Objective: The FACE‐Q Skin Cancer Module is a Patient‐Reported Outcome Measure (PROM) utilized to assess outcomes following facial skin cancer resection. However, the lack of Minimal Important Difference (MID) estimates hinders the interpretability of the PROM scores. This study established MID estimates for the four outcome scales from the FACE‐Q Skin Cancer Module using distribution‐based methods. Methods: A prospective cohort study at four hospitals in the United States, enrolled participants who underwent Mohs Micrographic Surgery (MMS) for facial skin cancer between April 2020 and April 2022. Participants completed the Satisfaction with Facial Appearance, Appearance‐related Psychosocial Distress, Cancer Worry, and Appraisal of Scars scales at four time points: pre‐operatively, 2‐week, 6‐month, and 1‐year post‐surgery. Results: A total of 990 patients participated in the study, with completion rates of 98.4% for the pre‐operative assessment, 70.8% at 2 weeks, 59.3% at 6 months, and 60.4% at 1 year. MID estimates, calculated using 0.2 standard deviation and 0.2 standardized response mean, were determined for the four scales. The mean MID estimates, based on a Rasch transformed score ranging from 0 to 100, were 5 for the Appraisal of Scars scale and 4 for the remaining three scales. Conclusion: This multicenter study provides valuable MID estimates for the FACE‐Q Skin Cancer Module, specifically for the MMS patient population, enabling clinicians and researchers to better interpret scores, determine appropriate sample sizes, and apply the findings in clinical care. Mucous membrane pemphigoid is a rare autoimmune disease affecting mucosal surfaces. Pediatric cases are exceptionally rare, one subtype being vulvar pemphigoid. Juvenile vulvar pemphigoid can be challenging to diagnose due to its rarity and subtle initial symptoms. We present a case of an 8‐year‐old girl successfully diagnosed early in the disease course via histopathology, and immunofluorescence. Detecting MMP can be complex due to variations in epitope binding typically not included in commercial ELISA assays, necessitating comprehensive workup. Missed diagnosis may lead to progression to systemic involvement with severe consequences; thus, timely diagnosis and treatment are crucial. [ABSTRACT FROM AUTHOR]

Dupilumab is an interleukin‐4 receptor antagonist important in the treatment of refractory atopic dermatitis (AD), particularly among pediatric patients. Two boys with a history of AD and cardiac transplant who developed psoriasiform dermatitis in response to dupilumab therapy are reported. These patients paradoxically developed an immune‐mediated adverse drug reaction despite taking systemic immunosuppressive agents. While the literature suggests possible pathomechanisms for psoriasiform dermatitis despite immunosuppression, further research is necessary to better characterize this unique and unexpected phenomenon. Objective: Patients undergoing Mohs Micrographic Surgery (MMS) for facial non‐melanoma skin cancer (NMSC) experience appearance‐related psychosocial distress due to its post‐surgical esthetic changes. However, little is known about its development over a longer follow‐up period. This study prospectively assessed appearance‐related psychosocial distress in patients undergoing MMS for facial NMSC over a 1‐year follow up period. Methods: Patients who had MMS for facial NMSC between September 2020 and October 2021 were invited to answer the FACE‐Q Skin Cancer ‐ appearance‐related psychosocial distress scale preoperatively, 2 weeks, 6 months, and 1 year after surgery. Results: A total of 217 patients completed the questionnaire at baseline. In addition, 158 (72.8%), 139 (64.1%), and 120 (55.3%) questionnaires were successfully answered 2 weeks, 6 months, and 1 year after surgery, respectively. Patients with a peripheral lesion presented higher appearance‐related psychosocial distress scores at baseline than patients with a central lesion (p = 0.02). There was a decreasing trend in appearance‐related psychosocial distress over time, but without a significant result (baseline—2‐week; p = 0.73, 2‐week—6‐month; p = 0.80, 6‐month—1‐year; p = 0.17, baseline—1‐year; p = 0.23). Patients with secondary intention healing and graft reconstruction methods experienced more appearance‐related psychosocial distress over time than patients with primary wound closures (p = 0.03). Conclusions: Patients still experience appearance‐related psychosocial distress 1 year after MMS. These patients may benefit from targeted