2. Adherence to chest physiotherapy, airway clearance techniques and physical exercise by children and adolescents with cystic fibrosis.

Introduction: chest physiotherapy, airway clearance techniques, and physical exercise are associated with better outcomes in cystic fibrosis, but adherence to these recommendations remains a challenge. Objective: to assess adherence to chest physiotherapy, airway clearance techniques, and physical exercise by children and adolescents with cystic fibrosis and analyze the factors related to adherence. Methods: retrospective study of children and adolescents with cystic fibrosis from a pediatric referral center in Brazil. Data were collected from electronic records of the multidisciplinary team responsible for caring for these patients at the referral center. Results: the sample included all 83 patients registered at the referral center, with a mean age of 7.88 ± 4.88 years. Among them, 28.9% did not have weekly chest physiotherapy, 66.3% did not practice physical exercise regularly, and only 38.6% practiced airway clearance techniques from 6 to 7 times a week. Higher adherence to airway clearance techniques was associated with higher mean age (p<0.01) and correlated with FEV1 (r=-0.39; p=0.03), FEF25-75% (r =-0.36; p=0.02), FEV1/FVC (r =-0.43; p=0.01) and lower mean Shwachman-Kulczycki Score (r=-0.34; p<0.01). Higher physical exercise adherence was associated with a higher mean age (p<0.01). Individuals using public financing of physiotherapy services had a lower Shwachman-Kulczycki Score (p=0.02) than those using non-public services. The metropolitan region had a higher percentage of individuals using nonpublic services than other regions (p<0.01). Conclusion: adherence to chest physiotherapy, airway clearance techniques and physical exercise was lower than recommendations. Airway clearance techniques and physical exercise adherence increased with age, and airway clearance techniques adherence was associated with the high severity of disease. These results suggest that patients only increase adherence when they get older and the disease worsens, highlighting the need for strategies to increase adherence early. [ABSTRACT FROM AUTHOR]

RESUMO Objetivo identificar os motivos que levam o indivíduo a seguir ou não a recomendação da necessidade do uso do aparelho de amplificação sonora individual (AASI) e a utilização desse dispositivo ao longo dos anos. Estratégia de pesquisa orientações do Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) foram seguidas. As bases de dados pesquisadas foram: Pubmed, Pubmed Central, Scopus, Embase, Web of Science e Cochrane, utilizando os seguintes descritores: “Hearing Aids”, “Patient Compliance” e o indicador booleano AND. Critérios de seleção artigos originais primários com desenhos prospectivos, retrospectivos, observacionais ou experimentais; que levantassem, relatassem, verificassem ou analisassem os motivos de adesão ou não ao uso de AASI, independentemente de ser usuário experiente ou não; realizados com a população jovem, adulta e idosa; com indivíduos com qualquer grau, tipo e configuração de perda auditiva; com adaptações unilaterais ou bilaterais e em português, inglês e espanhol. Resultados incluídos 27 estudos na análise. Verificaram-se duas situações distintas no processo de reabilitação auditiva: aceitação à indicação do uso do AASI e a continuidade do uso do dispositivo de maneira efetiva. Os aspectos que auxiliaram positivamente, negativamente ou não interferiram nessas duas fases foram compilados e apresentados. Conclusão os fatores de maior relevância para a não aquisição do AASI são: percepção de baixo custo-benefício, falta de entendimento da real necessidade e dificuldade de aceitação do uso, enquanto que os que mais impactam na continuidade do uso são: qualidade sonora do AASI, dificuldades de manuseio e percepção de pouco benefício.

Resumo Introdução A doença renal crônica apresenta-se como um problema de saúde pública por causa de sua prevalência, dos custos envolvidos no tratamento e da alta taxa de morbimortalidade. Objetivo Avaliar a não adesão ao regime terapêutico de pacientes em hemodiálise e fatores associados. Método Estudo transversal com pacientes que realizam hemodiálise em um hospital universitário e duas clínicas privadas conveniadas ao Sistema Único de Saúde. Padrões para avaliação da não adesão ao regime terapêutico hemodialítico tiveram como base os indicadores estabelecidos pelo The Dialysis Outcomes and Practice Patterns Study: restrição hídrica, regime dietético, regime medicamentoso e terapia hemodialítica. Não conformidade em pelo menos um dos aspectos do tratamento foi considerada não aderência à terapêutica. Resultados Indivíduos com idade inferior a 60 anos tiveram maior probabilidade de não aderir à restrição hídrica, à terapêutica e aos regimes dietético e medicamentoso. Pacientes anúricos tiveram maior chance de não adesão à restrição hídrica e à terapêutica. Indivíduos com hipoalbuminemia e hipoemoglobinemia tiveram maior probabilidade de não aderir à restrição hídrica, enquanto pacientes que se autodeclararam não brancos apresentaram maior chance de não aderir à terapêutica. Nenhuma variável explicativa da adesão à terapia hemodialítica se manteve significativa no modelo final da regressão logística. Conclusão A inconformidade relacionada aos aspectos do tratamento pode resultar em aumento de hospitalização e gastos com saúde, piora do estado clínico, maiores intercorrências no tratamento e aumento da taxa de mortalidade.

