This article highlights key themes from history, current practice, and future directions in missiology. It traces the development of missiology as a discipline, highlighting some of the key voices, groups and movements along the way. It provides a brief overview of the history of missiology, noting voices and influences from the Majority World. The social, political, and religious changes of the twentieth century had a significant impact on missiology, and the article considers some of the missiological movements and organisations which emerged during that time. The issue of evangelism and social action in ecumenical councils receives particular attention. Contextualization and attitudes to culture are considered in some detail, and a critique is offered of some of the contemporary understandings of contextualization. Various themes in missiology are explored, including liberation; the significance of women’s voices; the increasing importance of contextual theologies; the urgency of caring for creation; issues around land; and the emergence of Indigenous voices and theologies in each of these areas. Questions of ecclesiology are also discussed from a missional perspective. The article concludes with some key questions for missiology. Colonial assumptions of mission are considered, arguing that in the non-Western world, mission and colonialism were two sides of the same coin of imperialism. Therefore, more recent missiological reflection from the Majority World is urgently needed to correct this. A non-Western approach to mission is considered via a brief study of missio Dei and the Bantu concept of ubuntu, which argues for a holistic approach to mission and careful stewardship of creation. Finally, the role of pneumatology in mission is highlighted, and an integrated understanding of divine and human action in mission is called for, based on participation in the work of the Spirit, which has sometimes been lacking in missiological reflection. The entire article is written with a commitment to listen to and learn from diverse and global voices. It considers mission from liberative, communal, and collaborative perspectives.

It is unclear whether a dominant hemispherectomy/hemispherotomy in someone with Rasmussen’s Encephalitis (RE) may produce a satisfactory outcome when performed over the age of 40 years. Important questions include whether RE may continue to evolve three decades after onset, and whether a hemispherectomy may adequately shift language function when performed in older ages. Two cases illustrate seizure, language, motor and functional outcomes after dominant hemispherotomies. The cases were selected from an epilepsy surgery database of procedures performed at a private hospital in Cape Town, South Africa, spanning the period 1998–2023. A man in his 40s with epilepsy since childhood and dominant hemisphere RE partially regained impaired comprehension and ambulation, while expressive language function did not recover post-hemispherotomy. By contrast, a young teenage patient with dominant hemisphere RE demonstrated considerable recovery of expressive and receptive language and ambulation post-surgery. Both remain seizure-free. These two cases demonstrate that a dominant hemispherotomy, when performed on a quadragenarian, may produce a satisfactory, albeit inferior, functional outcome in comparison to when performed in childhood. RE may cause progressive neurological dysfunction in the late thirties and older and should be considered in patients presenting with functional decline decades after disease onset.

Diamonds produced using chemical vapor deposition (CVD) have found many applications in various fields of science and technology. Many applications involve polycrystalline CVD diamond films of micron thicknesses. However, a variety of optical, thermal, mechanical, and radiation sensing applications require more bulky CVD diamond samples. We report the results of a magnetic resonance and structural study of a thick, sizable polycrystalline CVD diamond disc, both as-prepared and treated with e-beam irradiation/high-temperature annealing, as well as gamma irradiation. The combination of various magnetic resonance techniques reveals and enables the attribution of a plentiful collection of paramagnetic defects of doublet and triplet spin origin. Analysis of spectra, electron, and nuclear spin relaxation, as well as nuclear spin diffusion, supports the conclusion of significant macro- and micro-inhomogeneities in the distribution of nitrogen-related defects.

