Amaç: Uterin papiller seröz karsinom hastalarında preoperatif serum CA-125 düzeylerini hastalığın yayılımını predikte etmek için araştırmak.Materyal ve metod: Uterin papiller seröz karsinom /mix seröz karsinom ve endometrioid karsinom için opere edilen hastalarda; preoperatif CA-125 seviyelerinin yanı sıra yaş, bildirilen tümör boyutu, lenf nodu tutulumu, lenfovasküler boşluk invazyonu, myometriyal invazyon derinliği ve evre gibi verileri topladık.Bulgular: Yirmi altı Uterin papiller seröz karsinom /mix seröz karsinom olgusu ve kırk beş endometrioid karsinom olgusu analizimize dahil edildi. Uterin papiller seröz karsinom /mix seröz karsinom ve endometrioid karsinom hastalarının ortalama yaşları sırasıyla 63.9 ve 58 idi. Seröz kanser grubunu CA-125 açısından incelediğimizde; CA-125 ile yaş (≤60 vs >60), tümör boyutu (≤40 vs >40mm), myometriyal invazyon derinliği, nod metastazı veya lenfovasküler boşluk invazyonu arasında hiçbir ilişki yoktu. Öte yandan, ortalama CA-125 seviyeleri, FIGO (Uluslararası Jinekoloji ve Obstetrik Federasyonu) Evre III/IV (medyan=54 U/ml) olan hastalarda FIGO evre I/II'ye (medyan=11 U/mL) anlamlı olarak yüksekti (p=0,002). Uterus dışına yayılan tümörler için ortalama CA-125 düzeyi 281,2 U/ml ve uterusta sınırlı tümörler için 14,3 U/ml idi (p=0,002). Ekstrauterin yayılmayı tahmin etmek için optimum eşik değeri 28.5 U/mL idi (duyarlılık%75, özgüllük%89). Preoperatif CA-125 düzeyi, ekstrauterin tutulum ve ileri evre ile ilişkilendirildi.Sonuç: Preoperatif CA-125 düzeyi, ekstrauterin tutulum ve ileri evre için iyi bir prediktördür. Cerrahi prosedürlerin planlanmasında, 28,5 U/mL'den daha düşük CA-125 değerine sahip Uterin papiller seröz karsinom vakalarına daha az agresif bir tedavi yöntemi öneriyoruz., Aim: To investigate the CA-125 levels in uterine serous carcinoma (UPSC) patients to predict the extent of disease.Materials and Method: Medical records of patients operated for UPSC/mix serous carcinoma, endometrioid carcinoma; their preoperative CA-125 levels, as well as parameters including age, tumor size, lymph node involvement, lymphovascular space invasion (LVSI), depth of myometrial invasion (MI) and stage were documented.Results: Twenty-six UPSC/mix serous carcinoma cases and forty-five endometrioid carcinoma cases were included. When we analyzed the serous cancer group in terms of CA-125; there was no association between CA-125 and age (≤60 vs >60), tumor size (≤40 vs >40mm), MI, lymph node metastasis or LVSI. The mean CA-125 levels were significantly higher among patients with stage III/IV (median=54 U/ml), compared to stage I/II (median=11 U/mL). The mean CA-125 value for tumors spread beyond the uterus were 281.2 U/ml, and 14.3 U/ml for the tumors confined within the uterus (p=0.002). The optimum threshold value to predict extrauterine spread was 28.5 U/mL. CA-125 level was related to extrauterine involvement and advanced stage.Conclusion: CA-125 level is a useful test for extrauterine involvement and advanced stage. We suggest a less aggressive treatment modality with CA-125

We aimed to report a case of conservatively managed recurrent bilateral theca lutein cysts associated with a singleton pregnancy. A 31-year-old woman with a singleton pregnancy, bilaterally multicystic adnexal mass and within the normal beta-hCG range was referred at 13-week pregnancy. She had a history of bilaterally enlarged ovaries up to 18 cm in her first pregnancy. Magnetic resonance imaging showed 112x142x136 mm right ovary and 197x118x242 mm left ovary at 21-week pregnancy. The right multicystic ovary resolved spontaneously during the third trimester. Thin-walled multicystic left ovarian cyst with clear serous fluid content without a solid component measured about 25x15x20 cm in size during cesarean section. Left ovarian cystectomy was performed. At the final pathology, normal morphological findings were detected in the left ovarian cyst. The follow up could be a reasonable choice in women with enlarged multicystic ovaries without malignant signs such as papillary, complex and/or solid structure., Tekil bir gebelikle ilgili tekrarlayan ve konservatif yaklaşım gösterilen tekrarlayan bir bilateral teka lutein kisti vakasını sunmyaı amaçladık. 31 yaşında 13 haftalık gebeliği olan ve gebeliğe göre normal beta-hCG değerine sahip hasta bilateral multikistik adneksial kitle sebebiyle refere edildi. Manyetik rezonans görüntüleme 21. haftada sağ overin 112x142x136 mm olduğunu ve sol overin de 197x118x242 mm olduğunu gösterdi. Sağdaki multikistik yapı üçüncü trimesterde spontan resolve oldu. Sezaryen sırasında gözlenen sol overdeki solid komponent içermeyen ince duvarlı seröz içeriğe sahip multikistik yapı yaklaşık 25x15x20 cm boyutunda idi. Sol ovarian kistektomi yapıldı. Nihai patolojide sol ovarian kistte normal morfolojik bulgular izlendi. Papillarite, kompleks yapı veya solid yapılar gibi malign bulguların olmadığı kadınlarda takip mantıklı bir seçenek olabilir.

