Purpose: International guidelines for the management of patients with low back pain (LBP) are available, but there are no India-specific guidelines for the diagnosis and management of LBP in primary care. This article aims to provide clinical insights for the diagnosis and treatment of LBP for primary care practitioners in India. Methods: A 3-step modified Delphi method was used to get insights and opinions from a panel of 9 multidisciplinary specialist (neurology, orthopedics, and pain management). In rounds 1 and 2, 203 and 60 statements, respectively, were circulated to the expert panel and they were asked to respond with an “agree” or “disagree” to each statement and provide comments. At least 80% of the experts had to agree for a statement to reach a concordance. More than 80% concordance was reached for 175 and 32 statements in rounds 1 and 2, respectively. Round 3 was a direct interaction with the experts. Results: There was good agreement among the expert panelist in round 3 for the consensus-derived statements from the earlier two rounds of the survey. The panel agreed that diagnostic triage is important to exclude specific spinal pathology, routine imaging is not recommended unless suspecting serious pathology, specialist referral is needed for serious pathologies/no improvement in pain with treatment, and management is to be individualized depending on etiology and patient factors. Conclusions: Primary care physicians are the first point of contact for patients with back pain. These consensus recommendations will assist primary care physicians in India to choose an appropriate course of action for managing patients with LBP.

This review highlights the evolution in the scope of eye banking activities in India. The roles and responsibilities of the eye banks have expanded to match the requirements and necessities that arose because of advancements in keratoplasty techniques and the need for corneal surgeons. Adoption of different types of corneal preservation solutions and methods of donor cornea preservation was a consequence of the changing and unprecedented times. The growth of eye banking in India has been a commendable journey and has placed the country in an “almost sufficient” category in eye banking and corneal transplantation. There exists a potential to reach the status of cornea surplus country in the future. The rules and laws governing eye banking operations need dynamic amendments based on the trends in keratoplasty, surgical practice patterns, and status of self-sufficiency. Newer technological advances and quality measures must be adopted in donor cornea evaluation and tissue preparation. Research should be integrated as an important component of eye banking, especially in developing novel methods of donor preservation, and evaluating and validating the existing and changing practices of eye banking.

Background: Acquired Immunodeficiency Syndrome (AIDS) continues to pose a significant public health threat worldwide, with India experiencing a substantial burden. Integrated Counseling and Testing Centers (ICTCs) play a pivotal role in addressing HIV/AIDS by providing crucial healthcare access and dispelling misconceptions surrounding the disease. Objectives: To study the profile of PLHIV patients attending ICTC centre. Methodology: This descriptive record-based study was conducted at the ICTC of Kasturba Medical College Hospital, Mangalore, aiming to profile HIV-positive attendees. Data on sociodemographic characteristics, mode of transmission, referrals, and counseling practices were collected from records of 137 seropositive attendees using convenient sampling. Statistical analysis was conducted using SPSS version 11.5. Results: The study revealed that most attendees were male (63.5%), married (78.8%), and aged 40 and above (40.1%). Heterosexual contact was identified as the primary mode of transmission (95.6%), with no cases attributed to blood transfusion. Private health facilities were the primary referrers (72.9%), and ICTC mainly referred patients to Antiretroviral Therapy (ART) centers (72.3%). Post-test counseling included spousal testing (33.6%) and condom counseling (90.5%), with limited condom uptake (18.9%) and follow-up of seropositive attendees (35.8%). Conclusion: The study underscores the critical role of ICTCs in HIV/AIDS management, highlighting trends in attendee profiles, transmission modes, referrals, and counseling practices. It emphasizes the necessity for targeted interventions to enhance follow-up and promote condom usage, alongside continued efforts to mitigate HIV transmission.

