The aim of this study is to evaluate the quality of posts performed by dental students at King Abdulaziz University Dental Hospital (KAUDH), Jeddah, Saudi Arabia, utilizing periapical radiographs and whether the prosthetic criteria were strictly met by students.Materials and Methods: This cross-sectional study included 661 digital periapical radiographs of posts performed by 6th-year dental students from September 2018 to April 2019. The assessment included the following factors: patient’s age, patient’s gender, tooth type, and arch. Post-related factors including post type, shape, diameter, length, and status of the remaining gutta percha were also evaluated. Statistical analysis was performed using SPSS. Descriptive statistics were generated, and the association between different variables was determined using chi-square test at p< 0.05.Results: Posts were used to restore maxillary teeth (67%), and were particularly placed in premolars (44%). The majority of used posts were prefabricated tapered fiber posts (90%). The results were as follows: post diameter equal to one-third of root diameter, 50% met criterion; post length equal to two-thirds of root length, 33%; post length equal to or more than crown height, 93%; absence of space between gutta percha (GP) and post, 74%; and length of the remaining GP equal to 3– 5 mm, 68%. Overall, 11% of the posts met all the ideal prosthetics criteria.Conclusion: Most qualities of post and core restorations that were radiographically assessed were found to be acceptable and within the recommended prosthodontic criteria.Keywords: post and core, post diameter, post length, post shape, gutta percha length, prefabricated post, cast post

Purpose: Although there are many researches that focus on the relationship between the vertebral artery and uncinate process (UP), there were no publications concerning difference in the dimensions of the UP between the normal spine and degenerative spine, especially in Chinese patient. The purpose of this study is to determine the anatomic parameters that can be used as a guide for the procedure in intervertebral foramen decompression and for analysis of the morphometric change in the UP of the cervical spondylosis patients. Methods: Forty patients from January 2016 to January 2019 were enrolled in this study. Three-dimensional computed tomography scans of the cervical spine were performed. The patients were subdivided into two groups which were nondegenerative cervical spine group (20 cases) and degenerative cervical spine group (20 cases). Six parameters concerning the height, width and angle of the UP were measured. Results: In nondegenerative group, the average pedicle width was 3.63 mm–5.91 mm from C3 to C7. The average width of safe UP resection will be 3.06 mm at C3, 3.12 mm at C4, 3.28 mm at C5, 2.74 mm at C6 and 2.01 mm at C7. The average safe depth will be 6.04 mm at C3, 6.52 mm at C4, 7.61 mm at C5, 6.07 mm at C6 and 5.09 mm at C7. There are statistic difference between degenerative group and nondegenerative group, especially in the parameter minimum height of UP, maximum height of UP, medial border's distance of UP and later border's distance of UP. Conclusion: In this retrospective study, our results suggest that for the Chinese patients who suffered from cervical spondylosis could be performed intervertebral foraminotomy decompression by resecting part of the UP. The safe range within the spinal canal was up to 6.73 mm of width between inferior vertebral endplate and superior vertebral endplate in the intervertebral space and up to 5.09 mm of depth from medial border of the UP to the lateral side atC3 to C7 without interfering the spinal nerve root and vertebral artery. The translational potential of this article: Our study found the safe margin to perform intervertebral foramen decompression to the UP for the cervical spondylosis patients. This may help to improve safeness of the surgical procedure and provide data for future robotic surgery.

