Zusammenfassung Hintergrund Schwerhörigkeit ist ein relevanter Risikofaktor einer Demenz. Bisher kann eine kognitive Beeinträchtigung oder Demenz von Personen mit Hörbeeinträchtigungen (HB) wegen des eingeschränkten Sensoriums von gängigen kognitiven Screeningverfahren nicht adäquat erfasst werden. Deshalb besteht ein Bedarf eines solchen an Hörgeschädigte angepassten Screenings. Ziel der Studie war es, ein kognitives Screening für Menschen mit HB zu entwickeln und zu evaluieren. Material und Methoden Hierzu wurden drei Aufgaben, eine Wortflüssigkeitsaufgabe, der Trail Making Test A (TMT-A) und eine Subtraktionsaufgabe in einem neuen kognitiven Screening, dem O‑DEM, zusammengefasst. Dieser wurde in einem ersten Schritt an 2837 Patient*innen ohne subjektive HB und anschließend an 213 Patient*innen mit objektiv erfassten HB evaluiert und mit dem Hearing-Impaired Montreal Cognitive Assessment (HI-MoCA) verglichen. Ergebnisse Es zeigte sich, dass jeder O‑DEM-Subtest signifikant zwischen keinen, leichten und mittleren bis ausgeprägten kognitiven Beeinträchtigungen unterscheiden kann. Basierend auf dem Mittelwert und der Standardabweichung der Menschen ohne kognitive Beeinträchtigungen wurde eine Transformation der Rohwerte vorgenommen und ein Gesamtscore mit einer maximalen Ausprägung von 10 festgelegt. Ebenso zeigte die Untersuchung an den hörgeschädigten Patient*innen, dass der O‑DEM genauso sensitiv wie der HI-MoCA zwischen Menschen mit und ohne kognitive Beeinträchtigungen differenzieren kann. Schlussfolgerungen Der O‑DEM ist gegenüber anderen Verfahren ein vergleichbar schnell durchzuführendes Screening zur Detektion von leichten und mittleren kognitiven Beeinträchtigungen bei Menschen mit HB.

Zusammenfassung Hintergrund Die Digitalisierung im Gesundheitswesen hat unter der COVID-19-Pandemie rasant zugenommen. Bislang fand Hörtraining nach Cochleaimplantation meist Face-to-Face statt, doch die Kontaktvermeidung erschwert diesen Therapieansatz. Material und Methoden Insgesamt 42 erwachsene Cochleaimplantat(CI)-Träger im Alter von 53,8 (±15,6) erhielten 1×/Woche über 5 Wochen Videotherapie im Rahmen der Folgetherapie nach Cochleaimplantation. Nach jeder Therapieeinheit erfolgte eine Dokumentation hinsichtlich des Ablaufs und der Therapieinhalte. Nach Studienende wurden neben einer Kosten-Nutzen-Analyse das Konzept und die Benutzerfreundlichkeit anhand der System Usability Scale (SUS) und eines eigenen Abschlussfragebogens zur Videotherapie sowie die Therapeuten-Patienten-Beziehung mit der Skala Therapeutische Allianz – Revised (STA-R) sowohl von Patienten als auch von Therapeuten bewertet. Ergebnisse Gleichermaßen hoch schätzten beide Usergruppen die Benutzerfreundlichkeit ein (87,9 vs. 93,0). Trotz des fehlenden persönlichen Kontakts wurde die therapeutische Allianz sehr positiv angesehen (87,8 % vs. 84,8 %). Die therapeutischen Bedürfnisse der Patienten konnten in 47,6 % vollständig durch die Videotherapie abgedeckt werden. Der größte Vorteil für die Patienten lag in der Zeit- und Kostenersparnis. Für die Rehabilitationseinrichtung entstanden zunächst mehr Kosten aufgrund einer längeren Therapievorbereitung. Auch traten in > 75 % der ersten Therapieeinheiten technische Probleme auf. Langfristig war die Durchführung der Therapie hierdurch nicht beeinträchtigt. Schlussfolgerung Videogestütztes Hörtraining wird als nützlich beurteilt und auch zukünftig gewünscht. Ob die positiv erlebte therapeutische Allianz auch über einen längeren Therapiezeitraum aufrechtzuerhalten sein wird und wie effektiv Videotherapie ist, bedarf weiterer Studien.

