Abstract

Automated vehicles are pictured as the future of transportation, and facilitating safer driving is only one of the many benefits. However, due to the constantly changing role of the human driver, users are easily confused and have little knowledge about their responsibilities. Being the bridge between automation and human, the human-machine interface (HMI) is of great importance to driving safety. This study was conducted in a static driving simulator. Three HMI designs were developed, among which significant differences in mental workload using NASA-TLX and the subjective transparency test were found. An electroencephalogram was applied throughout the study to determine if differences in the mental workload could also be found using EEG's spectral power analysis. Results suggested that more studies are required to determine the effectiveness of the spectral power of EEG on mental workload, but the three interface designs developed in this study could serve as a solid basis for future research to evaluate the effectiveness of psychophysiological measures. Abstract

Photoneutron reactions on $^{208}$Pb in the Giant Dipole Resonance energy region have been investigated at the $\gamma$-ray beam line of the NewSUBARU facility in Japan. The measurements made use of quasi-monochromatic laser Compton backscattering $\gamma$-ray beams in a broad energy range, from the neutron threshold up to 38 MeV, and of a flat-efficiency moderated $^3$He neutron detection system along with associated neutron-multiplicity sorting methods. We report absolute cross sections and mean photoneutron energies for the $^{208}$Pb$(\gamma,\,inX)$ reactions with $i$~=~1 to 4. The fine structure present in the $^{208}$Pb$(\gamma,\,n)$ cross sections at incident energies lower than 13~MeV has been observed. The photoabsorption cross section has been obtained as the sum of the $(\gamma,\,inX)$ reaction cross sections. By reproducing the measured ring-ratio values at excitation energies below the two neutron separation energy, we were able to extract estimations on the $^{208}$Pb$(\gamma,\,n)$ photoneutron energy spectra and on the partial photoneutron cross sections for leaving the residual $^{207}$Pb in its ground and first two excited states. The present results are compared with data from the literature and statistical model calculations.

Abstract

Connected automated vehicles (CAV), which incorporate vehicle-to-vehicle (V2V) communication into their motion planning, are expected to provide a wide range of benefits for individual and overall traffic flow. A frequent constraint or required precondition is that compatible CAVs must already be available in traffic at high penetration rates. Achieving such penetration rates incrementally before providing ample benefits for users presents a chicken-and-egg problem that is common in connected driving development. Based on the example of a cooperative driving function for bottleneck traffic flows (e.g. at a roadblock), we illustrate how such an evolutionary, incremental introduction can be achieved under transparent assumptions and objectives. To this end, we analyze the challenge from the perspectives of automation technology, traffic flow, human factors and market, and present a principle that 1) accounts for individual requirements from each domain; 2) provides benefits for any penetration rate of compatible CAVs between 0 % and 100 % as well as upward-compatibility for expected future developments in traffic; 3) can strictly limit the negative effects of cooperation for any participant and 4) can be implemented with close-to-market technology. Abstract

Bioimaging refers to a collection of methods to visualize the internal structures and mechanisms of living organisms. The fundamental tool, the microscope, has enabled seminal discoveries like that of the cell as the smallest unit of life, and continues to expand our understanding of biological processes. Today, we can follow the interaction of single molecules within nanoseconds in a living cell, and the development of complete small organisms like fish and flies over several days starting from the fertilized egg. Each image pixel encodes multiple spatiotemporal and spectral dimensions, compounding the massive volume and complexity of bioimage data. Proper handling of this data is indispensable for analysis and its lack has become a growing hindrance for the many disciplines of the life and biomedical sciences relying on bioimaging. No single domain has the expertise to tackle this bottleneck alone. As a method-specific consortium, NFDI4BIOMAGE seeks to address these issues, enabling bioimaging data to be shared and re-used like they are acquired, i.e., independently of disciplinary boundaries. We will provide solutions for exploiting the full information content of bioimage data and enable new discoveries through sharing and re-analysis. Our RDM strategy is based on a robust needs analysis that derives not only from a community survey but also from over a decade of experience in German BioImaging, the German Society for Microscopy and Image Analysis. It considers the entire lifecycle of bioimaging data, from acquisition to archiving, including analysis and enabling re-use. A foundational element of this strategy is the definition of a common, cloud-compatible, and interoperable digital object that bundles binary images with their descriptive and provenance metadata. Abstract

