Introduction: This study evaluated hand and forearm injuries caused by angle grinders and identify the risk factors. Methods: Between 2020 and 2022, 79 patients with hand and forearm injuries due to angle grinder were retrospectively evaluated. Data were obtained from the hospital database and telephone interviews with patients. Age, gender, education level, employment status and work experience, smoking, and alcohol use were recorded. The injury location and structures were evaluated. The time of injury and use of protective gloves were recorded. The Modified Hand Injury Severity Score (MHISS) was used to determine the severity of the injury. The return to work times of actively working patients were recorded. Results: Seventy-nine patients with angle grinder injuries were included in the study. All patients were male. Dominant side injury was in 17 (21.5%) patients. The most frequently injured area was the dorsal hand, and the most frequently injured structure was the extensor tendon. The mean MHISS was 49.15 (4-280) and 39 (49.4%) patients had minor injuries. The mean return to work was 7.13 (2-32) weeks. No correlation was found between age, work experience, and MHISS (p=0.167 p=0.389). There was a correlation between the use of protective gloves and MHISS (p=0.002). Conclusion: Injuries to the hand and forearm caused by angle grinder usage can result in serious work-related disabilities. These injuries can be reduced by increasing safety precautions and training.

Difficult airway is a serious condition that can be fatal and is frequently encountered during general anesthesia applications. Tracheobronkopathy osteochondroplasty (TBO) is a rare benign disease that is one of the causes of unpredictable difficult airway. A 45-year-old male patient with no comorbidities was transferred to the operating room for elective cholecystectomy. He couldn’t be intubated after standard monitoring and induction of anesthesia. The patient, who had papillomatous lesions in the trachea detected by bronchoscopy, was awakened to be scheduled for rigid bronchoscopy. TBO was detected in pathological examination of the samples taken in rigid bronchoscopy. TBO is a rare benign disease. Its etiology is unknown. It may be asymptomatic or may present with persistent dry cough, hemoptysis, dyspnea, recurrent lower respiratory tract infection, atelectasis and difficult intubation. Some patients may be diagnosed for the first time in the operating room due to difficult intubation.In cases with advanced tracheal stenosis, invasive airway intervention may be required or it may have a fatal course. The use of a laryngeal mask in the perioperative period may be an ideal option in a patient known to have TBO before. Unexpected difficult airway management in the operating room is important for anesthesiologists. According to the difficult airway society algorithm, waking the patient from a planned operation is the safest way until the problem is detected. We did not need invasive intervention because our patient also had elective surgery and ventilation was not difficult. In addition, we think that it is an important decision to make a diagnosis using fiberobtic bronchoscopy in operating room conditions and to wake up our patient with sugammadex without repeated attemps of tracheal intubation and delay elective surgery. In coclusion, it should be kept in mind that TBO may be one of the unpredictable causes of difficult intubation in the operating room.

