Background: Metastatic breast cancers are frequently encountered in cytology and require immunocytochemistry (ICC). In this study, traditional and multiplex ICC (mICC) for GATA‐binding protein 3 (GATA3), gross cystic disease fluid protein 15 (GCDFP15), and mammaglobin (MMG) were performed with the aim of validating mICC in cell blocks, with further single‐cell expression pattern analysis to identify the single markers and combinations of markers most sensitive in subtypes of breast cancer. Methods: GATA3, GCDFP15, and MMG were paired with OptiView 3,3′‐diaminobenzidine and Ventana DISCOVERY Purple and Blue, respectively, with cyclical and serial staining. Bright‐field imaging was performed with the Mantra 2 system and analyzed with the inForm Tissue Finder (Akoya Biosciences). Cell detection and phenotyping were further confirmed by two pathologists. Results: In the 36 cases studied, traditional ICC and mICC demonstrated good concordance (kappa coefficient, >0.5; p <.01) at three cutoffs (1%, 5%, and 50%), except for GATA3 at the 1% cutoff. Single‐marker positivity outnumbered double‐marker positivity and the exceedingly rare triple‐marker positivity (<3%). GATA3 was the leading single marker–positive phenotype in all breast cancer subtypes, except for MMG in estrogen receptor–positive, progesterone receptor–positive, and human epidermal growth factor receptor 2–positive (ER+/PR+/HER2+) breast cancers. Limited to two markers, GATA3/MMG included the greatest number of tumor cells for luminal breast cancers (ER+/PR+/HER2+, 60.6%; ER+/PR+/HER2+, 31.4%), whereas HER2‐overexpressed breast cancers (27.4%) and triple‐negative breast cancers (26.4%) favored the combination of GATA3/GCDFP15. Conclusions: For a single marker, GATA3 displayed the highest sensitivity. The addition of MMG for hormone receptor‐positive breast cancers and GCDFP15 for hormone receptor‐negative breast cancers further increased sensitivity. The low proportion of multimarker‐positive cells suggested that the coexpression observed with traditional ICC is attributable to intratumoral heterogeneity, not genuine coexpression. Urine cytology is a noninvasive, widely used diagnostic tool for screening and surveillance of genitourinary tract neoplasms. However, the absence of unified terminology and clear objective morphological criteria limits the clinical benefit of urine cytology. The Paris System for Reporting Urine Cytology (TPS) was developed with the goal of standardizing reporting and improving urine cytology performance in detecting high‐grade malignancy (HGM). We aimed to evaluate potential effects of TPS on improving urine cytology diagnostic performance and clinical utility by conducting a systematic review and meta‐analysis. We searched six electronic databases to identify cross‐sectional and cohort studies written in English assessing the accuracy of urine cytology in detecting genitourinary tract malignancies of patients under surveillance or with clinical suspicion of malignancy from January 2004 to December 2022. We extracted relevant data from eligible studies to calculate relative distribution of cytology diagnostic categories; ratio of atypical to HGM cytology diagnosis; and risk of HGM (ROHGM) and HGM likelihood ratio (HGM‐LR) associated with cytology diagnostic categories. We used a generalized linear mixed model with logit transformation to combine proportions and multilevel mixed‐effect logistic regression to pool diagnostic accuracy measurements. We performed meta‐regression to evaluate any significant difference between TPS and non‐TPS cohorts. We included 64 studies for 99,796 combined total cytology samples, across 31 TPS and 49 non‐TPS cohorts. Pooled relative distribution [95% confidence interval (CI)] of negative for high‐grade urothelial carcinoma (NHGUC)/negative for malignancy (NM); atypical urothelial cells (AUC); suspicious for high‐grade urothelial carcinoma (SHGUC)/suspicious for malignancy (SM); low‐grade urothelial neoplasm (LGUN); and HGM categories among satisfactory cytology cases were 83.8% (80.3%–86.9%), 8.0% (6.0%–10.6%), 2.2% (1.4%–3.3%), 0.01% (0.0%–0.1%), and 4.2% (3.2%–5.5%) in TPS versus 80.8% (76.8–2.7%), 11.3% (8.6%–14.7%), 1.8% (1.2%–2.7%), 0.01% (0.0%–0.1%), and 3.3% (2.5%–4.3%) in non‐TPS cohorts. Adopting TPS classification resulted in a significant increase in the frequency of NHGUC and a reduction in AUC cytology diagnoses, respectively. The AUC/HGM ratio in TPS cohort was 2.0, which showed a statistically significant difference from the atypical/HGM ratio of 4.1 in non‐TPS cohort (p‐value: 0.01). Moreover, the summary rate (95% CI) of LGUN called AUC on cytology significantly decreased to 20.8% (14.9%–28.3%) in the TPS compared with 34.1% (26.4%–42.8%) in non‐TPS cohorts. The pooled ROHGM (95% CI) was 20.4% (6.2%–50.0%) in nondiagnostic (NDX), 15.5% (9.6%–24.2%) in NHGUC, 40.2% (30.9%–50.2%) in AUC, 80.8% (72.9%–86.8%) in SHGUC, 15.1% (5.7%–34.3%) in LGUN, and 91.4% (87.3%–94.3%) in HGM categories in TPS studies. NHGUC, AUC, SHGUC, and HGM categories were associated with HGM‐LR (95% CI) of 0.2 (0.1–0.3), 0.9 (0.6–1.3), 6.9 (2.4–19.9), and 16.8 (8.3–33.8). Our results suggest that TPS 1.0 has reduced the relative frequency of AUC diagnosis, AUC/HGM ratio, and the frequency of LGUNs diagnosed as AUC on cytology. Adopting this classification has improved the clinical utility of SHGUC and HGM cytology diagnoses in ruling in high‐grade lesions. Aims: The neutrophil–lymphocyte ratio (NLR) is a systemic reflection of cancer‐associated inflammation and a prognostic marker for breast cancer. For the local tumour microenvironment, tumour‐infiltrating lymphocytes (TILs) and tumour‐associated macrophages (TAMs) are also highly correlated with breast cancer survival. This study aimed to explore the relationship between the circulating and local immune microenvironment, and to further delineate the prognostic role of NLR in breast cancer patients receiving neoadjuvant chemotherapy (NAC). Methods: A cohort of breast cancer patients receiving NAC with subsequent surgery was retrieved. Clinical data were reviewed. Histological slides and CD8 immunohistochemistry from biopsy (pre‐chemotherapy) and excision (postchemotherapy) specimens were assessed for TILs and TAMs. Results: A total of 146 patients were included. There was a