Rationale & objectiveUnderlying kidney disease is an emerging risk factor for more severe coronavirus disease 2019 (COVID-19) illness. We examined the clinical courses of critically ill COVID-19 patients with and without pre-existing chronic kidney disease (CKD) and investigated the association between the degree of underlying kidney disease and in-hospital outcomes.Study designRetrospective cohort study.Settings & participants4,264 critically ill patients with COVID-19 (143 patients with pre-existing kidney failure receiving maintenance dialysis; 521 patients with pre-existing non-dialysis-dependent CKD; and 3,600 patients without pre-existing CKD) admitted to intensive care units (ICUs) at 68 hospitals across the United States.Predictor(s)Presence (vs absence) of pre-existing kidney disease.Outcome(s)In-hospital mortality (primary); respiratory failure, shock, ventricular arrhythmia/cardiac arrest, thromboembolic events, major bleeds, and acute liver injury (secondary).Analytical approachWe used standardized differences to compare patient characteristics (values>0.10 indicate a meaningful difference between groups) and multivariable-adjusted Fine and Gray survival models to examine outcome associations.ResultsDialysis patients had a shorter time from symptom onset to ICU admission compared to other groups (median of 4 [IQR, 2-9] days for maintenance dialysis patients; 7 [IQR, 3-10] days for non-dialysis-dependent CKD patients; and 7 [IQR, 4-10] days for patients without pre-existing CKD). More dialysis patients (25%) reported altered mental status than those with non-dialysis-dependent CKD (20%; standardized difference=0.12) and those without pre-existing CKD (12%; standardized difference=0.36). Half of dialysis and non-dialysis-dependent CKD patients died within 28 days of ICU admission versus 35% of patients without pre-existing CKD. Compared to patients without pre-existing CKD, dialysis patients had higher risk for 28-day in-hospital death (adjusted HR, 1.41 [95% CI, 1.09-1.81]), while patients with non-dialysis-dependent CKD had an intermediate risk (adjusted HR, 1.25 [95% CI, 1.08-1.44]).LimitationsPotential residual confounding.ConclusionsFindings highlight the high mortality of individuals with underlying kidney disease and severe COVID-19, underscoring the importance of identifying safe and effective COVID-19 therapies in this vulnerable population.

Background and purposeSelf-management programs may improve quality of life and self-efficacy for stroke survivors, but participation is low. In a randomized controlled trial of a complex, multidisciplinary, team-based secondary stroke prevention intervention, we offered participants Chronic Disease Self-Management Program (CDSMP) workshops in addition to clinic visits and home visits. To enhance participation, workshops were facilitated by community health workers who were culturally and linguistically concordant with most participants and scheduled CDSMP sessions at convenient venues and times. Over time, we implemented additional strategies such as free transportation and financial incentives. In this study, we aimed to determine factors associated with CDSMP participation and attendance.MethodsFrom 2014 to 2018, 18 CDSMP workshop series were offered to 241 English and Spanish-speaking individuals (age ≥40 years) with recent stroke or transient ischemic attack. Zero-inflated Poisson regression was used to identify factors associated with participation and attendance (ie, number of sessions attended) in CDSMP. Missing values were imputed using multiple imputation methods.ResultsNearly one-third (29%) of intervention subjects participated in CDSMP. Moderate disability and more clinic/home visits were associated with participation. Participants with higher numbers of clinic and home visits (incidence rate ratio [IRR], 1.06 [95% CI, 1.01-1.12]), severe (IRR, 2.34 [95% CI, 1.65-3.31]), and moderately severe disability (IRR, 1.55 [95% CI, 1.07-2.23]), and who enrolled later in the study (IRR, 1.12 [95% CI, 1.08-1.16]) attended more sessions. Individuals with higher chaos scores attended fewer sessions (IRR, 0.97 [95% CI, 0.95-0.99]).ConclusionsLess than one-third of subjects enrolled in the SUCCEED (Secondary Stroke Prevention by Uniting Community and Chronic Care Model Teams Early to End Disparities) intervention participated in CDSMP; however, participation improved as transportation and financial barriers were addressed. Strategies to address social determinants of health contributing to chaos and engage individuals in healthcare may facilitate attendance. Registration: URL: https://www.clinicaltrials.gov. BackgroundRecurrent strokes are preventable through awareness and control of risk factors such as hypertension, and through lifestyle changes such as healthier diets, greater physical activity, and smoking cessation. However, vascular risk factor control is frequently poor among stroke survivors, particularly among socio-economically disadvantaged blacks, Latinos and other people of color. The Chronic Care Model (CCM) is an effective framework for multi-component interventions aimed at improving care processes and outcomes for individuals with chronic disease. In addition, community health workers (CHWs) have played an integral role in reducing health disparities; however, their effectiveness in reducing vascular risk among stroke survivors remains unknown. Our objectives are to develop, test, and assess the economic value of a CCM-based intervention using an Advanced Practice Clinician (APC)-CHW team to improve risk factor control after stroke in an under-resourced, racially/ethnically diverse population.Methods/designIn this single-blind randomized controlled trial, 516 adults (≥40 years) with an ischemic stroke, transient ischemic attack or intracerebral hemorrhage within the prior 90 days are being enrolled at five sites within the Los Angeles County safety-net setting and randomized 1:1 to intervention vs usual care. Participants are excluded if they do not speak English, Spanish, Cantonese, Mandarin, or Korean or if they are unable to consent. The intervention includes a minimum of three clinic visits in the healthcare setting, three home visits, and Chronic Disease Self-Management Program group workshops in community venues. 19. Effect of Heparin on Recanalization in Acute Stroke Patients with Intra-Arterial Thrombi.

