Background Objectives Material and Methods Results Conclusions and Clinical Relevance Canine aural cholesteatoma (more appropriately named tympanokeratoma) is an epidermoid cyst whose aetiopathogenesis remains poorly recognised in veterinary medicine. There are a few reports published, possibly because it may be underdiagnosed.To characterise the clinical aspects of dogs with tympanokeratoma, to describe the otoendoscopic, advanced imaging and histopathological findings of tympanokeratoma and to report the best approach to diagnose canine auricular tympanokeratoma in a retrospective study.Of 890 dogs with suspected tympanokeratoma and otitis media, 100 animals underwent advanced imaging and otoendoscopy at radiology and dermatology reference centres in Brazil.Most affected dogs were male (71%) neutered (95%) with an average age of 6.8 years. Ninety‐one of the 100 affected dogs were brachycephalic. Otitis externa (OE; 81%) was the main non‐neurological manifestation observed. The main neurological clinical manifestations observed were: “head tilt” (66%), ataxia (31%) and nystagmus (25%). Advanced imaging findings could not propose a presumptive diagnosis of tympanokeratoma in 60 of 100 (60%) of the dogs. The absence of tympanic membrane and the presence of a dense pearly yellowish material resembling keratin in the tympanic bulla, after myringotomy, was the main otoendoscopic finding. The advanced imaging findings did not correlate with otoendoscopy and histopathological findings in more than half of the dogs.Tympanokeratoma should be suspected in brachycephalic dogs with OE and peripheral vestibular syndrome, and samples of keratinous material from the middle ear associated with histopathological results may be the best approach for the diagnosis. Background: The purpose of this multicenter study was to retrospectively investigate the prognostic significance of the tumor microenvironment, in relation to survival in a large cohort of patients with laryngeal squamous cell carcinoma (LSCC), using the method proposed by the International TILs Working Group in breast cancer. Methods: All consecutive patients with biopsy‐proven LSCC who underwent total laryngectomy (TL) between January 2014 and January 2023 were retrospectively included in the study. A retrospective review of medical records including surgical, pathological and follow‐up reports was performed. The density of TILs was determined according to the recommendations of the International TILs Working Group. Results: The study group included 186 patients with LSCC. High TILs were statistically correlated with reduced size and extension of primary tumor (pT stage) with a statistically significant value (S: p = 0.01; P: p = 0.0003) and without needs of salvage therapy (S: p = 0.03; P: p = 0.004). Low TILs were indicative of worse prognosis. Conclusions: Our study confirmed the protective value of TILs and the prognostic role of the tumor microenvironment in LSCC; furthermore, our results showed that the score proposed by the International TILs Working Group for breast cancer can be applied to LSCC. Background: Damage due to respiratory viruses increases the risk of bacterial and fungal coinfections and superinfections. High rates of invasive aspergillosis are seen in severe influenza and COVID‐19. This report describes CAPA cases diagnosed during the first wave in the biggest reference centre for severe COVID‐19 in Mexico. Objectives: To describe the clinical, microbiological and radiological characteristics of patients with invasive pulmonary aspergillosis associated with critical COVID‐19, as well as to describe the variables associated with mortality. Methods: This retrospective study identified CAPA cases among individuals with COVID‐19 and ARDS, hospitalised from 1 March 2020 to 31 March 2021. CAPA was defined according to ECMM/ISHAM consensus criteria. Prevalence was estimated. Clinical and microbiological characteristics including bacterial superinfections, antifungal susceptibility testing and outcomes were documented. Results: Possible CAPA was diagnosed in 86 patients among 2080 individuals with severe COVID‐19, representing 4.13% prevalence. All CAPA cases had a positive respiratory culture for Aspergillus species. Aspergillus fumigatus was the most frequent isolate (64%, n = 55/86). Seven isolates (9%, n = 7/80) were resistant to amphotericin B (A. fumigatus n = 5/55, 9%; A. niger, n = 2/7, 28%), two A. fumigatus isolates were resistant to itraconazole (3.6%, n = 2/55). Tracheal galactomannan values ranged between 1.2 and 4.05, while serum galactomannan was positive only in 11% (n = 3/26). Bacterial coinfection were documented in 46% (n = 40/86). Gram negatives were the most frequent cause (77%, n = 31/40 isolates), from which 13% (n = 4/31) were reported as multidrug‐resistant bacteria. Mortality rate was 60% and worse prognosis was seen in older persons, high tracheal galactomannan index and high HbA1c level. Conclusions: One in 10 individuals with CAPA carry a resistant Aspergillus isolate and/or will be affected by a MDR bacteria. High mortality rates are seen in this population. [ABSTRACT FROM AUTHOR]

