Abstract Skeletal muscle tissue has the largest mass in the human body, accounting for 45% of the total weight. Muscle injuries can be caused by bruising, stretching or laceration. The current classification divides these injuries into mild, moderate and severe. The signs and symptoms of grade I lesions are edema and discomfort; grade II, loss of function, gaps and possible ecchymosis; and grade III, complete rupture, severe pain and extensive hematoma. The diagnosis can be confirmed by ultrasound, which is dynamic and cheap, but examiner dependent; and magnetic resonance imaging (MRI), which provides better anatomical definition. The initial phase of the treatment consists in protection, rest, optimal use of the affected limb, and cryotherapy. Nonsteroidal anti-inflammatory drugs (NSAIDs), ultrasound therapy, strengthening and stretching after the initial phase and range of motion without pain are used in the clinical treatment. On the other hand, surgery has precise indications: hematoma drainage and muscle-tendon reinsertion and reinforcement.

In this review, the authors discuss available MR imaging techniques for evaluating the morphologic and compositional characteristics of articular cartilage in clinical settings related to football players. Applications of each technique for monitoring and assessing cartilage injury in athletes and supervising the effects of treatments are discussed in detail. In addition, we've summarized studies that evaluated the presence of chondral lesions or their repair in football players.

ABSTRACT Objective: To evaluate the nature and rate of sports injuries in medical students, as well as the risk factors at these events. Methods: All student-athletes (218) from a Medical School, integrated in at least one of the six team sport modalities (soccer, rugby, indoor soccer, handball, basketball, and volleyball) in 2017, were included. Injuries affecting their performance, regardless of time loss, were included. Athlete-exposure (A-E) was defined as one student-athlete participating in one practice or game. Results: Injury rates were significantly higher in junior medical students (1st - 3rd year) (7.58 per 1000 A-E, 95%CI = 6.11-9.06) than in senior medical students (4th - 6th year) (4.49 per 1000 A-E, 95%CI = 3.26-5.73) (p < 0.001). Multi-sports athletes had higher injury rates (10.69 per 1000 A-E, 95%CI = 8.22-13.17) than single-sport athletes (4.49 per 1000 A-E, 95%CI = 3.51-5.47) (p = 0.002). More than 60% of reported injuries occurred in the lower limbs and the mechanism that accounted for most injuries in games was player contact (51%); whereas in practice, it was non-contact (53%). Conclusion: Junior medical students present a higher injury rate than seniors. Medical students practicing more than one modality had a higher injury rate than those involved in just one sport modality. Level of Evidence IV, Cross-Sectional Study.

ObjectiveTo evaluate thermography application as a complementary method in preventing muscle injury in professional soccer players. Methods A longitudinal prospective study with 28 professional soccer players that composed a first division of Brazilian’s soccer team between 2015 and 2016. In both seasons (2015 and 2016), muscle injuries were documented and classified in grade of severity, by ultrasound. During the following season (2016), infrared medical thermography was applied twice a week (48 hours after game) and if a difference of temperature was detected higher than 0.4°C, a prevention protocol was initiated. Muscle injuries in 2016 were documented.ResultsIn 2015, the total number of muscle injuries was 11. In 2016, the total number of muscle injuries was 4 (p=0.04). It represents an incidence/player of 78% in 2015 and 28% in 2016, corresponding to a decrease of 64% in 2016. Seven players played in the first team in both seasons. Among these seven players, muscle injuries were reduced from 8 (in 2015) to 3 (in 2016)—a decrease of 63% in the season we used thermographic monitoring (p=0.06).ConclusionThe pilot data provide a promising catalyst for a rigorous RCT that could examine whether thermography can contribute to a muscle injury prevention programme.