counseling. Additionally, predictors of more appearance‐related psychosocial distress, such as secondary intention healing and graft reconstruction methods, may benefit from additional psychological care. Annular erythema of infancy (AEI) is characterized by self‐limited eruptions of erythematous, annular to polycyclic patches and plaques, the etiology of which is thought to involve a hypersensitivity reaction to an unknown antigen. We present a case of AEI mistaken for systemic mastocytosis due to elevated serum tryptase. We were unable to find prior reports of an association between AEI and elevated tryptase in the literature. Learners may also need support processing emotional reactions to clinical encounters and supervisors' feedback about them;13 similarly, supervisors should reflect on how they respond to learners' feedback. Establishing psychological safety in clinical supervision: Multi-professional perspectives Psychological safety (PS) is the belief that individuals can engage in interpersonally risky behaviours, such as disagreeing, asking questions and reporting mistakes, without negative consequences.1 There is a growing recognition of the need to establish PS in health professions education to promote patient safety and quality improvement2 and to create an effective and inclusive learning environment.3-5 PS has been linked to positive outcomes in clinical learning, such as improved student ability to learn from errors,6 willingness to report adverse patient events7 and efforts to reduce mistakes.8 Psychological safety is the belief that individuals can engage in interpersonally risky behaviours ... without negative consequences. Depending on their experiences and beliefs, learners can also hold varied perspectives on health professions education and interpersonal risk.7 In light of the significant global events that occurred in 2020 and 2021, it is critical to recognise many learners may be experiencing greater psychological, social and/or economic stress. Objective: Nonmelanoma skin cancer (NMSC) is one of the most diagnosed cancers in the world, with the number of new occurrences rising every year. Most patients with facial skin cancer experience cancer‐related worry. Yet, little is known about their worry during the period after cancer treatment. This study aimed to assess the long‐term change of cancer worry after surgical treatment in patients with NMSC. Methods: Patients undergoing surgery for facial NMSC between December 2017 and March 2020 were asked to complete the FACE‐Q Skin Cancer—Cancer Worry scale before (baseline), 3‐month, and 1‐year post‐surgery. Results: A total of 151 patients completed the baseline and 3‐month, and 99 (65.6%) the 1‐year post‐operative survey. A significant decrease in cancer worry score was seen between baseline and 3‐month post‐surgery (p < 0.001). No difference was found between the 3‐month and 1‐year post‐surgery scores (p = 0.78). Less improvement in cancer worry was seen for patients who had one facial skin cancer in their medical history (p = 0.001) and patients who had a history of facial surgery (p < 0.001). Conclusion: Post‐surgery patients still experience cancer worry. Therefore, targeted counseling might be of value when coping with cancer‐related concerns. Metal halide perovskites (MHP) can be made more stable through the addition of small amounts of cesium. Despite the improvement, these multication absorbers still display strong environmental sensitivity to any combination of factors, including water, oxygen, bias, temperature, and light. Here, the relationship is elucidated between light absorption, charge carrier radiative recombination, and relative humidity (rH) for the Cs0.05FA0.79MA0.16Pb(I0.83Br0.17)3 composition, revealing partially reversible reductions in the extinction coefficient and fully reversible 25× enhancements in absolute light emission registered across the same humidity cycles up to 70% rH. With in situ excitation wavelength‐dependent measurements, irreversible changes are identified in the perovskite after a single cycle of humidity‐dependent photoluminescence (PL) performed with 450 nm excitation. Filicide is the purposeful killing of one or more children by a parent, step‐parent, or other parental figure. Revenge filicide is a poorly understood, rare form of filicide in which a parent murders their child to cause emotional harm to the child's other parent. This descriptive study presents an international case series consisting of 62 revenge filicide cases from nine countries. Perpetrators were about