Abstract

ABSTRACTTo acquire baseline data for a remote Pergau Hydroelectric Power Plant reservoir, a hydrological and water quality examination of feeder rivers and reservoir water was conducted in 2013. The water balance of the reservoir was determined, and gauging and sampling were carried out in dry and wet periods. Dilution gauging was used to estimate the feeder river discharge. The total water flow into the reservoir increases nearly fivefold between the dry and wet seasons, while river discharge increases two to fivefold. All of the river intakes had Class 1 NWQS water quality. The lake’s water quality was Class 1 up to the top 3 meters, but below that, at some places, the water quality deteriorated to Class II. In the dry season, the trophic status of Pergau Reservoir is eutrophic as measured by TP (59.21) and chlorophyll-a (52.36) and the TSI (SD) was 53.93 Eutrophication occurrences will cause serious limitations in water use applicability. This research contributes to the biogeographical and limnological understanding of the Pergau catchment, as well as laying the groundwork for more sophisticated hydro-ecological investigations. Anthropogenic activities, together with run-off from agricultural operations and the presence of algae, are some of the sources of contamination noted in the study. Stricter legislation, stricter enforcement of current standards, matching of non-technical and techno-social remedial actions, and education are among the recommendations made for the Pergau Reservoir’s protection.

2. Impacts of tilapia aquaculture on native fish diversity at an ecologically important reservoir

Abstract

Background The Temengor Reservoir is the second largest reservoir in Peninsular Malaysia. Located in the northwestern state of Perak, it was selected to develop a large-scale tilapia (Oreochromis niloticus) aquaculture facility within the Aquaculture Industrial Zone (AIZ) in 2008 due to its favourable environmental conditions. No record of tilapia has ever been reported in the natural waters prior to this. However, a post-establishment study recorded tilapia sightings in the natural waters of this lake. The cultured tilapia was easily recognizable with the elongated mouth and body, and long caudal fin. It is postulated that these were escapees from the floating cages that had invaded the natural waters and would negatively impact the native fish species. To test our hypothesis, we investigated the impact of the aquaculture facility on native fish diversity through a spatial design. Methods The study was focused on assessing the impact of tilapia culture at sites nearer to the AIZ vs more distant sites, the former with a greater likelihood of receiving escapees. Two major sites were chosen; within 5 km (near-cage) and within 5–15 km (far-cage) radii from the AIZ. Fish sampling was conducted using multiple mesh sizes of gill nets (3.7, 5.1, 6.5, 7.6, and 10.2 cm) deployed at the littoral zone of the sampling points. Species diversity, abundance, dietary habits, and habitat preference were investigated. Results The CPUE (individual/hour) of native fish species at the far-cage site of the AIZ Reservoir was found to be significantly higher (p < 0.05) than that at the near-cage site. Principal component analysis (PCA) based on diet and habitat preferences showed that the tilapia, O. niloticus had almost overlapping diet resources and habitat with native fish species. Conclusion We conclude that there is a correlation between the reduced catches of native species (based on CPUE) and the high presence of tilapia. Thus, appropriate actions must be implemented for strategic and effective planning in terms of native fish conservation.

Abstract

Gracilaria red algae are notable for their economic importance as agrophytes, sold as salad vegetable, and used as the base for selected food and nonalcoholic beverages. A wild population of Gracilaria exists in coastal areas of Sarawak, Malaysian Borneo, but there is only limited knowledge on species diversity and its abundance leaving the untapped economic potential of this resource. This study was carried out to determine diversity of wild Gracilaria populations in Lawas, Santubong, and Asajaya, Sarawak, using the combination of morphological character examination and 5′ region of the mitochondrial cytochrome c oxidase 1 (CO1-5P) gene analysis. Identification of the species using morphological characters revealed three species, namely, Gracilaria changii, G. blodgettii, and G. arcuata, had been collected from the sampling sites. However, based on 672 bp CO1-5P gene sequence analysis, all the three species were identified as G. blodgettii; besides, low genetic divergence values (0.17%–0.34%) were scored between samples in this study with the same species in GenBank. In the phylogenetic trees, all samples in this study group together with other G. blodgettii have high bootstrap values; thus, this species is monophyletic. This study implies that species identification of Gracilaria and other seagrass taxa which have a phenotypic plasticity problem should include the CO1-5P gene analysis as it is a reliable gene marker for species diversity assessment.

Abstract

Out of the 1000 species of freshwater fish documented in the South-East Asian Tropics, 420 species can be found in Malaysia. Based on experimental gill net studies, Perak River recorded a total of 107 fish species, which comprises of 33 families with Cyprinidae as the dominant family, with 43 species. The fluctuating number of species and species replacement in the upstream direction in this river reflects the orientation of the River Continuum Concept. Gradient changes of salinity, habitat heterogeneity, water velocity and riverbed materials are some of the factors that may contribute to the fluctuation and species replacement.

Abstract

An inventory study of fishes was carried out from June 2003 to May 2004 at Sungai Enam and Sungai Telang, which was last logged 40 years ago. In spite of the negative impacts of logging, the study recorded a total of 21 fish species in these headwaters, comprising nine families. Sungai Telang recorded 19 species with 11 species classified as “locally rare”, whereas Sungai Enam recorded 11 species with nine species classified as “locally rare”. When species from previous studies were included, the total number of species recorded in Sungai Enam and Sungai Telang was 28 and 27 species respectively. The presence of Devario regina, Neolissochilus soroides and Poropuntius smedleyi in all inventory studies conducted indicate that both headwaters are healthy, and function as sources of clean water, nutrient supplies and fish recruits for Temengor Reservoir.

