We use measurements of trace gases from the Microwave Limb Sounder and polar stratospheric clouds (PSCs) from the Cloud‐Aerosol Lidar with Orthogonal Polarization to investigate how the extraordinary stratospheric water vapor enhancement from the 2022 Hunga eruption affected polar processing during the 2023 Antarctic winter. Although the dynamical characteristics of the vortex itself were generally unexceptional, the excess moisture initially raised PSC formation threshold temperatures above typical values. Cold conditions, especially in early July, prompted ice PSC formation and unusually severe irreversible dehydration at higher levels (500–700 K), while atypical hydration occurred at lower levels (380–460 K). Heterogeneous chemical processing was more extensive, both vertically (up to 750–800 K) and temporally (earlier in the season), than in prior Antarctic winters. The resultant HCl depletion and ClO enhancement redefined their previously observed ranges at and above 600 K. Albeit unmatched in the satellite record, the early‐winter upper‐level chlorine activation was insufficient to induce substantial ozone loss. Chlorine activation, denitrification, and dehydration processes ran to completion by July/August, with trace gas evolution mostly following the climatological mean thereafter, but with chlorine deactivation starting slightly later than usual. While cumulative ozone losses at 410–550 K were relatively large, probably because of the delayed chlorine deactivation, they were not unprecedented. Thus, ozone depletion was unremarkable throughout the lower stratosphere. Although Hunga enhanced PSC formation and chemical processing in early winter, saturation of lower stratospheric denitrification, dehydration, and chlorine activation (as is typical in the Antarctic) prevented an exceptionally severe ozone hole in 2023. Plain Language Summary: The 2022 eruption of the Hunga volcano produced an increase in stratospheric water vapor unmatched in the satellite record. Excess moisture pervaded the Antarctic stratosphere at the beginning of the 2023 Southern Hemisphere winter. Cold moist conditions prompted formation of ice clouds in the Antarctic stratosphere at higher altitudes than usual. Falling ice particles caused unusually severe permanent removal of water vapor from these levels and deposited it at lower altitudes. Chemical processing on polar stratospheric cloud particles occurred both earlier in the season and over a broader vertical domain than typical, resulting in exceptional early‐winter upper‐level conversion of chlorine from nonreactive into ozone‐destroying forms. Chemical processing ran its course by midwinter (i.e., July/August), with trace gases, including ozone, mostly following average behavior thereafter. Deactivation (reconversion of chlorine into nonreactive forms) started slightly later than in most years. While cumulative seasonal ozone losses were relatively large at some levels, probably because of the delayed chlorine deactivation, they were not unprecedented. Thus, although Hunga accelerated and expanded the vertical extent of chemical processing in early winter, that chemical processing reached completion as usually happens in Antarctic midwinter, preventing an extraordinarily severe ozone hole in 2023. Dietary selenium restriction in mammals causes bodily selenium to be preferentially retained in the brain relative to other organs. Almost all the known selenoproteins are found in brain, where expression is facilitated by selenocysteine (Sec)-laden selenoprotein P. The brain also expresses selenocysteine lyase (Scly), an enzyme that putatively salvages Sec and recycles the selenium for selenoprotein translation. We compared mice with a genetic deletion of Scly to selenoprotein P (Sepp1) knockout mice for similarity of neurological impairments and whether dietary selenium modulates these parameters. We report that Scly knockout mice do not display neurological dysfunction comparable to Sepp1 knockout mice. Feeding a low-selenium diet to Scly knockout mice revealed a mild spatial learning deficit without disrupting motor coordination. Additionally, we report that the neurological phenotype caused by the absence of Sepp1 is exacerbated in male vs. female mice. These findings indicate that Sec recycling via Scly becomes limiting under selenium deficiency and suggest the presence of a complementary mechanism for processing Sec. Our studies illuminate the interaction between Sepp1 and Scly in the distribution and turnover of body and brain selenium and emphasize the consideration of sex differences when studying selenium and selenoproteins in vertebrate biology. Background/Aims: The aims of this study were to clarify the relationship between body mass index (BMI) and sexual difficulties and to investigate if BMI influenced sexual satisfaction, over and above the effects of sexual difficulties. Methods: Cross-sectional analyses of a nationally representative computer-assisted telephone interview. Eight thousand, six hundred and fifty-six respondents were recruited by random digit dialling in 