Abstract

Objective: We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy. Methods: A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered. Results: The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively. Conclusions: This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

Abstract

OBJECTIVE:\ud We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy.\ud \ud METHODS:\ud A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered.\ud \ud RESULTS:\ud The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively.\ud \ud CONCLUSIONS:\ud This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

Abstract

Until 2014, the management of invasive alien species has been poorly addressed in Europe. Recently, the Regulation 1143/14 on invasive alien species (IAS) was issued by the European Community. A first list of "IAS of Union Concern" was presented in 2016 and subsequently updated in 2017. The species included in that list are subjected to a number of commercial restrictions, and EU member states are required to control or eradicate them. The inclusion of an IAS in this list is based on the results of a risk-assessment procedure. At present, this list contains 49 IAS, none of which marine. Member states can decide either to adopt the EU list as it is, or to integrate it with additional "IAS of national concern". Italy has opted for the second way and is currently developing its own national list, which may eventually include marine IAS. Italy is a major "first hub" for marine introductions in the Mediterranean Sea, with over 200 multicellular marine alien species reported so far. Italy, with its central position in the Mediterranean Sea, shares several species with the Western European countries, carried by ships or with shellfish stocks, as well as a number of Indo-Pacific species with the Eastern Mediterranean countries, entered through the Suez Canal. Therefore, the roadmap that Italy is currently tracing towards the possible inclusion of marine species in the list of IAS to be managed and controlled, is also relevant for other European and Mediterranean countries. Abstract

The aim of this study was to investigate the benthic fauna associated to nearshore vermetid reefs. Sampling was conducted along the Mediterranean coast of Spain, in three different areas, using scraping techniques, to characterize the taxonomic assemblage living among and inside the Dendropoma empty shells. The results showed high abundance and very low taxa diversity.

Abstract

AbstractThe population structure of Jujubinus striatusand Jujubinus exasperatus(Trochidae), common gastropod grazers of Posidonia oceanicaleaves, was investigated in the seagrass bed off Lacco Ameno of Ischia (Gulf of Naples). Sampling was performed monthly, from July 1981 to June 1982, at five stations along a depth gradient (1, 3, 10, 15 and 25 m), using a hand-towed net. Throughout the study period, both populations were characterised by seasonal fluctuations in abundance, with a total of 550 individuals counted and measured. The two populations analysed showed different distributional trends along the depth gradient. Jujubinus striatussettled and occurred mainly at shallower stands (1–3 m depth), while J. exasperatuswas found deeper (10–15 m). Both species are semelparous. Juveniles and small individuals (shell height < 4 mm) were mainly found during the late winter and early spring months. The survivorship curves of both Jujubinusspecies showed a very low mortality rate in the first few months after settlement. Demographic analysis suggests an indeterminate growth rate after sexual maturity, with a lifespan of about 22 months for J. striatusand 17 months for J. exasperatus. For both species the growth rate is fast during the first couple of months after settlement. The findings of this study, compared with earlier work, reveal a certain number of shared traits in the life cycle of the most common gastropod species living on the leaves of P. oceanicaseagrass: semelparous reproduction, spring settlement, fast early growth and short lifespan (2 years maximum).

Abstract

An association between essential mixed cryoglobulinemia and hepatitis C virus infection has been documented by many reports. Some clinical manifestations such as purpura, arthralgia, vascular lesions and peripheral neuropathies are also connected with the presence of detectable cryoglobulins. The association between HCV infection, the presence of mixed cryoglobulinemia and peripheral neuropathy is well documented. The aim of this study was to define the possible presence of peripheral neuropathy in HCV patients without detectable cryoglobulins and the possible association with the different genotypes.Twenty patients (11 females, 9 males) with chronic HCV hepatitis and without detectable cryoglobulins were submitted to neurological and electrophysiological studies to detect a possible peripheral neurological involvement. In all patients the HCV infection was assumed by the presence of antibodies to HCV with ELISA assay and then confirmed with recombinant immunoblot assay. HCV genotyping was obtained by INNO LIPA in 15 out of 20 patients. In 4 patients a sural nerve biopsy was possible.Genotype 1b was present in 80% of patients, while 1a in 13.3% and 4 in 6.6%. Thirteen patients had positive neurological anamnesis (65%), while neurological examination was positive in 40% of the cases. Electromyographic study was positive in 50% of subjects. The sural nerve biopsies agreed with axonal degeneration in amyelinated fibres.Our results suggest a possible peripheral neurological system involvement in patients with HCV infection without cryoglobulins.