Abstract

Telemetry is an emerging method to study fish ecology and a reliable tool that provides useful information for managing and conserving aquatic fauna and river habitats. However, the reliability of telemetry depends on several technical factors. Implantation of the acoustic transmitter is one of the major aspects that ensure the survivability of the animal subject when released into the wild. Studies on the technicalities involving telemetry methods are limited; therefore, this study investigated the surgical insertion of an acoustic transmitter into the peritoneal cavity of Channa lucius and Neolissochilus soroides or locally known as Bujuk and Tengas. A severity index was used to rate the appearance of surgical wounds observed on the day of release into the river. Fish mortality and complications such as bleeding were not observed in both species post-surgery. The progress and prognosis of wound healing of C. lucius were better compared to N. soroides, with generally lesser inflammation and more sutures shed. Despite visually severe inflammation on N. soroides on day-12 and day-19 post-surgery, there was a good indication that skin integuments were healing. This data suggests that C. lucius has faster wound healing abilities compared to N. soroides. All fishes implanted with the acoustic telemetry method survived up to 244 days upon release into the Tembat River, Hulu Terengganu, Malaysia, and retained the tag throughout the tracking period.

11. Incomplete recovery of [ATP] in vivo following muscular fatigue in young and older adults

Muscular fatigue (i.e., acute fall in peak power) may result in ATP loss, which is slow to recover in vivo due to the formation of inosine monophosphate (IMP). Slow recovery of ATP may alter the muscle’s ability to resist fatigue during subsequent work; this effect could be more problematic in older muscle. Based on the lower metabolic economy (ME; W·cm-3·mM ATP-1) and greater fatigue reported in older muscle during dynamic contractions, our objective was to use 31-phosphorus magnetic resonance (MR) spectroscopy to noninvasively quantify changes in intracellular energy metabolites and pH in vivo during and after maximal isotonic knee extensions, to test the hypotheses that fatigue would be greater, and [ATP] would decline more and be slower to recover in older compared with younger muscle. Nineteen young (10 female, 31±5 yr, mean+SD) and 17 older (9F, 72±4 yr) healthy adults completed 120 contractions (0.5 Hz, torque=20% of voluntary maximum) while positioned supine in a 3T MR system. Muscle fatigue: peak power for final 3 contractions/peak power for highest 3 contractions x 100. Muscle fat-free contractile volume, determined by MRI, was used to normalize peak power, yielding specific power (W·cm-3). MR spectra from the vastus lateralis muscle were recorded during rest, contractions, and 10 min of recovery. Phosphocreatine (PCr), inorganic phosphate (Pi), phosphomonoesters (PME) and ATP peaks were fit in jMRUI and relative concentrations determined assuming resting [ATP]=8.2 mM. Intracellular [IMP], pH, ATP production (ATPTOT, mM) and ME (specific power/ATPTOT) were calculated. Statistics: t-test and rmANOVA. 12. The Inter-Seasonal Phytoplankton Compositions and Density in Response to Abiotic Factors in Puah Reservoir Hulu Terengganu Hydroelectric Dam, Peninsular Malaysia

Abstract

The distribution of the phytoplankton community in different locations of Puah Reservoir, Malaysia was investigated from May to December 2019 to examine the phytoplankton compositions and density. Seven sampling locations were selected namely Terengganu Mati (P1), Limbing Besar (P2), Temba Outlet (P3), Pela-gong (P4), Sireh (P5), Centre Dam (P6) and Power Intake (P7). A total of 543 cell/mL were recorded from 35 genera in the reservoir. These identified species belonged to six major plankton groups: 12 green algae (35.29%), nine blue-green algae (26.47%), nine diatom (23.53%), two golden algae (5.88%), two filamentous (5.88%) and one flagellate (2.94%). During this period, Chlorophyta was the most abundant group (40% of the total phytoplankton), followed by Bacillariophyceae (29%), Pyrrhophyta (19%), Cyanophyta (12%) and Chrysophyceae (1%). The highest composition of phytoplankton was recorded at P7 (32%), followed by P3 (16%), P1 (14%) and P4 and P2 with the lowest (8% and 7%), respectively. The lowest density was observed during dry season (162 cell/mL) and high density during wet season (412 cell/mL). Species richness was discreetly greater in the wet season however, evenness index was ≥0.8, thereby indicating a similarity in species abundance. The water temperature, pH and dissolved oxygen correlate positively with phytoplankton at P = 0.01. The overall mean values of temperature for wet and dry season were 29.3 ± 1.79 °C and 27.5 ± 1.55 °C while dissolved oxygen (DO), biochemical oxygen demand (BOD), chemical oxygen demand (COD) and total suspended solid (TSS) were 16.2 mg/L and 4.7 mg/L, 3.4 mg/L and 2.9 mg/L, 15.2 mg/L and 12.7 mg/L, and 0.5 mg/L and 2.3 mg/L, respectively. Water transparency, pH and DO were found to be important factors characterising each sampling location.

14. Determinants of Manufacturing Firms’ Financial Performance in Pakistan

Abstract

Investment in working capital by and large shows better returns than investment in fixed assets. As such proper management of working capital rightfully attracts a lot of attention. The objective of this research is to examine the impact of size and working capital management efficiency on firms’ financial performance using 10 years (2004- 2013) secondary data of 153 firms listed on Pakistan Stock Exchange and employing regression and ratio analyses. Results show that performance-wise large firms are better whereas WCME-wise small and medium firms are better. These findings indicate that better performance of large firms is not because of efficient utilization of working capital - rather it may be due to some other factors and these firms can further improve their performance if working capital is managed more efficiently.

