A growing number of studies suggest that individuals can develop long‐term foraging specializations independently of phenotypic or environmental variation, yet little is known about how the foraging niche is acquired. The early learning of the foraging niche hypothesis suggests a key role of vertical cultural transmission in shaping the foraging niche of vertebrates. In birds, direct evidence from natural conditions is limited to a single study that cross‐fostered two related species. To date, no study has tested whether the diet received as an offspring determines the diet delivered as a parent within a single species. We tested the early learning of the foraging niche hypothesis using a Mediterranean population of great tits Parus major, which show great diet variability and moderate consistency in the diet they provide to their offspring across years. To do this, we recorded prey delivered to 9–14 day‐old chicks over twelve years. Then we assessed vertical transmission of dietary specialization using data (percentage of caterpillars, spiders, and other prey types, as well as mean prey size) from individuals recorded as a chick and as an adult. We standardised the data to control for environmental factors and ran a Linear Model for each prey type to measure individuals' consistency within the group (relative consistency), correlating the diet they received as a chick and the one they provided to their own chicks at the adult stage. The correlations between the diet received as a chick and the diet provided as a parent were either not significant or negative. Hence, although individuals showed relatively consistent foraging niches across years regarding their parental provisioning behaviour, these diet preferences were not correlated to the diet they received in the nest. Background and Objective: To evaluate the diagnostic accuracy and clinical usefulness of endobronchial ultrasound‐guided transbronchial needle aspiration (EBUS‐TBNA) for mediastinal staging of centrally located T1N0M0 non‐small cell lung cancer (NSCLC) clinically staged with positron emission tomography/computed tomography (PET/CT). Methods: We conducted a study that included patients with centrally located T1N0M0 NSCLC, clinically staged with PET/CT who underwent EBUS‐TBNA for mediastinal staging. Patients with negative EBUS‐TBNA underwent mediastinoscopy, video‐assisted mediastinoscopic lymphadenectomy (VAMLA) and/or lung resection with systematic nodal dissection, that were considered the gold standard. The sensitivity, specificity, negative predictive value (NPV), positive predictive value (PPV), overall accuracy of EBUS‐TBNA for diagnosing mediastinal metastases (N2 disease) and the number needed to treat (NNT: number of patients needed to undergo EBUS‐TBNA to avoid a case of pathologic N2 disease after resection) were calculated. Results: One‐hundred eighteen patients were included. EBUS‐TBNA proved N2 disease in four patients. In the remaining 114 patients who underwent mediastinoscopy, VAMLA and/or resection there were two cases of N2 (N2 prevalence 5.1%). The sensitivity, specificity, NPV, PPV and overall accuracy for diagnosing mediastinal metastases (N2 disease) were of 66%, 100%, 98%, 100% and 98%, respectively. The NNT was 31 (95% CI: 15–119). Conclusion: EBUS‐TBNA in patients with central clinically staged T1N0M0 NSCLC presents a good diagnostic accuracy for mediastinal staging, even in a population with low prevalence of N2 disease. We obtain a fundamental gap estimate for classes of bounded domains with quantitative control on the boundary in a complete manifold with integral bounds on the negative part of the Ricci curvature. This extends the result of Oden, Sung, and Wang [Trans. Amer. Math. Soc. 351 (1999), no. 9, 3533–3548] to Lp$L^p$‐Ricci curvature assumptions, p>n/2$p>n/2$. To achieve our result, it is shown that the domains under consideration are John domains, what enables us to obtain an estimate on the first nonzero Neumann eigenvalue, which is of independent interest. Background: Alzheimer's disease (AD) is a complex disease conditioned by environmental and genetic factors. Over the last few years, Genome‐Wide Association Studies (GWAS) have gained increasing interest representing the basis of a personalized medicine that aims to facilitate interventions at the preclinical stages. In most studies, the integration of the high‐dimensional GWAS data is accomplished via polygenic risk scores (PRSs) representing a measure of the genetic risk associated with a given phenotype. For the first time, this study aimed to explore the predictive value of a PRS developed to predict conversion to AD in patients with mild cognitive impairment (MCI) (AD‐PRS) (Bellenguez et al., 2022) on cognitive decline once the AD dementia stage has been established Method: The modulatory effect of genetics on cognitive decline was studied using a sample of 3921 subjects with AD dementia (clinical dementia rating (CDR) ≥1) evaluated at Ace Alzheimer Center Barcelona. Cognitive decline, operationalized as changes in Mini Mental State Examination (MMSE) scores over time, was modeled using multivariate regression models considering clinical and sociodemographic data, and the AD‐PRS. Interaction effects between the AD‐PRS and the other covariates were explored, and subsequently, mediation analyses were applied. Afterward, Machine Learning techniques were used to select the single nucleotide polymorphisms (SNPs) forming the AD‐PRS with the highest predictive value on cognitive decline. Result: Multivariate regression models showed a significant interaction of the AD‐PRS with years of education (coef =.0006; p‐value =.005). Increased scores of AD‐PRS combined with higher formal education were found to be associated with greater cognitive decline. Furthermore, Machine Learning models identified 18 SNPs related to cognitive decline in AD, demonstrating that genetic information improves the predictive performance of the models. Conclusion: This is the first study that demonstrates the predictive capacity of the AD‐PRS on cognitive decline in AD dementia. Years of education were shown to modulate, but not mediate, the effect of the AD‐PRS on cognitive progression. Additionally, a subset of SNPs comprising the AD‐PRS with the highest predictive value of cognitive decline in AD was identified. Background: Progressive supranuclear palsy (PSP) is a rare neurodegenerative tauopathy. We genotyped all the available histopatologically confirmed PSP cases from the Spanish National Biobank Network and patients with a probable PSP diagnosis recruited from research groups linked to the Dementia Genetics Spanish Consortium (DEGESCO). Here, we present the results of the genome‐wide association study (GWAS) performed after the first stage of this project. Method: DNA samples were genotyped by the Centro Nacional de Genotipado (Santiago de Compostela, Spain) using the Affymetrix Axiom 815K Spanish biobank array. After quality control, we kept 156 histopathological (discovery) and 197 clinical (validation) PSP samples. Then, we selected 650 and 800 sex‐matched controls from GR@ACE/DEGESCO for stratified genome‐wide association of the discovery and validation sets, respectively (Table 1). We ran PC1‐4 adjusted logistic regressions with plink v2.00 and meta‐analyzed the discovery and validation sets using METAL. Finally, we meta‐analyzed our results with 36 previously reported SNPs1 reaching the largest PSP sample size to date (N = 3053). Result: We found a strong genome‐wide significant (GWS) signal associated to the MAPT H1/H2 haplotype (rs8070723 G: OR = 3.40; 95%CI = 2.64‐4.39; p = 2.60·10−20, Figure 1) and identified a previously unreported signal in chromosome 7p13 (rs76827509 T: OR = 0.45; 95%CI = 0.35‐0.58; p = 3.09·10−10). We replicated, with nominal significance, 5 GWS SNPs reported in previous GWAS (Table 2) and found significant effects for NFASC and TOMM40. Because APOE and TOMM40 are in strong linkage disequilibrium, we also checked the effect of APOE variants. We found a protective effect (OR = 0.66; 95%CI = 0.48‐0.90; p = 9.75·10−3) of the rs429358 C allele (APOE ε4) in our dataset. Conclusion: The strong GWS signal of the H1 haplotype confirms the validity of our GWAS results2. We were able to replicate several PSP loci (Table 1). The NFASC variant reached borderline GWS significance after meta‐analysis and is a good PSP risk factor candidate. Finally, the APOE ε4 allele was protective for PSP in our cohort. Background and purpose: Non‐(acute disseminated encephalomyelitis) (non‐ADEM) encephalitis and/or fluid attenuated inversion recovery hyperintense lesions in anti‐myelin‐oligodendrocyte‐glycoprotein‐associated encephalitis with seizures (FLAMES) are rarely described in patients with myelin oligodendrocyte glycoprotein (MOG) antibodies (Abs). The aim was (i) to describe the clinical features and disease course of children and adults with non‐ADEM encephalitis and/or FLAMES associated with MOG Abs and (ii) to describe their association with other central nervous system autoantibodies. Methods: This was a systematic review following the PRISMA guidelines. Patients fulfilled criteria for non‐ADEM encephalitis and/or FLAMES, and all were MOG Ab positive. Results: In total, 83 (79%) patients with non‐ADEM encephalitis (48 also had FLAMES) and 22 (21%) with isolated FLAMES were included. At the first episode, children (n = 45) had more infections (11/45, 24.4%; p = 0.017) and more of the phenotype consisting of non‐ADEM encephalitis (42/45, 93.3%; p = 0.014) than adults (n = 38). Children had more episodes consistent with working memory deficits (25/54, 46.3%; p = 0.014) but fewer psychiatric symptoms (16/54, 29.6%; p = 0.002). Twenty‐eight (40.6%) of 69 patients had N‐methyl‐d‐aspartate receptor (NMDAR) Abs in cerebrospinal fluid (CSF), being more frequent in adults (19/29, 65.5%; p < 0.001). Compared to negatives, positive CSF NMDAR Abs had more relapses (14/20, 70%; p = 0.050), required ventilatory support more frequently (8/34, 23.5%; p = 0.009) and had more psychiatric