Purpose: This study investigated the influence of cleanroom conditions on the discard rates of donor corneas in a German university eye bank. Methods: Discard rates were analysed from 2017 to 2020 at the LIONS Cornea Bank at Saarland University Medical Center. 1941 corneas from 971 donors were included. 1262 corneas (65.1%) were stored in a class D cleanroom from 2017 to 2019 and processed in a cleanroom class A sterile bank (group 1). 679 corneas (34.9%) were continuously stored in a class B cleanroom and processed in a class A cleanroom safety cabinet in the same room from 2019 to 2020 (group 2). The target parameter of this work was the number of contamination‐related discards. Although they cannot be influenced by the spatial conditions, the discards due to insufficient endothelial quality, serology, contraindications, scars and technical causes were also recorded. Statistical analysis was performed using SPSS and various testing procedures. Results: In group 1, significantly more corneas were discarded due to positive serology (6.9%|3.8%, p = 0.020). There was no significant change between both groups for either contamination or the other reasons for discard. Conclusion: Optimization of hygiene standards from cleanroom class D to B did not reduce contamination. The integration of research activities in universities of applied sciences (UASs) has led to the transformation of these universities into organisations with two primary processes: research and education. Although many believe in the benefits of combining research and education in one organisation, which is referred to as synergy in this study, research–education synergies have rarely been empirically investigated, particularly in the UAS context. Thus, this research investigates the intended synergy between the research and education of UASs by conducting a document analysis of their university‐wide strategic policy. The findings show that UASs aim for synergies among people, UAS organisations and outside UAS organisations, with a focus on education‐oriented synergies. This study provides an initial understanding of the strategic aims of UASs considering research–education synergy. Background: Priming with ChAdOx1 followed by heterologous boosting is considered in several countries. Nevertheless, analyses comparing the immunogenicity of heterologous booster to homologous primary vaccination regimens and natural infection are lacking. In this study, we aimed to conduct a comparative assessment of the immunogenicity between homologous primary vaccination regimens and heterologous prime‐boost vaccination using BNT162b2 or mRNA‐1273. Methods: We matched vaccinated naïve (VN) individuals (n = 673) with partial vaccination (n = 64), primary vaccination (n = 590), and primary series plus mRNA vaccine heterologous booster (n = 19) with unvaccinated naturally infected (NI) individuals with a documented primary SARS‐CoV‐2 infection (n = 206). We measured the levels of neutralizing total antibodies (NTAbs), total antibodies (TAbs), anti‐S‐RBD IgG, and anti‐S1 IgA titers. Results: Homologous primary vaccination with ChAdOx1 not only showed less potent NTAb, TAb, anti‐S‐RBD IgG, and anti‐S1 IgA immune responses compared to primary BNT162b2 or mRNA‐1273 vaccination regimens (p < 0.05) but also showed ~3‐fold less anti‐S1 IgA response compared to infection‐induced immunity (p < 0.001). Nevertheless, a heterologous booster led to an increase of ~12 times in the immune response when compared to two consecutive homologous ChAdOx1 immunizations. Furthermore, correlation analyses revealed that both anti‐S‐RBD IgG and anti‐S1 IgA significantly contributed to virus neutralization among NI individuals, particularly in symptomatic and pauci‐symptomatic individuals, whereas among VN individuals, anti‐S‐RBD IgG was the main contributor to virus neutralization. Conclusion: The results emphasize the potential benefit of using heterologous mRNA boosters to increase antibody levels and neutralizing capacity particularly in patients who received primary vaccination with ChAdOx1. To compare prevalence of positive PCR tests for herpesviruses between patients with and without a history of clinical corneal endothelial allograft rejection (AGR). Retrospective cross‐sectional study with two‐group comparison. A total of 307 aqueous humor (AH) samples from 235 Patients and 244 eyes who underwent penetrating keratoplasty or Descemet membrane endothelial keratoplasty or had a diagnostic AH aspiration due to clinical AGR between 2019 and 2023 were tested for DNA of herpes simplex virus (HSV), varicella‐zoster virus (VZV), cytomegalovirus (CMV), and Epstein‐Barr virus (EBV). PCR test results were compared between the two groups (with/without AGR). Another sub‐analysis examined the results of patients without a history of herpetic keratitis. A total of 8% of eyes with clinical AGR (9/108) had a positive PCR result for one of the herpesviruses (HSV:3, CMV:3, EBV:2, VZV:1). All patients in the group without AGR had negative PCR results for all previous viruses (0/136). The difference was statistically significant (p < 0.001). The sub‐analysis of eyes without a history of herpetic keratitis also revealed significantly more positive herpes PCR results (7/87) in eyes with AGR than in eyes without AGR (0/42, p = 0.005). Clinical AGR after keratoplasty shows a significant correlation to viral replication. Herpetic infection and AGR could occur simultaneously and act synergistically. Liquefied petroleum gas (LPG) storage fires and explosions occur due to uncontrolled gas leaks and the gradual breakdown of associated safety barriers. By installing an effective safety barrier, these accidents can be greatly reduced. However, this study assesses the probability of failure of emergency safety barriers (ESBs) to help decision makers understand how they can support decisions to reduce the risks associated with LPG storage. In this context, an extension of the event tree analysis is proposed named emergency event tree analysis (EETA). The aim of this paper is to develop an integrated approach that uses interval type‐2 fuzzy sets and Analytic Hierarchy Process (AHP) method and emergency event tree analysis to handle uncertainty in the failure probability assessment of emergency safety barriers (ESBs). In addition, a case study on the failure probability assessment of the emergency safety barriers of the LPG plant in Algeria based on the proposed methodology is provided and carried out to illustrate its effectiveness and feasibility. The results demonstrated the ability of interval type‐2 fuzzy sets and the AHP method to provide highly reliable results and to evaluate the failure probability of emergency safety barriers in emergencies situations. However, the classical event tree analysis (CETA) does not take into account the possibility of assessing the emergency consequences of different accident scenarios. Consequently, it only allows you to estimate the occurrence probability of accident scenarios. The results of this study show that the value of the probability of failure of the emergency safety barriers can be used to estimate