Personalized treatment strategies based on non‑invasive biomarkers have potential to improve patient management in patients with newly diagnosed glioblastoma (GBM). The residual tumour burden after surgery in GBM patients is a prognostic imaging biomarker. However, in clinical patient management, its assessment is a manual and time‑consuming process that is at risk of inter‑rater variability. Furthermore, the prediction of patient outcome prior to radiotherapy may identify patient subgroups that could benefit from escalated radiotherapy doses. Therefore, in this study, we investigate the capabilities of traditional radiomics and 3D convolutional neural networks for automatic detection of the residual tumour status and to prognosticate time‑to‑recurrence (TTR) and overall survival (OS) in GBM using postoperative [11C] methionine positron emission tomography (MET‑PET) and gadolinium‑enhanced T1‑w magnetic resonance imaging (MRI). On the independent test data, the 3D‑DenseNet model based on MET‑PET achieved the best performance for residual tumour detection, while the logistic regression model with conventional radiomics features performed best for T1c‑w MRI (AUC: MET‑PET 0.95, T1c‑w MRI 0.78). For the prognosis of TTR and OS, the 3D‑DenseNet model based on MET‑PET integrated with age and MGMT status achieved the best performance (Concordance‑Index: TTR 0.68, OS 0.65). In conclusion, we showed that both deep‑ learning and conventional radiomics have potential value for supporting image‑based assessment and prognosis in GBM. After prospective validation, these models may be considered for treatment personalization.

Abstract

(1) Background: The sensitivity of head and neck squamous cell carcinoma (HNSCC) to ionizing radiation, among others, is determined by the number of cells with high clonogenic potential and stem-like features. These cellular characteristics are dynamically regulated in response to treatment and may lead to an enrichment of radioresistant cells with a cancer stem cell (CSC) phenotype. Epigenetic mechanisms, particularly DNA and histone methylation, are key regulators of gene-specific transcription and cellular plasticity. Therefore, we hypothesized that specific epigenetic targeting may prevent irradiation-induced plasticity and may sensitize HNSCC cells to radiotherapy. (2) Methods: We compared the DNA methylome and intracellular concentrations of tricarboxylic acid cycle metabolites in radioresistant FaDu and Cal33 cell lines with their parental controls, as well as aldehyde dehydrogenase (ALDH)-positive CSCs with negative controls. Moreover, we conducted a screen of a chemical library targeting enzymes involved in epigenetic regulation in combination with irradiation and analyzed the clonogenic potential, sphere formation, and DNA repair capacity to identify compounds with both radiosensitizing and CSC-targeting potential. (3) Results: We identified the histone demethylase inhibitor GSK-J1, which targets UTX (KDM6A) and JMJD3 (KDM6B), leading to increased H3K27 trimethylation, heterochromatin formation, and gene silencing. The clonogenic survival assay after siRNA-mediated knock-down of both genes radiosensitized Cal33 and SAS cell lines. Moreover, high KDM6A expression in tissue sections of patients with HNSCC was associated with improved locoregional control after primary (n = 137) and post-operative (n = 187) radio/chemotherapy. Conversely, high KDM6B expression was a prognostic factor for reduced overall survival. Abstract

Utilizing a novel and exact inversion scheme, we determine the complex linear conductivity $\sigma (\omega )$ from the linear magnetic ac-susceptibility which has been measured from 3\,mHz to 50\,MHz in fields between 0.4\,T and 4\,T applied parallel to the c-axis of a 250\,nm thin disk. The frequency derivative of the phase $\sigma ''/\sigma '$ and the dynamical scaling of $\sigma (\omega)$ above and below $T_g(B)$ provide clear evidence for a continuous phase transition at $T_g$ to a generic superconducting state. Based on the vortex-glass scaling model, the resulting critical exponents $\nu $ and $z$ are close to those frequently obtained on films by other means and associated with an 'isotropic' vortex glass. Abstract