Resumo Objetivo: identificar o perfil das gestações e prevalência de adesão à consulta puerperal entre puérperas adolescentes comparadas a não adolescentes, assistidas em um ambulatório de hospital de ensino do interior de Minas Gerais. Background: Chronic kidney disease presents itself as a public health problem because of its prevalence, the costs involved in treatment and the high rate of morbidity and mortality. Objective: To evaluate non-adherence to the therapeutic regimen of hemodialysis patients and associated factors. Method: A cross-sectional study with patients undergoing hemodialysis at a University Hospital and two private clinics agreed to the Unified Health System. Standards for non-adherence to the hemodialysis regimen were based on indicators established by The Dialysis Outcomes and Practice Patterns Study: water restrictions, dietary, drug regimen, and hemodialysis therapy. Non-compliance in at least one aspect of treatment was considered non-adherence to therapy. Results: Individuals younger than 60 years were more likely to be non-adherent to water restriction, dietary and drug regimens, and therapy. Anuric patients were more likely to be non-adherent to water restriction and therapy. Individuals with hypoalbuminemia and hypohemoglobinemia were more likely not to adhere to water restriction, while patients who self-declared themselves to be non-whites had a higher chance of not adhering to therapy. No explanatory variables of adherence to hemodialysis therapy remained significant in the final logistic regression model. Conclusion: Non-conformity related to treatment aspects can result in increased hospitalization and health expenses, worsening of clinical status, greater intercurrences in treatment and increase in mortality rate. 10. Adesão aos cuidados domésticos de casos suspeitos de Covid-19 em isolamento domiciliar

RESUMO Objetivo Analisar a associação entre as características sociodemográficas e a adesão aos cuidados domésticos de casos suspeitos de Covid-19 em isolamento domiciliar. Método: Estudo transversal, analítico. A coleta de dados ocorreu em unidade mista de saúde do Ceará, com 50 participantes suspeitos de Covid-19. Aplicou-se formulário de caracterização sociodemográfica, clínica e de avaliação dos cuidados em relação ao isolamento domiciliar. Para análise estatística, foram utilizados os testes Mann-Whitney e a razão de verossimilhança. Os princípios éticos das pesquisas com seres humanos foram seguidos. Resultados: A maioria dos homens (57,9%; p = 0,010) e das pessoas que conheciam alguém, pessoalmente, que teve/tem diagnóstico de Covid-19 (92,1%; p = 0,040) evitou aglomerações; 65,4% dos participantes com companheiro (a) descartaram o lixo adequadamente (p = 0,047); e 81,6% das pessoas com menor escolaridade aderiram à lavagem de roupas pessoais, de cama e toalhas com sabão comum e água (p = 0,043). Conclusão e implicações para a prática: As características sociodemográficas influenciaram a adesão a cuidados específicos, como evitamento de aglomerações, tratamento de lixo contaminado e lavagem de roupas pessoais, de cama e toalhas. Os cuidados domiciliares precisam ser mais enfatizados em consultas e pelos veículos de divulgação.

11. Adesão à Terapia Hormonal Adjuvante com Tamoxifeno e Anastrozol utilizando ARMS-12 e MMAS-4

Introdução: Entre os canceres de mama, aproximadamente 75% das mulheres são receptores hormonais positivos, sendo estas mais propensas a responderem a hormonioterapia com anastrozol e tamoxifeno. Apesar de eficazes, apresentam taxas significativas de não adesão. Objetivo: Avaliar a adesão a terapia hormonal adjuvante com tamoxifeno e anastrozol em pacientes atendidos nos Ambulatórios da Mastologia e de Quimioterapia do Hospital São Paulo entre os anos de 2019 e 2020. Método: Estudo transversal com 102 mulheres, realizado entre os meses de setembro de 2019 e marco de 2020. A adesão a terapia hormonal adjuvante foi avaliada utilizando-se as escalas Morisky Medication Adherence Scale (MMAS-4) e Adherence to Refills and Medications Scale of 12 items (ARMS-12). Resultados: A média de idade foi de 61,5 anos (59,3-63,6). Entre as pacientes, 27,7% faziam uso de tamoxifeno e 72,3% de anastrozol. Relataram desconforto em relação ao uso do medicamento 84,4%, sendo as ondas de calor (42,2%) e as dores articulares (55,9%) os mais frequentes. A escala de ARMS>12 foi pontuada por 79,2%; cerca de 90% das mulheres pontuaram a MMAS-4 até dois pontos, porém não houve diferença significativa entre os tipos de hormônios utilizados para escalas de adesão (p=0,815 e p=0,489). Conclusão: A adesão a hormonioterapia observada foi relativamente baixa, independentemente da endocrinoterapia, podendo essas pacientes estarem em risco de inadequação quanto a resposta clínica.