Popular theorizing about happiness pursuit emphasizes universal paths to happiness, but other theorizing acknowledges different people achieve happiness in different ways (i.e., subjective well‐being, SWB). The present work extended this latter perspective by examining how antagonistic pursuit of a grandiose identity ('narcissistic antagonism') – generally thought to reduce narcissistic people's SWB – may relate to increased cognitive well‐being (i.e., a component of SWB) for narcissistic people with lower self‐esteem. In Study 1, participants (N = 417) reported their narcissism, self‐esteem, narcissistic antagonism, and general life satisfaction (to index cognitive well‐being). In Study 2 (pre‐registered), participants (N = 450) reported their narcissism, self‐esteem, narcissistic antagonism, general and domain‐specific life satisfaction, and general affect (to index affective well‐being, which is a different component of SWB). Both studies revealed narcissistic antagonism related to increased life satisfaction only for more (vs. less) narcissistic people with lower (vs. higher) self‐esteem. Study 2 not only replicated this interactive pattern on satisfaction across various life domains but also revealed the interaction may be related to increased negative affect. Selective exposure is the tendency to gather viewpoint‐congenial versus viewpoint‐uncongenial information. Extant models of selective exposure suggest this tendency occurs because people anticipate reading congenial (vs. uncongenial) information will cause more favourable intrapersonal consequences. However, these models ignore the notion that people's information choices are, in part, symbolic gestures designed to convey identity‐relevant beliefs to an audience through information display. Drawing from perspectives that emphasize human consumption as symbolic and a way to signal one's identity, we suggest that selective exposure pertains not only to information processing but also to conveying identity through information display. Experiment 1 showed that people characterize information display as a way to communicate their views to an audience. Experiments 2–4 showed that people are averse to displaying uncongenial versus congenial information (without processing the information), anticipate feeling more uncomfortable and more inauthentic merely displaying (without processing) uncongenial versus congenial information, and that people's intentions to engage in selective exposure in daily life are a function of their belief that selective‐exposure displays convey their identity. None of these studies or findings can be generated from extant selective‐exposure theories. Ageratina pichinchensis is utilized in traditional medicine for the treatment of dermatomycosis and inflammation. The aim of this study was to evaluate the clinical and mycological effectiveness of the topical administration of an enecalin standardized extract of A. pichinchensis for treating onychomycosis in patients with type 2 diabetes mellitus (DM2). A double blind, randomized, and controlled clinical trial was carried out that included patients with DM2 and who had mild or moderate onychomycosis. Participants were administered topically, for 6 months, a lacquer containing the encecalin standardized extract of A. pichinchensis (experimental group) or 8% ciclopirox (control group). In a large percentage of both, the control group (77.2%) and the experimental group (78.5%), clinical efficacy was detected as a decrease in the number of affected nails and a reduction in the severity of nail involvement. Without exhibiting statistically significant differences between groups, the encecalin standardized extract of A. pichinchensis was clinically and mycologically effective in the treatment of mild and moderate onychomycosis in patients with DM2. The treatment of onychomycosis in patients with DM2 implies a greater challenge, while control of blood glucose levels in these patients, played a very important role in the response of patients to treatment. Introduction: Benign paroxysmal positional vertigo (BPPV) of the horizontal semicircular canal (hSCC) can present with otoconia blocking its lumen (canalith jam), with signs and symptoms that make it difficult to distinguish from central nervous system pathology. Objective: Here we report two cases of canalith jam affecting the hSCC and offer a theoretical mechanism based on known vestibular neurophysiology. Methods: We use video‐oculography to document the canalith jam and show the moment the otoconia loosen. Results: Canalith jam is a rare form of BPPV remedied with repositioning