Purpose: Endobronchial electromagnetic transponder beacons (EMT) provide real-time, precise positional data of moving lung tumors. We report results of a phase 1/2, prospective, single-arm cohort study evaluating the treatment planning effects of EMT-guided SABR for moving lung tumors. Methods and Materials: Eligible patients were adults, Eastern Cooperative Oncology Group 0 to 2, with T1-T2N0 non-small cell lung cancer or pulmonary metastasis ≤4 cm with motion amplitude ≥5 mm. Three EMTs were endobronchially implanted using navigational bronchoscopy. Four-dimensional free-breathing computed tomography simulation scans were obtained, and end-exhalation phases were used to define the gating window internal target volume. A 3-mm expansion of gating window internal target volume defined the planning target volume (PTV). EMT-guided, respiratory-gated (RG) SABR was delivered (54 Gy/3 fractions or 48 Gy/4 fractions) using volumetric modulated arc therapy. For each RG-SABR plan, a 10-phase image-guided SABR plan was generated for dosimetric comparison. PTV/organ-at-risk (OAR) metrics were tabulated and analyzed using the Wilcoxon signed-rank pair test. Treatment outcomes were evaluated using RECIST (Response Evaluation Criteria in Solid Tumours; version 1.1). Results: Of 41 patients screened, 17 were enrolled and 2 withdrew from the study. Median age was 73 years, with 7 women. Sixty percent had T1/T2 non-small cell lung cancer and 40% had M1 disease. Median tumor diameter was 1.9 cm with 73% of targets located peripherally. Mean respiratory tumor motion was 1.25 cm (range, 0.53-4.04 cm). Thirteen tumors were treated with EMT-guided SABR and 47% of patients received 48 Gy in 4 fractions while 53% received 54 Gy in 3 fractions. RG-SABR yielded an average PTV reduction of 46.9% (P < .005). Lung V5, V10, V20, and mean lung dose had mean relative reductions of 11.3%, 20.3%, 31.1%, and 20.3%, respectively (P < .005). Dose to OARs was significantly reduced (P < .05) except for spinal cord. At 6 months, mean radiographic tumor volume reduction was 53.5% (P < .005). Conclusions: EMT-guided RG-SABR significantly reduced PTVs of moving lung tumors compared with image-guided SABR. EMT-guided RG-SABR should be considered for tumors with large respiratory motion amplitudes or those located in close proximity to OARs.

Knowledge co-production (KCP) is presented as an effective strategy to inform responses to complex coastal and marine social-ecological challenges. Co-production processes are further posited to improve research and decision outcomes in a wide range of problem contexts (e.g., biodiversity conservation, climate change adaptation), for example, by facilitating social learning among diverse actors. As such, KCP processes are increasingly centered in global environment initiatives such as the United Nations Decade of Ocean Science for Sustainable Development. However, KCP is not a panacea, and much uncertainty remains about its emergence and implementation, in particular, the manner in which broader governance contexts determine the interplay of knowledge, power, and decision-making. Three objectives guide our analysis: (1) to interrogate more fully the interplay among social relations of power, knowledge production practices, and the (colonial) governance contexts in which they are embedded; (2) to consider the challenges and limitations of KCP in particular places by drawing attention to key governance themes and their implications for achieving better outcomes; and (3) to work toward a fuller understanding of “deep KCP” that cautions against a tendency to view knowledge processes in coastal and marine governance settings as an instrumental or techno-managerial problem. A qualitative and reflective approach was used to examine multiple dimensions of the interplay of KCP, governance, and power in several marine and coastal contexts, including Canada, New Zealand, and Papua New Guinea. In particular, our analysis highlights the importance of: (1) recognizing diverse motivations that frame co-production processes; (2) the manner in which identities, positionality, and values influence and are influenced by governance contexts; (3) highlighting governance capacity with respect to spatial and temporal constraints; (4) institutional reforms necessary for KCP and the links to governance; and (5) the relationship between knowledge sharing, data sovereignty, and governance. We seek to encourage those involved in or considering co-production initiatives to engage carefully and critically in these processes and make co-production more than a box to tick.

This paper argues that the reason that the Church of England has strug­gled to relate to lay pioneering is because its primary mode of engagement of resourcing and equipping is out of step with the realities of lay pioneering. It argues that despite numerous recommendations to release the laity in mis­sion and ministry, when it happened through grassroots communities which became known as ‘fresh expressions’, the Church of England was unable to recognize it. By exploring both the “organizational story” and the “grassroots story”, this paper demonstrates that the problem is the Church of England’s reflex to view everything through a lens of resourcing and equipping. This lens means all problems are framed as deficit, in this case of the laity, which are remedied through the resources of the church. The paper reveals that this lens causes it to miss the gifts and challenges of lay pioneering, and makes it unable to engage in the mutual relationships called for in the report “Setting God’s People Free” (Archbishops Council, 2017). The paper calls for a deeper engage­ment by the Church of England with grassroots stories of lay pioneers and to allow the narrative of resourcing and equipping to be interrupted. It suggests that attentive listening to lay pioneers and their stories can lead to more mutual and reciprocal engagement and as a result enrich the Church of England and other denominations.