The survivorship curves of bothGibbulaspecies, derived from the size-frequency distribution of shells, showed a very low mortality rate in the first months after settlement. Demographic analysis ofG. umbilicarissuggests a rather slow growth rate and a polymorphic maturation strategy with a lifespan of 1 year for most individuals, but lifespan may also reach about 2 years in a few specimens.Gibbula ardenshas a lifespan of about 1 year and a fast growth rate, especially during the first couple of months after settlement. Abstract

Many different articular symptoms may appear in patients with HCV hepatitis, but in a relatively large number of patients no rheumatic symptoms are present. This sonographic study was undertaken to detect the possible presence of early articular changes in HCV patients without any rheumatic manifestations.The knee, hip and shoulder were evaluated in a cohort of 29 consecutive HCV patients without any rheumatic symptoms. Results were compared with those obtained by the evaluation of 29 healthy subjects who were negative for markers of HCV and HBV infections.Results showed the presence of alterations in 96.5% of the patients, with significant differences in comparison to controls (p0.0001). Slight inflammatory changes were found in all the joints examined. The knee was involved in 79.3% of the cases, the hip in 27.6% and the shoulder in 89.6%.Our preliminary study shows the presence of joint changes in the majority of cases. To the best of our knowledge this is the first ultrasonographic study to focus on joint evaluation in patients with HCV hepatitis. Broader epidemiological and virological investigations, in particular for the HCV subtype and HLA genotype, will be required to elucidate the relationship between HCV infection and rheumatic symptoms.

Abstract

Background: Few data are available on the virological and clinical outcomes of advanced liver disease patients retreated after first-line DAA failure. Aim: To evaluate DAA failure incidence and the retreatment clinical impact in patients treated in the advanced liver disease stage. Methods: Data on HCV genotype, liver disease severity, and first and second line DAA regimens were prospectively collected in consecutive patients who reached the 12-week post-treatment and retreatment evaluations from January 2015 to December 2016 in 23 of the PITER network centers. Abstract

Rhodolith beds (RBs) are bioconstructions characterized by coralline algae, which provide habitat for several associated species. Mediterranean RBs are usually located in the mesophotic zone (below 40 m), and thus are frequently remote and unexplored. Recently, the importance and vulnerability of these habitats have been recognized by the European Community and more attention has been drawn to their investigation and conservation. This study reports the results of an extensive monitoring program, carried out within the Marine Strategy Framework Directive (2008/56/EC), in six sites off the Campania coast (Italy, Mediterranean Sea). New insights were given into the distribution, cover, vitality (i.e., live/dead rhodolith ratio), structural complexity, and coralline algae composition of RBs. Remotely operated vehicles (ROV) investigations allowed the description of several RBs, and the discovery of a RB with rhodolith cover &gt, 65% offshore the Capri Island. Only two sites (Secchitiello and Punta Campanella) showed a very low mean cover of live rhodoliths (&lt, 10%), hence, not being classifiable as RBs. The collected rhodoliths were mostly small pralines (~2 cm), spheroidal to ellipsoidal, with growth-forms ranging from encrusting/warty to fruticose/lumpy. Coralline algae identification revealed a high diversity within each bed, with a total of 13 identified taxa. The genus Lithothamnion dominated all sites, and Phymatolithon calcareum and Lithothamnion corallioides, protected by the Habitats Directive (92/43/EEC), were detected in all RBs.

Abstract

The population structure of Jujubinus striatus and Jujubinus exasperatus (Trochidae), common gastropod grazers of Posidonia oceanica leaves, was investigated in the seagrass bed off Lacco Ameno of Ischia (Gulf of Naples). Sampling was performed monthly, from July 1981 to June 1982, at five stations along a depth gradient (1, 3, 10, 15 and 25 m), using a hand-towed net. Throughout the study period, both populations were characterised by seasonal fluctuations in abundance, with a total of 550 individuals counted and measured. The two populations analysed showed different distributional trends along the depth gradient. Jujubinus striatus settled and occurred mainly at shallower stands (1–3 m depth), while J. exasperatus was found deeper (10–15 m). Both species are semelparous. Juveniles and small individuals (shell height < 4 mm) were mainly found during the late winter and early spring months. The survivorship curves of both Jujubinus species showed a very low mortality rate in the first few months after settlement. Demographic analysis suggests an indeterminate growth rate after sexual maturity, with a lifespan of about 22 months for J. striatus and 17 months for J. exasperatus. For both species the growth rate is fast during the first couple of months after settlement. The findings of this study, compared with earlier work, reveal a certain number of shared traits in the life cycle of the most common gastropod species living on the leaves of P. oceanica seagrass: semelparous reproduction, spring settlement, fast early growth and short lifespan (2 years maximum).