Background and Aims: The aim of the study was to evaluate the effect of mode of mechanical ventilation; pressure-controlled ventilation (PCV) vs. volume-controlled ventilation (VCV) on airway pressures, intra-abdominal pressure (IAP) and intra-operative surgical bleeding in patients undergoing lumbar spine surgery. Methods: This was a prospective, randomised study that included 50 American Society of Anesthesiologists class I and II patients undergoing lumbar spine surgery who were mechanically ventilated using PCV or VCV mode. The respiratory parameters (peak and plateau pressures) and IAP were measured after anaesthesia induction in supine position, 10 min after the patients were changed from supine to prone position, at the end of the surgery in prone position, and after the patients were changed from prone to supine position. The amount of intraoperative surgical bleeding was measured by objective and subjective methods. Results: The primary outcome was the amount of intraoperative surgical bleeding. It was significantly less in the PCV group than in the VCV group (137 ± 24.37 mL vs. 311 ± 66.98 mL) (P = 0.000). Similarly, on comparing other parameters like peak inspiratory pressures, plateau pressures and IAP, the patients in PCV group had significantly lower parameters than those in VCV group (P < 0.05). No harmful events were recorded. Conclusion: In patie,nts undergoing lumbar spine surgery, use of PCV mode decreased intraoperative surgical bleeding, which may be related to lower intraoperative respiratory pressures and IAP.

Hereditary gingival fibromatosis (HGF) is a proliferative fibrous lesion causing severe gingival enlargement, affecting the esthetics, as well as posing various periodontal problems. This case report addresses the diagnosis and treatment of one such rare case of HGF where the patient presented with generalized diffuse gingival enlargement involving the maxillary and mandibular arches extending on the buccal and lingual/palatal surfaces and covering the incisal/occlusal third of the tooth, resulting in altered esthetics, difficulty in speech, and mastication. Gingivectomy was carried out in all the four quadrants using diode laser. The healing was uneventful; the patient was satisfied with her esthetics and was able to resume her oral hygiene practices. Even though recurrence cannot be predicted, the risk of recurrence can be outweighed with the psychological and functional benefits. Long-term follow-up will be required to evaluate the predictability of the different surgical techniques.

Background: Annual prevalence of chronic neck pain in general population averages between 30%-50%. Among various causes of chronic neck pain cervical facet joint involvement accounts for about 54% to 67%. Interventional procedures such as medial branch nerve block, trigger point injections and dry needling have been described for management of chronic neck pain. Objectives: To determine the clinical effectiveness of cervical medial branch block and trigger point injections in the management chronic neck pain. Design: Randomized, double blinded, prospective study conducted at two different centers in India, Pain Clinic, Department of Anaesthesiology, J.N. Medical College hospital, Aligarh Muslim University, Aligarh, Delhi Pain management centre, New Delhi, India. Methods: Data analysis was done from a total of 60 patients with 30 patients in each group. Group CMB received cervical medial branch block and group TP received trigger point injection with levobupivacaine (0.25%) and triamcinolone (20mg) at two different levels. There was no sham group. Outcome Measure: Numeric rating scale (NRS), Neck Pain Disability Index (NDI), weekly analgesic requirement and subjective evaluation of patients was done at 1st week, 3rd week, 6th week and 12th week of follow up and compared with the pre-procedure value. Results: Significant pain relief was observed in both the groups of study, at 1st week of study, compared to baseline. Further follow up showed significantly better pain relief in patients receiving medial branch nerve block than patient receiving tender point injections at 3rd, 6th week and 12th week. NDI score in-group CMB was significantly better as compared to group TP at 3rd, 6th and 12th week follow up. Subjective evaluation of pain showed significantly greater proportion of patient with good to excellent pain relief in group receiving medial branch block. Analgesic requirement was significantly less in group CMB compared to group TP during follow up. Conclusion: Cervical medial branch block with local anaesthetic and steroid may provide long-term relief in patients with chronic neck pain than trigger point injection with no major incidence of complications in either group.