significant positive correlation between pre‐ and postsurgery NLR at a cut‐off of 2.6 (median pre‐chemotherapy NLR) (P < 0.001). NLR pre‐chemotherapy was associated positively with necrosis on biopsy (P = 0.027) and excision (P = 0.021) and TAMs on excision (P = 0.049). NLR 1 year postsurgery was associated with high tumour stage (P = 0.050) and low histological grade (P = 0.008). TIL count was lower in NLR‐high cases at almost all time‐points by histological assessment and CD8 immunostaining (P < 0.050). In multivariate analysis, postsurgery NLR is an independent predictor for overall survival [OS; hazard ratio (HR) = 9.524, P < 0.001], breast cancer‐specific survival (BCSS) (HR = 10.059, P = 0.001) and disease‐free survival (DFS; HR = 2.824, P = 0.016). Conclusions: The association between NLR with tumour necrosis, TAMs and TILs illustrates an interaction between the circulating and local immune microenvironment. Aims: Trichorhinophalangeal syndrome‐1 (TRPS1) has been proposed as a novel breast marker with equally high expression in breast cancer (BC) subtypes, making it a useful diagnostic tool. Here, its expression was evaluated alongside other commonly used markers [GATA3, GCDFP15, mammaglobin (MGB) and SOX10] in a large cohort of BCs (n = 1852) and their corresponding nodal metastases. Its usefulness as a diagnostic tool and its correlation with clinicopathological features were assessed. Methods and results: TRPS1 was expressed at 75.8% overall in the BC cohort, with at least 58% expression among BC subtypes. It was less sensitive than GATA3 for luminal and HER2‐overexpressing (HER2‐OE) cancers (luminal A: 82 versus 97%; luminal B: 80 versus 95%; HER2‐OE: 62 versus 76%), but it was the most sensitive for TNBC (60 versus ≤ 41%). It showed a stable expression in nodal metastases (primary tumour 76 versus nodal metastasis 78%), unlike a reduced nodal expression for GATA3 (86 versus 77%). TRPS1 outperformed GATA3 in detecting non‐luminal cancers when paired with other breast markers. TRPS1 and GCDFP15 was the most sensitive combination in TNBC detection, with a 76% detection rate. For TRPS1‐negative and GCDFP15‐negative TNBCs, SOX10 was more sensitive than GATA3 (29 versus 24%). Conclusions: TRPS1 is a highly sensitive marker for all breast cancer subtypes, outperforming GATA3 in non‐luminal cancers and displaying the highest sensitivity for TNBC detection when combined with GCDFP15. Fine‐needle aspiration cytology (FNAC) is a versatile diagnostic procedure uniquely suited for tissue biopsy of breast carcinomas and axillary metastases and/or recurrences. With the expanding treatment options and accompanying theragnostic tests, it is crucial to recognize the developments on ancillary testing and digital cytopathology techniques related to aspiration cytology of metastatic breast carcinoma. In this review, we aim to summarize and update the evidence of immunocytochemistry, for the detection of carcinoma cells (epithelial markers), confirmation of breast primary (breast‐specific markers), assessment of surrogate immunostains (hormone receptors, ki‐67 proliferative index and HER2) and theragnostic biomarkers, with discussion on potential diagnostic pitfalls, followed by the application of molecular tests, and digital cytopathologic techniques for assessing metastatic breast carcinoma in cytology. Introduction: Compared with urothelial lesions of the upper urinary tract, the diagnostic performance of urine cytology in detection of renal cell carcinomas is underreported. This study aims to establish the role of urine cytology in the assessment of renal carcinomas by a multi‐institute review of urine cytology from nephrectomy confirmed renal cell carcinomas, referenced against renal urothelial and squamous cell carcinomas. Methods: Records of nephrectomy performed from the 1990s to 2020s at three hospitals were retrieved and matched to urine cytology specimens collected within 1 year prior. Patient demographics, specimen descriptors, and histology and staging parameters were reviewed and compared against cytologic diagnoses. Results: There were 1147 cases of urine cytology matched with renal cell carcinomas, with 666 renal urothelial/squamous carcinomas for comparison. The detection rate for urothelial/squamous (atypia or above [C3+]: 63.1%; suspicious or above [C4+]: 24.0%) were higher than renal cell carcinoma (C3+: 13.1%; C4+: 1.5%) (p < 0.001). The positive rate for upper tract urine exceeded other collection methods at 45.0% (C3+) and 10.0% (C4+) (p <.01). Other factors associated with increased positive rates were male sex, collecting duct carcinoma histology, nuclear grade, and renal/sinus involvement (p <.05). Multivariate analysis revealed additional positive correlations with presence of sarcomatoid tumor cells, lymphovascular invasion, and perinephric fat involvement (p <.05). Larger lesion size and higher urine volume did not improve detection rates (p <.05). Conclusions: The detection rate of renal cell carcinomas is suboptimal compared with urothelial carcinomas, although urine samples collected from cystoscopy or percutaneous nephrostomy significantly outperformed voided urine specimens. Introduction: Specific diagnosis of endometrial carcinomas on cervical cytology is difficult with few useful cytomorphological clues reported. This study reviews a cohort of cervical cytology to investigate the presence of keratinization in atypical glandular cells (AGC), an undescribed cytomorphological clue for identifying endometrial endometrioid carcinomas on cervical cytology. Methods: Cervical cytology slides from patients with a histologic diagnosis of endometrial endometrioid carcinoma were reviewed for the presence of keratinization associated with AGCs. Corresponding histology slides were reviewed for tumour grading and degree of squamous differentiation. Results: In total, 42 cases of cervical cytology specimens from 41 patients were retrieved, including 7 (16.7%) with keratinization associated with AGCs seen and 35 (83.3%) without. Comparison of histologic grading did not demonstrate an association with the presence of keratinization on cytology (p = 0.565). Corresponding histology slides were available for 37 cases. Cytologic and histologic keratinization were associated statistically (p = 0.002). Frank keratinization was seen on histologic slides of five cases, with four also showing cytologic keratinization. Area of squamous differentiation, including squamous morule formation, did not correlate with keratinization on cytologic preparation (p = 0.185). Conclusion: Histologic and cytologic keratinization are observed in endometrioid endometrial carcinomas. Such is reflected in cervical cytology by the presence of orangeophilic, rigid and acellular fragments within or associated with AGC clusters. Keratinization, when identified with AGCs, should be regarded as a cytologic clue suggestive of an endometroid carcinoma of endometrial origin. [ABSTRACT FROM AUTHOR]

