Introducción: La conducta suicida se define como todo acto cometido en perjuicio propio de quien lo ejecuta, independientemente del grado de letalidad. Objetivo: Identificar los factores de riesgo asociados a la conducta suicida en adultos pertenecientes al área urbana del municipio Fomento. Métodos: Se realizó un estudio observacional, descriptivo de corte transversal, en el período comprendido entre enero de 2021 y diciembre de 2022. La población estuvo conformada por 40 pacientes, se realizó un muestreo aleatorio simple seleccionando una muestra de 31 adultos expuestos a factores de riesgo que tuvieron intentos suicidas. Se estudiaron las variables, edad, sexo, estado civil, nivel de escolaridad y entre los factores de riesgo: la depresión, los trastornos de la personalidad , el consumo de alcohol y otras drogas, así como situaciones estresantes de la vida cotidiana relacionados con el trabajo , la sociedad y el hogar. Para la recopilación de la información se utilizó la encuesta, la observación y análisis documental. Los resultados se presentaron a través de tablas y se utilizó el cálculo porcentual. Resultados: El sexo femenino, el grado de escolaridad preuniversitario y las edades entre 19 y 29 años fueron los más relacionados con la conducta suicida. En su mayoría estaban casados y se destacó como principal factor de riesgo las situaciones estresantes de la vida cotidiana (83,87%) seguida de la depresión (35,48 %). Conclusiones: Los principales factores de riesgo asociados a la conducta suicida en la población estudiada son las situaciones estresantes de la vida cotidiana y la depresión.

- Abstract
Cell-free hemoglobin (CFH) is used to detect hemolysis and was recently suggested to trigger acute lung injury. However, its role has not been elucidated in severe acute respiratory distress syndrome (ARDS) patients undergoing extracorporeal membrane oxygenation (ECMO). We investigated the association of carboxyhemoglobin (COHb) and haptoglobin - two indirect markers of hemolysis - with mortality in critically ill patients undergoing veno-venous ECMO (VV-ECMO) with adjusted and longitudinal models (primary aim). Secondary aims included assessment of association between COHb and haptoglobin with the development of ventilator-associated pneumonia (VAP) and with hemodynamics. We retrospectively collected physiological, laboratory biomarkers, and outcome data in 147 patients undergoing VV-ECMO for severe ARDS. Forty-seven patients (32%) died in the intensive care unit (ICU). Average levels of COHb and haptoglobin were higher and lower, respectively, in patients who died. Higher haptoglobin was associated with lower pulmonary (PVR) and systemic vascular resistance, whereas higher COHb was associated with higher PVR. Carboxyhemoglobin was an independent predictor of VAP. Both haptoglobin and COHb independently predicted ICU mortality. In summary, indirect signs of hemolysis including COHb and haptoglobin are associated with modulation of vascular tone, VAP, and ICU mortality in respiratory ECMO. These findings suggest that CFH may be a mechanism of injury in this patient population.

- Abstract
Background: Veno-venous extracorporeal membrane oxygenation (V-V ECMO) is a rapidly expanding life-support technique worldwide. The most common indications are severe hypoxemia and/or hypercapnia, unresponsive to conventional treatments, primarily in cases of acute respiratory distress syndrome. Concerning potential contraindications, there is no mention of microbiological history, especially related to multi-drug resistant (MDR) bacteria isolated before V-V ECMO placement. Our study aims to investigate: (i) the prevalence and incidence of MDR Gram-negative (GN) bacteria in a cohort of V-V ECMOs; (ii) the risk of 1-year mortality, especially in the case of predetected MDR GN bacteria; and (iii) the impact of annual hospital V-V ECMO volume on the probability of acquiring MDR GN bacteria. Methods: All consecutive adults admitted to the Intensive Care Units of 5 Italian university-affiliated hospitals and requiring V-V ECMO were screened. Exclusion criteria were age < 18 years, pregnancy, veno-arterial or mixed ECMO-configuration, incomplete records, survival < 24 h after V-V ECMO. A standard protocol of microbiological surveillance was applied and MDR profiles were identified using in vitro susceptibility tests. Cox-proportional hazards models were applied for investigating mortality. Results: Two hundred and seventy-nine V-V ECMO patients (72% male) were enrolled. The overall MDR GN bacteria percentage was 50%: 21% (n.59) detected before and 29% (n.80) after V-V ECMO placement. The overall 1-year mortality was 42%, with a higher risk observed in predetected patients (aHR 2.14 [1.33–3.47], p value 0.002), while not in ‘V-V ECMO-acquired MDR GN bacteria’ group (aHR 1.51 [0.94–2.42], p value 0.090), as compared to ‘non-MDR GN bacteria’ group (reference). Same findings were found considering only infections. A larger annual hospital V-V ECMO volume was associated with a lower probability of acquiring MDR GN bacteria during V-V ECMO course (aOR 0.91