Abstract

BACKGROUND & AIMS: The Primary Biliary Cholangitis (PBC) Obeticholic Acid (OCA) International Study of Efficacy (POISE) randomized, double-blind, placebo-controlled trial demonstrated that OCA reduced biomarkers associated with adverse clinical outcomes (ie, alkaline phosphatase, bilirubin, aspartate aminotransferase, and alanine aminotransferase) in patients with PBC. The objective of this study was to evaluate time to first occurrence of liver transplantation or death in patients with OCA in the POISE trial and open-label extension vs comparable non-OCA-treated external controls. METHODS: Propensity scores were generated for external control patients meeting POISE eligibility criteria from 2 registry studies (Global PBC and UK-PBC) using an index date selected randomly between the first and last date (inclusive) on which eligibility criteria were met. Cox proportional hazards models weighted by inverse probability of treatment assessed time to death or liver transplantation. Additional analyses (Global PBC only) added hepatic decompensation to the composite end point and assessed efficacy in patients with or without cirrhosis. RESULTS: During the 6-year follow-up, there were 5 deaths or liver transplantations in 209 subjects in the POISE cohort (2.4%), 135 of 1381 patients in the Global PBC control (10.0%), and 281 of 2135 patients in the UK-PBC control (13.2%). The hazard ratios (HRs) for the primary outcome were 0.29 (95% CI, 0.10-0.83) for POISE vs Global PBC and 0.30 (95% CI, 0.12-0.75) for POISE vs UK-PBC. In the Global PBC study, HR was 0.20 (95% CI, 0.03-1.22) for patients with cirrhosis and 0.31 (95% CI, 0.09-1.04) for those without cirrhosis; HR was 0.42 (95% CI, 0.21-0.85) including hepatic decompensation. Abstract

Abstract Background Despite the numerous advantages of the nursing process, nursing students often struggle with utilizing this model. Therefore, studies suggest innovative teaching methods to address this issue. Teaching based on real clinical cases is considered a collaborative learning method that enhances students’ active learning for the development of critical thinking and problem-solving skills. In this method, students can acquire sufficient knowledge about patient care by accessing authentic information. Objective The aim of the present study was to investigate the experiences of nursing students and faculty members regarding the implementation of nursing process educational workshops, based on real case studies. Design A qualitative descriptive study. Participants 9 Nursing students and 7 faculty members from the Isfahan School of Nursing and Midwifery who attended the workshops. Methods This qualitative descriptive study was conducted from 2021 to 2023. Data was collected through semi-structured individual and focus group interviews using a qualitative content analysis approach for data analysis. Results After analyzing the data, a theme titled “Breaking Taboos in the Nursing Process” was identified. This theme consists of four categories: “Strengthening the Cognitive Infrastructure for Accepting the Nursing Process,” “Enhancing the Applicability of the Nursing Process,” “Assisting in Positive Professional Identity,” and “Facilitating a Self-Directed Learning Platform.” Additionally, thirteen subcategories were obtained. Conclusion The data obtained from the present study showed that conducting nursing process educational workshops, where real clinical cases are discussed, analyzed, and criticized, increases critical thinking, learning motivation, and understanding of the necessity and importance of implementing the nursing process. Therefore, it is recommended that instructors utilize this innovative and effective teaching method for instructing the nursing process.

Abstract

Background: Nursing students’ safe medication administration is a crucial aspect of ensuring patient safety. Clinical simulation is a novel teaching method applied to reduce factors threatening medication safety in the education of nursing students. Therefore, the aim of the present study was to conduct a review study in this field. Materials and Methods: The present review was conducted using keywords of education, clinical competence, medication safety, medication error, nursing student, clinical simulation, and educational strategies. Suitable articles published between 2000 and 2023 were systematically searched in various national and international online databases, including SID, Magiran, Ovid, Scopus, Web of Science, PubMed, ProQuest, and Google Scholar in both English and Persian languages. Results: A total of 855 articles were retrieved from the initial search findings. After eliminating duplicate and irrelevant articles based on predetermined criteria, a qualitative selection was conducted using CONSORT, MMAT, and JBI critical appraisal checklists. Ultimately, a selection of 11 articles was made, specifically focusing on medication safety education for nursing students through clinical simulation. The findings showed that using clinical simulation improved medication safety and safety knowledge among nursing students. Additionally, this approach was found to enhance their competence in medication administration. Conclusions: Based on the findings, clinical simulation is a safe and effective approach for enhancing nursing students’ proficiency in medication safety and administration. Therefore, it is recommended that nursing education authorities implement these findings to improve nursing students’ knowledge, performance, and competency in safely administering medications.

Abstract

The urban development planning process in Tehran municipality has been implemented for about two decades in the form of compiling four medium-term urban development plans of Tehran. Despite the efforts made, there are challenges to achieving the goals. This research has analyzed the barriers and identified solutions to improve the system of the development plan of Tehran by formulating the mess and using soft systems methodology during a systemic intervention by planning the learning process in practice. The innovative research method is the embedding of mess formulation in soft systems methodology. Formulating the mess analyses the barriers to identifying inefficiencies about aspects of how power, wealth, knowledge, value, and aesthetics are produced and distributed in a social system. Soft systems methodology is an action-research approach for learning human systems in complex problem situations. After a thematic review by referring to the key actors involved in formulating Tehran's urban development plan, first, the problem situation was analyzed, and a rich picture of the problem was drawn. According to formulating the mess, the obstacles were presented and discussed through some sessions in five subsystems of generation and distribution of power, wealth, knowledge, value, and aesthetics. Later, a conceptual model of the ideal system of urban development planning was designed. Then, desirable and executable system changes were identified based on a comparison of the ideal system and the current system, and they were proposed in the form of purposeful activities toward system improvement. Tehran's urban development planning requires a major change in the approach toward planning and solving complex urban problems. This study is an application of systems thinking approaches in learning in practice and managing the complexity of the diversity of views of urban planners in the experience of urban development planning in Tehran.