Occipital neuralgia is a neuropathic pain disorder with distinctive diagnostic and therapeutic challenges. Many pain physicians and neurologists consider this to be a nonexistent condition which merely represents a variant of cervicogenic headache emanating from C1 to C4 nerve roots. Repetitive microtrauma from a hyperextended neck (painting ceilings or working with computers for long hours with a high focal point) may result in the development of this condition. Failure of conservative management necessitates greater occipital nerve (GON) block in the treatment of occipital neuralgia. Conventionally, GON has been blocked at superior nuchal line through a landmark-guided technique using occipital artery pulsations as guidance or using ultrasound as done in classical distal technique at the same site. Another less frequently used technique under ultrasound is the one where the nerve is blocked at a proximal site. We describe here a case report where we chose proximal technique done under ultrasound, superficial to the obliquus capitis inferior muscle (OCIM), which has a higher success rate and allows for a more precise blockade of the nerve before it branches out as in distal technique.

Purpose: Effective communication lies at the heart of a patient--doctor relationship. Communication skills (CS) teaching and assessment is not a part of the postgraduate (PG) curriculum. Lack of effective CS in current PG students' results in patient distrust and dissatisfaction, conflicts, and compromised healthcare. The regulatory authorities of medical education have recognized the need to inculcate soft skills among medical graduates, one of which is CS. The purpose of this study was to assess the need for teaching CS to ophthalmology PG students and develop and introduce a module for the same. Methods: In this prospective, interventional study done at the ophthalmology department of a tertiary hospital, a validated 8 day CS workshop was conducted for 60 PG students through interactive lectures, observations, video sessions, and role plays. Feedbacks were obtained through narratives, validated Google survey, reflections and verbal method and analyzed. Results: In the needs assessment done on 27 departmental faculties, 20 faculties found poor communication to be a major reason for patient complaints. All faculties agreed that CS should be taught to medical students. Statistically significant improvement in CS awareness was noted among students after the workshop. Lack of CS training, work burden, and language were identified as main barriers to effective communication. All the students were satisfied with the workshop and wanted it to be conducted regularly. Conclusion: This study establishes that CS training is essential to improve patient satisfaction and patient-doctor relationships. Barriers to effective CS could be identified, for which possible solutions could be found.

The importance of pink esthetics as well as increased patient comfort has been gaining its pace in dentistry. Moreover, when it comes to the treatment of gingival recession, the ideologies of the periodontists have changed over time, i.e., from “extension to prevention;” it has now become “conserve to preserve.” Utilizing this same principle, pinhole technique was introduced comprising sutureless surgery with minimal surgical intervention, thus providing maximum comfort to the patient while undergoing treatment and postoperatively. This article presents a case report of two cases who were treated with a minimally invasive pinhole surgical technique, with complete root coverage and minimal complications.

Neuropathic pain (NeP) constitutes a major pain-related disorder, which is often underdiagnosed and undertreated. Adverse physical, psychological, and economic consequences associated with NeP lead to poor quality of life. Burden of NeP in developing countries like India is colossal. Various international guidelines provide effective approaches to diagnose and manage NeP. However, differences in the genetic makeup of Indian population can result in subtle differences in clinical response, considering their low body weight, drug metabolism ability, and pain perception. Similarly, treatment-related adverse effects may also vary. Practice of Indian physicians may also differ for choice of drugs based on their availability and affordability. In the absence of country-specific guidelines, this document could serve as a guiding tool for health-care providers, ensuring uniformity in the treatment of NeP. Thus, applicability of all recommendations from any of these guidelines in Indian setting demands careful evaluation. Clinical experience of Indian physicians suggests that there are lot many challenges (e.g., busy outpatient departments, nonavailability of screening questionnaires in regional languages, and availability and affordability of medications) faced by them when managing NeP. In addition, in India, there are no country-specific guidelines that would help them to address these challenges. The objective for this consensus was to develop an expert opinion guideline to harmonize the management of NeP in India. The expert panel consisted of experts from various specialties such as pain medicine, anesthesiology, diabetology, neurology, and orthopedics. The panel critically reviewed the existing literature evidence and guideline recommendations to provide India-specific consensus on the management of NeP. The final consensus document was reviewed and approved by all the experts. This expert opinion consensus will help health-care professionals as a guiding tool for effective management of NeP in India. Use of Douleur Neuropathique 4 (DN4) questionnaire for NeP screening should be routine in day-to-day clinical practice. For effective utilization of DN4 questionnaire, it should be converted to regional language. If DN4 questionnaire screening fails to identify NeP, it should not be disregarded and should not replace the sound clinical judgment from the treating physician. Diagnostic tests may be considered as a supplement to clinical judgment. Cost-effective treatment should be the initial choice. Dosing should be individualized based on efficacy and tolerability. Tricyclic antidepressants (TCAs), gabapentinoids, and serotonin-norepinephrine reuptake inhibitors (SNRIs) can be considered among initial choices. Tramadol can be considered as a second-line add-on treatment for NeP if there is partial response to the first-line agent either alone or in combination. Fixed-dose combination (FDC) of gabapentinoids such as pregabalin (75 mg) with TCA such as nortriptyline (10 mg) is synergistic and improves treatment adherence. Among other treatments, Vitamin B12 (methylcobalamin) can be used either alone or in combination for the management of NeP. Use of Vitamin D and steroids should be limited to specific NeP in individual cases. Referral to pain specialists can be considered if two drugs fail to provide relief in NeP.

