Inequality represents an extreme environment to which humans must respond. One phenomenon that contributes to this growing extreme is precarity or the intersection of uncertainty and some form of inequality. While precarity has an important intellectual history in the fields of sociology and sociocultural anthropology, it has not been well studied in the field of human biology. Rather human biologists have engaged with the study of closely related concepts such as uncertainty and resource insecurity. In this article, we propose that human biology take on the study of precarity as a novel way of investigating inequality. We first provide a brief intellectual history of precarity which is followed by a review of research on uncertainty and resource security in human biology which, while not exhaustive, illustrates some key gaps that precarity may aid us in addressing. We then review some of the pathways through which precarity comes to affect human biology and health and some of the evidence for why the unpredictable nature of precarity may make it a unique physiological stress. A case study based on research in Nuñoa, Peru provides an important example of how precarity can elucidate the influences of health in an extreme setting, albeit with insights that apply more broadly. We conclude that precarity holds important potential for the study of human biology, including helping us more effectively operationalize and study uncertainty, encouraging us to explore the predictability of resources and stressors, and reminding us to think about the intersectional nature of stressors. Although the trajectories of children's social skills have been examined across biological sexes, less research has examined these patterns for racially and ethnically diverse children in the United States. The purpose of this study was to estimate the longitudinal growth trajectories of social skills, while examining the influence of biological sex, race, and ethnic identities using an adaptation of the commonly used Social Skills Rating Scale. A large nationally representative sample of elementary school children in the United States were assessed annually from kindergarten through fifth grade (N = 11,792). Structural equation modeling was used to examine latent growth curve models of teacher‐rated interpersonal skills and self‐control. After finding evidence of longitudinal invariance, results suggested a quadratic growth pattern best represented the trajectories of social skills ratings. Teacher ratings for African American children demonstrated a moderate declining linear trajectory throughout elementary school when compared with White children (reference group) and their Asian and Hispanic peers. Children from families with higher socioeconomic status and girls showed moderate to large differences in social skills ratings in kindergarten and stronger growth across time. Implications of the findings with future directions for research are discussed. Basal forebrain cholinergic neurons (BFCNs) represent the main source of cholinergic innervation to the cortex and hippocampus and degenerate early in Alzheimer's disease (AD) progression. Phenotypic maintenance of BFCNs depends on levels of mature nerve growth factor (mNGF) and mature brain‐derived neurotrophic factor (mBDNF), produced by target neurons and retrogradely transported to the cell body. Whether a reciprocal interaction where BFCN inputs impact neurotrophin availability and affect cortical neuronal markers remains unknown. To address our hypothesis, we immunolesioned the nucleus basalis (nb), a basal forebrain cholinergic nuclei projecting mainly to the cortex, by bilateral stereotaxic injection of 192‐IgG‐Saporin (the cytotoxin Saporin binds p75ntr receptors expressed exclusively by BFCNs) in 2.5‐month‐old Wistar rats. At 6 months post‐lesion, Saporin‐injected rats (SAP) showed an impairment in a modified version of the 5‐Choice Serial Reaction Time Task (5‐choice task). Postmortem analyses of the brain revealed a reduction of Choline Acetyltransferase‐immunoreactive neurons compared to wild‐type controls. A diminished number of cortical vesicular acetylcholine transporter‐immunoreactive boutons was accompanied by a reduction in BDNF mRNA, mBDNF protein levels, markers of glutamatergic (vGluT1), and GABAergic (GAD65) neurons in the SAP‐group compared to the controls. NGF mRNA, NGF precursor, and mNGF protein levels were not affected. Additionally, cholinergic markers correlated with the attentional deficit and BDNF levels. Our findings demonstrate that while cholinergic nb loss impairs cognition and reduces cortical neuron markers, it produces differential effects on neurotrophin availability, affecting BDNF but not NGF levels. Aims: The locus coeruleus (LC) is the main source of noradrenaline (NA) in the mammalian brain and has been found to degenerate during the initial stages of Alzheimer's disease (AD). Recent studies indicate that at late stages of the amyloid pathology, LC‐pathological alterations accelerate AD‐like pathology progression by interfering with the neuromodulatory and anti‐inflammatory properties of NA. However, the impact of LC degeneration at the earliest stages of amyloidosis on the AD‐like pathology is not well understood. Methods: The LC was lesioned in wild‐type and McGill‐R‐Thy1‐APP transgenic (APP tg) rats by administering N‐(2‐chloroethyl)‐N‐ethyl‐bromo‐benzylamine before amyloid plaque deposition. Cognitive deficits and AD‐like neuropathological changes were measured after the LC lesion. Results: Four months post‐treatment, rats displayed a decrease in brain noradrenergic innervation. The LC lesion in APP tg‐treated rats enhanced cognitive deficits and decreased hippocampal cholinergic innervation and neurotrophin expression. In addition, the APP tg‐treated rats displayed an increased microglial and astroglial cell number in close vicinity to hippocampal amyloid‐beta burdened neurons. The recruited microglia showed cellular alterations indicative of an intermediate activation state. Conclusions: Our results indicate that early LC demise aggravates the early neuroinflammatory process, cognitive impairments, cholinergic deficits and neurotrophin deregulation at the earliest stages of the human‐like brain amyloidosis. In this study, we report that LC demise at incipient stages of amyloid‐like pathology promoted cholinergic deficits, neurotrophin and cytokine deregulation, and cognitive deficits. These changes were accompanied by increased microglial and astroglial cells within the vicinity of CA1‐Aβ‐burdened neurons and an increase in microglial reactivity. Overall, our findings highlight pathological mechanisms arising from the disruption of NA signalling and loss of LC‐noradrenergic innervation at the earliest stages of the human‐like AD neuropathology. An extremely rare example of well‐preserved emergent Holocene fossil reefs exists at Kiritimati Island, central Pacific. Fossil reefs are rich geological archives of paleoenvironmental change. The first paleoecological surveys of two fossil reefs are presented, revealing high coral cover (40–50%) and low diversity (6 genera). Fossil coral ages suggest reefs exhibited disparity in the timing of reef development (4,113 and 1,915 cal yBP) and ecological surveys show different coral compositions (Acropora or Porites dominant), between reefs. Results constrain two discrete episodes of reef shutdown (at 2,905 and 1,705 cal yBP) as lagoonal reefs thrived, and subsequently died off, through the late Holocene. Shifts in physio‐chemical conditions associated with reduced lagoon flushing following storm‐driven changes in atoll rim morphology are argued as the driver for the staged reef die‐off. The findings have implications for interpreting past and future eco‐morphological change on atolls, given projected increases in storminess with climate change. Plain Language Summary: Fossil coral reefs include the skeletal remains of corals and are rarely preserved in good condition above modern sea level. Such reefs are rich archives that hold information about past environmental and ecological conditions at the time of reef development. A rare and expansive example of fossil reefs grew during the period 7,000 to 1,000 years ago at