Fish behavior after passage or transfer around dams is a critical component in determining whether the goals of these efforts are achieved, but these behaviors are often poorly understood. An elevator was constructed in the lowermost hydroelectric dam on the Menominee River, Wisconsin–Michigan; it is the first elevator specifically designed to capture Lake Sturgeon Acipenser fulvescens for upstream transfer above two dams, providing access to high‐quality spawning and early life habitat. Our objectives were to determine whether (1) Lake Sturgeon transferred upstream remained upstream for at least one spawning opportunity; (2) spawning opportunity, time to reach the next dam upstream, and residency in different segments of the river were related to sex, capture method (elevator versus electrofishing), and season of transfer; and (3) the probability of fish transitioning back downstream of the two dams varied among months. We evaluated posttransfer behaviors of 139 Lake Sturgeon that were captured in the elevator or by electrofishing, implanted with acoustic transmitters, transferred upstream (in spring or fall) from fall 2014 to spring 2017, and monitored until fall 2018 using 20–23 stationary acoustic receivers deployed throughout the river. Most Lake Sturgeon (91%) remained upstream for at least one spawning opportunity. The probability of remaining for one spawning opportunity was not related to sex, fish capture method, or season of transfer. Residency times within the two impoundments and time to reach the next dam upstream varied among individual fish. A multistate model indicated that monthly survival after upstream transfer was high and that Lake Sturgeon typically remained above both dams in late fall to early spring, with most downstream movements occurring in April and May. Our results indicate that Lake Sturgeon transferred upstream have the potential to contribute offspring that may help to bolster the Lake Sturgeon population in Lake Michigan, but additional research may help in determining whether these contributions occur. Impact Statement Lake Sturgeon that were transferred upstream of two dams remained upstream of the dams for at least one spawning opportunity, representing the first step in determining whether transfers can be used to increase Lake Sturgeon abundance in the Great Lakes and beyond. Background. Little research has been conducted to explore the postoperative outcomes of obese patients after transfemoral transcatheter aortic valve replacement (TF-TAVR). Objective. We investigated the influence of body mass index (BMI) on 1-year outcomes after TF-TAVR. Methods. We included retrospectively 1609 high- and intermediate-risk TAVR patients (mean EuroSCORE II 21 ± 11) operated under general anesthesia between March 2014 and March 2018 in central hospital, Bad Berka, Germany. We stratified the patients according to BMI. Results. Our demographic data analysis showed 41% of patients were male and the mean age was 78 (range, 61–92 years). According to the WHO classification, 33% patients had normal weight, 42% were defined as overweight, and 22% were obese. Obese patients showed statistically significant difference in their clinical parameters as having higher incidence of hypertension, diabetes mellitus, pulmonary hypertension, and chronic obstructive pulmonary disease; on the contrary, obese patients were found to be younger than others. We found no differences in valve types and sizes among the different BMI categories. Our mortality rate during the 1-year follow-up period was 17.8% (287 patients). Mortality was significantly higher in patients with BMI < 25 kg/m2 (1 year mortality 149 patients 28.2% in patients with BMI < 25 kg/m2 vs. 138 patients 12.6% in patients with BMI ≥ 25 kg/m2; P = 0.0001). Even after considering the confounding risk factors, BMI ≥ 25 kg/m2 was independently associated with reduced 1 year mortality (odds ratio (OR): 0.36, 95% confidence interval (CI): 0.21–0.6; P = 0.01) in multivariate logistic regression analysis. The rate of vascular complication was higher in patients with BMI ≥ 25 kg/m2. However, the rate of blood transfusion was higher in patients with BMI < 25 kg/m2. All other serious complications occurred with equal distribution in both groups. Conclusion. In our single-center study, BMI ≥ 25 kg/m2 was independently associated with lower 1 year mortality after TF-TAVR. [ABSTRACT FROM AUTHOR]

Background: Transjugular intrahepatic portosystemic shunt (TIPS) placement leads to a reduction in portal pressure and an improvement in survival in patients with recurrent and refractory ascites and variceal haemorrhage. Prediction of post-TIPS survival is primarily determined by factors identified before the TIPS procedure, as data collected during or after TIPS implantation are limited. The aim of the study was to evaluate the influence of early hemodynamic changes after TIPS placement on survival, in order to refine post TIPS management., Methods: In this prospective bicentric study, consecutive patients (n = 105) undergoing TIPS placement for ascites or variceal haemorrhage underwent measurement of portal pressure gradient (PPG) immediately at TIPS insertion (PPG0) and 24 h later (PPG24h) and the ΔPPG was calculated from PPG24h and PPG0 (ΔPPG = PPG24h-PPG0). Kaplan-Meier survival analysis and uni- and multivariable regression analyses were conducted to identify survival predictors., Results: Patients with lack of increased ΔPPG exhibited poorer 90-day and 1-year survival compared to patients with increased ΔPPG. This worse survival was independent of The Model for End-Stage Liver Disease (MELD) score, Child-Pugh score, bilirubin levels, creatinine and the Freiburg index of post-TIPS survival (FIPS) > 0.92. Among these patients with poorer outcome, elevated bilirubin (> 25 μmol/L) further distinguished survivors from non-survivors., Conclusion: Lack of increased ΔPPG post-TIPS insertion identifies a high-risk patient group with worse survival.
