- Abstract
Introduction: Ophthalmic conditions and dementia appear to overlap and may share common pathways, but research has not differentiated dementia subtypes. Methods: Diagnoses of cataracts, age‐related macular degeneration (AMD), diabetic retinopathy (DR), and glaucoma were based on medical histories and International Classification of Diseases, Ninth Revision (ICD‐9) codes for 3375 participants from the Cardiovascular Health Study. Dementia, including Alzheimer's disease (AD) and vascular dementia (VaD), was classified using standardized research criteria. Results: Cataracts were associated with AD (hazard ratio [HR] = 1.34; 95% confidence interval [CI] = 1.01‐1.80) and VaD/mixed dementia (HR = 1.41; 95% CI = 1.02‐1.95). AMD was associated with AD only (HR = 1.87; 95% CI = 1.13‐3.09), whereas DR was associated with VaD/mixed dementia only (HR = 2.63; 95% CI = 1.10‐6.27). Discussion: Differential associations between specific ophthalmic conditions and dementia subtypes may elucidate pathophysiologic pathways. Lack of association between glaucoma and dementia was most surprising from these analyses. [ABSTRACT FROM AUTHOR]

- Abstract
Reduced glomerular filtration rate defines chronic kidney disease and is associated with cardiovascular and all-cause mortality. We conducted a meta-analysis of genome-wide association studies for estimated glomerular filtration rate (eGFR), combining data across 133,413 individuals with replication in up to 42,166 individuals. We identify 24 new and confirm 29 previously identified loci. Of these 53 loci, 19 associate with eGFR among individuals with diabetes. Using bioinformatics, we show that identified genes at eGFR loci are enriched for expression in kidney tissues and in pathways relevant for kidney development and transmembrane transporter activity, kidney structure, and regulation of glucose metabolism. Chromatin state mapping and DNase I hypersensitivity analyses across adult tissues demonstrate preferential mapping of associated variants to regulatory regions in kidney but not extra-renal tissues. These findings suggest that genetic determinants of eGFR are mediated largely through direct effects within the kidney and highlight important cell types and biological pathways.

Most (∼70%) of strokes are first-ever strokes, and hence to substantially reduce the neurological burden, primary prevention is crucial. Here, highlights of the National Institute of Neurological Disorders and Stroke workshop ‘Stroke Risk Assessment and Future Stroke Primary Prevention Trials’ held January 12–13, 2004 are summarized. The Workshop discussions focused on stroke risk assessment; the high-risk vs. population-based approaches to primary prevention; desirable characteristics of candidate treatments and potential novel treatments, such as the ‘polypill’; subclinical disease as risk assessment tool and as surrogate outcome, and methodological issues in stroke primary prevention trials. The importance of assessing cognitive decline as an important consequence of covert and overt vascular injury of the brain was emphasized. 1. Recognize the palliative care needs of ICU patients with severe acute brain injury and their family members 2. Describe some ways in which clinicians can identify family members in the ICU who are at risk for poor outcomes Use of a palliative care needs checklist may help identify patients and families at risk for poor outcome. To evaluate the association between palliative care needs and outcome among patients with severe acute brain injury (SABI) and their family members. We enrolled ICU patients with SABI, defined as stroke, traumatic brain injury, or hypoxic ischemic encephalopathy and a Glasgow Coma Scale ≤12, and their primary family member. Clinicians completed a palliative care needs checklist, and family were asked the same questions at enrollment. Questions considered pain and distressing symptoms, social and spiritual support needs, concerns about prognosis, or a need to readdress goals of care. Family completed surveys at enrollment and at 3 months. Primary outcome was symptoms of anxiety or depression among family. Using multiple linear regression, we evaluated the association between needs and outcomes at enrollment and 3 months. Over 30 months, we enrolled 209 patient-family pairs. Family had a mean age 51.1 years (SD 16), 64.4% were women, and 32.9% identified as BIPOC. At least one palliative care need was identified more often by family than by clinicians (88.2% vs 52.6%, kappa = 0.00). After adjustment for confounders, clinician identification of any need was not associated with primary outcome but with family decisional regret at 3 months (b = 18.2, p < 0.01). Family identification of any need was associated with increased symptoms of depression (b = 1.0, p < 0.01) and anxiety ((b = 2.5, p = 0.01) among family and decreased patient quality of life at 3 months. Significant palliative care needs exist in patients with SABI and family members, although there was poor agreement between clinicians and families. The clinician-completed checklist failed to predict who was at increased risk for psychological distress. Checklists may be an efficient way to identify patients or families at high risk for poor outcomes, but their implementation requires further research. [ABSTRACT FROM AUTHOR]