Aspirates of liver abscess are frequently encountered in routine practice and are often of a low index of suspicion. However, necrotic liver metastasis clinically and radiologically mimics liver abscesses, and malignant cells can be obscured in an inflammation‐rich background on cytology. Anti‐melanoma differentiation‐associated gene 5 (MDA5) dermatomyositis is characterized by serological detection of anti‐MDA5 antibody and rapidly progressive interstitial lung disease. In this study, the largest cohort of skin biopsies to date of anti‐MDA5 dermatomyositis was reviewed and compared with cases of dermatomyositis with negative serology. Findings contribute to the histological diagnosis and evaluation of the severity of cutaneous inflammation in anti‐MDA5 dermatomyositis. Skin biopsies collected over a 7‐year period from individuals with clinically and histologically confirmed dermatomyositis with anti‐MDA5 serology were reviewed. A total of 46 cases with 17 anti‐MDA5 positive cases were retrieved. Patients with positive antibody were younger (53.7 vs. 60.6 years, p =.013). No differences in epidermal changes (p >.05) were observed. Pertaining to interface changes, anti‐MDA5 dermatomyositis showed a higher degree of pigmentary incontinence (p =.014), suggesting increased and sustained cutaneous inflammation. Periodic acid–Schiff (PAS) stain demonstrated a greater degree of basement membrane thickening (p =.045). Other parameters, including dermal inflammation, dermal mucin deposition and vasculitic/vasculopathic features did not show statistical difference between anti‐MDA5 positive and negative dermatomyositis (p >.05). Findings suggest increased cutaneous inflammation for anti‐MDA5 dermatomyositis. In skin biopsies, marked pigmentary incontinence or basement membrane thickening should raise suspicion of anti‐MDA5 dermatomyositis. [ABSTRACT FROM AUTHOR]

Introduction: Endometrial lesions are morphologically diverse and uncommon on cervical smears, with its detection rate and associated diagnostic categories uncharacterized. In this study, cervical smears matched to histologically proven endometrial hyperplasias and carcinomas were reviewed and compared with cervical in‐situ‐carcinomas/carcinomas, aiming to detail the diagnostic performance of cervical smears for upper tract and glandular lesions. Methods: Pathology reports of cervical smears, hysterectomies, endometrial and cervical biopsies from 1995 to 2021 were retrieved. Diagnoses of cervical smears were matched to endometrial hyperplasias and carcinomas, or cervical carcinomas and reviewed. Results: Totally 832 cervical smears (272 cervical carcinomas, 312 endometrial carcinomas, and 248 hyperplasias) were included. Considering all cytologic glandular diagnosis as positive, the detection rate of cervical adenocarcinoma‐in‐situ was the highest (64.3%), followed by cervical adenocarcinoma (63.8%), endometrial carcinoma (31.7%), and hyperplasia (with atypia–8.5%; without atypia–2.3%) (p < 0.001). Endometrial hyperplasia was most often diagnosed as atypical squamous cells of undetermined significance (ASCUS) (5.0%) or atypical glandular cells, not otherwise specified (3.6%) without indication of endometrial origin. For endometrial carcinomas, higher FIGO grading and endocervical involvement were associated with higher detection rates across all diagnostic categories (p = 0.002–0.028). High FIGO grade was associated with suspicious/favor neoplastic (C4) (31.1%vs10.3%, p < 0.001) and carcinoma (C5) (17.8% vs. 5.6%, p = 0.005) categories, but not for all glandular diagnoses combined (33.3% vs. 31.0%, p = 0.761). Conclusion: Detection rates for endometrial lesions are lower than cervical lesions but not insignificant. Endometrial hyperplasia should be recognized as a differential of human papilloma virus‐negative ASCUS and prompt consideration of investigation of the upper genital tract. [ABSTRACT FROM AUTHOR]

Aims: Sentinel lymph node (SLN) biopsy is the current standard assessment for tumour burden in axillary lymph node (ALN). However, not all SLN+ patients have ALN metastasis. The prognostic implication of SLN features is not clear. We aimed to evaluate predictive factors for ALN metastasis and the clinical value of SLN features. Methods and results: A total of 228 SLN+ and 228 SLN– (with matched year and grade) cases were included. Clinicopathological features in SLN, ALN and primary tumours, treatment data and survival data were analysed according to ALN status and outcome. Except for larger tumour size and the presence of LVI (both P < 0.001), no significant differences were found in SLN– and SLN+ cases. Only 31.8% of SLN+ cases with ALN dissection had ALN metastasis. The presence of macrometastases (MaM), extranodal extension (ENE), deeper level of tumour invasion in SLN and more SLN+ nodes were associated with ALN metastasis (P ≤ 0.025). Moreover, isolated tumour cells (ITC) and level of tumour invasion in SLN were independent adverse prognostic features for disease‐free survival and breast cancer‐specific survival, respectively. Interestingly, cases with ITC located in the subcapsular region have better survival than those in cortex (OS: χ2 = 4.046, P = 0.044). Conclusions: Our study identified features in SLN, i.e. the level of tumour invasion at SLN and tumour size in SLN as useful predictors for both ALN metastasis and breast cancer outcome. Background: Pemphigus is a group of immunobullous dermatoses characterized by the presence of autoantibodies directed against adhesion molecules of keratinocytes, with pemphigus vegetans being the rarest form, accounting for 1%-2% of all cases of pemphigus. Pemphigus vegetans is characterized