6. Three-dimensional dose verification of adaptive radiotherapy at an MR-linac

The vision of accurate real-time image-guided radiotherapy (RT) has led to the development of MR-Linacs consisting of MRI scanners integrated within RT treatment units. The safe clinical operation of these devices requires to calculate and verify complex clinical dose distributions in patients exposed to the static magnetic field of the MR-scanner. The effects of the magnetic field on patient dose are particularly pronounced at tissue-air interfaces where an increase in dose occurs due to the electron return effect (ERE). State of the art Monte Carlo (MC) dose calculation algorithms account for this effect, and form the basis for the implementation of unprecedented on-line adaptive treatment strategies. Of particular concern are the treatment sites involving the lung and the pelvic region where the ERE can impact significantly on the dose. In this thesis, I have developed 3D-dosimetry techniques based on PRESAGE(R) dosimeters for the validation of these novel techniques. In contrast to most conventional radiation detectors, this dosimeter is not affected by the magnetic field and provides uncompromised 3D dose information, suitable for end-to-end testing of adaptive workflows in MRI-guided RT. The hitherto inability of retrieving dose using the 3D dosimeters periphery was solved, enabling for the first time, measuring the ERE in 3D and benchmarking the results with MC calculations. By exploiting the full volume of PRESAGE(R) dosimeters and using a commercial phantom, I developed a reproducible and versatile methodology that was easily adapted to perform a variety of tests at the Elekta MR-linac. Different treatment sites and clinically available workflows were assessed, and excellent 3D agreement between calculations and measurements confirmed their correct implementation. With the same methodology, I performed the first dose verification of MLC-tracked RT at the Elekta MR-linac.

This research explores homicidal bereavement experiences, particularly for individuals with longer-term difficulties after their loss, and evaluates the impact of a residential, psychoeducational intervention offered by a national charity, Escaping Victimhood (EV). To achieve those goals, a longitudinal mixed method design was implemented prospectively with participants as they attended an EV intervention and retrospectively with those who had attended 2-5 years before. The literature review (Chapter 1) demonstrates that violent losses (homicide) tend to have greater impacts on individuals' lives (e.g., psychological, financial and social difficulties), compared with non-violent deaths, given the particular characteristics of the homicide itself and aftermath post-event. Furthermore, this review highlighted a lack of evidence-based research investigating what psychological interventions have been evaluated. This led to the systematic review (Chapter 2) to evaluate which interventions are most available, as well as how EV elements compared with those interventions. This is followed by three empirical chapters (chapters 3-5). Chapter 3 (qualitative study) explores the individuals' perceptions about the post-homicide reality, impacts and experiences of support. Chapter 4, a longitudinal study estimates patterns of psychological difficulties, coping and resilience over time and post-EV intervention. Finally, Chapter 5 qualitatively explored how 29 EV participants (14 were interviewed 6-9 months and 15 at 2-5 years post-EV intervention) progressed over time and what changes, barriers to recovery and future hopes they report. The overall discussion (Chapter 6) considers the findings of the two reviews and empirical studies performed where the findings from the outlined independent studies are embedded in three main areas, namely: 1) Post-homicide reality; 2) Psychological difficulties & coping and resilience patterns; and 3) Interventions and support needs. Finally, limitations, recommendations for EV, clinical practice, policy and future research directions are included.