Background: In older adults is common to identify changes in body balance and gait pattern, requiring the use of walking aids to maintain safe mobility. Despite the benefits of these devices, some people do not comply with their use. Objective: To explore older adults' perspectives regarding the factors associated with non-compliance to walking aids. Methodology: A qualitative, descriptive exploratory study was carried out. Semi-structured interviews were conducted with 11 older adult users of a Day Care Unit from a Private Institution of Social Solidarity in the region of Lisbon and Tagus Valley. Content analysis was performed to analyse data. The consolidated criteria for reporting qualitative research (COREQ) checklist was applied to this study. Results: The factors associated with non-compliance to walking aids encompass four categories: social context, the symbolism of the device, involvement of health professionals, and low critical judgment. Conclusion: There are different factors associated with non-compliance to walking aids. It is necessary to structure intervention programs that allow the elimination or reduction of the effect of these factors to increase older adults' compliance. [ABSTRACT FROM AUTHOR]

Objective: to describe the perception of people with arterial hypertension of the aspects that facilitate and hinder treatment adherence. Method: descriptive qualitative study developed with 16 people met in Nursing consultations. Interviews were conducted with a semi-structured guide and the content analysis technique was used. Data were collected between January and August 2017. Results: having willpower, family and multidisciplinary support, knowledge about the pathology and forms of prevention and fear of death facilitated adherence; on the other hand, little knowledge, laziness, lack of urban infrastructure and climatic conditions, habit of consuming unhealthy foods, alcoholic beverages and tobacco, cost of treatment and forgetfulness of taking the medication hindered adherence. Conclusion: the treatment of hypertension causes changes in the dynamics of life, being essential to have willpower, family and professional support, in addition to overcoming unhealthy habits.

Objetivo: analizar los factores intervinientes en la adhesión al tratamiento directamente observado en tuberculosis, según la percepción de pacientes y enfermeros de unidades básicas de salud. Método: investigación cualitativa y descriptiva con la participación de 13 enfermeros y 52 pacientes de 12 Unidades Básicas de Salud de Belém-Pará, Brasil. Para la producción de los datos, se realizaron entrevistas individuales, complementadas con observación no participativa. Los datos se sometieron a análisis de contenido temático. Resultados: surgieron dos categorías del análisis: Percepciones de pacientes y enfermeros sobre el tratamiento directamente observado, y Operacionalización del tratamiento directamente observado: aspectos positivos y limitantes. Conclusión: incluso con las debilidades señaladas, las unidades tienen potencial para desarrollar la estrategia. Los enfermeros reconocen su importancia, pero necesitan más apoyo institucional; y los pacientes, cuando están bien informados, logran buenos índices de adhesión al tratamiento. Este conocimiento puede proporcionar mayor seguridad a los enfermeros al realizar el seguimiento de los pacientes.

Objective: To assess clinicians’ knowledge about pulmonary rehabilitation, and identify the barriers faced when referring patients with health insurance to pulmonary rehabilitation. Methods: This was a survey-based cross-sectional study conducted in 2019, at a private reference hospital in São Paulo, Brazil. Eligible participants were physicians registered with the following specialties: internal medicine, geriatrics, cardiology, pulmonology or thoracic surgery. Results: We collected 72 responses, and 99% of participants recognized chronic obstructive pulmonary disease as a potential indication for pulmonary rehabilitation; less often (75%), they listed interstitial lung disease, bronchiectasis and pulmonary hypertension. Most participants (67%) incorrectly associated pulmonary rehabilitation with lung function improvement, while 28% of cardiologists and 35% of internists/geriatricians failed to recognize benefits on mood disorders. Notably, 18% of participants recommended pulmonary rehabilitation only to patients on supplemental oxygen and 14% prescribed only home physical therapy, patterns more commonly seen among non-respiratory physicians. The three most perceived barriers to referral and adherence were health insurance coverage (79%), transportation to pulmonary rehabilitation center (63%) and lack of social support (29%). Conclusion: Financial, logistic and social constraints pose challenges to pulmonary rehabilitation enrollment, even for patients with premium healthcare insurance. Moreover, physician knowledge gaps may be an additional barrier to pulmonary rehabilitation referral and adherence. Providing continued medical education, incorporating automatic reminders in electronic medical records, and using telerehabilitation tools may improve pulmonary rehabilitation referral, adherence, and ultimately, patient care. Método: estudo de caso único realizado com quatro irmãos com doença falciforme acompanhados em um Centro de Referência no estado da Bahia, Brasil. Os dados foram colhidos por questionário, entrevista aberta e desenho-estória com tema. Resultados: os temas emergidos foram: Preconceito e discriminação promovem o autoisolamento; Dificuldade para deslocamento impede as idas ao serviço; A unidade de saúde é um espaço que amplia dúvidas e incertezas; Desesperança leva à desistência do tratamento. Conclusão: amparo social, suporte emocional, instrumental e educacional são essenciais para reduzir a vulnerabilidade da família que vivencia a doença falciforme. Conteúdos abordados nas unidades de saúde necessitam incluir perspectivas de experiências positivas, fomentando esperança e construção de sentidos para a vida no processo de adoecimento crônico, a fim de aumentar a adesão ao tratamento.