Günlük yaşamda önemli bir yer tutan haber; gündemle, çevrede gerçekleşen olaylarla, farklı gelişmelerle ilgili bilgilenmeye yardımcı ve hatta alınan kararlara etki edebilme potansiyeli olan bir bilgi türü şeklinde tanımlanabilir. Dijital ortamların kullanımının ve kullanıcı etkileşiminin artmasıyla, alışılagelmiş gazetecilik bakış açısıyla yapılan haber tanımı sınırlı kalabilmektedir. Bu durum, değişen medya ortamına ve bunun etkilerine odaklanan haber okuryazarlığı eğitiminde haberin nasıl tanımlanması gerektiği sorusunu gündeme getirmektedir. Bu noktada, haber kullanıcılarının haberi nasıl algıladıkları ve nasıl tanımladıklarını anlamaya çalışmak önemlidir. Bu çalışmada, bir grup lisans öğrencisinin eleştirel düşünmeyi temel alan haber okuryazarlığı eğitimi almadan önce ve aldıktan sonraki haber algılarının değerlendirilmesi amaçlanmıştır. Yarı deneysel araştırma tasarımı kullanılarak, eğitim öncesi ve sonrasında öğrencilere öz değerlendirme testleri ile ön ve son testler uygulanmıştır. Bu testlerle öğrencilerin, haber kavramını bilme bakımından kendilerini nasıl değerlendirdikleri, haberi farklı örnekler içerisinde ayırt edip edemedikleri ve nasıl tanımladıklarından yola çıkılarak haber algıları anlaşılmaya çalışılmıştır. Sonuçta, haber kavramını bilme yönünden öğrenciler kendilerini özellikle eğitim sonrasında daha yüksek puanlarla değerlendirmişlerdir. Farklı içeriklere yönelik örnekler arasında haberi ayırt etme bakımından da öğrencilerin genelde iyi bir performans sergiledikleri ancak eğitim sonrasında yarıdan fazlasının köşe yazısı türündeki örneği de haber olarak işaretledikleri belirlenmiştir. Ayrıca, öğrencilerin eğitim sonrasındaki tanımlarında, “doğru”, “güvenilir”, “kesin”, “5N1K kuralına uygun” gibi haber niteliklerine odaklanan ifadeler kullandıkları görülmüştür. Tüm bunlardan hareketle, haber kullanıcılarının bir içeriğe haber niteliği atfederken haber okuryazarlığının temelini oluşturan eleştirel düşünme becerilerini kullanmalarının önemli olduğu söylenebilir. Bu becerilerin bireylere kazandırılabilmesi, yaşam boyu öğrenme pratiklerine uyarlanmış, zamana yayılmış ve farklı seviyelere uygun bilgi, haber gibi okuryazarlık türleri ya da eleştirel düşünmeye yönelik olarak tasarlanacak eğitimlerle mümkün hale gelebilir.

Objective: This study aimed to determine the efficacy and long-term outcomes of pyeloplasty in children with poorly functioning kidneys. Materials and Methods: Twenty-six patient charts were reviewed who underwent pyeloplasty with poorly functioning kidneys from 2008 to 2020. Patients were divided into two groups based on DRF; Group 1: 0-10%, and Group 2: between 10-30%. Patient demographics, preoperative and postoperative anteroposterior diameter (APD), parenchymal renal thickness (PT) ratio, and differential renal function (DF) were analyzed to confirm the postoperative benefits and potential predictors of renal functional recoverability. The parameters of patients younger than one year of age were also compared to those of older patients. Results: The renal function of 12 of 26 patients’ was 0.0001). The postoperative PT ratio and postoperative DF were increased in Group II, but not in Group I. DF and PT ratios also improved in Group II in patients younger than 1 year of age (p=0.014, p=0.032 respectively). Hypertension was detected in 5 patients (41.6%) during follow-up in Group I. We report the case of a patient with lymphedema who experienced an osteoporotic vertebral fracture following lumbar sympathetic ganglion block (LSGB). A 66-year-old woman with intractable lymphedema of the right leg went through surgery for cervix cancer. Because she was resistant to conventional treatments, we planned to commence LSGB treatment. Thus, she received two sessions of LSGB administered with a steroid and local anesthetic mixture. After the second session, the patient underwent severe lumbar pain. After thorough anamnesis and physical examination, we decided to use magnetic resonance imaging to rule out the possibility of metastasis. Contrast-enhanced magnetic resonance imaging, which was assessed by an expert spine radiologist, revealed an osteoporotic fracture of the L4 vertebra. The patient’s severe pain considerably diminished in the third month after beginning antiosteoporotic and analgesic medications, orthosis, and exercises. We speculate that the use of steroids in LSGB may lead to osteoporotic vertebral fracture.