2004-2005. Only those in a sexually active, heterosexual relationship were included in the current analyses. Results: After adjustments for demographic factors, both overweight and obese male and female participants were more likely to report worrying during sex about whether their body was unattractive. Among women, associations were also found between higher BMI and lack of interest in sex. No other significant associations between BMI and sexual difficulties were evident. There was an association between BMI and extreme physical pleasure for women but not men over and above the effects of sexual difficulties, with obese women being more likely than normal weight women to report extreme physical pleasure. No associations were found for either men or women between BMI and whether or not they reported extreme emotional or sexual satisfaction with their relationship. Conclusions: With the exception of body image difficulties, there is little association between BMI and self-reported sexual difficulties. Furthermore, extreme sexual and emotional satisfaction appeared to be associated with the presence or absence of sexual difficulties and not overly influenced by BMI. Objectives Given the importance of adherence to combination antiretroviral therapy (cART) for the reduced morbidity and improved mortality of people living with HIV infection (PLWH), we set out to determine which of a number of previously investigated personal, socioeconomic, treatment-related and disease-related factors were independently associated with self-reported difficulty taking antiretroviral therapy (ART) in an Australian sample of PLWH. Methods Using data from a national cross-sectional survey of 1106 PLWH, we conducted bivariate and multivariable analyses to assess the association of over 70 previously investigated factors with self-reported difficulty taking ART. Factors that maintained an association with reported difficulty taking ART at the level of α=0.05 in the multivariable logistic regression analysis were considered to be independently associated with reported difficulty taking ART. Results A total of 867 (78.4%) survey respondents were taking antiretroviral medication at the time of completing the HIV Futures 6 survey. Overall, 39.1% of these respondents reported difficulty taking ART. Factors found to be independently associated with reported difficulty taking ART included younger age, alcohol and party drug use, poor or fair self-reported health, diagnosis of a mental health condition, living in a regional centre, taking more than one ART dose per day, experiencing physical adverse events or health service discrimination, certain types of ART regimen and specific attitudes towards ART and HIV. Conclusions Thirteen previously investigated factors were found to be independently associated with reported difficulty taking ART, reaffirming the dynamic nature of adherence behaviour and the ongoing importance of addressing adherence behaviour in the clinical management of PLWH. Setting Australia-wide, including cities, regional towns, and rural areas. Population A total of 3448 Australian women aged between 16 and 64 years. Methods Women were surveyed using random-digit dialling throughout 2004 and 2005. Main outcome measures Prevalence of sexual problems and ratings of sexual satisfaction, relationship satisfaction, and sexual pleasure. Results From a weighted sample of 2721 women, 447 (16.4%) reported having had a tubal ligation, with 85.0% currently aged between 40 and 64 years. Having a tubal ligation was not associated with any specific sexual problem, such as physical pain during sex or an inability to reach orgasm. In fact, after controlling for age and other sociodemographic differences, sterilised women were significantly less likely than non-sterilised women to lack an interest in having sex (OR 0.69, 95% CI 0.54–0.89), to take ‘too long’ to reach orgasm (OR 0.69, 95% CI 0.50–0.96), to experience vaginal dryness during sex (OR 0.70, 95% CI 0.50–0.96), and to find sex unpleasurable (OR 0.64, 95% CI 0.46–0.90). Sterilised women were also more likely to experience extremely high levels of sexual satisfaction (OR 1.66, 95% CI 1.27–2.18), relationship satisfaction (OR 1.29, 95% CI 1.01–1.67), and sexual pleasure (OR 1.59, 95% CI 1.20–2.12). Conclusions Our findings suggest no adverse effects, and possibly some benefits, for the sexual lives of women undergoing tubal ligation. This article uses matched employer-employee data for the State of Georgia to examine workers' earnings experience through the information technology (IT) sector's employment boom of the mid-1990s and bust in the early 2000s. The results show that even after controlling for pre-boom individual characteristics, transitioning out of the IT sector to a non-IT industry generally resulted in a large wage penalty. However, IT service workers who transitioned to a non-IT industry still fared better than workers who took a non-IT employment path. For IT manufacturing workers, there is no benefit to having been touched by technology, likely because of the nontransferability of