episodes (28/34, 82%; p < 0.001) or abnormal movements (14/34, 41.2%; p = 0.008). Apart from an older age in FLAMES, positive and negative CSF NMDAR Ab groups shared similar features. Conclusion: Non‐ADEM encephalitis patients with MOG Abs show specific clinical and radiological features, depending on the age at first episode. The presence of MOG Abs in non‐ADEM encephalitis patients should not rule out to test other autoantibodies, especially concomitant NMDAR Abs in patients with suggestive symptoms such as behavioural or movement alterations. Tomato chlorosis virus (ToCV, genus Crinivirus, family Closteroviridae) is an emerging plant virus first identified in the mid‐1990s affecting tomato in Florida. Today, the virus is present in at least 39 countries and territories around the world, and this number has not stopped growing. ToCV prevalence in tomato has been reported to frequently reach 100%, with a consequent loss in production. Although ToCV infects mainly tomato, it has been reported to infect a wide range of plants, both cultivated and wild, including many economically important crops, such as pepper and potato. To date, 119 species of host plants belonging to 28 families have been recorded. ToCV is transmitted in nature by whiteflies belonging to two genera, namely members of the Bemisia tabaci complex and Trialeurodes vaporariorum and Trialeurodes abutiloneus. This review summarises what is known about ToCV, an example of an emerging plant pathogen, and is the basis of the Association of Applied Biologists' Description of Plant Viruses for ToCV. The increasing number of people living with human immunodeficiency virus, HIV, (PLWH) have an elevated incidence of risk for noncommunicable comorbidities, the aetiology of which remains incompletely understood. While sleep disturbances are often reported in PLWH, it is unknown to what extent they relate to changes in the circadian and/or sleep homeostatic processes. We studied the relationship between sleep characteristics, circadian phase, and HIV status in older adults from the HAALSI (Health and Ageing in Africa: a Longitudinal Study of an INDEPTH Community in South Africa) subsample of the Agincourt Health and Demographic Surveillance System in South Africa (n = 187, 36 human immunodeficiency virus positive [HIV+], age: 66.7 ± 11.5 years, range 45—93 years), where HIV prevalence is high and (in contrast to the global north) does not associate significantly with potentially confounding behavioural differences. In participants with valid actigraphy data (n = 172), regression analyses adjusted for age and sex indicated that HIV+ participants had slightly later sleep onset (β =.16, p =.039), earlier sleep offset times (β = −.16, p =.049) and shorter total sleep times (β = −.20, p =.009) compared to the HIV negative (HIV−) participants. In a subset of participants (n = 51, 11 HIV+), we observed a later dim light melatonin onset (DLMO) in HIV+ (21:16 ± 01:47) than in HIV− (20:06 ± 00:58) participants (p =.006). This substantial difference remained when adjusted for age and sex (β = 1.21; p =.006). In 36 participants (6 HIV+) with DLMO and actigraphy data, median phase angle of entrainment was −6 min in the HIV+ group and +1 h 25 min in the HIV− group. DLMO time correlated with sleep offset (ρ = 0.47, p =.005) but not sleep onset (ρ = −0.086, p =.623). Collectively, our data suggest that the sleep phase occurred earlier than what would be biologically optimal among the HIV+ participants. This is the first report of a mistimed circadian phase in PLWH, which has important potential implications for their health and well‐being, especially given the well‐established relationships between circadian asynchrony and sleep deprivation with poorer health outcomes. Introduction: Current recommendations for regional stroke destination suggest that patients with severe acute stroke in non‐urban areas should be triaged based on the estimated transport time to a referral thrombectomy‐capable center. Methods: We performed a post hoc analysis to evaluate the association of pre‐hospital workflow times with neurological outcomes in patients included in the RACECAT trial. Workflow times evaluated were known or could be estimated before transport allocation. Primary outcome was the shift analysis on the modified Rankin score at 90 days. Results: Among the 1,369 patients included, the median time from onset to emergency medical service (EMS) evaluation, the estimated transport time to a thrombectomy‐capable center and local stroke center, and the estimated transfer time between centers were 65 minutes (interquartile ratio [IQR] = 43–138), 61 minutes (IQR = 36–80), 17 minutes (IQR = 9–27), and 62 minutes (IQR = 36–73), respectively. Longer time intervals from stroke onset to EMS evaluation were associated with higher odds of disability at 90 days in the local stroke center group (adjusted common odds ratio (acOR) for each 30‐minute increment = 1.03, 95% confidence interval [CI] = 1.01–1.06), with no association in the thrombectomy‐capable center group (acOR for each 30‐minute increment = 1.01, 95% CI = 0.98–1.01, pinteraction = 0.021). No significant interaction was found for other pre‐hospital workflow times. In patients evaluated by EMS later than 120 minutes after stroke onset, direct transport to a thrombectomy‐capable center was associated with better disability outcomes (acOR = 1.49, 95% CI = 1.03–2.17). Conclusion: We found a significant heterogeneity in the association between initial transport destination and neurological outcomes according to the elapse of time between the stroke onset and the EMS evaluation (ClinicalTrials.gov: NCT02795962). This article examines the meaning of health among middle‐aged and older adults in a rural South African setting, where 72 percent of adults aged 40 and over are living with a major chronic condition, and 81 percent report good or very good health. We draw on a unique mixed‐methods dataset that includes a population‐based survey with disease biomarkers (hypertension, diabetes, HIV), self‐assessments of health including self‐rated health, functional ability and medication use, as well as nested qualitative life history interviews with survey participants including questions about lived experiences of health. We conduct survey trend analysis and ordinal logistic regression, as well as inductive and deductive coding of qualitative interviews, and triangulate findings across data sources. Overall, we find that self‐rated health and functional ability are not associated with biometric disease indicators; however, we find that gendered familial expectations, life course stage, and the socioepidemiological context work together to regulate the salience of illness as people age. The study highlights the utility of research with multiple measures of health in illuminating the challenges of aging amidst the complex epidemiological transitions that increasingly characterize low‐ and middle‐income countries. The tomato chlorosis virus (ToCV), transmitted by whitefly species of the genera Bemisia and Trialeurodes in a semipersistent manner, causes significant losses in solanaceous crops including tomato (Solanum lycopersicum) and sweet pepper (Capsicum annuum). Worldwide reports of natural and experimental infection of sweet pepper plants with ToCV are contradictory, raising the question of whether the critical factor determining infection is related to the susceptibility of sweet pepper cultivars or the genetics of virus isolates. In this work, ToCV isolates obtained from different hosts and geographical origins were biologically and molecularly analysed, transmitted by B. tabaci MEAM1 and MED, and the reaction of different sweet pepper cultivars was evaluated under different environmental conditions. Brazilian ToCV isolates from tomato, potato (S. tuberosum), S. americanum, and Physalis angulata did not infect plants of five sweet pepper cultivars when transmitted by B. tabaci MEAM1. Temperatures did not affect the sweet pepper susceptibility to tomato‐ToCV isolates from São Paulo, Brazil, and Florida, USA. However, sweet pepper‐ToCV isolates from Spain and São Paulo, Brazil, were transmitted efficiently to sweet pepper plants by B. tabaci MEAM1 and MED. Although the results indicated that ToCV isolates from naturally infected sweet pepper plants seem to be better adapted to plants of C. annuum, phylogenetic analyses based on the complete nucleotide sequences of RNA1 and RNA2 as well as the p22 gene did not reveal significant nucleotide differences among them. Introduction/Aims: Very few studies analyzing the pattern of muscle involvement in magnetic resonance imaging (MRI) of patients with McArdle disease have been reported to date. We aimed to examine the pattern of muscle fat replacement in patients with McArdle disease. Methods: We performed a retrospective study including all patients with genetically confirmed McArdle disease followed in our center from January 2010 to March 2021. Clinical data were collected from the medical record. Whole‐body MRI was performed as part of the diagnostic evaluation. The distribution of muscle fat replacement and its severity were analyzed. Results: Nine patients were included. Median age at onset was 7 y (range, 5–58) and median age at the time when MRI was performed was 57.3 y (range, 37.2–72.8). At physical examination, four patients had permanent weakness: in three the weakness was limited to paraspinal muscles, whereas in one the weakness involved the paraspinal and proximal upper limb muscles. Muscle MRI showed abnormalities in six of the seven studied patients. In all of them, fat replacement of paravertebral muscles was found. Other muscles frequently affected were the tongue in three, subscapularis in three, and long head of biceps femoris and semimembranosus in two. Discussion: Our findings suggest that paraspinal muscle involvement is common in McArdle disease and support the need to include this disease in the differential diagnosis of the causes of paraspinal muscle weakness. The equatorial retroflection of