- Abstract
In the initial phases of veno-venous extracorporeal membrane oxygenation (VV ECMO) support for severe acute respiratory distress syndrome (ARDS), ultraprotective controlled mechanical ventilation (CMV) is typically employed to limit the progression of lung injury. As patients recover, transitioning to assisted mechanical ventilation can be considered to reduce the need for prolonged sedation and paralysis. This study aimed to evaluate the feasibility of transitioning to pressure support ventilation (PSV) during VV ECMO and to explore variations in respiratory mechanics and oxygenation parameters following the transition to PSV. This retrospective monocentric study included 191 adult ARDS patients treated with VV ECMO between 2009 and 2022. Within this population, 131 (69%) patients were successfully switched to PSV during ECMO. Pressure support ventilation was associated with an increase in respiratory system compliance (p = 0.02) and a reduction in pulmonary shunt fraction (p < 0.001). Additionally, improvements in the cardiovascular Sequential Organ Failure Assessment score and a reduction in pulmonary arterial pressures (p < 0.05) were recorded. Ninety-four percent of patients who successfully transitioned to PSV were weaned from ECMO, and 118 (90%) were discharged alive from the intensive care unit (ICU). Of those who did not reach PSV, 74% died on ECMO, whereas the remaining patients were successfully weaned from extracorporeal support. In conclusion, PSV is feasible during VV ECMO and potentially correlates with improvements in respiratory function and hemodynamics.

7. A Nature-Based Approach Using Felled Burnt Logs to Enhance Forest Recovery Post-Fire and Reduce Erosion Phenomena in the Mediterranean Area

The easy implementation of a nature-based solution (NBS) to address a prompt stabilisation of burnt areas may be crucial in the Mediterranean forest environment. A plot scale hydrological and ecological survey was conducted in a pine forest of the Aspromonte Massif (southern Italy) affected by a high fire severity in summer 2021. The hydrological response to 30 rainfall event groups is analysed by monitoring nine sloping plots distributed into three plot blocks (forested according to the pre-fire situation, burnt with randomly directed felled logs and burnt with manually felled logs redirected along contour lines). The hydrological response of bare soil is mitigated by about 30% through the combined effects of the NBS and the vegetation cover by pioneer species. Although the regeneration of Pinus radiata is scarcer in the arranged plots, the spontaneous vegetation, once gone, prepares better edaphic conditions for the triggering of forest dynamics assured by the auto-succession of Pinus radiata. In terms of woody mechanical resistance, NBS durability is compatible with the time required for seedlings to regenerate the forest ecosystem. The results achieved so far encourage further research on higher slopes and complementary aspects (vegetal and animal biodiversity, economic factors, etc.).

Abstract We report a subtype of immune‐mediated encephalitis associated with COVID‐19, which closely mimics acute‐onset sporadic Creutzfeldt–Jakob disease. A 64‐year‐old man presented with confusion, aphasia, myoclonus, and a silent interstitial pneumonia. He tested positive for SARS‐CoV‐2. Cognition and myoclonus rapidly deteriorated, EEG evolved to generalized periodic discharges and brain MRI showed multiple cortical DWI hyperintensities. CSF analysis was normal, except for a positive 14‐3‐3 protein. RT‐QuIC analysis was negative. High levels of pro‐inflammatory cytokines were present in the CSF and serum. Treatment with steroids and intravenous immunoglobulins produced EEG and clinical improvement, with a good neurological outcome at a 6‐month follow‐up.