equally likely to be male or female. Over half of the sample had an active mental disorder of any type, most often a personality disorder. A history of domestic violence was common. The mean victim age was 6 years. They were typically killed by asphyxiation, firearms, or knives, and preschool age children were targeted most frequently. In half of the cases, more than one child was murdered. Post‐crime suicidal behavior was commonplace, and one‐third of the sample died by suicide. Four revenge filicide subtypes were identified: rejection, custody/visitation dispute, infidelity/jealousy, and argument/conflict. These categories may prove useful in future research and for helping to identify children at high risk of becoming filicide victims. All surviving offenders were criminally convicted, and in only one case was a mental health defense successful in lessening culpability. Trisomy 20 is a genetic abnormality in which individuals have an extra copy of chromosome 20. Complete trisomy 20 is rare and believed to be incompatible with life. A mosaic form of trisomy 20, in which only some cells or tissues contain the extra chromosome, is a relatively commonly encountered chromosomal abnormality found during prenatal testing, and c. 90% result in a normal phenotype. However, despite the absence of a consistent phenotype, certain findings have been reported across multiple cases of mosaic trisomy 20. These include an array of morphological findings, developmental delays, and learning disabilities. Beyond physical manifestations, a wide range of developmental and learning delays have also been reported. In this work, we provide an overview of the trisomy 20 literature and a case report of a young adult male with mosaic trisomy 20 who committed homicide. His developmental and life history, eventual diagnosis of mosaic trisomy 20, similarities and differences in his condition compared with prior research findings, and potentially new phenotypic findings associated with trisomy 20 that he manifested (childhood visual hallucinations, self-injury, polydactyly) are presented. Additionally, the potential role of this genetic diagnosis in his neuropsychiatric history and its successful application as a mitigating factor at his capital sentencing trial are described. We did not identify other similar cases during our search of major scientific and legal databases. As a backdrop, the use of genetics in criminal trials is on the rise, and courts are increasingly likely to accept behavioral genetics evidence; therefore, it is crucial that the legal system is well acquainted with the opportunities and limitations of these approaches. Summary: Objective: Stevens‐Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare and potentially fatal adverse skin reactions that are most commonly triggered by certain medications. One class of medications that has been highly associated with SJS/TEN reactions is antiepileptic drugs (AEDs). We sought to quantify the risk of SJS/TEN associated with AEDs as a class, as well as individual AEDs, in the United States. Methods: An analysis was performed of the US Food and Drug Administration Adverse Event Reporting System (FAERS) from July 2014 through December 2017. Rates of SJS/TEN were calculated for each AED compared with all other non‐AEDs. Reporting odds ratios (RORs), proportional reporting ratios (PRRs), and 95% confidence intervals (CIs) were calculated using OpenEpi. Results: With 198 reports, AEDs had more reports of SJS/TEN than any other medication class. AEDs as a class had an ROR of 8.7 (95% CI 7.5‐10.2) and a PRR of 8.7 (95% CI 7.5‐10.2) compared with all other non‐AEDs. The AEDs with the highest risk estimates were zonisamide (ROR 70.2, 95% CI 33.1‐148.7; PRR 68.7, 95% CI 32.9‐143.5), rufinamide (ROR 60.0, 95% CI 8.3‐433.5; PRR 58.9, 95% CI 8.4‐411.5), clorazepate (ROR 56.0, 95% CI 7.8‐404.1; PRR 55.1, 95% CI 7.8‐385.0), lamotrigine (ROR 53.0, 95% CI 43.2‐64.9; PRR 52.2, 95% CI 42.7‐63.7), phenytoin (ROR 26.3, 95% CI 15.5‐44.7; PRR 26.1, 95% CI 15.4‐44.2), and carbamazepine (ROR 24.5, 95% CI 16.0‐37.5; PRR 24.3, 95% CI 16.0‐37.1). Significance: Although AEDs as a class were associated with 9 times the risk of SJS/TEN compared with non‐AEDs, there were 6 AEDs with risk estimates greater than 20. Increased awareness of this risk among both prescribers and patients, particularly variations in risk among different AEDs, along with education on early recognition of SJS/TEN signs/symptoms, may help mitigate the number and severity of these adverse events. Since