Introduction: Cirrhosis is one of the major causes of morbidity and mortality worldwide. Portal hypertension is the major contributor of cirrhosis‐related complications and is defined as a hepatic venous pressure gradient (HVPG) > 5 mmHg. Measurement of HVPG is an invasive, difficult, and costly procedure. Therefore, it is only performed in specialized centers. Liver stiffness measured with transient elastography is one of the most studied noninvasive markers of portal hypertension, and spleen elastography has recently emerged as an important adjuvant tool. The development of a new probe (100 Hz) that more reliably reflect the grade of portal hypertension evaluated by spleen stiffness measurement has improved the accuracy of this technique. The aim of this work was to evaluate the accuracy of spleen stiffness with the new dedicated probe to predict the presence of high‐risk varices, as well as to determine the ideal cutoff to predict it. Methods: Prospective study of cirrhotic patients admitted to upper endoscopy that were also submitted to liver and spleen elastography with the 100‐Hz probe by the same blinded operator in a tertiary center. Results: We included 209 cirrhotic patients, with mean age of 61.9 years (±9.9), 77.0% male. The most common etiology was alcoholic liver disease (72.7%). The median value of liver elastography was 25.3 [4.5–75] kPa, and the median value of spleen elastography was 42.4 [7.6–100] kPa. At the cutoff of 53.25 kPa, we obtained sensitivity of 100% and specificity of 72.6% to predict high‐risk varices, and, according to this cutoff, 133/175 of esophagogastroduodenoscopy could have been spared (76.0%), while according to Baveno guidelines, only 51/175 would have been spared (29.1%). Conclusion: In the era of noninvasive exams, spleen elastography with the 100‐Hz probe emerges as an excellent tool for prediction of presence of high‐risk varices. At the cutoff of 53.25 kPa, spleen elastography avoids upper endoscopy for screening for high‐risk varices, promising to be become part of the hepatologists' daily routine. [ABSTRACT FROM AUTHOR]

Background: A gingival display higher than 3 mm is considered a characteristic of a gingival smile (GS). Several etiological factors have been associated to GS and for this reason various treatments have been proposed according to its etiology. The aim of this study is to present a case with an alternative technique to treat GS with minimally invasive vestibular incision subperiosteal tunnel access (VISTA) and polymethyl methacrylate (PMMA) implants for lip repositioning. Methods and Results: The patient, a 25‐year‐old woman diagnosed with excessive gingival exposure, had previously done botulinum toxin (BoNT) injections and did not like the result. She had a major subnasal depression and the upper lip would lodge in there during spontaneous smile. She was informed about all options of treatment and agreed to a lip repositioning installing a customized PMMA implant. Conclusion: This is the first study to our knowledge to present a case with use of a PMMA implant associated with the VISTA approach for lip repositioning and gingival smile correction. The results are encouraging, and the patient was satisfied with results accomplished by this technique. Background and Aim: Colon capsule endoscopy (CCE) has become a minimally invasive alternative for conventional colonoscopy. Nevertheless, each CCE exam produces between 50 000 and 100 000 frames, making its analysis time‐consuming and prone to errors. Convolutional neural networks (CNNs) are a type of artificial intelligence (AI) architecture with high performance in image analysis. This study aims to develop a CNN model for the identification of colonic ulcers and erosions in CCE images. Methods: A CNN model was designed using a database of CCE images. A total of 124 CCE exams performed between 2010 and 2020 in two centers were reviewed. For CNN development, a total of 37 319 images were extracted, 33 749 showing normal colonic mucosa and 3570 showing colonic ulcers and erosions. Datasets for CNN training, validation, and testing were created. The performance of the algorithm was evaluated regarding its sensitivity, specificity, positive and negative predictive values, accuracy, and area under the curve. Results: The network had a sensitivity of 96.9% and a specificity of 99.9% specific for the detection of colonic ulcers and erosions. The algorithm had an overall accuracy of 99.6%. The area under the curve was 1.00. The CNN had an image processing capacity of 90 frames per second. Conclusions: The developed algorithm is the first CNN‐based model to accurately detect ulcers and erosions in CCE images, also providing a good image processing performance. The development of these AI systems may contribute to improve both the diagnostic and time efficiency of CCE exams, facilitating CCE adoption to routine clinical practice. [ABSTRACT FROM AUTHOR]