Abstract

Over the last decade, the issue of work-life balance has gained visibility internationally due to ongoing usage of ICT that creates technostress on employee. Work-life balance should be a key concept to all employers today, especially to those with an increasing number of people in the organization and due to the additional demand for work that they imply. The best work-life balance situation is when an employer can manage work timetables and schedules for some leisure activities in a way that suits the employees. Managing both work and life aspects by ensuring a good relationship between them should be the primary objective. It creates a quality of life for each individual. Therefore, work stress is undoubtedly one of the employees' most significant problems, especially involving computer technology such as mobile phone technology, car alarms, emails, calls, and voicemails. That is called technostress. Technostress is the term used to describe the stress phenomenon induced by the inability to manage, due to using computer technology. A range of literature exists that shows the relationship between technostress creator and worklife balance either based on online data, google scholar, and PhD theses. This systematic literature review had been conducted by using PICO criteria. The four elements included in the PICO used to formulating these research questions are population, intervention, comparison, and outcome. This paper presents a systematic literature review using PRISMA collection method, to provide collective evidence on their implementation and how important work-life balance is for employees. The review reports on 120 sources from 1300 articles, providing insights into the range of technostress creator and work-life balance in existence. It also highlights the benefits and challenges encountered through work-life balance policy implementation for future research.

22. K-means and agglomerative clustering for source-load mapping in distributed district heating planning

Abstract

This study introduces a high-resolution, data-driven approach for optimizing district heating networks using source-load mapping, focusing on Stockholm as a case study. The methodology integrates detailed building energy performance data (2014–2022) with geographic data from the Swedish Survey Agency, employing advanced clustering techniques such as K-means Clustering, Agglomerative Clustering, DBSCAN, Spectral Clustering, and Gaussian Mixture Model (GMM) Clustering to identify optimal locations for distributed heat sources, including data centers, supermarkets, and water bodies. Quantitative results show that these environmentally friendly sources could supply 54 % of Stockholm’s total annual heat demand of 7.7 TWh/year, equating to 4.2 TWh from residual heat sources. Data centers contribute 0.48 TWh, water bodies provide 3.4 TWh, and supermarkets contribute 0.3 TWh annually. Economic analysis further reveals that 98 % of residual heat sources are economically viable, with marginal costs of heat (MCOH) for data centers, supermarkets, and water bodies estimated at 12.7 EUR/MWh, 16.0 EUR/MWh, and 20.0 EUR/MWh, respectively—well below the Open District Heating (ODH) market price of 22.0 EUR/MWh. The policy implications of these findings are profound. Policymakers can leverage this methodology to identify economically viable heat sources, enabling the creation of regulations that incentivize the integration of distributed heat sources into existing district heating networks. This can lead to reduced energy costs, enhanced sustainability, and more resilient energy systems. Practically, urban planners and energy utilities can use clustering insights to optimize the placement of new infrastructure, such as data centers, ensuring they are strategically located in high-demand zones. Furthermore, the study’s methodology can be replicated in other urban contexts, offering cities worldwide a scalable tool for improving the efficiency and sustainability of their heating networks. These findings support the transition to low-carbon energy solutions and provide actionable recommendations for the long-term development of urban energy systems.

Abstract

An investigation study was conducted in Bukit Merah Reservoir (BMR) for the assessment of arsenic concentration in the surface sediment in 23 sampling stations. The sediment samples were digested and analysed for arsenic using Inductively Coupled Plasma-Optical Emission Spectrometry (ICP-OES). Sediment parameters such as pH (4.42 ± 0.71), redox potential (121.77 ± 42.45 mV), conductivity (205.7 ± 64.07 μS cm–1) and organic matter (25.35 ± 9.34%) were also examined. The main objectives of this study are to determine the arsenic distribution and concentration and at the same time to assess the enrichment of arsenic using the geoaccumulation index (Igeo) and enrichment factor (EF). This study shows the total arsenic concentration in the surface sediment of BMR is 4.302 ± 2.43 mg kg–1 and found to be below the threshold value of Canadian Interim Sediment Quality Guidelines (ISQG). High arsenic concentration is recorded near the southern part of the lake where anthropogenic activities are prevalent. Based on Igeo, 13% of sampling stations are categorised as moderately polluted, 52.2% as unpolluted to moderately polluted and the rest is categorised as unpolluted. EF shows 78.3% stations are classified as extremely high enrichment and the rest as very high enrichment. This finding provides important information on the status of arsenic contamination in BMR and creating awareness concerning the conservation and management of the reservoir in the future.

Abstract

Vertical and longitudinal phytoplankton distribution was conducted (February 2019 - January 2020). Transparency, pH & TSS were high at photic water but decreased gradually into aphotic except pH that was opposite. The chemical parameters were low at the surface and increased with deeper water. The physical parameters except pH were slightly lower during the wet season but conductivity and total phosphorous were slightly higher during the dry season. There were six phytoplankton groups which comprised Chlorophyta(37%), Pyrrhophyta(24%), Bacillariophyceae(20%), Cyanopyta(17%), Chrysophyta(1%) & Charophyta(1%). A total of 1,472 species were recorded from 57 genera. The mean values of phytoplankton (mg/L) in Riverine, Transitional and Lacustrine were 18.79, 13.46 & 24.68 and 2.88, 2.55 & 2.00 for photic & aphotic respectively. The ANOVA results showed significant diference between longitudinal (photic & aphotic) with regards to phytoplankton at P=0.05. This means that the higher phytoplankton density reported at photic shows more availability of nutrients and the water's stability and better health status of the Reservoir. Phytoplankton encountered in the water body reflect the average ecological condition, and therefore, can be used as an indicator of water quality.