the onset of the COVID‐19 pandemic, the growing body of literature has largely focused on the adult population. Reported symptoms among children appear to be consistent with those in adults, including fever, respiratory symptoms, and gastrointestinal symptoms, though children may experience an overall milder disease course. Viral exanthems with possible association to COVID‐19 have been reported in pediatric patients. We describe a 10‐month‐old boy with Gianotti‐Crosti syndrome in the setting of recent SARS‐CoV‐2 RT‐PCR positive testing to increase physician awareness and add to the collection of cutaneous manifestations of COVID‐19. Early melanoma and non-melanoma skin cancer of the facial area are primarily treated with surgery. Little is known about the outcomes of treatment for facial skin cancer patients. The objective of the study was to identify concerns about aesthetics, procedures and health from the patients' perspective after facial skin surgery. Semi-structured in-depth interviews were conducted with 15 participants. Line-by-line coding was used to establish categories and develop themes. We identified five major themes on the impact of skin cancer surgery: appearance-related concerns; psychological (e.g., fear of new cancers or recurrence); social (e.g. impact on social activities and interaction); physical (e.g. pain and swelling) concerns and satisfaction with the experience of care (e.g., satisfaction with surgeon). The priority of participants was the removal of the facial skin cancer, as this reduced their overall worry. The aesthetic outcome was secondary but important, as it had important implications on the participants' social and psychological functioning. The participants' experience with the care provided by the surgeon and staff also contributed to their satisfaction with their treatment. Asian Americans ( AAs) are the second largest foreign-born population in the United States. Contrary to the 'model minority' stereotype that this group is unitarily well adjusted and high achieving, recent research has revealed substantial differences in mental health adjustment among AA children. Although research to identify the risk processes for mental health problems among AA children is underway, it has paid little attention to related asset and protective processes. This article selectively reviews the theory and empirical evidence on a set of child-, family-, and neighborhood-level characteristics for their potential asset or protective roles in AA children's mental health adjustment. These characteristics include (a) child factors (maintenance of heritage culture, bilingualism, coping, and emotion regulation), (b) family factors (authoritative parenting and parental support), and (c) a neighborhood factor (ethnic community). Overall, systematic efforts to identify asset and protective factors for AA children's mental health and understand the underlying developmental mechanisms are nascent. In this report, we seek to (i) identify a potential neuroimaging endophenotype for bipolar disorder (BD) in emotion regulatory and autonomic circuitry in young first-degree relatives of persons with BD; and (ii) replicate our previous work identifying the functional neuroanatomy of working memory (WM) in an older sample of relatives of persons with BD. Ten adolescent and young adult (age 13-24) unmedicated, non-ill, first-degree relatives of persons with BD (RELS) and 10 demographically comparable healthy controls performed a 2-back WM task and a 0-back control task during functional magnetic resonance imaging (fMRI). fMRI data were collected on a 1.5 Tesla scanner and analyzed using SPM-2. Mood was assessed on the day of scanning. The groups did not differ on any demographic, neuropsychological, or in-scanner task performance variables. In contrast to controls, RELS showed (i) weak task-dependent modulation activity in the cerebellar vermis (CV), insula, and amygdala/parahippocampal region, and (ii) exaggerated modulation of activity in the frontopolar cortex and brainstem, even after controlling for potential confounders. Many of the group differences were driven by differences in activity in the low-level (0-back) baseline task. Young, unmedicated RELS exhibited altered task-dependent modulation of frontopolar, CV, and insula activity during WM, especially during the low-level (0-back) baseline task. Results are largely consistent with our initial study of older adult RELS, suggesting these alterations may represent biomarkers of genetic risk for BD. [ABSTRACT FROM AUTHOR]