Large simulation efforts are required to provide synthetic galaxy catalogs for ongoing and upcoming cosmology surveys. These extragalactic catalogs are being used for many diverse purposes covering a wide range of scientific topics. In order to be useful, they must offer realistically complex information about the galaxies they contain. Hence, it is critical to implement a rigorous validation procedure that ensures that the simulated galaxy properties faithfully capture observations and delivers an assessment of the level of realism attained by the catalog. We present here a suite of validation tests that have been developed by the Rubin Observatory Legacy Survey of Space and Time (LSST) Dark Energy Science Collaboration (DESC). We discuss how the inclusion of each test is driven by the scientific targets for static ground-based dark energy science and by the availability of suitable validation data. The validation criteria that are used to assess the performance of a catalog are flexible and depend on the science goals. We illustrate the utility of this suite by showing examples for the validation of cosmoDC2, the extragalactic catalog recently released for the LSST DESC second Data Challenge.

The emissions of volatile organic compounds (VOCs) from petroleum product tankers potentially represent a significant source of VOCs in port cities. Emission factors are used to estimate the produced VOCs. VOC emissions from transit operations were simulated using a two part model of heat and mass transfer. Using local meteorological data of air temperatures, solar radiation and wind speed, the heat transfer within the tank was modeled. Results showed that bulk cargo temperature remained relatively steady at 25–28°C, the oil surface oscillated diurnally by 1–2°C, and the deck temperature oscillates diurnally by 15–20°C. The solar insolation had the largest effect on the tank temperatures. VOC emissions for two crude oils and gasoline, two tank configurations, and two meteorological conditions were estimated using a model derived from a mass balance on the tank and the obtained temperature profile. Only 3 of 8 scenarios had pressure increases large enough to cause venting of VOC. C2-C5 compounds constituted the majority of VOCs released from crude oils and ethanol made up the majority of the VOCs released from the gasoline carrying barge. The calculated daily emission factors for crude oil and gasoline (barge) were 10 mg/L/day and 135 mg/L/day respectively.

Abstract Background Mission of Mercy (MOM) emergency dental clinics are a resource for populations lacking access to dental care. We designed a MOM event incorporating health equity components with established community partners who shared a common vision of addressing the oral health, physical health, and social service needs of Maryland and Washington, DC area residents. Although studies have explored associations between oral and chronic health conditions, few studies to our knowledge have examined the relationship between these conditions and receipt of dental services. Therefore, this study explored these associations and the opportunity for better care coordination. Methods Oral health data from the 2014 Mid-Maryland Mission of Mercy and Health Equity Festival event was analyzed. A descriptive analysis assessed frequencies and percentages of participant sociodemographics characteristics, oral health and chronic disease risk(s), and dental services delivered. Chi-square tests and multivariate logistic regression were conducted to determine the associations between 1) oral health and chronic disease risk(s) and dental services; and 2) oral health and chronic disease risk(s) and participant characteristics. Results Approximately 66.2% (n = 666) of the 1007 participants had one or more chronic conditions and/or risk factors (diabetes, high blood pressure, and tobacco use). Category: Midfoot/Forefoot Introduction/Purpose: Hallux rigidus is a common degenerative joint condition of the foot. In advanced stages of the disease, extensive procedures have been utilized including Keller arthroplasty, interpositional arthroplasty (IPA), arthrodesis, total joint replacement or synthetic cartilage replacement. IPA is a surgical procedure that attempts to maintain joint motion through insertion of a biologic spacer into the joint. However, there is still a paucity of overall clinical data regarding outcomes and complication rates following IPA procedure for the treatment of hallux rigidus.The purpose of the current study was to systematically review the outcomes of IPA in the treatment of hallux rigidus. Methods: A systematic search of the MEDLINE, EMBASE and Cochrane Library databases was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Outcomes collected and analysed included: AOFAS score, VAS score, SF36 score, range of motion, radiographic parameters, and postoperative complications. The level and quality of evidence were recorded and assessed. Results: Sixteen studies with a total of 433 patients met inclusion/exclusion criteria. The mean AOFAS improved from 56.8±7.3 (range, 43.2-64.3) preoperatively to 84.0±6.7 (range, 71.6-90.0) postoperatively. The preoperative weighted mean total ROM was 37.7±16.2 degrees and the postoperative weighted mean total ROM was 60.3±13.4 degrees. Five studies examined joint space narrowing on plain radiographs. The mean preoperative joint space was 1.2±0.2 mm (range, 1 -1.5) and the mean postoperative joint space was 2.5±0.5 mm (range, 1.9-3). The complication rate was 18.2% with metatarsalgia as the most commonly reported complication. One study was Level III and 15 studies were Level IV. Conclusion: This systematic review demonstrates improvement in functional and ROM outcomes following IPA. The procedure however has a high complication rate. There is a low level and quality of evidence in the current literature with inconsistent reporting of data. Therefore, further well designed studies must be carried out to determine the efficacy of IPA in the treatment of hallux rigidus.

