Introduction: Past research shows that structural racism contributes to disparities in cardiometabolic health among racially/ethnically minoritized populations. Methods: This cross-sectional study examined the correlation between census tract-level racialized economic segregation and child health metrics among a racially and ethnically diverse cohort of 350 children (ages 6.5?13.8) from Minneapolis?St. Paul, MN. Results: A consistent cardiometabolic and cortisol outcome gradient was observed across the index of concentration at the extremes tertiles, such that health risk factors increased as tract privilege decreased. Conclusion: Racialized economic segregation was associated with less favorable child health outcomes, underscoring the potential importance of place-based interventions for promoting children?s health.

Background Previous studies have found that exposure to childhood environmental stress is associated with cardiometabolic risk. However, it is not known whether individual health behaviors disrupt this relationship. This study prospectively evaluated the relationship between cumulative environmental stress in a low‐income sample and cardiometabolic risk in middle childhood and examined whether child health behaviors attenuated this relationship. Methods and Results In a cohort of children (n=338; 57% Hispanic children; 25% Black children), environmental stressors (family and neighborhood factors representing disadvantage/deprivation) and child health behaviors (accelerometry measured physical activity; parent‐reported screen time and diet recalls) were measured over 5 time points beginning when children were aged 2 to 4 years and ending when they were aged 7 to 11 years. Children's cardiometabolic risk factors (body mass index, blood pressure, triglyceride/high‐density lipoprotein ratio, glucose, hemoglobin A1c, C‐reactive protein) were measured at 7 to 11 years. Emerging cardiometabolic risk was defined as having ≥1 elevations that exceeded clinical thresholds. Abstract Metabolic rewiring is the result of the increasing demands and proliferation of cancer cells, leading to changes in the biological activities and responses to treatment of cancer cells. The mitochondrial citrate transport protein SLC25A1 is involved in metabolic reprogramming offering a strategy to induce metabolic bottlenecks relevant to radiosensitization through the accumulation of the oncometabolite D-2-hydroxyglutarate (D-2HG) upon SLC25A1 inhibition (SLC25A1i). Previous studies have revealed the comparative effects of SLC25A1i or cell-permeable D-2HG (octyl-D-2HG) treatments on DNA damage induction and repair, as well as on energy metabolism and cellular function, which are crucial for the long-term survival of irradiated cells. Here, α-ketoglutarate (αKG), the precursor of D-2HG, potentiated the effects observed upon SLC25A1i on DNA damage repair, cell function and long-term survival in vitro and in vivo, rendering NCI-H460 cancer cells more vulnerable to ionizing radiation. However, αKG treatment alone had little effect on these phenotypes. In addition, supplementation with nicotinamide (NAM), a precursor of NAD (including NAD+ and NADH), counteracted the effects of SLC25A1i or the combination of SLC25A1i with αKG, highlighting a potential importance of the NAD+/NADH balance on cellular activities relevant to the survival of irradiated cancer cells upon SLC25A1i. Furthermore, inhibition of histone lysine demethylases (KDMs), as a major factor affected upon SLC25A1i, by JIB04 treatment alone or in combination with αKG supplementation phenocopied the broad effects on mitochondrial and cellular function induced by SLC25A1i. Taken together, αKG supplementation potentiated the effects on cellular processes observed upon SLC25A1i and increased the cellular demand for NAD to rebalance the cellular state and ensure survival after irradiation. Future studies will elucidate the underlying metabolic reprogramming induced by SLC25A1i and provide novel therapeutic strategies for cancer treatment.