No-tillage and soil mulching with pruning residues, applied in olive groves of the semi-arid Mediterranean environment, as erosion control practices still practice not fully studied. This study has evaluated the saturated hydraulic conductivity (Ksat), surface runoff (SR) and soil erosion (SL) under rainfall at plot scale throughout two years in four different management practices, total soil cover with a net (SP), mechanical tillage (MT) and mulching by vegetal residues at 3,5·103 and 17,5·103 kg ha−1 of dry matter (NTR350 and NTR1750), in an olive grove of Southern Italy. Ksat varied between 1.6 (MT) and 25.1 (NTR1750) mm/h. A clear reduction in runoff and soil losses was detected for the mulch-based practices when compared to MT, from 20 to 32% in the runoff coefficient and 75–80% in SL, with higher reductions in the NTR1750. This reduction in SL can be mainly explained by the reduction in SR and rain-splash, interrill and rill erosion, due to protection by mulch residues, which increased the vegetal cover and organic matter content of mulched plots. The vegetal cover was on average higher in SP (33%), NTR1750 (25%) and NTR350 (22%), and lower in MT (12%). The mean organic matter content of soil was 2.01%, 1.69%,1.34% and 0.82% for NTR1750, NTR350, SP and MT respectively.Overall, the results quantify the impact of soil mulching with pruning residues at different doses, which will provide guidelines to control and mitigate the hydrological response of clayey and steep soils in Mediterranean olive groves, analysing the associated environmental and economic benefits.

Introducción: la enfermedad periodontal y las enfermedades cardiovasculares son afecciones inflamatorias crónicas que se asocian epidemiológicamente; ambas tienen una etiología polimicrobiana.Objetivo: describir la posible asociación entre la enfermedad periodontal inflamatoria crónica y las enfermedades cardiovasculares.Métodos: se realizó una revisión actualizada de la literatura disponible. Los artículos se identificaron a través de la búsqueda automatizada en las bases de datos: PubMed, Scielo y Google Académico, en el período de enero a octubre de 2018. Fueron seleccionados 40 textos científicos, escritos en idioma español e inglés, publicados entre el año 2012 y el 2018, de los cuales: 37 eran artículos científicos de revistas y 3 libros.Conclusiones: numerosos estudios epidemiológicos y revisiones apoyan la asociación entre las enfermedades periodontales y las enfermedades cardiovasculares, independientemente de los factores de confusión.

- Abstract
Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

Introduction: lesions resulting from the interaction between pulp and periodontal disease are known as endoperiodontal lesions. Diagnosis and treatment are complex and prognosis is guarded and depends on timely and accurate diagnosis. Patient information: 12-year-old patient with throbbing pain, increased volume, fistula, history of pulpo-radicular treatment, and incisal trauma four years ago to the upper left central incisor. The periapical radiograph reported a lesion involving the apical middle thirds of the left upper incisors. Definitive diagnosis: combined endoperiodontal process, a primary endodontic lesion with secondary periodontal involvement in the upper left central incisor and primary periodontal lesion with secondary endodontic involvement in the left lateral incisor. Conclusions: laser therapy was applied after the endodontic and periodontal treatment carried out. A satisfactory clinical-radiographic result was achieved. [ABSTRACT FROM AUTHOR]

The analysis of shoreline changes is very important for coastal planning and management. In territories such as Calabria (Italy), characterized by significant anthropogenic pressures and various eroded coasts, the knowledge of the shoreline changes, and the factors that influence them, is necessary for management and planning of coastal areas. In fact, shoreline position is one of the most important indicators of coastal dynamics. From this point of view recent advances in remote sensing and GIS techniques allow to estimate with great precision the shoreline changes over the years. The paper analyzes the shoreline changes near the mouth of the Sant’Agata River (Reggio Calabria, Italy), carried out through the comparison of various cartography data. Furthermore, the paper analyzes the main factors influencing the coastal dynamics in order to identify possible correlation between these factors and the shoreline changes. The analysis of these factors shows that, in this case study, the rainfall regime has a considerable influence on shoreline change. The methodology described in this paper is particularly useful for better understanding the factors that most influence the coastal balance and, therefore, is applicable to many contexts which are similar to the Sant’Agata river mouth.