maneuvers. Conclusion: Clinicians should consider canalith jam as a mechanism for BPPV when the nystagmus is (a) Direction fixed with fixation removed and during positional testing; (b) Velocity dependent on supine head position; (c) Converts to geotropic directional changing nystagmus. The extant literature on interpersonal criticism suggests that relative to destructive criticism, constructive criticism significantly minimizes the injurious psychological aftermath people experience after failure. However, we propose the possibility that for some people, these psychological benefits of constructive criticism are less apparent. Specifically, we hypothesized that for people high in trait interpersonal vulnerability, a construct marked by maladaptive cognitive‐emotional responses in interpersonal contexts and/or by dysfunctional relational concerns, effects of constructive (vs. destructive) criticism on reduced maladaptive appraisals will diminish, and this diminution of reduced maladaptive appraisals will predict downstream effects on worsened post‐failure mood. An experiment had a college sample (N = 349) complete four instruments represented under an interpersonal vulnerability construct (IVC). Next, participants imagined failing in various domains and receiving subsequent feedback from emotionally important others. Feedback type was manipulated to typify either destructive criticism or constructive criticism. Finally, participants provided ratings of maladaptive appraisals and post‐failure mood. Latent variable path modelling supported hypotheses. Effects of constructive (vs. destructive) criticism on reduced maladaptive appraisals (namely, shame proneness) diminished as IVC increased, which predicted worsened post‐failure mood. Findings have implications for multidisciplinary theoretical perspectives relevant to interpersonal vulnerability and offer practical considerations for clinicians working with vulnerable clients. Objective: The present research profiled antisocial personality constructs in relation to tactical self-presentation behaviors and various beliefs associated with such tactical behavior.Method: An MTurk sample (N = 524; Mage  = 37.89; 61% female) completed indices of the Dark Triad (DT; narcissism, Machiavellianism, psychopathy) and self-reported their use of various self-presentation tactics, their beliefs about the subjective logic for executing the tactics (which encompassed ratings of the tactics' utility, ease of execution, and normativity), and the potential for each tactic to arouse self-recrimination.Results: Results revealed high convergence between the DT constructs on a relatively malignant approach to self-presentation. DT constructs related to enhanced usage, enhanced subjective logic, and reduced self-recrimination ratings for all the tactics, except pro-social ones (exemplification and apologizing). Nonetheless, results also revealed some notable anticipated instances of nonconvergences between the DT constructs and tactic usage.Conclusions: The findings highlight that DT constructs function rather similarly at the level of self-presentation and suggest value in considering the DT constructs as indicative of strategic, subjectively logical image cultivation and defense behavior. Galphimine-B (G-B), a compound isolated from Galphimia glauca, has been shown to possess important anxiolytic activity. In this study, we evaluated the effectiveness and tolerability of a G-B standardized extract (experimental treatment) that was administered daily for 10 weeks in patients with moderate or severe Generalized Anxiety Disorder (GAD). Alprazolam was used as control treatment and administered under the same conditions. A total of 167 patients were included. At the start of the study, the severe anxiety condition prevailed, with an average on the Hamilton Anxiety Scale of 35.1 ± 8.8 and 35.8 ± 8.1 points in the control and experimental groups, respectively. After the 10 weeks of administration, the average was reduced in the control group to 4.6 ± 6.5 points and in the experimental group to 3.5 ± 5.5 points. Therapeutic success in the control group was 85.7% and in the experimental group, 92.0%. A high proportion of patients (22.2%) treated with Alprazolam manifested daytime sleepiness, while in the group treated with the G-B standardized extract, daytime sleepiness was found in 4.7%. In conclusion, a G-B standardized extract demonstrated therapeutic effectiveness in patients with GAD, without exhibiting significant difference with Alprazolam, but showing fewer cases of daytime sleepiness. Abstract: Selective‐exposure bias refers to the tendency to predominantly seek out attitude‐consistent information and avoid attitude‐inconsistent information. Although researchers have proposed and tested several underlying psychological factors that might contribute to this tendency, the potential role of