Abstract

There has been a growing reliance on consumer generated content as a source of knowledge for hospitality product decision-making. People post their views, ratings, and reviews of products and services on such platforms, and companies and research organizations are interested in analyzing and extracting all of the comments. It is critical for hotels to take advantage of online customer review information in order to better understand their customers and enhance hotel efficiency. However, analyzing guest reviews is challenging as it contains abundance of data. Using The St Regis Kuala Lumpur as a case study, the objective of the paper is to profile the hotel online reviews using a Netnography approach, identify the hotel attributes that guest comment based on the hotel online reviews and examine the guest satisfaction attitude based on the hotel online reviews. A total of 388 reviews from TripAdvisor were collected. The findings revealed that most of the hotel guests who left comments were international tourists, and couples. Many of whom left excellent ratings for the hotel. The findings show there are nine attributes that are frequently mentioned namely, amenities, location, price, service, cleanliness, food and beverages, hotel and room environment. The results also show that guests left both positive and negative reviews in few common categories such as hotel cleanliness, hotel environment and amenities. Among all the attributes mentioned, satisfied guests tended to leave positive comments for things like hotel cleanliness, hotel and room environment, while dissatisfied guests emphasised the amenities or the lack thereof. The findings of this study make several theoretical and managerial inferences with regards to identifying hotel attributes that contribute to guest satisfaction and improves the understanding of what satisfies and dissatisfies guests from the comments left by the guests on TripAdvisor. The paper concludes with limitations and suggests future research.

Abstract

Abstract Background Dilatation of common bile duct (CBD) is mostly pathological and mainly occurs secondary to mechanical causes. We aimed to explore the prevalence of CBD dilatation in Intraductal Papillary Mucinous Neoplasms of the pancreas (IPMN) among patients referred to EUS. Methods A retrospective study of all patients who had an EUS diagnosis of IPMN from 2011 to 2019 at Galilee Medical Center were extracted. Control group including patients with other types of pancreatic cysts. Results Overall, 2400 patients were included in the study, of them 158 patients were diagnosed with pancreatic cysts, 117 patients (74%) diagnosed with IPMN (group A), and 41 patients (26%) diagnosed with other pancreatic cysts (group B). Univariate analysis showed significant association of IPMN (OR 3.8, 95% CI 1.3–11.5), resected gallbladder (GB) (OR 7.75, 95% CI 3.19–18.84), and age (OR 1, 95% CI 1.01–1.08) with CBD dilatation. Classifying IPMN to sub-groups using adjusted multivariate logistic regression analysis, only main duct-IPMN (MD-IPMN) significantly correlated with CBD dilatation compared to branch duct (BD)-IPMN and mixed type-IPMN (OR 19.6, 95% CI 4.57–83.33, OR 16.3, 95% CI 3.02–88.08). Conclusion MD-IPMN was significantly correlated with dilated CBD. Assessment of the pancreas is warranted in encountered cases of dilated CBD without obvious mechanical cause.

29. Habitat Use and Movement Activity of Neolissochilus soroides and Channa lucius during Post Inundation of Tembat Reservoir, Hulu Terengganu

Abstract

The drastic changes in hydroelectric reservoir development created a completely new ecosystem that affects the river, particularly in the inundated area. In this study, five Neolissochilus soroides and Channa lucius were surgically implanted with a unique coded acoustic transmitter to observe the habitat utilisation and movement activity in Tembat Reservoir after the inundation process. All of the individuals were released into the transition zone of the reservoir and observed using passive and active acoustic tracking devices from April to December 2018. Kruskal-Walis test showed no significant difference between the average size of core area for N. soroides and C. lucius, x2(1) = 1.320, p = 0.251. The home range also showed a similar result for N. soroides and C. lucius where there was an insignificant difference, x2(1) = 0.273, p = 0.602. However, duration wise, N. soroides spend more time in the transition zone, R1 (M = 2.71 hrs, SE = 0.38), and C. lucius in the riverine zone, R5 (M = 7.14 hrs, SE = 6.13) and R6 (M = 3.85 hours, SE = 6.36). From the active tracking survey, PCA identified three (62.32%) and four (71.19%) components with eigenvalues greater than 1 for N. soroides and C. lucius, respectively. Three most important habitat parameters for N. soroides were the size of river (r = 0.97), existence of canopy cover (r = 0.77), and substrate type (r = 0.79).While, for C. lucius were mesohabitat type (r = 0.79), distance to riffle (r = 0.97), existence of canopy cover (r = 0.90), and elevation (r = 0.90). A 24-hour movement frequency analysis for both species revealed two active hours of movement at dusk and dawn for both species. From the study, it is known that N. soroides and C. lucius respond differently towards the inundation of the reservoir. The findings in this study can be implemented for effective aquatic management and conservation plan to ensure sustainable dam development.

30. The Influence of Stream Barriers and The Hypothetical Origin of Highlands Fishes

Abstract

Instream barriers such as waterfalls and man-made dams may result in distinct fish assemblages on both sides of the barriers. To determine the influence of these barriers on fish assemblages, a study was conducted in Sungai Enam that have substantial barriers including an 18-m high waterfall with 6.1% slope. Fish were captured using a backpack electrofishing gear together with scoop nets. Thirteen fish species from six families were recorded along this stream, but only Poropuntius smedleyi (Cyprinidae) can be found along the stream and the reach above the waterfall. Studies on a second site, Sungai Kooi, that has a 65m high and 90° slope waterfall, revealed eight fish species from three families, but only P. smedleyi and Devario regina (Cyprinidae) occurred above the waterfall. The number of species in both streams increases from upstream to downstream. A chi-square test revealed that the total number of species among reaches in Sungai Enam differed significantly. Fish species found above the waterfall are known to have no special adaptations to climb or jump over the barriers. Therefore, to explain their existence above the substantial waterfalls especially in Sungai Kooi, we proposed three hypotheses: (1) the world was once flooded and there was no limitation to fish distribution, (2) fish species assemblages were separated by land uplifting, and (3) there are other stream branches enabling fish movement into respective streams. Further studies are required to confirm these hypotheses.