by verrucous vegetative lesions in addition to vesiculobullous lesions. Methods: We report a rare case of pemphigus vegetans presenting as an isolated vegetative lesion in the groin 3 months prior to the development of blisters. Owing to the atypical presentation, multiple biopsies were performed before and after corticosteroid treatment. Results: Comparing the histopathology of pre-treatment and post-treatment biopsy specimens, the resolution of intraepidermal microabscesses, and reduction in intraepidermal and dermal inflammatory infiltrates, spongiosis and interface change, attributable to treatment, were noted. However, direct immunofluorescence showed persistent intracellular intraepidermal deposition of IgG and C3 2 weeks into treatment, despite near-complete resolution of blisters on clinical examination. Clinical regression of the vegetative lesion was noted only after 6 weeks into corticosteroid treatment, while histopathological evidence of treatment was apparent at the second week. Conclusion: The current case illustrates the importance of a liberal use of immunofluorescence studies in establishing the uncommon yet significant diagnosis of pemphigus vegetans, particularly for vegetative lesions that are persistent, in the intertriginous areas and/or in the setting of concurrent cutaneous or mucosal symptoms. Evaluation of oestrogen receptor (ER) expression by immunostaining is essential in the pathological assessment of breast cancer. Its expression is intercorrelated with clinicopathological features, molecular typing, and treatment selection. The development of novel therapeutic agents related to ER status, the recent ASCO introduction of an ER-low positive category of breast cancers, and the ever-increasing plethora of diagnostic and theragnostic markers call for a timely update. In this article we aim to review the clinicopathological features of ER-positive breast cancers, with an emphasis on ER-low positive breast cancers, and a focus on updating the (i) assessment, reporting and interpretation of ER immunohistochemical (IHC) staining, (ii) correlations of ER status with other diagnostic and theragnostic markers, and (iii) implications for treatment selection and response. In the face of the developments in IHC and molecular techniques and targeted therapy, ER immunostaining is still expected to remain as the core component of prognostic and theragnostic assessment of breast cancers. Background: Pleural effusion can be caused by a wide range of benign and malignant conditions. Pleural biopsy and effusion cytology represent two key methods of pathological diagnosis. To compare the performance these two methods, a large cohort of matched pleural biopsy and effusion cytology with clinical follow‐up was reviewed. Methods: Pleural biopsies and effusion cytology specimens over a period of 18 years were retrieved. Cytology specimens collected within 7 days of pleural biopsy were matched. Reports were reviewed, and the cause for pleural effusion was determined by hospital disease coding and clinical data. Results: Totally, 3026 cases were included. The leading cause of benign effusion was tuberculosis (n = 650). Malignant pleural effusion (MPE) was more common in older females (p < 0.001) and mostly due to lung cancer (n = 959), breast cancer (n = 64), and mesothelioma (n = 48). The inadequate/insufficient (B1/C1) rate of biopsy was higher than cytology (15.6% vs. 0.3%) but the rates for other diagnostic categories were similar. Biopsy and cytology showed a correlation coefficient of 0.315, improving to 0.449 when inadequate/insufficient (B1/C1) cases were excluded. The ROM for benign cytology (C2) was lower than biopsy (B2) (p < 0.001). Compared with biopsy, the diagnostic accuracy was higher in cytology overall and for metastatic carcinomas (p < 0.001) but lower for hematolymphoid malignancies (p = 0.014) and mesotheliomas (p = 0.002). Conclusions: These results suggest that effusion cytology may be better for confirming benignity and diagnosing carcinomatous MPE. In these cases, pleural biopsy may be withheld to reduce procedural risks. However, for suspected hematolymphoid malignancies and mesothelioma, biopsy should be considered. [ABSTRACT FROM AUTHOR]

Background: Papillary thyroid carcinoma (PTC) is the most common primary malignant thyroid neoplasm and malignant diagnosis in thyroid aspirates. Metastatic adenocarcinoma of the lung is an under‐recognised mimicker which overlaps cytomorphologically and immunocytochemically with PTC. This case series reviews thyroid aspirates of metastatic lung carcinomas, aiming to address the similarities and methods of differentiating this entity from PTC. Methods: Thyroid aspirates of metastatic lung carcinomas were obtained by a computerised search. Clinico‐cytological features and ancillary test results were reviewed. A literature review was performed for published cases of metastatic lung carcinomas in thyroid aspirates. Results: A total of 14 cases were found, including nine adenocarcinomas, three TTF1‐positive non‐small cell lung carcinomas (NSCLCs), one small cell carcinoma and one squamous cell carcinoma. The adenocarcinomas and TTF1‐positive NSCLCs displayed PTC‐like features including papillae/papillary‐like fronds (n = 6/12, 50%), nuclear grooves (n = 5/12, 41.7%) and inclusions (n = 1/12, 8.3%), chromatin clearing (n = 3/12, 25%), calcifications (n = 3/12, 25%) and multinucleated giant cells (n = 2/12, 16.7%). Useful distinguishing features observed were prominent nucleoli, coarse chromatin, mitosis, and necrosis. TTF‐1 immunocytochemistry was positive in most cases (n = 5/6, 83.3%), while PAX8 and thyroglobulin were consistently negative. EGFR exon 19 deletion was detected on cell block preparation in a single case, corresponding to its lung primary. The literature search yielded 84 cases of metastatic malignancies, with lung carcinomas comprising of 3.6%–33.3% in case series of metastatic malignancies. Conclusion: Metastatic adenocarcinoma of lung and PTC share significant cytomorphological and immunocytochemical similarities. Whole‐exome sequencing of two patients with idiopathic complex neurodevelopmental disorder (NDD) identified biallelic variants of unknown significance within FIBCD1, encoding an endocytic acetyl group‐binding transmembrane receptor with no