the North Brazil Current (NBC) into the Equatorial Undercurrent (EUC) and its posterior tropical recirculation is a major regulator for the returning limb of the Atlantic Meridional Overturning Circulation. Indeed, most surface and thermocline NBC waters retroflect at the equator all the way into the central and eastern Atlantic Ocean, before they recirculate back through the tropics to the western boundary. Here, we use cruise data in the western equatorial Atlantic during April 2010 and reanalysis time series for the equatorial and tropical waters in both hemispheres in order to explore the recirculation pathways and transport variability. During the 1998–2016 period, the annual‐mean EUC transports 15.1 ± 1.3 Sv at 32°W, with 2.8 ± 0.4 Sv from the North Atlantic and 11.4 ± 1.3 Sv from the South Atlantic. At 32°W most of the total EUC transport comes from the western boundary retroflection south of 3°N (7.2 ± 0.9 Sv), a substantial fraction retroflects north of 3°N (5.6 ± 0.4 Sv), and the remaining flow (2.3 Sv) joins through the interior basin. The South Atlantic subtropical waters feed the EUC at all thermocline depths while the North Atlantic and South Atlantic tropical waters do so at the surface and upper‐thermocline levels. The EUC transport at 32°W has a pronounced seasonality, with spring and fall maxima and a range of 8.8 Sv. The 18 yr of reanalysis data shows a weak yet significant correlation with an Atlantic Niño index, and also suggests an enhanced contribution from the South Atlantic tropical waters during 2008–2016 as compared with 1997–2007. Plain Language Summary: In the western margin of the tropical Atlantic Ocean, the North Brazil Current (NBC) carries warm and salty waters across the equator northward into the Caribbean Sea. However, an important fraction turns eastward in the equatorial region, retroflecting into the Equatorial Undercurrent (EUC). We find that indeed most of the NBC‐EUC retroflection water comes from the tropical and subtropical South Atlantic Ocean, but we observe that the EUC is also fed by North Atlantic waters, mostly from the north‐eastern tropical Atlantic. Remarkably, these northern and southern water sources follow different retroflection pathways at different latitudinal bands and density layers. Subtropical water from the South Atlantic is present in all density layers while other sources reach the EUC at shallower layers. We also observe that the water transport associated with the NBC‐EUC retroflection changes along the year, with maxima in spring and fall. Further, from 2008 to 2016 the EUC has increased slightly, associated with a greater contribution of South Atlantic tropical waters. This interannual variability is a result of local and remote changes of the trade wind system of the tropical Atlantic combined with climatic modes of variability such as the Atlantic Niño. Introduction: DeSanto‐Shinawi syndrome is a rare neurodevelopmental disorder caused by loss‐of‐function variants of WAC, located on chromosome 10p12.1. This syndrome is characterized by dysmorphic facial features, intellectual disability, and behavioral problems. Case report: In this case report, we present a new deletion case and summarize the clinical data of previously reported individuals, comparing the similarities and differences between cases caused by point mutations versus those which are caused by deletions in the 10p region. Hamstring muscle injuries are the most prevalent among athletes who engage in sprinting activities. Their most frequent location is where the long head of the biceps femoris joins with the semitendinosus muscle to form the conjoint hamstring tendon. Just distal to this area, an additional group of fibers of the semitendinosus originate from medial aspect of biceps femoris. The objective of this study was to analyze the morphological characteristics of this union and to discuss its potential role in hamstring tears. Anatomical dissection was performed on 35 thighs. Samples obtained from this region were sectioned and stained with Masson's trichrome for further histological evaluation. A group of fibers from the semitendinosus muscle originating from the long head of the biceps femoris were observed in all 35 specimens. This origin was located 67 ± 12 mm from the ischial tuberosity and was 32 ± 14 mm in length. This group of muscle fibers had a width of 10.9 ± 5.3 mm and a thickness in the anteroposterior axis of 3.2 ± 1.4 mm. Its pennation angle was 9.2 ± 1.5 degrees. Microscopic examination showed muscle cells from both muscles contacting interposed tendinous tissue. In conclusion, fibers of the semitendinosus muscle consistently arise from the proximal aspect of the long head of biceps femoris. The morphological characteristics of this junction have functional implications. The horizontal component of the semitendinosus vector could pull the long head of the biceps femoris medially during its shortening—lengthening cycle, rendering it an intrinsic risk factor for hamstring injuries. Purpose The PHIRST study (Prospective Household cohort study of Influenza, Respiratory Syncytial virus, and other respiratory pathogens community burden and Transmission dynamics in South Africa) aimed to estimate the community burden of influenza and respiratory syncytial virus (RSV) including the incidence of infection, symptomatic fraction, and to assess household transmission. Participants: We enrolled 1684 individuals in 327 randomly selected households in a rural and an urban site over three consecutive influenza and two RSV seasons. A new cohort of households was enrolled each year. Participants were sampled with nasopharyngeal swabs twice‐weekly during the RSV and influenza seasons of the year of enrolment. Serology samples were collected at enrolment and before and after the influenza season annually. Findings to Date: There were 122 113 potential individual follow‐up visits over the 3 years, and participants were interviewed for 105 783 (87%) of these. Out of 105 683 nasopharyngeal swabs, 1258 (1%) and 1026 (1%) tested positive on polymerase chain reaction (PCR) for influenza viruses and RSV, respectively. Over one third of individuals had PCR‐confirmed influenza each year. Overall, there was influenza transmission to 10% of household contacts of an index case. Future Plans Future planned analyses include analysis of influenza serology results and RSV burden and transmission. Background: High‐throughput proteomic platforms are an useful strategy to detect novel biomarkers and pathways involved in Alzheimer's disease (AD) development. Due to the novelty of these techniques, an extensive quality control assessment is needed to ensure the reliability of those protein measurements. In this study, we aimed to assess the reproducibility and reliability of SomaScan and Olink proteomic platforms and select top‐performing proteins for further analysis. Method: To assess CSF proteomic measurements, we used the SomaScan 7k platform (an aptamer‐based technique) measuring 7335 human proteins, and Olink proteomics platform (Explore panel, an antibody‐based technique), measuring 3072 proteins in a subset of 509 individuals from ACE Alzheimer Center Barcelona (Spain). To assess intra‐ and inter‐platform reproducibility and reliability, we evaluated 1) the correlations of aptamers of two studies carried out in the same samples with SomaScan platform and 2) the correlation between SomaScan vs Olink proteomics, respectively. Then, we created a metric of 6 categories of reproducibility combining both intra and inter‐ correlation analysis, considering good (r > 0.5), modest (0.2 > r ≥ 0.5), poor (r ≤ 0.2) correlation. We considered proteins in the top 3 categories as potential candidates to further analysis. Result: From the intra‐platform analysis, only 2402 proteins (35.45%) showed a Pearson's correlation higher than 0.5. Regarding inter‐platform assessment, 2160 proteins were overlapping between both platforms. From these overlapping proteins, 712 proteins exhibited a Pearson's r > 0.5 (26.67%). We considered potential candidate proteins those with good intra‐ and inter‐platform correlation (n = 687, 9.02%), only good intra‐platform correlation (n = 2015, 26.45%) and modest intra‐ and inter‐platform correlation (n = 5, 0.06%) for analysis. Conclusion: The validation of the reproducibility and reliability of the new proteomics platforms is essential for the correct analysis of the results. Background: in the clinical course of Alzheimer's disease (AD) development, the dementia phase is commonly preceded by a prodromal AD phase, which is mainly characterized by reaching the highest levels of Aβ and p‐tau‐mediated neuronal injury and a mild cognitive impairment (MCI) clinical status. Because of that, most AD cases are diagnosed when neuronal damage is already established and irreversible. Therefore, a differential diagnosis of MCI causes in these prodromal stages is one of the greatest challenges for clinicians. Blood biomarkers are emerging as desirable tools for pre‐screening purposes, but the current results are still being analyzed and much more data is needed to be implemented in clinical practice. Because of that, plasma exosomes (pEXOs) are gaining popularity as a new source of biomarkers for the early stages of AD development Method: to identify an exosome proteomics signature linked to prodromal AD, we performed a cross‐sectional study on two groups of patients with early‐onset MCI (EOMCI) (Aβ +/‐) in which 184 biomarkers were measured in pEXOs, cerebrospinal fluid (CSF), and plasma samples using multiplex PEA technology of Olink© proteomics. Result: the obtained results showed that proteins measured in pEXOs from EOMCI patients with established amyloidosis correlated with CSF p‐tau181 levels, brain ventricle volume changes, white matter hyperintensities, and MMSE scores. In addition, the correlations of pEXOs proteins with different parameters were able to discriminate between EOMCI Aβ(+) and Aβ(‐) patients, whereas the CSF or plasma proteome did not. Conclusion: our findings suggest that pEXOs