Identity theft and related threats are increasingly common occurrences. This paper is an overview of the Identity Threat Assessment and Prediction (ITAP) project. It describes our use of news stories to gather raw data about incidents of identity theft, fraud, and abuse. Through these news stories, we seek to determine the methods and resources used to carry out these crimes; the vulnerabilities exploited; and the consequences of these incidents. The ITAP model is a large and continually growing, structured repository of such information. There are currently about 5,000 incidents captured in the model. To this body of information, we apply a variety of analytical tools that enable us to show and compare threats, losses, and trends. We discover notable and sometimes surprising results. A goal of this project is to be able to predict future threats, and to provide some concrete guidance on how to avoid them. [ABSTRACT FROM AUTHOR]

Abstract: Background: Palmoplantar psoriasis is a variant of psoriasis vulgaris which can severely impair quality of life. Objectives: The main objectives of this double‐blind, placebo‐controlled, randomized study were to assess the efficacy and impact on quality of life and work productivity of apremilast for the treatment of moderate‐to‐severe palmoplantar psoriasis. Methods: A total of 100 patients with moderate‐to‐severe palmoplantar psoriasis were randomized to either apremilast 30 mg bid or placebo for 16 weeks. At Week 16, all patients received apremilast 30 mg bid until Week 32. The primary endpoint was the proportion of patients who achieved a Palmoplantar Psoriasis Physician Global Assessment (PPPGA) of 0/1 at Week 16. Results: There was no significant difference in the proportion of patients who achieved a PPPGA of 0/1 at Week 16 between patients randomized to apremilast (14%) and placebo (4%; P = 0.1595). After 32 weeks of treatment with apremilast, 24% of patients achieved a PPGA of 0/1. In addition, apremilast was superior to placebo in achieving Palmoplantar Psoriasis Area Severity Index (PPPASI) 75 (apremilast: 22%; placebo: 8%; P = 0.0499), in improving PPPASI (apremilast: −7.4 ± 7.1; placebo: −3.6 ± 5.9; P = 0.0167), Dermatology Life Quality Index score (apremilast: −4.3 ± 5.1; placebo: −0.8 ± 4.5; P = 0.0004) and in reducing activity impairment (apremilast: −11.0 ± 22.3; placebo: 2.5 ± 25.5; P = 0.0063). Conclusion: Despite the absence of a significant difference between apremilast and placebo in proportion of patients achieving a PPPGA of 0/1, the presence of significant differences observed for several secondary endpoints suggests that apremilast may have a role in the treatment of moderate‐to‐severe palmoplantar psoriasis. ABSTRACT Finding direct evidence for atmospheric circulation change in terrestrial records of Holocene climate variability remains a fundamental challenge. Here we present the first combined stable oxygen and hydrogen isotopic palaeorecord from a peatland core in Newfoundland, Canada. Sphagnum cellulose samples were isolated from a core from Nordan's Pond Bog, Newfoundland, and analysed for δD values. Combined with existing δ18O data, the resulting δD/δ18O bi-plot correlates directly with existing measurements of the modern (late 20th century) isotopic composition of precipitation from GNIP stations in Nova Scotia and Labrador, implying a close relationship between the estimated isotopic composition of source water used by the mosses and that of the source precipitation. We use the relative variations between the two isotope records to test the hypothesis that atmospheric circulation changed in the millennium following the 8.2-ka BP climate event. The data reveal a secondary complex isotopic response ∼200 years (8250-8050 a BP) after a primary oxygen isotopic event that is widespread in the north Atlantic region. This secondary event is characterized by a divergence in oxygen and hydrogen isotope records that can most plausibly be explained by the augmentation of precipitation moisture from a more distant and more continental vapour source. Background The REFINE study examined the efficacy and safety of adding topical corticosteroid therapy to etanercept when stepping down from the initial dose of etanercept to the maintenance dose. Clinical responses