Background & Aims: Etiologic factor removal (ER) drives recompensation and improves portal hypertension in cirrhosis. Esophageal varices (EV) and portosystemic shunts (PSS) have been found in patients despite hepatic venous pressure gradient (HVPG) dropping below 10 mmHg after ER, questioning HVPG accuracy in reflecting true portal pressure in the setting of ER. We aim to evaluate the correlation of HVPG with direct portal pressure (DPP) in patients with persistence of EV after ER despite HVPG

Abstract Background The immune system has a central role in preventing carcinogenesis. Alteration of systemic immune cell levels may increase cancer risk. However, the extent to which common genetic variation influences blood traits and cancer risk remains largely undetermined. Here, we identify pleiotropic variants and predict their underlying molecular and cellular alterations. Methods Multivariate Cox regression was used to evaluate associations between blood traits and cancer diagnosis in cases in the UK Biobank. Shared genetic variants were identified from the summary statistics of the genome-wide association studies of 27 blood traits and 27 cancer types and subtypes, applying the conditional/conjunctional false-discovery rate approach. Analysis of genomic positions, expression quantitative trait loci, enhancers, regulatory marks, functionally defined gene sets, and bulk- and single-cell expression profiles predicted the biological impact of pleiotropic variants. Plasma small RNAs were sequenced to assess association with cancer diagnosis. Results The study identified 4093 common genetic variants, involving 1248 gene loci, that contributed to blood–cancer pleiotropism. Genomic hotspots of pleiotropism include chromosomal regions 5p15-TERT and 6p21-HLA. Genes whose products are involved in regulating telomere length are found to be enriched in pleiotropic variants. Pleiotropic gene candidates are frequently linked to transcriptional programs that regulate hematopoiesis and define progenitor cell states of immune system development. Perturbation of the myeloid lineage is indicated by pleiotropic associations with defined master regulators and cell alterations. Eosinophil count is inversely associated with cancer risk. A high frequency of pleiotropic associations is also centered on the regulation of small noncoding Y-RNAs. Predicted pleiotropic Y-RNAs show specific regulatory marks and are overabundant in the normal tissue and blood of cancer patients. Analysis of plasma small RNAs in women who developed breast cancer indicates there is an overabundance of Y-RNA preceding neoplasm diagnosis. Conclusions This study reveals extensive pleiotropism between blood traits and cancer risk. Pleiotropism is linked to factors and processes involved in hematopoietic development and immune system function, including components of the major histocompatibility complexes, and regulators of telomere length and myeloid lineage. Deregulation of Y-RNAs is also associated with pleiotropism. Overexpression of these elements might indicate increased cancer risk.

Background & Aims: Porto-sinusoidal vascular disorder (PSVD) encompasses a group of liver diseases with vascular abnormalities that can cause portal hypertension in the absence of cirrhosis. The new diagnostic criteria allow for coexistence with other liver diseases, however its relationship with chronic hepatitis B (CHB) remains unclear. This study aimed to assess HBV prevalence in a PSVD cohort and evaluate its clinical impact. Methods: This retrospective study was conducted on patients with PSVD at Hospital Clínic Barcelona. HBV serology was evaluated, and patients were categorized into HBV chronic infection, past infection, or no HBV exposure. Clinical characteristics and outcomes were compared. Results: We included 155 patients with PSVD. Prevalence of CHB and past HBV infection in patients with PSVD was higher than in the general population (5.8% vs. 0.5%, p

The surgically induced remission of liver disease represents a model to investigate the signalling processes that trigger the development of nonalcoholic steatohepatitis with the aim of identifying novel therapeutic targets. We recruited patients with severe obesity with or without nonalcoholic steatohepatitis and obtained liver and plasma samples before and after laparoscopic sleeve gastrectomy for immunoblotting, immunocytochemical, metabolomic, transcriptomic and epigenetic analyses. Functional studies were performed in HepG2 cells and primary hepatocytes. Surgery was associated with a decrease in the inflammatory response and revealed the role of mitogen-activated protein kinases. Nonalcoholic steatohepatitis was associated with an increased glutaminolysis-induced production of α-ketoglutarate and the hyperactivation of mammalian target of rapamycin complex 1. These changes were crucial for adenosine monophosphate-activated protein kinase/mammalian target of rapamycin-driven pathways that modulated hepatocyte survival by coordinating apoptosis and autophagy and affected methylation-related epigenomic remodelling enzymes. Hepatic transcriptome signatures and differentially methylated genomic regions distinguished patients with and without steatohepatitis. Our results suggest that the increased glutaminolysis-induced α-ketoglutarate production and the mammalian target of rapamycin complex 1 dysregulation play a crucial role in the inefficient adaptive responses leading to steatohepatitis in obesity.

