INTRODUCTION AND OBJECTIVES: HeartLogic is a multiparametric algorithm incorporated into implantable cardioverter-defibrillators (ICD). The associated alerts predict impending heart failure (HF) decompensations. Our objective was to analyze the association between alerts and clinical events and to describe the implementation of a protocol for remote management in a multicenter registry. METHODS: We evaluated study phase 1 (the investigators were blinded to the alert state) and phases 2 and 3 (after HeartLogic activation, managed as per local practice and with a standardized protocol, respectively). RESULTS: We included 288 patients from 15 centers. In phase 1, the median observation period was 10 months and there were 73 alerts (0.72 alerts/patient-y), with 8 hospitalizations and 2 emergency room admissions for HF (0.10 events/patient-y). There were no HF hospitalizations outside the alert period. In the active phases, the median follow-up was 16 (95%CI, 15-22) months and there were 277 alerts (0.89 alerts/patient-y); 33 were associated with HF hospitalizations or HF death (n=6), 46 with minor decompensations, and 78 with other events. The unexplained alert rate was 0.39 alerts/patient-y. Outside the alert state, there was only 1 HF hospitalization and 1 minor HF decompensation. NUBES ,EVAPORATION ,CLOUDS ,DROPS ,EVAPORACION ,GOTAS - Abstract
En este trabajo se presentan mediciones de la evaporación de una gota de agua sobre una superficie de vidrio, a una dada temperatura y humedad relativa del ambiente. En particular se determina la variación del diámetro de la superficie de contacto agua-vidrio en función del tiempo. Se reportan resultados correspondientes a la evaporación de una única gota y también para una población de gotas de agua sobre una superficie de vidrio. Se contrastan los resultados con el fin de determinar la influencia de las gotas vecinas sobre la evaporación de gotas individuales. Se comparan los resultados con los reportados por otros autores Measurements of the evaporation of a water droplet over a glass surface at controlled temperature and relative humidity are presented in this work. The diameter of the water-glass surface contact as a function of time is determined. Results belonging to a single droplet and a population of droplets are reported. These results are compared in order to determine the influence of the neighboring drops on the evaporation of individual drops. The results are also compared with those reported by other authors. - Abstract
We report a case of a 43 year old man from Spain, who has been diagnosed with Naxos disease. It is a hereditary disorder characterized by palmoplantar keratoderma, woolly hair and cardiomyopathy, which has been associated with a mutation in plakoglobin encoding gene in chromosome 17q21. In the patient, the direct sequencing of the plakoglobin gene discarded TG deletion at 2157 characteristic of Naxos disease. Analysis of the reported desmoplakin mutations associated with Carvajal Syndrome, another ARVC disease, that it is also accompanied with a skin and hair disorder, also failed to reveal mutations in desmoplakin gene. These results suggest the existence of other causative genes and/or other putative sites in desmoplakin/plakoglobin encoding genes than those recently published., Depto. de Medicina, Fac. de Medicina, TRUE, pub

- Abstract
En este trabajo se presenta un modelo numérico de convección atmosférica tridimensional, que incluye los procesos de condensación de vapor y que tiene en cuenta la variación de los parámetros físicos con la altura. Las ecuaciones pronósticas que se presentan son resueltas por diferencias finitas en una grilla tipo "stagger" con Δx = 500m y Δt = 10s. Se utilizan condiciones periódicas de contorno, en los contornos laterales, lo cual hace posible la resolución, por un método pseudo-espectral, de la ecuación diagnóstica resultante para la perturbación de presión. Un análisis cualitativo de los resultados obtenidos hasta el momento indican que la performance del modelo, en la descripción de la etapa de formación de una nube convectiva, es satisfactorio. Se presenta además una discusión de las ecuaciones a utilizar para simular trayectorias y crecimiento de un granizo en los campos obtenidos con el modelo.

- Abstract
Aims The aim of this study was to determine the utility of pulsed wave Doppler tissue imaging in the evaluation of regional left ventricular diastolic function in patients with ischaemic heart disease.Methods and Results In 30 normal subjects and 43 patients with ischaemic heart disease, Doppler tissue imaging was performed in each of the 16 segments of the myocardium. The following diastolic pulsed wave Doppler tissue imaging parameters were obtained for each segment: (1) regional early diastolic peak velocity (regional e wave cm.s−1); (2) regional late diastolic peak velocity (regional a wave cm.s−1); (3) regional diastolic e/a velocity ratio; and (4) the regional isovolumic relaxation time, defined as the time interval from the second heart sound to the onset of the diastolic E wave. In patients with ischaemic heart disease, each of these parameters was evaluated and compared in ischaemic and normally perfused segments, based on the presence or absence of obstructive lesions of the supplying coronary artery. In patients with coronary artery disease, several differences were observed between diseased and normal wall segments: the mean segmental peak early diastolic velocity (e wave) was reduced (mean±SD: 6·4±2·1cm.s−1vs 8·5±2·8cm.s−1;P<0·01); the e/a diastolic velocity ratio was decreased (0·95±0·3 vs 1·5±0·6, respectively;P<0·01) and the regional isovolumic relaxation time was prolonged (104±36·7ms vs 69·6±30ms;P<0·01. No differences were observed in any of these parameters between the normally perfused segments of ischaemic patients and normal subjects. Patients with a normal transmitral diastolic Doppler inflow pattern had a mean of 3·7±2·7 myocardial segments with a local e/a pulsed wave Doppler tissue imaging velocity ratio <1, fewer than those with an inverted diastolic transmitral Doppler inflow pattern (10·3±3 segments;P<0·001). Overall sensitivity and specificity for an inverted local e/a ratio and a local isovolumetric relaxation time ≥85ms were of 62% and 72% and 69% and 80%, respectively.Conclusion Regional diastolic wall motion is impaired at baseline in ischaemic myocardial segments, even when systolic contraction is preserved. Pulsed wave Doppler tissue imaging is a useful non-invasive technique which allows the assessment of regional diastolic performance and dynamics of the left ventricular myocardium. En el presente trabajo se resuelve la ecuación de transferencia de calor para un cilindro infinito expuesto a un flujo de aire que transporta una densidad homogénea de gotas de agua sobreenfriadas. Se supone que el flujo de aire es del tipo "potencial" y que todas las gotas que impactan sobre el cilindro congelan. Además, se tienen en cuenta todas las variaciones, a lo largo de la superficie del colector, de las fuentes y sumideros de calor, sean debidas a la inercia de las gotas como a los cambios producidos en la capa límite. Las condiciones de contorno se expresan en forma matricial y la solución del problema se encuentra invirtiendo la matriz correspondiente. In Turkey most farmers keep 1-2 buffaloes for family consumption and this system is very widespread in villages while farms with around 100 heads are located near to the big cities. These two most common housing systems were used to contribute to the somatic characterization of Anatolian buffalo in the context of a wide typification programme of this buffalo aimed to improve its productive and reproductive potentiality in agreement with a sustainable development. 76 males and 127 females of the Istanbul district and 32 males and 70 females raised in Danamandra vıllage of Silivri district were measured. On each buffalo, withers height, rump height, body length, chest depth and chest width were determined. The results showed a significant difference between males and females starting from 12 months in buffaloes of Danamandra village and from 3 years of age in animals of Istanbul district.