Background: With the advent of new provisional crown materials, it has become imperative to evaluate their marginal fit and strength to select the ideal provisional crown material. Aim: The purpose of this in vitro study was to evaluate and compare the vertical marginal fit and flexural strength of provisional crowns prepared using computer-aided design-computer-aided manufacturing (CAD-CAM) temporary material versus those fabricated using bis-acrylic composite-based autopolymerizing resin material. Materials and Methods: Eighty samples were divided into two equal Groups (I and II). Group I consisted of forty samples that were evaluated for flexural strength and Group II consisted of forty samples that were evaluated for their vertical marginal fit. Group I was subdivided as Group IA, i.e., bis-acrylic composite-based autopolymerizing resin material (Protemp™ 4) blocks and Group IB, i.e., CAD/CAM provisional material blocks. Similarly, Group II was subdivided as Group IIA, i.e., bis-acrylic composite-based autopolymerizing resin material (Protemp™ 4) crowns and Group IIB, i.e., CAD/CAM provisional material crowns. Marginal adaptation was evaluated using stereomicroscope and image analyzing software to measure the amount of marginal gap. For flexural strength, all specimens were subjected to a standard compression load in the universal testing machine until fracture occurred. Data were analyzed using Student's t-test (P = 0.001). Results: CAD/CAM provisional crowns showed better marginal adaptation (34.34 μm) as compared to bis-acrylic composite-based autopolymerizing resin material (Protemp™ 4) crowns (63.42 μm) (P < 0.001). The flexural strength of CAD/CAM blocks (94.06 megapascals [MPa]) was not statistically different from bis-acrylic composite-based autopolymerizing resin material (Protemp™ 4) blocks (101.41 MPa) (P > 0.001). Conclusion: Protemp™ 4 and CAD/CAM provisional materials have comparable flexural strength. However, the marginal fit of temporary crowns fabricated by CAD/CAM was found to be superior to the ones fabricated using bis-acrylic composite-based autopolymerizing resin material (Protemp™ 4).

Background: A deep level of sedation is required for magnetic resonance imaging (MRI) in children to ensure optimum image quality. The present study was conducted to evaluate the efficacy and safety of dexmedetomidine versus propofol for sedation in children undergoing MRI. Materials and Methods: A total of sixty children aged 2–10 years, having physical status 1 or 2 according to the American Society of Anesthesiologists, undergoing MRI were included in the study. Group D: (n = 30) received injection dexmedetomidine 2 μg/kg for 10 min followed by continuous infusion of 1.0 μg/kg/h. Group P (n = 30) received injection propofol 1 mg/kg bolus followed by continuous infusion of 100 μg/kg/min. Results: The mean time for onset of sedation in Group D was much longer than in Group P (P = 0.000). Mean duration of sedation was comparable in the two groups. The number of patients requiring increased infusion of study drug was significantly higher in Group D (30%) as compared to Group P (16.7%) (P < 0.05). The average recovery time in Group D was much longer than in Group P (P < 0.001). Conclusion: Propofol had an advantage of providing rapid onset of sedation and quicker recovery time. Dexmedetomidine resulted in a better preservation of respiratory rate and oxygen saturation, so it may be more suitable in children who are prone to respiratory depression. Hence, both the drugs could achieve required sedation in children posted for MRI.