Kiritimati Island, Kiribati. Surveys and radiometric dating were used to reveal, for the first time, the age, elevation, and coral ecology of two of the most well‐preserved fossil reefs on Kiritimati Island (between 4,000 and 2,000 years ago). Results show that the reefs flourished during different time periods and were dominated by different coral taxa, before dying off. We argue that reef die‐off is attributed to changing lagoon seawater conditions that became intolerable for coral growth. These changing water conditions were the result of storm‐induced changes at the atoll rim that enclosed the lagoon and reduced flushing. As tropical cyclones are projected to increase in frequency and severity under global climate change, our results are critical for understanding the future responses of mid‐oceanic atolls and their supporting ecologies. Background: Candida spp. infective endocarditis (CIE) although rare is associated with high morbidity and mortality. Risk factors include prosthetic heart valves and injection drug use (IDU). We reviewed all cases of CIE at our institution to describe the microbiology, treatment and outcomes of patients focusing on IDU as a predisposing factor. Methods: Retrospective cohort of patients with definite CIE between 2013 and 2019 at a university hospital was analysed. Demographic data collected included IDU, microbiologic, treatment and mortality. The primary outcome of interest was 12‐month, all‐cause mortality. Results: Twenty patients were included (one had two separate episodes of CIE); CIE accounted for 4% of total infective endocarditis (IE) cases during the study period. The median (IQR) age was 38 (30‐58) years, 10 (50%) had a previous history of IE, and 4 (20%) patients had prosthetic heart valves or an implanted cardiac device. Thirteen (65%) patients were IDU. The tricuspid valve was the primary valve involved (8/18, 44%), and C albicans was the most frequently isolated organism (8, 36%). Echinocandin was the most common treatment strategy (8, 40%). Only three (15%) patients underwent valve replacement during hospitalisation. There were no in‐hospital fatalities, and 5 (25%) patients died at one year; all were IDU (39% to 0%, p =.11). Conclusion: CIE is a rare infectious disease seen more commonly in the IDU population. Cardiac surgery was rarely performed, and long‐term mortality was 25%. PREMISE: Plant genome size ranges widely, providing many opportunities to examine how genome size variation affects plant form and function. We analyzed trends in chromosome number, genome size, and leaf traits for the woody angiosperm clade Viburnum to examine the evolutionary associations, functional implications, and possible drivers of genome size. METHODS: Chromosome counts and genome size estimates were mapped onto a Viburnum phylogeny to infer the location and frequency of polyploidization events and trends in genome size evolution. Genome size was analyzed with leaf anatomical and physiological data to evaluate the influence of genome size on plant function. RESULTS: We discovered nine independent polyploidization events, two reductions in base chromosome number, and substantial variation in genome size with a slight trend toward genome size reduction in polyploids. We did not find strong relationships between genome size and the functional and morphological traits that have been highlighted at broader phylogenetic scales. CONCLUSIONS: Polyploidization events were sometimes associated with rapid radiations, demonstrating that polyploid lineages can be highly successful. Relationships between genome size and plant physiological function observed at broad phylogenetic scales may be largely irrelevant to the evolutionary dynamics of genome size at smaller scales. Objective: Relatively little is known about sources of variation in human breastmilk composition. Previous research posits that composition may be affected by the low oxygen environment found at high altitude (HA). Our objective is to compare the milk composition of a sample of HA Andeans to previously published data on HA Tibetans and low altitude (LA) Peruvians. Methods: Mid‐feed milk samples were collected from 22 women and tested for lipid, carbohydrate and protein content in addition to dietary surveys and anthropometric measures. We examined relationships between maternal and infant characteristics (maternal BMI, adiposity, age, skinfold thickness, infant age) and milk composition using multiple regression analyses. A Welch‐Satterthause t‐test was used to compare summary measures from Andean sample and Tibetan samples. Results: Milk fat, sugar, and true protein content were correlated with different maternal and infant characteristics, with only waist to hip ratio negatively affecting fat and positively affecting sugar content in full models. Nuñoan and LA Peruvian milk exhibited significantly lower lactose levels (6.01 ±.89 g/100 mL and 6.38 ± 0.03 g/100 mL) than Tibetans (7.25 ± 0.35 g/100 mL). Fat content among Nuñoans was similar to that of Tibetans (4.5 ± 0.78 g/100 mL and 4.6 ± 1.51 g/100 mL, respectively), and higher than those of LA Peruvians. Conclusions: Both HA populations demonstrate relatively high fat content and both Peruvian samples exhibited relatively higher protein and lower lactose than Tibetan samples, suggesting that both population‐based differences in feeding behavior and climatic differences may influence composition. Organ transplantation saves and transforms lives. Failure to secure consent for organ retrieval is widely regarded as the single most important obstacle to transplantation. A soft opt-out system of consent for deceased organ donation was introduced into Wales in December 2015, whilst England maintained the existing opt-in system. Cumulative data on consent rates in Wales were compared with those in England, using a two-sided sequential procedure that was powered to detect an absolute difference in consent rates between England and Wales of 10%. Supplementary risk-adjusted logistic regression analysis examined whether any difference in consent rates between the two nations could be attributed to variations in factors known to influence UK consent rates. Between 1 January 2016 and 31 December 2018, 8192 families of eligible donors in England and 474 in Wales were approached regarding organ donation, with overall consent rates of 65% and 68%, respectively. There was a steady upward trend in the proportion of families consenting to donation after brain death in Wales as compared with England and after 33 months, this reached statistical significance. No evidence of any change in the donation after circulatory death consent rate was observed. Risk-adjusted logistic regression analysis revealed that by the end of the study period the probability of consent to organ donation in Wales was higher than in England (OR [95%CI] 2.1 [1.26-3.41]). Individuals with fibromyalgia are at greater risk for depressive symptoms than the general population, and this may be partially attributable to physical symptoms that impair day‐to‐day functioning. However, individual‐level protective characteristics may buffer risk for psychopathology. For instance, the ability to perceive a "silver lining" in one's illness may be related to better mental and physical health. We examined perceived silver lining as a potential moderator of the relation between fibromyalgia impact and depressive symptoms. Our sample of persons with fibromyalgia (N = 401) completed self‐report measures including the Fibromyalgia Impact Questionnaire‐Revised, Depression Anxiety Stress Scales, and the Silver Lining Questionnaire. Moderation analyses covaried age, sex, and ethnicity. Supporting hypotheses, increasing impact of disease was related to greater depressive symptoms, and perceptions of a silver lining attenuated that association. Despite the linkage between impairment and depressive symptoms, identifying positive aspects or outcomes of illness may reduce risk for psychopathology. Therapeutically promoting perception of a silver lining, perhaps via signature strengths exercises or a blessings journal, and encouraging cognitive reframing of the illness experience, perhaps via Motivational Interviewing or Cognitive Behavioral Therapy, may reduce