The maintenance of biological diversity is frequently enhanced in a heterogenous landscape by some level of disturbance. Thus, when a landscape becomes stabilized and homogenized through the spread of an invasive plant species, there may be consequences for native biodiversity, particularly for those biotas that depend on the pre‐existing, natural disturbance regime of that landscape. At Long Point, Ontario, Canada, a sandspit in Lake Erie, the community of anuran amphibians experienced the co‐incidence of 2 major disturbance events, 1 anthropogenic and 1 natural: an intervention to remove the invasive form of the common reed (Phragmites australis australis) and a spate of extensive dune washouts caused by high water levels and storms. As a result of the unequal distribution of disturbance in the landscape, different areas were affected by only the dune washouts, only the common reed treatment, both disturbances, or neither disturbance. From May to August 2021, we explored how the disturbances affected the resident frog and toad species habitat associations in the 4 different disturbance conditions in the landscape. By means of minnow traps, acoustic surveys, and visual surveys, we determined that the dune washouts had a greater influence on resident anurans than the common reed treatment. Ranid frogs (Ranidae) tended to inhabit non‐washout sites, whereas Fowler's toads (Anaxyrus fowleri) congregated in the newly formed, sand flats and shallow, de‐vegetated pools resulting from the washouts. Neither ranid frogs nor Fowler's toads demonstrated avoidance of the sites affected by the herbicide treatment and mechanical rollover of the common reed. This evidence of species sorting, which enables multiple species to persist in a dynamic and heterogeneous landscape, suggests that wildlife management in aid of threatened species recovery may find success by encouraging natural disturbance regimes in dynamic landscapes. Future management endeavors should consider refraining from complete dune stabilization in these landscapes to allow the natural washouts to occur at a higher frequency, while ensuring the continued effectiveness of the invasive common reed eradication. This article reviews nine soil carbon interventions to reduce soil carbon losses or increase stocks in grazing management systems, using their potential application in Aotearoa New Zealand (A‐NZ) as a case study. The interventions are classified into three strategies: (1) increasing carbon inputs through deep‐rooting and diverse species grasslands, reducing forage cropping and deferred grazing management; (2) increasing protection of carbon stocks by water table management on organic soils, reducing cropping on organic soils, full inversion tillage grassland renewal, the addition of clay minerals and enhanced rock weathering and (3) the adoption of integrated systems including establishing tree clusters into grasslands and agroforestry. We estimated the land area where these interventions could realistically be implemented in A‐NZ and assessed their potential impact on mitigating national overall agricultural greenhouse gas emissions. The potential impact of each intervention ranged from <1% to 2.5% of national agricultural greenhouse gas emissions over 20 years. However, confidence in these approaches is hindered by a lack of research data where these approaches have been tested for different soil types and conditions. Notably, water table management, which reduces carbon loss from organic soils, was the only intervention that could achieve moderate, short‐ and long‐term impacts with a confidence level assessed as 'likely'. We conclude that reducing further soil carbon losses and achieving modest increases in soil carbon stocks are possible but will require economic and political incentives that encourage the integration of multiple interventions at the farm scale. INTRODUCTION: Cerebral small vessel disease (SVD) is a common cause of stroke/vascular dementia with few effective treatments. Neuroinflammation and increased blood‐brain barrier (BBB) permeability may influence pathogenesis. In rodent models, minocycline reduced inflammation/BBB permeability. We determined whether minocycline had a similar effect in patients with SVD. METHODS: MINERVA was a single‐center, phase II, randomized, double‐blind, placebo‐controlled trial. Forty‐four participants with moderate‐to‐severe SVD took minocycline or placebo for 3 months. Co‐primary outcomes were microglial signal (determined using 11C‐PK11195 positron emission tomography) and BBB permeability (using dynamic contrast‐enhanced MRI). RESULTS: Forty‐four participants were recruited between September 2019 and June 2022. Minocycline had no effect on 11C‐PK11195 binding (relative risk [RR] 1.01, 95% confidence interval [CI] 0.98–1.04), or BBB permeability (RR 0.97, 95% CI 0.91–1.03). Serum inflammatory markers were not affected. DISCUSSION: 11C‐PK11195 binding and increased BBB permeability are present in SVD; minocycline did not reduce either process. Whether these pathophysiological mechanisms are disease‐causing remains unclear. Background: Perennial ryegrass (Lolium perenne) in New Zealand pastures is typically infected with the mutualist Epichloë fungal endophyte. This endophyte assists the plant in resisting biotic and abiotic stresses, but the standard strain of endophyte is toxic to livestock. Elite ryegrasses with selected endophytes have been developed to provide protective properties to the grass plant and lessen or eliminate the negative impacts on livestock. Methods: Using immunology and molecular techniques, the presence of endophyte infection and endophyte strain in ryegrass tillers was determined for 24 dairy pastures sampled for up to 7 years in regions of the North and South Islands. Results: In general, infection levels were high and showed small increases over time. Some pastures failed to reach 70% infection. The sown, selected endophytes were the dominant endophyte strains present and these were stable over time. Standard endophyte was the primary nonsown endophyte, and while generally low and so of little importance, it increased over time and for some pastures, this would have been detrimental to livestock. Pasture establishment technique influenced the level of contamination. Conclusions: Results reinforce the importance of following best practice procedures in the seed industry and on‐farm. A large stretch of the east coast of Australia experienced unprecedented rainfall and flooding over a two‐week period in early 2022. It is difficult to reliably estimate the likelihood of such a rare event from the relatively short observational record, so an alternative is to use data from an ensemble prediction system (e.g., a seasonal or decadal forecast system) to obtain a much larger sample of simulated weather events. This so‐called 'UNSEEN' method has been successfully applied in several scientific studies, but those studies typically rely on a single prediction system. In this study, we use data from the Decadal Climate Prediction Project to explore the model uncertainty associated with the UNSEEN method by assessing 10 different hindcast ensembles. Using the 15‐day rainfall total averaged over the river catchments impacted by the 2022 east coast event, we find that the models produce a wide range of likelihood estimates. Even after excluding a number of models that fail basic fidelity tests, estimates of the event return period ranged from 320 to 1814 years. The vast majority of models suggested the event is rarer than a standard extreme value assessment of the observational record (297 years). Aim: Quality of life (QoL) is a crucial and core outcome in assessing the effectiveness of treatments for cryptoglandular anal fistula. Despite its extensive impact, there is a lack of patient‐centred, disease‐specific QoL measurement instruments of adequate quality. The aim of this study is to develop a disease‐specific measurement instrument that can accurately measure QoL for patients with cryptoglandular anal fistula. Method: Semi‐structured qualitative patient interviews and a systematic review of current instruments were used to generate items for the draft instrument. This underwent successive rounds of cognitive interviews to refine its wording and structure. Individual item and overall scale content validity were determined by asking experts to rate the relevance of each item and those deemed irrelevant were removed. The final instrument then underwent psychometric testing and test–retest analysis to determine its sensitivity and stability. Results: A total of 148 patients were involved in item generation, scale development and psychometric testing. A 22‐item measurement instrument has been developed; it is scored on a scale of 0–100, where 0 indicates the worst QoL and 100 demonstrates perfect QoL. The scale demonstrates excellent internal consistency (Cronbach‐α = 0.927), strong content and construct validity [correlation with Perianal Disease Activity Index = −0.713, Hospital Anxiety and Depression Anxiety (−0.659) and Depression (−0.673) subscales and Short Form‐12 physical (0.609) and mental (0.589) component scales] and strong reliability and responsiveness. Conclusion: We have developed a cryptoglandular Anal Fistula Quality of Life scale (AF‐QoL), a comprehensive, disease‐specific patient reported outcome measure assessing QoL in patients with cryptoglandular anal fistula. Aim: Patient understanding of disease can guide decision‐making in the management of anal fistula. This prospective feasibility study aimed to assess the acceptability and methods of assessing the impact of viewing realistic models on patients with anal fistula. Methods: New referrals to a tertiary clinic participated in this single‐centre, parallel‐group randomized controlled study. Baseline characteristics, Decisional Conflict Scale and understanding of disease were assessed pre‐consultation. Participants were randomized to a standard consultation, where disease and treatment options were explained using magnetic resonance images and drawn diagrams, or a similar consultation supplemented with an appropriate generic three‐dimensional (3D) printed model. Understanding of disease and proposed surgery, Decisional Conflict Scale and ratings of visual aids were assessed post‐consultation, along with 3D model feedback. Results: All 52 patients who were approached agreed to be randomized (25 standard, 27 3D consultation). Understanding of disease increased post‐consultation in both groups. Post‐consultation decisional conflict (0, no; 100, high decisional conflict) was low (median 27 post‐standard vs. 24 post‐3D consultation). Patients scored highly on measures assessing understanding of proposed surgery. 