Aim Recurrent pregnancy loss (RPL) poses a challenge in reproductive medicine because the etiology is often unknown. Here we investigated the frequency of mutations in the Factor V Leiden (FVL), prothrombin (FII), and methylene tetrahydrofolate reductase (MTHFR) genes in women with RPL and healthy women. Methods Blood samples were obtained from patients with ≥2 consecutive pregnancy losses and no identifiable etiology before 12 weeks of pregnancy (n=145). The control group comprised 105 age-matched women with ≥2 live births. Results The frequency of homozygotes for FVL 1691AA was 15 (10.3%) in patients and three (2.86%) in controls (p=0.073), while for FII 20210AA it was eight (5.5%) and one (0.9%), respectively (p=0.055). For two polymorphisms in MTHFR, genotype frequencies of 89 (61.4%) were found in patients and 55 (52.4%) in controls for 677TT (p=0.322), and 89 (61.4%) and 62 (59%) for 1298CC, respectively (p=0.810). Conclusion Despite a trend towards significance for FII G20210A, no significant differences in genotype frequencies of these polymorphisms between patients and controls was found. No evidence of the role of FVL G1691A, MTHFR C677T, and MTHFR A1298C in RPL in our Turkish cohort was found; however, further investigation of FII as a culprit gene in RPL is warranted.

Background Endometriosis is a complex disease with a spectrum of pain symptoms from mild dysmenorrhea to debilitating pelvic pain. There is no concrete evidence in the literature whether endometriotic cyst per se, causes pain spectrum related to the disease. The aim of the present study was to evaluate the effect of surgical removal of endome- triomas on pain symptoms. Materials and Methods In this prospective, observational, before-after study, which was conducted between March 2012 and January 2013 in Training and Research Hospital,Adana, Turkey, a total of 23 patients including 16 sexually active and 7 vir- gin symptomatic women were questioned for non-cyclic pelvic pain (NCPP), intensity of the NCPP, presence of cyclic dysmenorrhea, and dyspareunia before and after the endometrioma operation. Participants who were sonographically diagnosed and later pathologically confirmed as having endometrioma without sign and symptoms of deep infiltrative endometriosis (DIE) were also questioned for pain symptoms before and after the laparoscopic removal of cyst wall. Patients with intraabdominal adhesions, history of pelvic inflammatory disease, and pathological diagnosis other than endometrioma were excluded. No ancillary procedures were applied for pain management, but if pain was present, pelvic peritoneal endometriotic lesions were ablated beside the removal of ovar- ian endometriotic cysts. Results Out of 23 cases with endometrioma, 91 and 78% reported to have NCPP and dysmenorrhea, respectively, before the operation, while 60 and 48%, respec- tively, after the operation (McNemar’s test, P=0.016 for both figures). Among the sexually active cases, 31% (5/16) had dyspareunia before the operation and only 1 case reported the pain relief after the operation (McNemar’s test, P=1). Intensity of NCPP were reported to be none (8.7%), moderate (21.7%), severe (56.5%) and un- bearable (13%) before the operation and decreased to none (43.5%), mild (43.5%), moderate (4.3%) and severe (8.7%) after the operation (Wilcoxon signed-rank test, P < 0.001). Conclusion In symptomatic cases with ovarian endometrioma, without sign and symptoms of DIE, laparoscopic removal of the cysts with/without ablation of the peritoneal endometriotic lesions yields relief of NCPP and cyclic dysmenorrhea, but not dyspareunia.

