Surgeons from LMICs and HICs have distinct needs and challenges [[4]], and authorship alone is not necessarily a definitive measure of which parties are directing the Global Surgery agenda. As various academic institutions and advocacy groups have entered the Global Surgery space, peer-reviewed publications pertaining to surgical care in low- and middle-income countries (LMICs) have dramatically increased. Indeed, if these HIC surgeons do not produce publications in which they are designated as "first" or "last" authors, they may lose their academic positions and with it the privilege to participate in Global Surgery efforts.