3D models were rated highly, with 96% of patients wanting to see them again in future consultations. Conclusions: Three‐dimensional printed fistula models are a welcome addition to outpatient consultations with results suggesting that understanding of surgery is improved. A future trial should be powered to detect whether 3D models result in a significant improvement in understanding beyond traditional methods of explanation and explore the conditions in which models have their maximal utility. Aim: The management of anal fissure: ACPGBI position statement was written 15 years ago. [KLR Cross et al., Colorectal Dis, 2008]. Our aim was to update the guideline and provide recommendations on the most effective treatment for patients with anal fissures utilising a multidisciplinary, rigorous guideline methodology. Methods: The development process consisted of six phases. In phase 1 we defined the scope of the guideline. The patient population included patients with acute and chronic anal fissure. The target group was all practitioners (primary and secondary care) treating patients with fissures and, in addition, healthcare workers and patients who desired information regarding fissure management. In phase 2 we formed a guideline development group (GDG) including a methodologist. In phase 3 review questions were formulated, using a reversed PICO process, starting with possible recommendations based on the GDG's knowledge. In phase 4 a comprehensive literature search focused on existing systematic reviews addressing each review question, supplemented by more recent studies if appropriate. In phase 5 data were extracted from the included papers and checked by the GDG. If indicated, meta‐analysis of systematic review data was updated by the GDG. During phase 6 the GDG members decided what recommendations could be made based on the evidence in the literature and strength of the recommendation was assessed using 'grade'. Results: This guideline is divided into two sections: Primary care which includes (i) diagnosis; (ii) basic treatment; (iii) topical treatment; and secondary care which includes (iv) botulinum toxin therapy; (v) surgical intervention and (vi) special situations (including pregnancy and breast‐feeding patients, children, receptive anal intercourse and low‐pressure fissures). A total of 23 recommendations were formulated. A new term clinically healed was described by the GDG. Conclusion: This guideline provides an up‐to‐date evidence‐based summary of the current knowledge of the management of anal fissure and may serve as a useful guide for clinicians as well as a potential reference for patients. Marine multichannel and wide‐angle seismic data constrain the distribution of seamounts, sediment cover sequence and crustal structure along a 460 km margin‐parallel transect of the Hikurangi Plateau. Seismic reflection data reveals five seamount up‐to 4.5 km high and 35–75 km wide, with heterogeneous internal velocity structure. Sediment cover decreases south‐to‐north from ∼4.5 km to ∼1–2 km. The Hikurangi Plateau crust (VP 5.5–7.5 km/s) is 11 ± 1 km thick in the south, but thins by 3–4 km further north (∼7–8 km). Gravity models constructed along two seismic lines show the reduction in crustal thickness persists further east, coinciding with a bathymetric scarp. Gravity data suggest the transition in crustal thickness may reflect spatial variability in deformation and lithospheric extension associated with plateau breakup. Variability in the thickness of subducting crust may contribute to differences in megathrust geometry, upper‐plate stress state and high‐rates of contraction and uplift along the southern Hikurangi margin. Plain Language Summary: The thickness of crust arriving at subduction zones exerts a major influence on the configuration, structure, and distribution of stresses at plate boundaries. A region of thickened oceanic crust, the Hikurangi Plateau, is subducting beneath the North Island of New Zealand. To understand the structure of this Plateau, we analyze seismic data along a 460 km long transect, which reveals five large seamounts, and a 3–4 km reduction crustal thickness between the southern and northern areas of the Plateau. Gravity data show this difference is persistent across the plateau and reveal structures suggesting the crust may have thinned when the Hikurangi plateau separated from a second plateau (Manihiki), which is thought to have formed at the same time. We propose that the subduction of thicker crustal along the southern Hikurangi margin may contribute to the shallow angle of the subducting plate, and high rates of uplift and deformation in the overthrusting plate. Clustering is indispensable in the quest for robust vegetation classification schemes that aim to partition, summarise and communicate patterns. However, clustering solutions are sensitive to methods and data and are therefore unstable, a feature that is usually attributed to noise. Viewed through a central‐tendency lens, noise is defined as the degree of departure from type, which is problematic since vegetation types are abstractions of continua, and so noise can only be quantified relative to the particular solution at hand. Graph theory models the structure of vegetation data based on the interconnectivity of samples. Through a graph‐theoretic lens, the causes of instability can be quantified in absolute terms via the degree of connectivity among objects. We simulated incremental increases in sampling intensity in a dataset over five iterations and assessed classification stability across successive solutions derived using algorithms implementing, respectively, models of central‐tendency and interconnectivity. We used logistic regression to model the likelihood of a sample changing groups between iterations as a function of distance to the centroid and degree of interconnectivity. Our results show that the degree to which samples are interconnected is a more powerful predictor of instability than the degree to which they deviate from their nearest centroid. The removal of weakly interconnected samples resulted in more stable classifications, although solutions with many clusters were apparently inherently less stable than those with few clusters, and improvements in stability flowing from the removal of outliers declined as the number of clusters increased. Our results reinforce the fact that clusters abstracted from continuous data are inherently unstable and that the quest for stable, fine‐scale classifications from large regional datasets is illusory. The chemical interaction of Sn with H2 by X‐ray diffraction methods at pressures of 180–210 GPa is studied. A previously unknown tetrahydride SnH4 with a cubic structure (fcc) exhibiting superconducting properties below TC = 72 K is obtained; the formation of a high molecular C2/m‐SnH14 superhydride and several lower hydrides, fcc SnH2, and C2‐Sn12H18, is also detected. The temperature dependence of critical current density JC(T) in SnH4 yields the superconducting gap 2Δ(0) = 21.6 meV at 180 GPa. SnH4 has unusual behavior in strong magnetic fields: B,T‐linear dependences of magnetoresistance and the upper critical magnetic field BC2(T) ∝ (TC – T). The latter contradicts the Wertheimer–Helfand–Hohenberg model developed for conventional superconductors. Along with this, the temperature dependence of electrical resistance of fcc SnH4 in non‐superconducting state exhibits a deviation from what is expected for phonon‐mediated scattering described by the Bloch‐Grüneisen model and is beyond the framework of the Fermi liquid theory. Aim: Pilonidal disease is a benign condition that affects mainly the young. In existing literature, there is no consensus for best treatment, with multiple operative techniques described, some complex, resulting in a high proportion of failure and/or morbidity. The cleft closure (or cleft lift) described by Bascom and Bascom (Arch Surg, 137, 2002, 1146−50), by comparison, is a simple operation, resulting in healing in the majority and good cosmesis. Method: This is a single surgeon series, with the aim of evaluating outcomes for consecutive patients who underwent cleft closure surgery at two centres (St Mark's Hospital, London, and Queen Alexandra Hospital, Portsmouth) between 1995 and 2021 for either extensive, complex or recurrent pilonidal disease. Primary study outcomes that were measured included rate of primary healing, time to complete healing and rate of recurrence. Results: Some 714 patients had a cleft closure of whom 656 had documented follow‐up. Primary healing occurred in 60.7% (n = 398) rising to 88.5% by 12 weeks (n = 562) and 91.8% by 16 weeks. The remaining patients healed over the following weeks with only 19 wounds failing to heal completely (3%), requiring further surgery to achieve healing. After complete healing 5.3% of patients developed recurrent disease at a median of 12 months. Conclusion: Cleft closure is an effective operation for pilonidal disease. Overall, 97% of patients healed without further surgery. A 3% failure rate and 5.3% recurrence rate were observed. This technique could be considered as an alternative procedure to complex flaps or midline excision, in extensive, recurrent and unhealed pilonidal disease. [ABSTRACT FROM AUTHOR]

