Abstract

Abstract Background As stereotactic body radiotherapy (SBRT) is a highly dose-dense radiotherapy, adverse events of neighboring normal tissues are a major concern. This study thus aimed to clarify the frequency and degree of clinical symptoms in patients with rib fractures after SBRT for primary lung cancer and to reveal risk factors for rib fracture. Appropriate α/β ratios for discriminating between fracture and non-fracture groups were also investigated. Methods Between November 2001 and April 2009, 177 patients who had undergone SBRT were evaluated for clinical symptoms and underwent follow-up thin-section computed tomography (CT). The time of rib fracture appearance was also assessed. Cox proportional hazard modeling was performed to identify risk factors for rib fracture, using independent variables of age, sex, maximum tumor diameter, radiotherapeutic method and tumor-chest wall distance. Dosimetric details were analyzed for 26 patients with and 22 randomly-sampled patients without rib fracture. Biologically effective dose (BED) was calculated with a range of α/β ratios (1–10 Gy). Receiver operating characteristics analysis was used to define the most appropriate α/β ratio. Results Rib fracture was found on follow-up thin-section CT in 41 patients. The frequency of chest wall pain in patients with rib fracture was 34.1% (14/41), and was classified as Grade 1 or 2. Significant risk factors for rib fracture were smaller tumor-chest wall distance and female sex. Area under the curve was maximal for BED at an α/β ratio of 8 Gy. Conclusions Rib fracture is frequently seen on CT after SBRT for lung cancer. Small tumor-chest wall distance and female sex are risk factors for rib fracture. However, clinical symptoms are infrequent and generally mild. When using BED analysis, an α/β ratio of 8 Gy appears most effective for discriminating between fracture and non-fracture patients.

Abstract

Background: The purpose of this study is to clarify the utility of thallium-201 single photon \r \r emission tomography (Tl-SPECT) for predicting local recurrence after stereotactic radiotherapy \r \r (SRT) by comparing it with 18fl udeoxyglucose-positron emission tomography (FDG-PET). Since Tl-\r \r SPECT can be performed at general hospitals, the ability of Tl-SPECT to predict local recurrence \r \r after SRT is clinically important. Methods and materials: Between January 2007 and December 2011, \r \r 78 patients with stage I non-small cell lung carcinoma underwent Tl-SPECT and FDG-PET before SRT. \r \r The median follow-up period was 28 months (range, 6 to 66 months), and the subjects’ median age \r \r was 80-years-old (range, 50 to 90-years-old).\r Results: During the analysis of factors associated with recurrence outside of the radiation fi eld \r \r and all recurrent lesions, correlations were detected between recurrence and the early and delayed \r \r standardized uptake values obtained during FDG-PET. Although it is not possible to predict the \r \r local effects of radiation based on a tumor’s FDG uptake, the latter parameter is useful for \r \r predicting the overall risk of recurrence. This indicates that the recurrence rate could be \r \r reduced by performing chemotherapy after radiotherapy in cases exhibiting strong FDG uptake. It is \r \r suggested that tumors that exhibit a retention index of >50.68 on Tl-SPECT often recur.\r Conclusion: Local recurrence after SRT can be predicted by Tl-SPECT. Therefore, it is expected that \r \r the use of Tl-SPECT will lead to the identifi cation of patients that are at high risk of local \r \r recurrence. Such patients could be given more intensive treatment, which would lead to an \r \r improvement in the local control rate.

Abstract

Stereotactic body radiotherapy (SBRT) for oligometastases represents a recent trend in radiation oncology. While abundant data are available regarding the use of SBRT for the treatment of lung or liver oligometastases from various retrospective series and prospective trials, relatively little information has been accumulated for the treatment of oligometastases at sites other than the lungs and liver, particularly for sequential oligometastases in multiple organs. Oligometastases with primary lesions controlled is called “oligo-recurrence.” We describe herein the case of a lung cancer patient who developed repeated oligo-recurrence at multiple sites that were each controlled by radical radiotherapy and achieved long-term survival and discuss the merits of locally aggressive radiotherapy for this type of disease condition with reviewing the literature. Although further investigation should be undertaken to clarify the benefits, objectives, and methods of SBRT for the treatment of oligometastases, we believe utilization of SBRT may be worthwhile for patients with remote metastases who hope for treatment to acquire better local control and possible longer survival.

Abstract

Background Methods Results Conclusions The aim of this study was to evaluate the newly established oncological criteria of resectability of hepatocellular carcinoma (HCC) for selecting suitable candidates for systemic and combination therapy.The data of 156 consecutive HCC patients with intrahepatic target nodules who had initially received systemic therapy (lenvatinib and atezolizumab plus bevacizumab) were reviewed. The patients were classified into three groups according to the novel oncological criteria for resectability (R, resectable; BR1, borderline resectable 1; and BR2, borderline resectable 2). The prognostic ability and clinical utility for selecting this population to receive combined use of multiple systemic sequential and locoregional therapy was then evaluated. Combined use of systemic sequential therapy with more than two agents and locoregional treatment was defined as multidisciplinary combination therapy (MCT), while systemic sequential therapy only and repeated locoregional treatment was defined as a single treatment procedure (STP).Patients classified as R and BR1 had significantly better overall survival (OS) compared with BR2 (R vs. BR2, p = 0.012; BR1 vs. BR2, p = 0.004). However, there was no significant difference between R and BR1 (p = 1.000), in spite of significantly worse oncological status in the BR1 patients. Following a R0 resection and MCT, the BR1 patients had significantly better OS compared with those receiving STP or no additional treatment (median OS, not reached vs. 25.2 months and 20.1 vs. 11.3 months, respectively; p = 0.034).In patients with advanced HCC with intrahepatic target nodules the BR1 category is one of the favorable candidates for selecting those to be treated with MCT strategies.