33. The effect of the clinical supervision model on nursing internship students’ nursing process-based performance: an experimental study

Abstract

Abstract Background The nursing process is a systematic method for identifying the patient’s problems and planning to resolve them. It is also a crucial pillar of high-quality nursing care. Nursing internship students may lack the necessary skills to implement the nursing process due to the increased independence, the absence of constant professorial supervision, and limited experience. The clinical supervision model is a method of clinical education that bridges the gap between theory and practice. Objective This study was conducted to investigate the impact of the clinical supervision model on the performance of nursing internship students in each of the five stages of the nursing process, as well as overall. Method This experimental study was conducted in 2022. The 70 eligible internship students were conveniently selected and randomly assigned to either an intervention or a control group. In the present study, the clinical supervision model was implemented for the intervention group, while the control group received routine supervision. This was carried out over six sessions in three months. The data collection was conducted using a researcher-developed checklist of nursing process-based performance in both groups. Moreover, the Manchester questionnaire was used to evaluate the model in the intervention group. The variables considered as confounding factors included age, gender, marital status, number of monthly shifts, and grades of the nursing process credit completed in the third semester. SPSS version 16 software, descriptive statistics (frequency distribution, percentage, mean, and standard deviation), and analytical statistics (independent t-test, chi square, repeated measures Anova and LSD) were used to analyze the data. Results Intergroup analysis revealed that there was no significant difference between the scores of nursing process steps and the total score before the intervention in the control and intervention groups, as well as in baseline characteristics (P > 0.05). According to the intragroup analysis, the intervention group showed a significant increase in both the total scores and scores of nursing process steps over time (P 0.05). Finally, the “P-Value Intervention” demonstrated the effectiveness of this training model in improving the performance of the intervention group based on the nursing process compared to the control group. The mean score of the Manchester questionnaire in the intervention group was 136.74, indicating the high impact of implementing the clinical supervision model in the intervention group. Conclusion The results indicated that the implementation of the clinical supervision model led to improved utilization of the nursing process by nursing internship students at all stages. Therefore, it is recommended that nurse educators utilize the clinical supervision model by providing feedback on errors in action during supervision sessions to enhance the quality of nursing care provided by nursing internship students and improve patient safety in clinical environment.

Abstract

[BACKGROUND & AIMS] Primary biliary cholangitis (PBC) is a chronic liver disease in which autoimmune destruction of the small intra-hepatic bile ducts eventually leads to cirrhosis. Many patients have inadequate response to licensed medications, motivating the search for novel therapies. Previous genome-wide association studies (GWAS) and meta-analyses (GWMA) of PBC have identified numerous risk loci for this condition, providing insight into its aetiology. We undertook the largest GWMA of PBC to date, aiming to identify additional risk loci and prioritise candidate genes for in silico drug efficacy screening. [METHODS] We combined new and existing genotype data for 10, 516 cases and 20, 772 controls from five European and two East Asian cohorts. [RESULTS] We identified 56 genome-wide significant loci (20 novel) including 46 in European, 13 in Asian, and 41 in combined cohorts; and a 57th genome-wide significant locus (also novel) in conditional analysis of the European cohorts. Candidate genes at newly identified loci include FCRL3, INAVA, PRDM1, IRF7, CCR6, CD226, and IL12RB1, each having key roles in immunity. Pathway analysis reiterated the likely importance of pattern recognition receptor and TNF signalling, Jak-STAT signalling, and differentiation of TH1 and TH17 cells in the pathogenesis of this disease. Drug efficacy screening identified several medications predicted to be therapeutic in PBC, some well-established in the treatment of other autoimmune disorders. [CONCLUSIONS] This study has identified additional risk loci for PBC, provided a hierarchy of agents that could be trialled in this condition, and emphasised the value of genetic and genomic approaches to drug discovery in complex disorders. In this study, we analysed genetic information from 10, 516 people with PBC and 20, 772 healthy individuals recruited in Canada, China, Italy, Japan, UK, or USA. We identified several genetic regions associated with PBC. Each of these regions contains several genes. For each region, we used diverse sources of evidence to help us choose the gene most likely to be involved in causing PBC. Abstract

Background: We aimed to investigate the safety of endovascular procedures undertaken in a single outpatient center located in a rural, underserved area. Endovascular procedures for Peripheral Arterial Disease (PAD) have become increasingly common in outpatient settings; their safety is yet to be determined in a rural, underserved area with no stand-by vascular surgeon on site. Methods: We undertook a retrospective case review of endovascular procedures for the investigation and management of lower extremity PAD between December 2012 and August 2015. Patients were classified by Rutherford score, degree of stenosis and length of lesions. Complications were major (requiring hospitalization) or minor, including perforation, distal embolization, hematoma, and allergic reactions, which could be treated immediately in the catheterization laboratory with no sequelae. Patients were monitored in the facility and followed up using clinical, biochemical and radiological parameters at 24 h and 1 month. Results: A total of 692 patients underwent endovascular procedures for the investigation and/or treatment of PAD, of which 608 were interventional. Of these patients, 10.20% experienced procedural complications, of which 0.66% were classified as major, including wire retention and retroperitoneal hemorrhage. In total, 99.34% were discharged safely on the same day as the procedure. No adverse events were reported at follow up. Conclusion: Endovascular procedures for PAD can be performed safely in a rural outpatient setting with low complication rates. Most complications are minor and do not require hospitalization. Outpatient procedures for PAD are safe and may widen access to specialist procedures in areas of socio-economic deprivation.