Objectives: To report a single centre's experience of the feasibility, safety and patient acceptability of same‐day discharge robot‐assisted laparoscopic prostatectomy (RALP). Subjects/Patients and Methods: Between June 2015 and December 2021, a total of 180 pre‐selected consecutive patients underwent RALP with the intention to discharge on the same day as surgery. Cases were performed by two surgeons. An enhanced recovery after surgery (ERAS) programme was used. The feasibility of same‐day discharge was analysed, along with the complication rate, oncological outcomes, and postoperative patient experience. Results: Of 180 patients, 169 (93.8%) were successfully discharged on the same day as surgery. The median (range) age was 63 (44–74) years. The median (range) console time was 97 (61–256) min and blood loss was 200 (20–800) mL. The resection specimen pathology results were: pT2 69.4%, pT3a 24.4% and pT3b 6.5%. With regard to Gleason Grade Group (GGG), 25.9% had GGG 1, 65.7% had GGG 2–3 and 8.4% had GGG 4–5 disease. Positive surgical margins were present in 25 cases (14.7%), 18 (15.5%) of which occurred in pT2 cases, and seven (13.4%) in pT3 cases. There were no early (<90 days) biochemical relapses (defined as prostate‐specific antigen level >0.2 ng/mL). The 30‐day readmission rate was 3%. A total of 13 early (0–30 days) complications were observed, five of which were Clavien–Dindo grade ≥3, however, none of these would have been avoided had the patient remained in hospital on the first postoperative night. Of 121 consecutive patients, 107 (88%) returned a satisfaction questionnaire, and 92% of responders stated they preferred recovery at home, with 94% stating they felt ready to go home. Conclusion: Robot‐assisted laparoscopic prostatectomy combined with an ERAS programme allows patients to be safely discharged home on the same day of their surgery. Species often exhibit regionally specific habitat associations, so habitat association models developed in one region might not be accurate or even appropriate for other regions. Three programs to survey wetland‐breeding birds covering (respectively) Great Lakes coastal wetlands, inland Great Lakes wetlands, and the Prairie Pothole Region offer an opportunity to test whether regionally specific models of habitat use by wetland‐obligate breeding birds are transferrable across regions. We first developed independent, regional population density models for four species of wetland‐obligate birds: Pied‐billed Grebe (Podilymbus podiceps), Virginia Rail (Rallus limicola), Sora (Porzana carolina), and American Bittern (Botaurus lentiginosus). We then used adjusted pseudo‐R2 values to compare the amount of variation explained by each model when applied to data collected in each of the three regions. Although certain habitat characteristics, such as emergent vegetation and wetland area, were consistently important across regions, models for each species differed by region—both in variables selected for inclusion and often in the directionality of relationships for common variables—indicating that habitat associations for these species are regionally specific. When we applied a model developed in one region to data collected in another region, we found that explanatory power was reduced in most (71%) models. Therefore, we suggest that ecological analyses should emphasize regionally specific habitat association models whenever possible. Nonetheless, models created from inland Great Lakes wetland data had higher median explanatory power when applied to other regions, and the amount of explanatory power lost by other transferred models was relatively small. Thus, while regionally specific habitat association models are preferable, in the absence of reliable regional data, habitat association models developed in one region may be applied to another region, but the results need to be cautiously interpreted. Additionally, we found that median explanatory power was higher when local‐scale habitat characteristics were included in the models, indicating that regionally specific models should ideally be based on a combination of local‐ and landscape‐scale habitat characteristics. Conservation practitioners can leverage such regionally specific models and associated monitoring data to help prioritize areas for management activities that contribute to regional conservation efforts. Background and Aims. Australia's changing climate is already impacting the agriculture sector and will continue to do so in the future. To help respond to these impacts, the Climate Services for Agriculture (CSA) platform presents readily accessible climate data, including future climate projections, relevant to specific agricultural commodities. This wine industry example aims to demonstrate the functionality and utility of the CSA for national use across a broad range of commodities. Methods and Results. The platform includes commodity-relevant climate indices designed in consultation with experts to ensure that they are as salient to producers as possible; the wine-grape specific indices include measures of growing season temperature, rainfall, extreme heat, and frost. Here, we describe the research behind the wine-grape specific indices and present sample outputs from the CSA platform for a site within a selected winegrowing region. We note the CSA platform has been developed through an extensive and continuing user engagement initiative, ensuring it meets the needs of the agriculture community as they grapple with how to make decisions based on longer term climate projections. Conclusions. Provision of past, seasonal outlook, and future climate information for Australia and for a range of important agricultural commodities can help improve on-farm planning and decision-making to respond to climate risks. The wine industry provides a leading example of how to use these data for decision-making, noting ongoing adjustments will be needed. Significance of the Study. The CSA platform brings together historical climate data, seasonal climate outlooks, and future climate projections to assist agricultural producers to better manage climate variability and climate change. Background: There are reported variations in the intraoperative management of Crohn's disease. This consensus statement aimed to develop a standardised protocol for photographic documentation of intraoperative findings and critical procedural steps in ileocolonic Crohn's disease surgery. Methods: Colorectal surgeons with a specialist interest in minimally invasive surgery and inflammatory bowel disease were invited as committee members to develop a survey on the use of photo‐documentation in Crohn's disease surgery. A 15 item survey was developed on ethical considerations and applications of photo‐documentation in audit and quality control, research, and training. Results: There was strong agreement on the potential application of intraoperative photo‐documentation in Crohn's disease for training, research, quality control and tertiary referrals. Reviewers agreed that intraoperative staging required photo‐documentation of strictures, skip lesions, perforations, fat wrapping and mesenteric disease. The necessary steps to be photo‐documented were very specific to Crohn's disease surgery, such as views of anastomosis and strictureplasties, and extent of resection(s). Conclusions: Our consensus statement identified several items for appropriate intraoperative photo‐documentation in Crohn's disease surgery, to be used as an adjunct to accurate annotation of intraoperative findings and procedures. Waterfowl management is more effective when based on detailed information on population connectivity between breeding, wintering, and stopover sites. For the American black duck (Anas rubripes), a species of conservation concern, estimates for the fall age ratio at harvest differed depending on whether harvest data were derived from Canada or the United States, suggesting regional differences. Within Canada, hunters in Atlantic Canada were more likely to harvest black ducks from nearby breeding locations compared to hunters in southern Ontario and Quebec, Canada, who were more likely to harvest individuals from the Boreal Softwood and Taiga Shield of eastern Canada. Black ducks harvested in the United States are thought to originate predominantly from northern portions of the breeding range, leading to the flyover hypothesis, which postulates that black ducks produced in the Boreal Softwood and Taiga Shield region are less susceptible to harvest by hunters in Atlantic Canada and northeastern United States. To test the flyover hypothesis, we examined regional and temporal differences in the origins of harvested black ducks using feathers from wings (n = 664) submitted by hunters to the species composition and parts collection surveys across 3 hunting seasons (2017–2018, 2018–2019, 2019–2020). We used a likelihood‐based assignment method that relied on feather stable‐hydrogen isotopes (δ2H) and stable‐carbon isotopes (δ13C) to determine the natal or molt origin of individuals harvested within eastern Canada and the United States. We also used a spatial clustering technique to group harvested individuals by area of origin without a priori knowledge of such regions. Adult female black ducks originated farther south compared to males and juveniles. All sexes and ages of black ducks harvested in Atlantic Canada showed predominantly southern origins, while those harvested in the United States and other Canadian provinces primarily originated farther north within the boreal, supporting the flyover hypothesis. By contrast, we found no relationship between timing of harvest or peaks of migration and individual origin. After combining band returns and stable isotopes, we inferred 2 distinct stocks: the Mississippi flyway stock and the Atlantic flyway stock. We recommend that regional demographic parameters, particularly for Atlantic Canada, be directly measured to promote more effective conservation of black ducks and optimize harvest opportunities in the United States and Canada. Questions: Most clustering methods assume data are structured as discrete hyperspheroidal clusters to be evaluated by measures of central tendency. If vegetation data do not conform to this model, then vegetation data may be clustered incorrectly. What are the implications for cluster stability and evaluation if clusters are of irregular shape or density? Location: Southeast Australia. Methods: We define misplacement as the placement of a sample in a cluster other than (distinct from) its nearest neighbor and hypothesize that optimizing homogeneity incurs the cost of higher rates of misplacement. Chameleon is a graph‐theoretic