This study in Turkey evaluated the impact of age-based mandatory single-embryo transfer (SET) legislation with the subsequent increase in frozen-thawed embryo transfer (FT-ET) on pregnancy outcome of in vitro fertilization (IVF) patients. SET, FT-ET and double-embryo transfer were used in 5632 patients after legislation, while traditional IVF and FT-ET approach was used in 6029 patients before legislation. The cumulative pregnancy rate after legislation was slightly lower (38.2%) than before legislation (42.0%) but not significantly so. The single pregnancy rate for SET and traditional IVF were similar between the 2 groups (37.8% versus 28.7%), while multiple pregnancy rates were significantly higher before than after legislation (13.7% versus 0.3%). For FT-ET, the number of cycles was significantly higher after legislation (862 versus 616). SET yielded similar results to traditional IVF. In order to reduce multiple pregnancies without significantly decreasing pregnancy rates, SET might be a successful strategy. [ABSTRACT FROM AUTHOR]

Adult hematopoietic and other tissue stem cells have highly constrained cell cycling that limits their susceptibility to standard gene therapy vectors, which depend upon chromosomal integration. Using cytokine cocktails to increase transduction efficiency often compromises subsequent stem cell function in vivo. We previously showed that p21Waf1/Cip1/Sdi1 (p21) mediates stem cell quiescence in vivo and decreasing its expression ex vivo leads to an expansion of stem cell pool in vivo. Here, we report that application of p21 specific siRNA increased the gene transduction efficiency in hematopoietic stem cells while preserving cell multipotentiality. Both types of siRNA, synthesized siRNA and transcribed shRNA, reduced p21 expression in target cells by 85–98%. The effect of RNAi in these cells was transient and the level of p21 mRNA returned to base line 14–28 days after siRNA treatment. This brief interval of reduction, however, was sufficient to increase transduction efficiency to two- to four-fold in cell cultures, and followed by a seven- to eight-fold increase in mice. The RNAi treated, lentivector-transduced CD34+ cells retained multipotentiality as assessed in vitro by colony formation assay and in vivo by NOD/SCID mouse transplantation assay. Reduction of p21 resulted in an increased chromosomal integration of lentivector into target cellular DNA. Taken together, both synthesized and transcribed siRNA knocked down p21 expression in human CD34+ hematopoietic stem/progenitor cells. Silencing p21 expression increased gene transduction efficiency and vector integration while retaining stem cell multipotentiality. Thus, RNAi targeting of p21 is a useful strategy to increase stem cell gene transfer efficiency. Context. Polycystic ovary syndrome (PCOS) is associated with increased prevalence of preeclampsia (PE); microRNAs (miRs) could play an important role in the pathogenesis of PE and PCOS. Objective. To investigate the expression levels of miRs 155-5p and 518b in blood leukocytes of patients with PE and PCOS. Design. Using real-time quantitative PCR method, miR-155-5p and miR-518b were examined from PE, PCOS, PE+PCOS, and controls. Subjects and Methods. The relative expression of the target miRs in patient samples was compared to control samples. The results were calculated as relative quantification values. Results. Confounding variables were controlled using analyses for covariance. Significant differences were observed in miR-155-5p (p=0.008) and miRNA-518 (p=0.016) expression levels among the groups. miR-155-5p (p=0.014) and miR-518b (p=0.036) were upregulated in PCOS patients and miR-518b (p=0.028) were increased in cases with PCOS+PE. Near significant difference was found (p=0.06) in miR-518b expression levels in cases with PE, compared to controls. miR-518b was observed to be positively correlated with alanine transaminase in cases with PE (r=0.80; P=0.017) and PE+PCOS (r=0.80, p=0.017). Conclusions. Our preliminary findings suggested that expression profiling of miR-155-5p and miR-518b in blood leukocytes were upregulated in pregnant women with PCOS. Colorectal cancer is a prevalent malignancy with global significance. This retrospective study aimed to investigate the influence of stage and tumor site on survival outcomes in 284 colorectal cancer patients diagnosed between 2001 and 2017. Patients were categorized into four groups based on tumor site (colon and rectum) and disease stage (early stage and advanced stage). Demographic characteristics, treatment modalities, and survival outcomes were recorded. Bayesian survival modeling was performed using semi-competing risks illness-death models with an accelerated failure time (AFT) approach, utilizing R 4.1 software. Results demonstrated significantly higher time ratios for disease recurrence (TR = 1.712, 95% CI 1.489-2.197), mortality without recurrence (TR = 1.933, 