Objectives: Among the diagnostic features of the Neandertal mandible are the broad base of the coronoid process and its straight posterior margin. The adaptive value of these (and other) anatomical features has been linked to the Neandertal's need to cope with a large gape. The present study aims to test this hypothesis with regard to the morphology of the coronoid process. Materials and Methods: This admittedly simple, intuitive hypothesis was tested here via a comparative finite-element study of the primitive versus modified state of the coronoid process, using two-dimensional models of the mandible. Results: Our simulations demonstrate that a large gape has an unfavorable effect on the primitive state of the coronoid process: the diagonal, almost horizontal, component of the temporalis muscle resultant (relative to the long axis of the coronoid process) bends the process in the sagittal plane. Furthermore, we show that the modification of the coronoid process morphology alone reduces the process' bending in a wide gape increasing the compression to tension ratio. Discussion: These results provide indirect evidence in support of the hypothesis that the modification of the coronoid process in Neandertals is necessary for enabling their mandible to cope with a large gape. Mandibular coronoid process hyperplasia is a rare developmental condition. It commonly presents with progressive restriction of mouth opening, due to the impingement of the enlarged and elongated coronoid process on the distal aspect of the zygomatic arch during mandibular movement. It may present unilaterally or bilaterally. Surgical intervention to improve opening is advocated in cases with significant functional impairment, followed by post-operative physiotherapy. A case of bilateral coronoid process hyperplasia in a 28-year-old man treated with bilateral coronoidectomy, with subsequent post-operative relapse in degree of opening successfully treated via manipulation under intravenous sedation is presented. Objectives: To measure the lengths of the force and resistance arms, in order to calculate the mechanical advantage and muscular work of the human temporalis muscle (TM) in brachyfacial (BR) and dolichofacial (DO) subjects.Setting and Sample Population: Mandibles from 49 subjects of both genders (BR n = 9; DO n = 40) from the collection of the Laboratory of Human Anatomy at Universidade de Santa Cruz do Sul, Rio Grande do Sul, Brazil, were analyzed.Material and Methods: The distance between the condylar process and the coronoid process (insertion site of the TM) represented the length of the force arm (LFA ) of the TM. The distance between the condylar process and the mental protuberance represented the length of the resistance arm (LRA ). Thus, the mechanical advantage of the TM was obtained using the following ratio: LFA /LRA , while the muscular work (LRA /LFA ) of the TM was obtained using the inverse of this ratio.Results: When compared with the DO, the parameters of the BR are significantly greater, as shown by the LFA (6.0%) and mechanical advantage (8.2%; p = 0.0078). By contrast, our results show that in the DO, the LRA was 2.4% longer and the muscular work was 10.4% greater (p = 0.0087).Conclusion: The mechanical advantage of the TM in BR subjects is significantly greater than in DO subjects. Moreover, this greater mechanical advantage may explain, at least in part, the higher incidence of temporomandibular dysfunctions in BR subjects. [ABSTRACT FROM AUTHOR]

This article reports the case of a large aneurysmal bone cyst ( ABC) in a 12-year-old girl, who came to the Ophir Loyola Hospital ( Belém, Pará, Brazil) complaining of pain and debilitated because of a secondary infection. Radiographic and tomographic findings presented an expansion/destruction pattern, including a slight narrowing of the oropharynx and destruction of the right mandibular condyle and coronoid process, which according to the literature are rarely affected by this type of cyst. Because of the size and progression of the cyst, the treatment included block resection with a costochondral rib graft. The clinical and radiographic aspects of expansion with destruction, associated with trans-surgical moderate bleeding and the histopathological findings (dense connective tissue and numerous blood spaces) led to a final diagnosis of a mixed type ABC. Forces exerted by fibrous structures on the medial aspect of the canine elbow joint have been reported to be involved in elbow pathology. The purpose of this paper is to assess the relationships of the ligamentous and muscular structures of this region of the canine elbow joint, and how they relate to the medial coronoid process. Six