Dietary lipids and some pharmaceutical lipid excipients can facilitate the targeted delivery of drugs to the intestinal lymphatics. Here, the feasibility of magnetic resonance imaging (MRI) for imaging lipid uptake into the intestinal lymphatics was assessed, shedding light on which lymph nodes can be targeted using this approach. Three healthy male volunteers were scanned at 3.0 T at baseline, 120, 180, 240, and 300 min post high-fat meal. A sagittal multi-slice image was acquired using a diffusion-weighted whole-body imaging sequence with background suppression (DWIBS) (pre inversion TI = 260 ms). Changes in area, major, and minor axis length were compared at each time point. Apparent diffusion coefficient (ADC) was calculated (b = 0 and 600 s/mm2) across eight slices. An average of 22 nodes could be visualised across all time points. ADC increased at 120 and 180 min compared to the baseline in all three participants by an average of 9.2% and 6.8%, respectively. In two participants, mean node area and major axis lengths increased at 120 and 180 min relative to the baseline. In conclusion, the method described shows potential for repeated lymph node measurements and the tracking of lipid uptake into the lymphatics. Further studies should focus on methodology optimisation in a larger cohort.

Inroduction: The purpose of this study was to explore factors associated with willingness of African Americans and Latinos to participate in biomedical and public health research and to delineate factors that influence the decision to become a human subject. Methods: We present results from a 2010 random digit-dial telephone survey of 2,455 African American (N = 1191) and Latino (N = 1264) adults. We used standard measures to assess knowledge of research, terminology, informed consent procedures, previous participation in research, health care experiences, social support, risk perception, religiousness, and trust. Results: Over 60% of both African Americans and Latinos reported they believed people in medical research are pressured into participating. Over 50% said that it was not at all important to have someone of the same race/ethnicity ask them to participate. In a sub-sample of 322 African Americans and 190 Latinos who had previously been asked to participate in a research study, 63% of African Americans and 65% of Latinos consented to participate in a study. Finally, both African Americans (57%) and Latinos (68%) reported willingness to participate in future research. Overall, the multivariate analysis explained 29% of the variability in willingness to participate in future research. Conclusions: Results suggest that African Americans and Latinos have no automatic predisposition to decline participation in research studies. These results can inform culturally tailored interventions for ethical recruitment of minorities into research and clinical trials.

Introduction Efforts to address health disparities and achieve health equity are critically dependent on the development of a diverse research workforce. However, many researchers from underrepresented backgrounds face challenges in advancing their careers, securing independent funding, and finding the mentorship needed to expand their research. Methods Faculty from the University of Maryland at College Park and the University of Wisconsin-Madison developed and evaluated an intensive week-long research and career-development institute—the Health Equity Leadership Institute (HELI)—with the goal of increasing the number of underrepresented scholars who can sustain their ongoing commitment to health equity research. Results In 2010-2016, HELI brought 145 diverse scholars (78% from an underrepresented background; 81% female) together to engage with each other and learn from supportive faculty. Overall, scholar feedback was highly positive on all survey items, with average agreement ratings of 4.45-4.84 based on a 5-point Likert scale. Eighty-five percent of scholars remain in academic positions. In the first three cohorts, 73% of HELI participants have been promoted and 23% have secured independent federal funding. Conclusions HELI includes an evidence-based curriculum to develop a diverse workforce for health equity research. For those institutions interested in implementing such an institute to develop and support underrepresented early stage investigators, a resource toolbox is provided.