Background & Aims: Spontaneous portosystemic shunts (SPSS) develop frequently in cirrhosis. Changes over time and the effect of aetiological interventions on SPSS are unknown, so we aimed to explore the effect of these variables on SPSS evolution. Methods: Patients with cirrhosis from the Baveno VI-SPSS cohort were selected provided a follow-up abdominal CT or MRI scan was available. Clinical and laboratory data were collected at baseline and follow-up. Imaging tests were reviewed to evaluate changes in the presence and size of SPSS (large (L)-SPSS was ≥8 mm) over time. Regarding alcohol- or HCV-related cirrhosis, two populations were defined: cured patients (abstinent from alcohol or successful HCV therapy), and non-cured patients. Results: A total of 617 patients were included. At baseline SPSS distribution was 22% L-SPSS, 30% small (S)-SPSS, and 48% without (W)-SPSS. During follow-up (median follow-up of 63 months), SPSS distribution worsened: L-SPSS 26%, S-SPSS 32%, and W-SPSS 42% (p

Background & Aims: Whether severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection is a risk factor for splanchnic vein thrombosis (SVT) is unknown. This study aims to assess the impact of SARS-CoV-2 infection on the presentation and prognosis of recent SVT and to identify specific characteristics of SARS-CoV-2-associated SVT. Methods: This is a retrospective study collecting health-related data of 27 patients presenting with recent SVT in the context of SARS-CoV-2 infection in 12 Vascular Liver Disease Group (VALDIG) centres and in comparison with 494 patients with recent SVT before the SARS-CoV-2 pandemic. Results: Twenty-one patients with SARS-CoV-2 had portal vein thrombosis with or without thrombosis of another splanchnic vein, two had superior mesenteric vein thrombosis, one had splenic vein thrombosis, and three had hepatic vein thrombosis. Diagnosis of SVT was made 10 days (95% CI 0–24 days) after the diagnosis of SARS-CoV-2 infection. Fever (52 vs. 15%; p

Science - Abstract
The effect of hyaluronan-mediated motility receptor (HMMR) expression in BRCA1-associated breast cancer risk remains unknown. Here, HMMR overexpression induces the activation of cGAS-STING and non-canonical NF-κB signalling, instigating an immune permissive environment for breast cancer development.

930 - Abstract
This thesis examines degenerative joint disease (DJD) in non-human primates with the aim to detect potentially contributing factors to its presence. Locomotor behaviour has received little attention in studies on DJD in non-human primates as a potential causal factor, despite the great locomotor diversity of the order and the link between human bipedalism and DJD. The main goal was to explore the relationship between the disease and the stress exerted on joints, mainly as a result of locomotion. Locomotor adaptation and locomotor strategy were considered alongside with non-locomotor factors (principally body mass and age) as causal factors for joint degeneration. DJD was recorded for the main weight-bearing joints of 35 taxa of non-human primates, representing a wide variety of locomotor adaptations (vertical clingers and leapers, slow arboreal quadrupeds, leapers, quadrupeds, knuckle-walkers and suspensory primates). These taxa move according to different locomotor strategies, such as different speeds (fast and slow moving species) or different patterns of habitat use (arboreal, terrestrial or semi-arboreal species), which led to interactions with supports of different levels of compliance. The primate taxa studied showed different patterns of DJD expression, which were partially determined by locomotor behaviour. Primates of different locomotor adaptations showed different distribution patterns of DJD according to the levels of stress exerted on different joints or body compartments as a result of locomotion. Moreover, the adoption of different locomotor strategies was coupled with variability of DJD. Other factors, such as body mass or age showed significant correlations with DJD severity exhibiting different coefficients at different joints and body compartments, suggesting that their effect on DJD expression was likely to be important but only in combination with other factors. Multivariate analyses confirmed this conclusion, demonstrating the important effect of mechanical factors in variability of the early development of DJD, as well as a consistent effect of age, especially correlated with an increase in DJD severity and variability. DJD in non-human primates is clearly a multifactorial phenomenon, where stress related to locomotion plays an important role. A new perspective in the study of DJD was presented, providing a good basis for future studies from which other areas could benefit, such as sports medicine, evolutionary primatology or captive care studies.

Background and Purpose In real-world practice, the benefit of mechanical thrombectomy (MT) is uncertain in stroke patients with very favorable or poor prognostic profiles at baseline. We studied the effectiveness of MT versus medical treatment stratifying by different baseline prognostic factors. Methods Retrospective analysis of 2,588 patients with an ischemic stroke due to large vessel occlusion nested in the population-based registry of stroke code activations in Catalonia from January 2017 to June 2019. The effect of MT on good functional outcome (modified Rankin Score ≤2) and survival at 3 months was studied using inverse probability of treatment weighting (IPTW) analysis in three pre-defined baseline prognostic groups: poor (if pre-stroke disability, age >85 years, National Institutes of Health Stroke Scale [NIHSS] >25, time from onset >6 hours, Alberta Stroke Program Early CT Score 3), good (if NIHSS