could be able to provide information regarding the initial amyloidotic changes of AD. At this early stage, our results may indicate that pEXOs biomarker profile is associated with the AD‐specific neurodegeneration process governed by p‐tau and Aβ. Circulating exosomes may acquire a pathological protein signature of AD before raw plasma, becoming potential biomarkers for identifying subjects at the earliest stages of AD development. Background: The Alzheimer Association has recently remarked the diagnostic and prognostic revolution that plasma biomarkers will mean for AD diagnosis, and highlighting the need of further research to translate them as a realistic option in the clinical practice. However, many studies to date have assessed them in research cohorts. Here we evaluated the diagnostic performance and clinical applicability of plasma pTau181 as a predictive marker of dementia risk in a real‐world memory clinic cohort. Method: The ACE cohort encompass 1649 patients with subjective cognitive decline (SCD), mild cognitive impairment (MCI), AD dementia and other dementias, with a complete battery of neurological tests, biochemistry data, and paired CSF and plasma samples of the Ace Alzheimer Center Barcelona memory clinic. CSF AB42, AB40, pTau181 and tTau, and plasma pTau181 were measured using the Lumipulse G1200 automatic platform (Fujirebio Inc.). Complete cohort were divided in the Testing cohort (n = 1000) and Validation cohort (n = 649). Result: Plasma pTau181 significantly correlated (P<0.0001) with CSF pTau181 in MCI, AD dementia and other dementias, but not in SCD. Sex appeared not significant in plasma pTau181 levels, whereas age significantly influenced in the biomarker's performance. ROC curve of A+T+ patients vs rest showed an AUC = 0.8250 and P<0.0001. Specifically for MCI patients, ROC curve of MCI A+T+ vs MCI A‐T‐ showed and AUC = 0.8874 and P<0.0001. A cut off value of 1.300 pg/ml of plasma pTau181 exhibited a 93.6% sensibility, 72.4% specificity, 77.8% precision and only 8.3% of false negatives in MCI patients. Hazard ratio of survival of conversion analysis showed that MCI patients above this cut off value exhibited a 4.570 higher risk to convert to dementia than MCI under the cut off value. Validation cohort confirmed these findings. Conclusion: Plasma pTau181 could be useful as a pre‐screening biomarker of MCI patients with high risk to progress to dementia. However, positive patients will also require a CSF/PET confirmatory test. Plasma pTau181 by itself does not present a suitable performance in SCD patients. Another biomarker or a combination of biomarkers would be needed to this population. Individuals with posttraumatic stress disorder (PTSD) are at increased risk for the development of various forms of dementia. Nevertheless, the neuropathological link between PTSD and neurodegeneration remains unclear. Degeneration of the human basal forebrain constitutes a pathological hallmark of neurodegenerative diseases, such as Alzheimer's and Parkinson's disease. In this seed‐based resting‐state (rs‐)fMRI study identifying as outcome measure the temporal BOLD signal fluctuation magnitude, a seed‐to‐voxel analyses assessed temporal correlations between the average BOLD signal within a bilateral whole basal forebrain region‐of‐interest and each whole‐brain voxel among individuals with PTSD (n = 65), its dissociative subtype (PTSD+DS) (n = 38) and healthy controls (n = 46). We found that compared both with the PTSD and healthy controls groups, the PTSD+DS group exhibited increased BOLD signal variability within two nuclei of the seed region, specifically in its extended amygdaloid region: the nucleus accumbens and the sublenticular extended amygdala. This finding is provocative, because it mimics staging models of neurodegenerative diseases reporting allocation of neuropathology in early disease stages circumscribed to the basal forebrain. Here, underlying candidate etiopathogenetic mechanisms are neurovascular uncoupling, decreased connectivity in local‐ and large‐scale neural networks, or disrupted mesolimbic dopaminergic circuitry, acting indirectly upon the basal forebrain cholinergic pathways. These abnormalities may underpin reward‐related deficits representing a putative link between persistent traumatic memory in PTSD and anterograde memory deficits in neurodegeneration. Observed alterations of the basal forebrain in the dissociative subtype of PTSD point towards the urgent need for further exploration of this region as a potential candidate vulnerability mechanism for neurodegeneration in PTSD. Southern Ocean waters enter the South Atlantic Ocean through the Scotia Sea along pathways constrained by the bathymetry of the northern Scotia Sea passages. We use the Argo profiling‐float data set to calculate the water transports in and out of the region, focusing on the water balances down to the deepest isoneutral sampled in all passages (γn = 28.0 kg m−3, located between about 500 and 2,000 m in the Drake Passage and even shallower in the Northern Passages). Down to this reference level, the water inflow through the Drake Passage is 140.8 ± 7.4 Sv and the water outflow through the deeper portions of the Northern Passages is 115.9 ± 8.3 Sv, implying a leakage of about 25 ± 11.1 Sv over topography shallower than 1,000 m. Below the reference isoneutral and down to 2,000 m, an additional 23.4 Sv enter through the Drake Passage; when added to reported inputs of about 20 Sv through the South Scotia Ridge, this accounts well for the observed 43.4 Sv outflow–from 28.0 kg m−3 to 2,000 m–through the Northern Passages. Relative to the 2,000 m reference level, the mean barotropic contribution always represents over half the total transports. We also observe substantial seasonal and moderate interannual variations in the water transports and composition (peak differences occur seasonally in the Drake Passage, with a range of 111–174 Sv), associated with changes in water exchange across the frontal systems. Two independent measures set the water mean‐residence time in the Scotia Sea at about 6–8 months. Plain Language Summary: The Scotia Sea is the entryway for Southern Ocean waters into the western South Atlantic Ocean, with waters entering through the Drake Passage (the Southern Ocean narrowest conduit) and exiting through several deep northern passages. Most of this transit water follows convoluted bands characterized by sharp horizontal gradients in water properties (frontal systems). We use the large set of profiles of temperature and salinity from the Argo network of drifting ocean profilers to measure the amount, characteristics, and pathways of those waters entering and leaving the Scotia Sea down to 2,000 m, as well as to assess their seasonal and interannual variability. This data set allows us to explore the water transports and transformations down to a certain ocean density level that is well sampled within all passages. Down to this level, about 141 million cubic meters per second (Sverdrups) enter through the Drake Passage, and 116 Sverdrups eventually exit through the deeper portions of the Northern Passages. Our calculations also reveal variations in water transport at interannual scales, although these are fairly small compared with variability within a year. We also show that most water parcels take about 6–8 months to travel the Scotia Sea along pathways that often cross the frontal systems. Objective: To detect the spatial distribution of the sinkholes of the Peninsula of Yucatan (SPY) and identify those cenotes where microorganisms have been registered. Methods: The geographic coordinates of the SPYs were obtained from various databases, as well as from scientific publications relating to the terminology 'sinkholes', 'karst systems' and 'cenotes'. All coordinates were transformed into the Universal Transverse Mercator reference system (UTM) with datum WGS84. An infrared composite image was created with 432 RGB bands from the Landsat 8 satellite. The points with the location of the cenotes were imported into the Software TerrSet. Results: Total 1026 coordinates of sinkholes were recorded in the Yucatan Peninsula. In 18 sinkholes (<2%), microorganisms have been recovered and identified in various taxonomic levels, and only 6 sinkholes (<0.6%) has their biotechnological potential been evaluated. Conclusions: The microorganisms that inhabit the sinkholes of the Yucatan Peninsula are a reservoir with practically unexplored biotechnological potential. [ABSTRACT FROM AUTHOR]

Introduction: Experiences of violence during youth contravene young people's rights and increase the risk of depression and poor human immunodeficiency virus (HIV) care outcomes among youth living with HIV (YLWH). Intervention targets for mitigating the negative psychosocial effects of violence are needed, particularly in areas like rural South Africa where violence remains pervasive and mental healthcare is limited. This study aims to quantify the associations between physical and sexual violence and depressive symptoms in YLWH in rural South Africa and explore the modification of these associations by key measures of psychosocial well‐being. Methods: We conducted a cross‐sectional survey among 362 YLWH ages 12 to 24 in rural South Africa to ascertain participants' history of physical and sexual violence, current depressive symptoms (Center for Epidemiological Studies‐Depression Scale) and levels of social support (Medical Outcomes Social Support Scale), resilience (Conner‐Davidson Resilience Scale) and self‐esteem (Rosenberg Self‐Esteem Scale). Log‐binomial regression was used to estimate the association between history of physical or sexual violence and clinically meaningful depressive symptoms (scores ≥16). Effect measure modification by high versus low resilience, social support and self‐esteem was assessed using likelihood ratio tests (α = 0.20). Results: A total of 334 individuals with a median age of 21 (interquartile range: 16 to 23) were included in this analysis. Most participants were female (71.3%), single (81.4%) and