algorithm that emphasizes interconnectivity and thus is sensitive to the shape and distribution of clusters. We contrasted its solutions with those of traditional nonhierarchical and hierarchical (agglomerative and divisive) approaches. Results: Chameleon‐derived solutions had lower rates of misplacement and only marginally higher heterogeneity than those of k‐means in the range of 15–60 clusters, but their metrics converged with larger numbers of clusters. Solutions derived by agglomerative clustering had the best metrics (and divisive clustering the worst) but both produced inferior high‐level solutions to those of Chameleon by merging distantly‐related clusters. Conclusions: Graph‐theoretic algorithms, such as Chameleon, have an advantage over traditional algorithms when data exhibit discontinuities and variable structure, typically producing more stable solutions (due to lower rates of misplacement) but scoring lower on traditional metrics of central tendency. Advantages are less obvious in the partitioning of data from continuous gradients; however, graph‐based partitioning protocols facilitate the hierarchical integration of solutions. Dissolution dynamic nuclear polarisation (dDNP) of 13C‐labelled pyruvate in magnetic resonance spectroscopy/imaging (MRS/MRSI) has the potential for monitoring tumour progression and treatment response. Pyruvate delivery, its metabolism to lactate and efflux were investigated in rat P22 sarcomas following simultaneous intravenous administration of hyperpolarised 13C‐labelled pyruvate (13C1‐pyruvate) and urea (13C‐urea), a nonmetabolised marker. A general mathematical model of pyruvate‐lactate exchange, incorporating an arterial input function (AIF), enabled the losses of pyruvate and lactate from tumour to be estimated, in addition to the clearance rate of pyruvate signal from blood into tumour, Kip, and the forward and reverse fractional rate constants for pyruvate‐lactate signal exchange, kpl and klp. An analogous model was developed for urea, enabling estimation of urea tumour losses and the blood clearance parameter, Kiu. A spectral fitting procedure to blood time‐course data proved superior to assuming a gamma‐variate form for the AIFs. Mean arterial blood pressure marginally correlated with clearance rates. Kiu equalled Kip, indicating equivalent permeability of the tumour vasculature to urea and pyruvate. Fractional loss rate constants due to effluxes of pyruvate, lactate and urea from tumour tissue into blood (kpo, klo and kuo, respectively) indicated that T1s and the average flip angle, θ, obtained from arterial blood were poor surrogates for these parameters in tumour tissue. A precursor‐product model, using the tumour pyruvate signal time‐course as the input for the corresponding lactate signal time‐course, was modified to account for the observed delay between them. The corresponding fractional rate constant, kavail, most likely reflected heterogeneous tumour microcirculation. Loss parameters, estimated from this model with different TRs, provided a lower limit on the estimates of tumour T1 for lactate and urea. The results do not support use of hyperpolarised urea for providing information on the tumour microcirculation over and above what can be obtained from pyruvate alone. Objectives: Thoracostomy is often a required treatment in patients with thoracic trauma; however, performing a thoracostomy using traditional techniques can have complications. Ultrasound can be a beneficial tool for identifying the correct thoracostomy insertion site. We designed a randomized prospective study to assess if ultrasound guidance can improve thoracostomy site identification over traditional techniques. Methods: Emergency medicine residents were randomly assigned to use palpation or ultrasound to identify a safe insertion site for thoracostomy placement. The target population comprised of hemodynamically stable trauma patients who received an extended focused assessment with sonography for trauma (EFAST) and a chest computed tomography (CT) exam. The resident placed a radiopaque marker on the skin of the patient where a safe intercostal space was believed to be located, either by palpation or ultrasound. Clinical ultrasound faculty reviewed the CT to confirm marker placement relative to the diaphragm. A Fischer's exact test was used to analyze the groups. Results: One hundred and forty‐seven patients were enrolled in the study, 75 in the ultrasound group and 72 in the landmark group. This resulted in the placement of 271 total thoracostomy site markers, 142 by ultrasound and 129 by palpation and landmarks. The ultrasound group correctly identified thoracostomy insertion sites above the diaphragm in 97.2% (138/142) of patients, while the palpation group identified a safe insertion site in 88.4% (114/129) of patients (P =.0073). Conclusion: This study found that emergency medicine residents are more likely to identify a safe tube thoracostomy insertion site in trauma patients by using ultrasound, as compared to using landmarks and palpation. To date, ∼20% of the ocean floor has been surveyed by ships at a spatial resolution of 400 m or better. The remaining 80% has depth predicted from satellite altimeter‐derived gravity measurements at a relatively low resolution. There are many remote ocean areas in the southern hemisphere that will not be completely mapped at 400 m resolution during this decade. This study is focused on the development of synthetic bathymetry to fill the gaps. There are two types of seafloor features that are not typically well resolved by satellite gravity; abyssal hills and small seamounts (<2.5 km tall). We generate synthetic realizations of abyssal hills by combining the measured statistical properties of mapped abyssal hills with regional geology including fossil spreading rate/orientation, rms height from satellite gravity, and sediment thickness. With recent improvements in accuracy and resolution, it is now possible to detect all seamounts taller than about 800 m in satellite‐derived gravity and their location can be determined to an accuracy of better than 1 km. However, the width of the gravity anomaly is much greater than the actual width of the seamount so the seamount predicted from gravity will underestimate the true seamount height and overestimate its base dimension. In this study, we use the amplitude of the vertical gravity gradient (VGG) to estimate the mass of the seamount and then use their characteristic shape, based on well‐surveyed seamounts, to replace the smooth‐predicted seamount with a seamount having a more realistic shape. The deep seawater (>1,000 m) prevent any type of exploration from conventional satellite remote sensing. Echosounders aboard large vessels have mapped about 20% of the seafloor, however, vast areas in the southern hemisphere will not be mapped in our lifetimes. The deep ocean floor has more than 90% of the active volcanoes; hydrothermal circulation of seawater through the crust of the seafloor spreading ridges replenishes the nutrients needed for life on Earth. This study is an effort to fill the large gaps in seafloor coverage by creating a synthetic abyssal hill fabric using geological information such as the age of the seafloor, the spreading rate and direction when it formed, and the thickness of the sediments covering the original topography. In addition, we use the latest satellite‐derived gravity to estimate the locations and shapes of about 20,000 uncharted seamounts. The combination of mapped (20%) and synthetic (80%) topography is useful for modeling ocean circulation and ocean tides although it may give a false impression that 100% of the seafloor has been mapped. Accurate assessment of SARS‐CoV‐2 immunity is critical in evaluating vaccine efficacy and devising public health policies. Whilst the exact nature of effective immunity remains incompletely defined, SARS‐CoV‐2‐specific T‐cell responses are a critical feature that will likely form a key correlate of protection against COVID‐19. Here, we developed and optimized a high‐throughput whole blood‐based assay to determine the T‐cell response associated with prior SARS‐CoV‐2 infection and/or vaccination amongst 231 healthy donors and 68 cancer patients. Following overnight in vitro stimulation with SARS‐CoV‐2‐specific peptides, blood plasma samples were analysed for TH1‐type cytokines. Highly significant differential IFN‐γ+/IL‐2+ SARS‐CoV‐2‐specific T‐cell responses were seen amongst previously infected COVID‐19‐positive healthy donors in comparison with unknown / naïve individuals (p < 0·0001). IFN‐γ production was more effective at identifying asymptomatic donors, demonstrating higher sensitivity (96·0% vs. 83·3%) but lower specificity (84·4% vs. 92·5%) than measurement of IL‐2. A single COVID‐19 vaccine dose induced IFN‐γ and/or IL‐2 SARS‐CoV‐2‐specific T‐cell responses in 116 of 128 (90·6%) healthy donors, reducing significantly to 27 of 56 (48·2%) when measured in cancer patients (p < 0·0001). A second dose was sufficient to boost T‐cell responses in the majority (90·6%) of cancer patients, albeit IFN‐γ+ responses were still significantly lower overall than those induced in healthy donors (p = 0·034). Three‐month post‐vaccination T‐cell responses also declined at a faster rate in cancer patients. Overall, this cost‐effective standardizable test ensures accurate and comparable assessments of SARS‐CoV‐2‐specific T‐cell responses amenable to widespread population immunity testing, and identifies individuals at greater need of booster vaccinations. Marine multichannel and wide‐angle seismic data constrain crustal structure along a 530 km margin‐parallel transect of the Hikurangi subduction zone. The subducting Hikurangi Plateau crust (VP 5.0–7.4 km/s) is ∼1 km thicker (11 ± 1 km) and mantle velocities are ∼0.2 km/s higher (VP 8.3–8.5 km/s) beneath south/central Hikurangi relative to north Hikurangi. In the overthrusting plate, an abrupt 0.5 km/s south‐to‐north reduction in forearc wavespeeds occurs in concert with a change in seismic reflection character. We analyze legacy seismic data to show that the forearc transition likely reflects lateral variability in the updip extent of the Torlesse Backstop. Furthermore, we map this unit