the probability of occurrence of emergency consequences under different accident scenarios, improved the reliability and help prioritize emergency improvement measures. The study provides scientific and operational references for analyzing emergency consequences of the various accident scenarios in all fields such as petrochemical, maritime industry, and health occupational. Objectives: Self‐efficacy is important for adherence to transmission‐reducing behaviours (e.g., physical distancing) as also shown in the CHARIS project. We aimed to show that a theory‐based short message can increase physical distancing self‐efficacy and intentions to keep physical distance. Design: Structured telephone surveys with a randomly selected nationally representative sample of adults in Scotland (N = 497). Methods: Participants were randomly assigned to one of two experimental conditions: message condition (short message to increase self‐efficacy via vicarious experiences, verbal persuasion and emotional arousal) or control condition (no message). Followed by measures for self‐efficacy and intention for physical distancing on 4‐point scales. Adherence to physical distancing was assessed on a 5‐point frequency scale (never – always). Results: Using mediation analyses with bootstrapping procedures, we first confirmed that self‐efficacy was associated indirectly with adherence, via higher intentions in a partial mediation (unstandardized indirect effect.21, 95% CI.18–.25). The message increased self‐efficacy; participants receiving the message reported higher self‐efficacy (M = 4.23, SD =.80) compared to participants in the control condition (M = 4.08, SD =.77; standardized regression coefficient =.19, p <.05) and self‐efficacy affected intention (.48, p <.001). There was a small significant indirect effect of the message on intention via self‐efficacy (unstandardized indirect effect.07, CI.01–.14). Conclusions: Increasing self‐efficacy for physical distancing with a short message can successfully increase intention to physical distance via increased self‐efficacy. As both self‐efficacy and intentions are important predictors of adherence to transmission‐reducing behaviours short messages have potential to limit the spread of COVID‐19. [ABSTRACT FROM AUTHOR]

Galactosemia is an inborn disorder of carbohydrate metabolism of which early detection can prevent severe illness. Although the assay for galactose‐1‐phosphate uridyltransferase (GALT) enzyme activity has been available since the 1960s, many issues prevented it from becoming universal. In order to develop the Israeli newborn screening pilot algorithm for galactosemia, flow injection analysis tandem mass spectrometry measurement of galactose‐1‐phosphate in archived dried blood spots from newborns with classical galactosemia, galactosemia variants, epimerase deficiency, and normal controls, was conducted. Out of 431 330 newborns screened during the pilot study (30 months), two with classical galactosemia and four with epimerase deficiency were identified and confirmed. Five false positives and no false negatives were recorded. Following this pilot study, the Israeli final and routine newborn screening algorithm, as recommended by the Advisory Committee to the National Newborn Screening Program, now consists of galactose‐1‐phosphate measurement integrated into the routine tandem mass spectrometry panel as the first‐tier screening test, and GALT enzyme activity as the second‐tier performed to identify only newborns suspected to be at risk for classical galactosemia. Purpose: Descemet membrane endothelial keratoplasty (DMEK) accounts for >50% of all corneal transplants in Germany. So far, no data from such a large multicenter study have been published. Methods: This retrospective study included 3200 DMEKs at seven departments performed for Fuchs endothelial corneal dystrophy (FECD) or bullous keratopathy (BK). We evaluated best corrected visual acuity (BCVA, logMAR), endothelial cell density (ECD, cells/mm2), minimal corneal thickness (CT, μm), rebubbling‐, primary transplant failure‐ and immune reaction‐rate. Changes over time were evaluated by linear mixed models for repeated measures and correlation with case number by center by weighted linear regression. Results: For patients without vision‐limiting comorbidities (74% of all analysed eyes, n = 2270), mean BCVA improved from 0.6 ± 0.4 logMAR to 0.2 ± 0.2 logMAR 6 months (p < 0.001, n = 1441) and 0.1 ± 0.2 logMAR 12 months (p = 0.001, n = 1402) postoperatively. BK‐ had a worse BCVA compared to FECD‐patients (0.3 ± 0.5 vs. 0.1 ± 0.2 logMAR [p < 0.001] at 1 year). ECD declined from 2465 ± 259 cells/mm2 (n = 2876 preoperatively) to 1587 ± 433 cells/mm2 after 12 months (p < 0.001, n = 1237). Mean rebubbling rate was 0.4 ± 0.7/eye. 784 eyes (25%) received at least one rebubbling. More rebubblings correlated with a lower ECD, a worse BCVA, a higher CT, and higher transplant failure and rejection rates (p < 0.001, p = 0.013 for BCVA at 12 months). A single rebubbling did not influence the BCVA (p = 0.785). Graft failure rate was 3% (n = 67), rejection rate 1.5% (n = 48). Conclusion: Descemet membrane endothelial keratoplasty increases visual acuity with low transplant failure‐ and rejection‐rates. FECD has a better outcome than BK. While in principle exact, Kohn–Sham density functional theory—the workhorse of computational chemistry—must rely on approximations for the exchange–correlation functional. Despite staggering successes, present‐day approximations still struggle when the effects of electron–electron correlation play a prominent role. The limit in which the electronic Coulomb repulsion completely dominates the exchange–correlation functional offers a well‐defined mathematical framework that provides insight for new approximations able to deal with strong correlation. In particular, the mathematical structure of this limit, which is now well‐established thanks to its reformulation as an optimal transport problem, points to the use of very different ingredients (or features) with respect to the traditional ones used in present approximations. We focus on strategies to use these new ingredients to build approximations for computational chemistry and highlight future promising directions. Purpose: The aim of this study was to determine the prevalence and severity of cornea guttata (CG) in grafts after Descemet membrane endothelial keratoplasty (DMEK) and to investigate its impact on various clinical parameters during follow‐up. Methods: This retrospective study included 664 operations (DMEK and triple‐DMEK) on 466 patients. The prevalence and progression of CG after the operation were examined using endothelial specular microscopy images. The severity grade of CG was classified into four grades: G0 without CG, G1 – G3 with increasing severity of CG. Clinical parameters such as central corneal thickness (CCT), visual acuity (VA), endothelial cell density (ECD), pleomorphism and polymegalism were examined during a postoperative follow‐up time of 19.6 ± 15.8 months. Results: Cornea guttata (CG) appeared postoperatively in 124 (18.7%) eyes. 112 (16.9%) could