Abstract

It has still to be ascertained whether severe acute respiratory syndrome coronavirus 2 infection in pregnancy is associated with worse maternal and fetal outcomes compared to low risk gestations.This study aimed to evaluate maternal and perinatal outcomes in high- and low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection.This was a multinational retrospective cohort study involving women with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection from 76 centers from 25 countries in Europe, the United States, South America, Asia, and Australia from April 4, 2020, to October 28, 2020. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit, use of mechanical ventilation, or death. The secondary outcome was a composite measure of adverse perinatal outcome, including miscarriage, fetal loss, neonatal and perinatal death, and admission to the neonatal intensive care unit. All outcomes were assessed in high- and low-risk pregnancies. Pregnancies were considered high risk in case of either preexisting chronic medical conditions in pregnancy or obstetrical disorders occurring in pregnancy. The Fisher exact test and logistic regression analysis were used to analyze the data.A total of 887 singleton pregnancies who tested positive for severe acute respiratory syndrome coronavirus 2 infection using reverse transcription-polymerase chain reaction of nasal and pharyngeal swab specimens were included in the study. The risk of composite adverse maternal outcomes was higher in high-risk pregnancies than in low-risk pregnancies (odds ratio, 1.52; 95% confidence interval, 1.03-2.24; P=.035). In addition, women carrying high-risk pregnancies were at higher risk of hospital admission (odds ratio, 1.48; 95% confidence interval, 1.07-2.04; P=.002), presence of severe respiratory symptoms (odds ratio, 2.13; 95% confidence interval, 0.41-3.21; P=.001), admission to the intensive care unit (odds ratio, 2.63; 95% confidence interval, 1.42-4.88), and invasive mechanical ventilation (odds ratio, 2.65; 95% confidence interval, 1.19-5.94; P=.002). When exploring perinatal outcomes, high-risk pregnancies were at high risk of adverse perinatal outcomes (odds ratio, 1.78; 95% confidence interval, 0.15-2.72; P=.009). However, such association was mainly because of the higher incidence of miscarriage in high-risk pregnancies compared with that in low-risk pregnancies (5.3% vs 1.6%, P=.008); furthermore, there was no difference in other explored outcomes between the 2 study groups. At logistic regression analysis, maternal age (odds ratio, 1.12; 95% confidence interval, 1.02-1.22; P=.023) and high-risk pregnancy (odds ratio, 4.21; 95% confidence interval, 3.90-5.11; P.001) were independently associated with adverse maternal outcomes.High-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection were at higher risk of adverse maternal outcomes than low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection.

5. Evaluation of endometrial sampling results in postmenopausal patients; the importance of insufficient endometrial sampling

Abstract

AmaçPostmenopozal hasta populasyonunda endometrial örnekleme sonuçlarının değerlendirilmesi ve yetersiz endometrial örnekleme sıklığının belirlenmesi amaçlanmıştır.Gereç ve Yöntem Bu retrospektif çalışmada Bursa Şehir Hastanesi kadın hastalıkları ve doğum bölümüne Temmuz 2019 ve Mart 2021 tarihleri arasında başvuran postmenopozal dönemdeki 825 hastadan alınan endometrial biyopsi sonuçları değerlendirilmiştir.Bulgular Postmenopozal dönemde olan 825 hastadan alınan 922 endometrial örnekleme sonucu değerlendirildi. Hastaların yaş ortalaması 56.96±7.21 olup, ortalama menopoz süresi 7.43±7.14 yıldır. Hastalardan alınan 1. endometrial örneklemenin histopatolojik değerlendirmesi yapıldığında 25.45 % yetersiz endometrial örneklemeye rastlanırken postmenopozal kanama şikayeti ile başvuran hasta grubunda ise bu oran %21.61 dir. En sık endometrial patoloji endometrial polip (22.1%) olup çalışma grubumuzda malignite sıklığı %6.18 idi. Alt grup analizlerde malignite görülme sıklığı >60 yaş grubunda daha yüksek (%18.80) iken yaş ile birlikte yetersiz endometrial örneklemenin de arttığı belirlenmiştir. Birinci endometrial örnekleme sonucu yetersiz olan hastaların sadece %20 sinden tekrar biyopsi alınabilmiştir. Bu hastalardan 3 ünde endometrium kanseri ve 1 hastada atipili endometrial hiperplazi varlığı tespit edilmiştir. SonuçArtan yaş ile birlikte endometrium kanseri sıklığı artmasına rağmen yetersiz endometrial örneklemenin de arttığı gözlenmiştir. Özellikle bu hasta populasyonunda endometrium kanseri risk faktörlerini de göz önünde bulundurarak takiplere gelmesi konusunda yeterli bilgilendirmenin yapılması önem taşımaktadır., Aim: We aimed to evaluate the endometrial sampling results and to determine the frequency of non diagnostic endometrial sampling in the postmenopausal patient population.Materials and Method: In this retrospective study, endometrial biopsy results obtained from 825 postmenopausal women who applied to Bursa City Hospital's Obstetrics and Gynecology Department between July 2019 and March 2021 were evaluated.Results: 922 endometrial sampling results from 825 postmenopausal patients were evaluated. The mean age of the patients was 56.96±7.21 and the mean menopause time was 7.43±7.14 years. When the histopathological evaluation of the first endometrial sampling taken from the patients was performed, 25.45% of the patients had insufficient endometrial sampling, while this rate was 21.61% in the group of patients who presented with the complaint of postmenopausal bleeding. The most common endometrial pathology was endometrial polyp (22.1%), and the incidence of malignancy in our study group was 6.18%. In subgroup analyzes, the incidence of malignancy was higher (18.80%) in the age group >60 years, and it was also determined that insufficient endometrial sampling increased with age. Re-biopsy was obtained from only 20% of the patients whose first endometrial sampling result was insufficient. Endometrial cancer was detected in 3 of these patients and the presence of atypical endometrial hyperplasia in 1 patient.Conclusion: It has been observed that in addition to the increase in the incidence of endometrial cancer with advancing age, the frequency of insufficient endometrial sampling also increases. Considering the risk factors for endometrial cancer, these patients should be encouraged to continue their follow-up.