depressive symptoms in persons with fibromyalgia. Biocultural anthropology has long represented an important approach in the study of human biology. However, despite demonstrated utility, its somewhat amorphous identity leaves some scholars questioning just what it means to be biocultural. In this article, rather than providing proscriptive doctrine, we contribute to these conversations about the nature of biocultural anthropology by considering what biocultural research does. We begin with a consideration of some of the foundational themes of biocultural work including recognition of the dialectical nature of sociocultural and biological forces, interest in inequality, and incorporation of both evolutionary and political economic perspectives. To emphasize the consistency of biocultural work over time, we also trace these themes from originating work to their appearance in current research. We then identify some of the key actions of the biocultural approach, noting that biocultural work can execute any number though rarely all of these actions simultaneously. We then offer brief introductions to the articles that make up this special issue, highlighting the ways in which each piece undertakes key biocultural actions. Following these introductions, we provide a discussion of some of the types of biocultural work that are not present in this special issue, recognizing the breadth of biocultural research across multiple subfields of anthropology. Finally, we point to some potentially fruitful directions for future biocultural research. In the end, we conclude that while biocultural anthropology may not have a cohesive or set agenda, it does have a clear and recognizable form of content and methodology illuminated by its actions. Objectives: Chronic malnutrition remains a persistent global health issue. This mixed methods investigation in rural Peru examines the role of home food production (HFP) in reducing child malnutrition by testing the following hypotheses: (a) higher HFP will be associated with higher infant z‐scores than those with less HFP and (b) infants with higher HFP will exhibit improved longitudinal growth outcomes across 6 months. Methods: Ethnographic methods include semi‐structured interviews and participant observation. A household survey and anthropometric data were collected twice, from 86 infants under of 24 months old. A HFP index (HFI) was generated based on reports of animals for meat consumption, eggs, milk, and agricultural products for home consumption. Kruskal‐Wallis tests were used to examine patterns of HFI and infant anthropometrics. Multivariate regressions were used to examine the relationships between HFP and infant height for age (HAZ), weight for age (WAZ), and triceps skinfold for age (TSAZ) in both rounds and 6‐month change between rounds (6MΔ). Results: There were no relationships between HFI and infant z‐scores in round one, however, HFI was significantly positively related to HAZ (B = 0.091, P <.039) in round two and with 6MΔ in HAZ (B = 0.09, P <.047). HFI did not predict WAZ or TSAZ in either round. Conclusions: HFP represents an important influence of infant growth in Nuñoa, likely through nutritional improvement due to increased availability of animal‐sourced foods and through contribution to household economy and maternal empowerment. The objective of this translational study was to evaluate the FDA‐approved PediMag, CentriMag, and RotaFlow centrifugal blood pumps in terms of hemodynamic performance using simulated neonatal and pediatric extracorporeal membrane oxygenation (ECMO) circuits with different sizes of arterial and venous cannulae. Cost of disposable pump heads was another important variable for this particular study. The experimental circuit was composed of one of the centrifugal pump heads, a polymethylpentene membrane oxygenator, neonatal and pediatric arterial/venous cannulae, and 1/4‐inch ID tubing. Circuits were primed with lactated Ringer's solution and packed human red blood cells (hematocrit 35%). Trials were conducted at 36°C using the three pump heads and different cannulae (arterial/venous cannulae: 8 Fr/18 Fr, 10 Fr/20 Fr, and 12 Fr/22 Fr) at various flow rates (200–2400 mL/min, 200 mL/min increments) and rotational speeds. Pseudo patient pressure was 60 mm Hg. Real‐time pressure and flow data were recorded for analysis. The RotaFlow pump had a higher pressure head and flow range compared with the PediMag and CentriMag pumps at the same rotational speed and identical experimental settings (P < 0.001). The PediMag pump had lower flow output than others (P < 0.001). Small‐caliber arterial cannulae and higher flow rates predictably created higher circuit pressures and pressure drops. There was no significant difference in hemodynamic energy delivered to the pseudo patient with each of the three pumps. The arterial cannula had the highest pressure drop and hemodynamic energy loss in the circuit when compared to the oxygenator and arterial tubing. The RotaFlow centrifugal pump had a significantly better hemodynamic performance when compared to the PediMag and CentriMag blood pumps at identical experimental conditions in simulated neonatal and pediatric ECMO settings. In addition, the cost of the RotaFlow pump head ($400) is 20 to 30‐fold less than the other centrifugal pumps [CentriMag ($12 000) or PediMag ($8000)] that were evaluated in this translational study. The debate over the importance of pulsatile flow has been discussed since Hippocrates and Aristotle; however, in 1955, Wesolowski performed the initial laboratory studies in pulsatile versus nonpulsatile perfusion during cardiopulmonary bypass (CPB) procedures.[1] Although early CPB pulsatile pumps introduced greater complexity and only marginal benefits in the 1950s and 1960s, a few pulsatile roller pumps showed advanced commercial production and clinical use in the late 1970s and early 1980s after demonstrating lower mortality rates and a decreased need for postoperative intra-aortic balloon pulsation.[[2]] Despite this, the controversy over the benefits of pulsatile flow during acute and long-term extracorporeal circulation still continues.[4] Our pediatric cardiovascular research team at Penn State Children's Hospital and Penn State College of Medicine has a long-standing dedication to this controversial debate and uses a multidisciplinary translational research approach to further examine this issue.[[5]] The objective of this editorial is to clarify common myths related to the effects of pulsatile flow on circuit optimization and vital organ recovery in congenital heart surgery patients undergoing CPB using data generated at the Penn State Pediatric Cardiovascular Research Center. Myth: Pulsatile flow eliminates the adverse effects of CPB procedures on vital organ injury and reduces the length of ICU as well as hospital stay for congenital heart surgery patients. Objectives: Socioeconomic status (SES) is a powerful determinant of health, but the underlying biological mechanisms are poorly understood. This study investigates whether levels of DNA methylation at CpG sites across the genome are associated with SES in a cohort of young adults in the Philippines. Methods: DNA methylation was assayed with the Illumina HumanMethylation450 Bead Chip, in leukocytes from 489 participants in the Cebu Longitudinal Health and Nutrition Survey (mean age = 20.9 years). SES was measured in infancy/childhood and adulthood, and was based on composite measures of income, assets, and education. Genome‐wide analysis of variable probes identified CpG sites significantly associated with SES after adjustment for multiple comparisons. Functional enrichment analysis was used to identify biological pathways associated with these sites. Results: A total of 2,546 CpG sites, across 1,537 annotated genes, were differentially methylated in association with SES. In comparison with high SES, low SES was associated with increased methylation at 1,777 sites, and decreased methylation at 769 sites. Functional enrichment analysis identified over‐representation of biological pathways related to immune function, skeletal development, and development of the nervous system. Conclusions: Socioeconomic status predicts DNA methylation at a large