known function in the central nervous system. We found that FIBCD1 preferentially binds and endocytoses glycosaminoglycan (GAG) chondroitin sulphate‐4S (CS‐4S) and regulates GAG content of the brain extracellular matrix (ECM). In silico molecular simulation studies and GAG binding analyses of patient variants determined that such variants are loss‐of‐function by disrupting FIBCD1‐CS‐4S association. Gene knockdown in flies resulted in morphological disruption of the neuromuscular junction and motor‐related behavioural deficits. In humans and mice, FIBCD1 is expressed in discrete brain regions, including the hippocampus. Fibcd1 KO mice exhibited normal hippocampal neuronal morphology but impaired hippocampal‐dependent learning. Further, hippocampal synaptic remodelling in acute slices from Fibcd1 KO mice was deficient but restored upon enzymatically modulating the ECM. Together, we identified FIBCD1 as an endocytic receptor for GAGs in the brain ECM and a novel gene associated with an NDD, revealing a critical role in nervous system structure, function and plasticity. BACKGROUND: Fine‐needle aspiration (FNA) is a robust diagnostic technique often used for tissue diagnosis of metastatic carcinoma. For interpretation of FNA cytology, cell block immunohistochemistry (IHC) and clinicocytologic parameters are indispensable. In this review of a large cohort, the current report: 1) describes clinicocytologic parameters and immunoprofiles of aspirates of metastatic carcinoma, 2) compares the predictivity of immunostains and classical approaches for IHC interpretation, and 3) describes machine learning‐based algorithms for IHC interpretation. METHODS: Aspirates of metastatic carcinoma that had IHC performed were retrieved. Clinicocytologic parameters, IHC results, the corresponding primary site, and histologic diagnoses were recorded. By using machine learning, decision trees for predicting the primary site were generated, their performance was compared with 2 human‐designed algorithms, and the primary site was suggested in the historical diagnosis. RESULTS: In total, 1145 cases were identified. The 6 most populated groups were selected for machine learning and predictive analysis. With IHC input, the decision tree achieved a concordance rate of 94.5% and overall accuracy of 83.6%, which improved to 95.3% and 85.8%, respectively, when clinical data were incorporated and exceeded the human‐designed IHC algorithms (P <.001). The historical diagnosis was more accurate unless indeterminate diagnoses were regarded as discordant (P <.001). CDX2 and TTF‐1 immunostains had the highest weight in model accuracy, occupied the root of the decision trees, scored higher as features of importance, and outperformed the predictive power of cytokeratins 7 and 20. CONCLUSIONS: Cytokeratins 7 and 20 may be superseded in immunostaining panels, including organ‐specific immunostains such as CDX2 and TTF‐1. Transportation agencies build statistical models and predict the average crash frequency to identify hazardous road sections and make informed decisions to reduce crashes. In this paper, safety performance models (SPFs) were built and evaluated considering various pavement and roadway characteristics, including pavement friction, which is seldom available for analysis. Four count data models—Poisson model, negative binomial (NB) model, hurdle-NB model (HNB), and zero-inflated NB (ZINB) model—were built based on roadway characteristics and crash data provided by the Oklahoma Department of Transportation (ODOT). Pavement friction, roadway geometry, surface condition characteristics, and traffic exposure were considered the contributing factors to traffic crashes. Established models were compared in terms of the goodness-of-fit, zero inflation, and statistical significance of factors. The HNB model exhibited promising fitting performance with a manageable number of influencing variables. Coefficients in the HNB model suggest that adequate pavement friction and the presence of shoulders can significantly reduce the crash frequency and thus improve roadway safety performance. Potential issues of the statistical models, such as unobserved heterogeneity and multicollinearity, were also discussed. The relation between roadway infrastructure characteristics (including pavement friction) and roadway safety revealed in this study could assist in choosing the proper statistical model for better decision-making and selecting appropriate preventive treatments for improved roadway safety. Background: Papillary structures are frequently encountered in metastatic carcinomas from various organs and tumours of different histotypes. This study aims to investigate the predictive value of fine needle aspiration cytology (FNAC), immunohistochemistry (IHC) and the clinical parameters which can be examined in the assessment of the primary sites of metastatic carcinomas with papillary architecture. Methods: FNAC samples of metastatic carcinomas with papillary architecture were evaluated for overall cellularity, epithelial cohesion, background features, papillary architecture, cytology and IHC. The corresponding clinical information was also reviewed. Results: A total of 130 cases were included. The most common primary sites were thyroid (38.5%), lung (30.8%) and gynecological organs (22.3%); the others were pancreaticobiliary, urothelial, colorectal, and esophageal. Age (P = 0.039), biopsy site (P < 0.001) and laterality (P = 0.006) correlated with primary site. Papillary structures were confirmed on biopsy/excision of most cases (n = 85/87, 97.7%). Thyroid primaries exhibited broad papillary stalks, thin lining epithelium, fewer epithelial polymorphs, and the presence of background giant cells and histiocytes (P = 0.021‐ < 0.001). Low‐grade cytological features, nuclear grooves and inclusions (P < 0.001) were seen in thyroid primaries. High‐grade features (P < 0.001‐0.49), multinucleated tumour cells, apoptotic bodies and mitoses (P < 0.001‐0.49) were more common in lung/gynecological primaries. Multivariate analysis identified nuclear/cytoplasmic ratio, chromatin character, the presence of nuclear grooves and mitosis as independent features (P = 0.001‐0.024). TTF1/TGB/PAX8 panel results showed good agreement with the cytological assessment and site of primary. Conclusion: Papillary