attending school (53.0%). Ninety‐four participants (28.1%) reported a history of physical or sexual violence and 92 individuals (27.5%) had clinically meaningful depressive symptoms. Meaningful depressive symptoms were significantly higher among participants with a history of physical or sexual violence as compared to those with no history of violence (adjusted prevalence ratio: 2.01; 95% CI: 1.43, 2.83). However, this association was significantly modified by social support (p = 0.04) and self‐esteem (p = 0.02). Conclusions: In this setting, the prevalence of meaningful depressive symptoms was significantly higher among YLWH with a history of physical or sexual violence as compared to those without a history of violence. However, higher levels of self‐esteem or social support appeared to mitigate this association. Summary: Background and objectives: Cutaneous polyarteritis nodosa (CPAN) is a comparatively rare form of vasculitis that affects small arteries and arterioles in the panniculus and dermo‐subcutaneous junction. Limited information is available regarding its course in the European population. The aim of this study is to characterize the manifestations and prognostic markers of recurrence in CPAN. Patients and methods: We report a retrospective study of patients with clinical and histopathologic evidence of CPAN, which was treated at two tertiary referral centers in Spain between 1989 and 2019. Results: 31 patients were included. The most frequent manifestation was subcutaneous nodules (90.3 %); ulcers were frequent at diagnosis (35.5 %). Two thirds of the patients had at least one extracutaneous manifestation. Seventeen patients (54.8 %) experienced relapse. The strongest predictor of recurrence was ulceration in the initial episode (OR 18.6; 95 % CI 2.73–38; p < 0.01). The pre‐treatment results of laboratory parameters associated with inflammation (such as C‐reactive protein and neutrophil‐to‐lymphocyte ratio) were significantly higher in the relapsing group. There were no disease‐related deaths and none of the patients developed systemic PAN. Conclusions: Although CPAN is a vasculitis limited to the skin, symptoms may involve adjacent skeletal muscle or peripheral nerves. While the condition is not life‐threatening, the presence of ulceration and elevation of certain laboratory parameters predicts a worse prognosis. [ABSTRACT FROM AUTHOR]

Systemic sclerosis can involve the skin in a limited or diffuse way. We searched MEDLINE database for a combination of MeSH terms and non-Mesh terms including "systemic sclerosis" "morphea", "keloid", "nodular scleroderma", "keloidal scleroderma", "nodular morphea" and "keloidal morphea". Patients with nodular or keloidal scleroderma seem to show less than half of the joint and oesophageal involvement (34.1% and 41.5%, respectively) compared to patients with systemic sclerosis but without nodular or keloidal plaques and fibrotic skin lesions. Size‐selective harvesting of wild ungulates can trigger a range of ecological and evolutionary consequences. It remains unclear how environmental conditions, including changes in habitat, climate, and local weather conditions, dilute or strengthen the effects of trophy hunting. We analyzed horn length measurements of 2,815 male ibex (Capra pyrenaica) that were harvested from 1995 to 2017 in Els Ports de Tortosa i Beseit National Hunting Reserve in northeastern Spain. We used linear mixed models to determine the magnitude of inter‐individual horn growth variability and partial least square path models to evaluate long‐term effects of environmental change, population size, and hunting strategy on horn growth. Age‐specific horn length significantly decreased over the study period, and nearly a quarter (23%) of its annual variation was attributed to individual heterogeneity among males. The encroachment of pine (Pinus spp.) forests had a negative effect on annual horn growth, possibly through nutritional impoverishment. The harvesting of trophy and selective individuals (e.g., small‐horned males) from the entire population increased horn growth, probably because it reduced the competition for resources and prevented breeding of these smaller males. Local weather conditions and population size did not influence horn growth. Our study demonstrates how habitat changes are altering the horn growth of male ibex. We suggest that habitat interventions, such the thinning of pine forests, can contribute to securing the sustainability of trophy hunting. Even in situations where size‐selective harvesting is not causing a detectable phenotypic response, management actions leading to the expansion of preferred land cover types, such as grass‐rich open areas, can have a positive effect on ungulate fitness. Forest encroachment on open meadows and heterogeneous grasslands is pervasive throughout Mediterranean ecosystems. Therefore, our management recommendations can be extended to the landscape level, which will have the potential to mitigate the side effects of habitat deterioration on the phenotypic traits of wild ibex. © 2020 The Wildlife Society. The encroachment of coniferous forests and the consequent loss of natural pastures had a negative effect on the annual horn growth of male ibex inhabiting a Mediterranean ecosystem. This original article outlines a theoretical path and posterior critical analysis regarding two relevant matters in modern nursing: patterns of knowing in nursing and commodification contexts in contemporary health systems. The aim of our manuscript is to examine the development of basic and contextual nursing knowledge in commodified contexts. For this purpose, we outline a discussion and reflexive dialogue based on a literature search and our clinical experience. To lay the foundation for an informed discussion, we conducted a literature search and selected relevant articles in English, Spanish, and Portuguese that included contents on patterns of knowing, commodification, and nursing published from 1978 to 2017. Globalization, commodification, and austerity measures seem to have negative effects on nursing. Work conditions are worsening, deteriorating nurse–patient relationships, and limiting reflection on practice. Nurses must develop knowledge to challenge and participate in institutional organization and public health policies. Development of nursing knowledge may be difficult to achieve in commodified environments. Consequently, therapeutic care relationships, healthcare services, and nurses' own health are compromised. However, by obtaining organizational, sociopolitical, and emancipatory knowledge, nurses can use strategies to adapt to or resist commodified contexts while constructing basic knowledge. Introduction: Although HIV prevalence is exceptionally high in South Africa, HIV testing rates remain below targeted guidelines. Older adults living with HIV are substantially more likely to remain undiagnosed than younger people. Cognitive function and literacy could play key roles in HIV status knowledge due to the decision‐making processes required around weighing the costs and benefits of testing, navigating testing logistics and processing results. We aimed to assess the independent relationships among each of cognitive function, literacy and education with HIV status knowledge in a population‐based sample of older adults living in a rural South African community with high HIV prevalence. Methods: We analyzed data from a population‐based study of 5059 men and women aged 40 years and older in rural South Africa (Health and Aging in Africa: A Longitudinal Study of an INDEPTH community (HAALSI)). HAALSI surveys, conducted between 2014 and 2015, queried self‐reported literacy, educational attainment and HIV status knowledge. Laboratory tests were conducted to assess true HIV sero‐status. Cognitive function was assessed with a battery of cognitive tests measuring time orientation, immediate and delayed recall, and numeracy and coded using confirmatory factor analysis as a z‐standardized latent variable. We estimated the relationship between the outcome of HIV status knowledge and each of three exposures: (1) latent cognitive z‐score, (2) literacy and (3) education, using confounder‐adjusted modified Poisson regression models in the study population overall and stratified by HIV sero‐status. Results: We found that HIV status knowledge was higher among those with higher cognitive z‐scores (adjusted Prevalence Ratio (aPR) (95% CI): 1.18 (1.14, 1.21) per standard deviation unit), and among literate participants (aPR (95% CI): 1.24 (1.16, 1.32) vs. non‐literate participants). Taken together, the associations with literacy and cognitive function completely attenuated the otherwise positive association between educational attainment and HIV status knowledge. The magnitudes of effect were generally similar among laboratory‐confirmed HIV‐negative and HIV‐positive participants. Conclusions: Campaigns that target older adults in rural South Africa with HIV testing messages should carefully consider the cognitive and literacy levels of the intended audience. Objective: Recently, the ASC-1 complex has been identified as a mechanistic link between amyotrophic lateral sclerosis and spinal muscular atrophy (SMA), and 3 mutations of the ASC-1 gene TRIP4 have been associated with SMA or congenital myopathy. Our goal was to define ASC-1 neuromuscular function and the phenotypical spectrum associated with TRIP4 mutations.Methods: Clinical, molecular, histological, and magnetic resonance imaging studies were made in 5 families with 7 novel TRIP4 mutations. Fluorescence activated cell sorting and Western blot were performed in patient-derived fibroblasts and muscles and in Trip4 knocked-down C2C12 cells.Results: All mutations caused ASC-1 protein depletion. The clinical phenotype was purely myopathic, ranging from lethal neonatal to mild ambulatory adult patients. It included early onset axial and proximal weakness, scoliosis, rigid spine, dysmorphic facies, cutaneous involvement, respiratory failure, and in the older cases, dilated cardiomyopathy. Muscle