Abstract Background In ICU patients, digestive tract colonization by multidrug-resistant (MDR) Gram-negative (G−) bacteria is a significant risk factor for the development of infections. In patients undergoing extracorporeal membrane oxygenation (ECMO), colonization by MDR bacteria and risk of subsequent nosocomial infections (NIs) have not been studied yet. The aim of this study is to evaluate the incidence, etiology, risk factors, impact on outcome of gastrointestinal colonization by MDR G− bacteria, and risk of subsequent infections in patients undergoing ECMO. Methods This is a retrospective analysis of prospectively collected data: 105 consecutive patients, treated with ECMO, were admitted to the ICU of an Italian tertiary referral center (San Gerardo Hospital, Monza, Italy) from January 2010 to November 2015. Rectal swabs for MDR G− bacteria were cultured at admission and twice a week. Only colonization and NIs by MDR G− bacteria were analyzed. Results Ninety-one included patients [48.5 (37–56) years old, 63% male, simplified acute physiology score II 37 (32–47)] underwent peripheral ECMO (87% veno-venous) for medical indications (79% ARDS). Nineteen (21%) patients were colonized by MDR G− bacteria. Male gender (OR 4.03, p = 0.029) and duration of mechanical ventilation (MV) before ECMO > 3 days (OR 3.57, p = 0.014) were associated with increased risk of colonization. Introduction: Natural and Traditional Medicine allows professionals to broaden the scientific scope in an integrative way, showing their performance in the health areas of Pinar del Río. Objective: to identify the elements that limits the practice of therapeutic modalities of Natural and Traditional Medicine at primary health care in Pinar del Río municipality. Methods: a descriptive observational study was conducted between May 2016 and July 2017 in the four polyclinics of the municipality of Pinar del Río. The study target group included the doctors and professors of the Basic Work Team (U=1 006). The collection of sample was conducted through cases by authority criteria, and within these by simple random cases, sample (n=234). Results: Comprehensive Medicine Specialists with more than five years of experience predominated, 30.3 % do not applied Natural and Traditional Medicine, 67.9 % considered it useful, 58.8% recognized its effectiveness, phytotherapy prevailed (69.6 %). Conclusion: the application of Natural and Traditional Medicine still demands a priority by the professionals in the primary health care as an alternative to its application, a not minor part expresses not to make use of it, everything indicates that it is an element not to be left to the spontaneity taking care of the scientific and ethical aspects of its necessary prescription as therapeutic modalities.

In the second half of the twentieth century, after the end of the Second World War, a considerable anthropogenic pressure was observed in most of the Mediterranean territories. This process has caused the expansion of existing settlements and the construction of numerous new towns, often located very close to rivers. A frequent consequence of this process is the transformation of several rivers through planform changes, narrowing, channelization and culverting to recover spaces where inhabited centers expanded, and the construction of structures interacting with rivers. This issue is very important in territories such as the Metropolitan City of Reggio Calabria, in southern Italy, which is an interesting case study due to the considerable anthropogenic pressures observed in the last 70 years. The main goal of this paper is to evaluate the effects of anthropogenic pressure in the last 70 years on some rivers of the Metropolitan City of Reggio Calabria in terms of the following issues: planform changes, channelization, culverting, and the presence of structures and infrastructures interacting with rivers. The specific goals of this paper are the quantification of the effects of anthropogenic pressure on the rivers of the study area analyzing sixteen parameters, the identification of possible conditions of hydraulic hazard through the analysis of past events, and the proposal of structural and non-structural mitigation interventions. In many rivers of the study area, the significant effects of anthropogenic pressure are visible through rivers that pass above highways, barred rivers, rivers replaced by roads and numerous crossing roads with a missing levee.

Introduction: Ankle fractures are usually frequent in emergency departments worldwide, with an incidence of 187/100,000 inhabitants per year. Especially the type B fracture according to Webers classification, which may lead to long-term osteoarthritis in approximately 14%. It is essential to recognize that stability in the ankle joint is the fundamental pillar in the correct treatment strategies in ankle trauma. Objective: to describe current information related to ankle bone fractures, etiology, anatomy, epidemiology, mechanism of action, presentation, classification, evaluation, prognosis, treatment and complications of ankle fractures. Methodology: a total of 38 articles were analyzed in this review, including review and original articles, as well as clinical cases, of which 26 bibliographies were used because the other articles were not relevant to this study. The sources of information were PubMed, Google Scholar and Cochrane; the terms used to search for information in Spanish, Portuguese and English were: ankle fracture, fractura do tornozelo, ankle, tibia, fibula, ankle fracture. Results: Bimalleolar ankle fractures occur in a quarter of the patients and trimalleolar fractures in the remaining 5% to 10%. The incidence of ankle fractures is close to 187 per 100,000 inhabitants per year. Open fractures are infrequent, representing only 2 % of all fractures of the ankle joint. In children, these injuries are also frequent, occupying the second place after hand and wrist injuries, especially in those between 10 and 15 years of age. Likewise, pediatric ankle fractures occur in a 2:1 male to female ratio, representing 5% of all fractures in children and approximately 9% to 18% of all fissure injuries. Conclusions: the ankle joint is complex, in gynglimus, formed by the fibula, the tibia and the talus and also deeply related to the ligamentous complexes. The bony anatomy that provides stability is formed by the distal part of the tibia and fibula, its articulation with the talus and with each other. Generally ankle fractures are caused by different trauma mechanisms such as impact, twisting and crushing injuries. Ankle injury depends on several factors such as mechanism, chronicity, bone quality, patients age, magnitude, direction, impact velocity and foot position. A complete and comprehensive medical history is essential in the medical evaluation. X-rays are the first-line adjunctive tests that aid in the evaluation of an injury that impacts the ankle. The classification system is important for the treatment decision. The treatment of fractures of the ankle bones can be performed conservatively or surgically, depending on certain criteria, and immobilization should be performed afterwards to reduce the risk of complications. It is essential to follow the ATLS scheme in order to define and manage any alteration that may be life-threatening for the patient. Ankle fracture-dislocation requires urgent manipulation to recover the ankle mortise. KEY WORDS: fracture, ankle, tibia, fibula, bones.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation.METHODS: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic-from March 1 to Sept 13, 2020-at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing.FINDINGS: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46-60]) were included in the study. Median ECMO duration was 15 days (IQR 8-27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients did not die in hospital, and 547 (95%) of 577 patients for whom data were available were alive at 6 months. 102 (24%) of 431 patients had returned to full-time work at 6 months, and 57 (13%) of 428 patients had returned to part-time work. At 6 months, respiratory rehabilitation was required in 88 (17%) of 522 patients with available data, and the most common residual symptoms included dyspnoea (185 [35%] of 523 patients) and cardiac (52 [10%] of 514 patients) or neurocognitive (66 [13%] of 512 patients) symptoms.INTERPRETATION: Patient's age, timing of cannulation (FUNDING: None.