1.480-2.510), and mortality after recurrence (TR = 1.847, 1.147-2.178) in early-stage colon cancer compared to early-stage rectal cancer. Furthermore, patients with advanced-stage rectal cancer exhibited shorter survival times for disease recurrence than patients with early-stage colon cancer. The interaction effect between the disease site and cancer stage was not significant. These findings, derived from the optimal Bayesian log-normal model for terminal and non-terminal events, highlight the importance of early detection and effective management strategies for colon cancer. Early-stage colon cancer demonstrated improved survival rates for disease recurrence, mortality without recurrence, and mortality after recurrence compared to other stages. Introduction: COVID-19 causes severe inflammatory respiratory distress syndrome. The global pandemic caused millions of cases of morbidity and mortality worldwide. Patients may present with variable symptoms including dyspnea, fever, and GIT manifestations. The HMOX-1 gene is located on the long (q) arm of chromosome 22 at position 12.3. HMOX-1 is expressed in all mammalian tissues at basal levels and is considered as a stress response enzyme. HMOX-1 has a specific polymorphic site with variable GT(n) repeats at the promotor region. Several authors evaluated the HMOX-1 GT(n) promoter polymorphism in different inflammatory conditions. We evaluated HMOX-1 promoter polymorphism in relation to serum Hemoxygenase level and inflammatory makers (CRP, Ferritin, PCT, IL-6 and D-dimer) in patients affected by SARS-COV-2 disease., Subjects and Methods: Ninety patients confirmed to be infected with COVID-19 were followed up till the study end point (recovery and discharge or death). HMOX-1 promotor GT(n) polymorphism was evaluated using Sanger sequencing. HMOX-1 enzyme serum level was measured by ELISA and the level of different inflammatory markers was assessed by available commercial kits., Results: A novel Single nucleotide polymorphism (SNP) (A > G) - rs13057211 in the GT(n) region of HMOX-1 promoter gene was found in 40 (61.5%) COVID-19 patients out of the studied 65 patients. This (A > G) SNP was associated with higher mortality rate in COVID-19 as it was detected in 27 patients (75% of the patients who succumbed to the disease) (p = 0.021, Odds ratio = 3.7; 95% CI:1.29-10.56). Serum IL-6 (Interleuken-6) was positively correlated the length of Hospital Stay (LOHS) and procalcitonin (PCT); (p = 0.014, r: 0.651 and p < 0.001, r:0.997) respectively while negatively correlated with levels of HMOX-1 enzyme serum level (p = 0.013, r: -0.61). CRP correlated positively with LOHS (p = 0.021, r = 0.4), PCT (p = 0.044, r = 0.425) and age (p < 0.001, r = 0.685). Higher levels of D-Dimer and PCT were observed in patients with the long repeat. There was no significant difference between patients who recovered and those who died from COVID-19 as regards HMOX-1 level and GT(n) polymorphism., Conclusion: We report a novel SNP (A > G, rs13057211) in the GT(n) region of HMOX-1 promoter gene that was associated with mortality in COVID-19 patients, however no significant difference was found in HMOX-1 serum level or HMOX-1 (GT)n repeats within the studied groups., (© 2023. Medical research frequently relies on Cox regression to analyze the survival distribution of cancer patients. Nonetheless, in specific scenarios, neural networks hold the potential to serve as a robust alternative. In this study, we aim to scrutinize the effectiveness of Cox regression and neural network models in assessing the survival outcomes of patients who have undergone treatment for colorectal cancer. We conducted a retrospective study on 284 colorectal cancer patients who underwent surgery at Imam Khomeini clinic in Hamadan between 2001 and 2017. The data was used to train both Cox regression and neural network models, and their predictive accuracy was compared using diagnostic measures such as sensitivity, specificity, positive predictive value, accuracy, negative predictive value, and area under the receiver operating characteristic curve. The analyses were performed using STATA 17 and R4.0.4 software. The study revealed that the best neural network model had a sensitivity of 74.5% (95% CI 61.0-85.0), specificity of 83.3% (65.3-94.4), positive predictive value of 89.1% (76.4-96.4), negative predictive value of 64.1% (47.2-78.8), AUC of 0.79 (0.70-0.88), and accuracy of 0.776 for death prediction. For recurrence, the best neural network model had a sensitivity of 88.1% (74.4-96.0%), specificity of 83.7% (69.3-93.2%), positive predictive value of 84.1% (69.9-93.4%), negative predictive value of 87.8% (73.8-95.9%), AUC of 0.86 (0.78-0.93), and accuracy of 0.859. The Cox model had comparable results, with a sensitivity of 73.6% (64.8-81.2) and 85.5% (78.3-91.0), specificity of 89.6% (83.8-93.8) and 98.0% (94.4-99.6), positive predictive value of 84.0% (75.6-90.4) and 