were shown to be similar in patients who remained on etanercept 50 mg twice weekly ( BIW) and those who received etanercept 50 mg once weekly ( QW) plus topical therapies through week 24. Objective The purpose of this analysis was to evaluate the effect of treatment on health-related quality of life ( HRQoL) for patients in REFINE. Methods All patients received etanercept 50 mg BIW for 12 weeks and were then randomized to etanercept 50 mg BIW or etanercept 50 mg QW plus topical corticosteroid as required to clear through week 24. HRQoL measures included the Dermatology Life Quality Index ( DLQI), Treatment Satisfaction Questionnaire for Medication ( TSQM) and the Economic Implications of Psoriasis Patient Questionnaire. No comparative testing was performed for this descriptive analysis. Missing data were imputed using the last observation carried forward. Results For 287 randomized patients (144 etanercept; 143 etanercept plus topical), the mean change [standard deviation ( SD)] in DLQI from baseline to week 24 was 10.7 (7.8) for etanercept and 9.9 (6.9) for etanercept plus topical. Mean change ( SD) in TSQM effectiveness, convenience, side-effects and global satisfaction was 27.1 (36.1), 14.8 (25.9), −0.7 (22.0) and 26.7 (32.5) for the etanercept arm and 32.5 (40.3), 18.5 (29.0), 1.3 (19.4) and 28.4 (35.9) for etanercept plus topical. Economic implications, including healthcare visits, employment status, work productivity, ability to perform daily activities and out-of-pocket expenses were similar between treatment arms. Conclusion At week 24 of REFINE, measures of HRQoL were numerically similar in patients who stayed on etanercept 50 mg BIW and patients who received etanercept 50 mg QW plus topical therapies. Background There is a paucity of data on the use of etanercept in patients who have previously failed a different tumour necrosis factor ( TNF) alpha antagonist. Objectives To study etanercept in patients who did not achieve a satisfactory response to adalimumab or who lost their response to adalimumab or infliximab and to explore the role of anti-adalimumab and anti-infliximab antibodies in etanercept response. Methods Patients with psoriasis who did not achieve a satisfactory response to adalimumab or who lost their response to adalimumab or infliximab were included. All patients received etanercept 50 mg twice a week for 12 weeks followed by 50 mg once a week for 12 more weeks. Anti-infliximab and anti-adalimumab antibodies were measured at baseline. The primary objective was to study the efficacy of etanercept using the proportion of patients who achieved a physician global assessment ( PGA) of 0 or 1. Results A total of 81 patients were included. The proportion of patients who achieved a PGA of 0 or 1 after 24 weeks of etanercept was 20.0% (95% CI 4.8-35.2%) for patients who had an unsatisfactory response to adalimumab, 35.1% (95% CI 19.0-51.3%) and 35.7% (95% CI 7.0-64.4%) for patients who lost their response to adalimumab and infliximab respectively. The proportion of patients who achieved a PGA of 0 or 1 at week 24 was numerically higher for patients who had anti-adalimumab or anti-infliximab antibodies (36.5%) as compared to those without (17.2%; P = 0.08). Background Topical corticosteroids are used with systemic therapies for treatment of plaque psoriasis, but data from randomized clinical trials to document efficacy of combination therapy are lacking. Objective To evaluate efficacy and safety of adding topical corticosteroid therapy from the time that etanercept dosage is reduced from initial label dose [50 mg twice weekly ( BIW)] to maintenance dose [50 mg once weekly ( QW)]. Methods In this phase 3b, multicentre, randomized, open-label study, patients with moderate-to-severe plaque psoriasis received etanercept 50 mg BIW for 12 weeks, and then were randomized to etanercept 50 mg BIW or 50 mg QW plus topical agent as needed to achieve static physician global assessment ( sPGA) status of clear for 12 weeks. Endpoints included percentage change in Psoriasis Area and Severity Index (PASI) score from week 12 to week 24 (primary endpoint); proportion of patients achieving 50% improvement in (PASI 50), PASI 75 and PASI 90; patients achieving sPGA of clear/almost clear; and change in affected body