This work deals with the development of graphene oxide (GO)-based self-assembling membranes as possible innovative proton conductors to be used in polymer electrolyte membrane fuel cells (PEMFCs). Nowadays, the most adopted electrolyte is Chemours’ Nafion; however, it reveals significant deficiencies such as strong dehydration at high temperature and low humidity, which drastically reduces its proton conductivity. The presence of oxygenated moieties in the GO framework makes it suitable for functionalization, which is required to enhance the promising, but insufficient, proton-carrying features of GO. In this study, sulfonic acid groups (–SO3H) that should favor proton transport were introduced in the membrane structure via a reaction between GO and concentrated sulfuric acid. Six acid-to-GO molar ratios were adopted in the synthesis procedure, giving rise to final products with different sulfonation degrees. All the prepared samples were characterized by means of TGA, ATR-FTIR and Raman spectroscopy, temperature-dependent XRD, SEM and EDX, which pointed out morphological and microstructural changes resulting from the functionalization stage, confirming its effectiveness. Regarding functional features, electrochemical impedance spectroscopy (EIS) as well as measurements of ion exchange capacity (IEC) were carried out to describe the behavior of the various samples, with pristine GO and commercial Nafion® 212 used as reference. EIS tests were performed at five different temperatures (20, 40, 60, 80 and 100 °C) under high (95%) and medium (42%) relative humidity conditions. Compared to both GO and Nafion® 212, the sulfonated specimens demonstrate an increase in the number of ion-carrying groups, as proved by both IEC and EIS tests, which reveal the enhanced proton conductivity of these novel membranes. Specifically, an acid-to-GO molar ratio of 10 produces a six-fold improvement of IEC (4.23 meq g−1) with respect to pure GO (0.76 meq g−1), while a maximum eight-fold improvement (5.72 meq g−1) is achieved in SGO-15.

CYP105AS1 is a cytochrome P450 from Amycolatopsis orientalis that catalyzes monooxygenation of compactin to 6-epi-pravastatin. For fermentative production of the cholesterol-lowering drug pravastatin, the stereoselectivity of the enzyme needs to be inverted, which has been partially achieved by error-prone PCR mutagenesis and screening. In the current study, we report further optimization of the stereoselectivity by a computationally aided approach. Using the CoupledMoves protocol of Rosetta, a virtual library of mutants was designed to bind compactin in a pro-pravastatin orientation. By examining the frequency of occurrence of beneficial substitutions and rational inspection of their interactions, a small set of eight mutants was predicted to show the desired selectivity and these variants were tested experimentally. The best CYP105AS1 variant gave >99% stereoselective hydroxylation of compactin to pravastatin, with complete elimination of the unwanted 6-epi-pravastatin diastereomer. The enzyme-substrate complexes were also examined by ultrashort molecular dynamics simulations of 50 × 100 ps and 5 × 22 ns, which revealed that the frequency of occurrence of near-attack conformations agreed with the experimentally observed stereoselectivity. These results show that a combination of computational methods and rational inspection could improve CYP105AS1 stereoselectivity beyond what was obtained by directed evolution. Moreover, the work lays out a general in silico framework for specificity engineering of enzymes of known structure.

