2. LLMD: A Large Language Model for Interpreting Longitudinal Medical Records

Abstract

We introduce LLMD, a large language model designed to analyze a patient's medical history based on their medical records. Along with domain knowledge, LLMD is trained on a large corpus of records collected over time and across facilities, as well as tasks and labels that make nuanced connections among them. This approach is critical to an accurate picture of patient health, and has distinctive advantages over models trained on knowledge alone, unlabeled records, structured EHR data, or records from a single health system. The recipe for LLMD continues pretraining a foundational model on both domain knowledge and the contents of millions of records. These span an average of 10 years of care and as many as 140 care sites per patient. LLMD is then instruction fine-tuned on structuring and abstraction tasks. The former jointly identify and normalize document metadata, provenance information, clinical named-entities, and ontology mappings, while the latter roll these into higher-level representations, such a continuous era of time a patient was on a medication. LLMD is deployed within a layered validation system that includes continual random audits and review by experts, e.g. based on uncertainty, disease-specific rules, or use-case. LLMD exhibits large gains over both more-powerful generalized models and domain-specific models. On medical knowledge benchmarks, LLMD-8B achieves state of the art accuracy on PubMedQA text responses, besting orders-of-magnitude larger models. On production tasks, we show that LLMD significantly outperforms all other models evaluated, and among alternatives, large general purpose LLMs like GPT-4o are more accurate than models emphasizing medical knowledge. We find strong evidence that accuracy on today's medical benchmarks is not the most significant factor when analyzing real-world patient data, an insight with implications for future medical LLMs.'

Abstract

"Consolidated Gold Fields was a major British mining house founded by Cecil Rhodes in 1892. Diversifying from its South African gold interests, the company invested widely during the following century. This included investments in the Western Australian gold sector from the 1920s and exploration and mining activities elsewhere in Australia and the Territory of New Guinea. In the 1960s, Consolidated Gold Fields Australia (CGFA) was formed. CGFA had ambitious plans and the financial backing from London to establish itself as one of the main diversified mining companies in Australia. Investments were held in the historic Mount Lyell Mining and Railway Company, in Renison, and it was one of the first groups to develop iron ore deposits in the Pilbara of Western Australia. It also acquired a major interest in mineral sands. While the London-based Consolidated Gold Fields ceased to exist in 1989, taken over and dismembered by renowned corporate raider Hanson Plc, its Australian subsidiary, renamed Renison Goldfields Consolidated (RGC), continued for another nine years as a diversified mining group before it suffered its own corporate demise, facilitated by Hanson. CGFA and RGC were important participants in Australia’s post–World War II mining sector. This book is a history of a once great British mining-finance house and its investments in Australia. Consolidated Gold Fields had a rich and broad history in Australia; its ultimate fate did not demonstrate its potential as an Australian mining company."

Abstract

In this paper we introduce a new neural architecture for sorting unordered sequences where the correct sequence order is not easily defined but must rather be inferred from training data. We refer to this architecture as OrderNet and describe how it was constructed to be naturally permutation equivariant while still allowing for rich interactions of elements of the input set. We evaluate the capabilities of our architecture by training it to approximate solutions for the Traveling Salesman Problem and find that it outperforms previously studied supervised techniques in its ability to generalize to longer sequences than it was trained with. We further demonstrate the capability by reconstructing the order of sentences with scrambled word order.

Abstract

The Louvre Museum recently placed online Mariette's notebooks from the mid 19th century which include information on the find-spots of stelae from the Lesser Vaults of the Memphis Serapeum, the underground tomb complex of the Apis bulls at Saqqara. Some of these stelae name the reigning pharaoh and/or give dates for the bulls. The tombs had been ransacked but it seems that enough stelae remained close to their original positions to help determine the approximate location of each bull burial, a subject which has long been debated. Analysis of Mariette's documents suggests a greater overlap between Dynasties 22 and 25 than is generally accepted. [ABSTRACT FROM AUTHOR]

Abstract

Avian encephalomyelitis (AE) is a disease caused by the avian encephalomyelitis virus (AEV) of the genus Tremovirus in the family Picornaviridae. Recently, cases of turkey poults showing neurological signs were submitted to the veterinary diagnostic laboratories at South Dakota State University and the University of Minnesota. The affected birds were showing nervous neurological signs such as tremors, inability to stand, torticollis, and wing drop. Clinical signs were observed by 3 weeks of age. Necropsy of birds revealed no significant gross lesions in the internal organs, including the brain. There was no significant bacterial growth in the brains. Microscopic examination of various sections of the brain revealed multifocal lymphocplasmacytic perivascular cuffs in the cerebellum and cerebral cortex. The brain samples were processed for detection and whole genome sequencing by next-generation sequencing. Three full-length polyprotein sequences (6405 nt) of AEV were assembled. All three sequences shared 99.9–100% nucleotide and 100% amino acid identities with each other. Only 77.7–78.5% of nucleotide and 90.3–92.5% of amino acid identities with AEV field strains and vaccine sequences were available in GenBank. This indicates that a new divergent variant of AEV is circulating in the field and causing AE outbreaks in the Midwest region. [ABSTRACT FROM AUTHOR]

Abstract

Experienced grant writers know that reviewers are quick to decide whether they like or do not like the proposals they are reading. Therefore, much of the success of any given proposal rides on the strength of the first page--its capacity to "sell" the research idea to skeptical reviewers. This paper describes a writing technique designed to elicit a positive response from grant reviewers at the outset. While the three-paragraph template does not guarantee a winning outcome, it will set the stage for high reviewer scores now required for success in the increasingly competitive world of sponsored research.
