Introduction: Technology that enables the orthopaedic surgeon to deliver a surgical plan with precision and reproducibility continues to evolve. Robotic TKA represents the next phase of technological development in knee arthroplasty. Multiple propriety robotic platforms are now available for use in TKA, and one such system is the OMNIBotics platform. We conducted a review of this system, reporting its history and principles of use, published outcomes and our own personal experience performing OMNI robotic-assisted TKAs. Materials and methods: A literature review was conducted using the PRISMA guidelines. Thirteen papers were included for the final review. Results: The OMNIBot is an accurate and consistent delivery tool in TKA surgery and compares favourably to instrumented, navigation-assisted and patient-specific cutting guides. The OMNIBot has been shown to be a reliable tool for delivering different alignment philosophies as well as planning and achieving tibio-femoral coronal balancing. The utility of the system is increased when the robot is used in conjunction with a soft-tissue tensioning device—the BalanceBot. Data regarding PROM's are limited, however results to date show reliable outcomes. Survival analysis of RATKA using the OMNIBot is limited to one study which reported 99.26% survivorship at 3 years. We report our own outcomes using the OMNIBot, having performed 766 TKA's using the OMNIBot, since 2014 with 99.48% survivorship at 6 year follow-up. Conclusion: The OMNIBot platform is an imageless, passive robotic system available since 2007, with over 30,000 TKA's being performed with its assistance. It has a small physical footprint, is relatively inexpensive and time efficient. Our review demonstrates a high level of precision of the surgical planning, with a modestly improved accuracy compared to conventional and navigation technology. Metformin therapy is limited in patients with chronic kidney disease ( CKD) due to the potential risk of lactic acidosis. This open-label observational study investigated metformin and lactate concentrations in patients with CKD (n = 22; creatinine clearances 15-40 ml/min) and in two dialysed patients. Patients were prescribed a range of metformin doses (250-2000 mg daily) and metformin concentrations were compared with data from healthy subjects (scaled to 1500 mg twice daily). A subset of patients (n = 7) was controlled on low doses of metformin (250 or 500 mg daily). No correlation between metformin and lactate concentrations was observed. Three patients had high lactate concentrations (>2.7 mmol/l) and two had high metformin concentrations (3-5 mg/l), but none had any symptoms of lactic acidosis. Reducing metformin dosage and monitoring metformin concentrations will allow the safe use of metformin in CKD, provided that renal function is stable. [ABSTRACT FROM AUTHOR]

Clinical exome sequencing (CES) is increasingly being used as an effective diagnostic tool in the field of pediatric genetics. We sought to evaluate the parental experience, understanding and psychological impact of CES by conducting a survey study of English‐speaking parents of children who had diagnostic CES. Parents of 192 unique patients participated. The parent's interpretation of the child's result agreed with the clinician's interpretation in 79% of cases, with more frequent discordance when the clinician's interpretation was uncertain. The majority (79%) reported no regret with the decision to have CES. Most (65%) reported complete satisfaction with the genetic counseling experience, and satisfaction was positively associated with years of genetic counselor (GC) experience. The psychological impact of CES was greatest for parents of children with positive results and for parents with anxiety or depression. The results of this study are important for helping clinicians to prepare families for the possible results and variable psychological impact of CES. The frequency of parental misinterpretation of test results indicates the need for additional clarity in the communication of results. Finally, while the majority of patients were satisfied with their genetic counseling, satisfaction was lower for new GCs, suggesting a need for targeted GC training for genomic testing. The four members of the ErbB family of receptor tyrosine kinases are involved in development and tumorigenesis of the mammary gland. Whereas the epidermal growth factor receptor, ErbB2 and ErbB3 are positively associated with various cancers, clinical studies of ErbB4 in breast cancer are contradictory. Results from tissue culture analyses and some clinical studies suggested that ErbB4 is either a tumor suppressor or is a negative regulator of ErbB2-driven tumors. Neu-Cre-ErbB4flox/null mice in which ErbB4 was inactivated by Cre-lox-mediated recombination in the mammary gland developed MMTV-Neu-driven mammary tumors with a similar latency period to mice with one or two wild-type ErbB4 alleles. Moreover, there was no difference in the histologies of tumors that developed, nor in the propensity to form lung metastases.