be classified as G1, 9 (1.4%) as G2 and only 3 (0.5%) as G3. The examination of clinical parameters showed significant differences between healthy and low‐grade CG (G0/G1) and high‐grade CG (G2/G3). A significant deterioration was found in the corrected distance visual acuity (CDVA) (p = 0.02). CCT showed an increase between G0 (534 ± 58 μm) and G2 (549 ± 71 μm)/G3 (558 ± 56 μm) with a p‐value of 0.02. Additionally, a significant increase in pleomorphism (p = 0.003) and polymegalism (p = 0.04) was detected. Conclusion: Cornea guttata (CG) prevalence after DMEK and triple‐DMEK was found to be 18.7%, although most of these cases were classified as low‐grade CG and showed no clinical significance. Objectives: To examine the ability of four models of behaviour, namely, Protection Motivation Theory (PMT), the Common Sense Self-Regulation Model (CS-SRM), and Social Cognitive Theory and the Reasoned Action Approach (SCT and RAA) to understand adherence to transmission-reducing behaviours (TRBs) advised by national governments for suppression of SARS-CoV2.Design: A series of six cross-sectional telephone surveys of a random representative sample of adults living in Scotland.Methods: Self-reported adherence to three TRBs (physical distancing, wearing a face covering and handwashing), PMT, CS-SRM, and SCT/RAA constructs, and sociodemographic variables were measured each week for 6 weeks (n = ~500 p/w; third June-15th July) via a 15 min telephone survey.Results: Adherence was high ('Always' or 'Most times') throughout for physical distancing and handwashing, and, when mandated, for wearing a face covering. Older people were more adherent to all TRBs. Constructs from all three models predicted all three TRBs. Intention and self-efficacy (SCT/RAA) were the only beliefs to predict to all three TRBs each week and for all groups equally; intention was the strongest predictor. The predictive utility of PMT and CS-SRM varied by TRB and by group. Of note was the observation that several illness beliefs were associated with adherence only for those who believed they had not had COVID-19.Conclusions: The CHARIS project has identified beliefs about specific behaviours, the illness and the risks associated with lower adherence rates that might be addressed in national interventions. Background: This study assesses the reliability of successive corneal biomechanical response measurements by the Corneal Visualization Scheimpflug Technology (CST, Corvis ST®, Oculus Optikgeräte, Wetzlar, Germany) in different keratoconus (KC) stages. Methods: A total of 173 eyes (15 controls: 15 eyes, and 112 KC patients: stages 1|1–2|2|2–3|3|3–4|4, n = 26|16|36|18|31|26|5 according to Topographical KC Classification, TKC) were repeatedly examined five times with the CST, each after repositioning the patient's head and re‐adjusting the device. Tomographical analysis (Pentacam HR®; Oculus, Wetzlar, Germany) was performed once before and once after CST measurements. Outcome measures included (1) A1 velocity, (2) deformation amplitude (DA) ratio 2 mm, (3) integrated radius, (4) stiffness parameter A1 and (5) Ambrósio relational thickness to the horizontal profile (ARTh). The Corvis Biomechanical Index (CBI) is reported to be extracted out of these parameters. Mean values of the five measurements and Cronbach's α were calculated as a measure for reliability. Results: Ambrósio relational thickness to the horizontal profile and SPA1 were significantly higher in controls (534|123) compared to TKC1 (384|88), TKC2 (232|66), TKC3 (152|55) and TKC4 (71|27; p < 0.0001). The other parameters were similar in controls and TKC1 (A1 velocity: 0.148|0.151 m/s; integrated radius: 8.2|8.6 mm−1), but significantly higher in TKC stages 2 to 4 (DA ratio 2 mm: 5.5|6.3|8.0; A1 velocity: 0.173|0.174|0.186 m/second; integrated radius: 10.9|12.8|19.0 mm−1; p < 0.0001). All parameters proved to be highly reliable (Cronbach's α ≥ 0.834) and the corneal tomography remained unaffected. Conclusions: The individual parameters included in the CBI (consisting of ARTh, SPA1, DA ratio 2 mm, A1 velocity and integrated radius) are highly reliable but differ KC stage‐dependently. Background: "Classical" echocardiographic signs of Fabry cardiomyopathy (FC), such as left ventricular hypertrophy (LVH), posterolateral strain impairment (PLSI), and papillary muscle hypertrophy may be of limited diagnostic accuracy in clinical practice. Our aim was to evaluate the diagnostic value of left atrial (LA) strain impairment compared to "classical" echocardiographic findings to discriminate FC. Methods: In standard echocardiographic assessments, we retrospectively analyzed the diagnostic value of the "classical" red flags of FC as well as LA strain in 20 FC patients and in 20 subjects with other causes of LVH. Receiver operating characteristic (ROC) curve analysis was performed to assess the respective diagnostic accuracy. Results: FC was confirmed in 20 patients by genetic testing. In the LVH group, 12 patients were classified by biopsy to have hypertrophic cardiomyopathy, two had hypertensive heart disease, and six LVH combined with borderline myocarditis. Global and regional left ventricular (LV) strain was not significantly different between groups while LA strain was significantly impaired in FC (Left atrial reservoir strain (LASr) 19.1%±8.4 in FC and 25.6%±8.9 in LVH, p = 0.009; left atrial conduction strain (LAScd) ‐8.4%±4.9 in FC and ‐15.9%±8.4 in LVH, p < 0.01). LAScd, with an area under the curve (AUC) of.81 (95% confidence interval [CI].66–.96) showed the highest diagnostic accuracy to discriminate FC. The PLSI pattern showed an AUC of.49, quantification of papillary muscle hypertrophy an AUC of.47. Urea cycle disorders (UCDs), including OTC deficiency (OTCD), are life‐threatening diseases with a broad clinical spectrum. Early diagnosis and initiation of treatment based on a newborn screening (NBS) test for OTCD with high specificity and sensitivity may contribute to reduction of the significant complications and high mortality. The efficacy of incorporating orotic acid determination into routine NBS was evaluated. Combined measurement of orotic acid and citrulline in archived dried blood spots from newborns with urea cycle disorders and normal controls was used to develop an algorithm for routine NBS for OTCD in Israel. Clinical information and genetic confirmation results were obtained from the follow‐up care providers. About 1147986 newborns underwent routine NBS including orotic acid determination, 25 of whom were ultimately diagnosed with a UCD. Of 11 newborns with OTCD, orotate was elevated in seven but normal in two males with early‐onset and two males with late‐onset disease. Orotate was also elevated in archived dried blood spots of all seven retrospectively tested historical OTCD patients, only three of whom had originally been identified by NBS with low citrulline and elevated glutamine. Among the other UCDs emerge, three CPS1D cases and additional three retrospective CPS1D cases otherwise reported as a very rare condition. Combined levels of orotic acid and citrulline in routine NBS can enhance the detection of UCD, especially increasing the screening sensitivity for OTCD and differentiate it from CPS1D. Our data and the negligible extra cost for orotic acid determination might contribute to the discussion on screening for proximal UCDs in routine NBS. [ABSTRACT FROM AUTHOR]