29. Modelling the Event-Based Hydrological Response of Mediterranean Forests to Prescribed Fire and Soil Mulching with Fern Using the Curve Number, Horton and USLE-Family (Universal Soil Loss Equation) Models

The SCS-CN, Horton, and USLE-family models are widely used to predict and control runoff and erosion in forest ecosystems. However, in the literature there is no evidence of their use in Mediterranean forests subjected to prescribed fire and soil mulching. To fill this gap, this study evaluates the prediction capability for runoff and soil loss of the SCS-CN, Horton, MUSLE, and USLE-M models in three forests (pine, chestnut, and oak) in Southern Italy. The investigation was carried out at plot and event scales throughout one year, after a prescribed fire and post-fire soil mulching with fern. The SCS-CN and USLE-M models were accurate in predicting runoff volume and soil loss, respectively. In contrast, poor predictions of the modelled hydrological variables were provided by the models in unburned plots, and by the Horton and MUSLE models for all soil conditions. This inaccuracy may have been due to the fact that the runoff and erosion generation mechanisms were saturation-excess and rainsplash, while the Horton and MUSLE models better simulate infiltration-excess and overland flow processes, respectively. For the SCS-CN and USLE-M models, calibration was needed to obtain accurate predictions of surface runoff and soil loss; furthermore, different CNs and C factors must be input throughout the year to simulate the variability of the hydrological response of soil after fire. After calibration, two sets of CNs and C-factor values were suggested for applications of the SCS-CN and USLE-M models, after prescribed fire and fern mulching in Mediterranean forests. Once validated in a wider range of environmental contexts, these models may support land managers in controlling the hydrology of Mediterranean forests that are prone to wildfire risks.

- Abstract
Background: During the first coronavirus disease 2019 (COVID-19) pandemic wave, an unprecedented number of patients with respiratory failure due to a new, highly contagious virus needed hospitalization and intensive care unit (ICU) admission. The aim of the present study was to describe the communication and visiting policies of Italian intensive care units (ICUs) during the first COVID-19 pandemic wave and national lockdown and compare these data with prepandemic conditions. Methods: A national web-based survey was conducted among 290 Italian hospitals. Each ICU (active between February 24 and May 31, 2020) was encouraged to complete an individual questionnaire inquiring the hospital/ICU structure/organization, communication/visiting habits and the role of clinical psychology prior to, and during the first COVID-19 pandemic wave. Results: Two hundred and nine ICUs from 154 hospitals (53% of the contacted hospitals) completed the survey (202 adult and 7 pediatric ICUs). Among adult ICUs, 60% were dedicated to COVID-19 patients, 21% were dedicated to patients without COVID-19 and 19% were dedicated to both categories (Mixed). A total of 11,102 adult patients were admitted to the participating ICUs during the study period and only approximately 6% of patients received at least one visit. Communication with family members was guaranteed daily through an increased use of electronic devices and was preferentially addressed to the same family member. Compared to the prepandemic period, clinical psychologists supported physicians more often regarding communication with family members. Fewer patients received at least one visit from family members in COVID and mixed-ICUs than in non-COVID ICUs, l (0 [0-6]%, 0 [0-4]% and 11 [2-25]%, respectively, p < 0.001). 33. Computational design and screening of enzyme enantioselectivity

- Abstract
The aim of this thesis is to explore computational methodologies for the design and screening of enzyme variants for use in green chemistry. Methods such as docking calculations, Rosetta enzyme redesign, molecular dynamics simulations and quantum mechanical calculations are used to predict the catalytic activity and enantioselectivity of enzyme variants. Pipelines incorporating such methods will reduce the amount of costly and time-consuming laboratory work that is needed for biocatalyst development by protein engineering. Target enzymes are cytochrome P450s and epoxide hydrolases, both of which are important for industrial synthesis of fine chemicals and pharmaceuticals. In general the activity and selectivity of enzymes depends on multiple steps of the catalytic cycle, e.g. formation and dynamics of enzyme-substrate complexes, energy of transition states, and release of products. This complexity may hinder the use of computational methods for design and prediction of better enzymes, but simplifications are possible. For example, enantioselectivity is mainly determined by the conformation of substrate bound in the active site, which in this thesis was controlled using Rosetta enzyme design software. Because Rosetta often gives a number of solutions that is too large for experimental evaluation, ranking of the predicted designs is important. The thesis shows that molecular dynamics simulations can be used for that purpose. By scoring the frequency of reactive poses, molecular dynamics reveals enzyme variants in which substrate binds in a position that supports the desired activity or selectivity. In cases where major changes in the geometry of the bound substrate occur during the reaction, quantum mechanical methods (hybrid DFT) were used to further examine the selectivity. Based on the results, the thesis recommends a pragmatic balance between accuracy and cost of computational methods when developing in silico pipelines for the design of tailored enantios

