Complementary medicines (CMs) that are formulated to facilitate weight loss are a popular and widely available treatment option, particularly among women. There is limited research regarding their use in the South African context. The aim of this study was to gather information on the use and perceived effectiveness of CMs for weight loss among adult women. A quantitative, descriptive survey design was used. Johannesburg, South Africa. A purposive sample of women over the age of 18 years who had used CMs for weight loss within the past five years were recruited from health stores and a university campus healthcare centre. Data were obtained through a self-administered 29-item, paper-and-pen questionnaire. Responses were descriptively analysed using cross-tabulation, multiple response analysis and frequency tables. Data from 160 questionnaires were analysed and the results demonstrated that a wide variety of CM products are used for weight loss, with branded combination products, green tea, Garcinia cambogia and conjugated linoleic acid being the most popular. Most participants also adopted lifestyle interventions such as exercise and calorie restriction during their weight loss attempt. While minor side effects were experienced by over half of the participants, the majority were satisfied with their product choice as well as the amount of weight that they lost. Most participants perceived their CM weight loss product of choice to be effective. However, other concomitant weight-loss strategies that were employed may have been responsible for these results. Despite their popularity, there is only very limited research surrounding the safety and efficacy of these weight-loss products, with most studies being conducted on single constituents rather than whole formulations as sold on the market. Healthcare providers should encourage the disclosure of the use of CM weight-loss products to ensure safe and effective patient care and mitigate the potential risks associated with their inappropriate use. Background: The increased popularity of complementary medicine has created the need for patients to receive accurate information from nurses who are front-line healthcare providers. Studies have demonstrated that patients are more likely to discuss other medication and therapy use with nurses, rather than with general practitioners or other health professionals. It is, therefore, important to determine nurses' knowledge and attitudes towards complementary medicine. Aim: The aim of this study was to determine nursing students' knowledge and attitudes toward the use of complementary medicine. Setting: The study was conducted with Baccalaureus Curationis (nursing) students registered at a large public university in Gauteng, South Africa. Methods: This research utilised a quantitative approach with a prospective, descriptive survey design. A convenience sample comprising registered Baccalaureus Curationis nursing students (n = 202) was utilized. Participants completed a 34-item, paper-and-pencil questionnaire to determine their knowledge, personal use and attitude toward complementary medicine modalities. Results: Questionnaires were completed by 126 nursing students with 119 questionnaires included for analysis. Despite a reported lack of knowledge regarding complementary medicine and limited personal use, participants had an overall positive attitude towards complementary medicine. Almost half of the participants reported enquiring about its use in history taking and were willing to refer patients to a complementary medicine practitioner. Conclusion: A positive attitude towards complementary medicine modalities might suggest a willingness from students to improve their knowledge of the various modalities and to refer to a complementary medicine practitioner when indicated. In the rural Western Cape local municipality of Kannaland, the word 'bossiesmedisyne' (lit, bushes medicine), refers to plant and sometimes animal material used to treat and alleviate a wide range of health problems, ranging from colds to cancer. Based on three months of ethnographic fieldwork, the paper advances the argument that the different kinds of healing these medicines provide are inseparable from the sociocultural and environmental contexts in which they are enveloped. It goes on to argue for a balanced interpretation of the meanings ascribed to bossies, based on a dialectical relation between claims to power, and phenomenological experiences of medicines and the landscapes in which they grow. The use of traditional, complementary, and alternative medicine (TCAM) can lead to delays and interruptions in the HIV continuum of care. This study explores reasons for TCAM use in people living with HIV on antiretroviral therapy (ART) in Eswatini and compares interrupted care between different types of TCAM users. Data were collected using surveys in the MaxART study (a test-and-treat trial) between 2014 and 2017 to assess the exposure, namely visiting a TCAM provider. Additionally, visit dates were retrieved from clinic records to assess the outcome, interrupted