Bu çalışmada, 2017M1-2022M1 dönemleri arasındaki veriler kullanılarak Bitcoin (BTC) ile Karbon Emisyonu (CO2) arasındaki ilişki incelenmiştir. Son zamanlarda yapılan çalışmalara istinaden kripto para ve enerji piyasalarının spekülatif ve kırılgan yapıya sahip olduğu ve bundan dolayı değişkenlerin doğrusal olmayan bir forma sahip olabileceği konusuna dikkat çekildiği gözlenmektedir. Dolayısıyla bu bilgiler çerçevesinde çalışmada öncelikle Luukkonen vd. (1988), Harvey vd. (2008) doğrusallık testi ve Kapetanios vd. (2003) doğrusal olmayan birim kök testi ile değişkenlerin doğrusallık sınaması yapılmaktadır. Akabinde değişkenlerin doğrusal olmayan forma sahip olduğu tespit edildiği için çalışmada Kapetanios vd. (2006) Doğrusal Olmayan Eşbütünleşme analizi kullanılmaktadır. Kapetanios vd. (2006) testi bulgularına göre BTC ile CO2 arasında uzun dönemde doğrusal olmayan bir eşbütünleşme ilişkisi olduğu tespit edilmektedir. Bu durum BTC ile CO2 arasındaki ilişkinin uzun dönemde dengeye doğrusal olmayan bir şekilde yakınsadığı sonucunu göstermektedir. Değişkenler arasında doğrusal olmayan eşbütünleşme ilişkisini tespit ettikten sonra bu ilişkinin yönünü belirlemek amacıyla yapılan Granger nedensellik testi sonucuna göre ise Bitcoin’den Karbon Emisyonuna doğru tek yönlü nedensellik olduğu tespit edilmektedir. Bu bulgu, BTC üretiminde kullanılan enerjinin çevre dostu kaynaklardan elde edilmesine yönelik politikaların benimsenmesi gerektiği biçiminde yorumlanabilir.

Aging is a major risk factor for cognitive impairment. Aerobic exercise benefits brain function and may promote cognitive health in older adults. However, underlying biological mechanisms across cerebral gray and white matter are poorly understood. Selective vulnerability of the white matter to small vessel disease and a link between white matter health and cognitive function suggests a potential role for responses in deep cerebral microcirculation. Here, we tested whether aerobic exercise modulates cerebral microcirculatory changes induced by aging. To this end, we carried out a comprehensive quantitative examination of changes in cerebral microvascular physiology in cortical gray and subcortical white matter in mice (3–6 vs. 19–21 months old), and asked whether and how exercise may rescue age-induced deficits. In the sedentary group, aging caused a more severe decline in cerebral microvascular perfusion and oxygenation in deep (infragranular) cortical layers and subcortical white matter compared with superficial (supragranular) cortical layers. Five months of voluntary aerobic exercise partly renormalized microvascular perfusion and oxygenation in aged mice in a depth-dependent manner, and brought these spatial distributions closer to those of young adult sedentary mice. These microcirculatory effects were accompanied by an improvement in cognitive function. Our work demonstrates the selective vulnerability of the deep cortex and subcortical white matter to aging-induced decline in microcirculation, as well as the responsiveness of these regions to aerobic exercise.