number of CpG sites across the genome. The scope of these associations is commensurate with the wide range of biological systems and health outcomes that are shaped by SES, and these findings suggest that DNA methylation may play an important role. Summary: Plant nucleotide‐binding leucine‐rich repeat (NLR) disease resistance proteins recognize specific pathogen effectors and activate a cellular defense program. In Arabidopsis thaliana (Arabidopsis), Resistance to Ralstonia solanacearum 1 (RRS1‐R) and Resistance to Pseudomonas syringae 4 (RPS4) function together to recognize the unrelated bacterial effectors PopP2 and AvrRps4. In the plant cell nucleus, the RRS1‐R/RPS4 complex binds to and signals the presence of AvrRps4 or PopP2.The exact mechanism underlying NLR signaling and immunity activation remains to be elucidated. Using genetic and biochemical approaches, we characterized the intragenic suppressors of sensitive to low humidity 1 (slh1), a temperature‐sensitive autoimmune allele of RRS1‐R.Our analyses identified five amino acid residues that contribute to RRS1‐RSLH1 autoactivity. We investigated the role of these residues in the RRS1‐R allele by genetic complementation, and found that C15 in the Toll/interleukin‐1 receptor (TIR) domain and L816 in the LRR domain were also important for effector recognition. Further characterization of the intragenic suppressive mutations located in the RRS1‐R TIR domain revealed differing requirements for RRS1‐R/RPS4‐dependent autoimmunity and effector‐triggered immunity.Our results provide novel information about the mechanisms which, in turn, hold an NLR protein complex inactive and allow adequate activation in the presence of pathogens. Background: Dietetics practice educators are instrumental in the development of future dietitians. The present study aimed to explore dietetics practice educators' experiences of, and challenges faced in, dietetics workforce preparation. Methods: This study was guided by interpretive description methodology. Purposive and maximum variation sampling were used to recruit 18 dietetics practice educators from a variety of practice areas and locations across Australia. In‐depth, semi‐structured interviews were digitally recorded then transcribed verbatim and inductive coding of data was managed with nvivo (QSR International Pty Ltd, Doncaster, VIC, Australia). Multiple researchers analysed the data to develop preliminary themes using template analysis before final themes were identified. Results: Three main themes were developed: nurturing others; seeing the flaws; and soldiering on. Practice educators take great satisfaction in witnessing students develop and are focused on cultivating future practitioners with authentic learning activities. However, they are impacted by perceived shortcomings of the systems in which they operate and consider that broadening the scope of dietetics placements to better align with contemporary practice could benefit graduates and the profession. Despite these challenges, practice educators are pragmatic in getting on with their roles and recognise the advantages gained from student placements. Conclusions: Despite facing numerous challenges, practice educators derive benefits from their role in workforce preparation. Evidence is required on how students can demonstrate competence in contemporary areas of practice and on how to enable the scope of student placements to be broadened. Such strategies could support practice educators to overcome challenges and help ensure the dietetics profession of tomorrow is relevant and responsive. [ABSTRACT FROM AUTHOR]

Background: Dietetics students are a widely researched group. As emerging dietitians, they can provide valuable insights to inform how dietetics education programmes may be enhanced to meet contemporary healthcare needs. This review aimed to systematically synthesise dietetics students' experiences of dietetics workforce preparation. Methods: MEDLINE, CINAHL, Embase, PsycINFO, ERIC, Informit and ProQuest Dissertations and Theses Global were searched to identify research published until June 2017. Studies investigating dietetics students' experiences of dietetics workforce preparation, and employing qualitative data collection and analysis methods were included. Data analysis was guided by thematic synthesis, where themes were constructed through an iterative and inductive process. Study quality was appraised using the RATS Qualitative Research Review Guidelines. Results: From the 3301 records identified, five studies met the inclusion criteria and the views of 120 dietetics students from two countries over a 9‐year period were synthesised. The overarching theme of 'navigating through the ups and downs' was underpinned by four main themes: enduring hurdles; reconciling expectations; transforming self; and making and breaking connections. Quality appraisal results rated selection bias as being inadequate/inappropriate across all studies. Conclusions: Dietetics students undertake a transformational journey through dietetics education. They are inspired by seeing what is possible through meaningful encounters with practitioners in diverse settings. However, they are challenged by competitive environments and perceived ideals that are embedded in the profession. Strategies that focus on exposing dietetics students to inspirational practitioners, increasing and celebrating diversity in academic/placement settings, and incentivising collaboration across dietetics education, could act as catalysts to enhance the experience of future dietetics students and the nutrition‐related health of those they will serve. The experimental circuit consisted of an i‐cor diagonal pump, a Medos Hilite 800 LT oxygenator, an 8Fr Biomedicus arterial cannula, a 10Fr Biomedicus venous cannula, and six feet of 1/4 in ID tubing for arterial and venous lines. The circuit was primed with lactated Ringer's solution and packed red blood cells (hematocrit 40%). Trials were conducted at various heart rates (90, 120, and 150 bpm) and flow rates (200, 400, and 600mL/min) under nonpulsatile and pulsatile mode with pulsatile amplitudes of 1000–4000rpm (1000 rpm increments). Real‐time pressure and flow data were recorded for analysis. The i‐cor pump was capable of creating nonpulsatile and electrocardiography (ECG)‐synchronized pulsatile flow, and automatically reducing pulsatile frequency by increasing the assist ratio at higher heart rates. Reduced pulsatile frequency led to lower hemodynamic energy generation but did not affect circuit pressure drop. Pulsatile flow delivered more hemodynamic energy to the pseudopatient when compared with nonpulsatile flow. The pump generated more hemodynamic energy with higher pulsatile amplitudes. The i‐cor pump can automatically adjust the pulsatile assist ratio to create pulsatile flow at higher heart rates, although this caused some hemodynamic energy loss. Compared with nonpulsatile flow, pulsatile flow generated and transferred more hemodynamic energy to the neonate during ECLS (200–600mL/min), especially at high pulsatile amplitudes and low flow rates. Objectives: Nuñoa, Peru has been the site of anthropological research on human adaptability, growth, and health since the 1960s. Previous studies documented a nascent secular trend in growth between 1964 and 1984. However, no secular trend was evident when the population was re‐examined in 1999, following an intervening period of sociopolitical upheaval. Subsequently, the region experienced considerable economic development reflected in secular increases in height. We seek to evaluate trends in growth and BMI among Nuñoan children over 50 years. Materials and methods: Anthropometric data for 929 children collected in 2015 are compared with anthropometric data for 394 children from 1964, 912 children from 1984, and 292 children from 1999. The data were converted to z‐scores using the WHO 2007 growth standards. anova is used to evaluate rates of stunting, underweight, overweight, and obesity across the study periods. Results: We observe small increases in stature and weight between the 1960s and 1980s, almost no change between the 1980s and 1990s and statistically significant reductions in stunting and increases in height