Despite substantial declines since 2000, lower respiratory infections (LRIs), diarrhoeal diseases, and malaria remain among the leading causes of nonfatal and fatal disease burden for children under 5 years of age (under 5), primarily in sub-Saharan Africa (SSA). The spatial burden of each of these diseases has been estimated subnationally across SSA, yet no prior analyses have examined the pattern of their combined burden. Here we synthesise subnational estimates of the burden of LRIs, diarrhoea, and malaria in children under-5 from 2000 to 2017 for 43 sub-Saharan countries. Some units faced a relatively equal burden from each of the three diseases, while others had one or two dominant sources of unit-level burden, with no consistent pattern geographically across the entire subcontinent. Using a subnational counterfactual analysis, we show that nearly 300 million DALYs could have been averted since 2000 by raising all units to their national average. Our findings are directly relevant for decision-makers in determining which and targeting where the most appropriate interventions are for increasing child survival.

Thereafter, an alternating magnetic field generates heat which may be used to release the thermosensitively bound drug and for hyperthermia. Until today, only superficial tumors can be treated with this method. Our aim was to transfer this method into an endoscopic setting to also reach the majority of tumors located inside the body. To find the ideal endoscopic magnetic field trap, which accumulates the most SPION, we first developed a biophysical model considering anatomical as well as physical conditions. Entities of choice were esophageal and prostate cancer. The magnetic susceptibilities of different porcine and rat tissues were measured with a superconducting quantum interference device. All tissues showed diamagnetic behavior. The evaluation of clinical data (computed tomography scan, endosonography, surgical reports, pathological evaluation) of patients gave insight into the topographical relationship between the tumor and its surroundings. Both were used to establish the biophysical model of the tumors and their surroundings, closely mirroring the clinical situation, in which we could virtually design, place and evaluate different electromagnetic coil configurations to find optimized magnetic field traps for each tumor entity. By simulation, we could show that the efficiency of the magnetic field traps can be enhanced by 38-fold for prostate and 8-fold for esophageal cancer. Therefore, our approach of endoscopic targeting is an improvement of the magnetic drug-targeting setups for SPION tumor therapy as it holds the possibility of reaching tumors inside the body in a minimal-invasive way. Abstract

Radiomics analyses provide a promising avenue for enabling personalized radiotherapy. Most frequently, prognostic radiomics models are based on features extracted from medical images that are acquired before treatment. Here, we investigate whether combining data from multiple timepoints during treatment and additionally from multiple imaging modalities can improve the predictive ability of radiomics models. We extracted radiomics features from computed tomography (CT) images acquired before treatment as well as two and three weeks after the start of radiochemotherapy for 55 patients with locally advanced head and neck squamous cell carcinoma (HNSCC). Additionally, we obtained features from FDG-PET images taken before treatment and three weeks after start of therapy. Cox proportional hazards models were then built based on features of the different image modalities, treatment timepoints and combinations thereof using two different feature selection methods in a five-fold cross-validation approach. Based on the cross-validation results, feature signatures were derived and their performance was independently validated. Discrimination regarding loco-regional control was assessed by the concordance index (C-index) and log-rank tests were performed to assess risk stratification. The best prognostic performance was obtained for timepoints during treatment for all modalities. Overall, CT was the best discriminating modality with an independent validation C-index of 0.78 for week two and week two and three combined. However, none of these models achieved a statistically significant patient stratification. Models based on FDG features from week three provided both, satisfactory discrimination (C-index=0.61 and 0.64) and a statistically significant stratification (p=0.044 and p<0.001) but produced highly imbalanced risk groups. Abstract