The multifunctional protein translationally controlled tumour protein (TCTP) was previously identified as necessary for infection by the potyvirus pepper yellow mosaic virus. Using turnip mosaic virus (TuMV) as a model to study potyvirus biology, we confirmed that TCTP has a positive effect on virus infection. Living cell confocal microscopy demonstrated that TCTP colocalises with 6K2‐tagged replication vesicles and with a perinuclear globular structure typically observed during potyvirus infection. Also, TCTP silenced protoplasts showed reduced virus accumulation, quantified by qRT‐PCR, which suggests an effect on virus replication, translation or other intracellular process. Finally, TCTP silencing in plants reduced the accumulation of two species belonging to Orthotospovirus and a Begomovirus genus, which are not closely related to potyviruses. In this study, we synthesized eight new compounds containing the 2‐amino‐cycloalkyl[b]thiophene and acridine moieties (ACT01 and ACS01‐ACS07). None tested compounds presented human erythrocyte cytotoxicity. The new compounds presented antipromastigote activity, where ACS01 and ACS02 derivatives presented significant antileishmanial activity, with better performance than the reference drugs (tri and pentavalent antimonials), with respective IC50 values of 9.60 ± 3.19 and 10.95 ± 3.96 μ m. Additionally, these two derivatives were effective against antimony‐resistant Leishmania (Leishmania) amazonensis strains. In addition, binding and fragmentation DNA assays were performed. It was observed that the antileishmanial activity of ACS01 is not associated with DNA fragmentation of the promastigote forms. However, it interacted with DNA with a binding constant of 104  m−1. In partial least‐squares studies, it was observed that the most active compounds (ACS01 and ACS02) showed lower values of amphiphilic moment descriptor, but there was a correlation between the lipophilicity of the molecules and antileishmanial activity. Furthermore, the docking molecular studies showed interactions between thiophene–acridine derivatives and the active site of pyruvate kinase enzyme with the major contribution of asparagine 152 residue for the interaction with thiophene moiety. Background and Aim Immunization against hepatitis B virus ( HBV) infection is recommended in patients with inflammatory bowel disease ( IBD), particularly in those under immunosuppressive therapy. Limited data are available about IBD patient's response to HBV vaccination. We assessed the response rate to HBV vaccination in IBD patients and evaluated the impact of different factors on the efficacy of HBV vaccination. Methods Anti- HBs titers were measured in a cohort of IBD patients under treatment with infliximab and/or azathioprine. Vaccination was considered efficient when anti- HBs titers were higher than 10 IU/L. Results We have identified 217 patients with IBD under infliximab who were vaccinated for hepatitis B, 172 (79%) with Crohn's Disease and the remaining with ulcerative colitis; 114 patients (53%) were male and mean age was 33 ± 11 years. Overall, HBV vaccine was successful in 164 (76%) patients. Only 14 patients were vaccinated after infliximab was initiated, and only two of them had antibody levels above 10 IU/L. Among the patients that received vaccination before the beginning of infliximab, 88% of those who were vaccinated before starting azathioprine developed antibodies in contrast to 55% who already were under azathioprine. In multivariable analysis, treatment with infliximab (adjusted OR [95% CI]: 17.642 [8.514-33.937]) and with azathioprine (adjusted OR [95% CI]: 3.344 [1.653-9.145]) were the only factors associated with weaker response to HBV vaccination. Conclusion The response rate to the standard HBV vaccination in IBD patients is low mainly in those treated with infliximab and/or azathioprine. [ABSTRACT FROM AUTHOR]