Aim: COVID‐19 is associated with an exacerbated inflammatory response that can result in fatal outcomes. Systemic inflammation is also a main characteristic of periodontitis. Therefore, we investigated the association of periodontitis with COVID‐19 complications. Materials and Methods: A case–control study was performed using the national electronic health records of the State of Qatar between February and July 2020. Cases were defined as patients who suffered COVID‐19 complications (death, ICU admissions or assisted ventilation), and controls were COVID‐19 patients discharged without major complications. Periodontal conditions were assessed using dental radiographs from the same database. Associations between periodontitis and COVID 19 complications were analysed using logistic regression models adjusted for demographic, medical and behaviour factors. Results: In total, 568 patients were included. After adjusting for potential confounders, periodontitis was associated with COVID‐19 complication including death (OR = 8.81, 95% CI 1.00–77.7), ICU admission (OR = 3.54, 95% CI 1.39–9.05) and need for assisted ventilation (OR = 4.57, 95% CI 1.19–17.4). Similarly, blood levels of white blood cells, D‐dimer and C Reactive Protein were significantly higher in COVID‐19 patients with periodontitis. Conclusion: Periodontitis was associated with higher risk of ICU admission, need for assisted ventilation and death of COVID‐19 patients, and with increased blood levels of biomarkers linked to worse disease outcomes. Recognition systems using multimodal biometrics attracts attention because they improve recognition efficiency and high‐security level compared to the unimodal biometrics system. In this study, the authors present a secure multimodal biometrics recognition system based on the deep learning method that uses convolutional neural networks (CNNs). The authors propose two multimodal architectures using the finger knuckle print (FKP) and the finger vein (FV) biometrics with different levels of fusion: the features level fusion and scores level fusion. The features extraction for FKP and FV are performed using transfer learning CNN architectures: AlexNet, VGG16, and ResNet50. The key step aims to select separate features descriptors from each unimodal biometrics modality. After that, the authors combine them using the proposed fusion approaches were support vector machine or Softmax applies as classifiers to increase the proposed system security. The efficiency of the proposed algorithms is tested using publicly available biometrics databases. The experimental results show that the proposed fusion architectures achieve an accuracy of 99.89% and an equal error rate of 0.05%. Background: Corneal confocal microscopy (CCM) allows visualizing slightest alterations within the corneal subbasal nerve plexus (SNP). Recent CCM studies based on the analysis of three–five CCM images per eye assumed a reduced corneal nerve fibre length (CNFL) in keratoconus (KC). Methods: The SNP of KC patients (n = 23, 13 contact lens wearing, 10 noncontact lens wearing) and patients without KC (n = 16) was examined by 10 CCM images of one eye per patient. The CNFL per frame area was calculated, and the SNP tortuosity was quantified by measuring (a) the amplitude of the curves and (b) the area under the curve (AUC) formed by the SNP. Results: Analysing 390 non‐overlapping confocal images revealed the CNFL (mm/mm2) to be significantly lower in KC (16.4 ± 1.9 mm/mm2) than in healthy corneae (23.8 ± 3.3 mm/mm2, p < 0.0001; mean ± SD; p‐value calculated using the Mann–Whitney U‐test), without a difference between contact lens wearing and noncontact lens wearing KC patients (p = 0.4). Amplitudes and AUCs analysed as median with 25th and 75th percentile were significantly increased in KC (amplitude 33/23/41 µm and AUC 2839/1545/3444 µm2) compared to healthy corneae (amplitude 24/18/28 µm and AUC 1870/1193/2327 µm2, p < 0.0001). Conclusions: Corneal confocal microscopy (CCM) visualizes slightest alterations within the SNP in KC including (a) a significantly lower CNFL and (b) an enhanced winding course of the SNP. The significantly lower CNFL observed in KC may support the hypothesis of a neurodegenerative aspect of the disease and might be a measure to be correlated with the severity and progression of the disease. This clinical investigation compared the clinical performance of two marketed ophthalmic viscoelastic devices (OVDs): the bacterially derived Healon PRO OVD (test) and the animal-derived Healon OVD (control) under normal use conditions during cataract removal and lens implantation. This prospective, multicenter, randomized, parallel, participant/evaluator masked, postmarket investigation enrolled 139 subjects (170 eyes), 116 (143 eyes) of which were treated (73 test; 70 control group). Both test and control OVDs were used, at a minimum, to inflate the anterior chamber and protect the endothelium prior to cataract extraction according to the standard procedure. The surgeon completed a postsurgery OVD clinical performance questionnaire, and intraocular pressure (IOP) was measured before surgery and at the 1 day postoperative visit with Goldmann applanation tonometry. Any IOP measurement of 30 mmHg or higher was considered a "spike" and recorded as a study-specific, serious adverse event. The bacterially derived Healon PRO OVD was found to be statistically noninferior to the overall clinical performance of the animal-derived Healon OVD control; thus, the primary hypothesis was satisfied. There were no statistically significant differences between OVD groups for any of the additional endpoints relating to IOP changes or to safety, thus satisfying additional hypotheses. The Healon PRO OVD showed statistically significant improvements in surgeon ratings for ease of injectability, transparency/visibility, and ease of IOL placement. The safety profile was also similar between OVD groups with regards to serious and/or device-related adverse events, as well as medical and lens findings. The results of this clinical investigation support the safety and effectiveness of the bacterially derived, currently marketed Healon PRO OVD and indicate that the intraocular surgical performance was similar between the two OVDs. Purpose: Any corneal microsurgeon may have inadvertently transplanted a donor corneal tissue with (subclinical) keratoconus or prior keratorefractive surgery, which typically leads to postoperative refractive surprises and cannot always be detected in the eye bank with only slit lamp. Given the increased number of keratorefractive procedures performed in the last decades, eye banks will soon have to face this problem more intensively. Thus, the efficiency and reliability of anterior segment optical coherence tomography (AS‐OCT) as a non‐invasive and sterile screening method in the eye