ABSTRACT Objectives Several cases of bilateral diffuse pigmented villonodular synovitis (PVNS) or tenosynovial giant cell tumor have been described in the literature. Nevertheless, some presentations are rare and differential diagnoses are necessary. Methods The purpose of this study was to perform a systematic review of the literature related to PVNS and to report a rare supra-patellar bilateral and focal presentation. We performed a systematic data review in the Pubmed Clinical Queries database using MeSH and keywords related to PVNS and tenosynovial giant cell tumor. Results Two cases of bilateral and local PVNS had been previously described, but neither was localized in the supra-patellar compartment. To our knowledge, this case report is the first to describe supra-patellar bilateral and localized PVNS of the knee. This case involves a 28 -year-old woman with bilateral localized PVNS of the supra-patellar recess of the knee. MRI showed a low-signal intensity nodule in T1- and T2-weighted images. These were associated with hemosiderin pigmentation. Conclusion The most important finding of the case reported is related to rarity and location. Histopathology analysis confirmed a rare case of hemosiderin pigmentation in the capsular nodule with internal non-pigmented villous content. Lipoma arborescens in the supra-patellar form must be ruled out as a differential diagnosis since it occurs in the same site. Level of Evidence IV; Case series.

ABSTRACT Objective To clinically and radiologically evaluate patients who received meniscal suture using the outside-in technique, comparing magnetic resonance imaging (MRI), arthro-magnetic resonance imaging (arthro-MRI), and arthro-computed tomography (arthro-CT) to evaluate the healing of meniscal sutures. Methods We evaluated eight patients with an average follow-up of 15 months. The evaluation analyzed clinical parameters using the Lysholm and IKDC scores as well as MRI, arthro-MRI, and arthro-CT imaging. Results At the end of the follow-up period, mean Lysholm score was 89.5 and mean IKDC score was 78.6. In the MRI, signs of meniscal healing were observed in 50% of the cases. The arthro-MRI and arthro-CT showed signs of healing in 75% of cases. There was a positive correlation between arthro-MRI and arthro-CT results in all the cases studied (kappa correlation index=1). Conclusion Meniscal suture using the outside-in technique presented good or excellent results in 87.5% of our patients. The arthro-CT and arthro-MRI showed the same level of accuracy in detecting healing of the sutured region of the meniscus. Level of Evidence IV; Case series.