Abstract

Population structures of the gastropods Gibbula umbilicaris and Gibbula ardens (Trochidae), the two most common species of the genus Gibbula living in shallow beds of the Mediterranean seagrass Posidonia oceanica, were investigated in the seagrass bed off Lacco Ameno of Ischia (Gulf of Naples), at six stations along a depth gradient (1, 3, 5, 10, 15 and 25m). Sampling was performed on a monthly basis for 1 year. A total of 853 individuals were counted and measured. The two gastropod populations analysed show a rather similar distributional trend in all seasons and depths. Recruitment occurred at the shallowest stations (1-3m depth), where the populations were characterised by higher values of abundance and biomass, confirming that G. umbilicaris and G. ardens are typical species of the shallow community of P. oceanica seagrass beds. Juveniles were exclusively found in 1 month of the whole year: G. umbilicaris in April, G. ardens in July. The survivorship curves of both Gibbula species, derived from the size-frequency distribution of shells, showed a very low mortality rate in the first months after settlement. Demographic analysis of G. umbilicaris suggests a rather slow growth rate and a polymorphic maturation strategy with a lifespan of 1 year for most individuals, but lifespan may also reach about 2 years in a few specimens. Gibbula ardens has a lifespan of about 1 year and a fast growth rate, especially during the first couple of months after settlement. Therefore, on the basis of lifespan and recruitment time, both species seem to be semelparous.

Abstract

Non-alcoholic fatty liver disease (NAFLD) is a rapidly emerging chronic liver disorder and will increasingly be a critical global health problem. Diet is an important pathogenic factor of NAFLD and it is well documented that the increased prevalence of NAFLD during the last decades was associated with deep modification of dietary habits, especially increased intakes of fats and simple carbohydrates. As the disease stems from excess calorie intake and lack of physical activity, the correction of unhealthy lifestyles is the basis of any prevention and treatment strategy while drugs should remain a second-line of treatment. NAFLD patients should receive counselling for a low carbohydrate and low saturated fat diet, avoidance of fructose-enriched soft drinks and increased consumption of fruits and vegetables. The use of functional foods in NAFLD has been poorly studied and up to day only few reports are encouraging and incite the promotion of functional food approach on NAFLD prevention on the basis of food and nutritional genomics.

Abstract

AIM: To evaluate the long-term eradication of hepatitis C virus (HCV) infection and liver-related complications in chronically infected patients that have achieved sustained virological response. METHODS: One hundred and fifty subjects with chronic hepatitis C (CHC) or cirrhosis and sustained virological response (SVR) between the years of 1989 and 2008 were enrolled in a long-term clinical follow-up study at the Gastrointestinal and Liver Unit of the University Hospital of Naples “Federico II”. At the beginning of the study, the diagnosis of HCV infection was made on the basis of serum positivity for antibodies to HCV and detection of HCV RNA transcripts, while a diagnosis of chronic hepatitis was formulated using imaging techniques and/or a liver biopsy. SVR was achieved by interferon-based therapy, both conventional and pegylated, with and without ribavirin treatment. The patients were evaluated for follow-up at a median length of 8.6 years, but ranged from 2-19.9 years. Among them, 137 patients had pre-treatment CHC and 13 had cirrhosis. The patients were followed with clinical, biochemical, virological, and ultrasound assessments on a given schedule. Finally, a group of 27 patients underwent a liver biopsy at the beginning of the study and transient elastography at their final visit to evaluate changes in liver fibrosis. RESULTS: The median follow-up was 8.6 years (range 2-19.9 years). HCV RNA remained undetectable in all patients, even in patients who eventually developed liver-related complications, indicating no risk of HCV recurrence. Three liver-related complications were observed: two cases of hepatocellular carcinoma and one case of bleeding from esophageal varices resulting in an incidence rate of 0.23%/person per year. Further, all three complications took place in patients diagnosed with cirrhosis before treatment began. Only one death due to liver-related causes occurred, resulting in a mortality rate of 0.077% person per year. This amounts to a 99.33% survival rate in our cohort of patients after therapy for HCV infection. Finally, of the 27 patients who underwent a liver biopsy at the beginning of the study, a reduction in liver fibrosis was observed in 70.3% of the cases; only three cases registering values of liver stiffness indicative of significant fibrosis. CONCLUSION: Patients with CHC and SVR show an excellent prognosis with no risk of recurrence and a very low rate of mortality. Our data indicate that virus-eradication following interferon treatment can last up to 20 years.