Abstract Species response traits mediate environmental effects on species distribution. Traits are used in joint and multispecies distribution models (JSDMs and MSDMs) to enable community‐wide shared parameters that characterise niche filtering along environmental gradients. Multispecies machine learning SDMs, however, do not use traits as their inclusion requires an additional taxonomic dimension that is incompatible with their usual tabular inputs. This has confined trait mediation in SDMs to hierarchical Bayesian models. Here we provide a novel artificial neural network (ANN) architecture that solves this dimensionality problem. Our ANN includes species traits (via a time distributed layer) and is therefore able to identify not only species‐specific responses to the environment, but also shared responses across the community that are mediated by species traits. Model performance evaluated at the species level not only quantifies the reliability of species predictions, but also their departure from an average response dictated by traits only. We apply our model to two unique long‐term spatio‐temporal of butterfly and moth datasets collected across the United Kingdom between 1990 and 2019. In addition to species traits, predictors include numerous metrics derived from weather, land‐cover and topology data. For butterflies and moths we show convincing model performance for classifying species occupancy. We use SHAP (Shapley Additive exPlanations) to explain the ANN and show how trait‐mediated and species‐specific responses can be approximated, hence yielding ecological insights on the key drivers of species distribution. We highlight a range of drivers of change that determine occupancy, including wind, temperature as well as habitat type. We demonstrate that a trait‐based approach can be encoded as an ANN by using a time distributed layer. This brings ANNs unmatched predictive capabilities to the field of MSDMs, at the same time of lifting their reputed drawback of poor explainability.

Prior research suggests COVID-19 has amplified stress on Academic Clinician Frontline-Workers (ACFW). The aim of this paper is: (1) to better understand the experiences of ACFW during the COVID-19 pandemic including their mental-emotional wellbeing, academic productivity, clinical experiences, and (2) to examine any gender differences. A cross-sectional survey was administered to University of Minnesota/M Health Fairview systems’ faculty February-June 2021. Of the 291 respondents, 156 were clinicians, with 91 (58 %) identifying as Frontline-Workers (ACFW). Faculty wellbeing was assessed using validated measures in addition to measures of productivity and sociodemographics. For example, ACFW reported a higher Work-Family Conflict (WFC) scores compared to non-ACFW (26.5 vs. 24.1, p = 0.057) but did not report higher Family-Work Conflict (FWC) scores (17.7 vs. 16.3, p = 0.302). Gender sub-analyses, revealed that women ACFW compared to men ACFW reported higher WFC scores (27.7 vs. 24.1, p = 0.021) and FWC (19.3 vs. 14.3, p = 0.004). Academically, ACFW reported submitting fewer grants and anticipated delays in promotion and tenure due to the COVID-19 (p = 0.035). Results suggest COVID-19 has exacerbated ACFW stress and gender inequities. Reports of anticipated delay in promotion for ACFW may pose a challenge for the long-term academic success of ACFW, especially women ACFW. In addition, women may experience higher FWC and WFC as compared to men. Schools of academic medicine should consider re-evaluating promotion/tenure processes and creating resources to support women ACFW as well as ACFW caregivers.

Background The diagnosis of peritonitis among peritoneal dialysis (PD) patients is based on clinical presentation, dialysis effluent white blood cell (WBC) count, and dialysis effluent culture. Peritoneal fluid WBC count is very important in the initial diagnosis of peritonitis. The purpose of this work was to determine the optimal number of peritoneal WBCs with different clinical presentations at admission to define PD-related peritonitis. Methods Medical records of chronic PD patients who underwent work-up for suspected peritonitis between 2008 and 2019 were reviewed retrospectively. Results of all peritoneal WBC count tests during this period were collected. Clinical manifestations and follow-up analysis of each peritoneal WBC count were performed. Results The peritoneal WBC count cutoff of 100/μL recommended by International Society for Peritoneal Dialysis provided specificity of only 35%. Increasing peritoneal WBC count cutoff to 150, 200, and 250/μL provided sensitivity around 98% and gradually increasing specificity. The chi-square automatic interaction detector model of statistical analysis determined that peritoneal WBC count below 230/μL combined with absence of inflammatory markers (fever, increased C-reactive protein) ruled out peritonitis with 99.8% sensitivity. Peritoneal fluid WBC count cutoff of 230/μL provided specificity of 89% and good positive and negative likelihood scores of 8.3 and 0.03, respectively. Peritoneal fluid polymorphonuclear count has lower discriminating ability for peritonitis compared to peritoneal fluid WBC count. Conclusion Increasing peritoneal fluid WBC count cutoff to 230/μL in suspected PD-related peritonitis could improve specificity without compromising the sensitivity of the test.