Ablative radiotherapy is a highly efficient treatment modality for patients with metastatic prostate cancer (PCa). However, a subset of patients does not respond. Currently, this subgroup with bad prognosis cannot be identified before disease progression. We hypothesize that markers indicative of radioresistance, stemness and/or bone tropism may have a prognostic potential to identify patients profiting from metastases-directed radiotherapy. Therefore, circulating tumor cells (CTCs) were analyzed in patients with metastatic PCa (n = 24) during radiotherapy with Cell-Search, multicolor flow cytometry and imaging cytometry. Analysis of copy-number alteration indicates a polyclonal CTC population that changes after radiotherapy. CTCs were found in 8 out of 24 patients (33.3%) and were associated with a shorter time to biochemical progression after radiotherapy. Whereas the total CTC count dropped after radiotherapy, a chemokine receptor CXCR4-expressing subpopulation representing 28.6% of the total CTC population remained stable up to 3 months. At once, we observed higher chemokine CCL2 plasma concentrations and proinflammatory monocytes. Additional functional analyses demonstrated key roles of CXCR4 and CCL2 for cellular radiosensitivity, tumorigenicity and stem-like potential in vitro and in vivo. Moreover, a high CXCR4 and CCL2 expression was found in bone metastasis biopsies of PCa patients. In summary, panCK+CXCR4+ CTCs may have a prognostic potential in patients with metastatic PCa treated with metastasis-directed radiotherapy.

Abstract

Background Tumor hypoxia is associated with resistance to radiotherapy and chemotherapy. In head and neck squamous cell carcinoma (HNSCC), nimorazole, an oxygen mimic, combined with radiotherapy (RT) enabled to improve loco-regional control (LRC) in some patients with hypoxic tumors but it is unknown whether this holds also for radio- chemotherapy (RCTx). Here, we investigated the impact of nimorazole combined with RCTx in HNSCC xenografts and explored molecular biomarkers for its targeted use. Methods Irradiations were performed with 30 fractions in 6 weeks combined with weekly cisplatin. Nimorazole was applied before each fraction, beginning with the first or after ten fractions. Effect of RCTx with or without addition of nimorazole was quantified as permanent local control after irradiation. For histological evaluation and targeted gene expression analysis, tumors were excised untreated or after ten fractions. Using quantitative image analysis, micromilieu parameters were determined. Results Nimorazole combined with RCTx significantly improved permanent local control in two tumor models, and showed a potential improvement in two additional models. In these four models, pimonidazole hypoxic volume (pHV) was significantly reduced after ten fractions of RCTx alone. Our results suggest that nimorazole combined with RCTx might improve TCR compared to RCTx alone if hypoxia is decreased during the course of RCTx but further experiments are warranted to verify this association. Differential gene expression analysis revealed 12 genes as potential for RCTx response. When evaluated in patients with HNSCC who were treated with primary RCTx, these genes were predictive for LRC. Conclusions Nimorazole combined with RCTx improved local tumor control in some but not in all HNSCC xeno-grafts. We identified prognostic biomarkers with the potential for translation to patients with HNSCC.

Editorial In summary, ESTRO clinical and other guidelines are soundly based, but always require validation, careful and consistent implementation, and focused education and training on their use, as well as local monitoring of that. As guidelines develop and older ones become superseded, there should be a clear mechanism to inform the community of the status of specific guidelines, especially when they have been replaced by new or updated versions. An important scientific aim for the future is to further advance guidelines and their individual statements into fully evidence-based instruments. These, in principle, could be directly linked to growing data-bases, allowing feedback mechanisms and thus continuous optimization. Another challenging research topic is the interplay of guidelines with growing opportunities and demands of personalized approaches of treatment.