bank to detect corneal grafts with curvature and/or thickness abnormalities was assessed. Methods: 1315 donor corneal tissues mounted in sterile tissue cultivation flasks were imaged using an AS‐OCT. The corneal tissues were preserved in organ culture medium I and transferred into organ culture medium II (containing dextran T‐500 6%) for deswelling at least 24 h prior to measurement and keratoplasty. Depth raster scans were performed through the posterior surface of the corneal tissues within a 7 mm central zone to create 3D volume data. The volume data set was preprocessed to eliminate flask reflections artefacts and to define the region of interest (ROI). Subsequently, the adaptation of a sphero‐cylindrical surface model was carried out with raytracing. Thus, the radii of curvature for the front and back surfaces can be determined and a corneal thickness map is generated. Results: The mean steep/flat front surface radius was 7.46 ± 0.29 (5.07–9.29)/7.69 ± 0.24 (6.70–9.50) mm, the corresponding values for the back surface being 6.47 ± 0.32 (5.00–8.00)/6.80 ± 0.31 (5.81–8.00) mm and the mean central thickness was 610 ± 84 (379–1457) μm. Abnormalities (beyond ±2 or ± 3 SD) were found in 140 or 34 corneas (10.6% or 2.6%) for anterior surface curvature, 120 or 38 corneas (9.1% or 2.9%) for the posterior surface, and 55 or 16 corneas (4.2% or 1.2%) for thickness. Conclusions: The AS‐OCT provides an objective, sterile and semi‐automated screening method to identify corneal morphological and refractive alterations (e.g. keratoconus, status post PRK/LASIK) and further optimize corneal donor selection in the eye bank. Cancer cachexia is a complex disorder that is driven by inflammation and metabolic imbalances, resulting in extreme weight loss. Adipose tissue, a main player in cancer cachexia, is an essential metabolic and secretory organ consisting of both white adipose tissue (WAT) and brown adipose tissue. Its secretory products, including adipokines and cytokines, affect a wide variety of central and peripheral organs, such as the skeletal muscle, brain, pancreas, and liver. Therefore, a combination of metabolic alterations, and systemic inflammation dysregulation of both anti‐inflammatory and proinflammatory modulators contribute toward adipose tissue wasting in cancer cachexia. Growing evidence suggests that, during cancer cachexia, WAT undergoes a browning process, resulting in increased lipid mobilization and energy expenditure. In this review, we have summarized the characteristics of cancer cachexia and WAT browning. Furthermore, this review describes how adipose tissue becomes inflamed in cancer, shedding light on the combinatorial action of multiple secreted macromolecules, cytokines, hormones, and tumor mediators on adipose tissue dysfunction. We also highlight the inflammatory responses, energy utilization defects, and molecular mechanisms underlying the WAT dysfunction and browning in cancer cachexia. Further, the actual mechanisms behind the loss of adipose tissue are unknown, but have been attributed to increased adipocyte lipolysis, systemic inflammation, and apoptosis or reduced lipogenesis. The understanding of adipose tissue dysfunction in cancer cachexia will hopefully promote the development of new therapeutic approaches to prevent or treat this wasting syndrome. Background Neutrophil gelatinase-associated lipocalin (NGAL) is an inflammatory protein with gaining increasing interest for its use as marker in blood and cerebrospinal fluid (CSF) for several chronic diseases. Its biochemical properties make it an attractive marker. However, changes in blood and CSF NGAL concentrations during the diurnal rhythm in the elderly are unknown. This information is important for its optimal use as marker in studies with older people. Methods Serial paired plasma and CSF samples were obtained from 8 healthy elderly males over a 30-hour period. NGAL and cortisol were quantified with ELISA. Results No significant changes in plasma and CSF NGAL concentrations over time were found, whereas cortisol (included as internal control) concentrations displayed significant changes over time. Significant circadian patterns were found for plasma NGAL and for cortisol in both plasma and CSF. However, CSF NGAL concentrations did not follow a diurnal pattern in elderly males. Conclusions This study illustrates the temporal regulation of NGAL in plasma and CSF, which potentially is a useful reference for studies measuring NGAL as biomarker in older individuals. Direct alcohol fuel cells are a promising source of future energy generation for small and portable devices. Platinum is considered the best catalyst for electro-oxidation of alcohols in fuel cells but the major hurdles with platinum catalysts are high cost of platinum as well as low selectivity, slow reaction kinetics and carbonaceous poisoning associated with platinum. This particular research reports electro-oxidation of methanol and ethanol over platinum electrodeposited on ZSM-5 without any carbon additive. From the cyclic voltammetry and chronoamperometry, linear sweep voltammetry, tafel plot and multiple scan analyses, it is found that Pt/ZSM-5/C electrodes can catalyze electro-oxidation of methanol and ethanol with higher efficiency. ZSM-5 supports the formation of smaller sized platinum nanoparticles in a dispersed manner on the zeolite support and thus increases the active surface area of the metal for catalytic activity. It favors adsorption of alcohol molecules on the modified electrode surface and thus increases the diffusion process. It also stabilizes the modified electrodes. [ABSTRACT FROM AUTHOR]

Background The BSP090 project aims at establishing European Pharmacopoeia Reference Substances in combination with the corresponding ELISA methods for the quantification of major allergens in allergen products. Two sandwich ELISAs proved suitable for quantification of Bet v 1, the major birch pollen allergen, in preceding phases of BSP090. Methods Two Bet v 1-specific ELISA systems were compared with respect to accuracy and precision in a ring trial including 13 laboratories. Model samples containing recombinant rBet v 1.0101 as well as native birch pollen extracts were measured independently at least three times in each facility. The assessment was completed with a comparative quantification of Bet v 1 in 30 marketed birch allergen products in one laboratory, simulating the future use as reference method. Results In the collaborative study, both candidate ELISAs confirmed their suitability to quantify recombinant and native Bet v 1. ELISA-A showed higher precision and lower interlaboratory variability, yet ELISA-B exhibited slightly higher accuracy. Subsequent parallel measurement of Bet v 1 in a panel of 'real-life' birch allergen products indicated better repeatability of ELISA-B. Both systems detected substantial differences in Bet v 1 content between allergen products, but the effect was more pronounced using ELISA-B due to persistently higher values compared to ELISA-A. Conclusions In the collaborative study, no deciding differences were observed between the two candidate