10. Myositis-Associated Interstitial Lung Disease: The Experience of a Tertiary Center.

Background: Interstitial lung disease (ILD) is a common extra-muscular manifestation of idiopathic inflammatory myopathies (IIMs), often associated with a poorer prognosis and increased mortality risk. Methods: This retrospective study aimed to characterize lung involvement and treatment response in an IIM cohort at a Portuguese tertiary center, followed between June 2016 and March 2024. We analyzed data from high-resolution computed tomography (HRCT) scans and pulmonary function tests (PFTs) to assess associations with autoantibody profiles and treatment regimens. Results: A total of 198 patients were included, with 69 (34.8%) exhibiting ILD. Antisynthetase syndrome (ASyS) and dermatomyositis were the most common diagnoses among IIM-ILD patients, with ASyS being significantly more frequent in this group than in non-ILD patients (p < 0.001). Anti-Jo1 and anti-MDA-5 antibodies were more frequent in ILD patients (p < 0.001 and p = 0.021), while anti-Mi2 antibodies were less common (p = 0.002). Non-specific interstitial pneumonia (NSIP) was the most common radiological pattern (69.5%). IIM-ILD patients presented with significantly lower forced vital capacity (FVC) and diffusing capacity of the lung for carbon monoxide (DLCO) compared to non-ILD patients (p < 0.001 for all values). Longitudinal analysis showed improved DLCO (p = 0.022) and stable or improved FVC (p = 0.097), especially with intravenous immunoglobulin (IVIg) and azathioprine (AZA). Combination therapies including IVIg with mycophenolate mofetil (MMF) or rituximab (RTX) also improved DLCO and FVC. Most ILD patients (89.6%) had stable HRCT patterns over time. Conclusions: Our findings highlight the potential for stabilizing or even improving lung function in IIM-ILD with appropriate immunosuppressive therapy, particularly with regimens incorporating IVIg and AZA, and combination therapies. Background Physical inactivity is a major risk factor for the development of chronic diseases, and it is increasingly prevalent in the Portuguese population. Pharmacists’ role in promoting physical activity (PA) is still not well established, although health promotion is foreseen by law in Portugal. Competing tasks and location where the pharmacy is embedded can hinder this promotion in their daily practice. Objective The aim of this study was to identify the main barriers and facilitators of physical activity promotion (PAP) in Portuguese community pharmacies and explore possible pathways for future implementation of physical activity promotion. Methods In-depth, semi-structured interviews were conducted with purposively enrolled community pharmacists. Participant recruitment was aligned with data saturation. Data analysis comprised a mixed model of a deductive theme mapping strategy using the Theoretical Domains Framework (TDF) for the behaviour of promoting physical activity and an inductive approach for any other relevant themes and which might influence PA promotion. Results Data saturation was reached at eleven interviews. Barriers and facilitators for the behaviour of promoting PA were identified from 11 out of the 14 TDF domains. Following an inductive approach, other emerging codes were clustered in additional seven major themes. Highlighted barriers focused on domains #1 – Knowledge, #10 – Memory, Attention and Decision Processes and #13 – Environmental Context and Resources. Community mapping, establishment of remuneration models and the use of digital technologies were suggested as additional potential contributors to scale up PAP. Conclusion Community pharmacists are well placed inside their communities to serve as a focal point for signposting, engagement with other healthcare professionals and community resources and activities organized by the pharmacy itself. Background: New drugs for locally advanced or metastatic breast cancer have led to clinical benefits, aside with increasing costs to healthcare systems. The current financing model for health technology assessment (HTA) privileges real-world data. As part of the ongoing HTA, this study aimed to evaluate the effectiveness of palbociclib with aromatase inhibitors (AI) and compare it with the efficacy reported in PALOMA-2. Methods: A population-based retrospective exposure cohort study was conducted including all patients initiating treatment in Portugal with palbociclib under early access use and registered in the National Oncology Registry. The primary outcome was progression free survival (PFS). Secondary outcomes considered included time to palbociclib failure (TPF), overall survival (OS), time to next treatment (TTNT), and proportion of patients discontinuing treatment due to adverse events (AEs). The Kaplan–Meier method was used and median, 1- and 2-year survival rates were computed, with two-sided 95% confidence intervals (95%CI). STrengthening the Reporting of OBservational studies in Epidemiology (STROBE) guidelines for reporting observational studies were used. Results: There were 131 patients included. Median follow-up was 28.3 months (IQR: 22.7–35.2) and median duration of treatment was 17.5 months (IQR: 7.8–29.1). Median PFS was 19.5 months (95%CI 14.2–24.2), corresponding to a 1-year PFS rate of 67.9% (95%CI 59.2–75.2) and a 2-year PFS rate of 42.0% (95%CI 33.5–50.3). Sensitivity analysis showed median PFS would increase slightly when excluding those not initiating treatment with the recommended dose, raising to 19.8 months (95%CI 14.4–28.9). By considering only patients meeting PALOMA-2 criteria, we could observe a major difference in treatment outcomes, with a mean PFS of 28.8 months (95%CI 19.4–36.0). TPF was 19.8 months (95%CI 14.2–24.9). Median OS was not reached. Median TTNT was 22.5 months (95%CI 18.0–29.8). A total of 14 patients discontinued palbociclib because of AEs (10.7%). Conclusions: Data suggest palbociclib with AI to have an effectiveness of 28.8 months, when used in patients with overlapping characteristics to those used in PALOMA-2. However, when used outside of these eligibility criteria, namely in patients with less favorable prognosis (e.g., presence of visceral disease), the benefits are inferior, even though still favorable.