Background: The aim of this study was to compare the potential of bioactive glass particles of different size ranges to affect bone formation in periodontal defects, using the guided tissue regeneration model in dogs. Methods: In six dogs, 2-wall intrabony periodontal defects were surgically created and chronified on the mesial surfaces of mandibular third premolars and first molars bilaterally. After 1 month, each defect was randomly assigned to treatment with bioabsorbable membrane in association with bioactive glass with particle sizes between 300 and 355 µm (group 1) or between 90 and 710 µm (group 2), membrane alone (group 3), or negative control (group 4). The dogs were sacrificed 12 weeks after surgeries, and histomorphometric measurements were made of the areas of newly formed bone, new mineralized bone, and bioactive glass particle remnants. Results: With regard to the area of bioactive glass particle remnants, there was a statistically significant difference between groups 1 and 2, favoring group 1. There were greater areas of mineralized bone in groups 1 and 2 compared to groups 3 and 4 (P<0.05). Conclusion: The bioactive glass particles of small size range underwent faster resorption and substitution by new bone than the larger particles, and the use of bioactive glass particles favored the formation of mineralized bone. Aim: The aim of this randomized, controlled, clinical study was to compare two surgical techniques with the acellular dermal matrix graft (ADMG) to evaluate which technique could provide better root coverage. Material and Methods: Fifteen patients with bilateral Miller Class I gingival recession areas were selected. In each patient, one recession area was randomly assigned to the control group, while the contra-lateral recession area was assigned to the test group. The ADMG was used in both groups. The control group was treated with a broader flap and vertical-releasing incisions, and the test group was treated with the proposed surgical technique, without releasing incisions. The clinical parameters evaluated before the surgeries and after 12 months were: gingival recession height, probing depth, relative clinical attachment level and the width and thickness of keratinized tissue. Results: There were no statistically significant differences between the groups for all parameters at baseline. After 12 months, there was a statistically significant reduction in recession height in both groups, and there was no statistically significant difference between the techniques with regard to root coverage. Conclusions: Both surgical techniques provided significant reduction in gingival recession height after 12 months, and similar results in relation to root coverage. [ABSTRACT FROM AUTHOR]

Background: The purpose of this study was to evaluate acellular dermal matrix (ADM) as a membrane for guided tissue regeneration (GTR) in the treatment of mandibular Class II furcation lesions and to compare it to a bioabsorbable membrane. Methods: Class II furcation lesions were created surgically and chronified in six mongrel dogs on the buccal surfaces of mandibular third and fourth premolars, bilaterally. After 1 month, GTR was performed. Each side was assigned randomly to the control group (CGr: bioabsorbable membrane made of polyglycolic acid: trimethylene carbonate) or the test group (TGr: ADM as a membrane). Clinical measurements of the width and thickness of the keratinized tissue (WKT and TKT, respectively) were made before GTR. The dogs were sacrificed 12 weeks following GTR, and histomor phometric analysis was performed. Area measurements were taken of new tissue, epithelium, connective tissue, and new bone; bone height and new cementum were measured. Results: The formation of new bone, cementum, and periodontal ligament were similar in both groups. There were no statistically significant differences between the groups for any histomorphometric measurement. The TGr had a statistically significant increase in WKT and TKT after 3 months. The mean TKT gain was 1.03 mm for the TGr and 0.26 mm for the CGr (P<0.05). Conclusion: The ADM demonstrated histomorphometric results similar to the bioabsorbable membrane and resulted in a greater increase in the thickness of the keratinized tissue. [ABSTRACT FROM AUTHOR]

In this work, the effect of carbon nanotubes (CNT) on the biodegradability of poly(3‐hydroxybutyrate‐co‐3‐hydroxyvalerate) (PHBV) nanocomposites with 1 and 2% (w/w) of CNT has been evaluated by biodegradation assays in aqueous medium containing microorganisms extracted from garden soil. After biodegradation process, samples of PHBV and PHBV/CNT were compared regarding magnitude of degradation by visual macroscopic analysis, by measurement of percentage of weight loss and by scanning electron microscopy (SEM). The samples were further characterized regarding roughness and hydrophilicity by contact angle measurement. The final residue resultant from biodegradation of PHBV/CNT was analyzed in a FEI Inspect F50 field emission scanning electron microscope (FESEM) in order to verify the presence of CNT. Our results indicated that CNT did not compromise the biodegradability of PHBV matrix although the biodegradation rate has decreased with the increase of CNT content as shown by the weight loss measurements. SEM analysis of PHBV/CNT nanocomposites after biodegradation process showed increased amount of CNT exposed at the matrix surface suggesting the possibility of CNT recovering after the biodegradation. © 2019 Wiley Periodicals, Inc.