between the 1990s and 2015. Discussion: There were relatively small differences in stature and weight and a high incidence of stunting between 1964 and 1999 attributed to extremely poor economic and health conditions. A large secular increase in height (7–8 cm on average) and consequent reduction in rates of stunting between 1999 and 2015 occurred due to improved social and economic conditions. The objective of this study is to compare hemodynamic performances under different pulsatile control algorithms between Medos DeltaStream DP3 and i‐cor diagonal pumps in simulated pediatric and adult ECLS systems. An additional pilot study was designed to test hemolysis using two pumps during 12h‐ECLS. The experimental circuit consisted of parallel combined pediatric and adult ECLS circuits using an i‐cor pump head and either an i‐cor console or Medos DeltaStream MDC console, a Medos Hilite 2400 LT oxygenator for the pediatric ECLS circuit, and a Medos Hilite 7000 LT oxygenator for the adult ECLS circuit. The circuit was primed with lactated Ringer's solution and human packed red blood cells (hematocrit 40%). Trials were conducted at various flow rates (pediatric circuit: 0.5 and 1L/min; adult circuit: 2 and 4L/min) under nonpulsatile and pulsatile modes (pulsatile amplitude: 1000–5000rpm [1000 rpm increments] for i‐cor pump, 500–2500rpm [500 rpm increments] for Medos pump) at 36°C. In an additional protocol, fresh whole blood was used to test hemolysis under nonpulsatile and pulsatile modes using the two pump systems in adult ECLS circuits. Under pulsatile mode, energy equivalent pressures (EEP) were always greater than mean pressures for the two systems. Total hemodynamic energy (THE) and surplus hemodynamic energy (SHE) levels delivered to the patient increased with increasing pulsatile amplitude and decreased with increasing flow rate. The i‐cor pump outperformed at low flow rates, but the Medos pump performed superiorly at high flow rates. There was no significant difference between two pumps in percentage of THE loss. The plasma free hemoglobin level was always higher in the Medos DP3 pulsatile group at 4 L/min compared to others. Pulsatile control algorithms of Medos and i‐cor consoles had great effects on pulsatility. Although high pulsatile amplitudes delivered higher levels of hemodynamic energy to the patient, the high rotational speeds increased the risk of hemolysis. Use of proper pulsatile amplitude settings and intermittent pulsatile mode are suggested to achieve better pulsatility and decrease the risk of hemolysis. Background: Acetylcholinesterase inhibitors represent four out of six drugs for treating Alzheimer's disease (AD), with time‐limited efficacy likely due to the progressive loss of cholinergic neurons. The selective allosteric M1 muscarinic and sigma‐1 receptor agonist Anavex 3‐71 (aka AF710B) [1] takes advantage of the fact that acetylcholine postsynaptic M1 muscarinic brain receptor levels remain unchanged in AD. Previously, Anavex 3‐71 treatment attenuated AD hallmarks in McGill‐R‐Thy1‐APP transgenic rats when administered at advanced stages of the AD‐like pathology [2]. However, preventive strategies instead of late treatment should be more effective in AD. Therefore, we tested whether Anavex 3‐71 administration during early amyloid pathology stages could prevent cognitive impairment, plaque deposition and neuroinflammation. Method: Pre‐plaque, seven‐month‐old McGill‐R‐Thy1‐APP rats were subjected to daily oral administration of Anavex 3‐71 (10 µg/kg) for seven months. After one month of drug interruption (wash‐out), we performed Novel Object Recognition (NOR), Morris Water Maze (MWM) and Social Preference (SP) behavioural tests. Subsequently, brains were extracted, fixed and analyzed by histochemistry and immunohistochemistry. Result: McGill‐APP rats had deficits in discriminating the novel object in the NOR and locating the hidden platform in the MWM compared to wild‐type littermates. Anavex 3‐71 restored the abilities of McGill‐APP rats in the NOR task to wild‐type levels and partially rescued the deficits in the MWM. In the SP task, Anavex 3‐71‐treated McGill‐APP rats exhibited diminished deficits in SP interaction time with a "stranger rat". Treatment significantly reduced the number of mature Thioflavin‐S positive plaques in the subiculum compared to vehicle‐treated McGill‐APP rats and diminished the number of diffuse plaques in the cortex and hippocampus. Furthermore, Anavex 3‐71 treatment significantly reduced microglia and astrocyte recruitment towards CA1 hippocampal Aβ‐burdened neurons compared to untreated McGill‐APP rats. Conclusion: The long‐lasting effect of Anavex 3‐71 in preventing cognitive decline of McGill‐R‐Thy1‐APP rats, even after a wash‐out period would suggest some preventative, disease‐modifying, properties of the compound over the AD‐like amyloid pathology. Aim: Late‐onset Alzheimer's disease (LOAD) accounts for 95% of all Alzheimer's cases and is genetically complex in nature. Overlapping clinical and neuropathological features between AD, FTD and Parkinson's disease highlight the potential role of genetic pleiotropy across diseases. Recent genome‐wide association studies (GWASs) have uncovered 20 new loci for AD risk; however, these exhibit small effect sizes. Using NGS, here we perform association analyses using exome‐wide and candidate‐gene‐driven approaches. Methods: Whole‐exome sequencing was performed on 132 AD cases and 53 control samples. Exome‐wide single‐variant association and gene burden tests were performed for 76 640 nonsingleton variants. Samples were also screened for known causative mutations in familial genes in AD and other dementias. Single‐variant association and burden analysis was also carried out on variants in known AD and other neurological dementia genes. Results: Tentative single‐variant and burden associations were seen in several genes with kinase and protease activity. Exome‐wide burden analysis also revealed significant burden of variants in PILRA (P = 3.4 × 10−5), which has previously been linked to AD via GWAS, hit ZCWPW1. Screening for causative mutations in familial AD and other dementia genes revealed no pathogenic variants. Variants identified in ABCA7, SLC24A4, CD33 and LRRK2 were nominally associated with disease (P < 0.05) but did not withstand correction for multiple testing. APOE (P = 0.02) and CLU (P = 0.04) variants showed significant burden on AD. Conclusions: In addition, polygenic risk scores (PRS) were able to distinguish between cases and controls with 83.8% accuracy using 3268 variants, sex, age at death and APOE ε4 and ε2 status as predictors. Abstract: Objective: To validate a method for measuring endotoxin‐core antibodies (EndoCAb) from dried blood spots (DBS)—drops of capillary whole blood collected and dried on filter paper—as an indicator of environmental enteropathy (EE) in infancy and early childhood. Methods: A commercially available enzyme immunoassay kit was adapted for use with DBS, with optimized steps for sample elution. Technical validation included analysis of linearity/recovery, precision and reliability, lower limit of detection, and correspondence between matched plasma and DBS samples. Validation in a field‐based setting was implemented with samples from Peruvian infants (n = 82; age = 2–33 months) collected at two time points six months apart. Results: A high correspondence between plasma and DBS levels of EndoCAb was observed (R2 = 0.93, P < .001). The lower limit of detection was found to be 0.01 GMU/mL. Interassay coefficient of variation (CV) was 10.9% and 8.06% for low and high controls, respectively. Mean intra‐assay CVs were 3.22% and 1.83%, respectively. In a sample of Peruvian infants, EndoCAb levels increased with age as expected (P < .001). Age explained nearly 34.6% of the variance in EndoCAb across the sample. Objectives Changing economic activities bring significant cultural, epidemiological, and nutritional transitions. Infancy represents a critical period when rapid growth and metabolic programming occur, making infants particularly vulnerable to long-lasting biological changes due to such transitions. This paper examines the relationship between residence in distinct economic production zones (urban, herding, and dairy) and early growth in a highland district of Peru. Methods The length, weight, body mass index, and triceps skinfold of 93 infants (55 boys and 38 girls, aged 2 to 24 months) from 3 economic zones were measured at two time points 6 months apart. Z-scores were calculated based on the WHO Growth Reference for children under 5 years of age. Comparisons were made to the reference group and between economic zones to examine the impacts of different economic activities on infant growth. Results Female infants had significantly higher height-for-age z-scores (HAZ) than males ( P < 0.05). Across zones, dairying infants had the highest HAZ scores and the lowest prevalence of stunting. There was a significant association between zone and HAZ at baseline. While these associations were attenuated in measurements, taken 6 months later, longitudinal growth monitoring revealed differential patterns of weight gain and adiposity deposition across the zones with herding infants showing consistent high adiposity and urban infants gaining both weight and adiposity between rounds. Discussion Changes in economic activity are bringing about nutritional and epidemiological transitions in the Nuñoa district. Several dedicated commercial lab-based micro-computed tomography (μCT) systems exist, which provide high-resolution images of samples, with the capability to also deliver in-line phase contrast. X-ray phase contrast is particularly beneficial when visualizing very small features and weakly absorbing samples. The raw measured projections will include both phase and absorption effects. Extending our previous work that addressed the optimization of experimental conditions at the commercial ZEISS Xradia 500 Versa system, single-distance phase-contrast imaging is demonstrated on complex biological and material samples. From data captured at this system, we demonstrate extraction of the phase signal or the correction of the mixed image for the phase shift, and show how this procedure increases the contrast and removes artefacts. These high-quality images, measured without the use of a synchrotron X-ray source, demonstrate that highly sensitive, micrometre-resolution imaging of 3D volumes is widely accessible using commercially advanced laboratory devices. Phosphorus is among the essential nutrients applied to sugarcane (Saccharum officinarum L.) fields in the form of a fertilizer mixture (N, P, and K) in southwestern Florida. Sorption coefficient is used for modeling P movement, and in this study, we hypothesized that the sorption coefficient determined using fertilizer mixture (N, P, and K) will be significantly different from values determined using KCl and CaCl2, the electrolytes most commonly used for conducting sorption experiments. Supporting electrolytes, 0.01 mol L-1 KCl, 0.005 mol L-1 CaCl2, deionized (DI) water, simulated Florida rain, and fertilizer mixture prepared in Florida rain were used to characterize P sorption. Immokalee (Sandy, siliceous, hyperthermic Arenic Alaquods) and Margate (Sandy, siliceous hyperthermic Mollic Psammaquents) are the dominant mineral soils used for sugarcane production in southwestern Florida; we used the A and Bw horizons of Margate soil and the A and Bh horizons of the Immokalee soil for sorption experiments in this study. Freundlich sorption isotherms described P sorption data. The Freundlich sorption isotherm coefficients followed the trend 0.005 mol L-1 CaCl2 > 0.01 mol L-1 KCl ≈ fertilizer mixture > simulated Florida rain ≈ DI water. Sorption coefficients were used for modeling P movement with HYDRUS 1D; similar P results were obtained with the 0.01 mol L-1 KCl and fertilizer mixture electrolyte treatments. The sorption coefficient for DI water and simulated Florida rain overpredicted P movement. Reasons for performing study Polo, one of the world's oldest sports, is unique in merging human skill and balance with animal agility and performance in a contact sport. These modern-day 'centaurs' offer medical, dental and veterinary scientists an unrivalled, if quirky, opportunity to collaborate. Collection of epidemiological data on injuries to UK polo riders and ponies is the first step. Objectives To measure the reported risk and risk factors for injuries to UK polo ponies, their perception and mitigation by player-owners. Study design A retrospective cohort design and telephone interviews were used. Methods Data on equine injuries, preseason training and risk perception were collected from a random sample of player-owners using a structured questionnaire. Injuries were defined as requiring veterinary treatment. Frequencies were represented as percentages and 95% confidence intervals ( CIs). Risk factors for injuries were identified by univariable and multivariable analyses. Results The cumulative incidence of player-owner-reported injury was 10.6% (95% CI 8.4-12.7). Tendon injuries were most common (4.3%; 95% CI 2.9-5.7), followed by wounds and splints. The only risk factor was stabling all season (odds ratio 4.79; 95% CI 1.46-15.73). Tendon injuries were perceived as the major risk and hard ground the most important risk factor. Risk mitigation practices were bandaging before exercise (45.7%; 95% CI 34.8-56.5), checking tendons (84.0%; 95% CI 76.0-91.9), cold hosing (40.7%; 95% CI 30.0-51.4), bandaging (38.3%; 95% CI 27.7-48.9) and using clays and coolants after exercise (24.7%; 15.3-34.1). Cuts and wounds were considered most frequent by only 2.5% (95% CI 0.0-3.6) of players but were the second most common injury, accounting for 21.6% of veterinary treatments. Splints accounted for 12.5% of injuries. Conclusions The risk of injury to polo ponies is similar to that in the general horse population; musculoskeletal injuries, particularly tendon injuries, are most common, followed by wounds and splints. Proponents of global market expansion claim that economic growth brings prosperity and creates a more equitable global society, yet these efforts may result in the exacerbation of economic disparities that translates into significant disparities in health. These trends underscore the importance of research examining the social determinants of health in an increasingly unequal world. This article brings together concepts from biocultural anthropology, developmental origins of health and disease, and research examining the long-term effects of early environments on social capital to formulate the notion of biosocial inheritance. Biosocial inheritance is the process whereby social adversity in one generation is transmitted to the next through reinforcing biological and social mechanisms that impair health, exacerbating social and health disparities. Such a theoretical framework considers contemporary and historic political-economic forces that shape inequalities in human health, across generations. We examine the biosocial determinants of growth in the Andes as a case study of biosocial inheritance, developing a model that will illuminate the contemporary and historic sources of persistent poverty in low-income communities around the world. By highlighting the biosocial mechanisms underpinning the intergenerational transmission of poverty, this model allows for the formulation of interventions aimed at breaking the cycle of poverty and a reconsideration of the way we think about poverty and social mobility. For two decades the search for genes involved in Alzheimer's disease brought little reward; it was not until the advent of genome-wide association studies ( GWAS) that genetic associations started to be revealed. Since 2009 increasingly large GWAS have revealed 20 loci, which in itself is a substantial increase in our understanding, but perhaps the more important feature is that these studies have highlighted novel pathways that are potentially involved in the disease process. This commentary assembles our latest knowledge while acknowledging that the casual functional variants, and undoubtedly, other genes are still yet to be discovered. This is the challenge that remains and the promise of next-generation sequencing is anticipated as there are a number of large initiatives which themselves should start to yield information before long. SUMMARY A new mesh optimisation scheme, reduced