Abstract Colonisation credit refers to the temporal lag between positive conservation actions and species' responses and may be one of the reasons we fail to meet short‐term conservation targets. This is particularly evident in woodlands which take decades to develop and harbour slow colonising species. Given global objectives to increase woodland cover, it is important to know the timeframe within which colonisation credit will be fulfilled. The colonisation of woodland plants was examined in recent woodlands, created between 15 and 80 years ago, and located adjacent or isolated from existing ancient woodlands. Colonisation credit was calculated as the proportion of understory woodland plant species in the nearest ancient woodland which had not colonised recent woodlands. Looking at individual species traits also allowed us to tease apart their impact on the species colonisation and establishment ability. Spatial adjacency between created and ancient woodland reduced colonisation credit by an average of 28%, and more mature created woodlands (50–80 years old) had fulfilled 24% more of their colonisation credit on average than younger created woodlands (15–21 years old). However, mature woodlands created adjacent to ancient woodlands had still only been colonised by an average of 72% of the available species pool. Plants which had reached adjacent created woodlands were dispersed by a range of mechanisms, where those that had reached more isolated sites were largely dispersed by birds or mammals. Low community weighted mean shade tolerance, high community weighted nutrient affiliation, and the dominance of Hedera helix suggest that competition from dominant natives may be preventing certain species establishing in new woodlands. This research demonstrates the need to account for appropriate time‐lags when setting biodiversity targets, with most sites still displaying colonisation credit decades after they were created. The results also indicate that spatially targeting woodland creation adjacent to species‐rich mature woodlands should be prioritised. Still, poor local habitat conditions may lead to the dominance of specific competitors which prevent a range of other species from establishing. Local management interventions such as translocations and tree thinning may ameliorate this but further research is needed.

Abstract Contemporary analyses of insect population trends are based, for the most part, on a large body of heterogeneous and short‐term datasets of diurnal species that are representative of limited spatial domains. This makes monitoring changes in insect biomass and biodiversity difficult. What is needed is a method for monitoring that provides a consistent, high‐resolution picture of insect populations through time over large areas during day and night. Here, we explore the use of X‐band weather surveillance radar (WSR) for the study of local insect populations using a high‐quality, multi‐week time series of nocturnal moth light trapping data. Specifically, we test the hypotheses that (i) unsupervised data‐driven classification algorithms can differentiate meteorological and biological phenomena, (ii) the diversity of the classes of bioscatterers are quantitatively related to the diversity of insects as measured on the ground and (iii) insect abundance measured at ground level can be predicted quantitatively based on dual‐polarization Doppler WSR variables. Adapting the quasi‐vertical profile analysis method and data clustering techniques developed for the analysis of hydrometeors, we demonstrate that our bioscatterer classification algorithm successfully differentiates bioscatterers from hydrometeors over a large spatial scale and at high temporal resolutions. Furthermore, our results also show a clear relationship between biological and meteorological scatterers and a link between the abundance and diversity of radar‐based bioscatterer clusters and that of nocturnal aerial insects. Thus, we demonstrate the potential utility of this approach for landscape scale monitoring of biodiversity.

Formaldehyde-free hair-straightening products are hair-smoothening solutions widely used by professional beauty salons. Formaldehyde-free hair straighteners do not technically contain formaldehyde; however, they contain other chemicals such as glyoxyloyl carbocysteine which releases formaldehyde upon contact with heat. Moreover, its by-product glyoxylate may convert to oxalate; both compounds have potential nephrotoxic effect. Here, we report a case of a 41-year-old woman who presented to the emergency room with weakness, nausea, vomiting, and stage 3 acute kidney injury (AKI) according to Kidney Disease: Improving Global Outcomes (KDIGO) acute kidney injury staging shortly after exposure to formaldehyde-free hair-straightening product; other causes of AKI were excluded such as preceding acute illness, drug history, or other nephrotoxic agent exposure. On physical examination, the patient was pale, and her vital signs were normal. The urine microscopy and serologic workup were not indicative. Kidney core biopsy revealed interstitial edema, acute interstitial nephritis, and oxalate crystal nephropathy. Kidney function completely recovered after a short course of steroid therapy. In this case, AKI was a complication caused by exposure to hair-straightening products branded as formaldehyde free but actually containing other chemical products which release formaldehyde and other toxic chemicals when heated during the straightening procedure and may cause systemic toxicity, particularly kidney injury. Different cosmetic products are widely in use, but not all are under tight regulation, and therefore, it is important to raise the awareness among both medical teams and consumers of possible adverse health effects of different cosmetic products.