ABSTRACT Objective: To identify the incidence of injuries, their main characteristics, and the way they were managed throughout 2016 in two major series of a professional soccer championship in São Paulo, Brazil. Methods: This prospective study used an electronic questionnaire previously developed by the Medical Committee of the Paulista Soccer Federation which was sent to the team doctors after each match. Results: Two hundred and fifty-nine injuries occurred during 361 matches, and the incidence of injury per 1000 hours of game play was 21.32. Strikers were the most affected by injury; the most frequent diagnosis was muscle injury and the legs were predominantly affected. Most of the injuries occurred in the last 15 minutes of the first half and only 7.7% required surgical treatment. Conclusions: Muscle injuries were the most frequent, with most occurring in forwards and in the legs. Approximately half of the injuries occurred after contact and the vast majority was treated without surgery. MRI was the most requested exam and most injuries were classified as moderate (8 to 28 lost play days). ABSTRACT Objective: To obtain information on the incidence and nature of sports injuries at a Brazilian university. Method: Data from 396 student amateur athletes (61% male) playing 15 different sports during the 2013 season were retrospectively evaluated. Subjects completed the National Collegiate Athletic Association Injury Surveillance System questionnaire at the conclusion of the 2013 sports season. Injuries that resulted in at least one day of time lost were included. Exposure was defined as one student amateur athlete participating in one practice or game and is expressed as an athlete-exposure (A-E). Results: Injury rates were significantly greater in games (13.13 injuries per 1000 A-Es, 95% CI = 10.3-15) than in practices (4.47 injuries per 1000 A-Es, 95% CI = 3.9-5.1). The mechanisms that accounted for the most injuries in games and practices were player contact (52.9%) and non-contact (54.5%), respectively. Ankle ligament sprains were the most common injury (18.2% of all reported injuries). A relatively high incidence of anterior cruciate ligament injury was also observed (0.16 injuries per 1000 A-Es). Conclusion: Brazilian student amateur athletes are at great risk of sustaining non-contact injuries such as ankle sprains and anterior cruciate ligament injuries. ABSTRACT Objective: To compare the transparent 3D computed tomography (CT) image protocol against conventional 3D-CT image-rendering protocol to assess femoral tunnel position in anatomic anterior cruciate ligament (ACL) reconstructions . Methods: Eight knee CT scans from cadavers were analyzed by image rendering 3D-CT protocol, using Rhinoceros(r) software. The central point of the ACL tunnel was set using the sagittal plane. Same CT scans were analyzed using transparent 3D-CT measurement protocol with OsiriX(r) software. Central point of the ACL tunnel was set using sagittal, coronal and axial planes. The grid system described by Bernard and Hertel was used to compare tunnel positions between protocols, using height and length parameters . Results: There was a significant difference between measurements using image rendering 3D-CT and transparent 3D-CT protocol for height (23.8 ± 7.9mm and 33.0 ± 5.0mm, respectively; p=0.017) and no differences for length (18.6 ± 4.2mm and 18.3 ± 4.5mm, respectively; p=0.560) . Conclusion: Height in transparent CT protocol was different and length was the same as compared to 3D-CT rendering protocol in Bernard and Hertel method for tunnel measurements. ABSTRACT Objective: Several studies have already described the main injuries to soccer players during FIFA World Cup events; however, little is known about the main reasons spectators require medical assistance during these matches. The aim of this study is to assess the number of cases and main reasons spectators required medical assistance during the 2014 World Cup matches in Brazil. Methods: Data were collected from spectators who received medical assistance on all game days, and factors related to the assistance provided were analyzed. Results: Medical assistance was given to spectators in a total of 6,222 cases during the 64 games played in Brazil, an average of 97.2 times per game. The total number of spectators removed from the stadiums by ambulance was 167, a mean of 2.6 removals per game. The main reasons spectators required medical assistance during the World Cup games were headache, gastrointestinal problems, and trauma. Conclusions: Most spectators required medical assistance during the World Cup games for headache, gastrointestinal problems, and trauma; this information is fundamental to develop new prevention strategies and plan medical assistance for large-scale events. Although anterior cruciate ligament (ACL) reconstruction is considered a successful procedure in restoring knee stability, few studies have addressed the issue of aerobic capacity after ACL surgery. Soccer players need technical, tactical and physical skills to succeed, such as good knee function and aerobic capacity. Our purpose is to evaluate aerobic fitness in ACL injured professional football players and six months after ACL reconstruction compared to a control group. Twenty athletes with ACL injury were evaluated and underwent ACL reconstruction with hamstrings autograft, and were compared to twenty healthy professional soccer players. The methods used to evaluate aerobic fitness were maximum oxygen uptake (VO2max) and ventilatory thresholds with a treadmill protocol, before and six months after surgery, compared to a control group. Knee function questionnaires, isokinetic strength testing and body composition evaluation were also performed.Median ACL-injured patients age was 21 years old, and controls 20.5 years old. (n.s.). Preoperative VO2max in the ACL injured group was 45.2 ± 4.3 mL/kg/min, postoperative 48.9 ± 3.8 mL/kg/min and controls 56.9 ± 4.2 mL/kg/min. (p< .001 in all comparisons). Body composition evaluation was similar in all situations. Knee function questionnaires and quadriceps peak torque deficit improved after surgery but were significantly lower compared to controls.Aerobic fitness is significantly reduced in professional soccer players with ACL injury, and six months of rehabilitation was not enough to restore aerobic function after ACL reconstruction, compared to non-injured players of the same level.

Shockwave treatment is an option within orthopedics. The exact mechanism through which shockwaves function for treating musculoskeletal diseases is unknown. The aim of this study was to make a qualitative analysis on the effectiveness of shockwave treatment among patients with musculoskeletal pathological conditions and pseudarthrosis. Searches were conducted in the Cochrane Library, Medline and Lilacs databases. Thirty-nine studies that reported using shockwave treatment for musculoskeletal diseases were found. Their results varied greatly, as did the types of protocol used. The studies that evaluated the effectiveness of shockwave treatment for lateral epicondylitis, shoulder tendinopathy, knee osteoarthrosis, femoral head osteonecrosis and trochanteric bursitis reported inconsistent results for most of their patients. Those that evaluated patients with calcifying tendinopathy, plantar fasciitis, Achilles tendinopathy, patellar tendinopathy and pseudarthrosis showed benefits. Shockwave treatment is a safe and non-invasive method for chronic cases in which conventional techniques have been unsatisfactory and should be used in association with other treatment methods for tendinopathy. Further quality studies are needed.