ELISAs. Further comparison under conditions simulating the intended use combined with the criterion of long-term availability enabled the selection of one Bet v 1-specific ELISA for proposal as European Pharmacopoeia standard method. Mycotic keratitis is a rare but severe ophthalmological illness, which has to be taken very seriously with a potentially sight‐threatening outcome. Over the last two decades, an increase of cases with mycotic keratitis has been noticed, which has possibly caused an increased use of soft contact lenses. Therapy is often lengthy and difficult. In a lot of cases, surgical therapy, i.e. corneal transplantation is indicated. For this reason, it is essential to discuss during the clarification process the necessity of a keratoplasty during the course of treatment. In this talk, we would like to give an overview on the typical clinical signs, symptoms, diagnostics and therapy options as well as confocal microscopy for diagnosis of mycotic keratitis. ICRS are polymethylmethacrylate (PMMA) implants developed to be inserted in the corneal stroma aiming to remodel the ectatic corneal curvature. In 2000, ICRS have been introduced for the implantation in KC treatment. Since then, several authors have demonstrated the efficacy of this surgical procedure for improving the refractive and topographic results of KC patients. Moreover, the method has been extended to a larger spectrum of corneal ectasia, such as pellucid marginal degeneration (PMD) and iatrogenic ectasia after laser in situ keratomileusis (LASIK). Our aim was to evaluate the functional results, i.e. how uncorrected and corrected distance visual acuity can be improved by implantation of ICRS. Progression of ectasia seems to be retarded. However, consistent follow‐up visits at close intervals are necessary to identify complications at an early stage. Background: Previous studies have reported significant differences in corneal biomechanics between keratoconic and healthy corneas. Corvis ST® (CST – OCULUS, Wetzlar, Germany) and Ocular Response Analyser® (ORA – Reichelt Inc., Depew, NY, USA) are two single central air‐puff tonometers and are commonly used to analyse the corneal biomechanics. Implantation of intracorneal ring segments (ICRS) using a femtosecond laser represents a reliable surgical option to widen the spectrum of the stage‐related therapy of keratoconus for selected patients. Our purpose was to analyse the change in corneal biomechanical properties after ICRS implantation in keratectasia. Methods: This retrospective monocentric study included 118 patient eyes that underwent femtosecond laser‐assisted ICRS implantation (INTACS SK®, Addition Technology Inc., Des Plaines, IL, USA) for keratectasia. Biomechanical analysis was performed using both ORA, with determination of Corneal resistance factor (CRF), Corneal hysteresis (CH) and Keratoconus Match Index (KMI), as well as by CST with determination of stiffness parameter A1 (SP‐A1), Ambrosio's related thickness to the horizontal profile (Arth), integrated radius (IR), deformation amplitude‐ratio (DA‐ratio) as well as Corvis Biomechanical Index (CBI) and Tomographical Biomechanical Index (TBI). Data collection was performed preoperatively and after 6 months postoperatively for ORA and CST and additionally after 1 and 2 years for ORA. Results: The CRF decreased significantly postoperatively (5.8 ± 1.7 mmHg) compared to preoperatively (6.8 ± 3.7 mmHg; p < 0.001), and increased again during follow‐up (6.2 ± 1.9 mmHg; p = 0.001), without regaining preoperative values. CH and KMI did not change significantly. SP‐A1, IR and DA‐ratio increased significantly after ICRS implantation (p < 0.001). Arth decreased significantly from 190 ± 88 to 157 ± 108. However, there was no significant postoperative change for CBI and TBI. Conclusions: Corneal biomechanical properties showed inconsistent changes after ICRS implantation. Statement of Problem. Low scientific evidence is identified in the literature for combining implant placement in fresh extraction sockets with immediate function. Moreover, the few studies available on immediate implants in postextraction sites supporting immediate full-arch rehabilitation clearly lack comprehensive protocols. Purpose. The purpose of this study is to report outcomes of a comprehensive protocol using CAD-CAM technology for surgical planning and fabrication of a surgical template and to demonstrate that immediate function can be easily performed with immediate implants in postextraction sites supporting full-arch rehabilitation. Material and Methods. 14 subjects were consecutively rehabilitated (13 maxillae and 1 mandible) with 99 implants supporting full-arch fixed prostheses followed between 6 and 24 months (mean of 16 months). Outcome measures were prosthesis and implant success, biologic and prosthetic complications, pain, oedema evaluation, and radiographic marginal bone levels at surgery and then at 6, 12, 18, and 24 months. Data were analyzed with descriptive statistics. Results. The overall cumulative implant survival rate at mean follow-up time of 16 months was 97.97%. The average marginal bone loss was 0,9 mm. Conclusions. Within the limitations of this study, the results validate this treatment modality for full-arch rehabilitations with predictable outcomes and high survival rate after 2 years. Dopamine D2 receptor agonists represent a first line treatment option in young patients with signs and symptoms of idiopathic Parkinson's disease. An association between the use of D2 receptor agonists in Parkinson's disease patients and heart failure has been reported. The identification of the underlying mechanism is needed to minimize the resultant cardiovascular morbidity. In a phase I clinical trial, a D2 receptor agonist (pramipexole) was administered to 52 healthy male subjects following a dose escalation scheme. Serialmeasurements of resting blood pressure, heart rate, and derived parameters including pulse pressure, pulsatile stress, and rate pressure product were analysed. Statistically significant and clinically relevant increases inmost of the assessed parameters were found. Ten subjects were removed prematurely from the trial because of clinically significant increases in blood pressure and/or heart rate requiring immediate intervention with IV rescue medications including a selective β-1 blocker. The observed drug-related changes in vital signs were of clinical relevance and might explain some of the cardiovascular morbidity reported in patients receiving D2 receptor agonist in clinical settings. We suggest that the additional use of a β-1 blocking agent might mitigate the risk of cardiovascular morbidity among patients receiving long-term D2 receptor agonists. [ABSTRACT FROM AUTHOR]