Introdução e objetivo: Hipertensão arterial e diabetes mellitus como problemas de saúde pública se evidenciam pela alta prevalência, impacto socioeconômico e dificuldades na adesão ao tratamento. O objetivo desta pesquisa foi investigar a adesão ao tratamento farmacológico e não farmacológico de usuários do Sistema Único de Saúde, internados devido a complicações decorrentes do insucesso no controle destas doenças. Materiais e métodos: É um estudo descritivo, de abordagem quantitativa. Utilizou-se a entrevista estruturada como instrumento junto a 11 pessoas. Resultados: Destas, 10 faziam uso de medicamentos em domicílio, 4 relataram esquecer de toma-los. Referente ao tratamento não farmacológico, 3 seguiam dieta com restrições alimentares, 10 se consideravam ativos fisicamente e 1 praticava exercícios físicos regularmente. Discussão e conclusão: Os resultados indicam a necessidade de se olhar o usuário em sua totalidade, identificando fatores que o levam a não adesão aos tratamentos disponíveis e fomentando estratégias para melhorar sua adesão aos mesmos. ABSTRACT Adherence to treatment to prevent health problems related to high blood pressure and diabetes Introduction and objective: Hypertension and diabetes mellitus as public health problems are evidenced by the high prevalence, socioeconomic impact and difficulties in the treatment adherence. The objective of this research was to investigate the adherence to pharmacological and non pharmacological treatment for the users of the Unified Health System, hospitalized due complications because of the failure of the control of these diseases. Materials and methods: It is a descriptive study with a quantitative approach. We used the structured interview as instrument with 11 people. Results: Of these, 10 were using medicines at home, 4 reported to forget to take them. Concerning the non-pharmacological treatment, 3 followed diet with dietary restrictions, 10 were considered by themselves physically active and 1 practiced exercises regularly. Discussion and conclusion: The results indicate the need of looking at the pacient as an entire part, identifying factors that lead to a non-adherence to the available treatment and promoting strategies to improve the adherence to them.

Resumo Objetivo Identificar os fatores associados à não adesão ao tratamento antirretroviral em portadores de HIV/ AIDS em um Hospital de referência em Manaus. Métodos Estudo com abordagem quantitativa, transversal de base hospitalar, desenvolvido com 100 participantes com HIV/AIDS em acompanhamento ambulatorial. Para a coleta de dados foi utilizado o questionário autoaplicável, denominado “Questionário para a avaliação da adesão ao tratamento antirretroviral em pessoas com HIV/AIDS” (CEAT–VIH). Foi realizada análise descritiva, empregado o teste de qui-quadrado de Pearson chi-square para o valor de p. Resultados Predominou o sexo masculino (57%), faixa etária entre 40 a 59 anos (34%) escolaridade de 2º Grau (49%), sem vínculo empregatício (84%), renda mensal de 1 a 3 salários mínimos (54%), solteiros (47%), heterossexuais (76%), com parceiro sexual (56%), sem vida sexual ativa (61%), tempo de diagnóstico entre 6 meses a 5 anos (59%), nenhuma internação hospitalar (59%). O nível de adesão predominante foi a média adesão (85%). As variáveis sociodemográficas que tiveram associação estatisticamente significantes com a adesão TARV foram a orientação sexual (p=0,010) e o tempo de diagnóstico (p=0,035). Conclusão O estudo mostrou que pessoas que convivem com HIV aderem a TARV, porém com média adesão e os principais fatores associados a esse resultado foram a orientação sexual e o tempo de diagnóstico.

Resumo Objetivo: Identificar o comportamento de adesão do paciente renal crônico ao regime terapêutico nas suas quatro dimensões: hemodiálise uso de medicamentos, dieta e restrição hídrica. Métodos: Estudo descritivo, transversal com abordagem quantitativa, desenvolvido em dois centros de hemodiálise no Estado do Rio de Janeiro. Para coleta de dados, foi utilizado um questionário de avaliação sobre a adesão do portador de doença renal crônica em hemodiálise. Os dados foram analisados por meio de estatística descritiva simples. Resultados: O domínio que apresentou maior percentual de pacientes não aderentes foi a hemodiálise, com 32%. Já a medicação foi o domínio com maior percentual de pacientes aderentes, 93,6%. Conclusão: A adesão à terapêutica é um comportamento dinâmico e, como tal, merece monitorização constante.