At Republic 519a-521b, Socrates claims that each guardian must return from his/her contemplation to run Kallipolis. Quite reasonably, Glaucon objects that they would be making the guardian's life worse than it could be. This is sometimes referred to as “the happy philosopher problem”. But rather than answering Glaucon, Socrates admonishes him that their focus is instead on the role of the class of guardians and the happiness of the whole city. It turns out this admonition is the last in a string of similar admonitions that Socrates gives to his interlocutors. This paper examines Socrates' admonition to Glaucon, and its relation to Socrates’ other warnings to focus on the happiness of the city. By examining these admonitions, we can defend Socrates' dismissal of Glaucon's question and the happy philosopher question at 519d. The paper concludes by examining a few strategies for interpreting Socrates’ reluctance to engage Glaucon’s question.

The Visible Infrared Imaging Radiometer Suite (VIIRS) on-board the second Joint Polar Satellite System (JPSS) completed its sensor level testing in February 2018. The JPSS-2 (J2) mission is scheduled to launch in 2022 and will be very similar to its two predecessor missions, the Suomi National Polar-orbiting Partnership (SNPP) mission, launched on 28 October 2011 and JPSS-1 (renamed NOAA-20) launched on 18 November 2017. VIIRS instrument has 22 spectral bands covering the spectrum between 0.4 and 12.6 μm: 14 reflective solar bands (RSB), 7 thermal emissive bands (TEB) and one day-night band (DNB). It is a cross-track scanning radiometer capable of providing global measurements through observations at two spatial resolutions, 375 m and 750 m at nadir for the imaging bands and moderate bands, respectively. This paper will provide an overview of J2 VIIRS characterization methodologies and calibration performance during the pre-launch testing phases performed by the National Aeronautics and Space Administration (NASA) VIIRS Characterization Support Team (VCST) to evaluate the at-launch baseline radiometric performance and generate the parameters needed to populate the sensor data record (SDR) Look-Up-Tables (LUTs). Our analysis results confirmed the good performance of J2 VIIRS, in general as good as previous VIIRS instruments and all non-compliances are expected to have low impact on data quality. Key sensor performance metrics include the signal to noise ratio (SNR), radiance dynamic range, reflective and emissive bands calibration performance, polarization sensitivity, spectral performance, response versus scan-angle (RVS) and scattered light response. A set of performance metrics generated during the pre-launch testing program will be compared to both the SNPP and JPSS-1 VIIRS sensors.

This paper will also provide an assessment of the sensor pre-launch radiometric performance, such as the sensor signal to noise ratios (SNRs), radiance dynamic range, reflective and emissive bands calibration performance, polarization sensitivity, spectral performance, response-vs-scan (RVS), and scattered light response. A set of performance metrics generated during the pre-launch testing program will be compared to both the VIIRS sensor specification and the SNPP VIIRS pre-launch performance.

The VIIRS instrument on board the S-NPP spacecraft has successfully operated for more than four years since its launch in October 2011. Many VIIRS environmental data records (EDR) have been continuously generated from its sensor data records (SDR) with improved quality, enabling a wide range of applications in support of users in both the operational and research communities. This paper provides a brief review of sensor on-orbit calibration methodologies for both the reflective solar bands (RSB) and the thermal emissive bands (TEB) and an overall assessment of their on-orbit radiometric performance using measurements from instrument on-board calibrators (OBC), as well as regularly scheduled lunar observations. It describes and illustrates changes made and to be made for calibration and data quality improvements. Throughout the mission, all of the OBC have continued to operate and function normally, allowing critical calibration parameters used in the data production systems to be derived and updated. The temperatures of the on-board blackbody (BB) and the cold focal plane assemblies are controlled with excellent stability. Despite large optical throughput degradation discovered shortly after launch in several near- and short-wave infrared spectral bands and strong wavelength-dependent solar diffuser degradation, the VIIRS overall performance has continued to meet its design requirements. Also discussed in this paper are challenging issues identified and efforts to be made to further enhance the sensor calibration and characterization, thereby maintaining or improving data quality.