social influence has not been addressed. In the present research, we address this issue. In four experiments (total N = 645), participants’ selective‐exposure bias was significantly reduced when the bias was portrayed as non‐normative (vs. normative). In Experiment 4, we obtained evidence for the possibility that this social‐norm manipulation could result in effects on attitudes through information selection. Ca2+-sensor proteins are generally implicated in insulin release through SNARE interactions. Here, secretagogin, whose expression in human pancreatic islets correlates with their insulin content and the incidence of type 2 diabetes, is shown to orchestrate an unexpectedly distinct mechanism. Single-cell RNA-seq reveals retained expression of the TRP family members in β-cells from diabetic donors. Amongst these, pharmacological probing identifies Ca2+-permeable transient receptor potential vanilloid type 1 channels ( TRPV1) as potent inducers of secretagogin expression through recruitment of Sp1 transcription factors. Accordingly, agonist stimulation of TRPV1s fails to rescue insulin release from pancreatic islets of glucose intolerant secretagogin knock-out(−/−) mice. However, instead of merely impinging on the SNARE machinery, reduced insulin availability in secretagogin−/− mice is due to β-cell loss, which is underpinned by the collapse of protein folding and deregulation of secretagogin-dependent USP9X deubiquitinase activity. Therefore, and considering the desensitization of TRPV1s in diabetic pancreata, a TRPV1-to-secretagogin regulatory axis seems critical to maintain the structural integrity and signal competence of β-cells. Previous clinical studies have demonstrated the antifungal effectiveness of Ageratina pichinchensis extracts when topically administered to patients with dermatomycosis. The objective of this study was to evaluate the effectiveness and tolerability of a 7% standardized extract of A. pichinchensis (intravaginal) in patients with vulvovaginal candidiasis. The extract was standardized in terms of its encecalin content and administered during 6 days to patients with Candida albicans-associated vulvovaginitis. The positive control group was treated with Clotrimazole (100 mg). On day 7 of the study, a partial evaluation was carried out; it demonstrated that 94.1% of patients treated with Clotrimazole and 100% of those treated with the A. pichinchensis extract referred a decrease or absence of signs and symptoms consistent with vulvovaginal candidiasis. In the final evaluation, 2 weeks after concluding administration, 86.6% of patients in the control group and 81.2% (p = 0.65) of those treated with the A. pichinchensis extract demonstrated therapeutic success. Statistical analysis evidenced no significant differences between the two treatment groups. With the results obtained, it is possible to conclude that the standardized extract from A. pichinchensis, intravaginally administered, showed therapeutic and mycological effectiveness, as well as tolerability, in patients with vulvovaginal candidiasis, without noting statistical differences in patients treated with Clotrimazole. Children exposed in utero to cannabis present permanent neurobehavioral and cognitive impairments. Psychoactive constituents from Cannabis spp., particularly Δ9-tetrahydrocannabinol ( THC), bind to cannabinoid receptors in the fetal brain. However, it is unknown whether THC can trigger a cannabinoid receptor-driven molecular cascade to disrupt neuronal specification. Here, we show that repeated THC exposure disrupts endocannabinoid signaling, particularly the temporal dynamics of CB1 cannabinoid receptor, to rewire the fetal cortical circuitry. By interrogating the THC-sensitive neuronal proteome we identify Superior Cervical Ganglion 10 ( SCG10)/stathmin-2, a microtubule-binding protein in axons, as a substrate of altered neuronal connectivity. We find SCG10 mRNA and protein reduced in the hippocampus of midgestational human cannabis-exposed fetuses, defining SCG10 as the first cannabis-driven molecular effector in the developing cerebrum. CB1 cannabinoid receptor activation recruits c-Jun N-terminal kinases to phosphorylate SCG10, promoting its rapid degradation in situ in motile axons and microtubule stabilization. Thus, THC enables ectopic formation of filopodia and alters axon morphology. These data highlight the maintenance of cytoskeletal dynamics as a molecular target for cannabis, whose imbalance can limit the computational power of neuronal circuitries in affected offspring. Galphimia glauca produces compounds denominated galphimines (galphimine- A, galphimine-B and galphimine- E). Due to their important anxiolytic activity, we initiated in vitro cultures of the species with the purpose of developing a biotechnological process for obtaining galphimines. In this work, we stimulated the biosynthesis and excretion