Aims Cardiac resynchronization therapy (CRT) confers sustained therapeutic benefits to patients suffering from congestive heart failure (CHF) due to systolic dysfunction associated with ventricular dyssynchrony. Biventricular (BiV) pacing has, thus far, been the preferred method, as it corrects both electrical and mechanical dyssynchrony. Left ventricular (LV) only pacing, which has conferred similar benefits in pilot studies, may be an alternative treatment method. ‘Biventricular vs. left univentricular pacing with ICD back-up in heart failure patients’ (B-LEFT HF) is an international, prospective, randomized, parallel-design, double-blind, clinical trial to examine whether LV only pacing is as safe and effective as BiV pacing in patients suffering from CHF. Methods and results The trial will randomly assign 172 patients to either LV only or BiV pacing. The study has prospectively defined efficacy endpoints to be evaluated at 6 months, which are (i) changes in functional capacity and degree of reverse remodelling (primary) and (ii) changes in the heart failure clinical composite response (secondary). Conclusion Because LV only pacing in CRT is likely to be technically less challenging and costly than BiV, a specifically designed study is needed to compare the safety and effectiveness of the two configurations. B-LEFT HF has been designed to settle this critical issue.

- Abstract
Background: Electrophysiological characterization of ventricular tachycardia (VT) isthmus sites is complex and time-consuming. We aimed at developing and validating a global mapping strategy during programmed ventricular stimulation (PVS) to reveal the underlying electrophysiological properties of the infarct-related substrate and to enable identification of highly heterogeneous activation sites associated with protected VT isthmus sites., Methods and Results: Experimental study that included 22 pigs with established myocardial infarction undergoing in vivo characterization of the anatomical and functional myocardial substrate associated with potential arrhythmogenicity. High-density sequential activation maps during ventricular pacing and VT were compared with single-beat maps using a 64-pole basket catheter positioned in the left ventricle. Further analyses were performed using a novel local activation time-dispersion score to identify regional activation time heterogeneities on both baseline drive pacing and each of the extrastimuli of the PVS protocol. Basket catheter splines covered a median of 81.2% of the endocardial surface of the left ventricle. Basket-catheter-derived single-beat activation maps (N=16) during pacing showed a linear relationship with high-density sequential activation maps. Induction of ventricular arrhythmias was associated with higher local activation time-dispersion score values on single-beat global maps during PVS (N=6, 46 arrhythmia induction attempts). Single-beat-derived local activation time-dispersion score maps during successive coupled extrastimuli of the PVS showed a progressive increase in the predictive performance to identify monomorphic VT isthmus sites within the scar region (area under the curve = 0.779 in S2, area under the curve = 0.859 in S4; N=7)., Conclusions: Sixty-four-pole-derived single-beat local activation time-dispersion score global maps during PVS identify infarct-related endocardial regions with highly heterogeneous activation times that are associated with protected VT isthmus sites.

- Abstract
Electromechanical characterization during atrial fibrillation (AF) remains a significant gap in the understanding of AF-related atrial myopathy. This study reports mechanistic insights into the electromechanical remodeling process associated with AF progression and further demonstrates its prognostic value in the clinic. In pigs, sequential electromechanical assessment during AF progression shows a progressive decrease in mechanical activity and early dissociation from its electrical counterpart. Atrial tissue samples from animals with AF reveal an abnormal increase in cardiomyocytes death and alterations in calcium handling proteins. High-throughput quantitative proteomics and immunoblotting analyses at different stages of AF progression identify downregulation of contractile proteins and progressive increase in atrial fibrosis. Moreover, advanced optical mapping techniques, applied to whole heart preparations during AF, demonstrate that AF-related remodeling decreases the frequency threshold for dissociation between transmembrane voltage signals and intracellular calcium transients compared to healthy controls. Single cell simulations of human atrial cardiomyocytes also confirm the experimental results. In patients, non-invasive assessment of the atrial electromechanical relationship further demonstrate that atrial electromechanical dissociation is an early prognostic indicator for acute and long-term rhythm control., (© 2023.