care. Open-ended questions were analysed with qualitative content analysis (n = 602) and closed questions with bivariable and multivariable analysis (n = 202). Out of 202 participants, 145 (72%) never used TCAM, 40 (20%) ever used, and 17 (8%) is currently using TCAM (diviners, herbalists, and religious healers). No differences in interrupted care were found comparing never (reference category), past (Odds Ratio: 1.31, 95% confidence interval: 0.63–2.72), and current users (1.34, 0.47–3.77), while adjusting for gender, time since HIV diagnosis, and time on ART. Contextual factors affecting the choice for TCAM were the influence of family, advice from the health facility, and religious beliefs. Individual factors include trust in biomedical care, type of illness, no need for additional care, and practical reasons such as financial means. Ethnopharmacological relevance The aim of the study was to identify and document plants traditionally used to manage HIV and treat its opportunistic infections (OIs) in Mpoza, a rural village located in the Mount Frere Alfred Nzo District, Eastern Cape Province, South Africa. Materials and methods: Semi-structured interviews and focus group discussions (FGDs) were conducted with 18 traditional health practitioners from January 2012 to August 2012 to obtain information about medicinal plants used in the management of HIV and treatment of OIs. Results Seventeen plant species belonging to 12 families were identified for the management of HIV and treatment of OIs in Mpoza. The identified plant species belonged mostly to the families Asparagaceae (12%), Araliaceae (12%), Apiaceae (12%), Xanthorrhoeaceae (12%) and Lamiaceae (12%). The remaining 40% of identified plant species was evenly split over seven families - Urticaceae, Hypoxidaceae, Leguminosae, Verbenaceae, Rosaceae, Compositae and Rutaceae. The most frequently used medicinal plants were Hypoxis hemerocallidea (85%), Asparagus densiflorus (68%) and Lessertia frutescens (68%). The leaves (43.5%) and roots (21.7%) were the most frequently used plant parts, usually prepared as infusions and decoctions for oral administration. Conclusion This study provides documentation of medicinal plants used in the management of HIV and treatment of commonly associated OIs, which might provide a potential lead that will significantly contribute in reducing the burden of HIV infections in South Africa. Ethnopharmacological relevance This paper presents results of an ethnobotanical survey of medicinal plants used for the management of candidiasis and related fungal infections in the Venda area, South Africa. Materials and methods Ethnobotanical data about the uses of plants were gathered from eleven rural traditional healers using semi-structured interviews. Results A total of 45 species belonging to 24 different families were identified, of which the dominant family was the Fabaceae with 13 species (28.9%) followed by the Asteraceae and Solanaceae with 3 species each (6.7 %). A total of 28 of these plant species (62.2%) have been shown to have anticandidal activity and 14 species (31%) have been recorded for antifungal uses in the literature. Amongst the 45 species recorded, 51% were trees, 33% were shrubs, and 16% were herbs. The most widely used plant species were Acacia caffra , Clerodendrum glabrum, Croton gratissimus, Elaeodendron transvaalense, Faurea saligna, Hippocratea longipetiolata, Osyris lanceolata, Richardia brasiliensis, Schkuhria pinnata, Schotia brachypetala, Spilanthes acmella, Strychnos potatorum, Vangueria infausta subsp. infausta and Withania somnifera . The plant parts mostly used in the therapeutic preparations were roots (27.7%), bark (23.2%), and a combination of roots, bark (18.7%) and leaves (14.3%). Decoctions (44.4%), infusions (20%), macerations (17.7%), burning (11.4%) and paste (6.5%) were used. Most of the herbal remedies were administered orally. The main factors threatening the conservation status of these plants are unsustainable methods of harvesting, logging for firewood, building materials and crafts. Conclusion The Venda area is rich in plant diversity and local indigenous knowledge of medicinal plants can play an important role as a model for low cost primary health care. Abstract: Ethnopharmacological relevance: Boophone disticha is the most common member of the South African Amaryllidaceae used extensively in traditional medicine of the various indigenous population groups, including the Sotho, Xhosa and Zulu as well as the San. This survey was carried out to identify and highlight areas relevant to the traditional usage of Boophone disticha. Pharmacological aspects were examined with the purpose of reconciling these with the traditional usage of the plant. In relation to phytochemical make-up, particular attention was paid on how its alkaloid constitution might corroborate the various biological effects manifested by the plant. Materials and methods: Information gathering involved the use of four different database platforms, including Google Scholar, ScienceDirect, SciFinder® and Scopus. Arrangement