Abstract

Background and Aims: Adalimumab (ADA) biosimilars have been included into the therapeutic armamentarium of inflammatory bowel disease (IBD); however, comparative data on the efficacy and safety of the different ADA biosimilars after replacing the ADA originator for a non-medical reason remains scarce. We aimed to compare in a real-life setting the efficacy and safety of four ADA biosimilars SB5, APB501, GP2017, and MSB11022 in IBD patients after replacing the originator for a non-medical reason., Methods: A multicenter retrospective study was performed on consecutive IBD patients, analyzing clinical, laboratory, and endoscopic data. The primary endpoints of the study were maintenance of clinical remission and safety of the different biosimilars., Results: 153 patients were enrolled, 26 with UC and 127 with CD. Clinical remission was maintained in 124 out of 153 (81%) patients after a median (IQR) follow-up of 12 (6-24) months, without any significant difference between the four ADA biosimilars. ADA biosimilars dosage was optimized in five patients (3.3%). Loss of remission was significantly higher in UC patients (10/26 patients, 38.5%) than in CD patients (19/127 patients, 14.9%, p<0.025). Adverse events occurred in 12 (7.9%) patients; the large majority were mild., Conclusions: No difference in efficacy and safety was found between ADA biosimilars when used to replace the ADA originator for a non-medical reason. However, in UC patients the replacement of ADA originator for this reason should be carefully assessed.

31. The Psychosocial Burden of HCV Infection and the Impact of Antiviral Therapy on the Quality of Life in Liver and Kidney Transplant Recipients: A Pilot Study.

Abstract

Background: Therapy with direct-acting antivirals (DAA) for HCV is safe and effective in the liver (LT) and kidney transplant (KT) recipients; however, data on the quality of life (QoL) of patients are scanty. This pilot study is aimed at prospectively evaluating the QoL in LT and KT recipients before and after DAA treatment., Methods: We prospectively enrolled 17 LT and 11 KT recipients with HCV infection starting a sofosbuvir-based antiviral therapy for 12 weeks. All participants before (T0), 12 (T12), and 24 (T24) weeks after the end of the therapy completed the Short Form Health Survey (SF-36) questionnaire, the Zung Self-rating Depression Scale, and State-Trait Anxiety Inventory (STAI-Y1-Y2)., Results: At T0, LT and KT patients were similar for gender, age, BMI, smoking habits, marital status, mean liver stiffness values at Fibroscan, and HCV genotype distribution ( p > 0.05). There were no significant differences between the 2 groups in STAI-Y1, STAI-Y2, Zung, and SF-36 scores ( p > 0.05). At T12, all the participants showed a sustained virological response (SVR). All items of the SF-36 questionnaire improved from the pretreatment to posttreatment period within the LT group, and the 4 domains role-physical, bodily pain, social function, role-emotional, and mental health reached statistical significance ( p < 0.05 in all cases). On the contrary, in KT patients, there was no significant improvement in SF-36 mean scores compared to at baseline at T12 and T24., Conclusions: This pilot study suggested that DAA therapy is associated with a significant improvement of the QoL only in LT recipients. Abstract

This data set describes the abundance of 50 invertebrate taxa found in intertidal mussel beds along the Atlantic Canadian coast. This information resulted from a regional-scale study that investigated the effects of wave exposure on the richness and composition of invertebrate assemblages from intertidal mussel beds. Abundance data are provided for taxa representing the Annelida, Arthropoda, Bryozoa, Chordata, Cnidaria, Echinodermata, Mollusca, Nematoda, Nemertea, and Platyhelminthes. The data characterize mussel beds from wave-sheltered and wave-exposed locations spanning 315 km of the coast of Nova Scotia. Univariate and multivariate analyses revealed that the compositional structure of these invertebrate assemblages differed markedly depending on wave exposure. Overall, because of its taxonomic diversity, the inclusion of data for basal, intermediate, and top trophic levels, and the coverage of two extremes of environmental stress, this data set could be useful to test broader aspects of ecological theory. Areas of ecology that could advance using this data set are those concerning environmental stress models of community organization, abundance-occupancy relationships, species co-occurrence, species abundance distributions, dominance and rarity, spatial scales of population and community variation, and distribution of functional and phylogenetic diversity. Use of this data set for academic or educational purposes is allowed as long as the data source is properly cited. 33. Distribution and Characterization of Deep Rhodolith Beds off the Campania coast (SW Italy, Mediterranean Sea).