order mesh optimisation, is introduced. The technique uses proper orthogonal decomposition to reduce the number of dimensions in a mesh optimisation problem. This reduction in dimensionality allows the expression of the optimisation problem globally rather than the more traditional local mesh optimisation or smoothing algorithms. To perform the optimisation, the recently developed gradient-free technique modified cuckoo search is applied. The effectiveness of the algorithm is shown by considering the problem of optimising meshes for use in co-volume techniques. Co-volume techniques require the existence of two mutually orthogonal meshes; this is achieved by utilising the Delaunay-Voronoi dual. A combination of considering the problem globally and the use of a gradient-free technique results in a scheme that significantly outperforms previous methods in solving this particular problem. Although the examples presented in this paper are specific to optimising dual meshes, the technique is general and can be simply modified to any mesh optimisation problem. The differential effect of illicit drug use on cognitive function in first-episode psychosis and healthy controls. Objective:: Illicit drug use can result in impairment in cognitive function in healthy individuals. Individuals with a psychotic disorder also show a deficit in cognitive function. Drug use may simply contribute to the characteristic cognitive deficit found in psychosis or alternatively result in a 'double deficit'. This study aims to investigate the association between drug use and cognitive function at the first-episode of psychosis and in community-matched controls. Method:: One hundred and seventy-seven patients at the first episode of psychosis completed a battery of neuropsychological tests. Those that had used drugs in the previous year ( n = 80) were compared with those who had not used drugs in the previous year ( n = 97). A subsample of the first-episode psychosis patients were compared with community-matched controls ( n = 110) according to drug-use status. Results:: Patients with a first episode of psychosis who had used drugs performed equally to those who had not used drugs on neuropsychological tests. In contrast, healthy controls who had used drugs in the previous year performed worse on tests of executive function and working memory compared with those controls that had not used drugs. Morgan K, Bartrop R, Telfer J, Tennant C. A controlled trial investigating the effect of music therapy during an acute psychotic episode. Objective: To investigate the effects of music therapy, as an adjunct to pharmacological therapy during an acute psychotic episode. Method: Sixty participants were quasi-randomised into either a treatment or control group. Standardised psychological assessments [Brief Psychiatric Rating Scale (BPRS), Calgary Interview Guide for Depression, Nurses' Observation Scale for Inpatient Evaluation (NOSIE-30) and Depression Anxiety Stress Scale (DASS21)] were conducted before and after the sessions to determine whether there were any significant changes in outcomes. Results: Statistically significant changes in BPRS scores were seen in the treatment group ( n = 25) compared with the control group ( n = 24). No significant differences were seen in the results of the Calgary, NOSIE-30 or DASS21 scores. Despite the treatment group, having a 9.3% decrease in their length of stay in hospital as opposed to the control group, this did not reach statistical significance. No significant differences were found when comparing the two groups in their doses of antipsychotic, benzodiazepine, mood stabilising or antidepressant medication or at the 1-month follow-up assessment. Conclusion: Most of the variables tested in our study but one did not point at any advances of adding music therapy to pharmacological treatment. The finding of improvement in Brief Psychiatric Rating Scale could be an indicator of music therapy as a useful adjunct to pharmacotherapy during an in-patient hospital stay for the few patients amongst those suffering acute psychosis that accept to participate in music therapy. K. Morgan (2011) Neuropathology and Applied Neurobiology 353-357 ? Genome?wide association studies (GWAS) promise a significant impact on the understanding of late?onset Alzheimer's disease (LOAD) as the genetic components have been estimated to account for 60-80% of the disease. The recent publication of results from large GWAS suggests that LOAD is now one of the best?understood complex disorders. Four recent large LOAD GWAS have resulted in the identification of nine novel loci. These genes are CLU- clusterin, PICALM- phosphatidylinositol?binding clathrin assembly protein, CR1- complement receptor 1, BIN1- bridging integrator 1, ABCA7- ATP?binding cassette transporter, MS4A cluster - membrane?spanning 4?domains subfamily A, CD2AP- CD2?associated protein, CD33- sialic acid?binding immunoglobulin?like lectin and EPHA1- ephrin receptor A1. Collectively, these genes now explain around 50% of LOAD genetics and map on to three new pathways linked to immune system function, cholesterol metabolism and synaptic cell membrane processes. These three new pathways are not strongly linked to the amyloid hypothesis that has driven so much recent thinking and open up avenues for intensive research with regard to the potential for therapeutic intervention. Summary Reasons for performing study: Saddle fit is well recognised as an important factor for the health and performance of riding horses. However, only few studies have addressed general effects of different saddle construction details within a group of horses. Objective: To assess the influence of girth strap placement, traditional vs. v-system, and panel flocking material, wool vs. synthetic foam, on the saddle pressure pattern during riding. Methods: Six horses were ridden by 3 riders in sitting and rising trot and sitting canter. Saddle pressure was measured with 3 different saddle variants: 1) wool flocked panels and traditional girthing (baseline); 2) wool flocked panels and v-system girthing; and 3) foam filled panels and traditional girthing. From the pressure data, a number of descriptive variables were extracted. These were analysed using ANCOVA models with horse, rider, saddle, seat (sitting/rising, trot only) and speed as independent variables. Results: With foam filled panels stride maximum pressures under the hind part of the saddle increased by 7-12% and the area under the saddle with a stride mean pressure >11 kPa increased by 114 cm2 in trot and 127 cm2 in canter. With v-system girthing, the latter variable also increased, but only by 53 and 38 cm2 in trot and canter, respectively. In addition, stride maximum pressures under the front part of the saddle tended to increase (≤9%). Conclusions: Both flocking material and girthing have a significant influence on the saddle pressure and should thus be considered in saddle fitting. Wool seems a better flocking material than foam of the type used in the current study. For girthing, traditional placement seems equally good if not better than the v-system. Aliment Pharmacol Ther 31, 1186–1191 Background Recent genome-wide association studies based on adult and paediatric populations have implicated >30 genes/loci as susceptibility loci for Crohn’s disease (CD). Aims To investigate whether reported genes/loci were also associated with CD in Canadian children. Design and Methods A case-control design was implemented at three paediatric gastroenterology clinics in Canada. Children ≤18 years of age with a confirmed diagnosis of CD were recruited along with controls. Single nucleotide polymorphisms (SNPs) in five genome-wide association studies reported genes/loci were genotyped. Associations between individual SNPs and CD were examined. Results A total of 406 cases and 415 controls were studied. The mean (±s.d.) age of the cases was 12.3 (±3.2) years. Most cases were male (56.6%), had ileo-colonic disease (L3 ± L4, 52.0%) and inflammatory behaviour (B1 ± p, 86.9%) at diagnosis. Allelic association analysis (two-tailed) showed that three of the five targeted SNPs were significantly associated with overall susceptibility for CD ( ZNF365, r10995271, P = 0.001; PTPN2, rs1893217, P = 0.005; STAT3, rs744166, P = 0.01). Associations with SNP rs4613763 in the PTGER4 locus were marginally nonsignificant ( P = 0.07). The ZNF365 and STAT3 SNPs were predominantly associated with ileal disease with or without colonic involvement. Objective: To determine if substance use (particularly cannabis) is more frequent among first episode psychosis patients and associated with a more problematic clinical presentation. Method: All first episode psychosis (FEP) patients presenting to secondary services were recruited from London and Nottingham, over 2 years, in the Aetiology and Ethnicity of Schizophrenia and Other Psychoses study broad framework. Clinical and sociodemographic variables were assessed using a set of standardized instruments. A schedule was created to retrospectively collate substance use data from patients, relatives and clinicians. Results: Five hundred and eleven FEP were identified. They used three to five times more substances than general population. Substance use was associated with poorer social adjustment and a more acute mode of onset. Cannabis use did not affect social adjustment, but was associated with a more acute mode of onset. Conclusion: Cannabis has a different impact on FEP than other substances. To study the instability of FMR1 triplet repeats in the general population, we screened a prospective sample of 24,449 anonymized mother–offspring pairs and analyzed transmissions of intermediate-size (45–54 triplets) and premutation-size (55–200 triplets) alleles. We screened all mothers for alleles ≥ 45 triplets by Southern blot and studied transmission of 545 maternal alleles to their offspring using polymerase chain reaction. Out of 21,411 maternal samples with conclusive results, we identified 250 carriers of at least one intermediate-size allele and 39 carrying a premutation-size allele. Out of a subsample of 430 transmissions of normal-size alleles (< 45 triplets), we observed four (< 1%) unstable transmissions. There were 6/90 intermediate-size unstable alleles (7%) and 11/25 unstable premutation-size alleles (44%). Two mothers transmitted a typical full mutation. The incidence of fragile X syndrome was thus 1/12,225 newborns (upper limit of 95% confidence interval: 1/4638 newborns), but larger in males (1/6209) than females (none detected in over 12,000 newborn females). Intermediate-size alleles were more unstable than normal-size alleles ( p = 0.0027), but more stable (about sixfold) than premutation-size alleles ( p < 0.0001). Unstable premutation-size alleles harbored the major fragile X haplotype (T50-T42-T62), and this haplotype appeared to be a good predictor of instability in premutations ( p = 0.02). Incidence and instability are important to determine the feasibility and cost effectiveness of putative FMR1 screening programs. Carriers of FMR1 alleles of 55+ triplets with no family history of the disease may have a significant risk of expansion to a full mutation in a single generation. [ABSTRACT FROM AUTHOR]

Background A recent genome-wide association study in adult patients with ulcerative colitis (UC) has implicated the interleukin 10 ( IL-10) gene as an important candidate gene. Moreover, a UC-associated single nucleotide polymorphism (SNP) rs3024405 was also significantly associated with adult Crohn’s disease (CD). Aims To examine whether IL-10-CD associations extended to paediatric-onset CD. Methods We implemented the case-control design at three paediatric gastroenterology clinics in Canada. CD patients (≤20 years) were recruited along with healthy controls. DNA samples were genotyped for tag-single nucleotide polymorphisms (tag-SNPs) in the IL-10 gene. Allelic, genotype and haplotype associations with CD were studied. Results A total of 270 patients and 336 controls were studied. The mean age (±s.d.) at diagnosis was 12.1 (±3.5). There were a slightly higher proportion of male patients (56.3%). Of the five IL-10 tag-SNPs, rs2222202 (C/T) ( P = 0.03) and rs1800871 (C/T) ( P = 0.05) showed significant allelic associations with CD. Specific IL-10 SNPs were associated with CD disease location and/or disease behaviour. Conclusions Our gene-wide analysis replicates recent findings of associations between IL-10 and adult CD, and suggests that these associations extend to paediatric-onset CD as well. Objective:  We sought to investigate the prevalence and social correlates of psychotic-like experiences in a general population sample of Black and White British subjects. Method:  Data were collected from randomly selected community control subjects, recruited as part of the ÆSOP study, a three-centre population based study of first-episode psychosis. Results:  The proportion of subjects reporting one or more psychotic-like experience was 19% ( n = 72/372). These were more common in Black Caribbean (OR 2.08) and Black African subjects (OR 4.59), compared with White British. In addition, a number of indicators of childhood and adult disadvantage were associated with psychotic-like experiences. When these variables were simultaneously entered into a regression model, Black African ethnicity, concentrated adult disadvantage, and separation from parents retained a significant effect. Conclusion:  The higher prevalence of psychotic-like experiences in the Black Caribbean, but not Black African, group was explained by high levels of social disadvantage over the life course. Smooth muscle contractile activity is a major regulator of function of the vascular system, respiratory system, gastrointestinal system and the genitourinary systems. Malfunction of contractility in these systems leads to a host of clinical disorders, and yet, we still have major gaps in our understanding of the molecular mechanisms by which contractility of the differentiated smooth muscle cell is regulated. This review will summarize recent advances in the molecular understanding of the regulation of smooth muscle myosin activity via phosphorylation/dephosphorylation of myosin, the regulation of the accessibility of actin to myosin via the actin-binding proteins calponin and caldesmon, and the remodelling of the actin cytoskeleton. Understanding of the molecular 'players' should identify target molecules that could point the way to novel drug discovery programs for the treatment of smooth muscle disorders such as cardiovascular disease, asthma, functional bowel disease and pre-term labour. The smooth muscle of the uterus during pregnancy presents a unique circumstance of physiological mechanotransduction as the tissue remodels in response to stretches imposed by the growing foetus(es), yet the nature of the molecular and functional adaptations remain unresolved. We studied, in myometrium isolated from non-pregnant (NP) and pregnant mice, the active and passive length-tension curves by myography and the expression and activation by immunoblotting of focal adhesion-related proteins known in other systems to participate in mechanosensing and mechanotransduction. In situ uterine mass correlated with pup number and weight throughout pregnancy. In vitro myometrial active, and passive, length-tension curves shifted significantly to the right during pregnancy indicative of altered mechanosensitivity; at term, maximum active tension was generated following 3.94 ± 0.33-fold stretch beyond slack length compared to 1.91 ± 0.12-fold for NP mice. Moreover, mechanotransduction was altered during pregnancy as evidenced by the progressive increase in absolute force production at each optimal stretch. Pregnancy was concomitantly associated with an increased expression of the dense plaque-associated proteins FAK and paxillin, and elevated activation of FAK, paxillin, c-Src and extracellular signal-regulated kinase (ERK1/2) which reversed 1 day post-partum. Electron microscopy revealed close appositioning of neighbouring myometrial cells across a narrow extracellular cleft adjoining plasmalemmal dense plaques. Collectively, these results suggest a physiological basis of myometrial length adaptation, long known to be a property of many smooth muscles, whereupon plasmalemmal dense plaque proteins serve as molecular signalling and structural platforms contributing to functional (contractile) remodelling in response to chronic stretch.