A Retocolite Ulcerativa (RCU) é uma doença idiopática, com inflamação crônica do intestino, que acomete predominantemente a camada mucosa do cólon e porções proximais. O curso da doença passa por períodos de atividade e de remissão, com uma minoria apresentando atividade constante. A severidade das crises e a resposta ao tratamento são variáveis e de difícil previsão. Objetivou-se relatar o caso de um paciente com Retocolite Ulcerativa e o desfecho da não aderência ao tratamento. Trata-se de um estudo descritivo realizado em um Hospital Geral no interior da Bahia, através de métodos subjetivos, exames de imagem e procedimentos cirúrgicos que constata que o paciente encontra-se com anemia, desnutrição e processo inflamatório (sepse) provavelmente agravado pela falta de adesão ao tratamento inicial de descoberta da doença. Pode - se fazer uma reflexão da importância da relação multidisciplinar na adesão ao tratamento. Estratégias educacionais podem otimizar a adesão ao tratamento. Estudos de intervenção são necessários para estabelecer o impacto dessas medidas no intuito de proporcionar um maior benefício na melhora desses pacientes evitando complicações mais graves e internações hospitalares. ABSTRACT Outcome of noncompliance colitis ulcerativa: case report Ulcerative colitis (UC) is an idiopathic disease with chronic inflammation of the intestine, which predominantly affects the mucous layer of the colon and proximal portions. The course of the disease goes through periods of activity and remission, with a minority showing constant activity. The severity of the seizures and response to treatment are variable and difficult to predict. The aim of this study was to report the case of a patient with ulcerative colitis and the outcome of nonadherence to treatment. This is a descriptive study carried out at a General Hospital in the interior of Bahia, using subjective methods, imaging tests and surgical procedures, which shows that the patient is suffering from anemia, malnutrition and an inflammatory process (sepsis) probably aggravated by lack Adherence to the initial treatment of disease discovery. It is possible to reflect on the importance of the multidisciplinary relationship in adherence to treatment. Educational strategies can optimize adherence to treatment. Intervention studies are necessary to establish the impact of these measures in order to provide a greater benefit in the improvement of these patients avoiding more serious complications and hospitalizations.

Objective: To evaluate the adherence to clinical glaucoma treatment in patients that benefit from the Brazilian National Glaucoma Program and receive antiglaucomatous eye drops free of charge. Methods: This cross-sectional study included 255 patients that benefit from the Brazilian National Glaucoma Program (BNGP) and were assisted at the Altino Ventura Foundation (FAV), Pernambuco state, Brazil. The Morisky's Adhesion Scale (MAS) was used to evaluate adhesion profile to treatment. Patients with a Morisky's Adhesion score = 6 was considered adherent to treatment. The analyzed variables included sex, educational level, distance from home to the treatment center, number of eye drops used, time since the glaucoma diagnosis, patients' subjective classification of their quality of vision, and prevalence of other comorbidities. Results: A total of 255 patients [167 (65.4%) females] were included in this study with a mean age was 65.9 ± 13.8 years (range, 11-99 years). Adherence to clinical glaucoma treatment was seen in 155 patients (60.8%). Worse adherence to treatment was observed in female patients (p=0.034), with low education level (p=0.046), who presented other comorbidities (p=0.048), living in remote areas (p=0.038), judged their vision as satisfactory (p=0.046), and who were recently diagnosed with glaucoma (p=0.001). Conclusion: 40% of patients who benefited from the Brazilian National Glaucoma Program were not compliant to treatment. Low adhesion rate was mostly seen in female patients, socially vulnerable, living in remote areas, recently diagnosed with glaucoma, using fewer eyedrops, presenting other comorbidities, and in those that considering themselves of having a satisfactory vision. Objective: To evaluate the level of self-referenced treatment adherence (TA) and its association with clinical and sociodemographic variables in patients with cystic fibrosis assisted at a reference center, as well as compare the level of self-referenced TA with that presumed by the multidisciplinary team. Methods: This is a cross-sectional study that included children and adolescents aged between 0-20 years with cystic fibrosis. Adolescents older than 14 years or their guardians, when younger than 14 years old, were interviewed using a standardized questionnaire. Professionals from the multidisciplinary clinic Filled out another form with their impressions of the patients' TA. Clinical and laboratory data were obtained in the medical records. The TA was considered satisfactory if the total adherence index (TAI) was equal or higher than 80%. Results: 53 patients were included with a median age of 112 months. The mean TAI was 83.2%. The mean TAIs for dornase alfa, pancreatic enzymes, continued use of inhaled tobramycin, vitamins supplements, nutritional supplements and dietary orientation was respectively: 86.1; 96.6; 78.6; 88.1; 51.8 and 78%. Children younger than 14 years presented better TA (p=0.021). The correlation between the self-referenced TA and the one presumed by the multidisciplinary team ranged from 0,117 to 0.402, being higher for Psychology and Nutrition professionals. Conclusions: The TAI was high particularly among children younger than 14 years. There was a positive correlation between the self-referenced TA and the one presumed by the Psychology (p=0.032) and the nutrition (p=0.012) professionals. [ABSTRACT FROM AUTHOR]