Üreter hasarı, jinekolojik cerrahilerde nadir görülen ancak korkulan bir komplikasyondur ve tamiri için sıklıkla laparotomiye ihtiyaç duyulur. Yakın zamanda, jinekolojik üreter hasarının laparoskopik tamiri ile ilgili cesaretlendirici birkaç makale yayınlanmıştır. Amacımız total laparoskopik histerektomide (TLH) hasarlanan ve laparoskopik olarak başarılı bir şekilde tamir edilen iki olguyu sunmaktır. İlk olguda, sağ kardinal ligament (KL) hizasına kadar sorunsuz devam eden operasyonda, KL diseksiyonu sırasında üreterovezikal bileşkenin (UVB) 7-8 cm distalinde üreter hasarı oluştu. İkinci olguda, UVB'nin yaklaşık 10 cm distalinden üreter hasarlandı. Her iki üreter hasarı da cerrahi sırasında fark edildi. Hasarlanan üreterler laparotomiye dönmeden ve ek trokar girişi yapılmadan sorunsuz bir şekilde onarıldı. Her iki olguda da mid-üretral hasar oluştuğundan üreteroüreterostomi tercih edildi. Üreter hasarı her ne kadar TLH sırasında nadir gelişen bir komplikasyon olsa da, eğer mümkünse, laparoskopik sütür ve düğüm teknikleri konusunda deneyimli bir jinekolog veya ürolog tarafından, laparoskopik olarak tamir edilmelidir. Ureter injuries are uncommon but dreaded complications in gynecologic surgery and a frequent cause of conversion to laparotomy. Recently, a few papers reported the repair of gynecologic ureteral injuries using laparoscopy with encouraging results. In these case reports, we aimed to present two laparoscopically repaired ureter injuries during total laparoscopic hysterectomies (TLH). In the first case, the ureter was transected during the dissection of the cardinal ligament, approximately 7 to 8 cm distal to the ureterovesical junction (UVJ), and in the second case, it was damaged approximately 10 cm distal to the UVJ. Both transections were identified during surgery. The injured ureter was repaired without converting to laparotomy or additional trocar insertion. Ureteroureterostomy was performed in both cases uneventfully. Although ureteric injury is a rare complication during TLH, it can be managed by the same surgeon laparoscopically during the same procedure.

Amaç: Kadından erkeğe cinsiyet değiştiren kişilerin uterus donörü olmaya uygunluklarını histolojik, cerrahi ve sosyal açıdan değerlendirmektir.Gereç ve Yöntemler: Bu prospektif kohort çalışmada, 31 kişiye kadından erkeğe cinsiyet değişimi için laparoskopik histerektomi ve bilateral salpingoooferektomi uygulandı. Bu kişilerin pelvik viserası ve çıkarılan uterusların histolojileri üreme potansiyeli açısından değerlendirildi. Operasyon sonrası uterus donörü olmaya dair tutumlarına dair bir anket uygulandı. Bulgular: Kişilerin ortalama ± standart sapma yaşı 28,5±5 yıldır. Ortalama testosteron desteği alma süresi 2,4 yıl iken destek süresince hastaların tümünün menstrüel periyotları düzensizdi. Herhangi bir kronik hastalık veya geçirilmiş abdominal cerrahileri yoktu. Çıkarılan uterus volümlerinin ortalaması 138±48 cm3'tür. Herhangi bir adenomyozis, endometriozis, polip, adezyon veya uterus anomalisi gözlemlenmedi veya raporlanmadı. Endometrial histolojileri proliferatif (%58), atrofik (%29) ve sekretuar (%13) paternde rapor edildi. Otuz bir kişinin 30'u (%96,7) uterus donörü olma yönünde pozitif bir tutum bildirirken, bir kişi herhangi bir fikri olmadığını söyledi. Bu çalışma ile kadından erkeğe cinsiyet değiştiren kişilerin sosyal, hukuki ve biyolojik açıdan donör olmaya uygun adaylar olduğu düşünülebilir Objective: To evaluate the eligibility of female-to-male (FtM) transgender people as donor candidates with regard to histologic, surgical, and social aspects.Materials and Methods: In this prospective cohort study, 31 FtM transgender people underwent standard hysterectomy and bilateral salpingooophorectomy for gender reassignment upon their request. The pelvic viscera of the transgender people was intraoperatively observed and the histology of the removed uteri were evaluated for fertility capacity and procurement surgery. A questionnaire was administered to explore their attitude towards uterus donation.Results: The mean ± standard deviation age was 28.5±5 years. The median duration of testosterone supplementation was 2.4 years; therefore, they all had irregular menstrual periods during this therapy. None had any previous abdominal surgery or additional morbidity. The mean uterine volume was 138±48 cm3. No adenomyosis, endometriosis, polyps, adhesions or uterine anomalies were either observed or reported. Endometrial histology was reported as proliferative (58%), atrophic (29%), and secretory (13%) pattern. Of the 31 transgender people, 30 (96.7%) had a positive attitude; only one had no opinion at the beginning. After detailed