Background Australia has eight state-based ambulance services and New Zealand (NZ) has two. Significant variation between their datasets compromises cross-border research opportunities and translation of research to improve patient care. Ambulance data harmonisation has occurred in the United States and United Kingdom however, to-date no data harmonisation has occurred in Australia. This study aims to compare ambulance service variables in Australia and NZ to identify opportunities and barriers for harmonisation. Method Available 2019 variables were mapped to each other and several international standardized terminology systems to identify variations and similarities in variable names and definitions, and harmonisation opportunities. Results Four Australian ambulance services used one electronic patient care record (ePCR) system, three used other ePCR systems, one used paper-based records and both NZ services used one ePCR system. Only the NZ services had mapped their variables to two international standardised terminology systems. Barriers to harmonisation included the variables collected, the variable definitions and the variable naming convention. The core variables available for mapping varied and numbered from 27-69. Differences included similar variable names having different definitions, variables that should have different definitions having the same, and naming convention for similar/same variables differing between services. Conclusions Ambulance service data harmonisation in Australia and NZ is possible and presents significant opportunities for improvement in patient outcomes and performance audit. It would facilitate quality, large-scale, high-impact collaborative national and international research. Background: In Australia approximately 25% of Emergency Department (ED) attendances are via ambulance. ED overcrowding in Australia, as in many countries, is common. Measures to reduce overcrowding include the provision of enhanced timely primary care in the community for appropriate low risk injury and illness. Therefore paramedic assessment and referral to a community home hospital service, in preference to transfer to ED, may confer clinical and cost benefit. Methods/Design: A randomised controlled trial. Consenting adult patients that call an ambulance and are assessed by paramedics as having an eligible low risk problem will be randomised to referral to ED via ambulance transfer or referral to a rapid response service that will assess and treat the patient in their own residence. The primary outcome measure is requirement for unplanned medical attention (in or out of hospital) in the first 48 hours. Secondary outcomes will include a number of other clinical endpoints. A cost effectiveness analysis will be conducted. Discussion: If this trial demonstrates clinical non-inferiority and cost savings associated with the primary assessment service, it will provide one means to safely address ED overcrowding. Emergency responders (police, fire, ambulance and defence force personnel) risk exposure to dangerous and traumatic events, and the possible subsequent development of post-traumatic stress disorder. Consequently, partners of these emergency responders risk developing secondary traumatic stress (STS) from vicarious exposure to the trauma through communication and engagement with their responders. A mixed-methods study of the partners of emergency responders in New Zealand examined the extent of such partner-associated STS. This article focuses on two research questions: to what extent were risk factors for STS identified within that population, and what did the participants believe may help them to mitigate the impact of STS. An online anonymous survey was developed and eligible participants completed a 17-item STS scale, a social support measure, and answered several open-ended questions. Of the 646 participants, twenty percent appear to be experiencing intrusion, arousal, and avoidance symptoms related to the trauma experienced by their responder. Almost half stated they have little or no emotional/informational support related to their responder's work. Thematic analysis of free-text responses identified the need for additional support and more direct communication/engagement from the organisations for partners to navigate their experiences of STS and the level of social support received and required.