Abstract

Rhodolith beds (RBs) are bioconstructions characterized by coralline algae, which provide habitat for several associated species. Mediterranean RBs are usually located in the mesophotic zone (below 40 m), and thus are frequently remote and unexplored. Recently, the importance and vulnerability of these habitats have been recognized by the European Community and more attention has been drawn to their investigation and conservation. This study reports the results of an extensive monitoring program, carried out within the Marine Strategy Framework Directive (2008/56/EC), in six sites off the Campania coast (Italy, Mediterranean Sea). New insights were given into the distribution, cover, vitality (i.e., live/dead rhodolith ratio), structural complexity, and coralline algae composition of RBs. Remotely operated vehicles (ROV) investigations allowed the description of several RBs, and the discovery of a RB with rhodolith cover >65% offshore the Capri Island. Only two sites (Secchitiello and Punta Campanella) showed a very low mean cover of live rhodoliths (<10%); hence, not being classifiable as RBs. The collected rhodoliths were mostly small pralines (~2 cm), spheroidal to ellipsoidal, with growth-forms ranging from encrusting/warty to fruticose/lumpy. Coralline algae identification revealed a high diversity within each bed, with a total of 13 identified taxa. The genus Lithothamnion dominated all sites, and Phymatolithon calcareum and Lithothamnion corallioides , protected by the Habitats Directive (92/43/EEC), were detected in all RBs.

34. Meiofaunal communities and nematode diversity characterizing the Secca delle Fumose shallow vent area (Gulf of Naples, Italy).

Abstract

Hydrothermal venting is rather prevalent in many marine areas around the world, and marine shallow vents are relatively abundant in the Mediterranean Sea, especially around Italy. However, investigations focusing on the characterization of meiofaunal organisms inhabiting shallow vent sediments are still scant compared to that on macrofauna. In the present study, we investigated the meiobenthic assemblages and nematode diversity inhabiting the Secca delle Fumose (SdF), a shallow water vent area located in the Gulf of Naples (Italy). In this area, characterized by a rapid change in the environmental conditions on a relative small spatial scale (i.e., 100 m), we selected four sampling sites: one diffusive emission site (H); one geyser site (G) and two inactive sites (CN, CS). Total meiofauna abundance did not vary significantly between active and inactive sites and between surface and deeper sediment layers due to a high inter-replicate variability, suggesting a pronounced spatial-scale patchiness in distribution of meiofauna. Nematofauna at site H presented the typical features of deep-sea vents with low structural and functional diversity, high biomass and dominance of few genera (i.e., Oncholaimus ; Daptonema ) while from site G we reported diversity values comparable to that of the inactive sites. We hypothesized that site G presented a condition of "intermediate disturbance" that could maintain a high nematode diversity. Environmental features such as sediment temperature, pH, total organic carbon and interstitial waters ions were found to be key factors influencing patterns of meiofauna and nematofauna assemblages at SdF. Even though the general theory is that nematodes inhabiting shallow vent areas include a subset of species that live in background sediments, this was not the case for SdF vent area. Abstract

Background: The use of direct antiviral agents (DAA) has radically modified the course of HCV hepatitis in renal patients. Aim of this study was to assess the effects of HCV eradication on quality of life (QOL) in renal transplant recipients (RTR), measured by CLDQ and SF-36., Methods: Sixteen RTR with well preserved GFR (mean: 60.3±19.3 ml/min) and chronic HCV infection with moderate liver stiffness (9.3±1.7 kPa) were given a sofosbuvir-based regimen for 12 weeks and had a 1 year follow-up., Results: At end of treatment (EOT) a complete viral clearance was observed in all the patients, with normalization of most laboratory data and a consistent reduction in liver stiffness. All these parameters remained stable after 1 year, as well as renal function and proteinuria. Questionnaire data showed consistent amelioration in different "emotional" domains at EOT, which persisted after 1 year and were associated with a globally improved QOL, although there was no change in most of the "physical" domains in both questionnaires. One patient under ribavirin developed an acute anemia and withdrew from the study, but no further adverse episode was observed throughout the study., Conclusions: Our data, while confirming the efficacy of oral DAA, show that HCV infection represents a heavy psychological burden in renal transplant recipients, greatly alleviated by viral eradication, which determines a significant improvement in QOL that represents an important outcome in management of all transplant recipients. Abstract