information about the procedure was given, 26 (84%) still wanted to volunteer for donation, but 4 (12%) changed their opinion to negative (p=0.12, McNemar test). Conclusion: The proposal of the FtM transgender population as uterus donor is a hypothetical model that has not been experienced before. Drain placement remains controversial and there has been nearly no prospective studies evaluating the effectiveness on the gynecologic oncology cases. While some surgeons put on drains most of the cases after surgery, the literature search on general surgery and gynecologic cases suggest that the benefits are questionable. The aim is to assess the effectiveness of drain placement in patients who have underwent gynecologic oncology surgery for benign or malign diseases. Quasi randomized 144 patients were divided into two groups; 75 of them were placed drainage (D) and 89 of them without drainage (WD).The subsequent parameters; operation time, BMI and age were recorded and mobilization time, oral intake and gas discharge after surgery were evaluated. Amaç: Levonorgestrel salan rahimiçi sistemin, östrojen replasman tedavisi altındaki ovariektomize sıçanların endometriumu üzerindeki etkilerini araştırmak. Yöntem: Yirmidört adet Sprague-Dawley sıçanı 4 gruba bölünerek, levonorgestrel salan intrauterin sistemin (3 µg/day) ve plasebo-içeren rahimiçi aracın (RİA) yerleştirilmesi için operasyona alındı. Operasyonu takiben, sıçanlar 30 gün süreyle östrojen (0.01 mg./kg.) ve plasebo replasman tedavisi almak üzere gruplara randomize edildi. Çalışmanın sonunda deneklerden endometrial örnekleme yapıldı. Alınan endometrial dokular morfolojik kriterler açısından incelenmek üzere ışık mikroskopisi ile incelendi. Grup I levonorgestrel salan RİA ve sistemik östrojen alan, Grup II levonorgestrel salan RİA ve plasebo alan, Grup III plasebo içeren RİA ve sistemik östrojen alan, Grup IV ise plasebo içeren RİA ve plasebo alan sıçanlar olarak belirlendi. Bulgular: Grup I'e ait endometrial morfoloji %50'sinde aktif olmayan endometrium, %33'ünde ödem, %16'sında atrofi olarak görülürken. Grup II'ye ait morfoloji %66'sında aktif olmayan endometrium, %33'ünde atrofi olarak saptandı. Grup III'te ise % 66'sında epitelyal hiperplazi, %16'sında ödem, %16'sında myometrial hiperplazi bulunurken, Grup IV’ ün. %83'ünde atrofi, % 16'sında ise aktif olmayan endometrium tespit edildi. Sonuç: Sistemik östrojen replasman tedavisi altındaki sıçanların endometriumlarını hiperplaziden korumada, progestagen içeren rahimiçi sistemin yeterli olduğu sonucuna ulaşılmıştır. Elde edilen veriler, sistemik östrojen alan postmenopozal kadınların endometriumlarını korumada gestagen içerem rahimiçi aracın yeterli olabileceği yönünde cesaret vericidir. ÖZETLeiomyomlar düz kas hücrelerinden oluşan en sık benign uterin tümörlerdir. Uterusta sub- mukozal, intramural ve subserozal olabildikleri gibi, servikal, broad ligament içinde ya da pedinküller içinde olabilir. Submukozal myomlar oldukça nadir görülmektedir. Bazı submu- közal myomlar pedinküllü olup servikal kanaldan vajinaya uzanıp kontraksiyonlara sebep olabilirler. Başlıca tedavi yöntemleri histerektomi ve myomektomi olmakla birlikte fertilite isteğine ve myomun büyüklüğüne göre tedavi yöntemi değişebilmektedir. Ektopik gebelik halen maternal mortalitenin önemli nedenlerinden biri olmaya devam etmektedir. Yardımla Üreme Teknikleri (YÜT)' nin yaygın ve sık kullanılması ile birlikte anormal yerleşimli gebeliklerin sıklığı da artmaktadır. Bu yazıda 39 yaşında sekonder infertil ve bilateral salpenjektomili hastaya uygulanan intrasitoplazmik sperm injeksiyonu embryo transferi sonrası sağ tubal stump ta meydana gelen ektopik gebelik vakasını sunduk. Maternal morbidite ve mortaliteyi azaltmak amacıyla erken tanı için İVF sikluslarında bilateral salpenjektomi yapılmış olsa da intrauterin gebelik izlenmediği durumlarda ayırıcı tanıda mutlaka stumpta ektopik gebelik düşünülmelidir., Ectopic pregnancy continues to be one of the important reasons for maternal mortality. With the widespread and frequent use of Assisted Reproductive Techniques (ART), the frequency of abnormally located pregnancies also rises. In this article, we presented a case of an ectopic pregnancy occurring in the right tubal stump after an intracytoplasmic sperm injection – embryo transfer performed on a 39 year-old patient with secondary infertility and bilateral salphingectomy. Although a bilateral salphingectomy was performed in IVF cycles for an early diagnosis in order to decrease maternal morbidity and mortality, an ectopic pregnancy in the stump should be definitely taken into consideration in cases where no intrauterine pregnancy is observed.