Abstract

The rapid development in social commerce creates a new opportunity for businesses. With the use of social technologies they are able to create an environment that is on a socially interactive platform. These social bonds can drive online social support in e-commerce, the result is shown by trusts created and increased intention to use social commerce. This article will examine the factors that triggered consumers’ intention to purchase using social commerce. A total of 184 respondents were chosen for this study to investigate factors that might contribute to customers’ purchasing intention. The findings provide a better understanding on factors that contribute to intention to purchase, which comprise shopping enjoyment, perceived usefulness, trust, security, perceived quality, perceived value, ease of use and transaction convenience. It also suggests that online firms should consider these contextual factors in order to facilitate consumers' adoption behavior.

Abstract

Objective: Conventional methods for anterior upper dorsal spine are not devoid of intra-operative complications due to anatomical constraints, a major blood vessel and vital organs. We analyzed the details of Trans- axillary approach without muscle cutting in tuberculosis, tumor, and traumatic lesions of T2 to T6 and upper limb causalgia.Materials and Methods: A prospective quasi-experimental study was conducted for 3 years included 30 patients presented with dorsal myelopathy due to D2 to D6 vertebral body involvement. Right and left sided approaches were utilized. Medical Research Council grading was used to assess neurological status.Results: Mean age was 32 ± 15 years, which included male 18.60% and female 12.40%. T4 vertebrae were commonly involved. Others were T5, T3 and T6 respectively. The majority of lesions were tuberculosis of the spine (n = 14), tumor (n = 8), trauma (n = 5) and causalgia of upper limb (n = 3). No intra-operative complications occurred, estimated blood was 80-100 ml and operative time was 120-160 minutes. Postoperative complications included pneumonia (n = 1), superficial wound infection (n = 3), death (n = 1). Serial follow up was for 2 years, Patients were discharged with improved neurological status and causalgia patients got immediate post-operative relief.Conclusions: Transaxillary approach is a unique anatomical corridor which is safe, feasible with promising results. It provides optimal decompression, reconstruction with better fixation and alignment in various pathologies of upper dorsal spine.

Abstract

There has been a growing reliance on consumer generated content as a source of knowledge for hospitality product decision-making. People post their views, ratings, and reviews of products and services on such platforms, and companies and research organizations are interested in analyzing and extracting all of the comments. It is critical for hotels to take advantage of online customer review information in order to better understand their customers and enhance hotel efficiency. However, analyzing guest reviews is challenging as it contains abundance of data. Using The St Regis Kuala Lumpur as a case study, the objective of the paper is to profile the hotel online reviews using a Netnography approach, identify the hotel attributes that guest comment based on the hotel online reviews and examine the guest satisfaction attitude based on the hotel online reviews. A total of 388 reviews from TripAdvisor were collected. The findings revealed that most of the hotel guests who left comments were international tourists, and couples. Many of whom left excellent ratings for the hotel. The findings show there are nine attributes that are frequently mentioned namely, amenities, location, price, service, cleanliness, food and beverages, hotel and room environment. The results also show that guests left both positive and negative reviews in few common categories such as hotel cleanliness, hotel environment and amenities. Among all the attributes mentioned, satisfied guests tended to leave positive comments for things like hotel cleanliness, hotel and room environment, while dissatisfied guests emphasised the amenities or the lack thereof. The findings of this study make several theoretical and managerial inferences with regards to identifying hotel attributes that contribute to guest satisfaction and improves the understanding of what satisfies and dissatisfies guests from the comments left by the guests on TripAdvisor. Abstract

This study is an empirical investigation into the impact of leverage deviations from the target on growth of companies listed on the Karachi Stock Exchange (KSE). It is posited that leverage variance impedes the growth of firms. The role of leverage deviations from the target on firms’ growth is analyzed by applying dynamic panel methodology of firms’ level annual accounting data. The results indicate that size of the firm, profitability, collateral values of assets, non-debt tax shield, firm-specific interest rate and spontaneous finance are significant determinants of the target leverage. It is found that firms, seldom have actual leverage equal to the target leverage, and more often the actual leverage deviate from the target level. This deviation, adversely affects the growth of firms. The empirical results indicate that upward deviation and downward deviation affect the company’s growth, differently. It is concluded that over-levered firms’ growth is more sensitive to leverage variance as compared to the under-levered firms.

Abstract

This study aims to determine the level of nine trace elements - As, Cd, Cr, Cu, Fe, Mn, Ni, Pb and Zn in liver, gill and muscle of Oreochromis niloticus in Bukit Merah Lake (BML). The concentration of trace elements was measured using Inductively Coupled Plasma-Optical Emission Spectrometers (ICP-OES). Cd and Ni were found below the detection level, while Cr was only detected in gill and muscle tissues. The Metal Pollution Index (MPI) established were liver > gill > muscle. The ranking order of trace elements in the gills was Fe > Zn > Mn > Pb > As > Cu > Cr. For the liver, the ranking order was Fe > Cu > Zn > As > Mn > Pb, while in the muscle, the ranking order was Fe > Zn > Pb > As > Cu > Mn > Cr. The estimated daily intake (EDI) for all the trace elements in this study was calculated based on 168 g.day-1 of Malaysians' fish consumption, indicating no potential risk. From the human health point of view, there was no significant non-carcinogenic risk of individual trace elements as evaluated by Target Hazard Quotient (THQ). As indicated by the Hazardous Index (HI), the cumulative effect of all trace elements also suggested no potential of noncarcinogenic risk. Abstract