BACKGROUND: It has still to be ascertained whether severe acute respiratory syndrome coronavirus 2 infection in pregnancy is associated with worse maternal and fetal outcomes compared to low risk gestations. OBJECTIVE: This study aimed to evaluate maternal and perinatal outcomes in high- and low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection. STUDY DESIGN: This was a multinational retrospective cohort study involving women with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection from 76 centers from 25 countries in Europe, the United States, South America, Asia, and Australia from April 4, 2020, to October 28, 2020. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit, use of mechanical ventilation, or death. The secondary outcome was a composite measure of adverse perinatal outcome, including miscarriage, fetal loss, neonatal and perinatal death, and admission to the neonatal intensive care unit. All outcomes were assessed in high- and low-risk pregnancies. Pregnancies were considered high risk in case of either preexisting chronic medical conditions in pregnancy or obstetrical disorders occurring in pregnancy. The Fisher exact test and logistic regression analysis were used to analyze the data. RESULTS: A total of 887 singleton pregnancies who tested positive for severe acute respiratory syndrome coronavirus 2 infection using reverse transcription-polymerase chain reaction of nasal and pharyngeal swab specimens were included in the study. The risk of composite adverse maternal outcomes was higher in high-risk pregnancies than in low-risk pregnancies (odds ratio, 1.52; 95% confidence interval, 1.03-2.24; P=.035). In addition, women carrying high-risk pregnancies were at higher risk of hospital admission (odds ratio, 1.48; 95% confidence interval, 1.07-2.04; P=.002), presence of severe respiratory symptoms (odds ratio, 2.13; 95% confidence interval, 0.41-3.21; P=.001), admission to the intensive care unit (odds ratio, 2.63; 95% confidence interval, 1.42-4.88), and invasive mechanical ventilation (odds ratio, 2.65; 95% confidence interval, 1.19-5.94; P=.002). When exploring perinatal outcomes, high-risk pregnancies were at high risk of adverse perinatal outcomes (odds ratio, 1.78; 95% confidence interval, 0.15-2.72; P=.009). However, such association was mainly because of the higher incidence of miscarriage in high-risk pregnancies compared with that in low-risk pregnancies (5.3% vs 1.6%, P=.008); furthermore, there was no difference in other explored outcomes between the 2 study groups. Abstract