Objective: We aimed to compare hysteroscopic diagnosis of patients who were diagnosed preoperatively with the salin infusion sonography and to discuss the efficiency and accuracy of hysteroscopy.Materials and Methods: Ninety-five patients who underwent hysteroscopy procedure in Dept of Gyn&Ob, Adana Numune Teaching and Research Hospital, between October 2011 to December 2012, included in this study. Preoperative sonohysterography was performed in all patients who scheduled for hysteroscopy with different indications. Patients underwent to hysteroscopy procedure after the sonohysterographic diagnoses recorded. Afterwards the diagnostic values of these two commonly used methods compared with histological diagnosis that considered the gold standard.Results: In condition of histological diagnosis was considered the gold standard we found that: diagnostic sensitivity and specifity of sonohysterography, respectively: polyp 90.62% and 90.48%, myoma 84.21% and 100%. Diagnostic sensitivity and specifity of histerography, respectively: polyp 96.78% and 90.48%, myoma 84.21% and 100%.Conclusion: Saline infusion sonography satisfied us about the focal lesions but in our routine practice hysterescopic approach is gone to widespreading and essential tool for the diagnosis and treatment of endometrial cavity lesions.Keywords: Hysteroscopy, salin infusion sonography, endometrial pathology, Amaç: Bu çalışmada, preoperatif dönemde salin infüzyon sonografisi (SİS) ile tanı konulan hastaların histeroskopik tanılarının karşılaştırılması ve histeroskopinin etkinlik ve güvenilirliğinin belirlenmesi amaçlanmıştır.Gereç ve Yöntem: Bu çalışma Eğitim ve Araştırma Hastanesi kadın hastalıkları ve doğum kliniğinde Ekim 2011- Aralık 2012 tarihleri arasında histeroskopi yapılan 95 hasta ile yapılmışır. Farklı nedenlerden dolayı histeroskopi planlanan tüm hastalara preoperatif SİS yapıldı. Daha sonra bu iki yöntemin tanısal değerleri karşılaştırıldı.Bulgular: Histolojik tanı altın standart olarak kabul edildiğinde polip tanısında SİS’in %90.62 sensitivite, %90.48 spesifite, myom tanısında %84.21 sensitivite, %100 spesifiteye sahip olduğu izlenmiştir. Histeroskopinin polip tanısında %96.87 sensitivite, %90.48 spesifite, myom tanısında ise %84.21 sensitivite, %100 spesifiteye sahip olduğu saptanmıştır. Sonuç: SİS fokal lezyonların tanısında oldukça doyurucu etkinliğe sahip olsa da histeroskopinin endomatrial kavite patolojilerinde giderek yaygınlaşan ve vazgeçilmez bir tanı ve tedavi yöntemi haline geldiğini görmekteyiz.Anahtar Kelimeler: Salin infüzyon sonografi, histeroskopi, endometrial patoloji

Tekrarlayan gebelik kayıpları (TGK) reprodüktif çağdaki kadınların yaklaşık %0.5-1'ini etkileyen önemli bir obstetrik problemdir. Etiyolojide birçok hastalık sorumlu tutulsa da vakaların %50-70 inde herhangi bir neden ortaya konulamamaktadır. Açıklanamayan TGK olgularında psikolojik destek verilmesi, haftalık ultrasonlarla yakın izlem ile ilgili yapılan çalışmalarda gebelik kaybı 2-4 kez daha azalmış olarak bildirilmiştir. Bu çalışmada yedi TGK ve multipl risk faktörleri olan bir olgunun başarılı gebeliği sunulmuştur., Recurrent pregnancy loss (RPL) is an important obstetric problem affecting about 0.5-1% of women in their reproductive period. Although many etiologic factors have been thought to be responsible, in 50-70% of cases no definitive factors can be found. In studies that assess unexplained RPL cases, providing psyhological support and close follow-up with weekly ultrasonography have been reported to decrease pregnancy losses up to 2-4 times. In this study a successful pregnancy following recurrent seven pregnancy losses in a woman with multiple risk factors is presented.