Background: Few data are available on the virological and clinical outcomes of advanced liver disease patients retreated after first-line DAA failure., Aim: To evaluate DAA failure incidence and the retreatment clinical impact in patients treated in the advanced liver disease stage., Methods: Data on HCV genotype, liver disease severity, and first and second line DAA regimens were prospectively collected in consecutive patients who reached the 12-week post-treatment and retreatment evaluations from January 2015 to December 2016 in 23 of the PITER network centers., Results: Among 3,830 patients with advanced fibrosis (F3) or cirrhosis, 139 (3.6%) failed to achieve SVR. Genotype 3, bilirubin levels >1.5mg/dl, platelet count <120,000/mm3 and the sofosbuvir+ribavirin regimen were independent predictors of failure by logistic regression analysis. The failure rate was 7.6% for patients treated with regimens that are no longer recommended or considered suboptimal (sofosbuvir+ribavirin or simeprevir+sofosbuvir±ribavirin), whereas 1.4% for regimens containing sofosbuvir combined with daclatasvir or ledipasvir or other DAAs. Of the patients who failed to achieve SVR, 72 (51.8%) were retreated with a second DAA regimen, specifically 38 (52.7%) with sofosbuvir+daclatasvir, 27 (37.5%) with sofosbuvir+ledipasvir, and 7 (9.7%) with other DAAs ±ribavirin. Among these, 69 (96%) patients achieved SVR12 and 3 (4%) failed. During a median time of 6 months (range: 5-14 months) between failure and the second DAA therapy, the Child-Pugh class worsened in 12 (16.7%) patients: from A to B in 10 patients (19.6%) and from B to C in 2 patients (10.5%), whereas it did not change in the remaining 60 patients. Following the retreatment SVR12 (median time of 6 months; range: 3-12 months), the Child-Pugh class improved in 17 (23.6%) patients: from B to A in 14 (19.4%) patients, from C to A in 1 patient (1.4%) and from C to B in 2 (2.9%) patients; it remained unchanged in 53 patients (73.6%) and worsened in 2 (2.8%) patients. Of patients who were retreated, 3 (4%) had undergone OLT before retreatment (all reached SVR12 following retreatment) and 2 (2.8%) underwent OLT after having achieved retreatment SVR12. Two (70%) of the 3 patients who failed to achieve SVR12 after retreatment, and 2 (2.8%) of the 69 patients who achieved retreatment SVR12 died from liver failure (Child-Pugh class deteriorated from B to C) or HCC complications., Conclusions: Failure rate following the first DAA regimen in patients with advanced disease is similar to or lower than that reported in clinical trials, although the majority of patients were treated with suboptimal regimens. Interim findings showed that worsening of liver function after failure, in terms of Child Pugh class deterioration, was improved by successful retreatment in about one third of retreated patients within a short follow-up period; however, in some advanced liver disease patients, clinical outcomes (Child Pugh class, HCC development, liver failure and death) were independent of viral eradication.