ABSTRACT An early clinical development study (phase I) was conducted to determine the usefulness of dried blood spot (DBS) sampling as an alternative to venous sampling for phenotyping and genotyping of CYP450 enzymes in healthy volunteers. Midazolam (MDZ) was used as a substrate for phenotyping CYP3A4 activity; the concentrations of MDZ and its main metabolite 1′-hydroxymidazolam (1-OH MDZ) were compared between the DBS method from finger punctures, plasma and whole blood (WB), drawn by venipuncture, whereby several methodological parameters were studied (i.e. punch width, amount of dots analyzed and storage time stability). Genotyping between DBS and venous WB samples was compared for CYP2D6 (*3, *4, *6), CYP2C19 (*2, *3), CYP3A4 (*1B) and CYP3A5 (*3C). In addition, the subject's and phlebotomist's satisfaction with venous blood sampling compared with the DBS method was evaluated using a standardized questionnaire. An LC-MS/MS method for the quantification of the MDZ and 1-OH MDZ concentrations in DBS samples was developed and validated in the range of 0.100-100 ng/mL. R4850 --> 756.1 --> Among females in the United States, Black females suffer from the highest rates of hypertension, stroke, coronary artery disease, heart failure, and total cardiovascular disease (CVD) mortality. Importantly, vascular dysfunction precedes overt CVD and predicts CVD risk. In this regard, a recent study reported that upper limb microvascular but not macrovascular function was blunted in Black relative to White women. However, this is not always a consistent finding and such investigations have not incorporated psychosocial measures, which are known to contribute to race/ethnicity‐related disparities in cardiovascular health. Given the strong relationship between psychosocial stress and health, Woods‐Giscombe, et al. recently developed the Giscombe Superwoman Schema Questionnaire (G‐SWS‐Q), which operationalizes the Superwoman Schema (SWS) Conceptual Framework to quantify psychological stress processes specifically associated with being a Black woman. Accordingly, we tested the hypothesis that Black females would have blunted post‐occlusive upper limb microvascular and macrovascular vasodilation relative to White females and that greater SWS endorsement by Black females would be associated with blunted vascular responses. METHODS: Seven Black (age: 22 ± 1 yr; BMI: 27.5 ± 1.6 kg/m2) and 6 White (age: 23 ± 1 yr; BMI: 25.9 ± 1.3 kg/m2) females participated. Post‐occlusive brachial artery flow‐mediated dilation (FMD) and forearm reactive hyperemia were assessed via well‐established procedures. Briefly, brachial artery diameter and blood velocity were measured continuously and recorded via Doppler ultrasound and a video capture system throughout a 2 min baseline, 5 min suprasystolic forearm cuff occlusion, and 3 min recovery. Offline analysis was performed via a continuous edge detection and blood velocity tracking software. Black participants also completed the G‐SWS‐Q, which is a 35‐item instrument designed to measure the cognitive, affective, and behavioral underpinnings associated with being a "Strong Black Woman" in the United States. RESULTS: Brachial artery FMD was lower in Black relative to White females (3.99 ± 1.15 vs. 7.34 ± 1.10%; p = 0.03); however, after correcting for post‐occlusive shear rate AUC via ANCOVA, group differences in FMD were less apparent (p = 0.07). Post‐occlusive peak blood velocity and blood flow AUC were not different between groups (p = 0.43 and p = 0.42, respectively). A moderate negative correlation was observed between overall SWS and FMD (r = ‐0.58, p = 0.09). Among the SWS subscales, a strong negative correlation was observed between "Obligation to Present an Image of Strength" and FMD (r = ‐0.70, p = 0.04), whereas "Intense Motivation to Succeed" was moderately negatively correlated with FMD (r = ‐0.64, p = 0.06). CONCLUSION: These preliminary data indicate that 1) upper limb macrovascular but not microvascular function may be blunted in young Black females and 2) stronger SWS endorsement may be associated with blunted vascular reactivity. Statistical assessment of water quality data, obtained at the South Florida Water Management District (West Palm Beach, Florida) S-65E structure, illustrated significant differences and inconsistency in nutrient concentrations (Daas and Ebadian, 2003). The evaluation showed that currently used water sampling techniques are inadequate because of deficiencies in functionality and sensitivity to flow conditions at the structure. Moreover, the study indicated the need for a more precise sampling technology, which would consider variations in the velocity profile across the water column during the sampling process. Therefore, a new prototype sampling technology was designed and tested in a pilot open channel. The new sampler had the capability to collect discrete aliquot volumes of water samples that are proportional to the corresponding point velocity at each of these depths. The purpose of this study is to statistically evaluate the influence of various sampling methodologies and flow conditions on the quality and comparability of total phosphorus concentration data collected over the course of 27 months at the South Florida Water Management District (SFWMD) S-65E structure. The data was obtained from the following sampling methodologies: U.S. Geological Survey (USGS) (Reston, Virginia) equal width increment spatially composite grab samples, USGS replicate samples, SFWMD grab samples, and SFWMD autosampler samples. Both significant and insignificant differences were reported from these comparisons. Parametric and nonparametric standard statistical tests for significance were carried out to evaluate the differences between the data. To avoid invalid conclusions of insignificant differences, we conducted tests for equivalence of the means and variances. Background and Objectives The aim of this study was to replace the 1st International Standard (IS) for prekallikrein activator (PKA) (code 82/530) with a new IS and European Biological Reference Preparation (BRP). The new standards were freeze dried 20% albumin solution containing PKA, the same solution that is tested using these standards. Aspects of the methodology for PKA determination were also examined as part of this study. Materials and Methods A batch of 20% albumin containing ≈ 30 IU/ml was donated by a manufacturer of blood products and dispensed into ampoules at the National Institute for Biological Standards and Control (NIBSC) to create the candidate 15 (02/168, sample B in the study) and at the European Directorate for the Quality of Medicines (EDQM) to create the candidate BRP (sample C in the study). The concentration of PKA in these preparations was determined in an international collaborative study involving 31 laboratories from 17 countries worldwide in comparison with the 1st 15 for PKA (82/53 0) containing 85 IU of PKA per ampoule. Participants were requested to perform their own in-house method, based on the current Ph. Eur. monograph for determination of PKA in albumin solutions. Participants were provided with sufficient samples to perform two or three assays and were asked to use their local prekallikrein substrate (PKS) and also to use a commercial PKS provided as part of the study, in order to investigate the importance of the source of PKS on the final potency values. Results Samples B and C emerged with identical PKA concentrations of 29 IU/ml, very close to the expected value. This figure