Anticoagulant use, such as heparin, is usually contraindicated in acute stroke patients. We present a study of patients, who were treated with intravenous heparin after a stroke that were also found to have an intraluminal thrombus. Prior studies imply that recanalization is achieved with heparin; however heparin should only prevent thrombus propagation. Therefore it is unclear whether and how IV heparin can achieve recanalization of intraluminal thrombi in acute stroke patients. A retrospective review of all acute stroke patients from a single stroke center who received a therapeutic IV heparin infusion from 5/2006 to 9/2011 were included in the study. We compared patients who had complete/partial recanalization and/or improved flow versus those that did not, with both these groups on a standard intravenous heparin infusion protocol. Demographic data was compared between the groups. Average partial thromboplastin time (PTT) during heparin infusion, time between computed tomography angiographies (CTAs), time from stroke onset to receiving IV heparin, and vessel occluded were also compared between groups. Forty-one patients (19 female, 22 male) were included in the study with a total of 55 vessels (either carotid, middle cerebral artery, anterior cerebral artery, posterior cerebral artery/posterior circulation) having intraluminal thrombi; 31 patients had 41 vessels with either partial or complete recanalization of effected vessels, while 10 patients had 14 vessels that did not have at least one vessel recanalize while on heparin. Using t-test we noted that the average PTT between the vessels that had partial/complete recanalization group (61.74) and nonrecanalization group (66.30) was not statistical significantly different (P=0.37).The average time in days on heparin between vascular imaging studies (CTA/conventional angiogram) in the group of vessels with partial/complete recanalization (7.12 days) and the ones with no change (6.11 days) was not significantly different between the two groups (P=0.59). Patient's vessels receiving heparin for 24 hours did not significantly differ either (P=0.17). This study compares patient characteristics associated with recanalization of intraluminal thrombi in acute stroke patients on heparin. Recanalization of intraluminal thrombi are not associated with average PTT or duration on heparin.

With public concerns about fostering the inclusion of women, especially those with low socio-economic status, governments and private entities have started to develop collaborative strategies to improve women's development. One such strategy is collaborations with sharing economy entities, which are online platforms that provide entrepreneurship opportunities for individuals despite geographical and structural barriers. This paper draws on Ostrom's concept of the commons to explore how to foster the inclusion of women as entrepreneurs in the sharing economy through collaboration. Thematic analysis reveals the nature and outcomes of policies geared towards women's development. While previous studies on the sharing economy have noted that the top-down nature of its operations has resulted in individualised gains, this paper highlights the collaboration within these settings – leading to individual and mutual monetary and non-monetary benefits – stemming from a shared digital and traditional commons with context-based ideologies and stakeholder actions. This paper gives practitioners a clear understanding of how they can successfully support and create a context for women's entrepreneurship in the age of digitisation. [ABSTRACT FROM AUTHOR]