surface area (BSA). Results Mean difference [95% confidence interval (CI)] between etanercept arm ( n = 140) and etanercept plus topical arm ( n = 142) in change in PASI score from week 12 to week 24 was 16.2% (−3.5%, 35.8%). PASI response rates were similar between groups. Percentage (95% CI) of patients achieving sPGA status of clear/almost clear was 40.6% (32.5%, 48.6%) and 45.8% (37.6%, 54.0%) at week 12 for patients in etanercept and etanercept plus topical arms, respectively, and 53.5% (45.3%, 61.7%) and 45.4% (37.2%, 53.6%) at week 24. Difference (95% CI) between groups in change in affected BSA from week 12 to week 24 was 4.9% (−23.4%, 33.2%). Conclusion Patients who received etanercept 50 mg QW at week 12 plus as-needed topical therapy and those who stayed on etanercept 50 mg BIW maintained clinical response through week 24 with no notable differences in PASI responses. ABSTRACT A combined pollen, charcoal and climatic record is presented from Cranes Moor, southern England, covering the period c. 10 500-5850 cal a BP. It is shown that the occurrence of burning is closely related to natural processes, including prevailing climatic conditions and vegetation composition. These burning events are often linked to an increase in the summer moisture deficit, implying that the timing of burning events is linked to periods of warmer/drier climate during the Holocene Thermal Maximum ( c. 11 000-5000 cal a BP). These events play an important role in the vegetation composition and succession around the site. The nature of the burning recorded at the site shows strong similarities with other records from northern Europe. This study throws caution on suggestions that fire in the Holocene record from areas such as the British Isles is linked only to human activity, and enhances the possibility that natural fire incidence played an important role in natural woodland structure dynamics. Background Scalp psoriasis is a difficult to treat and usually chronic manifestation of psoriasis. The CalePso study showed that CPS (Clobex® Shampoo) in maintenance therapy of scalp psoriasis (twice weekly) significantly increases the probability of keeping patient under remission during 6 months, compared with vehicle (40.3% relapses vs. 11.6% relapses, ITT). Objective The objective of the study was to assess the cost-effectiveness of a maintenance therapy with CPS vs. its vehicle in nine European countries. Methods A 24-week decision tree model was developed with 4-weekly time steps. The considered population has moderate scalp psoriasis successfully treated with a daily application of CPS up to 4 weeks. Data were taken from the CalePso study and from national experts' recommendations for alternative treatment choices, with their probabilities of success taken from literature to develop country-specific models. Health benefits are measured in disease-free days (DFD). The economic analysis includes drug and physician costs. A probabilistic sensitivity analysis (PrSA) assesses the uncertainty of the model. Results Depending on the country, the mean total number of DFDs per patient is 21-42% higher with CPS compared with vehicle, and the mean total cost is 11-31% lower. The mean costs per DFD are 30-46% lower with CPS compared with the vehicle. The PrSA showed in 1000 simulations that CPS is more effective vs. vehicle in 100% of the cases and less expensive than its vehicle in 80-99% of the cases. Summary 1 A literature comparison of 14 radiocarbon-dated macrofossil records of raised peat bog initiation indicates that there is a relationship between the prevailing climate and the character of the first ombrotrophic vegetation communities at peatland sites in Britain and Ireland. 2 All that is required for ombrotrophy is the separation of the mire growing surface from the influence of surface and subsurface waters. This could occur via vertical accumulation of the peat mass or a lowering of the water table. The establishment of bog species can be rapid once isolation occurs. 3 Peatlands may become ombrotrophic in a variety of water table conditions and climatic regimes. There are at least two distinctive routes to ombrotrophy, via a ‘dry-pioneer oligotrophic community’ or via a ‘wet-pioneer oligotrophic community’. 