- Abstract
The SCS-CN, Horton and USLE-family models are used worldwide, but few examples are available for olive groves on steep and clayey soils under mulching of pruning residues. We evaluate the accuracy in predicting runoff and soil loss of a steep (20%) and clayey olive grove subject to three soil conservation practices: mechanical tillage (MT), mulching with pruning residues (NTR) and standard protection (SP), measured at plot scale in Southern Italy during 30 months under natural rainfall. The models were calibrated by adjusting the Curve Numbers (for the SCS-CN model) and the C-factors (MUSLE and USLE-M); the Horton model was not calibrated. The model performance was assessed by qualitative and quantitative procedures. In all practices, the SCS-CN was more accurate for runoff predictions (mean difference of 7% compared with corresponding observations) compared with the Horton (mean difference of 30%). For soil erosion, the MUSLE showed better performance in soils subject to MT or total protection (differences lower than 10%), while the USLE-M was more reliable to simulate soil loss in mulched plots (differences of 8%). A set of Curve Numbers (95 for MT, 70 for SP and 85 for NTR) and C-factors (0.4 for MT, 0.2 for SP and 0.1 for NTR) are proposed for applications in steep slope and clayey soil olive orchards using the SCS-CN and USLE proposed models, respectively. Validation exercises in other environmental experimental conditions would enhance the export these models for runoff and erosion control in agricultural soils treated with mulching.

The easy implementation of ecologically-sound remediation measures for the prompt stabilisation of burned areas may be crucial in Mediterranean forest environments. Manual in situ contour redirection of burned felled logs could aid in soil erosion control and facilitate forest self-regeneration. In this study, a plot-scale runoff/sediment yield survey was conducted in Calabria, Italy, within a Mediterranean pine forest that was affected by an extreme wildfire spanning over 15,000 hectares in the summer of 2021. The hydrological response to 24 rainfall events was analysed after one year of monitoring using nine Wischmeier and Smith 20% sloping plots, which were distributed into three plot-blocks representing different conditions (forested, burned with randomly directed fallen logs, and burned with fallen logs manually redirected along contour lines). The post-fire condition (with felled logs in random positions) exhibited a consistent overall increase (approximately four times) in runoff and sediment yield compared to the pre-fire situation. This degradation effect was mitigated by approximately 30% through the manual redirection of burned logs, which promoted early (three to five weeks) vegetation regeneration (including tree emergence) and enhanced coverage as vegetation spread from the log positions. The results obtained so far provide encouraging insights and warrant further research on steeper slopes and complementary aspects (regulatory, biological, mechanical, economic, etc.).

42. Water Infiltration after Prescribed Fire and Soil Mulching with Fern in Mediterranean Forests

Prescribed fire is commonly used to reduce the wildfire risk in Mediterranean forests, but the soil’s hydrological response after fire is contrasting in literature experiences. The mulch treatment can limit the increases in runoff and erosion in the short term after a fire. The use of fern is preferable to straw, due its large availability in forests. However, no experiences of post-fire treatment with fern mulch have been found in the literature and therefore the mulching effectiveness has not been evaluated. This study has measured water infiltration rate (IR) and water repellency (SWR) using a rainfall simulator in three Mediterranean forest stands (pine, oak and chestnut) of Calabria (Southern Italy) after a prescribed fire and mulching treatment with fern in comparison to unburned soil. Prescribed fire reduced water infiltration in all forests in the short term compared to the unburned conditions, and increased SWR in pine and oak forests. These reductions in IR in the time window of disturbance after fire increased the runoff generation capacity in all soils, but had a lower effect on peak flows. However, soil mulching with fern limited the runoff rates and peak flows compared to the burned soils, but this treatment was less effective in pine forest. One year after fire, IR increased in burned soils (treated or not) over time, and SWR disappeared. The effects of mulching have disappeared after some months from fire. The study confirms the usefulness of mulching in broadleaves forest in the short term, in order to control the hydrological effects of prescribed fire in Mediterranean forests. Both post-fire management techniques should be instead adopted with caution in conifer forests.