OBJECTIVE: Develop an epidemiological study of injuries occurred among male professional football players during the Copa America 2011, held in Argentina. METHODS: We conducted a retrospective study of injuries sustained during the 43rd edition of the Copa America football in Argentina, in 2011. The lesions were evaluated by the medical department of the selections and reported to the CONMEBOL. The data were compiled and reported in accordance with rules established by the FIFA Medical Assessment and Research Centre (F-MARC) in 2005. RESULTS: There was a higher prevalence of lesions in the lower limbs. Thighs and knees were the most affected segments. The most frequent diagnoses were muscle injuries. The injuries were mostly minor degrees of severity and there was little difference in the prevalence of lesions according to the stages of the match, with slight predominance in the final 15 minutes. The incidence of lesions per 1,000 game hours was similar to the average found in the literature. CONCLUSIONS: The results obtained allowed us to outline a profile of the prevalence, distribution per body segment, minute in which occurred and severity of injuries in professional football players of participating teams in the Copa America 2011 in Argentina. The extreme rigor of referees may be partly attributed to the highly competitive nature of international tournaments. However, this results cannot be considered definitive because of the need to be compared to other epidemiological studies with same design using similar concepts and criteria. OBJETIVO: Desenvolver estudo epidemiológico sobre as lesões ocorridas entre os jogadores profissionais de futebol masculino durante a Copa América de 2011, na Argentina. MÉTODOS: Fez-se estudo retrospectivo das lesões ocorridas durante a 43ª edição da Copa América de Futebol na Argentina, em 2011. As lesões foram avaliadas pelo departamento médico das seleções e reportadas à Conmebol. Os dados foram compilados e descritos de acordo com as normas estabelecidas pelo Centro de Pesquisas e Avaliações Médicas (F-Marc) da Fédération Internationale de Football Association (Fifa) em 2005. RESULTADOS: Ocorreu maior prevalência de lesões nos membros inferiores, sendo coxas e joelhos os segmentos mais acometidos. Os diagnósticos mais frequentes foram lesões musculares. As lesões apresentaram em sua maior parte grau leve de severidade e ocorreu pequena diferença na prevalência de lesões de acordo com as etapas da partida, havendo discreto predomínio nos 15 minutos finais. A incidência de lesões por 1.000 horas de jogo foi similar à média encontrada na literatura. CONCLUSÕES: A análise dos resultados obtidos permitiu traçar um perfil da prevalência, distribuição por segmento corporal, minuto em que ocorreram e severidade das lesões dos jogadores de futebol profissional das seleções participantes da Copa América de 2011 na Argentina. O maior rigor dos árbitros pode ser em parte atribuído à grande competitividade de torneios internacionais. Entretanto, esses resultados não podem ser considerados definitivos em virtude da necessidade de ser comparados a outros estudos epidemiológicos de mesmo espectro que usem conceitos e critérios semelhantes.