Amaç: Sezaryen doğum ve spontan vajinal yolla doğumun yenidoğan iyilik hali üzerinde oluşturacağı etkiyi Transkranial Doppler (TCD) ve laboratuvar parametreleri ile karşılaştırarak araştırmaktır.Materyal ve Metod:Çalışma Nisan 2016 ve Kasım 2016 tarihleri arasında Ankara Keçiören Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniğine doğum için başvuran normal ve sezaryen yapılan hastalar ile yapıldı. Çalışmaya takiplerinde herhangi bir sistemik hastalığı ve doğum komplikasyonu olmayan 50 spontan vajinal doğum yapan hasta ve elektif amaçlı spinal anestezi uygulanarak sezaryenle doğum yapan 50 hasta dahil edildi. Hastalara doğum sonrası ilk 6-24 saat içinde yenidoğanın radyoloji uzmanınca TCD yapıldı. Ayrıca doppler ile birlikte aynı zaman aralığında yenidoğanlara hemogram parametreleri bakıldı. TCD indeksleri ve laboratuvar parametreleri her iki grup için ayrı ayrı değerlendirildi. Elde edilen sonuçlar istatiksel inceleme için SPSS programı kullanıldı.Bulgular: Araştırmaya kapsamında 50 normal spontan vajinal yolla doğum yapan ve 50 elektif sezaryen ile doğum yapan olmak üzere toplam 100 yenidoğan dahil edildi. Her iki gruba TCD ultrasonografi yapılarak anterior serebral arter (ACA) ve orta serebral arter (MCA) doppler parametreleri açısından karşılaştırdıklarında TCD ölçümünde alınan sezaryen ile doğumlarda ACA Pİ, Rİ, SD ortalama oranı sırasıyla 1.34±0.26, 0.71±0.12, 3.87±1.04; SağMCA Pİ, Rİ, SD sırasıyla 1.46±0.36, 0.75±0.07, 4.34±1.44; Sol MCA Pİ, Rİ ve SD ortalama oranları sırasıyla 1.42±0.32, 0.74± 0.08, 4.14±1.41 idi. Vajinal doğumlarda ise ACA Pİ, Rİ, ve S/D ortalama oranı sırasıyla 1.36±0.28, 0.72±0.08, 3.87±1.08; Sağ MCA Pİ, Rİ, SD ortalama oranları sırasıyla 1.44±0.35, 0.74± 0.08, 4.18±1.20; Sol MCA ortalama Pİ, Rİ, SD oranı sırasıyla 1.46±0.35, 0.74± 0.08, 4.26 ±1.34 idi ve Her iki grup doğumdan sonraki 6-24 saat içinde kranial MCA ve ACA dopler bulguları normal sınırlarda olup her iki grup doppler parametreleri açısından karşılaştırıldığında aralarında anlamlı fark saptanmadı (P˃0.05). Yine her iki grubun 1. ve 5. dakika apgarları, cinsiyet, kilo ve yoğun bakım ihtiyaçları açısından karşılaştırıldığında her iki grup arasında anlamlı fark bulunmadı. Her iki grup hemogram paramatreleri olarak Hb, PLT, MPV, WBC, Nötrofil, lenfosit ve nötrofil-lenfosit oranı (NLO) açısından da karşılaştırıldıklarında Hb, PLT, MPV, WBC, NLO sırasıyla ortalama değerleri 15.74 ±2.30, 238.4± 51.8, 6.76± 0.72,15.47 ±4.75, 2.06± 1.70 iken, vajinal doğumlarda sırasıyla 16.15± 1.72, 253.2 ±74.7, 7.26 ±0.79, 14.59±4.09, 1.67±0.58 olup her iki grup arasında ortalama değerler arasında anlamlı fark saptanmadı (P˃0.05).Aim: This study aims to investigate the effect of cesarean delivery (C/S) and vaginal delivery on newborn goodness condition parameters by comparing with Transcranial Doppler (TCD) and Neutrophil-Lymphocyte ratio parameters. Material and Method: This study was performed with patients attending Ankara Keçiören Training and Research Hospital Obstetrics and Gynecology clinic between April 2006 and November 2016. A total of 100 patients were enrolled in this study. The inclusion criteria were having no systemic disease and birth complications during vaginal delivery (50 patients) and C/S delivery under elective spinal anesthesia (50 patients). After delivery, transcranial MCA doppler investigation was made within 6-24 hours by an expert newborn radiologist. At the same time intervals, hemogram parameters of newborn infants were measured. TCD indexes and laboratory parameters of the two groups were evaluated separately. Statistical analyses were performed by using SPSS version 22.0 program.Results: This study included 100 patients, of these, 50 patients had a vaginal delivery, 50 patients had C/S. Transfrontal cranial doppler was applied to both groups and parameters of the middle cerebral artery and anterior cerebral artery were compared. There was no significant difference between groups. TCD measurement parameters MCA Pİ, Rİ, SD were 1.46±0.36, 0.75±0.07, 4.34±1.44 respectively, the right value of MCA Pİ, Rİ, SD were 1.46±0.36, 0.75±0.07, 4.34±1.44 respectively; the left value of MCA Pİ, Rİ, SD were 1.42±0.32, 0.74± 0.08, 4.14±1.41 respectively of C/S patients. For vaginal delivery, ACA Pİ, Rİ, SD measurementswere 1.36±0.28, 0.72±0.08, 3.87±1.08 respectively; the right value of MCA Pİ, Rİ, SD were 1.44±0.35, 0.74± 0.08, 4.18±1.20 respectively; the left value of MCA Pİ, Rİ, SD were 1.46±0.35, 0.74± 0.08, 4.26 ±1.34 respectively. Endometriosis of the uterine cervix is a rare lesion that is generally asymptomatic in gynaecological practice. We present a case with postcoital bleeding due to a cervical mass mimicking cervical polyp or fibroma which was histologically proven as cervical endometriosis later. Abstract