biopsies showed multiminicores, nemaline rods, cytoplasmic bodies, caps, central nuclei, rimmed fibers, and/or mild endomysial fibrosis. ASC-1 depletion in C2C12 and in patient-derived fibroblasts and muscles caused accelerated proliferation, altered expression of cell cycle proteins, and/or shortening of the G0/G1 cell cycle phase leading to cell size reduction.Interpretation: Our results expand the phenotypical and molecular spectrum of TRIP4-associated disease to include mild adult forms with or without cardiomyopathy, associate ASC-1 depletion with isolated primary muscle involvement, and establish TRIP4 as a causative gene for several congenital muscle diseases, including nemaline, core, centronuclear, and cytoplasmic-body myopathies. Objective: To estimate the incidence and analyze any cancer-associated factors in patients with systemic lupus erythematosus (SLE), differentiating between hormone-sensitive (HS) and non-HS cancers.Methods: This was a retrospective multicenter study of a patient cohort from the Systemic Lupus Erythematosus Registry of the Spanish Society of Rheumatology. Included were the first cancer post-SLE diagnosis, clinical and sociodemographic information, cumulative damage, severity, comorbidities, treatments, and refractoriness. Cancers were classified as HS (prostate, breast, endometrium, and ovarian) and non-HS (the remainder). The standardized incidence ratio (SIR) was calculated and logistic regression models were built.Results: A total of 3,539 patients (90.4% women) were included, 154 of whom had cancer (91% female), and 44 had HS cancer (100% female). The cancer SIR was 1.37 (95% confidence interval [95% CI] 1.15-1.59), with higher values in women age <65 years (SIR 2.38 [95% CI 1.84-2.91]). The SIR in women with HS versus non-HS cancer was 1.02 (95% CI 0.13-1.91) and 1.93 (95% CI 0.98-2.89). In HS versus non-HS cancers, SLE diagnostic age (odds ratio [OR] 1.04 [P = 0.002] versus 1.04 [P = 0.019]), and period of disease evolution (OR 1.01 [P < 0.001] versus 1.00 [P = 0.029]) were associated with cancer. The Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index (OR 1.27 [P = 0.022]) and angiotensin-converting enzyme (ACE) inhibitor prescriptions (OR 2.87 [P = 0.048]) were associated with non-HS cancers.Conclusion: Cancer incidence in patients with SLE was higher than in the Spanish population, particularly among young women. Background: Hippocampal sclerosis of aging (HS) is one of the most predominant causes of dementia in elder populations and it is often misdiagnosed as Alzheimer's disease (AD). HS has recently been found to show a hippocampal atrophy pattern that might be key for the differentiation of this pathology from AD in the early stages of dementia. Method: In this study, we aimed to find SNPs associated with AD diagnosis that might be related to HS. To that end, we calculated a polygenic risk score (PRS) combining SNPs associated with AD by Bellenguez et al. (2022) and assessed their relation with HS‐by‐proxy based on magnetic resonance imaging (MRI) data of hippocampal subfield volumes from 1,130 individuals without dementia syndrome. Result: Our results show a significant association of AD‐PRS with fimbria and hippocampal body and head volumes. Association with these hippocampal subfields was also observed on the individual effects of the AD‐related variants in the SHARPIN (rs34173062) and TNIP1 (rs871269) genes. Besides, these loci remained consistently associated to HS‐aging after increasing the sample size by including an independent cohort of healthy individuals (N = 729). Conclusion: The results presented in this study suggest that some SNPs that have recently been identified as AD variants might be closely related to atrophy in hippocampal subfields instead of AD, and hence, could be instrumental to refine AD and HS pathways, their interrelationships, and to elaborate specific treatment for each condition. Summary: Tomato chlorosis virus (ToCV) causes an important disease that primarily affects tomato, although it has been found infecting other economically important vegetable crops and a wide range of wild plants. First described in Florida (USA) and associated with a 'yellow leaf disorder' in the mid‐1990s, ToCV has been found in 35 countries and territories to date, constituting a paradigmatic example of an emergent plant pathogen. ToCV is transmitted semipersistently by whiteflies (Hemiptera: Aleyrodidae) belonging to the genera Bemisia and Trialeurodes. Whitefly transmission is highly efficient and cases of 100% infection are frequently observed in the field. To date, no resistant or tolerant tomato plants are commercially available and the control of the disease relies primarily on the control of the insect vector. Taxonomy: Tomato chlorosis virus is one of the 14 accepted species in the genus Crinivirus, one of the four genera in the family Closteroviridae of plant viruses. Virion and genome properties: The genome of ToCV is composed of two molecules of single‐stranded positive‐sense RNA, named RNA1 and RNA2, separately encapsidated in long, flexuous, rod‐like virions. As has been shown for other closterovirids, ToCV virions are believed to have a bipolar structure. RNA1 contains four open reading frames (ORFs) encoding proteins associated with virus replication and suppression of gene silencing, whereas RNA2 contains nine ORFs encoding proteins putatively involved in encapsidation, cell‐to‐cell movement, gene silencing suppression and whitefly transmission. Host range: In addition to tomato, ToCV has been found to infect 84 dicot plant species belonging to 25 botanical families, including economically important crops. Transmission: Like all species within the genus Crinivirus, ToCV is semipersistently transmitted by whiteflies, being one of only two criniviruses transmitted by members of the genera Bemisia and Trialeurodes. Disease symptoms: Tomato 'yellow leaf disorder' syndrome includes interveinal yellowing and thickening of leaves. Symptoms first develop on lower leaves and then advance towards the upper part of the plant. Bronzing and necrosis of the older leaves are accompanied by a decline in vigour and reduction in fruit yield. In other hosts the most common symptoms include interveinal chlorosis and mild yellowing on older leaves. Control: Control of the disease caused by ToCV is based on the use of healthy seedlings for transplanting, limiting accessibility of alternate host plants that can serve as virus reservoirs and the spraying of insecticides for vector control. Introduction: Particular fibroadipose infiltration patterns have been recently described by muscle imaging in congenital and later onset forms of LMNA-related muscular dystrophies (LMNA-RD).Methods: Scores for fibroadipose infiltration of 23 lower limb muscles in 34 patients with LMNA-RD were collected from heat maps of 2 previous studies. Scoring systems were homogenized. Relationships between muscle infiltration and disease duration and age of onset were modeled with random forests.Results: The pattern of infiltration differs according to disease duration but not to age of disease onset. The muscles whose progression best predicts disease duration were semitendinosus, biceps femoris long head, gluteus medius, and semimembranosus.Discussion: In LMNA-RD, our synthetic analysis of lower limb muscle infiltration did not find major differences between forms with different ages of onset but allowed the identification of muscles with characteristic infiltration during disease progression. The use of organic fertilizers from stock-raising activity is usually based on the nitrogen (N) criterion. The objective of our research is to evaluate whether this N criterion led to positive or environmentally risky changes, mainly in organic carbon (OC) storage and the availability of the main nutrients. Three biennial rotations of four crops were set up in a 6-yr time-frame. The treatments included a control (no N added); two mineral treatments, where 250 kg N ha-1 yr-1 was provided at two different times during the rotation; three cattle manure treatments, which provided 170, 250, and 500 kg N ha-1 yr-1; and four treatments in which the two lowest manure rates were complemented with mineral N (80 and 160 kg N ha-1 yr-1). Over the 6-yr period, the measured soil OC increases were equivalent to approximately 25, 43, and 35% of the manure OC applied, respectively, following the manure rate increases. Furthermore, equivalences were of -5, 23, and 25% when compared with full mineral fertilization. The positive slope of phosphorus availability was 1.2 mg Olsen-P kg-1 to 10 kg P ha-1 applied from manures (N/P = 4) when starting from a threshold of 15.9 mg Olsen-P kg-1. The availability of other nutrients (Mg, Zn, and Na) also increased with increasing manure rates. In a mediumterm experiment, the manure rate of 250 kg N ha-1 yr-1 optimized the nutrient recirculation. Objectives: To characterize the cardiovascular disease (CVD) profile of individuals aged 80 and older in rural South Africa. Design: First wave of population‐based longitudinal cohort. Setting: Agincourt subdistrict (Mpumalanga Province) in rural South Africa. Participants: Adults residents (N = 5,059). Measurements: In‐person interviews were conducted to obtain social, behavioral, economic, and clinical data. Prevalence of hypertension, diabetes, dyslipidemia, high waist‐to‐hip ratio, overweight and obesity, high‐risk high‐sensitivity C‐reactive protein, smoking, stroke, myocardial infarction, angina pectoris, and heart failure in individuals younger than 65, aged 65 to 79, and aged 80 and older were compared. Associations between self‐reported treatments and determinants of hypertension treatment in those aged 80 and older were assessed using multivariable regression. Results: Of 5,059 individuals included, 549 (10.8%) were aged 80 and older, and their CVD prevalence was 17.9% (stroke 3.8%, myocardial infarction 0.5%, angina pectoris 13.5%, heart failure 0.7%). Hypertension prevalence in this group was 73.8%, and along with angina pectoris, it