and detailing of this information is as reflected in the various sections of the paper. Results: Sixteen categories were identified under which Boophone disticha finds use in traditional medicine. These were shown to include general usage purposes, such as ‘cultural and dietary’, ‘well-being’, ‘personal injury’, ‘divinatory purposes’, ‘psychoactive properties’ and ‘veterinary uses’. Furthermore, traditional usage was seen to involve six body systems, including functions pertaining to the circulatory, gastrointestinal, muscular, neurological, respiratory and urinary systems. The four remaining categories relate to use for inflammatory conditions, cancer, malaria and tuberculosis. Overall, three areas were discernible in which Boophone disticha finds most usage, which are (i) ailments pertaining to the CNS, (ii) wounds and infections, and (iii) inflammatory conditions. In addition, several aspects pertaining to the toxic properties of the plant are discussed, including genotoxicity, mutagenicity and neurotoxicity. Conclusion: The widespread ethnic usage of Boophone disticha has justified its standing as a flagship for the Amaryllidaceae and its relevance to South African traditional medicine. Furthermore, its promising pharmacological and phytochemical profiles have stimulated significant interest in the clinical realm, especially in the areas of cancer and motor neuron disease chemotherapy. Abstract: Ethnopharmacological relevance: Traditional remedies from plants have been use to treat male reproductive related disorders for ages in South Africa. Aim: This study reviewed the current status of medicinal plants used in male reproductive healthcare as well as their effectiveness as a mode of treatment. Methodology: A detailed literature search was done by consulting books, peer-reviewed papers, scientific databases such as Scopus, Science direct and PubMed, and Google scholars. Keywords such as aphrodisiacs, conception, erectile dysfunction, fertility, infertility and sterility in relation to medicinal plants were used during the search. Plant species were selected based on their traditional use in different aspects of male reproductive healthcare. Results: A total of 61 plants species were found to be used in treating male impotency or as aphrodisiacs. However, only six species have been evaluated scientifically and these plants showed significant activities either in promoting conception by improving fertility and promoting erection or in contraception by decreasing sperm motility, virility and membrane integrity. Conclusion: The importance of herbal products in the treatment of male infertility and related sexual disorders is undeniable. Scientific evaluations of the six species have proved the empirical use of these plants in the improvement of male sexual disorders as well as validating their traditional uses. Objectives: The main aim of this study was to examine whether, and to what extent, community pharmacies have become sites for the practice of complementary and alternative medicine (CAM) as an example of "medical pluralism." Methods: Qualitative as well as quantitative methods such as a telephone survey of all pharmacies in Johannesburg, observations, and in-depth interviews with pharmacists and CAM healers were used. Results: The evidence presented in this paper, although based on a study of community pharmacies in Johannesburg only, can he interpreted as an indication of a general trend in urban areas in South Africa, that of involvement with CAM in the form of dispensing and sales of CAM products, provision of advice, and, in a few cases, employment of CAM practitioners to consult on their premises. Thus the two systems of CAM and allopathic medicine are being practiced within the same premises. However, the manner in which they operate, as portrayed in this study, is that of two separate systems existing in relative harmony side-by-side. Conclusion: Because of the constraints of the study, it is difficult to ascertain to what extent this is a First step toward the development of a more meaningful integration between the systems. It is much easier to demonstrate that this growth is driven by the pharmacists' response to the growing demand from the public and their readiness to seize the opportunity to expand their responsibilities and increase their profits. The aim of the study was to evaluate the biological activities and safety of commercial herbal concoctions manufactured in Ga Maja (Limpopo province). Microbial contamination was evaluated by spread-plating the concoctions on agar plates. The VITEK 2 instrument was used for identification of the pure cultures. Nutritional content of the concoctions was determined. Thin layer chromatography was used to analyze the chemical constituents of the extracts. The microdilution assay and bioautography were used to evaluate antimicrobial activity against selected microorganisms. Sodium, potassium, and zinc were elements most abundant in the concoctions. Phytochemical screening revealed the presence of various phytoconstituents. Acetone extracts of