Background and Aims: Infraclavicular (IC) approach of subclavian vein (SCV) catheterisation is widely used as compared to supraclavicular (SC) approach. The aim of the study was to compare the ease of catheterisation of SCV using SC versus IC approach and also record the incidence of complications related to either approach, if any. Methods: In the study, 60 patients enrolled were randomly divided into two groups of 30 patients each. In Gp. SC right SCV catheterisation was performed using SC approach and in Gp. IC catheterisation was performed using IC approach. Access time, success rate of cannulation, number of attempts to cannulate vein, ease of guidewire and catheter insertion and length of catheter inserted and any associated complications were recorded. Results: The mean access time in group SC for SCV catheterisation was 4.30 ± 1.02 min compared to 6.07 ± 2.14 min in group IC. The overall success rate in catheterisation of the right SCV using SC approach (29 out of 30) was better as compared with group IC (27 out of 30) using IC approach. First attempt success in the SC group was 75.6% as compared with 59.25% in the IC group. All successful subclavian vein catheterisations in SC group and IC group were associated with smooth insertion of guidewire following subclavian venipuncture. Conclusion: The SC approach of SCV catheterisation is comparable to IC approach in terms of landmarks accessibility, success rate and rate of complications.

Background: Clonidine is added to intrathecal local anesthetics to improve intraoperative analgesia and to increase the duration of sensory and motor block. Aim of this study was to evaluate and compare the effects of addition of two different doses of clonidine (15 and 30 mcg) to 11 mg hyperbaric bupivacaine in patients undergoing inguinal herniorrhaphy surgery under spinal anesthesia. Materials and Methods: Seventy-five patients enrolled in the study were randomly divided into three groups of 25 each. Group I patients received 11 mg hyperbaric bupivacaine, whereas groups II and III received 15 mcg and 30 mcg clonidine, respectively, as an adjuvant to 11 mg hyperbaric bupivacaine. The volume of solution was kept constant to 2.4 ml by adding saline wherever needed. Results: Highest level of sensory block, time to achieve this level, and highest Bromage scale recorded were comparable among the groups. The mean time to two-segment regression, regression of sensory block to L3 dermatome, and mean duration of motor block were the greatest in group III followed by group II and group I. There was significant fall in mean arterial pressure (MAP) in groups II and III as compared to group I (P = 0.04). Episodes of hypotension were more in group III than in group II. Conclusion: 30 mcg clonidine was associated with more incidence and duration of hypotension than 15 μg of clonidine. 15 mcg clonidine added to 11 mg hyperbaric bupivacaine provides better sensory and motor blockade for inguinal herniorrhaphy.

Pheochromocytoma is a rare catecholamine secreting tumour arising from chromaffin cells in the adrenal medulla or in the extra adrenal sites. They may produce receptor mediated effect on myocardium causing pressure induced cardiomyopathy leading to left ventricle dilatation, hypokinesia and eventually cardiac failure. Anaesthetic management of a patient with pheochromocytoma and dilated cardiomyopathy posted for open adrenalectomy using proseal laryngeal mask is reported.

Pain is among the most common symptom in cancer patients. It is estimated that 4 million cancer patients suffer pain globally and for most of these patients pain is not satisfactorily relieved. For over a decade; World Health Organization (WHO) has taken a lead in establishing a consensus for a scientifically valid method of relieving cancer pain that is relatively simple; inexpensive and easy to apply at community level; known as the WHO Three step Analgesic Ladder method.