Adhesion level to medicinal treatment by adults diagnosed with hypertension is identified by a descriptive and transversal study involving 213 people diagnosed with hypertension and participating in arterial hypertension campaign in a town in the hinterland of the state of São Paulo, Brazil. Data were collected by half-structured interviews and by Morisky & Green questionnaire. Most participants (84%) failed to adhere to medicinal treatment, with females complying most with adhesion. Adhesion impairments comprised difficulties in changes in lifestyle, irregular visits to the doctor´s and medicine timetable. Adhesion to medicine treatment is a complex and multifactor process that needs special attention by the multiprofessional health team to increase adhesion rates and life quality of patients. Resumo Introdução A doença renal crônica apresenta-se como um problema de saúde pública por causa de sua prevalência, dos custos envolvidos no tratamento e da alta taxa de morbimortalidade. Objetivo Avaliar a não adesão ao regime terapêutico de pacientes em hemodiálise e fatores associados. Método Estudo transversal com pacientes que realizam hemodiálise em um hospital universitário e duas clínicas privadas conveniadas ao Sistema Único de Saúde. Padrões para avaliação da não adesão ao regime terapêutico hemodialítico tiveram como base os indicadores estabelecidos pelo The Dialysis Outcomes and Practice Patterns Study: restrição hídrica, regime dietético, regime medicamentoso e terapia hemodialítica. Não conformidade em pelo menos um dos aspectos do tratamento foi considerada não aderência à terapêutica. Resultados Indivíduos com idade inferior a 60 anos tiveram maior probabilidade de não aderir à restrição hídrica, à terapêutica e aos regimes dietético e medicamentoso. Pacientes anúricos tiveram maior chance de não adesão à restrição hídrica e à terapêutica. Indivíduos com hipoalbuminemia e hipoemoglobinemia tiveram maior probabilidade de não aderir à restrição hídrica, enquanto pacientes que se autodeclararam não brancos apresentaram maior chance de não aderir à terapêutica. Nenhuma variável explicativa da adesão à terapia hemodialítica se manteve significativa no modelo final da regressão logística. Conclusão A inconformidade relacionada aos aspectos do tratamento pode resultar em aumento de hospitalização e gastos com saúde, piora do estado clínico, maiores intercorrências no tratamento e aumento da taxa de mortalidade. Abstract Background Chronic kidney disease presents itself as a public health problem because of its prevalence, the costs involved in treatment and the high rate of morbidity and mortality. Objective To evaluate non-adherence to the therapeutic regimen of hemodialysis patients and associated factors. Method A cross-sectional study with patients undergoing hemodialysis at a University Hospital and two private clinics agreed to the Unified Health System. Standards for non-adherence to the hemodialysis regimen were based on indicators established by The Dialysis Outcomes and Practice Patterns Study: water restrictions, dietary, drug regimen, and hemodialysis therapy. Non-compliance in at least one aspect of treatment was considered non-adherence to therapy. Results Individuals younger than 60 years were more likely to be non-adherent to water restriction, dietary and drug regimens, and therapy. Anuric patients were more likely to be non-adherent to water restriction and therapy. Individuals with hypoalbuminemia and hypohemoglobinemia were more likely not to adhere to water restriction, while patients who self-declared themselves to be non-whites had a higher chance of not adhering to therapy. No explanatory variables of adherence to hemodialysis therapy remained significant in the final logistic regression model. Conclusion Non-conformity related to treatment aspects can result in increased hospitalization and health expenses, worsening of clinical status, greater intercurrences in treatment and increase in mortality rate.

Resumo Objetivo Relacionar a adesão aos antidiabéticos orais e o controle metabólico de pessoas com Diabetes mellitus tipo 2. Métodos Estudo analítico, realizado com 201 participantes, de ambos os sexos, com idade acima dos 18 anos e diagnóstico de diabetes mellitus tipo 2, acompanhados em unidades básicas de saúde. Foram analisadas variáveis sociodemográficas, antropométricas, clínicas, relacionadas à adesão medicamentosa e ao controle metabólico. Para verificar a adesão à terapêutica medicamentosa utilizou-se o Teste de Morisky. Resumo OBJETIVO Identificar as crenças relacionadas à adesão a dieta de pacientes tratados com anticoagulante oral. MÉTODO Estudo descritivo, norteado pela Theory of Reasoned Action, realizado com 57 pacientes em uso contínuo de anticoagulantes orais, atendidos em ambulatório cardiológico geral em João Pessoa/PB e especializado em anticoagulação no Recife/PE. Os dados foram coletados no período de fevereiro a maio de 2017, com aplicação de formulário com perguntas abertas, por meio de entrevista, e submetidos a análise descritiva. RESULTADOS As crenças comportamentais positivas mais referidas foram melhorar a saúde/tratamento no ambulatório geral e controlar a coagulação, no serviço especializado. Filhos, esposo(a), médicos(as) e pais se destacaram, proporcionalmente, em ambos os cenários, como crenças normativas positivas. CONCLUSÃO No ambulatório especializado em anticoagulação oral os pacientes agregam mais crenças específicas acerca da interferência da dieta rica em vitamina K sobre a coagulação, quando comparados com os do ambulatório cardiológico geral.

ABSTRACT Objective: To develop a questionnaire that allows the early detection of patients at risk for poor adherence to medical and non-medical treatment in children and adolescents with chronic rheumatic diseases. Methods: The Pediatric Rheumatology Adherence Questionnaire (PRAQ) was applied in recently diagnosed patients within a period of one to four months after confirmation of the rheumatic disease. After six months, the patients’ adherence to the medical and non-medical treatment was assessed. An internal consistency analysis was conducted to eliminate redundant questions in the PRAQ. Results: A total of 33 patients were included in the pilot study. Six months after the PRAQ had been applied, poor global adherence was observed in seven (21.2%) patients and poor adherence to medical treatment in eight (24.2%) patients. No correlation was observed between the PRAQ scores and the percentages of adherence, as well as the stratification for each index, except for a tendency to a correlation between socioeconomic index and poor adherence to medical treatment (p=0.08). A new PRAQ questionnaire with 25 of the 46 original questions was generated as a result of the reliability analysis. Conclusions: The usefulness of this questionnaire in clinical practice should be still evaluated. Due to the importance of a tool for the early detection of rheumatic patients at risk of poor adherence to treatment, the new PRAQ questionnaire should be reviewed and applied in a larger study to better define its validity and reliability.