along‐strike and note a broad correlation between the backstop and down‐dip extent of shallow slow‐slip. We propose that the geological architecture of the overthrusting plate contributes to spatial variability in the location of shallow frictional transitions along the Hikurangi margin, impacting both seismic and tsunami hazard. To understand what factors impact subduction zone slip behavior, we analyze seismic data along a 530 km long transect spanning a transition from strong (south Hikurangi) to weak (central/north Hikurangi) interseismic locking. From south‐to‐north, we find that seismic wavespeeds in the overthrusting plate undergo an abrupt (∼10%) reduction, which coincides with a reduction in seismic reflectivity. We show that these changes likely reflect differences in the offshore (updip) extent of basement rocks within the forearc crust, which we map using seismic data. These maps also show that the offshore extent of basement rocks is broadly correlated with the maximum depth of shallow slow‐slip events. We propose that geological architecture of the overthrusting plate may contribute to spatial variability in megathrust slip‐behavior, impacting both seismic and tsunami hazard along the Hikurangi margin. Aim: This systematic review aimed to assess the outcomes of fistulotomy or fistulectomy and immediate sphincter repair (FISR) in relation to healing, incontinence and sphincter dehiscence both overall and in patients with high anal fistulae. Methods: Medline, Embase and The Cochrane library were searched for studies of patients undergoing FISR for anal fistula. Data regarding healing, continence and sphincter dehiscence were extracted overall and for high anal fistulae. The DerSimonian‐Laird random‐effects method was used for pooled analysis, heterogeneity between studies was assessed based on the significance of between‐study heterogeneity, and on the size of the I2 value. Risk of bias was assessed using the Cochrane risk of bias tool and the Newcastle‐Ottawa scale. Results: We identified 21 studies evaluating 1700 patients. Pooled analysis of healing reached 93% (95% CI: 91%–95%, I2 = 51% p‐value for heterogeneity = 0.004), with continence disturbance and worsening continence reaching 11% (95% CI: 6%–18%, I2 = 87% p < 0.001) and 8% (95% CI: 4%–13%, I2 = 74% p < 0.001), respectively. Subgroup analysis according to fistula height could only be conducted on limited data. Pooled healing in high anal fistulae was 89% (95% CI: 84%–94%, I2 = 76% p < 0.001), 16% suffered disturbance of continence (95% CI: 7%–27%, I2 = 89% p < 0.001), 8% worsening continence from baseline (95% CI: 2%–16%, I2 = 80% p < 0.001) and 2% suffered sphincter dehiscence (95% CI: 0%–10%, I2 = 89% p < 0.001). Conclusion: The evidence suggests FISR is a safe, effective procedure. However, data are limited by inconsistencies in reporting of continence and definition of fistula height, particularly high anal fistulae. Ecological community and ecosystem "red lists" have been developed in several jurisdictions to improve ecosystem‐level biodiversity protection. However, a challenge for the conservation and management of listed ecosystems is consistent identification in the field or from plot records. Ecosystem descriptions must have enough detail for positive identification but be broad enough that most instances are included. In many jurisdictions, descriptions are not supported by dichotomous keys or thresholds of ecosystem collapse and identification relies on the interpretation of trained individuals, with potential for opposing opinions. Using a structured process, we assessed the ability of experts to identify a critically endangered ecological community from vegetation plot samples. We compared the allocations made by experts with a numeric classification that underpinned the legal definition of the community. Overall, experts correctly identified the presence or absence of the community in 81% of samples although individual classification rates ranged from 63% to 94%. False positive rates varied among experts (7–50%) and experienced botanists did not necessarily perform better. Disturbance increased uncertainty and experts differed in their opinion about when the community had collapsed and was no longer recoverable. Inconsistent interpretation, in the absence of diagnostic keys and consensus models of collapse, will have implications for recovery and conservation of listed communities and ecosystems, and could impact the effectiveness of laws and policies designed to protect them. Aim: The aim of this work was to determine the factors associated with poor wound healing in patients with perianal Crohn's disease (pCD) who had undergone proctectomy in the era of biologic therapies. Method: Case record review was performed on 103 patients with pCD who underwent proctectomy at St Mark's Hospital, Harrow and the Western General Hospital, Edinburgh between 2005 and 2017. Healing rates at 6 and 12 months post‐proctectomy were considered; univariate analysis was performed. Results: Sixty out of 103 patients (58.3%) had failure of wound healing at 6 months and 41/103 (39.8%) at 12 months. In total, 63.1% (65/103) patients received biologic therapies prior to proctectomy; however, exposure to biologics was not a significant factor in predicting failure of wound healing at 12 months (infliximab p = 0.255; adalimumab p = 0.889; vedolizumab p = 0.153). Male gender was the only variable associated with poor wound healing at 12 months on univariate analysis (p = 0.017). A lower pre‐operative C‐reactive protein was associated with early wound healing at 6 months compared with at 12 months (p = 0.041) on univariate analysis. Other parameters not associated with rates of wound healing included smoking status, corticosteroid exposure, thiopurine exposure, number of previous biologics, perianal sepsis on MRI within the last 12 months, duration of CD prior to proctectomy and pre‐operative albumin. Conclusion: More than a third of patients had unhealed wounds 12 months after proctectomy. We report that unhealed wounds are more common in male patients. Sand and dust storms (SDS) are wind erosion events typically associated with dryland regions, although they can occur in most environments and their impacts are frequently experienced outside drylands because desert dust haze often is transported great distances. SDS represent hazards to society in numerous ways, yet they do not feature prominently in the disasters literature. This paper considers SDS in a hazard context by examining their ramifications in economic, physical, and social terms, with a focus on agriculture, health, transport, utilities, households, and the commercial and manufacturing sector. There are few assessments of the economic consequences of SDS and those studies that have been conducted lack consistency in data collection methods and analysis. SDS do not result in the significant damage to infrastructure usually associated with many disasters, but the cumulative effects on society can be significant because SDS occur more commonly than most other types of natural hazard. Aim: Discussions regarding sex after colorectal and pelvic floor surgery are often overlooked by clinicians. This is the first patient designed and delivered study to explore sexual function and practices after colorectal surgery. The aim was to explore the questions about sex that matter to patients and their partners following colorectal or pelvic floor surgery through a patient and public involvement survey. The results of this work will underpin the creation of a sex patient reported outcome measure. Methods: An anonymous online survey tool (Survey Monkey™) was disseminated via social media (Twitter, Facebook). Thematic analysis was applied to 130 free text comments posted by participants to identify key themes. Results: Some 632 individuals completed the survey. Most respondents were women (80% n = 507), 49.5% (n = 312) were married and 14% (n = 87) identified as LGBT+ (lesbian, gay, bisexual and transgender +). Indications for surgery varied: 34% were treated for ulcerative colitis (n = 214); 31% Crohn's (n = 196); 17% (n = 109) cancer; and 17% (n = 110) for perianal fistula. For patients who had a stoma formed (85%, n = 540), over half (51%, n = 324) lived with their stoma for 1–5 years. Respondents reported substantial alterations to their preferences for sexual positions, sexual activity and body confidence following surgery. Most respondents indicated that they were not offered advice about sex by a healthcare professional. Conclusions: The survey showed a substantial impact on the mechanics of sex following colorectal surgery. Few patients were offered preoperative information regarding sex, which has implications for informed consent. This study demonstrates a clear unmet need, voiced by patients, that open dialogue is necessary preoperatively to discuss sexual (dys)function. [ABSTRACT FROM AUTHOR]

