4. Value of D-Dimer in Risk Stratification for Thromboembolism in Patients With Atrial Fibrillation and Low CHA2DS2-VASc Score

Abstract

Atrial fibrillation (AF) can cause thrombi formation and subsequent emboli deposition in systemic arteries, leading to various organ ischemia and infarction. Anticoagulation therapy can reduce the risk of thrombus formation and embolization, and is initiated based on a patient’s risk score, which is frequently estimated with the CHA2DS2-VASc score. We present a case of thromboembolism (TE) where a low CHA2DS2-VASc score suggested a low-moderate risk of systemic embolization, but an elevated plasma D-dimer value prompted further investigation which revealed an intracardiac thrombus with renal embolism. The patient is a 63-year-old male with past medical history of hypertension and AF treated with ablation 2 years prior presenting with sharp right flank pain of 5-hour duration. Primary workup and imaging were unrevealing at the time, and a low CHA2DS2-VASc score was suggestive of aspirin therapy. However, an elevated D-dimer of 289 ng/mL and a transient increase in creatinine pointed to possible etiology of embolic origin. The diagnosis was confirmed with computed tomography (CT) with contrast and transesophageal echocardiogram, revealing renal infarcts and the source of the emboli, respectively. The patient was treated with heparin and transitioned to apixaban prior to discharge with full resolution of symptoms. Through this case, we wish to show D-dimer’s predictive value of TE, as well as its potential benefit in risk assessment in patients with AF.

Abstract

Comet 67P/Churyumov‐Gerasimenko displays a pronounced hemispherical dichotomy in surface morphology, where the southern hemisphere exhibits more erosional features than the northern hemisphere due to receiving much greater solar radiation. Consequently, it is generally assumed that particles are ejected from the southern hemisphere through sublimation and a significant fraction eventually descends as airfall, covering the northern terrains. To investigate this south‐to‐north material transfer during the comet's perihelion passage, we used photoclinometry to measure material redistribution within its most extensive smooth terrain deposit around the Imhotep region. However, our findings do not align with this expected trend. Instead, we show that local‐scale processes substantially impact the erosion and accumulation of material, with one area experiencing net erosion while another nearby region, just a few dozen meters away, sees sediment buildup. Our analysis underscores the complex interplay of processes shaping Comet 67P's surface and likely comets more generally. Plain Language Summary: Comets are composed of some of the most primitive materials in the solar system, having spent most of their lifetimes in the far reaches of the outer solar system, shielded from the Sun's radiation. Jupiter Family Comets (JFCs) are a class of comets that get pulled into the inner solar system due to Jupiter's gravity. While JFCs still preserve abundant primordial materials, their new orbits expose them to significantly higher solar insolation, resulting in sublimation‐driven surface activity. The processes driving this activity, however, need to be better constrained. Observations of comet 67P/Churyumov‐Gerasimenko's (a JFC) surface indicate that particles are ejected from its southern hemisphere through sublimation, and a significant fraction eventually descends as "airfall," covering its northern terrains. Abstract

Comets are active geological worlds with primitive surfaces that have been shaped to varying degrees by sublimation‐driven sediment transport and mass wasting processes. Rosetta's rendezvous with comet 67P/Churyumov‐Gerasimenko (67P) in 2014 provided data with the necessary spatial and temporal resolutions to observe many evolutionary processes on micro‐gravity worlds. Rosetta's observations have thus far revealed that many changes to the surface occurred within 67P's smooth terrains, vast sedimentary deposits that blanket a significant fraction of the nucleus. Understanding the global context of these changes, and therefore the sediment transport pathways that govern the evolution of 67P's surface, requires a thorough description of their changing morphologies and an evaluation of existing global‐scale spatial and temporal trends. Accordingly, we present a time‐resolved synthesis of erosion and deposition activity on comet 67P as it passed through its 13 August 2015 perihelion from September 2014 to August 2016. Our mapping results indicate that, around perihelion, sediment is globally redistributed inter‐regionally from 67P's more active south to the north. Equally important, however, are local, topographically influenced sediment transport processes, with large volumes of sediment moving intra‐regionally over sub‐kilometer distances. We also show evidence for regions of near‐zero net erosion/deposition between approximately 30°N–60°N latitude, which may act as terminal sedimentary sinks, with the remobilization of these materials hindered by multiple factors. Our work therefore provides the most complete mapping of sediment transport processes and pathways across 67P, a critical step toward understanding the global landscape evolution of both 67P and other comets. Plain Language Summary: Comets are some of the oldest materials in the solar system. When these ice‐rich bodies approach the sun, the ice at the surface can sublimate, breaking down solid materials into sediment that can be mobilized across and off of the comet's surface. On comet 67P, sublimated vapor can move material in many ways. Determining how and where materials move across this comet can help us understand how its ice content has changed since it formed and how other comet surfaces may change over time. We found that the location and timing of sediment transportation are linked to the location of most direct sunlight on the comet's surface. Additionally, we only observed migrating scarps of smooth materials in either outer, equatorial regions of the comet or within "internal" regions that receive extra heat from the comet itself. We also found that sediments moved within and between regions after their initial deposition. However, we observed almost no sediment transport on the top of either lobe, perhaps indicating that the amount of deposited material is limited and may not be easily eroded after deposition. We predict that the processes transporting sediments on 67P will affect comets of similar orbits and masses. 24. Value of D-Dimer in Risk Stratification for Thromboembolism in Patients With Atrial Fibrillation and Low CHA2DS2-VASc Score.