After cochlear implantation, auditory and speech training is usually necessary. Up until now, this has been performed on an out- or inpatient basis in direct contact with a therapist. Due to technical advances but also particularly because of an increasing use of digital media and a desire for mobility and independence, the demand for digital training programs is increasing in the field of hearing rehabilitation.In a first step, the most important elements of auditory training were determined on the basis of commonly used auditory and speech processing models, and the core features of a computer-based teletherapeutic hearing platform were defined. This process considered motivational elements and adaptive mechanisms as regularly used in educational and speech and language therapy. In a second step, an initial prototype of the "train2hear" teletherapeutic hearing platform was developed by an interdisciplinary research team.The core of the train2hear platform is an initial analysis according to the International Classification of Functioning, Disability, and Health (ICF), on the basis of which an individual training plan is designed. Various adaptive mechanisms enable the level of difficulty to be continually adapted to the individual user's learning progress. A videoconference tool enables contact between the patient and the therapist.The train2hear platform represents a first prototype of a computer-based auditory rehabilitation program in German. Further evaluation and development in a clinical study are needed to determine the feasibility and efficacy of the platform.EINLEITUNG: Nach einer Cochleaimplantation (CI) ist i.d.R. ein Hör- und Sprachtraining erforderlich, das bislang entweder ambulant oder stationär im direkten Kontakt mit einem Therapeuten erfolgt. Bedingt durch technische Weiterentwicklungen, aber vor allem durch die zunehmende Nutzung digitaler Medien und dem Wunsch nach Mobilität und Selbständigkeit steigt der Bedarf an digitalen Trainingsprogrammen auch im Rahmen der Hörrehabilitation.In einem ersten Schritt wurden entsprechend den gängigen Hör- und Sprachverarbeitungsmodellen die wichtigsten Elemente eines Hörtrainings definiert und die Kernbausteine für eine computerbasierte teletherapeutische Hörplattform bestimmt. Dabei wurden lerntherapeutisch orientierte Elemente der Motivationsförderung sowie Mechanismen der Adaptivität, wie sie im Rahmen der logopädischen Therapie eingesetzt werden, berücksichtigt. In einem zweiten Schritt erfolgte die Entwicklung eines ersten Prototyps der teletherapeutischen Hörplattform „train2hear“ im Rahmen eines interdisziplinären Forschungsprojektes.Kernbaustein des vorgestellten „train2hear“ Hörtrainings ist eine an die „International Classification of Functioning, Disability, and Health“ (ICF) angelehnte Eingangsanalyse, aufgrund derer ein individueller Trainingsplan erstellt wird. Verschiedene Adaptivitätsmechanismen dienen dazu, den Schwierigkeitsgrad kontinuierlich an den individuellen Lernfortschritt des Nutzers anzupassen. Ein teletherapeutisches Videotool ermöglicht den Austausch mit dem Therapeuten vor Ort.Die vorgestellte Plattform „train2hear“ stellt einen ersten deutschsprachigen Prototyp einer computerbasierten Hörrehabilitation dar. Dieser muss nun im Rahmen einer klinischen Studie im Hinblick auf die Anwendbarkeit und Effektivität evaluiert und weiterentwickelt werden.

Die in dem letzten Jahrhundert in den Industrienationen zu beobachtende Alterung der Gesellschaft fuhrt neben der erfreulichen Alterszunahme der Menschen auch zu einer Zunahme der Haufigkeit altersbedingter Erkrankungen und von Multimorbiditat. Neben der Schluckstorung und dem Schwindel sind sensorische Defizite, wie Riech- und Horstorungen, beim alten Menschen haufig und tragen wesentlich zur Lebensqualitat bei. Die Alzheimer-Demenz ist die haufigste Demenzform. Kurzlich wurde in epidemiologischen Studien gezeigt, dass Riech- und Horstorungen nicht nur eine Assoziation mit demenziellen Erkrankungen zeigen, sondern auch spezifische Risikofaktoren fur das Auftreten und die Progression der Erkrankung darstellen. Somit kommt der Erkennung und der adaquaten Behandlung z. B. von Horstorungen durch den HNO-Arzt eine immer wichtiger werdende Rolle zu, nicht nur um die Lebensqualitat der Patienten zu erhalten, sondern in Zukunft auch, um das Risiko fur Demenzerkrankungen zu reduzieren.

Background The senses serve as the crucial interface between the individual and the environment. They are subject to aging and disease processes. Methods This review is based on pertinent publications retrieved by a selective search in the Medline and Cochrane Library databases. Results Approximately 40% of persons aged 70 to 79 manifest dysfunction in at least one, and more than 25% in multiple senses. Sensory changes are accompanied by diverse comorbidities which depend on the particular sense(s) affected. The presence of sensory deficits is associated with an increased risk of developing dementia (OR: 1.49 [95% confidence interval: 1.12; 1.98] for dysfunction in a single sensory modality, 2.85 [1.88; 4.30] for dysfunction in three or more sensory modalities). The risk of developing depressive symptoms is elevated as well (OR 3.36 [2.28; 4.96]). The individual's ability to cope with the demands of everyday life is largely determined by the ability to carry out multisensory integration, in which the perceptions of the different senses are bound together. This function itself is subject to age-related changes that can be either adaptive or maladaptive; it can, therefore, serve as an indicator for pathological aging processes. Conclusion Sensory dysfunction in old age should be detected as early as possible. This implies the need for close collaboration of all of the involved disciplines. It would be desirable to develop sensory screening tests as well as a procedure for testing multisensory integration in routine clinical practice.