Hypoxis hemerocallidea and Kirkia wilmsii extracts had antioxidant activity. The minimum inhibitory concentrations values against test bacteria ranged between 0.02 and 0.63 mg/mL. Further studies are required to isolate bioactive compounds and evaluate their cytotoxicity. Background: The roll out of antiretroviral therapy in the South African public health sector in 2004 was preceded by the politicisation of HIV-infection which was used to promote traditional medicine for the management of HIV/ AIDS. One decade has passed since; however, questions remain on the extent of the use of traditional, complementary and alternative medicine (TCAM) by HIV-infected patients. This study therefore aimed at investigating the prevalence of the use of African traditional medicine (ATM), complementary and alternative medicines (CAM) by adult patients in the eThekwini and UThukela Health Districts, South Africa. Methods: A cross- sectional study was carried out at 8 public health sector antiretroviral clinics using interviewer-administered semi-structured questionnaires. These were completed from April to October 2014 by adult patients who had been on antiretroviral therapy (ART) for at least three months. Use of TCAM by patients was analysed by descriptive statistics using frequency and percentages with standard error. Where the associated relative error was equal or greater to 0.50, the percentage was rejected as unstable. A -value p <0.05 was estimated as statistically significant. Results: The majority of the 1748 participants were Black Africans (1685/1748, 96.40 %, SE: 0.00045), followed by Coloured (39/1748, 2.23 %, SE: 0.02364), Indian (17/1748, 0.97 %, SE: 0.02377), and Whites (4/1748, 0.23 %, SE: 0.02324), < 0.05. The prevalence of ATM use varied prior to (382/1748, 21.85 %) and after ART initiation p (142/1748, 8.12 %), <0.05, specifically by Black African females both before (14.41 %) and after uptake p (5.49 %), < 0.05. Overall, 35 Black Africans, one Coloured and one Indian (37/1748, 2.12 %) reported visiting p CAM practitioners for their HIV condition and related symptoms post ART. Conclusion: Despite a progressive implementation of a successful antiretroviral programme over the first decade of free antiretroviral therapy in the South African public health sector, the use of TCAM is still prevalent amongst a small percentage of HIV infected patients attending public healthcare sector antiretroviral clinics. Background: Many patients with human immunodeficiency virus infection use traditional, complementary, and alternative medicines and other practices to combat the disease, with some also using prescribed antiretroviral therapy provided by the public health sector. This study aimed to establish the awareness of public sector biomedical health care providers on the use of traditional, complementary and alternative medicines by HIV-infected patients who also used highly active antiretroviral therapy, and to determine whether this was based on patients seen or cases being reported to them. Potential risks of interactions between the prescribed antiretroviral and non-prescribed medication therapies may pose safety and effectiveness issues in patients using both types of treatment. Methods: A descriptive cross-sectional study, using a researcher administered semi-structured questionnaire, was conducted from June to August 2013 at ten public sector antiretroviral clinics in five regional, three specialised and two district hospitals in eThekwini Health District, South Africa. Questionnaires were administered through face-to face interview to 120 eligible participants consisting of doctors, nurses, pharmacists and post-basic pharmacist assistants in HIV clinical practice. The results are presented as percent or proportion with standard error (SE), or as frequency. Results: Ninety-four respondents completed the questionnaire, yielding a response rate of 78.3 %. Almost half (48/94) were aware of patients using African traditional herbal medicines, over-the-counter supplements, unnamed complementary Ayurveda medicines and acupuncture. Twenty-three of the 94 respondents (24.4 %) said they had consulted patients who were using both antiretroviral therapy and certain types of non-prescribed medication in the previous three months. Conclusions: Awareness among healthcare providers on patient use of traditional, complementary and alternative medicines was relatively high. Few respondents had seen patients who used mostly African traditional medicines, over-the counter supplements, and negligible complementary Ayurveda medicines and acupuncture, with caution being advised in the interpretation of the former. Further research is needed to investigate communication between healthcare providers and patients in this regard, and levels of acceptance of traditional, complementary and alternative medicines by biomedical health care workers in HIV public sector practice. South Africa's organically emerged cultural business of traditional healing is almost exclusively reliant on wild-harvested resources extracted from wilderness areas or