Abstract

Atrial fibrillation (AF) can cause thrombi formation and subsequent emboli deposition in systemic arteries, leading to various organ ischemia and infarction. Anticoagulation therapy can reduce the risk of thrombus formation and embolization, and is initiated based on a patient's risk score, which is frequently estimated with the CHA2DS2-VASc score. We present a case of thromboembolism (TE) where a low CHA2DS2-VASc score suggested a low-moderate risk of systemic embolization, but an elevated plasma D-dimer value prompted further investigation which revealed an intracardiac thrombus with renal embolism. The patient is a 63-year-old male with past medical history of hypertension and AF treated with ablation 2 years prior presenting with sharp right flank pain of 5-hour duration. Primary workup and imaging were unrevealing at the time, and a low CHA2DS2-VASc score was suggestive of aspirin therapy. However, an elevated D-dimer of 289 ng/mL and a transient increase in creatinine pointed to possible etiology of embolic origin. The diagnosis was confirmed with computed tomography (CT) with contrast and transesophageal echocardiogram, revealing renal infarcts and the source of the emboli, respectively. The patient was treated with heparin and transitioned to apixaban prior to discharge with full resolution of symptoms. Abstract

The dwarf planet (1) Ceres, the largest object in the main asteroid belt (1) with a mean diameter of about 950 kilometres, is located at a mean distance from the Sun of about 2.8 astronomical units (one astronomical unit is the Earth-Sun distance). Thermal evolution models suggest that it is a differentiated body with potential geological activity (2,3). Unlike on the icy satellites of Jupiter and Saturn, where tidal forces are responsible for spewing briny water into space, no tidal forces are acting on Ceres. In the absence of such forces, most objects in the main asteroid belt are expected to be geologically inert. The recent discovery (4) of water vapour absorption near Ceres and previous detection of bound water and OH near and on Ceres (refs 5-7) have raised interest in the possible presence of surface ice. Here we report the presence of localized bright areas on Ceres from an orbiting imager (8). These unusual areas are consistent with hydrated magnesium sulfates mixed with dark background material, although other compositions are possible. Of particular interest is a bright pit on the floor of crater Occator that exhibits probable sublimation of water ice, producing haze clouds inside the crater that appear and disappear with a diurnal rhythm. Slow-moving condensed-ice or dust particles (9,10) may explain this haze. We conclude that Ceres must have accreted material from beyond the 'snow line' (11), which is the distance from the Sun at which water molecules condense., We observed Ceres using the Framing Camera (FC) on board the Dawn spacecraft. Abstract

Aptamers are oligonucleotide ligands, either RNA or ssDNA, selected for high-affinity binding to molecular targets, such as small organic molecules, proteins or whole microorganisms. While reports of new aptamers are numerous, characterization of their specific interaction is often restricted to the affinity of binding (KD). Over the years, crystal structures of aptamer-protein complexes have only scarcely become available. Here we describe some relevant technical issues about the process of crystallizing aptamer-protein complexes and highlight some biochemical details on the molecular basis of selected aptamer-protein interactions. In addition, alternative experimental and computational approaches are discussed to study aptamer-protein interactions.

Abstract

We report a case of cerebrovascular accident with thromboembolic stroke etiology in a patient who had atrial flutter and negative transesophageal echocardiography (TEE) results. The increased D-dimer levels (1877 ng/mL) initiated referral for magnetic resonance imaging and magnetic resonance angiography of the brain that showed classic recanalization of an embolic thrombus in the angular branch of the left middle cerebral distribution. The D-dimer level of this patient was normalized after 3 months of anticoagulation therapy. Although TEE is considered the gold standard for evaluation of cardiac source of embolism, exclusion of intracardiac thrombus with TEE alone does not eliminate the risk of thromboembolic events. This case highlights the utility of D-dimer as a potential adjunct in the decision-making process to guide investigation of thromboembolism, determine subsequent therapy, and hence reduce the risk of embolic stroke recurrence.

Abstract

BACKGROUND: The 16 kDa heat shock protein (HSP) is an immuno-dominant antigen, used in diagnosis of infectious Mycobacterium tuberculosis (M.tb.) causing tuberculosis (TB). Its use in serum-based diagnostics is limited, but for the direct identification of M.tb. bacteria in sputum or cultures it may represent a useful tool. Recently, a broad set of twelve 16 kDa specific heavy chain llama antibodies (VHH) has been isolated, and their utility for diagnostic applications was explored. METHODOLOGY/PRINCIPAL FINDINGS: To identify the epitopes recognized by the nine (randomly selected from a set of twelve 16 kDa specific VHH antibodies) distinct VHH antibodies, 14 overlapping linear epitopes (each 20 amino acid long) were characterized using direct and sandwich ELISA techniques. Seven out of 14 epitopes were recognized by 8 out of 9 VHH antibodies. The two highest affinity binders B-F10 and A-23 were found to bind distinct epitopes. Sandwich ELISA and SPR experiments showed that only B-F10 was suitable as secondary antibody with both B-F10 and A-23 as anchoring antibodies. To explain this behavior, the epitopes were matched to the putative 3D structure model. Electrospray ionization time-of-flight mass spectrometry and size exclusion chromatography were used to determine the higher order conformation. A homodimer model best explained the differential immunological reactivity of A-23 and B-F10 against heat-treated M.tb. lysates. CONCLUSIONS/SIGNIFICANCE: The concentrations of secreted antigens of M.tb. in sputum are too low for immunological detection and existing kits are only used for identifying M.tb. in cultures. Here we describe how specific combinations of VHH domains could be used to detect the intracellular HSP antigen. Linked to methods of pre-concentrating M.tb. cells prior to lysis, HSP detection may enable the development of protein-based diagnostics of sputum samples and earlier diagnosis of diseases.