structures and cytological features are reproducible in FNAC assessment of metastases and their corresponding primary sites. Background: The aim of the International Academy of Cytology Yokohama System for Reporting Breast Fine Needle Aspiration Biopsy Cytopathology is to improve cytology practice. This study assessed cytologic diagnoses made with the system and its efficacy when it was applied by pathologists with different levels of experience. Methods: In all, 1080 cases of breast fine‐needle aspiration biopsy (FNAB) over a period of 16 years were reviewed and reclassified with the system. The category distribution and the diagnostic performance were compared with the original diagnoses. The concordance rates for diagnoses from pathologists with different levels of experience were also determined. Results: The distribution of cytologic diagnoses made with the system was as follows: 11.7% were insufficient, 56.6% were benign, 20.1% were atypical, 6.1% were suspicious for malignancy, and 5.6% were malignant. The rates for the insufficient and atypical categories were lower than the original diagnosis rates (13.1% and 23.8%, respectively). Overall, 120 cases (11.1%) were recategorized. Among those recategorized as benign, suspicious, or malignant with follow‐up data, 96.7% were correctly reclassified. A significant improvement in diagnostic performance was found with the system (P <.001). Such improvement was also seen in problematic breast lesions, including fibroepithelial lesions, papillary lesions, and low‐grade carcinomas. Pathologists with intermediate experience showed a higher concordance with an expert pathologist in the diagnoses than those with short experience (κ, 0.838 vs 0.634). Conclusions: The system effectively categorized the diagnoses, and the diagnostic performance of FNAB reporting was improved. The structured reporting also enhanced the reproducibility of reporting by pathologists with intermediate experience and, to some extent, those with short experience. Background: The cytologic diagnosis of papillary lesions of the breast is challenging because of the diverse morphology, including epithelial hyperplasia, atypia, low‐grade malignancy, and neuroendocrine differentiation; also, traditional malignant features such as necrosis and myoepithelial cell loss can be lacking. Thus, the diagnostic criteria for papillary lesions may differ from those for other breast lesions. This study evaluated various cytologic parameters in a large cohort to identify useful diagnostic features. Methods: Cytologic preparations of papillary lesions with histologic follow‐up were reviewed for features related to cellularity, epithelial cohesiveness, cellular and stromal architecture, cytomorphology, and background. Corresponding histologic slides were also reviewed. Results: In all, 153 cases were included. Epithelial discohesion, solid and cribriform patterns, atypical nuclear features, and mitoses (P ≤.001 to P =.017) were associated with malignancy. Cell balls, monolayer sheets, and features of cystic change (P <.001 to P =.016) were associated with benign lesions. Complex (P =.031) and slender (P =.026) papillae and neuroendocrine features (P <.001) were associated with malignancy. Hemorrhage, background, and infiltrating neutrophils (P <.001 to P =.025) were associated with malignancy; fibrotic broad papillary stromal fragments (naked papillary fronds [NPFs]; P =.043) were associated with benignity. The presence of any single parameter, including the absence of myoepithelial cells within epithelial structure, the presence of cytoplasmic granules, an increased amount of cytoplasm, and a nuclear to cytoplasmic (N/C) ratio greater than 0.7, which were identified by principal component analysis, yielded a sensitivity of 95.1% and a specificity of 100.0% in predicting malignancy. Conclusions: Methodological assessment of multiple features is recommended. Myoepithelial cells, cytoplasmic granules, the amount of cytoplasm, and the N/C ratio are key features for diagnosis. Aims: Confirmation of a breast origin for triple‐negative breast cancer (TNBC) is sometimes problematic. The traditional breast markers GATA‐binding protein 3 (GATA3), mammaglobin (MGB) and gross cystic disease fluid protein 15 (GCDFP15) have shown limitations in identifying TNBC. Here, we aimed to examine the diagnostic potential of the newly proposed TNBC marker, Sry‐related high‐mobility‐group/HMG box 10 (SOX10). Methods and results: We analysed and compared SOX10 expression with GATA3, MGB and GCDFP15 expression in a test cohort of 1838 invasive breast cancers (IBCs) by using tissue microarrays. The findings from the test cohort were further examined with a validation cohort of 42 TNBCs in whole sections. The overall expression rates of SOX10, GATA3, MGB and GCDFP15 were 6.9%, 83.1%, 47.0%, and 34.8%, respectively. Among the TNBCs within this cohort, the expression rates of SOX10, GATA3, MGB and GCDFP15 were 31.3%, 34.5%, 27.9%, and 25.2%, respectively. SOX10 was strongly associated with TNBC (P < 0.001), whereas all other traditional markers were associated with non‐TNBC (P < 0.001 for all). In addition, SOX10 was more correlated to basal‐like breast cancer (BLBC) (P = 0.001) than five‐marker‐negative subtype among the TNBCs. A high expression rate of SOX10 (81%) was confirmed in the validation cohort. Additionally, SOX10 expression was inversely correlated with GATA3 and GCDFP15 expression, so they may complement each other in TNBC detection. The SOX10–GATA3 combination yielded a sensitivity of 60.3% for TNBC detection in the test cohort. Conclusion: SOX10 is a reliable marker for identifying TNBC, and complements GATA3. Aims Materials and methods Results Conclusions This study evaluates the utility of breast specific markers in identifying breast cancer subtypes within metastatic settings. The subtype alteration in metastatic disease and its consequent impact on breast‐specific marker expression is also examined.GATA‐binding protein 3 (GATA3), mammaglobin (MMG), transcriptional repressor GATA binding 1 (TRSP1) and SRY‐box transcription factor 10 (SOX10) expression were assessed in a large cohort of metastatic breast cancer (MBC) cases and correlated with the characteristics of both MBC and primary breast cancer (PBC).GATA3 was the most sensitive in MBC (83.1%), followed by TRPS1 (77.0%), MMG (58.5%) and SOX10 (7.1%). This trend was consistent in hormonal receptor (HR)+ and HR− MBC. Combining GATA3/TRPS1 yielded the highest detection rates in the overall cohort (90.1%) and HR+ MBC (97.1%), while TRSP1/MMG was most effective in HR− (76.2%) and TN (71.1%) MBC. Marker expression did not correlate with metastatic site, except SOX10 in lung metastases (P = 0.031). Subtype discordance between MBC and PBC occurred in 43 cases (24.4%), with GATA3 expression in HR− MBC significantly linked to subtype discordance (P = 0.005). Conversely, SOX10 expression was significantly associated with subtype concordance in HR− and TNBC (P ≤ 0.003). Despite a higher expression of GATA3 in all HR− cases, TRSP1 outperformed GATA3 in detecting concordant HR− cases (64.0% versus 38.5%). TRPS1 and SOX10 were expressed in more than 50% of concordant TNBC cases.The expression of breast‐specific markers is mainly determined by the PBC subtype. Aims: Phyllodes tumours (PTs) of the breast are uncommon fibroepithelial neoplasms with the potential to recur and metastasise. Apart from histological grading, the expression of biological markers and its relationship with tumour behaviour have been topics of interest. The phosphatidylinositol 3‐kinase (PI3K)–AKT pathway regulates diverse biological functions, and is one of the most frequently deregulated pathways in cancers. Little is known of PI3K–AKT pathway alteration in PT. We aim to investigate the alterations in different component of AKT pathway in PTs. Methods and results: This study investigated the expression of four biological markers involved in this pathway (PTEN, INPP4B, PI3KCA and pAKT) in 134 PTs by the use of immunohistochemistry. According to an immunoscore incorporating staining intensity and proportion, low epithelial INPP4B expression (P = 0.045) was associated with recurrence. A trend of association was found for low epithelial PTEN expression with recurrence (P = 0.090). Interestingly, low epithelial INPP4B expression was also associated recurred tumours (P = 0.043). Stromal PI3KCA expression (P = 0.016) and pAKT expression (P = 0.006) were found to be correlated with increased histological grade, but an opposite trend was seen for stromal INPP4B expression (P = 0.018). In addition, epithelial and stromal PTEN expression, PI3KCA expression and pAKT expression showed strong correlations with each other (P ≤ 0.001). Conclusions: These findings indicate that alterations in AKT pathway activation may correlate with malignant transformation and recurrence in PT. Low epithelial INPP4B/PTEN expression is associated with shorter recurrence‐free survival. These observations suggest that the pathway may play a crucial role in the biological behaviour and progression of PT, and assessing the expression of this pathway may be of value in diagnosis, grading, prognostication, and management. A recurrent neural network (RNN) called CrackNet‐R is proposed in the article for fully automated pixel‐level crack detection on three‐dimensional (3D) asphalt pavement surfaces. In the article, a new recurrent unit, gated recurrent multilayer perceptron (GRMLP), is proposed to recursively update the internal memory of CrackNet‐R. Unlike the widely used long short‐term memory (LSTM) and gated recurrent unit (GRU), GRMLP is intended for deeper abstractions on the inputs and hidden states by conducting multilayer nonlinear transforms at gating units. CrackNet‐R implements a two‐phase sequence processing: sequence generation and sequence modeling. Sequence generation is specifically developed in the study to find the best local paths that are most likely to form crack patterns. Sequence modeling predicts timely probabilities of the input sequence being a crack pattern. In terms of sequence modeling, GRMLP slightly outperforms LSTM and GRU by using only one more nonlinear layer at each gate. In addition to sequence processing, an output layer is proposed to produce pixel probabilities based on timely probabilities predicted for sequences. The proposed output layer is critical for pixel‐perfect accuracy, as it accomplishes the transition from sequence‐level learning to pixel‐level learning. Using 3,000 diverse 3D images, the training of CrackNet‐R is completed through optimizing sequence modeling, sequence generation, and the output layer serially. The experiment using 500 testing pavement images shows that CrackNet‐R can achieve high Precision (88.89%), Recall (95.00%), and F‐measure (91.84%) simultaneously. A 67-year-old woman presented with postmenopausal vaginal bleeding. Full body imaging demonstrated an intrauterine mass with deep myometrial invasion but no nodal or other metastatic disease. Uterine curettage was performed. Histologically, the tumor was an endometrioid adenocarcinoma with sarcomatous element and a hepatoid component, the latter was immunohistochemically positive for alpha-fetoprotein, HepPar-1, and arginase-1. The patient underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy. Serum alpha-fetoprotein level decreased from 31896 ug/l preoperatively to 2063 ug/l postoperatively. Eight weeks later, a rise in serum alpha-fetoprotein was detected, and a biopsy-proven vaginal recurrence was diagnosed. Palliative chemotherapy led to tumor shrinkage and a concurrent decrease in the serum alpha-fetoprotein level. A rise in serum alpha-fetoprotein, refractory to second-line chemotherapy, was accompanied by subsequent development of ureteric obstruction, ascites, and radiological evidence of peritoneal metastases. This is an unusual case of uterine carcinosarcoma with an alpha-fetoprotein-producing hepatoid adenocarcinoma component. The CrackNet, an efficient architecture based on the Convolutional Neural Network (CNN), is proposed in this article for automated pavement crack detection on 3D asphalt surfaces with explicit objective of pixel-perfect accuracy. Unlike the commonly used CNN, CrackNet does not have any pooling layers which downsize the outputs of previous layers. CrackNet fundamentally ensures pixel-perfect accuracy using the newly developed technique of invariant image width and height through all layers. CrackNet consists of five layers and includes more than one million parameters that are trained in the learning process. The input data of the CrackNet