After years of intense fieldwork, our knowledge about the Neolithisation of the Pyrenees has considerably increased. In the southern central Pyrenees, some previously unknown Neolithic sites have been discovered at subalpine and alpine altitudes (1,000–1,500 m a.s.l.). One of them is Cueva Lóbrica, 1,170 m a.s.l., which has an occupation phase with impressed pottery dated ca. 5400 cal BCE. Another is Coro Trasito, 1,558 m a.s.l., a large rock shelter that preserves evidence of continuous occupations in the Early Neolithic, 5300–4600 cal BCE. Evidence of human occupation at higher altitudes has also been documented. In the Axial Pyrenees, at the Obagues de Ratera rock shelter, 2,345 m a.s.l., an occupation has been dated to around 5730–5600 cal BCE. At Cova del Sardo, in the Sant Nicolau Valley, at 1,780 m a.s.l., a series of occupations have been excavated, dated to ca. 5600–4500 cal BCE. These sites allow us to discuss patterns of occupation of the mountainous areas between the Late Mesolithic and Early Neolithic. Recent data suggest that the last hunter–gatherer occupied all altitudinal stages of the Pyrenees, both in the outer and inner ranges. A change in the settlement pattern seems to have occurred in the Early Neolithic, which consisted of a concentration of occupations in the valley bottom and mid-slopes, in biotopes favourable to both herding and agriculture.

- Abstract
This work is the second of a two-part research project focused on modeling solid-shell elements using a stabilized two-field finite element formulation. The first part introduces a stabilization technique based on the Variational Multiscale framework, which is proven to effectively address numerical locking in infinitesimal strain problems. The primary objective of the study was to characterize the inherent numerical locking effects of solid-shell elements in order to comprehensively understand their triggers and how stabilized mixed formulations can overcome them. In this current phase of the work, the concept is extended to finite strain solid dynamics involving hyperelastic materials. The aim of introducing this method is to obtain a robust stabilized mixed formulation that enhances the accuracy of the stress field. This improved formulation holds great potential for accurately approximating shell structures undergoing finite deformations. To this end, three techniques based in the Variational Multiscale stabilization framework are presented. These stabilized formulations allow circumventing the compatibility restriction of interpolating spaces of the unknowns inherent to mixed formulations, thus allowing any combination of them. The accuracy of the stress field is successfully enhanced while maintaining the accuracy of the displacement field. These improvements are also inherited to the solid-shell elements, providing locking-free approximation of thin structures., This work was supported by Vicerrectoría de Investigación, Desarrollo e Innovación (VRIDEI) of the Univeridad de Santiago de Chile, and the National Agency for Research and Development (ANID) Doctorado Becas Chile/2019 - 72200128 of the Government of Chile. - Abstract
This work studies the solid-shell finite element approach to approximate thin structures using a stabilized mixed displacement-stress formulation based on the Variational Multiscale framework. The work is divided in two parts. In Part I, the numerical locking effects inherent to the solid-shell approach are characterized using a variety of benchmark problems in the infinitesimal strain approximation. In Part II, the results are extended to formulate the mixed approach in finite strain hyperelastic problems. In the present work, the stabilized mixed displacement-stress formulation is proven to be adequate to deal with all kinds of numerical locking. Additionally, a more comprehensive analysis of each individual type of numerical locking, how it is triggered and how it is overcome is also provided. The numerical locking usually occurs when parasitic strains overtake the system of equations through specific components of the stress tensor. To properly analyze them, the direction of each component of the stress tensor has been defined with respect to the shell directors. Therefore, it becomes necessary to formulate the solid-shell problem in curvilinear coordinates, allowing to give mechanical meaning to the stress components (shear, twisting, membrane and thickness stresses) independently of the global frame of reference. The conditions in which numerical locking is triggered as well as the stress tensor component responsible of correcting the locking behavior have been identified individually by characterizing the numerical response of a set of different benchmark problems., Peer Reviewed, Postprint (published version)

Abstract Patients with morbid obesity frequently present non-alcoholic fatty liver (NAFL) and non-alcoholic steatohepatitis (NASH) associated with pro-atherogenic alterations. Laparoscopic sleeve gastrectomy (LSG) is an effective treatment for weight reduction, and for the remission of hepatic alterations. Using 1H-nuclear magnetic resonance (1H-NMR), we investigated the effects of LSG on lipoprotein and glycoprotein profile in patients with morbid obesity and liver disease. We included 154 patients with morbid obesity (49 non-NASH, 54 uncertain NASH, 51 definite NASH). A blood sample was obtained before surgery and, in patients with definite NASH, one year after surgery. Patients with NASH had increased concentrations of medium and small VLDL particles, VLDL and IDL cholesterol concentrations, IDL, LDL, and HDL triglyceride concentrations, and elevated glycoprotein levels. These changes were more marked in patients with type 2 diabetes mellitus. LSG produced significant decreases in the concentration of VLDL particles, VLDL cholesterol and triglycerides, an increase in the concentration LDL particles and LDL cholesterol concentrations, and a decrease in protein glycation. We conclude that patients with obesity and NASH had significant alterations in circulating levels of lipoproteins and glycoproteins that were associated with the severity of the disease. Most of these changes were reversed post-LSG.