open-access commons. The wild medicine business has been described for much of South Africa, although is little understood in Cape Town, the urban centerpiece of the Cape Floristic Region (CFR). A census of different traditional healer typologies in five typical working-class residential areas (representing ∼71,500 residents) was conducted to assess the nature and extent of traditional medicine harvesting and trade. Extrapolating these findings for the city reveals a local industry of more than 15,000 practitioners collectively conducting trade worth US $15.6 million per year. More than 40% of the volume of traditional medicines traded in the city is harvested from the CFR. Future conservation approaches must consider that the business of traditional healing and dispensing wild-harvested medicines is both economically important and culturally entrenched. Objective:This study was conducted to investigate the prevalence and patterns of medication use amongst a sample of school going children and adolescents with autism spectrum disorders (ASD) in the Western Cape, South Africa. Method:This was a descriptive, quantitative, analytic study. A survey questionnaire and the Nisonger Child Behaviour Rating Form (NCBRF) were administered to parents of children and adolescents recruited from two schools for children with ASD in Cape Town and from the Autism Action database. Results:A total of 24.6% of the 65 children used psychotropic medications. Antipsychotics were the most common reportedly used psychotropics followed by stimulants, antidepressants and mood stabilisers. Complementary and alternative medications were also commonly used with 40% of children using over the counter medications (OTC) and 15.4% being on a special diet for autism. Children of black African or coloured ethnicity were less likely to use OTC medication than children in the white/ Asian ethnic group. Conclusions:In keeping with international studies this sample of children with ASD was a highly medicated group. The findings of this pilot study were limited by the response rates and sample size, but provide valuable insight into medication use in the South African ASD population. [ABSTRACT FROM PUBLISHER]

Resumen Contextualizando la situación de los huérfanos en la región del África Meridional y basándose en la investigación cuantitativa y cualitativa, el presente artículo analiza las opciones de cuidados y las políticas de protección social para los huérfanos en Mozambique, focalizándose en los niños que se encuentran en los centros de apoyo a los huérfanos. En la búsqueda orientada a brindar nuevas perspectivas y una mayor comprensión de las experiencias de los niños que reciben cuidados y de la protección social de que éstos disponen, la investigación pone de relieve la mayor probabilidad de que los niños huérfanos que viven en instituciones informales de cuidados experimenten abusos, abandono y malos tratos respecto de aquellos que viven en instituciones de cuidados no gubernamentales. La investigación pone el acento en que se requiere una selección más cuidadosa de las familias adoptivas que acogen a los niños. Las recomendaciones incluyen la necesidad de centrarse en el desarrollo de capacidades y en unas reformas institucionales dirigidas a implementar políticas de protección social para los niños huérfanos, como parte de un piso de protección social general. Background: Morella serata is a South African medicinal plant used in the treatment of microbial infections and to enhance male sexual performance. There is dearth of information in scientific literature on its efficacy and safety. Methods: In the present study, the root extracts were investigated for the phytochemicals that may be present the antibacterial, anticandida activity using 96 wells microtitre plate method and cytotoxicity using brine shrimp (Artemia salina) lethality assay. Results: The qualitative phytochemical screening revealed the presence of tannins, saponins, flavonoids, terpenoids and steroids. All the extracts including water inhibited both Gram positive and Gram negative bacteria strains at minimum inhibitory concentrations (MIC) ranging from 0.09--6.25 mgmL-1. The best activity was observed in the acetone extract inhibiting all the bacteria tested at MIC range of 0.09--0.78 mgmL-1 except Shigella flexneri KZN that was inhibited at 1.56 mgmL-1. Similarly, all the extracts suppressed the growth of all Candida species and Trichophyton mucoides at MIC ranging from 0.13--3.13 mgmL-1. The cytotoxicity assay revealed potent cytotoxic potential of M. serata methanol and ethanol root extracts by displaying LC50 of 0.26 and 0.18 µgmL-1 respectively. Conclusion: The results obtained from the present study indicated broad spectrum antimicrobial activity and justifies the use of the plant in the treatment of infectious diseases. Traditional health practitioners offer a variety of interventions for people who choose traditional health practice services. Hearing impairment, which may cause significant difficulties in communication, employment and learning, is a condition for