cadavers of adult German shepherd cross-breed dogs were used in this study to make intra-articular and vascular injections of this region. Some joints were dissected and some were frozen to saw sagittal or dorsal cryosections to assess the relationships of the myotendinous structures. The brachialis muscle tendon passed through the division of the bicipital tendon of insertion which formed a fibrous tendon sheath that was reinforced by the oblique ligament. The biceps' brachii's main insertion is the radial tuberosity where it inserts along with the cranial branch of the oblique ligament and the cranial branch of the medial collateral ligament. Rotational and compression forces exerted by the insertion of the biceps brachii-brachialis tendon complex onto the ulna might influence medial coronoid disease. Here we report a new iguanodontian dentary found from the Lower Cretaceous Khok Kruat Formation, Nakhon Ratchasima, northeast Thailand. A unique character, which is an elongated and flat shape of the dentary ramus, makes it possible to assign the specimen to the new genus of non-hadrosaurid iguanodontian, Ratchasimasaurus suranareae gen. et sp. nov. R. suranareae shows both primitive and derived characters, such as a caudally inclined coronoid process and alveolar trough with a primitive crown impression, and a derived buccal shelf between tooth row and coronoid process. The discovery of a new iguanodontian from the Indochina Terrene, considering that the previously reported ' Probactrosaurus-like' iguanodontian, points out a great diversity of this group in the late Early Cretaceous in Thailand, and corresponds to the Asian iguanodontian diversity at that time. The aim of the present study was to verify the relationship between the angle of the coronoid process of the mandible in the latero-lateral direction and electromyographic activity of the anterior part of the temporal muscle in skeletal Class I and III individuals. Forty-five volunteers were assessed subdivided into two groups, according to angle ANB, in Class I and III. Two radiographic examinations were performed, one lateral cephalogram to measure angle ANB and one frontal cephalogram to measure the angle of the coronoid process. The electromyographic examination of the volunteers’ temporal muscles was performed. Statistical analysis of the data showed that the relationship between the angle of the coronoid process and the skeletal class was negative for both classes analysed. The relationship between electromyographic activity of the temporal muscle and angle of the coronoid process was negative for Class I individuals. In the Class III group, smaller angles of the coronoid process were related to higher values of electromyographic activity of the temporal muscle. There was no statistically significant difference between the electromyographic activity of the temporal muscle in skeletal Class I and III individuals. Based on the results obtained, it was concluded that there was no influence of the skeletal classes analysed on the angle of the coronoid; the angle of the coronoid process may be influenced by the electromyographic activity of the temporal muscle in the Class III group; the electromyographic activity of the temporal muscle is not influenced by the skeletal class in Class III individuals. Objective To determine if fatigue microdamage is associated with fragmented medial coronoid process (FMCP). Study Design Prospective study. Animals Thirty-eight dogs were admitted for subtotal coronoid ostectomy as treatment for FMCP. Surgical specimens of medial coronoid process (MCP) were retrieved from treated dogs. Normal MCP were collected from 5 age- and breed-matched dogs and 2 age-matched research beagles that were euthanatized for reasons other than elbow disease. Methods MCP specimens from affected elbows were classified into 4 groups based on disease severity. Specimens were bulk-stained for microdamage using 1% basic fuchsin, embedded in polymethylmethacrylate and sectioned at 130 μm. Specimens were evaluated using epifluorescent microscopy (425–440 excitation, 475 nm barrier filter) for diffuse damage (D.DX), osteocyte loss, and porosity. Cartilage thickness was evaluated using bright field microscopy. Qualitative observations were also made. Results MCP specimens from affected elbows had increased D.Dx, increased osteocyte loss, and greater porosity than normal groups. Specimens classified as severe FMCP (Classes 3, 4) had the most fatigue microdamage. Some Class 1 specimens, defined as having no visible fissuring of the articular cartilage (AC), had large microcracks in trabecular bone. Conclusions Accumulation of subchondral fatigue microdamage, is associated with MCP fragmentation. Fissuring occurs in the subchondral bone before gross fibrillation of articular cartilage develops. Clinical Relevance Accumulation of fatigue microdamage in the MCP is important in the pathogenesis of FMCP. The underlying cause of this microdamage accumulation must be identified before treatment plans that will prevent further osteoarthritis of the elbow joint can be designed. [ABSTRACT FROM AUTHOR]