increased with age (p<.001), whereas overweight and obesity (46.4%), dyslipidemia (39.1%), and smoking prevalences (3.1%) decreased (p<.001). Hypertension treatment was significantly associated with being aged 80 and older (odds ratio (OR)=1.48; 95% confidence interval (CI)=1.14–1.92, p=.003). Male sex (OR=0.73, 95% CI=0.66–0.88, p=.001), being an immigrant (OR=0.80, 95% CI=0.65–0.98; p=.03), higher socioeconomic status (OR=1.28, 95% CI=1.06–1.53, p=.009), and higher depression score (OR=1.12, 95% CI=1.05–1.19, p<.001) were associated with hypertension treatment in those aged 80 and older. Introduction: Adolescent girls and young women (AGYW) in South Africa bear a disproportionate burden of HIV. Community mobilization (CM), defined as community members taking collective action to achieve a common goal related to health, equity and rights, has been associated with increased HIV testing and condom use and has been called a 'critical enabler' for addressing the HIV epidemic. However, limited research has examined whether CM is associated with HIV incidence among AGYW. Methods: We examine the association of CM with incident HIV among AGYW (ages 13 to 21) enrolled in the HPTN 068 cohort in the Agincourt Health and socio‐Demographic Surveillance System, South Africa. This analysis includes 2292 participants residing in 26 villages where cross‐sectional, population‐based surveys were conducted to measure CM among 18‐ to 35‐year‐old residents in 2012 and 2014. HPTN 068 participants completed up to five annual visits that included an HIV test (2011 to 2016). Household‐level data were collected from AGYW parents/guardians and census data is updated annually. Mean village‐level CM scores were created using a validated community mobilization measure with seven components (social cohesion, social control, critical consciousness, shared concerns, organizations and networks, leadership and collective action). We used pooled generalized estimating equation regression with a Poisson distribution to estimate risk ratios (RR) for the association of village‐level CM score and CM components with incident HIV infection, accounting for village‐level clustering and adjusting for key covariates. Results: There were 194 incident infections over the follow‐up period. For every additional standard deviation of village‐level CM there was 12% lower HIV incidence (RR: 0.88, 95% CI: 0.79, 0.98) after adjusting for individual, household and community characteristics. CM components associated with lower HIV incidence included critical consciousness (RR: 0.88; CI: 0.79, 0.97) and leadership (RR: 0.87; CI: 0.79, 0.95); while not statistically significant, social cohesion (RR: 0.91; CI: 0.81, 1.01), shared concerns (RR: 0.90; CI: 0.81, 1.00), and organizations and networks (RR: 0.91; CI: 0.79, 1.03) may also play a protective role. Conclusions: These results suggest that having strong community social resources will reduce AGYW's risk of HIV acquisition. Work to mobilize communities, focusing on building social cohesion, shared concerns, critical consciousness, and effective and accountable leadership, can fortify prevention programming for AGYW. Abstract: Introduction: Community mobilization (CM) is increasingly recognized as critical to generating changes in social norms and behaviours needed to achieve reductions in HIV. We conducted a CM intervention to modify negative gender norms, particularly among men, in order to reduce associated HIV risk. Methods: Twenty two villages in the Agincourt Health and Socio‐Demographic Surveillance Site in rural Mpumalanga, South Africa were randomized to either a theory‐based, gender transformative, CM intervention or no intervention. Two cross‐sectional, population‐based surveys were conducted in 2012 (pre‐intervention, n = 600 women; n = 581 men) and 2014 (post‐intervention, n = 600 women; n = 575 men) among adults ages 18 to 35 years. We used an intent‐to‐treat (ITT) approach using survey regression cluster‐adjusted standard errors to determine the intervention effect by trial arm on gender norms, measured using the Gender Equitable Mens Scale (GEMS), and secondary behavioural outcomes. Results: Among men, there was a significant 2.7 point increase (Beta Coefficient 95% CI: 0.62, 4.78, p = 0.01) in GEMS between those in intervention compared to control communities. We did not observe a significant difference in GEMS scores for women by trial arm. Among men and women in intervention communities, we did not observe significant differences in perpetration of intimate partner violence (IPV), condom use at last sex or hazardous drinking compared to control communities. The number of sex partners in the past 12 months (AOR 0.29, 95% CI 0.11 to 0.77) were significantly lower in women in intervention communities compared to control communities and IPV victimization was lower among women in intervention communities, but the reduction was not statistically significant (AOR 0.53, 95% CI 0.24 to 1.16). Conclusion: Community mobilization can reduce negative gender norms among men and has the potential to create environments that are more supportive of preventing IPV and reducing HIV risk behaviour. Nevertheless, we did not observe that changes in attitudes towards gender norms resulted in desired changes in risk behaviours suggesting that more time may be necessary to change behaviour or that the intervention may need to address behaviours more directly. Background and purpose: Nemaline myopathy (NEM) has been associated with mutations in 12 genes to date. However, for some patients diagnosed with NEM, definitive mutations are not identified in the known genes, suggesting that there are other genes involved. This study describes compound heterozygosity for rare variants in ryanodine receptor type 3 (RYR3) gene in one such patient. Methods and results: Clinical examination of the patient at 22 years of age revealed a long narrow face, high arched palate and bilateral facial weakness. She had proximal weakness in all four limbs, mild scapular winging but no scoliosis. Muscle biopsy revealed wide variation in fibre size with type 1 fibre predominance and atrophy. Abundant nemaline bodies were located in perinuclear and subsarcolemmal areas, and within the cytoplasm. No likely pathogenic mutations in known NEM genes were identified. Copy number variation in known NEM genes was excluded by NEM‐targeted comparative genomic hybridization array. Next‐generation sequencing revealed compound heterozygous missense variants in the RYR3 gene. RYR3 transcripts are expressed in human fetal and adult skeletal muscle as well as in human brain and cauda equina samples. Immunofluorescence of human skeletal muscle revealed a ‘single‐row’ appearance of RYR3, interspersed between the ‘double rows’ of ryanodine receptor type 1 (RYR1) at each A–I junction. Conclusion: The results suggest that variants in RYR3 may cause a recessive muscle disease with pathological features including nemaline bodies. Objectives The innate alarm system (IAS) models the neurocircuitry involved in threat processing in posttraumatic stress disorder (PTSD). Here, we investigate a primary subcortical structure of the IAS model, the superior colliculus (SC), where the SC is thought to contribute to the mechanisms underlying threat-detection in PTSD. Critically, the functional connectivity between the SC and other nodes of the IAS remains unexplored. Experimental design We conducted a resting-state fMRI study to investigate the functional architecture of the IAS, focusing on connectivity of the SC in PTSD ( n = 67), its dissociative subtype ( n = 41), and healthy controls ( n = 50) using region-of-interest seed-based analysis. Principal observations We observed group-specific resting state functional connectivity between the SC for both PTSD and its dissociative subtype, indicative of dedicated IAS collicular pathways in each group of patients. When comparing PTSD to its dissociative subtype, we observed increased resting state functional connectivity between the left SC and the right dorsolateral prefrontal cortex (DLPFC) in PTSD. The DLPFC is involved in modulation of emotional processes associated with active defensive responses characterising PTSD. Moreover, when comparing PTSD to its dissociative subtype, increased resting state functional connectivity was observed between the right SC and the right temporoparietal junction in the dissociative subtype. The temporoparietal junction is involved in depersonalization responses associated with passive defensive responses typical of the dissociative subtype. Conclusions Our findings suggest that unique resting state functional connectivity of the SC parallels the unique symptom profile and defensive responses observed in PTSD and its dissociative subtype. The use of cattle manure ( CM) for fertilization presents challenges for optimizing nitrogen (N) use. Our work aimed to assess N efficiencies, in a 6-year experiment with three biennial rotations of four crops: oat-sorghum (first year) and ryegrass-maize (second year) in a rainfed humid Mediterranean area of Spain. Fertilization treatments included the following: control (no N), 250 kg mineral N ha−1 year−1 (250 MN), three CM rates (supplying 170, 250 and 500 kg N ha−1 year−1) and four treatments where the two lowest CM rates were complemented with either 80 or 160 kg mineral N ha−1 year−1. Treatments were distributed randomly in each of three blocks. Maximum dry-matter yield (~44-49 t ha−1 rotation−1) was achieved in the third rotation, and only the control and the 170 CM yielded significantly less. Within the limitations of the EU Nitrate Directive, the N steady state supply of 170 CM always requires a complement of mineral N (80 kg N ha−1) to maximize N agronomic efficiency. The maximum N-fertilizer replacement value (250 CM vs. 250 MN) was 0·67, without significant differences between the two treatments in other N-related efficiency indexes, which indicates that plants took advantage of residual-N effects. Nitrogen losses by leaching in the 250 CM treatment were around 5-7% of the N applied. This reinforces the sustainability of manure recycling in long cropping seasons. [ABSTRACT FROM AUTHOR]