which people consult with traditional health practitioners in South Africa. The training undergone by traditional health practitioners sets the tone for the types of intervention for persons with hearing loss. As there is a paucity of literature in this area, 15 traditional health practitioners were interviewed about the training they had undergone and more specifically with regards to hearing loss. The content thematic analysis which was conducted on the interview transcripts revealed that the training of traditional health practitioners has a significant supernatural component which includes a spiritual possession through which the ancestors pass on the inspired knowledge on how to assess and treat clients. The calling to become a traditional health practitioner is reportedly a divine vocation and cannot be refused. During their apprenticeships, traditional health practitioners learn to interpret the dreams sent by the ancestors and learn to hone their skills in the identification of appropriate treatments for hearing loss. The findings of this study suggest the need for open communication between traditional and modern health care providers so that traditional and allopathic health practitioners can learn from each other so as to provide an effective and appropriate intervention for their clients. Two-thirds of the global population consult the services of traditional health care practitioners (THPs) with 80% of the African population using traditional medicine (WHO, 2001a). Western conventional treatment approaches to health and specifically communication disorders in patients with Parkinson's disease (PD) have failed to recognize the significant role that THPs fulfill in the lives of a vast majority of African people and in comprehensive health care. This article contextualizes an exploratory and descriptive study on the perceptions of THPs regarding the treatment of patients with communication disorders resulting from PD. A purposive sample of 20 multilingual THPs was interviewed using a structured interview schedule. Results indicate that THPs and western practices view illness and healing from different frames of reference, yet complimentary, whilst striving for best treatment models of practice for the patient. The study contributed to the knowledge base of the significance of a multi-disciplinary approach when dealing with patients with PD and in health care in general. The aim of the study was to investigate traditional healing practices among the Venda and Tsonga speaking people of Limpopo Province, South Africa. The study employed a qualitative design. Four traditional healers, selected through purposive sampling were asked to participate in the present study. Semi-structured interviews were conducted with the participants. Seven themes emerged from the narratives of the four participants. The seven themes identifi ed are: a) the process of becoming a traditional healer; b) family and community reactions to the chosen career; c) patients treated; d) types of illnesses treated; e) diagnostic procedures used; f) treatment methods used; and, g) the notion of ethics by traditional healers. The study puts in perspective the ethical issues of competence, dealing with minor children and confidentiality. Some of the challenges associated with collaboration between traditional healing and the Western health care system are highlighted by the traditional healers. [ABSTRACT FROM AUTHOR]

This study highlights current global health biopolitics in conjunction with politics of indigeneity. The illustration is achieved by looking at the politics of knowledge at play in the pre-clinical trial of a traditional medicine in Cape Town, South Africa. The instance reunites healers and scientists in a common project to test the efficacy of a local wild shrub through the process of a randomised controlled trial, the current golden scientific standard to determine truth about the efficacy of medicines. How traditional forms of knowledge translate, or not, into this process is the first point I develop with relation to the trial. Inevitable reassessments of efficacy awakened in this process open a debate between representational learning of standardized scientific knowledge, and learning through embodied forms of knowledge in negotiations of how best to heal with medicines. I argue that the dynamics of these politics of knowledge within the trial lead on the one hand towards an optimal management of life at the molecular level, and on the other hand towards broader relational politics of life anchored in sounds and embodied forms of knowledge. Background: As ART programmes in African settings expand beyond the pilot stages, adherence to treatment may become an increasing challenge. This qualitative study examines potential barriers to, and facilitators of, adherence to ART in a workplace programme in South Africa. Methods: We conducted key informant interviews with 12 participants: six ART patients, five health service providers (HSPs) and one human resources manager. Results: The main reported barriers were denial of existence