Aim: In cases of prognostic uncertainty and equipoise as to the best management (prophylactic colectomy vs. surveillance) for dysplasia in inflammatory bowel disease (IBD), individualized discussion with the patient is required. Further understanding of patients' preferences is needed. Methods: A nationwide cross‐sectional survey was distributed to adult IBD patients who had never been diagnosed with dysplasia (dysplasia‐naïve) and those who had (dysplasia‐experienced). Risk perceptions and factors that influence management choices were explored. Results: There were 123 respondents. A substantial proportion (29%) of the dysplasia‐experienced respondents did not feel well informed about the associated cancer risk and/or its management by their clinical team. Contributing themes included contradictory advice and lack of personalized information regarding their cancer risk, alternative management options and impact on long‐term quality of life. Decisional regret and health‐related quality of life amongst those who chose either surveillance or surgery were comparable, but cancer‐related worry scores were elevated in the surveillance group. The dysplasia‐naïve respondents reported that they would only consider having a prophylactic colectomy if they had on average a 50% or even higher risk of developing cancer. On multivariable logistic regression analyses, predictors of colectomy or surveillance preference included ethnicity, personality traits such as health locus of control (whether health status is influenced by luck) and differences in perception of what a low risk of cancer is. Conclusions: This study identifies predictive factors that can influence decision‐making and satisfaction with the counselling process when IBD dysplasia is diagnosed. The segmentation of mid‐ocean ridges is a defining characteristic of seafloor spreading, yet some tectonic processes operating at segment boundaries remain poorly understood. Here, we analyze new satellite‐derived vertical gravity gradient (VGG) data, which reveal an abundance of off‐axis seafloor features that are oblique to ridges and transform faults and thus reflect the occurrence of ridge propagation at some segment boundaries. However, unlike many propagating ridges, these features commonly reverse direction leaving W‐shaped signatures in the seafloor which we refer to as "seesaw propagators" (SSPs). Using the VGG, we have documented these globally and find that: (1) SSPs are ubiquitous on seafloor that formed at half spreading rates between 10 and 40 mm year−1 and their total length is about 1/3 that of fracture zones. (2) The lithospheric age offset across SSPs (0–2.5 Ma) is less than transform faults (2–10 Ma), which likely reflects a rheological threshold, whereby only young and weak lithosphere allows for "non‐rigid" SSP behavior. (3) Isostatic modeling of well‐surveyed SSPs confirms that they formed on young and thin (3 km) lithosphere. (4) The directional changes of SSPs in both time and space appear largely uncorrelated and cannot be explained by previous regional‐scale models invoked to explain unidirectional ridge propagation and thus require a different driving force. Aim: There is a requirement for an expansive and up to date review of the management of emergency colorectal conditions seen in adults. The primary objective is to provide detailed evidence‐based guidelines for the target audience of general and colorectal surgeons who are responsible for an adult population and who practise in Great Britain and Ireland. Methods: Surgeons who are elected members of the Association of Coloproctology of Great Britain and Ireland Emergency Surgery Subcommittee were invited to contribute various sections to the guidelines. They were directed to produce a pathology‐based document using literature searches that were systematic, comprehensible, transparent and reproducible. Levels of evidence were graded. Each author was asked to provide a set of recommendations which were evidence‐based and unambiguous. These recommendations were submitted to the whole guideline group and scored. They were then refined and submitted to a second vote. Only those that achieved >80% consensus at level 5 (strongly agree) or level 4 (agree) after two votes were included in the guidelines. Results: All aspects of care (excluding abdominal trauma) for emergency colorectal conditions have been included along with 122 recommendations for management. Conclusion: These guidelines provide an up to date and evidence‐based summary of the current surgical knowledge in the management of emergency colorectal conditions and should serve as practical text for clinicians managing colorectal conditions in the emergency setting. Earth's surface topography/bathymetry and gravity fields provide important constraints on crustal structure and the tectonic processes that act on it due, for example, to plate flexure and mantle convection. Such studies require, however, high accuracy measurements at a wide range of spatial scales. During the past few decades much progress has been made in the acquisition of bathymetry and gravity data using both shipboard and satellite altimeter methods. Surprisingly, there have been few comparisons of these data. During April–June, 2019 we had the opportunity onboard a R/V Marcus G. Langseth cruise in the northwest Pacific Ocean to compare data acquired with an EM122 Kongsberg swath bathymetry system and a refurbished Bell Aerospace BGM‐3 gravimeter with the most recent global bathymetry and gravity fields. We find that while the recovery of bathymetry and gravity from satellite radar altimeter data in areas of sparse shipboard data has been impressive, root mean square discrepancies in the range 175.5–303.4 m and 2.6–6.3 mGal exist between shipboard and satellite‐derived data. While these discrepancies are small, they are highly correlated and therefore have implications for the density structure, rock type and geological processes occurring on the deep seafloor. Shipboard data should continue to be acquired, especially over features such as seamounts, banks, and ridges that are associated with short wavelength (<25 km wavelength) bathymetric and gravimetric features beyond that is recoverable in satellite‐derived data. Aim: Restorative proctocolectomy has been widely adopted as the procedure of choice for restoring gastrointestinal continuity following proctocolectomy. It is often associated with improved quality of life and high patient satisfaction; however, the development of a pouch anal fistula can cause significant morbidity. Pouch fistulas are notoriously difficult to treat and there is great heterogeneity in the management reported of these fistulas. A lack of classification, and the assumption that fistulas originating from completely different aetiologies will behave and respond similarly to a particular treatment strategy, precludes meaningful comparison of management outcomes. We aim to introduce consistency in the reporting of pouch fistulas using a novel classification system. Methods: A consensus process involving clinicians experienced in the management of pouch fistulas from two high volume tertiary centres was performed. Results: We propose that pouch anal fistulas should be classified into four distinct groups according to their aetiology: group 1, anastomotic related; group 2, inflammatory bowel disease related, with sub‐classifications Crohn's (type A) and non‐Crohn's (type B) in origin; group 3, cryptoglandular related; and group 4, malignancy related. Conclusion: Classification of pouch fistulas according to their aetiology will provide consistency in the literature and improve the quality of prospective evidence for the management of pouch fistulas. Models of bathymetry derived from satellite radar altimetry are essential for modeling many marine processes. They are affected by uncertainties which require quantification. We propose an uncertainty model that assumes errors are caused by the lack of high‐wavenumber content within the altimetry data. The model is then applied to a tsunami hazard assessment. We build a bathymetry uncertainty model for northern Chile. Statistical properties of the altimetry‐predicted bathymetry error are obtained using multibeam data. We find that a Von Karman correlation function and a Laplacian marginal distribution can be used to define an uncertainty model based on a random field. We also propose a method for generating synthetic bathymetry samples conditional to shipboard measurements. The method is further extended to account for interpolation uncertainties, when bathymetry data resolution is finer than ∼10 km. We illustrate the usefulness of the method by quantifying the bathymetry‐induced uncertainty of a tsunami hazard estimate. We demonstrate that tsunami leading wave predictions at middle/near field tide gauges and buoys are insensitive to bathymetry uncertainties in Chile. This result implies that tsunami early warning approaches can take full advantage of altimetry‐predicted bathymetry in numerical simulations. Finally, we evaluate the feasibility of modeling uncertainties in regions without multibeam data by assessing the bathymetry error statistics of 15 globally distributed regions. We find that a general Von Karman correlation and a Laplacian marginal distribution can serve as a first‐order approximation. The standard deviation of the uncertainty random field model varies regionally and is estimated from a proposed scaling law. Q fever, caused by the zoonotic bacterium Coxiella burnetii, is a globally distributed emerging infectious disease. Livestock are the most important zoonotic transmission sources, yet infection in people without livestock exposure is common. Identifying potential exposure pathways is necessary to design effective interventions and aid outbreak prevention. We used natural language processing and graphical network methods to provide insights into how Q fever notifications are associated with variation in patient occupations or lifestyles. Using an 18‐year time‐series of Q fever notifications in Queensland, Australia, we used topic models to test whether compositions of patient answers to follow‐up exposure questionnaires varied between demographic groups or across geographical areas. To determine heterogeneity in possible zoonotic exposures, we explored patterns of livestock and game animal co‐exposures using Markov Random Fields models. Finally, to identify possible correlates of Q fever case severity, we modelled patient probabilities of being hospitalized as a function of particular exposures. Different demographic groups consistently reported distinct sets of exposure terms and were concentrated in different areas of the state, suggesting the presence of multiple transmission pathways. Macropod exposure was commonly reported among Q fever cases, even when exposure to cattle, sheep or goats was absent. Males, older patients and those that reported macropod exposure were more likely to be hospitalized due to Q fever infection. Our study indicates that follow‐up surveillance combined with text modelling is useful for unravelling exposure pathways in the battle to reduce Q fever incidence and associated morbidity. Background: Introduction: The prognosis of dysplasia in patients with IBD is largely determined from observational studies from the pre‐videoendoscopic era (pre‐1990s) that does not reflect recent advances in endoscopic imaging and resection. Aims: To better understand the risk of synchronous colorectal cancer and metachronous advanced neoplasia (ie high‐grade dysplasia or cancer) associated with dysplasia diagnosed in the videoendoscopic era, and to stratify risk according to a lesion's morphology, endoscopic resection status or whether it was incidentally detected on biopsy of macroscopically normal colonic mucosa (ie invisible). Methods: A systematic search of original articles published between 1990 and February 2020 was performed. Eligible studies reported on incidence of advanced neoplasia at follow‐up colectomy or colonoscopy for IBD‐dysplasia patients. Quantitative and qualitative analyses were performed. Results: Thirty‐three studies were eligible for qualitative analysis (five for the meta‐analysis). Pooled estimated proportions of incidental synchronous cancers found at colectomy performed for a pre‐operative diagnosis of visible high‐grade dysplasia, invisible high‐grade dysplasia, visible low‐grade dysplasia and invisible low‐grade dysplasia were 13.7% (95% CI 0.0‐54.1), 11.4% (95% CI 4.6‐20.3), 2.7% (95% CI 0.0‐7.1) and 2.4% (95% CI 0.0‐8.5) respectively. The lowest incidences of metachronous advanced neoplasia, for dysplasia not managed with immediate colectomy but followed up with surveillance, tended to be reported by the studies where high definition imaging and/or chromoendoscopy was used and endoscopic resection of visible dysplasia was histologically confirmed. Conclusions: The prognosis of IBD‐dysplasia diagnosed in the videoendoscopic era appears to have been improved but the quality of evidence remains low. Populations of marsh‐breeding birds have declined throughout the southern Laurentian Great Lakes basin. To advance conservation of these species, we used occupancy modeling, a regional prioritization scheme, and data from Birds Canada's Great Lakes Marsh Monitoring Program (2016–2018) to describe species‐habitat relationships and identify priority habitat areas for 7 obligate marsh‐breeding bird species in southern Ontario, Canada: American bittern (Botaurus lentiginosus), common gallinule (Gallinula galeata), least bittern (Ixobrychus exilis), marsh wren (Cistothorus palustris), pied‐billed grebe (Podilymbus podiceps), sora (Porzana carolina), and Virginia rail (Rallus limicola). Given these species respond to land cover at widely varying spatial scales, we initially identified the most informative scale (buffer = 100 m, 200 m, 400 m, 800 m, 1,600 m, 3,200 m, or 6,400 m) for marsh, urban, agricultural, and forest cover to increase model performance. We also considered climate variables, whether sample sites were along a Great Lakes coastline or inland, and covariates influencing detection. Occupancy was best explained by land cover at a wide range of spatial scales depending on the species. All species except Virginia rail responded positively to marsh cover; American bittern and Virginia rail responded negatively to urban cover; least bittern, pied‐billed grebe, and Virginia rail responded negatively and sora responded positively to agricultural cover; and American bittern, common gallinule, marsh wren, and pied‐billed grebe responded negatively and Virginia rail responded positively to forest cover. Only American bittern responded negatively to mean May–June temperature; only pied‐billed grebe responded positively to start of growing season; and only Virginia rail had higher occupancy at inland marshes compared to coastal. We combined predictions from the best model for each of 5 species with reasonably good model fit (we excluded sora and Virginia rail) to identify priority habitat areas for marsh‐breeding birds. Expansion of wetland conservation work from existing priority areas based on waterfowl to also include these new additional priority areas based on marsh‐breeding birds will be an important step towards conservation of all birds, and will help slow or maybe even reverse declining population trends. 5‐fluorouracil (5‐FU) is an anticancer drug, which inhibits human thymidine phosphorylase (hTP) and plays a key role in maintaining the process of DNA replication and repair. It is involved in regulating pyrimidine nucleotide production, by which it inhibits the mechanism of cell proliferation and cancerous tumor growth. However, up to 80% of the administered drug is metabolized by dihydropyrimidine dehydrogenase (DPD). This work compares binding of 5‐FU and its analogues to hTP and DPD, and suggests strategies to reduce drug binding to DPD to decrease the required dose of 5‐FU. An important feature between the proteins studied here was the difference of charge distribution in their binding sites, which can be exploited for designing drugs to selectively bind to the hTP. The 5‐FU presence was thought to be required for a closed conformation. Comparison of the calculation results pertaining to unliganded and liganded protein showed that hTP could still undergo open–closed conformations in the absence of the ligand; however, the presence of a positively charged ligand better stabilizes the closed conformation and rigidifies the core region of the protein more than unliganded or neutral liganded system. The study has also shown that one of the three hinge segments linking the two major α and α/β domains of the hTP is an important contributing factor to the enzyme's open–close conformational twist during its inactivation–activation process. In addition, the angle between the α/β‐domain and the α‐domain has shown to undergo wide rotations over the course of MD simulation in the absence of a phosphate, suggesting that it contributes to the stabilization of the closed conformation of the hTP. The distribution of the Old World screw‐worm fly Chrysomya bezziana Villeneuve and the incidence of traumatic myiases (strikes) in livestock, humans and wild animals in Papua New Guinea (PNG) for the period 1973–1985 is described as a foundation for current considerations of risk to Australia. Field studies indicated myiasis caused by C. bezziana occurred throughout PNG to an altitude of 2590 m. Cattle and sheep were the most commonly recorded hosts. The impact of strikes on individual cattle properties varied from negligible levels to every animal being infested on average each year. The incidence of strikes was apparently related to vegetation cover with fewer strikes in open grassland and more strikes in wooded areas. There was no statistical correlation between rainfall and strike incidence although most properties reported that the number of strikes by C. bezziana was greatest after periods of rain. On a mixed‐breed sheep property in the highlands, Romneys suffered an annual strike rate of 20% compared with 9% in Perendales and 7% in Corriedales. The majority of strikes in sheep were in the vulva and anus region (59% of total). Repeated vulva strike in ewes resulted in reduced fertility such that annual lambing rates were reduced by 66% after three strikes and no lambs were produced after five consecutive strikes. Management practices such as dehorning, castration, tail docking and ear tagging frequently resulted in strikes. Deaths of livestock attributed directly to myiases from C. bezziana infestations were common. The incidence of C. bezziana infestations in wildlife and humans in PNG is documented. The results are discussed in relation to risks of its entry to Australia and potential impact of C. bezziana on the livestock industries and wildlife if C. bezziana was accidentally introduced and became established. An updated global bathymetry and topography grid is presented using a spatial sampling interval of 15 arc sec. The bathymetry is produced using a combination of shipboard soundings and depths predicted using satellite altimetry. New data consists of >33.6 million multibeam and singlebeam measurements collated by several institutions, namely, the National Geospatial‐Intelligence Agency, Japan Agency for Marine‐Earth Science and Technology, Geoscience Australia, Center for Coastal and Ocean Mapping, and Scripps Institution of Oceanography. New altimetry data consists of 48, 14, and 12 months of retracked range measurements from Cryosat‐2, SARAL/AltiKa, and Jason‐2, respectively. With respect to SRTM15_PLUS (Olson et al.,), the inclusion of these new data results in a ∼1.4‐km improvement in the minimum wavelength recovered for sea surface free‐air gravity anomalies, a small increase in the accuracy of altimetrically derived predicted depths, and a 1.24% increase, from 9.60% to 10.84%, in the total area of ocean floor that is constrained by shipboard soundings at 15‐arc sec resolution. Bathymetric grid cells constrained by satellite altimetry have estimated uncertainties of ±150 m in the deep oceans and ±180 m between coastlines and the continental rise. Onshore, topography data are sourced from previously published digital elevation models, predominately SRTM‐CGIAR V4.1 between 60°N and 60°S. ArcticDEM is used above 60°N, while Reference Elevation Model of Antarctica is used below 62°S. Auxiliary grids illustrating shipboard data coverage, marine free‐air gravity anomalies, and vertical gradient gradients are also provided in common data formats. Aim: The human parechovirus (HPeV) has emerged as a pathogen causing sepsis-like presentations in young infants, but there is a lack of data on HPeV presentations requiring intensive care support. We aimed to characterise the clinical presentation, disease severity, management and outcome of a population-based cohort of children with microbiologically confirmed HPeV infection requiring admission to paediatric intensive care units (PICUs) in Queensland, Australia during a recent outbreak.Methods: This was a multicentre retrospective study of children admitted to PICU between 1 January 2015 and 31 December 2016 with confirmed HPeV infection.Results: Thirty infants (median age 20 days) with HPeV genotype 3 were admitted to PICU, representing 16% of all children with HPeV admitted to hospital and 6.4% of non-elective PICU admissions in children <1 year of age. Children requiring PICU admission were younger than children admitted to hospital (P = 0.001). Apnoea, haemodynamic instability with tachycardia and seizures represented the main reasons for PICU admission. Eleven children (37%) required mechanical ventilation for a median duration of 62 h, 22 (73%) received fluid