Abstract

Purpose: Tumor hypoxia is a paradigmatic negative prognosticator of treatment resistance in head and neck squamous cell carcinoma (HNSCC). The lack of robust and reliable hypoxia classifiers limits the adaptation of stratified therapies. We hypothesized that the tumor DNA methylation landscape might indicate epigenetic reprogramming induced by chronic intratumoral hypoxia. Experimental Design: A DNA-methylome–based tumor hypoxia classifier (Hypoxia-M) was trained in the TCGA (The Cancer Genome Atlas)-HNSCC cohort based on matched assignments using gene expression–based signatures of hypoxia (Hypoxia-GES). Hypoxia-M was validated in a multicenter DKTK-ROG trial consisting of human papillomavirus (HPV)–negative patients with HNSCC treated with primary radiochemotherapy (RCHT). Results: Although hypoxia-GES failed to stratify patients in the DKTK-ROG, Hypoxia-M was independently prognostic for local recurrence (HR, 4.3; P ¼0.001) and overall survival (HR, 2.34; P ¼ 0.03) but not distant metastasis after RCHT in both cohorts. Hypoxia-M status was inversely associated with CD8 T-cell infiltration in both cohorts. Hypoxia-M was further prognostic in the TCGA-PanCancer cohort (HR, 1.83; P ¼0.04), underscoring the breadth of this classifier for predicting tumor hypoxia status. Conclusions: Our findings highlight an unexplored avenue for DNA methylation–based classifiers as biomarkers of tumoral hypoxia for identifying high-risk features in patients with HNSCC tumors.

This issue includes a perspective contribution by Lefresne et al. [1] on face-to-face scientific conferences versus virtual meetings and the implications for climate change. Generally, it is the policy of the Green Journal not to publish contributions with non-scientific content. In this specific case, the editors believe that the contribution by Lefresne and colleagues raises an important point that needs careful consideration and discussion in our scientific community. As scientists, we are at the forefront of research and strive to positively shape the future for humankind. Therefore, it is of key importance that the environmental impact of scientific activities is considered, and that our community develops and contributes to solutions to prevent further exacerbation of climate change with its substantial impact on health-related issues. This includes that pros and cons of face-to-face scientific meetings are carefully weighed.

Abstract

Neural-network-based outcome predictions may enable further treatment personalization of patients with head and neck cancer. The development of neural networks can prove challenging when a limited number of cases is available. Therefore, we investigated whether multitask learning strategies, implemented through the simultaneous optimization of two distinct outcome objectives (multi-outcome) and combined with a tumor segmentation task, can lead to improved performance of convolutional neural networks (CNN) and vision transformers (ViT). Model training was conducted on two distinct multicenter datasets for the endpoints loco-regional control (LRC) and progression-free survival (PFS), respectively. The first dataset consisted of pre-treatment computed tomography (CT) imaging for 290 patients and the second dataset contained combined positron emission tomography (PET)/CT for 224 patients. Discriminative performance was assessed by the concordance index (C-index). Risk stratification was evaluated using log-rank tests. Across both datasets, CNN and ViT model ensembles achieved similar results. Multitask approaches showed favorable performance in most investigations. Multi-outcome CNN models trained with segmentation loss were identified as the optimal strategy across cohorts. On the PET/CT dataset, an ensemble of multi-outcome CNNs trained with segmentation loss achieved the best discrimination (C-index: 0.29, 95% confidence interval (CI): 0.22-0.36) and successfully stratified patients into groups with low and high risk of disease progression (p=0.003). On the CT dataset, ensembles of multi-outcome CNNs and of single-outcome ViTs trained with segmentation loss performed best (C-index: 0.26 and 0.26, CI: 0.18-0.34 and 0.18-0.35, respectively), both with significant risk stratification for LRC in independent validation (p=0.002 and p=0.011). Abstract