Background Cochlear implants can provide auditory perception to many people with hearing impairment who derive insufficient benefits from hearing aid use. For optimal speech perception with a cochlear implant, postoperative auditory training is necessary to adapt the brain to the new sound transmitted by the implant. Currently, this training is usually conducted via face-to-face sessions in rehabilitation centers. With the aging of society, the prevalence of age-related hearing loss and the number of adults with cochlear implants are expected to increase. Therefore, augmenting face-to-face rehabilitation with alternative forms of auditory training may be highly valuable. Objective The purpose of this multidisciplinary study was to evaluate the newly developed internet-based teletherapeutic multimodal system Train2hear, which enables adult cochlear implant users to perform well-structured and therapist-guided hearing rehabilitation sessions on their own. Methods The study was conducted in 3 phases: (1) we searched databases from January 2005 to October 2018 for auditory training programs suitable for adult cochlear implant users; (2) we developed a prototype of Train2hear based on speech and language development theories; (3) 18 cochlear implant users (mean age 61, SD 15.4 years) and 10 speech and language therapists (mean age 34, SD 10.9 years) assessed the usability and the feasibility of the prototype. This was achieved via questionnaires, including the System Usability Scale (SUS) and a short version of the intrinsic motivation inventory (KIM) questionnaires. Results The key components of the Train2hear training program are an initial analysis according to the International Classification of Functioning, Disability and Health; a range of different hierarchically based exercises; and an automatic and dynamic adaptation of the different tasks according to the cochlear implant user’s progress. In addition to motivational mechanisms (such as supportive feedback), the cochlear implant user and therapist receive feedback in the form of comprehensive statistical analysis. In general, cochlear implant users enjoyed their training as assessed by KIM scores (mean 19, SD 2.9, maximum 21). In terms of usability (scale 0-100), the majority of users rated the Train2hear program as excellent (mean 88, SD 10.5). Age (P=.007) and sex (P=.01) had a significant impact on the SUS score with regard to usability of the program. The therapists (SUS score mean 93, SD 9.2) provided slightly more positive feedback than the cochlear implant users (mean 85, SD 10.3). Conclusions Based on this first evaluation, Train2hear was well accepted by both cochlear implant users and therapists. Computer-based auditory training might be a promising cost-effective option that can provide a highly personalized rehabilitation program suited to individual cochlear implant user characteristics.

BACKGROUND Speech audiometry plays an important role for hearing evaluation in children. Hearing impaired children, such as those with cochlear implant (CI) who frequently undergo testing, learn the test words by heart. Furthermore, the standard tests use words of former time. PATIENTS AND METHODS Using the most frequently words of modern children's literature a speech assessment test was developed for children between 1 to 8 years. The material was first evaluated on normal hearing children. In a second step the phoneme and word perception scores of 35 CI children were measured using 60 - 100 dB SPL according to the most comfortable level (MCL) in free sound field. RESULTS The performance-intensity function for the single words were highly comparable with those of the Freiburger or Gottinger speech perception test. The average speech reception threshold in CI children was 44 % (test I), 36 % (test II), 63 % (test III) and 69 % (test IV), respectively. Compared to the Mainzer and Gottinger speech perception test the new words were more difficult to understand than those of the known tests. However, a statistically significant difference could be noted only for test II. CONCLUSIONS The new developed "Wurzburger speech perception test" appears to be a useful additional tool for the evaluation of hearing impaired children in addition to the standard test procedures.

Background Recent studies revealed a predictive value of lymphatic vessel invasion (L1) for the nodal metastasizing and poor prognosis in malignant tumors at different sites. The monoclonal antibody D2-40 (podoplanin) stains specifically endothelial cells of lymphatic vessels and improves the search for L1. However, the importance of this immunohistochemical staining was not investigated in squamous cell carcinomas (SCC) of larynx and hypopharynx. Aim This study was performed to compare the diagnostic potential of convential and immunohistochemical determination of L1 in SCC of larynx and hypopharynx with special respect to the predictive value for nodal metastasizing and prognosis. Methods 119 SCCs of the larynx (n = 70) respectively hypopharynx (n = 49) were investigated. The lymphatic vessel invasion was assessed by conventional method (HE stain) and immunohistochemical staining with an antibody against D2-40 (DAKO, Germany). Immunohistochemistry was performed in accordance with manufacturer's protocol. L1 was searched microscopically in a standardized magnification (×200) in serial sections of tumor samples (1 section per cm tumor diameter). Results The immunohistochemical investigation did not show significant advantages for the prediction of regional nodal metastases. Despite a low sensitivity (< 50%) in both methods, the specifity can reach 80%. The negative predictive value in both methods seems acceptable (up to 80%), whereas the positive predictive value is not higher than 64%. Cases with L1 detected either conventionally or immunohistochemically did not show a significant shorter survival than cases with L0. However, a non-significant shorter survival was found. Only in SCC of hypopharynx, a combination of both methods revealed patients with a significant worse prognosis. Conclusion The status of lymphatic vessel invasion should be documented in standardized tumor reports. A benefit of an additional immunohistochemical investigation was not found, for the daily routine HE-stain seems sufficient.