Os sinais e sintomas das lesões grau I são edema e desconforto; grau II, perda de função, gap e equimose eventual; grau III, rotura completa, dor intensa e hematoma extenso. O diagnóstico pode ser confirmado por: ultrassom - dinâmico, barato, porém examinador-dependente; tomografia ou ressonância magnética - maior definição anatômica, porém estático. A fase inicial do tratamento se resume ao protocolo PRICE. AINH, ultrassom terapêutico, fortalecimento e alongamento após a fase inicial e amplitudes de movimento sem dor são utilizados no tratamento clínico. Alterações fisiológicas que ocorrem no aparellho locomotor devido ao envelhecimento como perda de massa muscular, perda do equilíbrio corporal, diminuição da massa óssea e osteoartrose causam limitações às atividade da vida diária do idoso, comprometendo sua qualidade de vida e o tornando mais frágil e dependente. Idosos que realizam atividade física periodicamente tem melhor independência funcional e melhor qualidade de vida do que aquele sedentário. Esse artigo aborda as principais alterações fisiológica do processo de envelhecimento e realiza uma revisão da literatura atual sobre os efeitos que o exercício físico causa no aparelho locomotor do idoso, especificando qual a melhor forma de prescrever atividade física nessa faixa etária.Physiological changes taking place on the locomotive apparatus as a result of aging, such as muscular mass loss, body balance loss, reduced bone mass and osteoarthrosis cause limitations to the daily activities of elderly people, compromising their quality of life and making them weaker and dependent. Aged people who regularly practice physical activities have a higher level of functional independence and a better quality of life than the sedentary ones. This article addresses the key physiological changes with aging and provides a review of current literature about the effects of physical exercises on the locomotive apparatus of elderly individuals, specifying the best ways to prescribe physical exercises to this age group.

OBJECTIVE: To investigate whether supplementation of carbohydrate together with peptide glutamine would increase exercise tolerance in soccer players. METHODS: Nine male soccer players (mean age: 18.4 ± 1.1 years; body mass: 69.2 ± 4.6 kg; height: 175.5 ± 7.3 cm; and maximum oxygen consumption of 57.7 ± 4.8 ml.kg-1.min-1) were evaluated. All of them underwent a cardiopulmonary exercise test and followed a protocol that simulated the movements of a soccer game in order to evaluate their tolerance to intermittent exercise. By means of a draw, either carbohydrate with peptide glutamine (CARBOGLUT: 50g of maltodextrin + 3.5g of peptide glutamine in 250 ml of water) or carbohydrate alone (CARBO: 50g of maltodextrin in 250 ml of water) was administered in order to investigate the enhancement of the soccer players' performances. The solution was given thirty minutes before beginning the test, which was performed twice with a one-week interval between tests. RESULTS: A great improvement in the time and distance covered was observed when the athletes consumed the CARBOGLUT mixture. Trata-se de um relato de caso de tumor de células gigantes de bainha do ligamento cruzado anterior, uma localização extremamente rara para esse tipo de lesão. O paciente do sexo feminino apresentava dor no joelho, sem relato de trauma anterior. Foi submetido ao exame clínico, ao estudo radiográfico e de ressonância magnética da região. Feita a hipótese diagnóstica de TGC de Bainha, o paciente foi então tratado com ressecção artroscópica do tumor. O diagnóstico foi confirmado com exame anátomo-patológico. O paciente evoluiu bem, com melhora dos sintomas referidos no pré-operatório.The author presents a case report of Tumor Giant Cells (TGC) localized on the anterior cruciate ligament sheath, an extremely rare site for this kind of lesion. A 37 y-o female patient presented with knee pain, with no history of previous trauma. She underwent clinical examination, X-ray study and magnetic resonance of the region. The diagnostic hypothesis of Sheath TGC was provided, and the patient was treated with tumor arthroscopy resection. Diagnosis was confirmed by anatomicopathological examination. By the end point assessment, none of the pre-operative symptoms were reported.

Abstract Objective To compare the accuracy of ultrasound (US) with that of magnetic resonance imaging (MRI) in the detection of aponeurosis lesions of the rectus abdominis/adductor longus muscles, to study the characteristics of the athletes and imaging findings associated with pubalgia, and to demonstrate the importance of each method in evaluating this condition. Materials and methods The present study was conducted from 2011 to 2016 with 39 professional soccer players: 15 with pubalgia and 24 without pubalgia. Age, field position, bodymass index (BMI), weekly training load, career length, and history of thigh/knee injury and lower back pain were recorded. The following tests were performed: radiographs (anteroposterior view of the pelvis in standing and flamingo positions) to evaluate hip impingement, sacroiliac joint, and pubic symphysis instability; US to analyze the common aponeurosis of the rectus abdominis/adductor longus muscles and inguinal hernias; and MRI for pubic bone degenerative alterations and edema, and lesions in the adductor and rectus abdominis muscles and their aponeurosis. Results There was an association between pubalgia, high BMI (p = 0.032) andmuscle alterations (p < 0.001). Two patients with pubalgia had inguinal hernias and one patient with pubalgia and two controls had sports hernias. Pubic degenerative changes were frequent in both groups. Aponeurosis lesions weremore frequent in patients with pain. The US detection had 44.4% sensitivity and 100% specificity. Conclusion The evaluation of athletic pubalgia should be performed with radiography, US, and MRI. High BMI, muscle injuries, geodes, and osteophytes are findings associated with pubalgia; US has low sensitivity to detect injuries of the common aponeurosis of the rectus abdominis/adductor longus muscles.