of galphimines with two-phase batch-type cell suspension cultures of G. glauca. The effect of nutritional variation and the 2,4-dichlorophenoxy acetic acid added to Murashige & Skoog (MS) culture medium was evaluated. Later, we evaluated the effect of the stimulation with calcium and methyl jasmonate (MeJ). The greatest production of galphimine-B (3.39 × 10−5 g/L day−1) was obtained on day 40 of kinetics, and induced by a treatment containing concentrations of nitrates and phosphate that are double of those normally used in MS medium, without sucrose but with added 2,4-dichlorophenoxy acetic acid (4 mg/L). Time of galphimine-B biosynthesis diminished due to the effect of Me J in combination with calcium, and induced the excretion (100%) of galphimine-B (6.35 × 10−5 g/L day−1) into the culture medium. Thus, the use of calcium and Me J comprises a viable alternative to stimulate the production and excretion of galphimine-B and galphimine- A in batch-type cultures of G. glauca in modified MS medium. Deficient energy metabolism and network hyperactivity are the early symptoms of Alzheimer's disease ( AD). In this study, we show that administration of exogenous oxidative energy substrates ( OES) corrects neuronal energy supply deficiency that reduces the amyloid-beta-induced abnormal neuronal activity in vitro and the epileptic phenotype in AD model in vivo. In vitro, acute application of protofibrillar amyloid-β1-42 (Aβ1-42) induced aberrant network activity in wild-type hippocampal slices that was underlain by depolarization of both the neuronal resting membrane potential and GABA-mediated current reversal potential. Aβ1-42 also impaired synaptic function and long-term potentiation. These changes were paralleled by clear indications of impaired energy metabolism, as indicated by abnormal NAD(P)H signaling induced by network activity. However, when glucose was supplemented with OES pyruvate and 3-beta-hydroxybutyrate, Aβ1-42 failed to induce detrimental changes in any of the above parameters. We administered the same OES as chronic supplementation to a standard diet to APPswe/ PS1dE9 transgenic mice displaying AD-related epilepsy phenotype. In the ex-vivo slices, we found neuronal subpopulations with significantly depolarized resting and GABA-mediated current reversal potentials, mirroring abnormalities we observed under acute Aβ1-42 application. Ex-vivo cortex of transgenic mice fed with standard diet displayed signs of impaired energy metabolism, such as abnormal NAD(P)H signaling and strongly reduced tolerance to hypoglycemia. Transgenic mice also possessed brain glycogen levels twofold lower than those of wild-type mice. However, none of the above neuronal and metabolic dysfunctions were observed in transgenic mice fed with the OES-enriched diet. In vivo, dietary OES supplementation abated neuronal hyperexcitability, as the frequency of both epileptiform discharges and spikes was strongly decreased in the APPswe/ PS1dE9 mice placed on the diet. Altogether, our results suggest that early AD-related neuronal malfunctions underlying hyperexcitability and energy metabolism deficiency can be prevented by dietary supplementation with native energy substrates. Eukaryotic pseudouridine synthases direct RNA pseudouridylation and bind H/ACA small nucleolar RNA (snoRNAs), which, in turn, may act as precursors of microRNA-like molecules. In humans, loss of pseudouridine synthase activity causes dyskeratosis congenita (DC), a complex systemic disorder characterized by cancer susceptibility, failures in ribosome biogenesis and telomere stability, and defects in stem cell formation. Considering the significant interest in deciphering the various molecular consequences of pseudouridine synthase failure, we performed a loss of function analysis of minifly ( mfl), the pseudouridine synthase gene of Drosophila, in the wing disc, an advantageous model system for studies of cell growth and differentiation. In this organ, depletion of the mfl-encoded pseudouridine synthase causes a severe reduction in size by decreasing both the number and the size of wing cells. Reduction of cell number was mainly attributable to cell death rather than reduced proliferation, establishing that apoptosis plays a key role in the development of the loss of function mutant phenotype. Depletion of Mfl also causes a proliferative disadvantage in mosaic tissues that leads to the elimination of mutant cells by cell competition. Intriguingly, mfl silencing also triggered unexpected effects on wing patterning and cell differentiation, including deviations from normal lineage boundaries, mingling of cells of different compartments, and defects in the formation of the wing margin that closely mimic the phenotype of reduced Notch activity. The Ca2+-binding proteins (CBPs) calbindin D28k, calretinin and parvalbumin are phenotypic markers of functionally diverse