Aim: The study aimed to find out whether there is a relationship between the mononeuropathies of the median, ulnar, radial, peroneal, and sciatic nerves and body mass index (BMI). Material and Methods: Patients whose clinical and electrodiagnostic findings were compatible with carpal tunnel syndrome (CTS), ulnar neuropathy at the elbow (UNE), radial neuropathy at the spiral groove (RNS), peroneal neuropathy at the fibular head (PNFH), and sciatic injury due to intramuscular injection (SNIII) were included in this retrospective cohort study. In addition, controls whose clinical and electrodiagnostic features were not compatible with mononeuropathy were included in the study. The BMI values of all participants were analyzed. Introduction: Carpal tunnel syndrome (CTS) is the most common entrapment mononeuropathy and can affect patients' daily living activities. In this study, it was aimed to find out whether there is a relationship between fatigue, neuropathic pain and neurophysiological features in CTS. Material and Method: Patients with clinical features compatible with CTS were included in this retrospective study. Median nerve compound muscle action potential (CMAP) and compound nerve action potential (CNAP) of the patients were analyzed. The Turkish version of the Fatigue Severity Scale (FSS) was administered to all patients. Patients with a mean FSS score of ≥4 were considered to have fatigue. In addition, Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) was applied to some patients. If the LANSS score was 12 or higher, these patients were considered to have neuropathic pain. Results: Thirty-two female and eight male CTS patients were included in the study. Thirty-two patients had clinically bilateral CTS and eight patients had right-sided CTS. Son zamanlarda, çevre durumları ve kirlilik giderici ajanlara ilgi, kaynağı fosil olmayan aktif karbon üretimi ile ilgili araştırmalara gereksinime ivme kazandırdı. Bu nedenle, tarımsal organik lignoselülozik atık malzemeler oldukça dikkat çekmektedir. Çalışmamızda aktif karbon üretiminde nar kabuğu kullanıldı. Nar kabuğundan yüzey alanı yüksek, adsorbsiyon özelliklerine uygun ve gözenek çapı büyük aktif karbon hem fiziksel hem de kimyasal aktivasyon teknikleri kullanılarak elde edilmesi amaçlandı. Kimyasal aktivasyon çalışmalarında ZnCl2 ve AlCl3’ün yüzey alanına etkisi incelendi ve denemeler farklı sıcaklık, süre ve konsantrasyonlar denenerek gerçekleştirildi. Uygun aktif karbon prosesi belirlendi. Sulu çözelti olarak kullanılan meyve suyuna belli derişimde pestisit ve piroliz edilen aktif karbon eklenerek analize alındı. Aktif karbonun, elma suyundaki eklenen pestisiti absorblama kapasitesi ölçüldü. Analiz sonuçlarına göre bu aktif karbonun elma suyuna eklenen pestisitlerin giderimi üzerindeki etkisi incelendi. Meyve suyuna eklenen farklı miktarlardaki pestisitten sonra değişik konsantrasyonlarda aktif karbon ilave edildi. Meyve suyu-pestisit karışımı değişen sürelerde karıştırıldıktan sonra meyve suyu ekstraktı AOAC 2007 Pestisit Analiz Yöntemi ile Quetchers kit kullanılarak analiz edildi. Yapılan deneyler sonucunda aktif karbon üretiminde nar kabuğunun yüksek verimli hammadde olabileceği sonucuna varıldı. Aynı zamanda çalışma koşulları uygun hale getirildiğinde, nar suyu üretiminde atık malzeme olan nar kabuğundan üretilen aktif karbonların meyve suyundan karbendazim pestisitinin uzaklaştırılmasında kullanılabilirliği tespit edildi.

Resumo Fundamento Além da doença arterial coronariana, a lipoproteína de não alta densidade (não-HDL-C) fornece informações preditivas de curto e longo prazo para muitas doenças inflamatórias crônicas, como acidente vascular cerebral, hemodiálise, pós-transplante renal, hepatoesteatose não alcoólica e vírus da imunodeficiência humana. Objetivos Este estudo examinou o valor preditivo do não-HDL-C medido antes do SARS-CoV-2 para mortalidade na infecção por COVID-19. Métodos Este estudo incluiu retrospectivamente 1.435 pacientes diagnosticados com COVID-19 e tratados na enfermaria de doenças torácicas em um único centro entre janeiro de 2020 e junho de 2022. Todos os pacientes incluídos no estudo apresentavam características clínicas e radiológicas e sinais de pneumonia por COVID-19. O diagnóstico de COVID-19 de todos os pacientes foi confirmado por uma reação em cadeia da polimerase estudada a partir de um swab orofaríngeo. A significância estatística foi estabelecida em p < 0,05. Resultados Os pacientes do estudo, incluindo 1.435 indivíduos, foram divididos em 712 pacientes no grupo de não sobreviventes e 723 no grupo de sobreviventes. Embora não tenha havido diferença entre os grupos em relação ao sexo, houve uma diferença de idade estatisticamente significativa. O grupo que não sobreviveu era mais velho. Idade, lactato desidrogenase (LDH), proteína C reativa (PCR), triglicerídeos, D-dímero e não-HDL-C foram fatores de risco independentes para mortalidade em análises de regressão. Na análise de correlação, idade, PCR e LDH foram positivamente correlacionados com não-HDL-C. Na análise ROC, a sensibilidade para não-HDL-C foi de 61,6% e a especificidade foi de 89,2%. Conclusão Acreditamos que o nível de não HDL-C estudado antes da infecção por COVID-19 pode ser usado como um biomarcador prognóstico para a doença.