Macrophages serve to maintain organ homeostasis in response to challenges from injury, inflammation, malignancy, particulate exposure, or infection. Until now, receptor ligation has been understood as being the central mechanism that regulates macrophage function. Using macrophages of different origins and species, we report that macrophage elasticity is a major determinant of innate macrophage function. Macrophage elasticity is modulated not only by classical biologic activators such as LPS and IFN-γ, but to an equal extent by substrate rigidity and substrate stretch. Macrophage elasticity is dependent upon actin polymerization and small rhoGTPase activation, but functional effects of elasticity are not predicted by examination of gene expression profiles alone. Taken together, these data demonstrate an unanticipated role for cell elasticity as a common pathway by which mechanical and biologic factors determine macrophage function.

Abstract The aims of this study were to estimate the prevalence of hookah smoking and describe its associated knowledge, attitudes, and behaviors among Ethiopian and Eritrean Americans. Approximately 400 participants completed an online survey on hookah tobacco use, perceptions of harm and social acceptability. Nearly 80% had tried hookah in their lifetime (ever users), and about 43% had smoked hookah in the past 30 days (current users). Hookah ever use was highest among people 24–29 years of age, but there were no significant differences by gender, education or employment. Ever users were less supportive of bans on hookah smoking in a bar or restaurant compared with never users, X2 (3, N = 396) = 57.56, p<.001. Additionally, ever users more often reported hookah smoking was less harmful than cigarette smoking compared with never users. Future research is needed to understand the predictors of hookah smoking in this subpopulation and inform interventions. [ABSTRACT FROM AUTHOR]

Background: There is an urgent need to discover Alzheimer's disease (AD) biomarkers that are both easily measured and reliable. Research into blood-based biomarkers for AD using transcriptomics and proteomics has been an attractive and promising area of research. However, to date researchers have not looked into the possibility of AD medication being a confounding factor in these studies.Objective: This study explored whether acetylcholinesterase inhibitors (AChEIs), the main class of AD medication, are a confounding factor in AD blood biomarker studies.Methods: The most promising blood transcriptomic and proteomic biomarkers from two recent studies were analyzed to determine if they were differentially expressed between AD subjects on AChEIs and subjects that were not.Results: None of the gene or protein biomarkers analyzed were found to be significantly altered between subjects in either group.Conclusion: This study found no evidence that AChEIs are a confounding factor in these published AD blood biomarker studies. Further work is needed to confirm that this is also the case for other proposed biomarkers. [ABSTRACT FROM AUTHOR]