Purpose In this paper a quick, easy and accessible methodology to estimate the sediment volume trapped behind a fully filled check dam system is proposed. As it is well known, check dams play an important role in the sediment balance between watershed and coastline. However, on a large scale, especially in those contexts where a great number of structures was installed, detailed surveys and measurements of sediment storage capacity would be extremely time-consuming and costly in terms of both economic efforts and human resources. Methods To this aim, the proposed method considers only four easy-to-obtain morphometric parameters to combine with the number of check dams. The method was calibrated on a sample of 912 check dams located in seven long-term studied watersheds and, therefore, validated in a sample of three regulated Spanish catchments with an independent dataset. Results At watershed level, the comparison between the calculated and estimated values showed a good capability of the method in evaluating the sediment volume trapped by the 912 studied check dams (RMSE ≈ 16,900 m3; R2 > 0.9). The validation revealed encouraging results with estimation errors below 25%. Conclusion The use of this accessible and easily usable method could represent a supporting tool for planning, monitoring and assessment of the environmental effects of control works. Moreover, these results are useful to carry out actions aimed to mitigate natural hazard and environmental as well as socio-economic problems of the watershed-coast system (e.g. shoreline retreat and morphological instability of the urban and tourist areas).

In order to predict and control the impacts of floods in torrents, it is important to verify the simulation accuracy of the most used hydrological models. The performance verification is particularly needed for applications in watersheds with peculiar climatic and geomorphological characteristics, such as the Mediterranean torrents. Moreover, in addition to the accuracy, other factors affect the choice of software by stakeholders (users, modellers, researchers, etc.). This study introduces a “performance matrix”, consisting of several evaluation parameters weighted by stakeholders’ opinions. The aim is to evaluate the accuracy of the flood prediction which is achieved by different models, as well as the pros and cons of software user experience. To this aim, the performances and requisites of four physical-based and conceptual models (HEC-HMS, SWMM, MIKE11 NAM and WEC-FLOOD) have been evaluated, by predicting floods in a midsized Mediterranean watershed (Mèsima torrent, Calabria, Southern Italy). In the case study, HEC-HMS and MIKE 11 NAM were the best computer models (with a weighted score of 4.45 and 4.43, respectively), thanks to their low complexity and computation effort, as well as good user interface and prediction accuracy. However, MIKE11 NAM is not free of charge. SWMM showed a lower prediction accuracy, which put the model in third place of the four models. The performance of WEC-FLOOD, although not being as good as for the other tested models, can be considered overall acceptable in comparison to the other well-consolidated models, considering that WEC-FLOOD is in the early stage of development. Overall, the proposal of the performance matrix for hydrological models may represent a first step in building a more complete evaluation framework of the hydrological and hydraulic commercial models, in order to give indications to allow potential users to make an optimal choice.

Background During the first coronavirus disease 2019 (COVID-19) pandemic wave, an unprecedented number of patients with respiratory failure due to a new, highly contagious virus needed hospitalization and intensive care unit (ICU) admission. The aim of the present study was to describe the communication and visiting policies of Italian intensive care units (ICUs) during the first COVID-19 pandemic wave and national lockdown and compare these data with prepandemic conditions. Methods A national web-based survey was conducted among 290 Italian hospitals. Each ICU (active between February 24 and May 31, 2020) was encouraged to complete an individual questionnaire inquiring the hospital/ICU structure/organization, communication/visiting habits and the role of clinical psychology prior to, and during the first COVID-19 pandemic wave. Results Two hundred and nine ICUs from 154 hospitals (53% of the contacted hospitals) completed the survey (202 adult and 7 pediatric ICUs). Among adult ICUs, 60% were dedicated to COVID-19 patients, 21% were dedicated to patients without COVID-19 and 19% were dedicated to both categories (Mixed). A total of 11,102 adult patients were admitted to the participating ICUs during the study period and only approximately 6% of patients received at least one visit. Communication with family members was guaranteed daily through an increased use of electronic devices and was preferentially addressed to the same family member. Compared to the prepandemic period, clinical psychologists supported physicians more often regarding communication with family members. Remote communication was widely adopted as a surrogate for family meetings. New strategies to favor a family-centered approach during the current and future pandemics are warranted.