manufacturing experience to other industries. To identify key relationships between the decision to take a break from antiretroviral treatments and potential clinical and social antecedents. An analysis of a self-completed survey of various clinical and social aspects of the lives of people living with HIV/AIDS (PLWHA) distributed nationally in Australia was carried out. The questionnaire was completed by a sample of 894 PLWHA. From this sample, three groups of interest were identified: a ‘no break’ group that had been using antiretrovirals continuously ( n=378), a ‘lifestyle break’ group that had taken a break for purely lifestyle reasons on their last occasion ( n=76), and a ‘clinical break’ group that had taken a break for purely clinical reasons on their last occasion ( n=99). Clinical breaks were found to be longer than lifestyle breaks and involved more consultation with doctors. Differences were also found between the two break types in effects on the respondents' health and well-being. The clinical break group was further found to experience a higher level of health difficulties on a range of variables, while the lifestyle break group was younger, had a longer experience of HIV and its consequences, and engaged in greater participation in activities associated with both pleasure and risk. The analyses offer a pattern of health and social factors that places the HIV-positive individual, as a social actor, squarely within the experience of treatment interruptions. Understanding the social motivations and correlates of treatment interruption offers new challenges in maximizing the efficacy of health maintenance and support for PLWHA. [ABSTRACT FROM AUTHOR]

This article focuses on worker preferences between defined-benefit and defined-contribution pension plans using a sample of faculty members at North Carolina State University (NCSU) in the U.S. Employer-provided pension plans are an important component of compensation for many workers. In general, pension plans can be divided into two basic types: defined benefit and defined-contribution. Defined-benefit plans promise a specified benefit at retirement, whereas defined-contribution plans are based on contributions into individual accounts, with benefits being determined by the value of the fund at retirement. The joint determination of job and pension is not an issue for the faculty at North Carolina State University (NCSU). Within 30 days of being employed, new faculty members at NCSU have a choice of enrolling in the state retirement plan, which is a final pay, defined-benefit plan, or one of three separate defined-contribution plans. Defined-benefit and defined-contribution plans differ in their expected values due to differences in worker characteristics and the types of risk borne by the employee. In a riskless world, a worker expecting to remain with a firm until retirement can determine the value of enrolling in a defined-benefit plan by projecting his or her earnings forward and applying the stated benefit formula.

Background and Objectives: Despite the promise of extended release naltrexone in the treatment of the opioid and alcohol use disorders, challenges with initiation and subsequent adherence have limited its potential. The purpose of this study is to identify the patient and treatment characteristics associated with adherence to extended release naltrexone., Methods: Retrospective cohort study of 155 veterans who initiated the medication in FY 2014 and FY2015. Medical records were abstracted for patient and treatment data including preferred drug and utilization of substance use treatment in the year before and after medication initiation., Results: Sample characteristics include 94% male, 70% domiciled, 60% without current legal problems, 30% employed, and preferred drug being opioids for 55% and alcohol for 45%. The mean of five extended release naltrexone injections did not differ by preferred drug. Treatment variables associated with medication adherence included concurrent substance use residential, individual, group, and psychiatric therapies (all p < .05) with inpatient detoxification admissions halved afterward (p < .0001) ., Discussion and Conclusions: Whereas most studies of extended release naltrexone have focused on patients with either alcohol or opioid use disorders for 6 months, this study allowed for a direct comparison of adherence in both groups over a year. The average treatment persistence in this veteran sample is greater than described in other public sector studies and may illustrate the importance of concurrent psychosocial therapies., Scientific Significance: Results extend the findings of other studies and add to an emerging appreciation of the factors associated with treatment retention for extended release naltrexone. Introduction and Aims: To examine patterns of illicit drug use among Australian Indigenous young people, identify correlates of frequent use separately in urban, regional and remote settings and characterise those who inject., Design and Method: Cross-sectional design at 40 Indigenous events. Self-complete