of HIV or of one's own positive status, use of traditional medicines, speaking a different language from the HSP, alcohol use, being away from home, perceived severity of side-effects, feeling better on treatment and long waiting times at the clinic. The key facilitators were social support, belief in the value of treatment, belief in the importance of one's own life to the survival of one's family, and the ability to fit ART into daily life schedules. Conclusion: Given the reported uncertainty about the existence of HIV disease and the use of traditional medicines while on ART, despite a programme emphasising ART counselling, there is a need to find effective ways to support adherence to ART even if the individual does not accept biomedical concepts of HIV disease or decides to use traditional medicines. Additionally, providers should identify ways to minimize barriers in communication with patients with whom they have no common language. Surveys of trade in wildlife products utilised in traditional medicine were conducted between 1997 and 1999 on the western boundary of the Kruger National Park, South Africa. Although the extent of trade and numbers of people operating in this sector were lower than other regions of South Africa, this study again highlighted the value of indigenous wildlife products to subsistence economies. Apart from cultural diversity, socio-economic differentiation occurred at a number of levels, (i) There were socio-economic differences between the traders and vendors. (ii) Gender differences were apparent, with women earning significantly less than men and having to undergo more social challenges in their operation in the market place, (iii) Operational differences were noted between the vendors and traders, as well as between the street and pension day vendors. (iv) Multidimensional Scaling (MDS) highlighted the socio-economic differentiation at the household level, emphasizing in particular one of the most vulnerable groups, the elderly. The potential influence of this socio-economic differentiation needs to be considered during the development of Community-Based Conservation programmes, if the dual aims of conserving biodiversity and improving the lives and livelihoods of those who traditionally rely on medicinal plants are to be met. Efforts to supplement income for daily livelihood needs have resulted in illegal bark harvesting for traditional medicine from natural forests in South Africa. A harvesters' association was formed and agreements were negotiated between the Forestry Department and the association. The recommendations resulting from these studies were legally implemented in May 2002. through an improved management plan and guidelines for sustainable hark harvesting from the Umzimkulu forests. Background: The purpose of this study was to determine, among the Indian community of Chatsworth, South Africa, the prevalence and utilisation patterns of complementary and alternative medicine (CAM), attitudes associated with CAM use and communication patterns of CAM users with their primary care doctors. Methods: Face-to-face structured interviews were conducted in Chatsworth, a suburb of Durban in which South Africans of Indian origin predominantly reside. Participants were 200 randomly selected adult English-speaking Indian residents. Results: The prevalence of CAM usage for period 2000/2001 was 38.5% (95% confidence interval 31.7% to 45.6%). Spiritual healing and herbal/natural medicines, including vitamins were the most common types of CAM used, accounting for 42.8% and 48.1% respectively of overall CAM usage. People used CAM to treat conditions including diabetes mellitus, headaches, arthritis and joint pains, stress, skin disorders, backaches, hypertension and nasal disorders. Half of the CAM users used allopathic medicines concurrently. The cost of CAM utilization over this 1-year period, incurred by 80.5% of users for the duration of therapy for their most troublesome condition was below R500 (approximately US$50). Age, sex, marital status, religion, level of education and income were shown not to influence the use of CAM. Greater than half (51.9%) of CAM users did so either upon the advice of someone they knew, or after noticing a CAM advertisement in the local press. Seventy-nine percent of CAM users indicated that they had positive outcomes with their treatments. Fifty four percent of CAM users (excluding those using spiritual healing only) failed to inform their doctors that they used CAM. The main reason given by half of this group was that informing their doctors did not seem necessary. Conclusion: The prevalence of CAM in Chatsworth is similar to findings in other parts of the world. Although CAM was used to treat many different ailments, this practice could not be attributed to any particular demographic profile. The majority of CAM users were satisfied with the effects of CAM. Findings support a need for greater integration of allopathic medicine and CAM, as well as improved communication between patients and caregivers regarding CAM usage. [ABSTRACT FROM AUTHOR]