Amaç: Bu çalışma kontraseptif etki gösteren vajinal halkanın, kadınlarda kabul görme oranı, yöntem kullanımına devamlılık ve yönteme bağlı yan etkileri ortaya koymak amacıyla planlanmıştır. Metod: Araştırma Adana iline bağlı üç hastanenin Aile Planlaması Ünitesine başvuran, 18-36 yaş aralığında, hormonal kontrasepsiyon istemi olan, 141 kadın ile tamamlanmıştır. Veriler Şubat-Aralık 2011 tarihleri arasında, araştırmacılar tarafından hazırlanan sosyo-demografik özellikler ve vajinal halka kullanımına yönelik soruları içeren veri formu ile toplanmıştır. Danışmanlık sonrası vajinal halka kullanmayı kabul eden kadınlarla, kendileri temin etmek şartıyla, çalışmaya devam edilmiş ve kadınlar telefonla ayda bir kez olmak üzere 2 ay takip edilmiştir. Veriler; yüzdelik, aritmetik ortalama ve standart sapma ile değerlendirilmiştir.Bulgular: Kadınların yaş ortalamasının 29.17±6.43 olduğu, %99.3'ünün evli olduğu bulunmuştur. Kadınların %68.8'inin daha önce başka bir kontraseptif yöntem kullandığı, %51.8'inin eşlerinin herhangi bir korunma yöntemi kullanmadığı ve % 11.4'ünün eşlerinin haberi olmadan korunacağı saptanmıştır. Kadınların tamamının vajinal halkayı duymadıkları belirlenmiştir. Kadınların %27.7'sinin vajinal halka kullanmayı kabul ettiği, bunların % 43.8'i kendi başına uygulanabilir olması nedeniyle kabul ettiği belirlenmiştir. Kadınların %54.9'u kullanım zorluğu, %37.8'i eşinin tedirgin olabileceğini düşündüğü için kullanmayı kabul etmediklerini ifade etmiştir Görüşmeler sonunda kadınların tamamının vajinal halka kullanmaya devam edeceği, tamamının hal-kaya yönelik vajende batma hissi yaşamadıkları, %89.3'ünün ilişki esnasında hissetmediği, %10.7'si-nin eşi tarafından hissedildiği, %96.6'sının ilişki sık-lığının değişmediği saptanmıştır.Karar: Araştırmaya katılan kadınların vajinal halka konusundaki farkındalık oranlarının düşük olduğu ancak kullanan kadınların bu yöntemden memnun oldukları saptanmıştır. Objective: The purpose of this study was to determine utilization rate of contraceptive vaginal ring, depending on the method of continuity and the side effects of the use of the method.Material and methods: The research was performed the ages of 18-36 women who request contraseptive methods in the three hospitals family planning unit. The data collected through the form of data, including questions on the use of the vaginal ring and socio-demographic characteristics between the February to December 2011.Results: The average age of the women was 29.17 ± 6:43 and 99.3% of women were married. The study revealed that, 68.8% of women with the already used by another method, 51.8% of the women's husbands did not use any contraceptive method, 11.4% of women used the method without the knowledge of her husband. All the women were heard nothing about the vaginal ring previously. 27.7% of the women agreed to use the vaginal ring, 43.8% of women were accepted due to be applied on its own. 54.9% of women in difficulty of use did not agree to use, 37.8% of these women said that her husband might be anxious. As a result, all women will continue to use the vaginal ring was found, all the women never felt stinging in the vagina, 89.3% of women did not feel during intercourse, 10.7% of women's husband felt vaginal ring and the frequency of intercourse was not change 96.6% of women.Conclusion: The low rate of awareness of the vaginal ring has been found among women, however, the women were satisfied with this method

Olgu 40 yaşında G9 P 4 canlı dağumu olan, düzensiz vaginal kanama şikayeti ile kliniğimize başvuran gerekli kan, biyokimya, görüntüleme ve pc sonucu maliğn mikst müllerian tümör tanısı ile cerrahi esnasında stage lemeye göre (Stage Ib) Tıp 3 radikal histerektomi, pelvik paraaortik lenf adenektomi, appendektomi, omentektomi batın yıkaması yapıldı. Piyeste immüno histokimyasal boyama çalışmaları (vimentin, SMA, östrajen, progesteron, LCAHMb-45 ve S-100) yapıldı. Uterus didelphys with blind hemivagina and ipsilateral renal agenesis is a very rare anomaly and may be found in a female with normal menstrual periods.We report two cases with a double uterus, unilateral vaginal obstruction and ipsilateral renal agenesis. Both of the patients were virgin and their common clinical presentation was that of the onset of pelvic pain and dysmenorrhea, in association with the presence of a pelvic mass. In both of the patients we accurate the diagnosis by the laparotomic approach. A greater awareness of this syndrome should lead to accurate diagnosis and excision of the obstructing vaginal septum offers a complete relief of symptoms while preserving reproductive capacity., Blind hemivajina ile birlikte görülen uterus didelfis ve ipsilateral renal agenezi normal menstruel siklusa sahip kadınlarda görülebilen nadir bir anomalidir. Bu olgu sunumunda çift uterus, unilateral vajinal obstrüksiyon ve ipsilateral renal agenezisi olan iki olgu raporlanmıştır. Her iki olguda pelvik ağrı, dismenore ve bununla ilişki olarak pelvik kitle tespit edilmiş virgin hastalardı. Bu sendromun farkına varılması ile vajinal septumun açılması reprodüktif kapasiteyi koruyarak semptomların tamamen düzelmesini sağlar.