Purpose: To perform a preclinical trial comparing the efficacy of fractionated radiotherapy versus radiochemotherapy with cisplatinum in HPV-positive and negative human head and neck squamous cell carcinoma (HNSCC) xenografts. Material and methods: Three HPV-negative and three HPV-positive HNSCC xenografts in nude mice were randomized to radiotherapy (RT) alone or to radiochemotherapy (RCT) with weekly cisplatinum. To evaluate tumour growth time, 20 Gy radiotherapy (± Cisplatin) were administered in 10 fractions over 2 weeks. Dose-response curves for local tumor control were generated for RT with 30 fractions over 6 weeks to different dose levels given alone or combined with cisplatinum (RCT). Results: One of three investigated HPV-negative and two out of three HPV-positive tumour models showed a significant increase in local tumour control after RCT compared to RT alone. Pooled analysis of HPV-positive tumour models showed a statistically significant and substantial benefit of RCT versus RT alone, with an enhancement ratio of 1.34. Although heterogeneity in response to both RT and RCT was also observed between the different HPV-positive HNSCC, these overall were more RT and RCT sensitive than HPV-negative models. Conclusion: The impact of adding chemotherapy to fractionated radiotherapy on local control was heterogenous, both in HPV-negative and in HPV-positive tumours, calling for predictive biomarkers. RCT substantially increased local tumour control in the pooled group of all HPVpositive tumours whereas this was not found in HPV-negative tumours. Omission of chemotherapy in HPV-positive HNSCC as part of a treatment de-escalation strategy is not supported by this preclinical trial.

The "MCDA for sustainability assessment – insights to Helmholtz Association activities" Working Paper aims to compile and reflect previous and on-going work within the Helmholtz Association related to MCDA, in particular, to present use cases and key methodological aspects. It has a focus on but is not limited to energy technologies and systems and is mainly based on the presentations held at the online workshop “Multi criteria decision analysis for sustainability assessment of energy technologies and systems”. The workshop was organized within the activities of the Helmholtz program ESD Topic 1 "Energy System Transformation" and took place on November 22nd, 2021.

Abstract

Background: Radiotherapy in patients with primary brain tumors may affect hippocampal structure and cause dyscognitive side-effects. Patients and methods: Using structural MRI and comprehensive neurocognitive evaluation, we investi- gated associations between hippocampal structure and memory deficits in 15 patients with WHO grade 3 and grade 4 gliomas receiving standard radio(chemo)therapy. Results: We did not find changes in hippocampal thickness or cognitive abilities three months after com- pleting radiotherapy. However, subjective memory impairment was associated with symptoms of depression, but not with objective memory performance, cortical thickness of the hippocampus or radi- ation dose. Conclusions: Irrespective of whether there is a bidirectional relationship between affective changes and subjective cognitive dysfunction in these patients, depressive symptoms remain a target for intervention to improve their quality of life. The results of our pilot study highlight that future assessment of side effects of radiotherapy concerning memory should include assessments of depressive symptoms.

Abstract

Patients with locally advanced head and neck squamous cell carcinoma (HNSCC) may benefit from personalised treatment, requiring biomarkers that characterize the tumour and predict treatment response. We integrate pre-treatment CT radiomics and whole-transcriptome data from a multicentre retrospective cohort of 206 patients with locally advanced HNSCC treated with primary radiochemotherapy to classify tumour molecular subtypes based on radiomics, develop surrogate radiomics signatures for gene-based signatures related to different biological tumour characteristics and evaluate the potential of combining radiomics features with full-transcriptome data for the prediction of loco-regional control (LRC). Using end-to-end machine-learning, we developed and validated a model to classify tumours of the atypical subtype (AUC [95% confidence interval]: 0.69 [0.53-0.83]) based on CT imaging, observed that CT-based radiomics models have limited value as surrogates for six selected gene signatures (AUC<0.60), and showed that combining a radiomics signature with a transcriptomics signature consisting of two metagenes representing the hedgehog pathway and E2F transcriptional targets improves the prognostic value for LRC compared to both individual sources (validation C-index [95% confidence interval], combined: 0.63 [0.55-0.73] vs radiomics: 0.60 [0.50-0.71] and transcriptomics: 0.59 [0.49-0.69]). These results underline the potential of multi-omics analyses to generate reliable biomarkers for future application in personalized oncology.