Background: Alzheimer's disease (AD) biomarkers are urgently needed for both early and accurate diagnosis and prediction of disease progression. Past research has studied blood-based proteins as potential AD biomarkers, revealing many candidate proteins. To date only limited effort has been made to investigate the disease specificity of AD candidate proteins and whether these proteins are also involved in other neurodegenerative or psychiatric conditions. Objective: This review seeks to determine if blood-based AD candidate protein biomarkers are disease specific. Methods: A two-stage systematic literature search was conducted. Firstly, the most consistently identified AD protein biomarkers in blood were determined from a list of published discovery or panel-based (>100 proteins) blood proteomics studies in AD. Secondly, an online database search was conducted using the 10 most consistently identified proteins to determine if they were involved in other brain disorders, namely frontotemporal lobe dementia, vascular dementia, Lewy body disease, Parkinson's disease, schizophrenia, depression, and autism. Results: Among the reviewed candidate proteins, plasma protease C1 inhibitor, pancreatic prohormone, and fibrinogen γ chain were found to have the least evidence for non-specificity to AD. All other candidates were found to be affected by other brain disorders. Conclusion: Since we found evidence that the majority of AD candidate proteins might also be involved in other brain disorders, more research into the disease specificity of AD protein biomarkers is required. Abstract Neuroblastomas constitute about 10% of childhood cancers and are responsible for 15% of pediatric cancer mortality. We evaluated the efficacy and the mechanism of cell death induced by CAY10404, a selective cyclooxygenase-2 (Cox-2) inhibitor in four human neuroblastoma cell lines (SH-EP, SH-SY5Y, SK-N-MC and MSN). Treatment with CAY10404 in the range of 15–115 µM revealed a dose-dependent decrease in cell number and an average IC 50 (inhibitory concentration 50%) of 60 µM. About 20–30% of the cells were terminal deoxynucleotidyltransferase-mediated UTP nick-end-labeling (TUNEL) positive 48 h after treatment. Western blot analysis of CAY10404-treated cells showed poly(ADP-ribose) polymerase (PARP) cleavage and cleaved caspase-3 signifying caspase activity and apoptotic cell death. Inhibitor-of-apoptosis proteins including X-linked inhibitor-of-apoptosis protein (XIAP) and survivin did not change significantly after CAY10404 treatment. Fluorescence activated cell sorter (FACS) analysis performed in two different cell lines 48 h following CAY10404 treatment showed a reduction in the number of cells in the G1 phase of the cell cycle and an increase in the number of cells in the G2 phase. When radioresistant SH-EP cells were treated with CAY10404, a 49% decrease in cell viability was observed relative to DMSO-treated cells; pretreatment with CAY10404 followed by ortho -voltage irradiation further enhanced cell death (58%) suggesting radiosensitization by CAY10404. BackgroundPatients with chronic respiratory diseases and those in the postdischarge period following hospitalization because of COVID-19 are particularly vulnerable, and little is known about the changes in their symptoms and physiological parameters. Continuous remote monitoring of physiological parameters and symptom changes offers the potential for timely intervention, improved patient outcomes, and reduced health care costs. ObjectiveThis study investigated whether a real-time multimodal program using commercially available wearable technology, home-based Bluetooth-enabled spirometers, finger pulse oximeters, and smartphone apps is feasible and acceptable for patients with chronic respiratory diseases, as well as the value of low-burden, long-term passive data collection. MethodsIn a 3-arm prospective observational cohort feasibility study, we recruited 60 patients from the Royal Free Hospital and University College Hospital. These patients had been diagnosed with interstitial lung disease, chronic obstructive pulmonary disease, or post–COVID-19 condition (n=20 per group) and were followed for 180 days. This study used a comprehensive remote monitoring system designed to provide real-time and relevant data for both patients and clinicians. Data were collected using REDCap (Research Electronic Data Capture; Vanderbilt University) periodic surveys, Remote Assessment of Disease and Relapses–base active app questionnaires, wearables, finger pulse oximeters, smartphone apps, and Bluetooth home-based spirometry. The feasibility of remote monitoring was measured through adherence to the protocol, engagement during the follow-up period, retention rate, acceptability, and data integrity. ResultsLowest-burden passive data collection methods, via wearables, demonstrated superior adherence, engagement, and retention compared with active data collection methods, with an average wearable use of 18.66 (SD 4.69) hours daily (77.8% of the day), 123.91 (SD 33.73) hours weekly (72.6% of the week), and 463.82 (SD 156.70) hours monthly (64.4% of the month). Highest-burden spirometry tasks and high-burden active app tasks had the lowest adherence, engagement, and retention, followed by low-burden questionnaires. Spirometry and active questionnaires had the lowest retention at 0.5 survival probability, indicating that they were the most burdensome. Adherence to and quality of home spirometry were analyzed; of the 7200 sessions requested, 4248 (59%) were performed. Of these, 90.3% (3836/4248) were of acceptable quality according to American Thoracic Society grading. Inclusion of protocol holidays improved retention measures. The technologies used were generally well received. ConclusionsOur findings provide evidence supporting the feasibility and acceptability of remote monitoring for capturing both subjective and objective data from various sources for respiratory diseases. The high engagement level observed with passively collected data suggests the potential of wearables for long-term, user-friendly remote monitoring in respiratory disease management. The unique piloting of certain features such as protocol holidays, alert notifications for missing data, and flexible support from the study team provides a reference for future studies in this field.