4 Tregaron South-east Bog does not fit the pattern suggested by the literature comparison. The Fen–Bog Transition (FBT) occurred in a period of increased effective precipitation but the first ombrotrophic community was indicative of relatively dry, ‘hummocky’ bog and a deep or unstable water table. 5 The transitional poor fen communities at Tregaron South-east Bog were short-lived. Sphagnum palustre mire lasted for 90 years compared to 300 years at Bolton Fell Moss in Cumbria, and the FBT was synchronous across much of the bog. 6 The Tregaron peatland complex has a long history of water table fluctuations as shown by the stratigraphy of the marginal peats. 7 Channel incision in the River Teifi could have contributed to the development of ‘dry hummock’ pioneer bog in the humid climatic conditions at 7300 cal. A fully humanized anti-CD4 antibody was studied for its effects on resting and activated CD4 T cells. Whereas the antibody was poorly lytic, it induced dramatic down-modulation of CD4 expression on both types of cell. In order to down-modulate CD4 on resting, normal CD4 T cells there was an absolute requirement for FcR-mediated cross-linking of the anti-CD4 antibody, and only CD4 levels were affected. When activated cloned T-cell lines were studied there was no requirement for cross-linking and several other cell surface markers were also affected. Although the total cellular CD4 was reduced in the down-modulated cells, as judged by Western blot analysis, that CD4 which remained was associated with p561ck. The results are discussed in relation to the potential use of humanized anti-CD4 antibodies in the therapy of autoimmune disease and the choice of antibody isotype for such a therapeutic antibody. Constitutive activation of Janus kinase (Jak) 2 is the most prevalent pathogenic event observed in the myeloproliferative disorders (MPD), suggesting that inhibitors of Jak2 may prove valuable in their management. Inhibition of the Aurora kinases has also proven to be an effective therapeutic strategy in a number of haematological malignancies. AT9283 is a multi-targeted kinase inhibitor with potent activity against Jak2 and Aurora kinases A and B, and is currently being evaluated in clinical trials. To investigate the therapeutic potential of AT9283 in the MPD we studied its activity in a number of Jak2-dependent systems. AT9283 potently inhibited proliferation and Jak2-related signalling in Jak2-dependent cell lines as well as inhibiting the formation of erythroid colonies from haematopoietic progenitors isolated from MPD patients with Jak2 mutations. The compound also demonstrated significant therapeutic potential in vivo in an ETV6-JAK2 (TEL-JAK2) murine leukaemia model. Inhibition of both Jak2 and Aurora B was observed in the model systems used, indicating a dual mechanism of action. Our results suggest that AT9283 may be a valuable therapy in patients with MPD and that the dual inhibition of Jak2 and the Aurora kinases may potentially offer combinatorial efficacy in the treatment of these diseases.

Hemodialysis was performed in 12 patients for 2 weeks each utilizing acetate dialysate containing 134 mEq/L sodium and dialysate containing 143 mEq/L sodium, achieved by the addition of sodium chloride or sodium bicarbonate to the acetate dialysate. Intradialytic morbidity was lower, dialysis hypoxemia less marked, and predialysis blood pH higher with the bicarbonate-than with the chloride-added dialysate. The long-term use of sodium bicarbonate-added dialysate in three patients was safe. Dialysate pH adjustment was not required. These findings suggest that the addition of sodium bicarbonate (50-75 g) to acetate dialysate may be preferred to sodium chloride for increasing dialysate sodium in selected patients.

A deeply comatose patient with severe phenobarbital intoxication was hemoperfused with an Amerlite XAD-4 column. She was receiving lidocaine for ventricular ectopy. The rate of lidocaine infusion had to be increased to control ventricular arrhythmias during hemoperfusion. Plasma clearance for lidocaine by the column was 122 ml/min at initiation and 75 ml/min at 2 h. These findings suggest that an increment in lidocaine infusion may be needed during resin hemoperfusion in patients receiving this drug because of its removal by the column.