boluses and 7 (23%) were treated with vasoactive agents for a median duration of 53 h. Median length of stay was 2.62 days. A total of 24 children (80%) fulfilled sepsis criteria, 14 (47%) severe sepsis and 7 (23%) septic shock criteria. Eight (27%) had abnormal brain magnetic resonance imaging. No patient died.Conclusions: We confirm that HPeV infection is an important cause of sepsis-like syndrome in infants with substantial associated morbidity. Summary: Almost all plant species interact with one or more symbioses somewhere within their distribution range.Bringing together plant trait data and growth responses to symbioses spanning 552 plant species, we provide for the first time on a large scale (597 studies) a quantitative synthesis on plant performance differences between eight major types of symbiosis, including mycorrhizas, N‐fixing bacteria, fungal endophytes and ant–plant interactions.Frequency distributions of plant growth responses varied considerably between different types of symbiosis, in terms of both mean effect and 'risk', defined here as percentage of experiments reporting a negative effect of symbiosis on plants. Contrary to expectation, plant traits were poor predictors of growth response across and within all eight symbiotic associations. Our analysis showed no systematic additive effect when a host plant engaged in two functionally different symbioses.This synthesis suggests that plant species' ecological strategies have little effect in determining the influence of a symbiosis on host plant growth. Reliable quantification of differences in plant performance across symbioses will prove valuable for developing general hypotheses on how species become engaged in mutualisms without a guarantee of net returns. Plant growth rates strongly determine ecosystem productivity and are a central element of plant ecological strategies. For laboratory and glasshouse‐grown seedlings, specific leaf area (SLA; ratio of leaf area to mass) is a key driver of interspecific variation in growth rate (GR). Consequently, SLA is often assumed to drive GR variation in field‐grown adult plants. However, there is an increasing evidence that this is not the general case. This suggests that GR – SLA relationships (and perhaps those for other traits) may vary depending on the age or size of the plants being studied. Here we investigated GR – trait relationships and their size dependence among 17 woody species from an open‐canopy, fire‐prone savanna in northern Australia. We tested the predictions that SLA and stem diameter growth rate would be positively correlated in saplings but unrelated in adults while, in both age classes, faster‐GR species would have higher light‐saturated photosynthetic rate (Asat), higher leaf nutrient concentrations, higher branch‐scale biomass allocation to leaf versus stem tissues and lower wood density (WD). SLA showed no relationship to stem diameter GR, even in saplings, and the same was true of leaf N and P concentrations, and WD. However, branch‐scale leaf:stem allocation was strongly related to GR in both age groups, as was Asat. Together, these two traits accounted for up to 80% of interspecific variation in adult GR, and 41% of sapling GR. Asat is rarely measured in field‐based GR studies, and this is the first report of branch‐scale leaf:stem allocation (analogous to a benefit:cost ratio) in relation to plant growth rate. Objectives: We sought to determine whether US‐guided lumbar puncture reduced the rate of lumbar puncture failures for providers at an academic teaching hospital with variable lumbar puncture and US experience compared to the traditional landmark‐based technique. Methods: We conducted a prospective randomized controlled trial to compare US‐guided lumbar puncture to the traditional landmark technique in an academic emergency department. Thirty‐five patients were randomized to either have their lumbar puncture performed either via the landmark or US‐guided technique. All procedures were completed by an emergency medicine resident with variable lumbar puncture and US experience. Procedural failures, the number of attempts, the time to completion, and patient pain scores were all recorded. Results: The adjusted odds ratio of successfully obtaining cerebrospinal fluid (CSF) in the US‐guided lumbar puncture group was 2.31 compared to the landmark‐based lumbar puncture group (P = .377). It took 1.54 times more attempts to obtain CSF in the landmark group as it did in the US group (P = .046). It seemed to have no effect on postprocedural pain or the time to obtain CSF. Conclusions: The use of US guidance to assist in lumbar punctures did not improve the procedural success rate over traditional landmark techniques in an academic setting with novice providers. Although using US for procedural guidance significantly decreased the number of attempts, it seemed to have no effect on postprocedural pain or the time to obtain CSF. [ABSTRACT FROM AUTHOR]

Summary: Background: Evidence from groups who have studied fistula aetiology and extrapolation from interventional studies supports a multifactorial hypothesis of Crohn's perianal fistula, with several pathophysiological elements that may contribute to fistula formation, persistence and resistance to treatment. Aim: An evidence synthesis of current understanding of pathophysiological factors underlying Crohn's perianal fistula is presented, exploring the fundamental reasons why some treatments succeed and others fail, as a means of focussing clinical knowledge on improving treatment of Crohn's perianal fistula. Methods: Evidence to support this review was gathered via the Pubmed database. Studies discussing pathophysiological factors underpinning perianal fistula, particularly in Crohn's disease, were reviewed and cross‐referenced for additional reports. Results: Pathophysiological factors that impact on success or failure of interventions for Crohn's perianal fistulae include the high‐pressure zone, obliterating the dead space, disconnecting the track from the anus, removing epithelialisation, eradicating sepsis and by‐products of bacterial colonisation, correcting abnormalities in wound repair and removing the pro‐inflammatory environment which allows fistula persistence. Most current interventions for Crohn's perianal fistulae tend to focus on a single, or at best two, aspects of the pathophysiology of Crohn's anal fistulae; as a result, failure to heal fully is common. Conclusions: For an intervention or combination of interventions to succeed, multiple factors must be addressed. We hypothesise that correct, timely and complete attention to all of these factors in a multimodal approach represents a new direction that may enable the creation of an effective treatment algorithm for Crohn's anal fistula. ABSTRACT: Background and Objective: Respiratory syncytial virus (RSV) is the most significant cause of acute respiratory infection (ARI) in early life. RSV and other respiratory viruses are known to stimulate substantial outgrowth of potentially pathogenic bacteria in the upper airways of young children. However, the clinical significance of interactions between viruses and bacteria is currently unclear. The present study aimed to clarify the effect of viral and bacterial co‐detections on disease severity during paediatric ARI. Methods: Nasopharyngeal aspirates from children under 2 years of age presenting with ARI to the emergency department were screened by quantitative PCR for 17 respiratory viruses and the bacterial pathogens Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis. Associations between pathogen detection and clinical measures of disease severity were investigated. Results: RSV was the most common virus detected, present in 29 of 58 samples from children with ARI (50%). Detection of S. pneumoniae was significantly more frequent during RSV infections compared to other respiratory viruses (adjusted effect size: 1.8, P: 0.03), and co‐detection of both pathogens was associated with higher clinical disease severity scores (adjusted effect size: 1.2, P: 0.03). Conclusion: Co‐detection of RSV and S. pneumoniae in the nasopharynx was associated with more severe ARI, suggesting that S. pneumoniae colonization plays a pathogenic role in young children. Abstract: The rate of elongation and thickening of individual branches (shoots) varies across plant species. This variation is important for the outcome of competition and other plant–plant interactions. Here, we compared rates of shoot growth across 44 species from tropical, warm temperate and cool temperate forests of eastern Australia. Shoot growth rate was found to correlate with a suite of traits including the potential height of the species, xylem‐specific conductivity, leaf size, leaf area per xylem cross‐section (LA/XA), twig diameter (at 40 cm length), wood density and modulus of elasticity. Within this suite of traits, maximum plant height was the clearest correlate of growth rates, explaining 50%–67% of the variation in growth overall (p < .0001), and 23%–32% of the variation (p < .05) in growth when holding the influence of the other traits constant. Structural equation models suggest that traits associated with “hydraulics,” “biomechanics” and the “leaf economics spectrum” represent three clearly separated axes of variation, with the hydraulic axis exhibiting the strongest alignment with height and largest independent contribution to growth (in the case of branch thickening). However, most of the capacity of these axes to predict growth was also associated with maximum height, presumably reflecting coordinated selection on multiple traits that together influence life histories. Growth rates were not strongly correlated with leaf nitrogen or leaf mass per unit leaf area. Correlations between growth and maximum height arose both across latitude (47%, p < .0001) and from within‐site differences between species (30%, p < .0001). Covariation between growth and maximum height was driven in part by variation in irradiance across sites as well as among canopy positions within sites (23%, p < .0001). A significant fraction of this shared variation was independent of irradiance (45%, p < .0001), reflecting intrinsic differences across species and sites.