subclasses of neurons in the adult brain. The developmental dynamics of CBP expression are precisely timed: calbindin and calretinin are present in prospective cortical interneurons from mid-gestation, while parvalbumin only becomes expressed during the early postnatal period in rodents. Secretagogin (scgn) is a CBP cloned from pancreatic β and neuroendocrine cells. We hypothesized that scgn may be expressed by particular neuronal contingents during prenatal development of the mammalian telencephalon. We find that scgn is expressed in neurons transiting in the subpallial differentiation zone by embryonic day (E)11 in mouse. From E12, scgn+ cells commute towards the extended amygdala and colonize the bed nucleus of stria terminalis, the interstitial nucleus of the posterior limb of the anterior commissure, the dorsal substantia innominata (SI) and the central and medial amygdaloid nuclei. Scgn+ neurons can acquire a cholinergic phenotype in the SI or differentiate into GABA cells in the central amygdala. We also uncover phylogenetic differences in scgn expression as this CBP defines not only neurons destined to the extended amygdala but also cholinergic projection cells and cortical pyramidal cells in the fetal nonhuman primate and human brains, respectively. Overall, our findings emphasize the developmentally shared origins of neurons populating the extended amygdala, and suggest that secretagogin can be relevant to the generation of functional modalities in specific neuronal circuitries. Objective: To determine which adipocytokines are differentially expressed as a function of body mass index (BMI), to compare expression of adipocytokines in abdominal subcutaneous and omental fat, and to correlate these findings with serum levels, BMI, and parameters of insulin resistance.Methods: Serum and subcutaneous (sc) and omental (om) tissue were obtained from lean and obese ovulatory women undergoing gynecologic surgery. We determined adipocytokine expression in sc versus om abdominal fat and related this to increasing BMI.Results: Serum leptin was higher and adiponectin lower in overweight subjects. Adipocytokines had higher expression in sc abdominal versus om adipose tissue with the most significant difference observed for leptin (P=0.01). As BMI increased, sc leptin expression increased and adiponectin expression decreased. The leptin/adiponectin ratio correlated significantly with BMI (R=0.84, P=0.0001).Conclusion: Increased adipocytokine expression correlates with BMI. Abdominal sc tissue has greater adipocytokine expression overall. The serum leptin/adiponectin ratio is highly correlated with BMI. : Near-infrared spectroscopy (NIRS) is a noninvasive optical monitor of regional cerebral oxygen saturation (rSO2). The aim of this study was to validate the use of NIRS by cerebral oximetry in estimating invasively measured mixed venous oxygen saturation (SvO2) in pediatric postoperative cardiac surgery patients.: Twenty patients were enrolled following cardiac surgery with intraoperative placement of a pulmonary artery (PA) or superior vena cava (SVC) catheter. Five patients underwent complete biventricular repair– complete atrioventricular canal (n = 3) and other (n = 2). Fifteen patients with functional single ventricle underwent palliative procedures– bidirectional Glenn (n = 11) and Fontan (n = 4). Cerebral rSO2 was monitored via NIRS (INVOS 5100) during cardiac surgery and 6 h postoperatively. SvO2 was measured from blood samples obtained via an indwelling PA or SVC catheter and simultaneously correlated with rSO2 by NIRS at five time periods: in the operating room after weaning from cardiopulmonary bypass, after sternal closure, and in the CICU at 2, 4, and 6 h after admission.: Each patient had five measurements (total = 100 comparisons). SvO2 obtained via an indwelling PA or SVC catheter for all patients correlated with rSO2 obtained via NIRS: Pearson's correlation coefficient of 0.67 (P < 0.0001) and linear regression ofr2 = 0.45 (P < 0.0001). Separate linear regression of the complete biventricular repairs demonstrated anr = 0.71,r2 = 0.50 (P < 0.0001). Bland–Altman analysis showed a bias of +3.3% with a precision of 16.6% for rSO2 as a predictor of SvO2 for all patients. Cerebral rSO2 was a more accurate predictor of SvO2 in the biventricular repair patients (bias−0.3, precision 11.8%), compared with the bidirectional Glenn and Fontan patients.: Regional cerebral oximetry via NIRS correlates with SvO2 obtained via invasive monitoring. However, the wide limits of agreement suggest that it may not be possible to predict absolute values of SvO2 for any given patient based solely on the noninvasive measurement of rSO2. Near-infrared spectroscopy, using the INVOS 5100 cerebral oximeter, could potentially be used to indicate trends in SVO2, but more studies needs to be performed under varying clinical conditions.