CONTEXT: A ganglion is a cystic formation close to joints or tendinous sheaths, frequently found in the wrist, foot or knee. Intra-articular ganglia of the knee are rare, and most of them are located in the anterior cruciate ligament. The clinical picture for these ganglia comprises pain and movement restrictions in the knee, causing significant impairment to the patient. Symptoms are non-specific, and anterior cruciate ligament ganglia are usually diagnosed through magnetic resonance imaging or arthroscopy. Not all ganglia diagnosed through magnetic resonance imaging need to undergo surgical treatment: only those that cause clinical signs and symptoms do. Surgical results are considered good or excellent in the vast majority of cases. CASE REPORT: A 29-year-old male presented with pain in the left knee during a marathon race. Physical examination revealed limitation in the maximum range of knee extension and pain in the posterior aspect of the left knee. Radiographs of the left knee were normal, but magnetic resonance imaging revealed a multi-lobed cystic structure adjacent to the anterior cruciate ligament, which resembled a ganglion cyst. The mass was removed through arthroscopy, and pathological examination revealed a synovial cyst. Patient recovery was excellent, and he resumed his usual training routine five months later.

ABSTRACT Objective: To compare anatomic anterior cruciate ligament (ACL) reconstruction between two tunnel positions in knees with isolated ligament tears. Methods: Anatomic ACL reconstruction was performed, from hip-to-toe, on 15 fresh cadaveric specimens. No associated lesions were created to enhance knee instability. The protocol was conducted in three states: (1) complete isolated ACL deficiency; (2) anatomic femoral and tibial anteromedial ACL reconstruction (AM REC); and (3) anatomic femoral and tibial central ACL reconstruction (Central REC). The reconstruction protocols were randomly assigned. The continuous mechanized pivot-shift test was recorded dynamically with a tracking system. Results: The Central REC group showed a smaller degree of internal rotation (0.6° ± 0.3° vs. 1.8° ± 0.3°, respectively, P < 0.05) and no difference in anterior translation (4.7 mm ± 0.4 mm vs. 4.5 mm ± 0.4 mm, respectively, P > 0.05) in the pivot-shift test, compared with the AM REC group. Conclusion: The central anatomic ACL reconstruction resulted in greater restriction of internal rotation than the anteromedial anatomic ACL reconstruction. Experimental Study on Cadaver.

ABSTRACT Objective: To determine the frequency of hip pain in competitive bodybuilders over three different bodybuilding competitions. Methods: This study evaluated bodybuilders recruited from three competitions during the year of 2016. All participants provided their informed consent and the study received IRB approval. Training routine, health condition, level of success on competitions, history of hip pain and physical examination of the hip were evaluated. Results: 113 bodybuilders were evaluated, mean age was 30.5 ± 8.65 years and mean BMI was 25.2 ± 3.65 kg/m2. Mean values for hip flexion, adduction, abduction, internal rotation, external rotation and distance between the knee and the table (FABER distance) were 116 ± 13, 23 ± 8, 71 ± 12, 40 ± 10, 36 ± 9 and 19 ± 4, respectively. Eight (7%) participants presented hip pain within the week prior to examination and only 2 (1,7%) presented with anterior impingement sign. None of the athletes who reported hip pain interrupted their physical training or performance. Conclusion: Symptomatic athletes continued their training program under the presence of hip pain. The frequency of hip pain among bodybuilders is high and may be underestimated in this study. Level of Evidence IV, Case series.