Introduction: Prevalence of HIV among young women in South Africa remains extremely high. Adolescent peer groups have been found to be an important influence on a range of health behaviours. The characteristics of young women's friendships might influence their sexual health and HIV risk via connections to sexual partners, norms around sexual initiation and condom use, or provision of social support. We investigated associations between young women's friendships and their Herpes Simplex Virus Type 2 (HSV-2) and HIV infection status in rural South Africa. Methods: Our study is a cross-sectional, egocentric network analysis. In 2011 to 2012, we tested 13- to 20-year-old young women for HIV and HSV-2, and collected descriptions of five friendships for each. We generated summary measures describing friend socio-demographic characteristics and the number of friends perceived to have had sex. We used logistic regression to analyse associations between friend characteristics and participant HIV and HSV-2 infection, excluding likely perinatal HIV infections. Results: There were 2326 participants included in the study sample, among whom HIV and HSV-2 prevalence were 3.3% and 4.6% respectively. Adjusted for participant and friend socio-demographic characteristics, each additional friend at least one year older than the participant was associated with raised odds of HIV (odds ratio (OR) = 1.37, 95% CI 1.03 to 1.82) and HSV-2 (adjusted OR=1.41, 95% CI 1.18 to 1.69). Each additional friend perceived to have ever had sex also raised the odds of HIV (OR = 1.29, 95% CI 1.03 to 1.63) and HSV-2 (OR=1.18, 95% CI 1.03 to 1.35). Discussion: We found good evidence that a greater number of older friends and friends perceived to have had sex were associated with increased risk for HSV-2 and HIV infection among young women. Conclusions: The characteristics of young women's friendships could contribute to their risk of HIV infection. Here, we report the first use of massive-scale RNA-sequencing to explore seagrass response to CO2-driven ocean acidification ( OA). Large-scale gene expression changes in the seagrass Cymodocea nodosa occurred at CO2 levels projected by the end of the century. C. nodosa transcriptome was obtained using Illumina RNA-Seq technology and de novo assembly, and differential gene expression was explored in plants exposed to short-term high CO2/low pH conditions. At high pCO2, there was a significant increased expression of transcripts associated with photosynthesis, including light reaction functions and CO2 fixation, and also to respiratory pathways, specifically for enzymes involved in glycolysis, in the tricarboxylic acid cycle and in the energy metabolism of the mitochondrial electron transport. The upregulation of respiratory metabolism is probably supported by the increased availability of photosynthates and increased energy demand for biosynthesis and stress-related processes under elevated CO2 and low pH. The upregulation of several chaperones resembling heat stress-induced changes in gene expression highlighted the positive role these proteins play in tolerance to intracellular acid stress in seagrasses. OA further modifies C. nodosa secondary metabolism inducing the transcription of enzymes related to biosynthesis of carbon-based secondary compounds, in particular the synthesis of polyphenols and isoprenoid compounds that have a variety of biological functions including plant defence. By demonstrating which physiological processes are most sensitive to OA, this research provides a major advance in the understanding of seagrass metabolism in the context of altered seawater chemistry from global climate change. Introduction: In South Africa, older adults make up a growing proportion of people living with HIV. HIV programmes are likely to reach older South Africans in home-based interventions where testing is not always feasible. We evaluate the accuracy of self-reported HIV status, which may provide useful information for targeting interventions or offer an alternative to biomarker testing. Methods: Data were taken from the Health and Aging in Africa: A Longitudinal Study of an INDEPTH Community in South Africa (HAALSI) baseline survey, which was conducted in rural Mpumalanga province, South Africa. A total of 5059 participants aged ≥40 years were interviewed from 2014 to 2015. Self-reported HIV status and dried bloodspots for HIV biomarker testing were obtained during at-home interviews. We calculated sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) for self-reported status compared to "gold standard" biomarker results. Logbinomial regression explored associations between demographic characteristics, antiretroviral therapy (ART) status and sensitivity of self-report. Results: Most participants (93%) consented to biomarker testing. Of those with biomarker results, 50.9% reported knowing their HIV status and accurately reported it. PPV of self-report was 94.1% (95% confidence interval (CI): 92.0-96.0), NPV was 87.2% (95% CI: 86.2-88.2), sensitivity was 51.2% (95% CI: 48.2-54.3) and specificity was 99.0% (95% CI: 98.7-99.4). Participants on ART were more likely to report their HIV-positive status, and participants reporting false-negatives were more likely to have older HIV tests. Conclusions: The majority of participants were willing to share their HIV status. False-negative reports were largely explained by lack of testing, suggesting HIV stigma is retreating in this setting, and that expansion of HIV testing and retesting is still needed in this population. Introduction: Despite the rollout of antiretroviral therapy (ART), challenges remain in ensuring timely access to care and treatment for people living with HIV. As part of a multi-country study to investigate HIV mortality, we conducted health facility surveys within 10 health and demographic surveillance system sites across six countries in Eastern and Southern Africa to investigate clinic-level factors influencing (i) use of HIV testing services, (ii) use of HIV care and treatment and (iii) patient retention on ART. Methods: Health facilities (n = 156) were sampled within 10 surveillance sites: Nairobi and Kisumu (Kenya), Karonga (Malawi), Agincourt and uMkhanyakude (South Africa), Ifakara and Kisesa (Tanzania), Kyamulibwa and Rakai (Uganda) and Manicaland (Zimbabwe). Structured questionnaires were administered to in-charge staff members of HIV testing, prevention of mother-to-child transmission (PMTCT) and ART units within the facilities. Forty-one indicators influencing uptake and patient retention along the continuum of HIV care were compared across sites using descriptive statistics. Results: The number of facilities surveyed ranged from six in Malawi to 36 in Zimbabwe. Eighty percent were government-run; 73% were lower-level facilities and 17% were district/referral hospitals. Client load varied widely, from less than one up to 65 HIV testing clients per provider per week. Most facilities (>80%) delivered services or interventions that would support patient retention in care such as delivering free services, offering PMTCT within antenatal care, pre-ART monitoring and adherence counselling. Many facilities under-delivered in several areas, however, such as targeted testing for high-risk groups (21%) and mobile testing (36%). There were also intra-site and inter-site differences, including in the delivery of Option B+ (ranging from 6% in Kisumu to 93% in Kyamulibwa), and nurse-led ART initiation (ranging from 50% in Kisesa to 100% in Karonga and Agincourt). Only facilities in Malawi did not require additional lab tests for ART initiation. Stock-outs of HIV test kits and antiretroviral drugs were particularly common in Tanzania. Conclusions: We identified a high standard of health facility performance in delivering strategies that may support progression through the continuum of HIV care. HIV testing policy and practice was particularly weak. Objective: To compare the prevalence of the main comorbidities in 2 large cohorts of patients with primary Sjögren's syndrome (SS) and systemic lupus erythematosus (SLE), with a focus on cardiovascular (CV) diseases.Methods: This was a cross-sectional multicenter study where the prevalence of more relevant comorbidities in 2 cohorts was compared. Patients under followup from SJOGRENSER (Spanish Rheumatology Society Registry of Primary SS) and RELESSER (Spanish Rheumatology Society Registry of SLE), and who fulfilled the 2002 American-European Consensus Group and 1997 American College of Rheumatology classification criteria, respectively, were included. A binomial logistic regression analysis was carried out to explore potential differences, making general adjustments for age, sex, and disease duration and specific adjustments for each variable, including CV risk factors and treatments, when appropriate.Results: A total of 437 primary SS patients (95% female) and 2,926 SLE patients (89% female) were included. The mean age was 58.6 years (interquartile range [IQR] 50.0-69.9 years) for primary SS patients and 45.1 years (IQR 36.4-56.3 years) for SLE patients (P < 0.001), and disease duration was 10.4 years (IQR 6.0-16.7 years) and 13.0 years (IQR 7.45-19.76 years), respectively (P < 0.001). Smoking, dyslipidemia, and arterial hypertension were associated less frequently with primary SS (odds ratio [OR] 0.36 [95% confidence interval (95% CI) 0.28-0.48], 0.74 [95% CI 0.58-0.94], and 0.50 [95% CI 0.38-0.66], respectively) as were life-threatening CV events (i.e., stroke or myocardial infarction; OR 0.57 [95% CI 0.35-0.92]). Conversely, lymphoma was associated more frequently with primary SS (OR 4.41 [95% CI 1.35-14.43]). The prevalence of severe infection was lower in primary SS than in SLE (10.1% versus 16.9%; OR 0.54 [95% CI 0.39-0.76]; P < 0.001).Conclusion: Primary SS patients have a consistently less serious CV comorbidity burden and a lower prevalence of severe infection than those with SLE.