This paper presents first report on a systematic investigation on developing quantum chemical descriptors on understanding biological activity (pIC50) of a series of 10 thiazolyl‐thiadiazines (ID‐01 to ID‐10) as Beta site amyloid precursor protein cleaving enzyme 1 (BACE‐1) protein inhibitors for Alzheimer diseases, under density functional theory. The interactions between inhibitors and model biomolecule are studied in terms of charge and energy transfer, where the target biomolecule at the host BACE‐1 protein is identified from the family of 20 amino acids, which are universal to all living organisms. The present study identifies electron affinity (EA), total electronic energy (E), and the electron transfer with amino acid (∆E) of thiazolyl‐thiadiazines as promising descriptors, which can explain about 90% of observed biological activity. The developed regression model for training set is also validated for unknown test set of homologous compounds. The developed quantum chemical descriptors for prediction of the biological activity of thiazolyl‐thiadiazines will certainly be an excellent addition in the QSAR parlance of drug development. We have fabricated a NaCoO
2 –Co3 O4 nanocomposite through high-temperature calcination of a Na,Co-MOF precursor. The resulting nanocomposite features Co3 O4 polyhedral nanoparticles decorated over NaCoO2 nanosheets. The composite possessed a surface area of 21.0 m2 g−1 , surpassing that of the MOF. The composite was studied for its capacity to adsorb NO2 gas (100 ppm) in ambient conditions, demonstrating a substantial capacity of 68.1 mg g−1 . However, this adsorption capacity was influenced negatively by factors such as moisture, increased gas flow rate, and reduced bed loading. Spectroscopic analysis and theoretical calculations revealed that NO2 exhibited a higher tendency to adsorb onto the NaCoO2 component than Co3 O4 . Apart from NO evolution, the adsorbed NO2 molecules underwent redox reactions on the composite's surface, yielding nitrite and nitrate species. This study delves into the surface reactivity of Li
2 MnO3 towards H2 S, SO2 and NO gases at room temperature, employing both experimental and theoretical calculations. Li2 MnO3 was synthesized using a conventional solid-state method. Then, Li2 MnO3 was exposed to 100 ppm concentrations of the aforementioned gases, with subsequent spectroscopic and X-ray diffraction analyses, revealing gas-surface interactions within a few atomic layers. X-Ray diffraction demonstrated minimal structural changes post-gas exposure, indicative of surface-level reactions. X-Ray photoelectron spectroscopy and theoretical calculations unveiled the dissociative adsorption of H2 S, resulting in various oxidized sulphur species. H2 S reactivity was most pronounced at Mn sites, where H2 S donated electrons during the process. SO2 adsorption yielded sulphate species, facilitated by electron transfer to Mn-sites. Conversely, NO adsorption identified distinctive Li and Mn sites on the oxide surface. By combining experimental and theoretical insights, it has been confirmed that the chemisorption of these toxic gases over the Li2 MnO3 surface is pertinent to their effective removal across temperature ranges. National data on patient characteristics, treatment, and outcomes of critically ill coronavirus disease 2019 (COVID-19) solid organ transplant (SOT) patients are limited. We analyzed data from a multicenter cohort study of adults with laboratory-confirmed COVID-19 admitted to intensive care units (ICUs) at 68 hospitals across the United States from March 4 to May 8, 2020. From 4153 patients, we created a propensity score matched cohort of 386 patients, including 98 SOT patients and 288 non-SOT patients. We used a binomial generalized linear model (log-binomial model) to examine the association of SOT status with death and other clinical outcomes. Among the 386 patients, the median age was 60 years, 72% were male, and 41% were black. Death within 28 days of ICU admission was similar in SOT and non-SOT patients (40% and 43%, respectively; relative risk [RR] 0.92; 95% confidence interval [CI]: 0.70-1.22). Other outcomes and requirement for organ support including receipt of mechanical ventilation, development of acute respiratory distress syndrome, and receipt of vasopressors were also similar between groups. There was a trend toward higher risk of acute kidney injury requiring renal replacement therapy in SOT vs. non-SOT patients (37% vs. 27%; RR [95% CI]: 1.34 [0.97-1.85]). Death and organ support requirement were similar between SOT and non-SOT critically ill patients with COVID-19.

A systematic density functional investigation on the structural, electronic and optical properties of the growth of (ZnO)6 cluster unit in the series of (ZnO)6n for n = 1–9 is reported in this paper. Different electronic properties of (ZnO)6n nanoclusters are analyzed in terms of HOMO–LUMO gap (HLG), ionization potential (IP), electron affinity (EA), chemical hardness (η) and electrophilicity index (ω), which all shows a zigzag behavior as the size of (ZnO)6n clusters increases. The electronic energy gain (ΔE) of the clusters identified an exceptionally stable 'magic' nanocluster, viz. (ZnO)42. Frontier orbitals analysis results indicate easy electron transfer in (ZnO)42 nanocluster system. The optical absorption spectra confirm that the magic (ZnO)42 nanocluster is active in the visible range (λ = 406.8 Å) of electromagnetic spectrum. Interestingly, like zigzag electronic properties, similar optical switching toward the growth of (ZnO)6 unit is also observed. The simulation results of electronic properties as well as the infrared spectra of magic (ZnO)42 cluster will open up a vista to the experimentalists for its possible synthesis, which in turn will help in the development of the visibly active magic (ZnO)42 nanocluster with novel applications in the fields of quantum dots or assembled materials.