was determined using a variety of statistical methods, with the participants' own calculated values and values calculated centrally at the EDQM using raw data. The value of 29 IU/ml was consistent and independent of the method of calculation, although interlaboratory variability was more sensitive to the statistical analysis method. There was no statistically significant difference in mean potencies when comparing results with the laboratories' own local substrate and the substrate provided for the study. All stability studies indicate that these lyophilized preparations of PKA in 20% albumin are extremely stable. Conclusions Samples B and C were established as the 2nd 15 (code 02/168) and PKA activator in albumin BRP batch 1 (Y0000263), respectively, with a potency of 29 IU per ampoule. Results from this study indicate that testing for PKA in albumin may be less sensitive to the source of PKS than previously feared. Allogeneic stem cell transplant (ASCT) recipients have severely impaired cell-mediated immunity as a result of their conditioning regimen, immunosuppressive therapy, and graft-versus-host disease (GVHD). Accordingly, they are susceptible to bacterial, viral, and fungal infections. Mycobacterial infections can also occur in these patients, although the incidence is not high, even in countries where tuberculosis (TB) is common. We describe four patients from our hospital who developed pulmonary T tuberculous infection in the post-transplant period over a 3-year period. During that time a total of 127 patients have undergone an ASCT, representing an incidence of TB of 2.3%. The pretransplant diagnosis was acute myeloid leukemia in three patients and chronic myeloid leukemia in one case. All four patients were treated with a combination of cyclosporine and corticosteroids for acute and/or chronic GVHD. Three of the four patients were born outside Australia, each from an area where TB is endemic. Two patients died within 2 weeks of the commencement of antituberculous therapy, the third is alive and well, and the fourth died of multi-organ failure and sepsis after 4 months in hospital. A higher index of suspicion of previous TB exposure and infection is required in the assessment of ASCT recipients, particularly in those born in areas where TB is common or endemic. Purpose: Studies using Corvis ST (CST, Oculus; Wetzlar, Germany) and Ocular Response Analyser (ORA, Reichert, Depew, NY, USA) have reported significant differences between keratoconic and healthy corneas. Implantation of intracorneal ring segments (ICRS) using a femtosecond laser represents a reliable surgical option to widen the spectrum of the stage‐related therapy of keratoconus in patients with contact lens intolerance, post‐LASIK‐keratectasia or pellucid marginal degeneration (PMD). Our aim was to evaluate the ability of the Corvis ST and Ocular Response Analyser to detect the possible stiffening effect after Intacs SK (Addition Technology, Inc.) implantation in patients with keratoconus. Patients and Methods: Between January 2018 and January 2019, ICRS were implanted in 31 eyes of 27 patients (age 31.9 ± 10.2 years) with clear central cornea. The patients had to fulfill the corneal diagnostic tests required for femtosecond laser‐assisted Intacs SK implantation in 80% depth. The following CST/ORA parameters were recorded: deformation amplitude Ratio (DA Ratio), stiffness parameter at applanation 1 (SP A1), Corvis biomechanical index (CBI), tomographic biomechanical index (TBI), Keratoconus Match Index (KMI), corneal hysteresis (CH) and corneal resistance factor (CRF) preoperatively and 6 months after the ICRS implantation. Results: DA Ratio (5.43 ± 1.0 vs 5.52 ± 1.1, p = 0.4), SP A1 (58.76 ± 17.5 vs 59.11 ± 17.8, p = 0.4) CBI (0.96 ± 0.1 vs 0.97 ± 0.1, p = 0.3), TBI (0.98 ± 0.0 vs 0.99 ± 0.1, p = 0.3) and KMI (−0.03 ± 0.3 vs −0.17 ± 0.2, p = 0.1) did not change significantly between both time‐points. However, CH (7.96 ± 1.2 vs 7.05 ± 0.9) and CRF (6.33 ± 1.2 vs 5.35 ± 1.2) decreased significantly from preoperative to postoperative measurement time‐point (p = 0.03 for both). Conclusion: Both devices ORA and CST are supposed to analyse the dynamic behavior of the cornea during its temporary deformation by an air puff. The lack of significant changes in the standard parameters of the CST reflects the inability of this device to document the stiffening effect of ICRS in keratoconus patients. In contrast, the standard ORA parameters CH and CRF, but not the KMI, showed a significant decrease 6 month postoperatively. Background_b_ As an unstable implant at time of placement has been identified as a major cause of early implant failure, the technical requirements of achieving and maintaining implant stability are important prerequisites for successful clinical outcomes with dental implants. Recently, new implant designs and surfaces have been introduced to decrease the risk of early failure of dental implants _B_Aim/Hypothesis_b_ To evaluate the short‐term clinical and radiographic efficacy of a novel implant design and to evaluate the relative importance of achieving primary stability at placement over a 36‐months period. _B_Material and Methods_b_ This retrospective study includes 64 partially or fully edentulous patients treated between June 2014 and September 2017. Outcome measures were‐ implant and prosthetic failures, peak insertion torque values + biological and mechanical complications, marginal bone loss (MBL)+ sulcus bleeding index (SBI)+ and plaque score (PS). Clinical and radiographic checks were performed as routinely in the office at implant placement, clinical check‐up before the permanent prosthesis at 3 months, and at the 6, 12, 24 and 36 months follow‐up visits. _B_Results_b_ A total of 119 (31 narrow platform 3.75 mm, 68 regular platform ‐ 62 RP 4.3 and 6 RP 5 mm‐ and 20 wide platform 5.5 mm) NobelParallel Conical Connection implants were placed in both post‐extraction sockets and healed sites.28 implant were immediately loaded. The mean insertion torque was 46.09 Ncm. 108 out of 119 implants (90.75%) were inserted with a torque superior to 35 Ncm. Each patient received a single or multiple‐unit prosthesis. After n‐month of follow‐up, only one implant failed (1.19%) in a healed site before prosthesis installation. No other biological or technical complications were observed during this short follow‐up. The mean MBL at baseline was +1.23 mm The mean MBL from insertion was −0.51 mm , 5.04% of the implants (_I_n _i_=_I_ _i_6) showed bone loss of more than 1 mm,. All implants showed no bleeding on probing and plaque accumulation was very low (4.20%). _B_Conclusions and Clinical Implications_b_ The Nobel Parallel Conical Connection implant can be considered as an effective treatment option for implant placement in the partially or fully edentulous patients over a 36‐months period in both jaws. This implant provides high but controlled insertion torques in all bone qualities and appear to be less technique‐sensitive than other implant systems because of its parallel apically tapered design and its simple, straight forward and versatile drilling protocol.