Objectives To evaluate the strength of association between maternal and pregnancy characteristics and the risk of adverse perinatal outcomes in pregnancies with laboratory confirmed COVID-19. Methods Secondary analysis of a multinational, cohort study on all consecutive pregnant women with laboratory-confirmed COVID-19 from February 1, 2020 to April 30, 2020 from 73 centers from 22 different countries. A confirmed case of COVID-19 was defined as a positive result on real-time reverse-transcriptase-polymerase-chain-reaction (RT-PCR) assay of nasal and pharyngeal swab specimens. The primary outcome was a composite adverse fetal outcome, defined as the presence of either abortion (pregnancy loss before 22 weeks of gestations), stillbirth (intrauterine fetal death after 22 weeks of gestation), neonatal death (death of a live-born infant within the first 28 days of life), and perinatal death (either stillbirth or neonatal death). Logistic regression analysis was performed to evaluate parameters independently associated with the primary outcome. Logistic regression was reported as odds ratio (OR) with 95% confidence interval (CI). Results Mean gestational age at diagnosis was 30.6±9.5 weeks, with 8.0% of women being diagnosed in the first, 22.2% in the second and 69.8% in the third trimester of pregnancy. There were six miscarriage (2.3%), six intrauterine device (IUD) (2.3) and 5 (2.0%) neonatal deaths, with an overall rate of perinatal death of 4.2% (11/265), thus resulting into 17 cases experiencing and 226 not experiencing composite adverse fetal outcome. Neither stillbirths nor neonatal deaths had congenital anomalies found at antenatal or postnatal evaluation. Furthermore, none of the cases experiencing IUD had signs of impending demise at arterial or venous Doppler. Neonatal deaths were all considered as prematurity-related adverse events. Of the 250 live-born neonates, one (0.4%) was found positive at RT-PCR pharyngeal swabs performed after delivery. The mother was tested positive during the third trimester of pregnancy. The newborn was asymptomatic and had negative RT-PCR test after 14 days of life. At logistic regression analysis, gestational age at diagnosis (OR: 0.85, 95% CI 0.8–0.9 per week increase; p Conclusions Early gestational age at infection, maternal ventilatory supports and low birthweight are the main determinants of adverse perinatal outcomes in fetuses with maternal COVID-19 infection. Conversely, the risk of vertical transmission seems negligible.

Abstract

WOS:000613461600006 PubMed ID: 32926494 Objectives To evaluate the maternal and perinatal outcomes of pregnancies affected by SARS-CoV-2 infection. Methods This was a multinational retrospective cohort study including women with a singleton pregnancy and laboratory-confirmed SARS-CoV-2 infection, conducted in 72 centers in 22 different countries in Europe, the USA, South America, Asia and Australia, between 1 February 2020 and 30 April 2020. Confirmed SARS-CoV-2 infection was defined as a positive result on real-time reverse-transcription polymerase chain reaction (RT-PCR) assay of nasopharyngeal swab specimens. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit (ICU), use of mechanical ventilation and death. Results In total, 388 women with a singleton pregnancy tested positive for SARS-CoV-2 on RT-PCR of a nasopharyngeal swab and were included in the study. Composite adverse maternal outcome was observed in 47/388 (12.1%) women; 43 (11.1%) women were admitted to the ICU, 36 (9.3%) required mechanical ventilation and three (0.8%) died. Of the 388 women included in the study, 122 (31.4%) were still pregnant at the time of data analysis. Among the other 266 women, six (19.4% of the 31 women with first-trimester infection) had miscarriage, three (1.1%) had termination of pregnancy, six (2.3%) had stillbirth and 251 (94.4%) delivered a liveborn infant. The rate of preterm birth before 37 weeks' gestation was 26.3% (70/266). Of the 251 liveborn infants, 69/251(27.5%) were admitted to the neonatal ICU, and there were five (2.0%) neonatal deaths. The overall rate of perinatal death was 4.1% (11/266). Only one (1/251, 0.4%) infant, born to a mother who tested positive during the third trimester, was found to be positive for SARS-CoV-2 on RT-PCR. Conclusions SARS-CoV-2 infection in pregnant women is associated with a 0.8% rate of maternal mortality, but an 11.1% rate of admission to the ICU. 13. Is there a relationship between the psychological state of infertile patient and ovarian reserve indicators?