Background: Broad-spectrum empirical antimicrobials are frequently prescribed for patients with coronavirus disease 2019 (COVID-19) despite the lack of evidence for bacterial coinfection. Aims: We aimed to cross-sectionally determine the frequency of antibiotics use, type of antibiotics prescribed, and the factors influencing antibiotics use in hospitalized patients with COVID-19 confirmed by polymerase chain reaction. Study Design: The study was a national, multicenter, retrospective, and single-day point prevalence study. Methods: This was a national, multicenter, retrospective, and single-day point-prevalence study, conducted in the 24-h period between 00:00 and 24:00 on November 18, 2020, during the start of the second COVID-19 peak in Turkey. Results: A total of 1500 patients hospitalized with a diagnosis of COVID-19 were included in the study. The mean age ± standard deviation of the patients was 65.0 ± 15.5, and 56.2% (n = 843) of these patients were men. Of these hospitalized patients, 11.9% (n = 178) were undergoing invasive mechanical ventilation or ECMO. It was observed that 1118 (74.5%) patients were receiving antibiotics, of which 416 (37.2%) were prescribed a combination of antibiotics. In total, 71.2% of the patients had neither a clinical diagnosis nor microbiological evidence for prescribing antibiotics. In the multivariate logistic regression analysis, hospitalization in a state hospital (p < 0.001), requiring any supplemental oxygen (p = 0.005), presence of moderate/diffuse lung involvement (p < 0.001), C-reactive protein > 10 ULT coefficient (p < 0.001), lymphocyte count < 800 (p = 0.007), and clinical diagnosis and/or confirmation by culture (p < 0.001) were found to be independent factors associated with increased antibiotic use. Conclusion: The necessity of empirical antibiotics use in patients with COVID-19 should be reconsidered according to their clinical, imaging, and laboratory findings.

In this study, the 5S method was applied in the tool room of a company operating in the aviation field and engaged in machining. Preparations for the 5S study were carried out in June 2020, and after this month, production was started according to the 5S method. Outputs such as defective parts rates due to cutting tools, costs due to tool room, machine waiting due to tool supply and tool preparation time were determined as performance evaluation criteria. Within the scope of preparation, first of all, lean production and 5S trainings were given to company employees. Personnel for the area where 5S will be applied have been determined and necessary planning has been made. Sorting, organizing, cleaning, standardization and discipline steps related to 5S have been successfully implemented, respectively. For the recovery data, the outputs of the first half of 2020 and the last half were compared and evaluated. As a result of this study, 66% improvements were observed in the cost of the tool room, 76% in the downtime rate of the machines due to the tool room, 78% in the scrap rate due to the faulty cutting tool, and 85% in the unnecessary movement load rates in the tool room. As a result of these improvement works, the efficiency of production has increased. With the gray relational analysis, where all outputs are evaluated jointly, it has been revealed that the most productive month in terms of production is November, when production is made by applying the 5S approach. Objective: The impact of the coronavirus disease 2019 (COVID-19) pandemic has been unceasingly ongoing worldwide. Recent bioinformatics analysis and epidemiologic studies have highlighted that the functional polymorphisms on the angiotensin converting enzyme (ACE) gene may have an impact on the clinical progress of COVID-19. In this study, we aimed to determine the impact of the ACE1 gene I/D polymorphism and ACE2 peptidase-2 domain variants on disease severity. Methods: Hundred patients with confirmed COVID-19 related pneumonia [50 patients with severe disease in intensive care unit (ICU) and 50 patients not in ICU] were compared on the basis of genetic and clinical characteristics. Genomic DNA was purified from peripheral blood lymphocytes with an automated QIA symphony DSP DNA Mini-Kit. The Sanger sequencing analysis was performed. The frequencies of ACE1 gene polymorphism and ACE2 PD variants were compared in patients hospitalized in ICU and those not in ICU. The Statistical Package for Social Sciences version 22.0 was used for statistical analysis. Results: The sequencing analysis of the ACE2 gene exon 1 and 2 revealed none of the polymorphisms investigated or any other variants in the present cohort. The frequencies of the ACE1 ID, DD, and II genotypes were 51%, 31%, and 18%, respectively. The frequency of the D allele was similar between the ICU and non-ICU groups (50.4% versus 49.6%). Older age and the presence of advanced stage radiologic abnormalities on admission were detected as independent predictors of ICU requirement. Conclusion: No effect of any ACE1 gene polymorphism on predicting ICU requirement was detected. To the best of our knowledge, this is the first study investigating the impact of ACE gene polymorphisms on clinical severity of COVID-19 in a Turkish cohort.