Giriş: Primer retroperitoneal tümörler retroperitoneal dokudan kaynaklanan nadir tümörlerdir. Bu tümörlerin geleneksel görüntüleme yöntemleri ve laboratuar teknikleri ile preoperatif tanısı zordur. Retroperitoneal tümörlerin tedavisi çoğunlukla benign patolojiye sahip olsalar da komplet cerrahi rezeksiyondur.Olgular: 2002-2005 yılları arasında, kliniğimizde preoperatif olarak adneksiyal kitle tanısı alarak opere edilen 310 olgudan 7 tanesi operasyon esnasında retroperitoneal kitle tanısı aldı (%2,25). Hastaların yaşları 28-62 arasında tespit edildi. Retroperitoneal tümör tanısı olan 7 olgunun 2 tanesi malign histolojiye sahipti. Bu tümörlerin 4 tanesinde histolojik orjin mezenkim, 3ünde ise ektoderm olarak saptandı. Bu yazıda retroperitoneal tümörlerin klinik bulguları, histopatolojik tanıları ve operasyon esnasında karşılaşılan zorluklar incelendi. Sonuç: Nadir görülse de jinekolojik cerrah retroperitoneal tümörlere hazırlıklı olmalıdır. Çoğunlukla benign olan bu tümörlerin primer tedavi yöntemi komplet rezeksiyondur. Komplet rezeksiyon kanama ve tümörün büyük damar ve kemik yapıya yapısık olması nedeniyle her zaman mümkün olamayabilir. Tümörün histolojik orjini belirlenerek malign benign ayrımına gidilmelidir., Background: Primary retroperitoneal tumors are rare with an origin from retroperitoneal tissues. As conventional imaging and laboratory techniques do not permit recognition of these tumors, preoperative diagnosis is not easy. Treatment of retroperitoneal tumors, most of which have benign pathology, is complete surgical resection.Cases: Between years 2002-2005, 7 of the 310 cases who were admitted to our clinic with a preoperative diagnosis of adnexial mass, revealed to have a primary retroperitoneal tumor intraoperatively (2,25%). Age of these patients ranged between 28-62. 2 of the 7 cases had a malign pathology, 4 of them had a mesenchymal origin histologically, in the remaining 3 patients ectoderm was the histological origin. In this manuscript, clinical findings, histopathological diagnosis of retroperitoneal tumors and pitfalls during the operations are reviewed. Conclusion: Despite its rare occurance, gynecologic surgeon should be familiar with retroperitoneal tumors. Primary treatment in these mostly benign tumors is complete resection which may not be possible in case of fixation of the tumor or bleeding. Histological origin should be determined during pathologic examination, distinction of benign and malign nature should be given.

ÖZETOOFEREKTOMİZE SIÇAN MODELİNDE LEVONORGESTREL SALGILAYAN İNTRAUTERİN ARACIN ÖSTROJEN REPLASMAN TEDAVİSİ SONRASI ENDOMETRİUM ÜZERİNE OLAN ETKİLERİNİN IŞIK MİKROSKOPİSİ VE TRANSMİSYON ELEKTRON MİKROSKOPİSİ DÜZEYİNDE ARAŞTIRILMASIPostmenopozal dönemdeki kadınlar gerekli östrojen yerine koyma tedavisi görürken endometriumlarını korumak için progesteron kullanmaktadırlar. Kullanılan bu progesteronların değişik sistemik yan etkileri mevcuttur. Bu yan etkileri en aza indirebilmek için gestagenlerin rahim içi araç yoluyla verilmesi iyi bir seçenek olarak düşünülmektedir. Ancak bu yöntemin endometrium üzerindeki etkinliği yeterince araştırılmamış bir konudur. Bu amaçla ooferektomize sıçan modelinde gestagenlerin sistemik yan etkilerinden kaçınmak için lokal intrauterin uygulamanın endometriuma olan etkileri, ışık ve transmisyon elektron mikroskopi düzeyinde araştırıldı.Yirmidört sıçan altışarlı eşit gruplara bölündü. Birinci gruba operasyonla rahim içi levonorgestrel salgılayan araç yerleştirildi ve bir ay süreyle östrojen replasman tedavisi uygulandı. İkinci gruba operasyonla rahim içi levonorgestrel salgılayan araç yerleştirildi ve bir ay süreyle plasebo tedavisi uygulandı. Üçüncü gruba operasyonla salgısı olmayan rahim içi araç yerleştirildi ve bir ay süreyle östrojen replasman tedavisi uygulandı. Dördüncü gruba operasyonla salgısı olmayan rahim içi araç yerleştirildi ve bir ay süreyle plasebo replasman tedavisi uygulandı. Tüm sıçanların bir ay sonraki ikinci operasyonlarında endometrial doku örnekleri ışık ve elektron mikroskopi takibi için ve sağ femur kemikleri mineral dansitesi ölçümü için alındı. Elde edilen bulgular istatistiksel olarak değerlendirildi.Tüm