Abstract

Aim: To evaluate the long-term eradication of hepatitis C virus (HCV) infection and liver-related complications in chronically infected patients that have achieved sustained virological response., Methods: One hundred and fifty subjects with chronic hepatitis C (CHC) or cirrhosis and sustained virological response (SVR) between the years of 1989 and 2008 were enrolled in a long-term clinical follow-up study at the Gastrointestinal and Liver Unit of the University Hospital of Naples "Federico II". At the beginning of the study, the diagnosis of HCV infection was made on the basis of serum positivity for antibodies to HCV and detection of HCV RNA transcripts, while a diagnosis of chronic hepatitis was formulated using imaging techniques and/or a liver biopsy. SVR was achieved by interferon-based therapy, both conventional and pegylated, with and without ribavirin treatment. The patients were evaluated for follow-up at a median length of 8.6 years, but ranged from 2-19.9 years. Among them, 137 patients had pre-treatment CHC and 13 had cirrhosis. The patients were followed with clinical, biochemical, virological, and ultrasound assessments on a given schedule. Finally, a group of 27 patients underwent a liver biopsy at the beginning of the study and transient elastography at their final visit to evaluate changes in liver fibrosis., Results: The median follow-up was 8.6 years (range 2-19.9 years). HCV RNA remained undetectable in all patients, even in patients who eventually developed liver-related complications, indicating no risk of HCV recurrence. Three liver-related complications were observed: two cases of hepatocellular carcinoma and one case of bleeding from esophageal varices resulting in an incidence rate of 0.23%/person per year. Further, all three complications took place in patients diagnosed with cirrhosis before treatment began. Only one death due to liver-related causes occurred, resulting in a mortality rate of 0.077% person per year. This amounts to a 99.33% survival rate in our cohort of patients after therapy for HCV infection. Finally, of the 27 patients who underwent a liver biopsy at the beginning of the study, a reduction in liver fibrosis was observed in 70.3% of the cases; only three cases registering values of liver stiffness indicative of significant fibrosis., Conclusion: Patients with CHC and SVR show an excellent prognosis with no risk of recurrence and a very low rate of mortality. Our data indicate that virus-eradication following interferon treatment can last up to 20 years.

Abstract

Objectives: Many different articular symptoms may appear in patients with HCV hepatitis, but in a relatively large number of patients no rheumatic symptoms are present. This sonographic study was undertaken to detect the possible presence of early articular changes in HCV patients without any rheumatic manifestations., Methods: The knee, hip and shoulder were evaluated in a cohort of 29 consecutive HCV patients without any rheumatic symptoms. Results were compared with those obtained by the evaluation of 29 healthy subjects who were negative for markers of HCV and HBV infections., Results: Results showed the presence of alterations in 96.5% of the patients, with significant differences in comparison to controls (p < 0.0001). Slight inflammatory changes were found in all the joints examined. The knee was involved in 79.3% of the cases, the hip in 27.6% and the shoulder in 89.6%., Conclusions: Our preliminary study shows the presence of joint changes in the majority of cases. To the best of our knowledge this is the first ultrasonographic study to focus on joint evaluation in patients with HCV hepatitis. Broader epidemiological and virological investigations, in particular for the HCV subtype and HLA genotype, will be required to elucidate the relationship between HCV infection and rheumatic symptoms.

Abstract

We determined the frequencies of antibodies to Anaplasma phagocytophila, the agent of human granulocytic ehrlichiosis (HGE), in different groups of adults and children from Westchester County, New York. The groups included 159 adult blood donors and 215 children who were seronegative for Borrelia burgdorferi antibodies, 118 adult patients and 57 children who were seropositive for B. burgdorferi antibodies, and 42 adult patients with culture-confirmed erythema migrans. Eighteen (11.3%) of the blood donors and 11 (5.1%) of the B. burgdorferi-seronegative children were found to have A. phagocytophila antibodies by indirect immunofluorescent-antibody assay (IFA). Nine of 42 (21.4%) patients with culture-confirmed erythema migrans tested at the baseline visit, 42 of 118 (35.6%) adults, and 3 of 57 (5.3%) children whose sera were reactive for B. burgdorferi antibodies also tested positive for A. phagocytophila antibodies. The geometric mean titer ranged from 219 to 315 for all groups, and the differences in titers among the groups were not statistically significant. Only one-third of the healthy blood donors reactive by IFA were confirmed to be positive by immunoblotting. We conclude that a significant proportion of adults and children without clinical evidence of HGE will test positive for A. phagocytophila antibodies when the conventional cutoff titer of 80 is used in the IFA. This information must be considered in interpretation of test results.

Abstract

The reduced glutathione (GSH)-conjugating capacities of the livers of trout, carp, zebra fish, guppy, and bluegill have been evaluated with several substrates preferentially metabolized by the major GSH-transferase isozymes. Carp and zebra fish lack GSH-transferase activity with 1,2-epoxy-3-(p-nitrophenoxy)propane. Guppy is characterized by the highest activities with 1-chloro-2,4-dinitrobenzene and the absence of activity toward trans-4-phenyl-3-buten-2-one. Enzyme activities with the other substrates demonstrate quantitative interspecific differences which presumably are of minor relevance to the metabolism and toxicity of chemicals.