This thesis aimed to adapt a survey tool to measure patients’ desires for information (EID), perceptions about medicines (PHM and PBM) and anxiety about illness (Ai and Ti) from the UK to Portugal and to explore how perceptions impact on medicines taking behaviours in different care settings. The survey tool was adapted using a multi-method approach comprising translation and back-translation, rating of equivalence, and assessment of understanding in various stages of revision. The tool was tested in a patient sample recruited in different care settings in Portugal and the UK and responses were used to explore validity and reliability. Data was used to compare scores to the scales between patient samples and to investigate factors influencing adherence behaviours. The survey tool was modified during validation, where the Ti scale was dropped for its validity and reliability. Four factors emerged in the course of factor analysis. One item was dropped in the PBM and PHM scales, which were weighed for comparison. Internal consistency of the Ai (α=0.756) and PBM (α=0.695) scales were good, while the EID (α=0.607) and PHM (α=0.536) were acceptable. All scales, except the EID, were consistent over time [p(r) < 0.001; p(t) > 0.05]. Portuguese, community recruited, female, higher educated and younger patients tended to desire more information. Patients prescribed more medicines tended to perceive medicines as more harmful. Anxiety tended to be higher in hospitalized patients, the elderly and lower educated and those prescribed more medicines. Patients feeling more anxious (ρ=0.420; p=0.000) and perceiving their health status as bad (ρ=-0.213 p=0.000) tended to perceive a greater harm from medicines. Logistic modelling showed that patients aged > 60 [Adj.OR=2.20 {1.38-3.53}] and patients scoring > 17 on the EID scale [Adj.OR=2.57; {1.36-4.84}] were more likely to be compliant (p < 0.05). These findings highlight the importance of tailoring information provision to patient’s needs.

Publisher Copyright: © 2023, The Author(s). Background: New drugs for locally advanced or metastatic breast cancer have led to clinical benefits, aside with increasing costs to healthcare systems. The current financing model for health technology assessment (HTA) privileges real-world data. As part of the ongoing HTA, this study aimed to evaluate the effectiveness of palbociclib with aromatase inhibitors (AI) and compare it with the efficacy reported in PALOMA-2. Methods: A population-based retrospective exposure cohort study was conducted including all patients initiating treatment in Portugal with palbociclib under early access use and registered in the National Oncology Registry. The primary outcome was progression free survival (PFS). Secondary outcomes considered included time to palbociclib failure (TPF), overall survival (OS), time to next treatment (TTNT), and proportion of patients discontinuing treatment due to adverse events (AEs). The Kaplan–Meier method was used and median, 1- and 2-year survival rates were computed, with two-sided 95% confidence intervals (95%CI). STrengthening the Reporting of OBservational studies in Epidemiology (STROBE) guidelines for reporting observational studies were used. Results: There were 131 patients included. Median follow-up was 28.3 months (IQR: 22.7–35.2) and median duration of treatment was 17.5 months (IQR: 7.8–29.1). Median PFS was 19.5 months (95%CI 14.2–24.2), corresponding to a 1-year PFS rate of 67.9% (95%CI 59.2–75.2) and a 2-year PFS rate of 42.0% (95%CI 33.5–50.3). Sensitivity analysis showed median PFS would increase slightly when excluding those not initiating treatment with the recommended dose, raising to 19.8 months (95%CI 14.4–28.9). By considering only patients meeting PALOMA-2 criteria, we could observe a major difference in treatment outcomes, with a mean PFS of 28.8 months (95%CI 19.4–36.0). TPF was 19.8 months (95%CI 14.2–24.9). Median OS was not reached. Median TTNT was 22.5 months (95%CI 18.0–29.8). A total of 14 patients discontinued palbociclib because of AEs (10.7%). Conclusions: Data suggest palbociclib with AI to have an effectiveness of 28.8 months, when used in patients with overlapping characteristics to those used in PALOMA-2. However, when used outside of these eligibility criteria, namely in patients with less favorable prognosis (e.g., presence of visceral disease), the benefits are inferior, even though still favorable. publishersversion published