97.4% (92.6-99.5), negative predictive value of 82.0% (75.6-90.4) and 88.8% (0.83-93.1), AUC of 0.82 (0.77-0.86) and 0.92 (0.89-0.95), and accuracy of 0.88 and 0.92 for death and recurrence prediction, respectively. In conclusion, the study found that both Cox regression and neural network models are effective in predicting early recurrence and death in patients with colorectal cancer after curative surgery. The neural network model showed slightly better sensitivity and negative predictive value for death, while the Cox model had better specificity and positive predictive value for recurrence. Introduction: Endocrown restorations have increased in popularity in recent years due to the advancement of both adhesive and restorative materials. The clinical success of endocrowns depends on several factors, including preparation design, material selection, fracture resistance, and marginal adaptation. The aim of this in vitro study was to compare the fracture strength of endocrown restorations fabricated from three different computer-aided design (CAD)/computer-aided manufacturing (CAM) materials., Methods: Thirty extracted mandibular first molars were selected. The teeth underwent conventional root canal treatment before being prepared for endocrown restoration. The teeth were allocated to three groups ( n =10), corresponding to each of the three ceramic materials used to fabricate the endocrowns they would receive. The ceramic materials used were zirconia-reinforced lithium silicate glass-ceramic (VITA Suprinity, VITA Zahnfabrik, Bad Säckingen, Germany), polymer-infiltrated hybrid ceramic (VITA Enamic, VITA Zahnfabrik, Bad Säckingen, Germany), and lithium disilicate glass-ceramic (IPS e.max CAD, Ivoclar Vivadent, Schaan, Liechtenstein). The specimens were scanned, and the digital impressions were transferred into design software to construct the endocrowns. The endocrowns were then milled and cemented. A universal testing machine (5969L3504, Instron, USA) was used for the fracture strength test at a crosshead speed of 1 mm/min until catastrophic failure occurred. Statistical analysis was performed using IBM Corp. Released 2015. IBM SPSS Statistics for Windows, Version 23.0. Armonk, NY: IBM Corp., Results: The one-way analysis of the variance test indicated a significant difference in the fracture strength between the different ceramic groups tested ( P =0.037). The Tukey posthoc test showed that the IPS e.max CAD group had marginally higher fracture strength values than the VITA Enamic group ( P =055). No significant differences in fracture strength values were found between the VITA Enamic and VITA Suprinity groups or between the VITA Suprinity and IPS e.max CAD groups ( P >0.05)., Conclusion: The reported fracture strength values for all the tested materials were higher than the strength required to resist masticatory forces. Introduction: The marginal seal and adaptation are important factors for successful restoration. An inadequate marginal seal can lead to bacterial microleakage, plaque accumulation, and eventually treatment failure This in vitro study aimed to compare the marginal gap of endocrowns fabricated from three different computer-aided design/computer-aided manufacturing (CAD/CAM) ceramic materials., Methods: Thirty extracted mandibular molars were selected for the study. Endocrown preparations were completed after root canal treatment. Teeth were divided into three groups to receive endocrowns fabricated of lithium disilicate ceramic (IPS-e.max CAD, Ivoclar Vivadent AG, Schaan, Liechtenstein), zirconia-reinforced lithium silicate ceramic (VITA Suprinity®, VITA Zahnfabrik, Bad Säckingen, Germany), and polymer-infiltrated ceramic (VITA Enamic®, VITA Zahnfabrik). The digital impressions were transferred to the design software to construct the endocrowns. The endocrowns were milled and cemented. The marginal fit was examined using a digital camera stereomicroscope at a magnification of 80X. Images were transferred to Image-J software (National Institutes of Health, Bethesda, Maryland, United States) to measure the marginal gap., Results: One-way ANOVA showed a significant difference in the marginal gap between the different ceramic groups (P=0.006). Tukey's Honest Significant Difference (HSD) post-hoc test showed that VITA Suprinity had significantly higher gap width values than VITA Enamic (P=0.005). No significant differences in gap width values were found between VITA Enamic and IPS e.max CAD or between VITA Suprinity and IPS e.max CAD (P>0.05)., Conclusion: The marginal gap of endocrown restorations varies with different CAD/CAM materials (zirconia-reinforced lithium silicate glass-ceramic, polymer-infiltrated hybrid ceramic, and lithium disilicate glass-ceramic), but are all within clinically acceptable marginal gap width., Competing Interests: The authors have declared that no competing interests exist., (Copyright © 2023, Attar et al.)