are feature maps generated by the feature extractor using the proposed line filters with various orientations, widths, and lengths. The output of CrackNet is the set of predicted class scores for all pixels. The hidden layers of CrackNet are convolutional layers and fully connected layers. CrackNet is trained with 1,800 3D pavement images and is then demonstrated to be successful in detecting cracks under various conditions using another set of 200 3D pavement images. The experiment using the 200 testing 3D images showed that CrackNet can achieve high Precision (90.13%), Recall (87.63%) and F-measure (88.86%) simultaneously. Compared with recently developed crack detection methods based on traditional machine learning and imaging algorithms, the CrackNet significantly outperforms the traditional approaches in terms of F-measure. ABSTRACT In this article, a suspended stripline bandpass filter using hybrid transmission line stepped impedance resonator (HTLSIR) is proposed. SSL has been proven to be an excellent transmission-line system for realizing different types of filters. However, due to the effective permittivity being too close to unity, the size of the distributed elements is usually very large. The novel resonator introduced in this letter contains a section of hybrid transmission line which consists of the SSL and the microstrip line. The characteristics of the HTLSIR have been analyzed clearly. The novel SSL filter is designed, fabricated, and tested. A highly efficient and robust scheme is proposed for analyzing electromagnetic scattering from electrically large arbitrary shaped conductors in a half space. This scheme is based on the electric field integral equation (EFIE) with a half-space Green’s function. The precorrected fast Fourier transform (p-FFT) is first extended to a half space for general three-dimensional scattering problems. A novel enhanced dual threshold incomplete LU factorization (ILUT) is then constructed as an effective preconditioner to improve the convergence of the half-space EFIE. Inspired by the idea of the improved electric field integral operator (IEFIO), the geometrical-optics current/principle value term of the magnetic field integral equation is used as a physical perturbation to stabilize the traditional ILUT perconditioning matrix. The high accuracy of EFIE is maintained, yet good calculating efficiency comparable to the combined field integral equation (CFIE) can be achieved. Furthermore, this approach can be applied to arbitrary geometrical structures including open surfaces and requires no extra types of Sommerfeld integrals needed in the half-space CFIE. ABSTRACT A compact ultrawideband (UWB) planar bandpass filter (BPF) using quad-T-stub-loaded ring resonator is presented. At this filter, two sets of the four high impedance lines at the two sides of the ring as the coupling fed structure are used to gain much stronger coupling effect. Compared with the conventional ring resonator filter, the bandwidth of the UWB BPF can be improved after we load four T-shaped stubs on the ring resonator. That is because these stubs can increase the effective electrical lengths, and also lower the second and third resonant frequencies of the ring resonator at no expense of the size increase. Finally, a prototype of the proposed UWB BPF with 3 dB fractional bandwidth of 101% has been designed and fabricated. A planar single improved dual-mode ring resonator loaded by four mushroom-shaped stubs based on a simple feed scheme is presented for design of dual- and tri-band bandpass filters (BPFs). The simple pair of feed lines with source-load coupling is used to not only feed the ring resonator but also generate the higher desired third passband when the length of feed lines changes. That is, a desired dual- or tri-band BPF can be designed based on the same single-ring resonator. For demonstration purpose, two experimental example filters have been fabricated and implemented. A compact size on-board printed antenna using capacitive coupled-fed excitation to generate multiple resonant modes for pentaband WWAN operation (GSM850/900/GSM1800/1900/UMTS2100) is presented in this paper. The proposed antenna occupies only a small footprint of 15 × 25mm2 on one corner of the circuit board and a protruded ground of 10 × 15 mm2 is displaced with close proximity to the antenna portion. The proposed antenna has a very simple structure which is composed of two separate strips: a loop strip with an inserted chip inductor and an L-shaped feeding strip. The loop strip is shorted to the ground and generates a resonant mode at 890 MHz to cover the GSM850/900 band (824-960 MHz) while the feeding strip contributes to the GSM1800/1900/UMTS210 band (1710-2170 MHz) operation.With such a small size, the proposed antenna can achieve compact integration on the circuit board of the mobile phone, thus the proposed scheme is quite suitable for the slim mobile phone application. A novel fifth‐order suspended stripline (SSL) low‐loss bandpass filter with four transmission zeros is proposed. The technique of utilising only one cross‐coupling structure introduces four transmission zeros to provide high selectivity. By adjusting the parameters of the coupling open loop, the performance of out‐of‐band of the filter can be changed. A detailed discussion of this effect of the coupling open loop is given. The highest out‐of‐band rejection of the filter is more than 70 dB. An SSL bandpass filter is designed, fabricated and measured. The measured results are in good agreement with the simulated results. We report a termneonate with hypoxic ischemic encephalopathywho underwent a 72-hour therapeutic hypothermia. He developed unstable body temperature associated with coagulase negative staphylococcus septicemia 2 weeks later which was promptly treated with intravenous antibiotics and made a good recovery. PubMed (a service of the U.S. National Library ofMedicine) was searched for the terms "therapeutic hypothermia" and "septicemia," with limits activated (humans, English, age 0-18 years).There were only 6 randomized controlled trials, 1 non-randomized controlled trial, 1 retrospective cohort, and 1 case-control trial, which showed no definite evidence of increased risk of septicemia or neutrophil dysfunction in infants following hypothermia therapy.