There are several methods for indirectly detecting exoplanets, such as transit, radial velocity, astrometry, and the conventional gravitational microlensing approach. These methods rely on observing the effects of exoplanets on the emission or motion of observed stars. All these techniques have focused on the optical or infrared domains. However, an alternative method for exoplanet detection via microlensing events involves planets orbiting the source star, creating a binary source system. In this study, we explore a novel approach to detecting and studying exoplanets exclusively through their radio emissions resulting from magnetospheric processes. We propose utilizing the Roman telescope as a survey observer to detect microlensing events. Subsequently, we investigate the potential for detecting planetary radio signals through follow-up observations of these microlensing events in the radio band using the SKA telescope. This method is viable due to the comparable radio emission levels of exoplanets and their parent stars, unlike optical and infrared emissions. We conduct a Monte Carlo simulation to replicate the observations by the Nancy Roman Telescope, followed by a follow-up observation in radio frequencies using the SKA telescope. We determine that approximately 1,317 exoplanets exhibit detectable signals by the SKA telescope during the 7-season observations by the Nancy Roman Telescope. This result indicates that such a method cannot only facilitate the direct detection of exoplanets but also enable the measurement of their magnetic field strength through analysis of their radio emissions.

Background: Utilizing the nursing process within clinical settings serves to enhance the scientific stature of the nursing field. Nonetheless, various research findings suggest that nursing internship students encounter difficulties when it comes to implementing the nursing process and may lack the necessary proficiency. Objectives: The aim of this investigation is to identify the perceptions of nursing internship students and their faculty mentors regarding the complexities and hindrances associated with implementing the nursing process within the clinical setting. Design: A qualitative content analysis. Participants: Nursing internship students and their faculty mentors from the Isfahan School of Nursing and Midwifery. Methods: In this study, individual interviews were conducted with 13 participants. The data for this study were gathered through these semi-structured interviews and subsequently analyzed using the Granheim and Lundman method. To ensure the validity and reliability of the data, the evaluative criteria of Lincoln and Guba were employed. Results: The obstacles unveiled in this study can be categorized into three primary domains, each with its own subcategories: 1-Student role ambiguity (1-1 Lack of practical autonomy; 1–2 Insufficient proficiency in the nursing process; 1–3 Motivational deficiency). 2- Organizational Challenges (2-1 Shortage of nursing staff; 2-2 Suboptimal interpersonal dynamics); And 3- Gradual Erosion of the Nursing Process. Conclusion: Recognizing the paramount importance of the nursing process in enhancing patient care quality is universally accepted. Therefore, it is imperative to systematically identify and tackle the challenges associated with its application. This study highlights that these challenges stem from various factors, including the multifaceted roles assumed by nursing students, organizational shortcomings within healthcare institutions, and the neglect or erosion of the nursing process in specific clinical settings. Addressing these issues is crucial for ensuring the effective utilization of the nursing process within the nursing profession and for optimizing patient care outcomes.

Background: Medication errors are the most serious and common events threatening patient safety. Nursing interns make medication errors due to inexperience and inadequate supervision.Objectives: This study aimed to examine the effects of the clinical supervision model (CSM) on medication safety competency and knowledge of nursing interns.Methods: This clinical trial was conducted in 2022 on 70 nursing interns. Data were collected using the Medication Safety Critical Element Checklist, the Medication Safety Knowledge Assessment, and the Manchester Clinical Supervision Scale. The CSM and routine supervision were conducted in six sessions for the intervention and control groups, respectively. Data were analyzed using the independent t-test, paired t-test, chi-square test, and repeated measures analysis of variance.Results: Between-group analysis showed that the difference between medication safety competence and knowledge scores in the control and intervention groups was not significant at baseline (P>0.05). Abstract

This study explored cumulative 127.5MW waste to energy (WtE) potential in five populous cities of Pakistan based on local waste characterization profiles and global standards. The 50MW WtE plant in Lahore using National electricity regulator codes and practices resulted in an attractive Levelized cost of electricity (LCOE) of US¢ 7.86/kWh over 25 years with a $151.5 million investment cost. The net savings to Lahore Waste Management Company can be $103.4 and $137.7 million respectively with and without tipping fees on account of waste disposal cost, bricks revenue using bottom ash, and waste fee. The project developers can get net savings of $16.9 and $51.5 million respectively with and without tipping fees other than LCOE. Furthermore, the greenhouse gas emissions of 216.6 million tons of CO2eq can be saved throughout plant life against 279 GWh/year energy generation, in terms of grid emission factor and current methane release into the atmosphere from the dumping site.

Abstract

Background Several studies have found that pharmacists can assist in screening and prevention of osteoporosis by referring patients for bone mineral density scans and counselling on lifestyle changes. In Malaysia, screening osteoporosis in all elderly women is not mandatory due to its cost. One approach to address this gap is to develop a pharmacist-led osteoporosis screening and prevention program. However, there is a paucity of data on the perspectives of Malaysian pharmacists in this area. Objective To explore the perspective of stakeholders (policy makers, doctors, pharmacists, nurses and patients) towards the role of pharmacists in osteoporosis screening and management. Setting A primary care clinic located within a teaching hospital in Kuala Lumpur, Malaysia. Method Patients (n = 20), nurses (n = 10), pharmacists (n = 11), doctors (n = 10) and policy makers (n = 5) were individually interviewed using a semi-structured topic guide. Purposive sampling was used. Interviews were transcribed verbatim and analysed using thematic analysis. Main outcome measure Perspective of stakeholders on the current and future role of pharmacists. Results All participants perceived pharmacists to be suppliers of medication, although there was some recognition of roles in providing medication advice. Nonetheless, these stakeholders were eager for pharmacists to expand their non-dispensing roles towards counselling, creating awareness and screening of osteoporosis. Interviewed pharmacists referred to their current role as 'robotic dispensers' and unanimously agreed to spread out to osteoporosis management role. Conclusion Under stakeholders there is a willingness to expand the role of pharmacists in Malaysia to non-dispensing roles, particularly in osteoporosis screening and management.