RESUMO Objetivo: refletir acerca da adesão ao tratamento da tuberculose à luz do cuidado de si em Michel Foucault. Método: reflexão baseada nos pressupostos de "A hermenêutica do sujeito" de Foucault e literatura pertinente. Resultados: o cuidado de si diz respeito à percepção do sujeito; seu autoconhecimento; e à execução do seu próprio cuidado. Na adesão ao tratamento da tuberculose, o sujeito apresenta mais domínio sobre si, a doença não representa perturbação externa, pois o cuidado desenvolvido ao longo da sua existência não pode ser perturbado, nem mesmo pela doença. Há fatores que podem intensificar essa perturbação: baixa escolaridade, desemprego, etilismo, reingresso após abandono e estigma. Conclusão: torna-se necessário o resgate dos princípios do cuidado de si, o que pode influenciar o voltar-se para si, para que exteriorize o perceber sobre seu tratamento, tornando possível a implementação de ações mais direcionadas à subjetividade do doente e favorecendo o processo de adesão.

43. Prevalence of refractive error and spectacle coverage in schoolchildren in two urban areas of Chile.

Objective. To assess the impact of efforts to reduce visual impairment by detecting and treating refractive errors (REs) among schoolchildren in two urban areas of Chile. Methods. In 2013, in the communities of Concepción and La Florida, we conducted a multicenter, multistage-sampling, cross-sectional study employing methods used by the Refractive Error Study in Children (RESC) survey. Best-corrected visual acuity was assessed using pinhole, with and without glasses. The coverage for correcting visual impairment due to refractive error was estimated. Results. We examined 5 412 schoolchildren, in the communities of Concepción (n = 3 049) and La Florida (n = 2 363). Uncorrected visual acuity (VA) of > 0.63 in at least one eye was found in 4 721 children (87.23%; 95% confidence interval (CI): 86.58%--88.30%). Of the remaining children, 1 017 of them (18.79%; 95% CI: 17.78%--19.80%) had a VA of < 0.5 in at least one eye. Of the 691 children with uncorrected visual impairment with both eyes open (12.77%; 95% CI: 11.9%--13.64%), 476 of them (8.76%; 95% CI: 8.03%--9.49%) achieved normal vision using best possible correction, and with 271 of the 476 (56.93%) having a normal presenting VA in the better eye. Conclusions. We found an increased coverage of services for RE-associated visual impairment and an increased percentage of children wearing glasses as compared to values reported for La Florida in an article published in 2000. Aim: to analyze the scientific literature on the main causes of adherence or not to the treatment of hypertension and type 2 diabetes mellitus. Method: an integrative literature review, which selected full articles published between 2005 and 2015, detected by the descriptors “medication adherence”, “patient compliance”,” type 2 diabetes mellitus”, “hypertension” and “nursing”. The searches were conducted in the Virtual Health Library (Nursing), LILACS and PubMed databases. Results: Twenty-six articles were selected, the factors associated with adherence were: young age, to have a healthy lifestyle and items related to therapy. On the other hand, among the factors that lead to poor adherence, there are lower income, little knowledge about the disease and complex treatment. Conclusions: the adherence factors to the treatment of hypertension and diabetes are related to the patient, to the type of treatment and health system, which constitute important issues to be approached by nurses

45. Respiratory therapy: a problem among children and adolescents with cystic fibrosis

Objective : To evaluate the level of self-reported adherence to physical therapy recommendations in pediatric patients (6-17 years) with cystic fibrosis (CF) and to ascertain whether the different levels of adherence correlate with pulmonary function, clinical aspects, and quality of life. Methods : This was a cross-sectional study. The patients and their legal guardians completed a questionnaire regarding adherence to physical therapy recommendations and a CF quality of life questionnaire. We collected demographic, spirometric, and bacteriological data, as well as recording the frequency of hospitalizations and Shwachman-Kulczycki (S-K) clinical scores. Results : We included 66 patients in the study. Mean age, FEV1 (% of predicted), and BMI were 12.2 ± 3.2 years, 90 ± 24%, and 18.3 ± 2.5 kg/m2, respectively. The patients were divided into two groups: high-adherence (n = 39) and moderate/poor-adherence (n = 27). No statistically significant differences were found between the groups regarding age, gender, family income, and total S-K clinical scores. There were statistically significant differences between the high-adherence group and the moderate/poor-adherence group, the latter showing lower scores for the "radiological findings" domain of the S-K clinical score (p = 0.030), a greater number of hospitalizations (p = 0.004), and more days of hospitalization in the last year (p = 0.012), as well as lower scores for the quality of life questionnaire domains emotion (p = 0.002), physical (p = 0.019), treatment burden (p < 0.001), health perceptions (p = 0.036), social (p = 0.039), and respiratory (p = 0.048). Conclusions : Low self-reported adherence to physical therapy recommendations was associated with worse radiological findings, a greater number of hospitalizations, and decreased quality of life in pediatric CF patients.