The aim of the study was to determine the effectiveness of brushite exposure to correct the marginal permeability of composite restorations in persons with a decompensated form of carious process activity; this study also examined the penetrating ability of brushite in the micro-cracks of the enamel-composite border Methods and Results: The study included 120 patients (age 18-40 years) with a decompensated course of the carious process, who underwent complete oral cavity sanitation with the direct composite restoration of 300 teeth. The obtained results were analyzed and interpreted in vivo (the clinical effectiveness of the restoration was assessed according to the Ryge criteria immediately after treatment and 1, 12, and 36 months after treatment) and in vitro (scanning electron microscopy of the enamel-composite joint after applying the composite and after treatment with the studied means). According to the purpose and objectives of the study, two groups of patients were formed: Group 1 (control, n=58): the prevention of recurrent caries after sanitation was carried out using a standard personal hygiene product containing fluorine; Group 2 (experimental, n=62): the prevention of recurrent caries after sanitation was carried out using a natural two-component complex for strengthening and remineralization of enamel RemarsGel. During statistical processing of the obtained data, immediately after sanitation and one month after it, the differences in the indicators of the Ryge criteria in Groups 1 and 2 were statistically insignificant. Twelve months after the sanitation, the Ryge restoration quality indicators in Group 2 were higher than in Group 1 (P≤0.001). After 36 months, the statistical significance of the difference in the restoration quality indicators increased (P≤0.0001). These results were confirmed by the data of scanning electron microscopy. Based on the results obtained, stating the state of the quality of the enamel-composite joint, it can be concluded that brushite crystals are highly effective in preventing violations of the integrity of the enamel-composite joint, which has a direct and immediate impact on the quality of restoration. Conclusion: We consider it necessary to add to the list of main indications for the use of the RemarsGel system one more, a scientifically based indication of the targeted use of the system in the presence of a significant amount of adhesive, both direct and indirect restorations in the oral cavity, especially in the decompensated course of the carious process.

This article provides an ethnographic analysis of the agency of women who reside in the rural areas of the Argentine Pampas, based on their promotion and production of agroecological family horticulture. The recognition of these women’s agency through care – care of their children, global care, and green care – offers a significant challenge to some metrocentric and Eurocentric feminist perspectives that claim care work can only be oppressive for women. The first of these types of care empowers women to improve the nutrition of their children. It also relates to another underlying type of care, which is to provide a sufficiently robust education as to ensure their children have a better and alternative future. The second type of care has the power to socially transform the territorial space of the district’s countryside and its marginalized populations which, through care, acquire greater public and political attention. The third type of care empowers women to transform and care for the environment, and is exercised by not using pesticides in horticultural production and by disseminating knowledge on the matter. In line with discussions of postcolonial feminism (Abu-Lughod, 1986; Mahmood, 2001; Suárez Navaz, 2008), I argue that certain properties that are attributed to women relative to caregiving – by way of a dichotomous view of gender relations – fuel their agency: for these women the cultivation of vegetables is a form of agency that actively combats food, training and labor inequality.

Abstract Background In Australia, demand for specialist infectious diseases services exceeds capacity to provide timely management of latent tuberculosis infection (LTBI) in areas of high refugee and asylum seeker settlement. A model for treating LTBI patients in primary care has been developed and piloted in a refugee-focused primary health service (Monash Health Refugee Health and Wellbeing [MHRHW]) and a universal primary care clinic. This study reports on the development and evaluation of the model, focusing on the model feasibility, and barriers and enablers to its success. Methods A convergent mix-methods design was used to evaluate the model for treating LTBI patients in primary care, where a prospective cohort study of patients commencing treatment either at MHRHW or the universal primary care clinic determined the model feasibility, while focus groups with clinicians directly involved in treating these patients explored barriers and enablers to sustainability and success of the model. Results From January 2017 to April 2018, 65 patients with confirmed LTBI presented at participating clinics. Treatment was accepted by 31 (48%) patients, of whom 15(48%) were treated at MHRHW and 16 (52%) at the universal primary care clinic. The 6-months’ treatment completion rate was higher at MHRHW compared to the universal primary care clinic (14 (93%) compared to 9 (56%) respectively, p = 0.0373). Reasons for non-completion included adverse reaction, opting out and relocation. At the completion of the pilot, 15 clinicians participated in two focus groups. Clinicians identified barriers and enablers for successful LTBI management at patient, provider, organisational and clinical levels. While barriers for treatment completion and adherence were consistent across the two pilot sites, enablers, such as resources to facilitate patient education and follow-up, were available only at MHRHW. Conclusion Screening and management of LTBI patients can be achieved within the primary care setting, considerate of barriers and enablers at patient, provider, organisational and clinical levels. Upscaling of a primary care response to the management of LTBI will require supporting primary care clinics with resources to employ dedicated clinical staff for patient education, follow-up communication and monitoring medication adherence.