The shape of and proportions between the surface areas of the medial coronoid process (MCP) and the fovea of the radial head were determined in 88 juvenile dogs and 146 adult dogs grouped as giant, large, mid-sized, chondrodystrophic, or small dogs. Thereby, the longitudinal (length) and transverse (width) extension of the MCP and fovea of the radial head have been measured. Original values were used to describe changes of the parameters attributed to growth. Normalized values (i.e. values expected in case of a width of the fovea of the radial head of 20 mm) were used to determine potential differences between constitutional types. All original values increased during growth ( P < 0.05) except for the width and length of the MCP in chondrodystrophic and small breeds. Normalized values revealed a proportional decrease in width and length of the MCP during growth ( P < 0.05) compared with the radial head. In adults, the normalized MCP was widest in giant dogs followed by large, mid-sized, small, and chondrodystrophic breeds. The MCP was also longest in giant dogs but shortest in large and chondrodystrophic dogs with those of large dogs being significantly ( P < 0.05) shorter than those in giant, mid-sized and small dogs. Present results suggest that a deficiency in length-growth of the MCP – which has been present especially in large dogs – results in smaller humeral contact areas and decreased weight-bearing capacity of the MCP. Because loading forces acting on the MCP increase with body weight, the condition noted in large dogs might increase the risk of fragmentation of the MCP in these. [ABSTRACT FROM AUTHOR]

A 20-week-old male golden retriever, which was not lame and showed no clinical signs of a fragmented medial coronoid process (FCP), was euthanased for another study and perfused intravenously with formaldehyde. Gross dissection revealed no abnormalities within the right elbow joint. The medial coronoid process was excised, embedded in methylmethacrylate, scanned in a microcomputed tomography (microCT) scanner and sectioned for histology. The microCT scans revealed a dense trabecular bone structure, much denser than in other dogs of similar age, which was considered to be responsible for the sclerosis visible at the base of the coronoid process in radiographs. Three-dimensional reconstructions indicated that there was a small step within the subchondral bone, extending from the apex towards the radial notch. Histology revealed a necrotic lesion between locally thickened articular cartilage and the subchondral bone, characteristic of osteochondrosis. [ABSTRACT FROM AUTHOR]

Muscles and bones may dynamically affect the functions of each other, resulting in changes in the activity of the muscle and/or morphological change in the bones. However, alterations of the morphology of the coronoid process have not been documented in relation to the temporal muscle between the mandible and the cranium. Angles of the coronoid process to the line through the right and the left frontotemporale were measured on posteroanterior (PA) cephalograms in patients with normal occlusions and Class II, division 1 malocclusions, and those with true skeletal Class III malocclusions. Standard deviation of the angle of the coronoid process in patients with Class III malocclusions was greater than those subjects with normal occlusion or Class II, division 1 malocclusions. Differences of angles between the right and the left coronoid process in each patient of either sex were significantly large in true skeletal Class III malocclusions (both P < 0·001). Large variation of angle of the coronoid process was related to prognathism, and this effect may represent adaptation of temporal muscle function to a variety of alterations in mandibular morphology. An unusual accidental displacement of a mandibular third molar crown into the parapharayngeal space in an adult female patient is reported. In combination with clinical examination, conventional and advanced radiographic techniques were utilised in order to locate the crown for retrieval surgery. Under general anaesthesia, an incision starting from buccal sulcus towards distobuccal angle of the second molar at gingival margin was extended to the coronoid process.