Objetivo: Avaliar as evidências disponíveis na literatura sobre os fatores relacionados à não adesão ao tratamento da hipertensão arterial sistêmica no período de 2010 a 2014. Método: Realizou-se uma revisão integrativa nas bases de dados MEDLINE, LILACS e BDENF, conduzida pela seguinte questão norteadora: qual a produção de conhecimento sobre os fatores associados a não adesão de pacientes à terapia anti-hipertensiva? Os descritores empregados na busca pelos estudos foram: hipertensão, cooperação do paciente e adesão à medicação. Após aplicação dos critérios de inclusão foram selecionados 10 estudos para análise e síntese. Resultados: Identificaram-se como principais fatores relacionados a não adesão ao tratamento o baixo nível de escolaridade, o esquecimento dos medicamentos, o baixo nível de conhecimento sobre a doença e regime terapêutico. Conclusão: Inúmeros fatores implicam no fenômeno da não adesão, assim, sugere-se o aprofundamento da temática e o uso de estratégias para aumentar a adesão ao tratamento.

BACKGROUND: In Brazil, since 2015, the treatment of hepatitis C is provided by SUS (Public Health System) with direct-acting antiviral (DAA). OBJECTIVE: To describe the rate of non-adherence patients to hepatitis C treatment by DAA, investigating the epidemiological data in a large database from Curitiba, Brazil. METHODS: Retrospective study with patients treated between January 2015 and June 2019. Patients were considered adherent when received all medication doses during their treatment. The following data were evaluated: gender, age, type of treatment, period of treatment, presence of diabetes or HIV, previous therapy, originated from SUS or private medicine, fibrosis grade and HCV genotype. ABSTRACT Objective: To evaluate the effect of the orientation group on therapeutic adherence and self-care among patients with chronic heart failure. Method: Randomized controlled trial with 27 patients with chronic heart failure. The intervention group received nursing consultations and participated in group meetings with the multi-professional team. The control group only received nursing consultations in a period of four months. Questionnaires validated for use in Brazil were applied in the beginning and in the end of the study to assess self-care outcomes and adherence to treatment. Categorical variables were expressed through frequency and percentage distributions and the continuous variables through mean and standard deviation. The comparison between the initial and final scores of the intervention and control groups was done through the Student’s t-test. Results: The mean adherence in the intervention group was 13.9 ± 3.6 before the study and 4.8 ± 2.3 after the study. In the control group it was 14.2 ± 3.4 before the study and 14.7 ± 3.5 after the study. The self-care confidence score was lower after the intervention (p=0.01). RESUMO A prática de exercícios terapêuticos por idosos visa melhorar o desempenho funcional. A prescrição de exercícios domiciliares é uma prática rotineira na fisioterapia, mas sua efetividade depende da adesão dos idosos, que frequentemente é muito baixa. O objetivo do estudo foi avaliar a adesão e retenção de idosas a um programa domiciliar e identificar os fatores associados a essa prática. Este estudo é observacional exploratório, composto por 96 idosas, (71,3±5,16 anos) submetidas a um programa de exercícios em casa. Foram avaliadas medidas clínicas, funcionais e um questionário de adesão para identificar os fatores motivadores e as barreiras. A taxa de retenção foi de 86% e a de adesão foi de 36%, com o mínimo de 85% de frequência. Os modelos de regressão aplicados não foram significativos, e nas análises estatísticas comparativas entre grupos foi verificada alta incidência de fatores negativos. A ausência de correlação entre pior desempenho funcional e físico com a baixa adesão evidencia que esse desfecho é complexo e multifatorial, e deve ser explorado também sob aspectos sociais, emocionais e culturais.

Objetivo Analisar a prevalência e os fatores associados a cooperação do paciente portador de hipertensão arterial na atenção primária. Métodos Estudo transversal que incluiu com 458 hipertensos selecionados aleatoriamente entre os usuários na assistência primária. As variáveis de estudo foram socioeconômicas, demográficas e clínicas e os instrumentos de pesquisa utilizados eram validados. Foi utilizado o modelo de regressão logística para análise estatística multivariada. Resultados As prevalências encontradas para cooperação do paciente foram 26,6%, 16,6% e 85,6% para os testes de Morisky-Green, Batalla e Haynes-Sackett, respectivamente. Análise de regressão logística ajustada mostrou classe econômica e fumo como os preditores independentes para a cooperação do paciente portador de hipertensão arterial. Conclusão A prevalência de adesão ao tratamento variou de acordo com o instrumento de avaliação utilizado. Os fatores associados à adesão ao tratamento foram: classe socioeconômica elevada, morar com companheiro e ser eutrófico.