sıçanların endometriumlarında görülen morfolojik bulgular 11 değişik histopatolojik başlık altında toplandı. Lökosit infiltrasyonu en sık birinci grupta görülürken ödem ve epitelde vakuolleşme en sık ikinci grupta, keratinize skuamöz metaplazi ve myometrial hiperplazi üçüncü grupta ve atrofi bulguları dördüncü grupta izlendi. Transmisyon elektron mikroskopisinde de paralel bulgular elde edildi. Kemik mineral yoğunlukları istatistiksel olarak anlamsız düzeyde değişiklikler gösterdi.Sadece östrojen tedavisi uygulanan üçüncü grupta görülen keratinize skuamöz metaplazi ve myometrial hiperplazi bulguları sıçanlarda östrojenin tek başına olumsuz etkisi olarak yorumlandı. Bu etkinin birinci grupta lokal gestajen etkisi ile önlendiği gözlendi. Işık mikroskopisi ve elektron mikroskopisi düzeyinde yapılan bu incelemeler sonucunda; postmenopozal rahmi alınmamış kadınların gestagenlerin sistemik yan etkilerinden korunmak ve endometriumlarında etkin ve güvenilir korumayı sağlamak için sistemik estrojen replasman tedavisi alırken lokal etkili levonorgestel salgılayan rahim içi araç takılarak daha güvenli ve yararlı bir hormon replasman tedavisi almaları sağlanabileceği görüşüne varıldı. SUMMARYLIGHT AND TRANSMISSION ELECTRON MICROSCOPICAL INVESTIGATIONS ON THE EFFECT OF LEVONORGESTREL RELEASING INTRAUTERINE DEVICE ON THE ENDOMETRIUM OF OVARIECTOMIZED RATS UNDER ESTROGEN REPLACEMENT THERAPY Postmenapausal women should use progesteron in order to protect their endometrium, while taking estrogen replacement therapy. The progesterons in use have variable systemic adverse effects. In order to minimize these adverse effects the local use of gestagens by the help of intrauterine device seems to be a logical option. Nevertheless the efficacy of these usage is not properly investigated. For this purpose, the endometrial effects of local intrauterine use in the ovariectomized rats -in order to get away from the systemic adverse effects- have been investigated at the light and transmission electron microscopic levels. Twenty-four rats have been subdivided into four groups containing six rats in each. In the first group an intrauterine device -releasing levonorgestrel- has been applicated by an operation and estrogen replacement therapy has been given for a period of one month. In the second group, an intrauterine device-releasing levonorgestrel- has been applicated by an operation and placebo treatment has been given for a period of one month. In the third group, an intrauterine device -releasing nothing- has been applicated by an operation and estrogen replecament therapy has been given for one month long. In the fourth group, an intrauterine device -releasing nothing- has been applicated by an operation and placebo treatment has been given for one month long. At the end of one month by the second operation, endometrial tissue samples have been collected for light and transmission electron microscopic investigation and right femurs have been extracted for bone mineral densitometric measurements. The revealed data have been statistically analysed. All morphologic findings in the endometrial samples of all groups have been grouped under eleven different histopathological criteria. The leukocyte infiltration has been seen at most in the first group whereas edema and vacuolisation in the epithelium have been observed most commonly in the second group, keratinized squamous metaplasia and myometrial hyperplasia most commonly in the third group and atrophy most commonly in the fourth group. The parallel findings have been revealed also in the transmission electron microscopical investigation. The bone mineral densities have been evaluated as having no statistically significant variabilities.Light and electron microscopical investigations suggest that it is possible to supply a safe and benificial hormone replacement therapy to postmenopausal non-hysterectomized women; taking systemic esterogen replacement therapy by the use of locally active levonorgestrel releasing intrauterine device in order to protect them from the systemic adverse effects of gestagens and to supply the efficient and safe protection in their endometrium.
