective calpain-2 inhibitor for 7 days after seizure initiation. Levels of the potassium chloride cotransporter 2 (KCC2) were decreased in mossy cells 7 days after seizures and this decrease was prevented by calpain-2 deletion or selective inhibition. Our results indicate that prolonged calpain-2 activation plays a critical role in neuropathology following seizures. Repeated concussion represents a serious health problem as it can result in various brain pathologies, ranging from minor focal tissue injury to severe chronic traumatic encephalopathy. The calcium-dependent protease, calpain, participates in the development of neurodegeneration following concussion, but there is no information regarding the relative contribution of calpain-1 and calpain-2, the major calpain isoforms in the brain. We used a mouse model of repeated concussions, which reproduces most of the behavioral and neuropathological features of the human condition, to address this issue. Deletion of calpain-2 or treatment with a selective calpain-2 inhibitor for 2 weeks prevented most of these neuropathological features. Changes in TAR DNA binding protein 43 (TDP-43) subcellular localization similar to those found in human amyotrophic lateral sclerosis and frontotemporal dementia were also prevented by deletion of calpain-2 or treatment with calpain-2 inhibitor.