Resumen Realizamos trabajo de campo etnográfico y analizamos sentidos y prácticas divergentes hacia talleres de crianza y parto respetado en las periferias de un distrito rural bonaerense. Los objetivos iniciales de estas iniciativas públicas y gratuitas no fueron totalmente alcanzados ya que lograron convocar a menos participantes de lo previsto. Llamamos por lo tanto nominalistas a las mujeres que acuden a los talleres, lugares donde se sostiene que la mujer tiene que volverse una con su “cría” para luego buscar “espacios de autonomía”. Ellas se distinguen a sí mismas de la mayoría de las mujeres que no acude a los talleres. Frente a las primeras, estas últimas no harían un “esfuerzo” ni pondrían atención especial en los procesos de parto y crianza que concebirían como “naturales”. Así se ve como no todo el mundo está “intrínsecamente” atraído por el paradigma del “parto respetado”.

Background and aims – Agricultural intensification and loss of farmland heterogeneity have contributed to population declines of wild bees and other pollinators, which may have caused subsequent declines in insect-pollinated wild plants. Material and methods – Using data from 37 studies on 22 pollinator-dependent wild plant species across Europe, we investigated whether flower visitation and seed set of insect-pollinated plants decline with an increasing proportion of arable land within 1 km. Key results – Seed set increased with increasing flower visitation by bees, most of which were wild bees, but not with increasing flower visitation by other insects. Increasing proportion of arable land had a strongly variable effect on seed set and flower visitation by bees across studies. Conclusion – Factors such as landscape configuration, local habitat quality, and temporally changing resource availability (e.g. due to mass-flowering crops or honey bee hives) could have modified the effect of arable land on pollination. While our results highlight that the persistence of wild bees is crucial to maintain plant diversity, we also show that pollen limitation due to declining bee populations in homogenized agricultural landscapes is not a universal driver causing parallel losses of bees and insect-pollinated plants.

An evaluation of accessibility, appropriateness, acceptability and efficiency of telephone consultations, implemented at Monash Health Refugee Health and Wellbeing (MH RHW) throughout the COVID-19 pandemic, was conducted. A convergent mix-methods design was used, with both patients (n = 50) and clinicians (n = 11) participating in a survey, and two focus groups (n = 14) involving clinicians being conducted. Service utilization data was sourced from the MH RHW database. During May to December 2020, 61% (n = 3012) of the consultations were conducted by telephone, 42% (n = 11) of these required interpreters in a 3-way conversation Most patients were satisfied with telephone as a medium for providing care and with the quality of telephone-based care. Similarly, clinicians considered telephone consultations to be an acceptable mode-of-care for most patients during the pandemic, however, expressed caution in relation to certain patient cohort. Finally, the provision of care by telephone was considered no more efficient than face-to-face service provision, as reflected in the time required for each consultation, with some clinicians reporting adverse workload outcomes. This study highlighted the benefits and challenges of telephone consultations from patient and clinician perspectives. It also highlighted the types of patients that may not be suited to telephone consultations. Overall, this study showed that telephone service delivery is a feasible option in providing care to people of refugee background and should be considered in future decisions as an ongoing Medicare (Australia’s universal healthcare insurance scheme) billing item. However, clinical discretion should prevail in determining the most appropriate means of delivering care.
