Background: The current study aimed to further our understanding of second opinions among medical oncology patients by examining the proportion of patients who sought a second opinion about their cancer treatment, and why. Methods: The study was conducted between 2013 and 2015 in three medical oncology clinics located in public hospitals in Australia: in metropolitan New South Wales, metropolitan Queensland, and in Tasmania. Those patients who provide written informed consent were asked to complete a brief paper and pencil survey in the clinic containing questions on sociodemographic, disease and treatment characteristics. Approximately 1 month later, participants were mailed a second paper and pencil survey which contained questions about whether they had sought a second opinion and their motivation for doing so. Non-responders were followed up by letter at 3 and 6 weeks. Results: Of 823 patients screened for eligibility, 698 eligible patients, 612 provided consent. Of those who consented, 355 completed both the initial survey and the second survey and were included in the analyses. Of the 57 patients who sought a second opinion, the most frequent reasons given for doing so were the need for reassurance (49.1%) and the need to consider the range of treatment options (41.8%). Of the 297 (83.6%) participants who did not seek a second opinion, the main reason was confidence in the first doctor (88.7%). Only 3.1% patients did not know that they could ask for a second opinion. Occasionally the doctor will initiate the referral for a second opinion. Conclusions: Our study suggests that a minority of cancer patients seek a second opinion at some phase during their care. Most did so for reassurance or to ensure that they had covered all of the treatment options and not because of discomfort or distrust of their treating doctor. Few patients reported a lack of awareness of second opinions. This suggests that second opinions form part of a patient-centred approach to information provision about care options. Since bail legislation was enacted in the 1970s, Australia has experienced a continual increase in the number of prisoners on remand. Amendments to bail legislation and police discretion have been shown to contribute to this increase. Further, an accused's vulnerability affects whether they are granted or denied bail, with vulnerable people being more likely to be denied bail. Vulnerability in the criminal justice system refers to factors such as race, age, sex and socioeconomic status. Many vulnerable people have multiple intersecting vulnerabilities, which further compounds their contact with the justice system. This study employed a qualitative content analysis of bail legislation for the Australian states of New South Wales (NSW), Tasmania, and Victoria, along with key correlating second reading speeches. The aim was to better understand the way in which bail decision-makers, such as police, consider vulnerability when making decisions about bail, in particular, if and how they are legislated to consider factors relating to vulnerability. The research found that only police in NSW and Victoria were required to consider an accused's vulnerability explicitly under the law. Background: The prevailing view that the vast majority of those who complete suicide have an underlying psychiatric disorder has been recently challenged by research on the contribution of "predicaments", in the absence of mental illness, to suicide. In this paper, we sought data to support the notion that forced marriage may lead to suicide without the presence of psychiatric disorder. Method: Historical records, newspapers, and the electronic media were searched for examples. Results: Two examples from ancient times and six from the last hundred years were located and described. Conclusion: These cases suggest that forced marriage may lead to suicide and complements earlier findings that loss of fortune, health, liberty, and reputation may lead to suicide in the absence of mental disorder. Dam safety is a significant issue being taken seriously worldwide. However, in Australia, although much attention is being devoted to the medium- to large-scale dams, minimal attention is being paid to the serious potential problems associated with smaller dams, particularly the potential cumulative safety threats they pose in catchments. This paper establishes the significance of this problem and reviews various Australian policies, including consideration of evidence from past research, to identify the potentially more effective policies with respect to smaller dams/cumulative safety assurance. New `case studies' research that recently tested the effectiveness and coverage of the "strong" dam-safety policy of New South Wales (NSW) is then reported. The case studies comprised 2 samples of 10 hazardous private reservoirs investigated for spillway adequacy in line with state-of-the-art practice: one sample comprised dams supervised under the NSW policy, while the other sample comprised only non-supervised dams. The case studies show the potential effectiveness of strong policy, but also the importance of registering all dams and the need for some form of supervision of even small dams due to either their individual or cumulative hazard potential. The State of Tasmania provides a `model' on how this can be best achieved in line with international best-practice, and this recently became the focus of a comprehensive study. This study involved strategic consideration and assessment of policy responses to the varying issues associated with small-dam safety and practical feedback from key policy actors. The results of this study are also reported here to provide guidance to any other jurisdiction world-wide needing to manage numerous small dams posing cumulative safety threats in catchments. The genus Alfredella Masner & Huggert is revised. Alfredella tasmanica Masner & Huggert is redescribed, Al. teres (Buhl), comb. nov. is transferred to Alfredella from Amitus Haldeman, and Al. auriel Lahey, sp. nov. (New South Wales, Tasmania) and Al. mephisto Lahey, sp. nov. (Western Australia) are described as new. The genus Masnerium Polaszek, syn. nov. is treated as a junior synonym of Amitus, and its type species, M. wellsae Polaszek, is transferred to Amitus as Am. wellsae (Polaszek), comb. nov. The relationship between Alfredella and morphologically similar genera is discussed, and a key is provided to distinguish between Aleyroctonus Masner & Huggert, Alfredella, and Amitus. We collected contemporary foraminiferal training sets from two salt marshes to enable more precise and accurate proxy historical sea-level reconstructions from southeastern Australia. Combined with an existing training set from Tasmania, this new regional set consists of 112 samples and 16 species of foraminifera, of which 13 are agglutinated. Cluster analyses group the regional training set into a high-elevation cluster, dominated by Trochamminita salsa, a mid-elevation cluster, dominated by Entzia macrescens and Trochammina inflata, and a mid-low elevation cluster dominated by Miliammina fusca and tidal-flat species. We develop transfer functions using local and regional training sets and assess their performance. Our resulting site-specific and chosen regional models are capable of predicting sea level with decimetre-scale precision (95% confidence intervals of 0.12-0.22 m). These results are comparable to other examples from around the world. When developing regional training sets, we advocate that the similarity in the environmental settings (particularly salinity) should be assessed as an alternative way of grouping sites, rather than simply using spatial proximity. We compare our findings with global results and conclude that salt marshes along microtidal coasts yield models with the lowest vertical uncertainties. Studies with the lowest uncertainties are located in the western Pacific and the western Atlantic, whereas those from the eastern Atlantic generally have larger tidal ranges and carry larger vertical uncertainties. Our models expand the existing region available for sea-level reconstruction and can be used to generate new late Holocene sea-level reconstructions across southeastern Australia.
