Abstract

Photovoltaic (PV) solar energy generating capacity has grown by 41 per cent per year since 2009.sup.1. Energy system projections that mitigate climate change and aid universal energy access show a nearly ten-fold increase in PV solar energy generating capacity by 2040.sup.2,3. Geospatial data describing the energy system are required to manage generation intermittency, mitigate climate change risks, and identify trade-offs with biodiversity, conservation and land protection priorities caused by the land-use and land-cover change necessary for PV deployment. Currently available inventories of solar generating capacity cannot fully address these needs.sup.1-9. Here we provide a global inventory of commercial-, industrial- and utility-scale PV installations (that is, PV generating stations in excess of 10 kilowatts nameplate capacity) by using a longitudinal corpus of remote sensing imagery, machine learning and a large cloud computation infrastructure. We locate and verify 68,661 facilities, an increase of 432 per cent (in number of facilities) on previously available asset-level data. With the help of a hand-labelled test set, we estimate global installed generating capacity to be 423 gigawatts (-75/+77 gigawatts) at the end of 2018. Enrichment of our dataset with estimates of facility installation date, historic land-cover classification and proximity to vulnerable areas allows us to show that most of the PV solar energy facilities are sited on cropland, followed by aridlands and grassland. Our inventory could aid PV delivery aligned with the Sustainable Development Goals. Patients were randomized to receive either post-operative opioid education by the surgical team (control arm) or pre-operative education via a novel 5-minute video plus post-operative education by the surgical team (intervention arm). All patients received follow-up phone calls on post-operative day (POD) 7 to assess self-rated knowledge of opioids on a scale of 1 to 10, 10 being most informed, and self-reported opioid use (primary outcome). Arkansas Prescription Drug Monitoring Program (AR PDMP) data were obtained to assess opioid dispensations at POD 90– 150 days.Results: Seventy-seven percent of participants completed POD7 survey. Participants in the intervention arm rated their knowledge of opioids after surgery significantly higher than those in the control arm (p=0.013). Data from the AR PDMP reveal trends (non-significant) that show increased use of opioids in the control group when compared to the video intervention group at POD 30 (88.7% vs 76%) and POD 90– 150 (22.6% vs 10%).Conclusion: Video-based, patient-focused opioid education can be effectively implemented in a large university hospital during the perioperative period and is effective for increasing a patient’s perception of opioid-related knowledge. A fully powered, randomized control trial is needed to further explore observed trends and determine if this novel tool can decrease chronic post-operative opioid dispensations.Keywords: opioids, perioperative, video education, acute pain, chronic pain, ambulatory surgery

To date, 18 living recommendations for the clinical care of pregnant and postpartum women with COVID-19 have been issued by the National COVID-19 Clinical Evidence Taskforce. This includes recommendations on mode of birth, delayed umbilical cord clamping, skin-to-skin contact, breastfeeding, rooming-in, antenatal corticosteroids, angiotensin-converting enzyme inhibitors, disease-modifying treatments (including dexamethasone, remdesivir and hydroxychloroquine), venous thromboembolism prophylaxis and advanced respiratory support interventions (prone positioning and extracorporeal membrane oxygenation). Through continuous evidence surveillance, these living recommendations are updated in near real-time to ensure clinicians in Australia have reliable, evidence-based guidelines for clinical decision-making. Abstract

Summary Sarcomas are a broad family of mesenchymal malignancies exhibiting remarkable histologic diversity. We describe the multi-platform molecular landscape of 206 adult soft tissue sarcomas representing 6 major types. Along with novel insights into the biology of individual sarcoma types, we report three overarching findings: (1) unlike most epithelial malignancies, these sarcomas (excepting synovial sarcoma) are characterized predominantly by copy-number changes, with low mutational loads and only a few genes ( TP53 , ATRX , RB1 ) highly recurrently mutated across sarcoma types; (2) within sarcoma types, genomic and regulomic diversity of driver pathways defines molecular subtypes associated with patient outcome; and (3) the immune microenvironment, inferred from DNA methylation and mRNA profiles, associates with outcome and may inform clinical trials of immune checkpoint inhibitors. Overall, this large-scale analysis reveals previously unappreciated sarcoma-type-specific changes in copy number, methylation, RNA, and protein, providing insights into refining sarcoma therapy and relationships to other cancer types.

Abstract

© 2014 Elsevier Inc. Recent genomic analyses of pathologically defined tumor types identify 'within-a-tissue' disease subtypes. However, the extent to which genomic signatures are shared across tissues is still unclear. We performed an integrative analysis using five genome-wide platforms and one proteomic platform on 3,527 specimens from 12 cancer types, revealing a unified classification into 11 major subtypes. Five subtypes were nearly identical to their tissue-oforigin counterparts, but several distinct cancer types were found to converge into common subtypes. Lung squamous, head and neck, and a subset of bladder cancers coalesced into one subtype typified by TP53 alterations, TP63 amplifications, and high expression of immune and proliferation pathway genes. Of note, bladder cancers split into three pancancer subtypes. The multiplatform classification, while correlated with tissue-of-origin, provides independent information for predicting clinical outcomes. All data sets are available for data-mining from a unified resource to support further biological discoveries and insights into novel therapeutic strategies.

Abstract

Adenocarcinoma of the lung is the leading cause of cancer death worldwide. Here we report molecular profiling of 230 resected lung adenocarcinomas using messenger RNA, microRNA and DNA sequencing integrated with copy number, methylation and proteomic analyses. High rates of somatic mutation were seen (mean 8.9 mutations per megabase). Eighteen genes were statistically significantly mutated, including RIT1 activating mutations and newly described loss-of-function MGA mutations which are mutually exclusive with focal MYC amplification. EGFR mutations were more frequent in female patients, whereas mutations in RBM10 were more common in males. Aberrations in NF1, MET, ERBB2 and RIT1 occurred in 13% of cases and were enriched in samples otherwise lacking an activated oncogene, suggesting a driver role for these events in certain tumours. DNA and mRNA sequence from the same tumour highlighted splicing alterations driven by somatic genomic changes, including exon 14 skipping in MET mRNA in 4% of cases. MAPK and PI(3)K pathway activity, when measured at the protein level, was explained by known mutations in only a fraction of cases, suggesting additional, unexplained mechanisms of pathway activation. 40. DLA Stock Location Policy Based on Percentage of System-Wide Demand: Focus on the Norfolk Geographic Region.

Abstract

The Department Of Defense (DoD) has consolidated the physical distribution functions for wholesale consumable material under the Defense Logistics Agency (DLA). As a consequence, service customers are worried that many repair parts that used to be geographically co-located would be moved to distant DLA supply depots. One DLA proposal is to stock material in a given geographic region if the demand in that region is above a certain percentage of system-wide demand. This study evaluates that proposal by looking at the demand of electronic items over a one-year period, during FY 92, in the Norfolk geographic region. Specifically, it compares transportation costs between maintaining the current DLA stockage policy and stocking all items at Defense Depot Norfolk. The study found that significant savings in second destination transportation costs can be achieved for those items that experienced at least 40% of their annual demand in the Norfolk area.