Background Health-related quality of life (HRQOL) is a critical aspect to consider when making treatment decisions for patients with non-Hodgkin-lymphoma (NHL). This international study by the European Organisation for Research and Treatment of Cancer (EORTC) tested the psychometric properties of two newly developed measures for patients with high-grade (HG)- and low-grade (LG)-NHL: the EORTC QLQ-NHL-HG29 and the EORTC QLQ-NHL-LG20 to supplement the core questionnaire (EORTC QLQ-C30). Methods Overall, 768 patients with HG-NHL (N = 423) and LG-NHL (N = 345) from 12 countries completed the QLQ-C30, QLQ-NHL-HG29/QLQ-NHL-LG20 and a debriefing questionnaire at baseline, and a subset at follow-up for either retest (N = 125/124) or responsiveness to change (RCA; N = 98/49). Results Confirmatory factor analysis showed an acceptable to good fit of the 29 items of the QLQ-NHL-HG29 on its five scales (symptom burden [SB], neuropathy, physical condition/fatigue [PF], emotional impact [EI], and worries about health/functioning [WH]), and of the 20 items of the QLQ-NHL-LG20 on its four scales (SB, PF, EI, and WH). Completion took on average 10 minutes. Test-retest reliability, convergent validity, known-group comparisons, and RCA find satisfactory results of both measures. A total of 31%-78% of patients with HG-NHL and 22%-73% of patients with LG-NHL reported symptoms and/or worries (e.g., tingling in hands/feet, lack of energy, and worries about recurrence). Patients reporting symptoms/worries had substantially lower HRQOL compared to those without. Discussion The use of the EORTC QLQ-NHL-HG29 and QLQ-NHL-LG20 questionnaires in clinical research and practice will provide clinically relevant data to better inform treatment decision-making. Introduction: Information systems are increasing prominent in institutions, as they become a primary resource in the management of health services and action strategies. One of the main goals of health information systems is to promote continuity of care, for which it is necessary to fulfill the requirements and the contents that enables its interoperability between the various intervening. Objetive: Understand the existing evidence regarding the advantages of a shared information system in nursing. Methods: A literature review was carried out, using the platform EBSCOhost with the search for the keywords “shar* information” present in the title AND “nursing” present in the abstract. Results: The existence of a shared information system has advantages to the patient, to the caregiver and to health professional, which includes improving communication between health professionals and patients/caregivers and the possibility of keeping them informed about their condition, promoting their literacy and ability to adhere to the treatment plan, as well as the adherence to healthy behaviours. Conclusion: The existence of a quality information system, made available in a simple, uniform and secure way, allows access to health data in a controlled manner, both by health professionals and citizens, facilitating the monitoring of health status., Introducción: Los sistemas de información son cada vez más importantes en las organizaciones, ya que se han convertido en un recurso primordial en la gestión de los servicios de salud y en las estrategias de acción. Uno de los principales objetivos de los sistemas de información en salud consiste en promover la continuidad de la atención, para lo cual es necesario cumplir con requisitos estructurales y de contenido que permitan su interoperabilidad entre los distintos profesionales. Objetivo: Conocer la evidencia existente sobre las ventajas de un sistema de información compartida en enfermería. Métodos: Se realizó una revisión de la literatura, utilizando plataforma EBSCOhost com la búsqueda de las palabras clave “shar* information” presente en el título Y “nursing” presente en el resumen. Resultados: La existencia de un sistema de información compartido trae ventajas para el paciente, el cuidador y el profesional de la salud, que incluyen la mejora de la comunicación entre los profesionales de la salud y los pacientes/cuidadores y la posibilidad de mantenerlos informados sobre su condición de salud, promoviendo su literaria y capacidad de adherirse al plan de tratamiento, así como la adopción de comportamientos saludables. Conclusión: La existencia de un sistema de información de calidad, disponible de forma sencilla, uniforme y segura, permite el acceso a los datos de salud de forma controlada, tanto por parte de los profesionales sanitarios como de los ciudadanos, facilitando el seguimiento del estado de salud., Introdução: Os sistemas de informação têm cada vez mais destaque nas organizações, uma vez que se tornaram um recurso primordial na gestão dos serviços de saúde e nas estratégias de ação. Um dos principais objetivos dos sistemas de informação na saúde é promover a continuidade dos cuidados, sendo para isso necessário cumprir requisitos estruturais e de conteúdos que possibilitem a sua interoperabilidade entre os vários intervenientes. Objetivo: Conhecer a evidência existente sobre as vantagens de um sistema de informação partilhado em enfermagem. Métodos: Foi realizada uma revisão da literatura, com utilização da plataforma EBSCOhost e com a pesquisa das palavras-chave “shar* information” presente no título AND “nursing” presente no resumo. Resultados: A existência de um sistema de informação partilhado traz vantagens para o cliente, para o cuidador e para o profissional de saúde, que passam pela melhoria da comunicação entre todos os intervenientes e pela possibilidade de mantê-lo informado sobre o seu estado de saúde, promovendo a sua literacia e capacidade de adesão ao plano de tratamento, bem como a adoção de comportamentos saudáveis. Conclusão: A existência de um sistema de informação de qualidade, disponibilizado de forma simples, uniforme e segura, possibilita o acesso aos dados de saúde, de forma controlada, tanto por parte dos profissionais de saúde, como do cidadão, facilitando o acompanhamento do estado de saúde.