surveys were administered to Indigenous people aged 16-29 years using mobile devices., Results: 2,877 participants completed the survey. One in five reported using cannabis at least weekly in the previous year, but the use of other drugs was less prevalent. Patterns of drug use were largely similar across regions, although more participants in urban and regional areas reported using ecstasy (12% vs 11% vs 5%) and cocaine (6% vs 3% vs 1%) and more reported weekly cannabis use (18% vs 22% vs 14%). Injecting was rare (3%) but those who did inject reported a high incidence of needle sharing (37%). Methamphetamine (37%), heroin (36%) and methadone (26%) were the most commonly injected drugs, and injecting was related to prison experience (AOR 5.3 95% CI 2.8-10.0)., Discussion and Conclusion: Attention is needed in relation to cannabis use, particularly among those Indigenous young people living in regional and urban settings. Also, although injecting is uncommon, it is associated with prison involvement. Priority must be given to reducing the numbers of Indigenous youth entering justice settings, delaying the age at first entry to justice settings, and reducing the risk of BBV acquisition while in custody through, for example, prison-based NSP, BBV education, and Indigenous-specific treatment that emphasises connection to country and culture. Introduction and Aims: Use of methamphetamine appears to be greater among gay men than in the general population, yet little data are available on factors associated with use, especially among older men. This study identifies factors associated with reported methamphetamine use among older Australian gay men., Design and Methods: Data were collected from an online survey involving 1135 HIV-positive and HIV-negative gay-identified men aged 40 years and older., Results: One in eight men in the sample, or 13%, reported using methamphetamine in the past 12 months. Patterns of reported use were similar across the country, with no significant differences between major states and territories. Reported methamphetamine use was considerably more prevalent among HIV-positive (24%) than HIV-negative men (11%). In a multivariable logistic regression, reported use was significantly greater among men in their 40s compared with those in their 50s and older (P = 0.002), who were single as opposed to being in an ongoing relationship (P = 0.03), who were HIV-positive (P = 0.003), and who reported using other drugs for non-medical purposes in the past 12 months (P < 0.001). Socioeconomic status and psychosocial variables, such as experiences of discrimination and psychological distress, were not significant factors for reported methamphetamine use., Discussion and Conclusions: Relatively high rates of reported use of methamphetamine were found across the country in this online sample of older Australian gay men. Aims: To assess the effectiveness of interventions directed at the prevention or reduction of use of illicit substances by young people or those directed at reducing harm caused by continuing use., Design: A systematic review was conducted. Reports were identified through electronic and hand searching and contact with known workers in the area. Studies were included if they reported evaluations of interventions targeting illicit drug use and provided sufficient detail of the intervention and design of the evaluation to allow judgements to be made of their methodological soundness. Meta-analyses were conducted combining the data of the methodologically sound studies. PARTICIPANTS AND SETTINGS TARGETED BY INTERVENTIONS: Evaluations of interventions were included if their targeted audience included young people aged between 8 and 25 years. Identified evaluations were delivered in a range of settings including: schools and colleges; community settings; the family; medical/therapeutic settings; mass media., Measurements: Data extracted from each report included details of design, content and theoretical orientation of intervention, setting of the intervention, target audience, methods, population size, subject refusal rates, rates of attrition, outcome measures, length of follow-up and findings, including statistical power., Findings: The majority of studies identified were evaluations of interventions introduced in schools and targeting alcohol, tobacco and marijuana simultaneously. These studies were methodologically stronger than interventions targeting other drugs and implemented outside schools. Meta-analyses showed that the impact of evaluated interventions was small with dissipation of programme gains over time. Interventions targeting hard to reach groups have not been evaluated adequately., Conclusions: Effort needs to be directed towards the development of improved evaluative solutions to the problems posed by these groups. There is still insufficient evidence to assess the effectiveness of the range of approaches to drugs education; more methodologically sound evaluations are required. There is also a need to target interventions to reflect the specific needs and experiences of recipients.