Abstract

Objective: This study explored the relationship between reduced ovarian reserve and the psychological state of infertile women., Materials and Methods: This cross-sectional, single-center study was conducted with 106 infertile women. The Beck Depression Inventory (BDI) was used to assess patients' propensity for depression. The data relating to infertility, such as causes of infertility, type of infertility (primary or secondary), duration of infertility, and treatment status [previous assisted reproductive technologies (ART) treatment and ART treatment failure] were recorded for each patient. The ovarian reserve was determined using laboratory tests [anti-Mullerian hormone (AMH); follicle-stimulating hormone (FSH)] and transvaginal ultrasonography to measure the antral follicle count (AFC) in each ovary., Results: There was no significant relationship between the total score obtained from the Beck depression scale and AFC, AMH, thyroid-stimulating hormone, FSH, estradiol, and prolactin measurements (p>0.05). There was no significant difference between the groups regarding depression levels based on the cause of infertility (p=0.412). Additionally, the type of infertility (primary, secondary) did not differ between the groups (p=0.586). 14. Levonorgestrel-releasing intrauterine device to treat abnormal uterine bleeding; not one treatment option fits all.

Abstract

Objective: Initially, medical treatment options are preferred in patients with abnormal uterine bleeding (AUB) who are hemodynamically stable. The aim of the present study was to investigate the effectiveness of a levonorgestrel-releasing intrauterine device (LNG-IUD) in reducing bleeding symptoms in patients with AUB stratified by underlying pathology., Material and Methods: In line with the polyp, adenomyosis, leiomyoma, malignancy (and hyperplasia), coagulopathy, ovulatory disorders, endometrial, iatrogenic and not otherwise classified classification system, patients who were administered LNG-IUD due to adenomyosis, endometrial hyperplasia, leiomyoma and AUB due to not otherwise classified causes were included in the study., Results: A total of 172 otherwise patients with a mean age of 42.58±5.00 years were included. The distributions in the adenomyosis, endometrial hyperplasia, leiomyoma and otherwise unclassified groups were 30.8%, 12.8%, 26.2%, and 30.2%, respectively. Overall effectiveness of LNG-IUD in reducing menstrual bleeding was 82%. The proportion whose bleeding decreased was 95.50% in the endometrial hyperplasia group, 88.70% in the adenomyosis group, 55.60% in the leiomyoma group and 92.30% in the not otherwise classified group. The power of the current study was 99%. The efficacy of LNG-IUD was significantly less in the leiomyoma group (p<0.05) and thus this group were more likely to require surgical intervention. The overall incidence of spotting was 50%. 15. Endometrial histopathology results and evaluation of endometrial cancer risk in geriatric women.

Abstract

The incidence of premalignant and malignant endometrial disorders increases during the postmenopausal period. In the literature, endometrial disorders are usually discussed in the context of menopausal status. But there are limited data regarding endometrial disorders in geriatric patients. Early diagnosis of endometrial cancers with aggressive behaviour that increases during the geriatric period may allow simpler treatment options and also decrease the treatment-associated morbidity risk. Records of geriatric patients who underwent an endometrial histopathological evaluation between 2011 and 2016 were evaluated. Clinical findings, transvaginal ultrasonography findings, endometrial sampling methods, and histopathological results were evaluated. A total of 188 patients were included in the study (mean age 70.3 ±5.6 years). The most common histopathological results were endometrial polyp, atrophic endometrium, and surface epithelium (26.6%, 22.3%, and 12.8%, respectively). None of the 57 patients without vaginal bleeding had endometrial cancer. In 131 patients with vaginal bleeding, mean endometrial thickness was 9.8 ±8.1 mm (2-49 mm) and the rate of endometrial disorders was 56.5% (74 patients). Endometrial cancer was diagnosed in 19 patients (10.1%), and 36.8% of them had non-endometrioid cancers. The presence of vaginal bleeding was significantly associated with the diagnosis of endometrial cancer and any endometrial disorder ( p = 0.001 and p = 0.000, respectively). The incidence of non-endometrioid endometrial cancers increased in the geriatric period. An endometrial histopathological examination should be considered, especially for patients with a history of vaginal bleeding. Further investigation of the endometrial thickness cut-off levels in the geriatric period will contribute to the literature., Competing Interests: Ethical approval for the present study was not required because it was retrospectively performed.
