Treat-to-target (T2T) recommendations for the use of systemic therapies (including biologics) in patients with moderate-to-severe plaque psoriasis have been published by a few groups of experts worldwide. However, there remains considerable variability in the choice of target severity measure and timing of milestones. To develop consensus recommendations for implementing T2T strategies for the management of moderate-to-severe plaque psoriasis using biologics. An expert group of Canadian dermatologists (the Committee) convened to develop a T2T consensus statement. They held a virtual meeting during which a preliminary set of criteria was created. These criteria were then reviewed, modified, and recirculated until unanimous agreement was achieved. The Committee agreed that defining treatment target is multidimensional and should reflect objective severity measures, as well as clinician and patient-reported outcomes. The Committee unanimously proposes a criterion-based system for determining the achievement of treatment target. The proposed T2T approach presented here provides a clinical framework for defining treatment success, measuring progress toward treatment success, recognizing when treatment modifications are warranted, and recommending treatment optimization strategies., (© 2022 Wiley Periodicals LLC.)

Introduction: Drug checking services for harm reduction and overdose prevention have been implemented in many jurisdictions as a public health intervention in response to the opioid overdose crisis. This study demonstrates the first on-site use of paper spray mass spectrometry for quantitative drug checking to address the limitations of current on-site drug testing technologies., Methods: Paper spray mass spectrometry was used to provide on-site drug checking services at a supervised consumption site in the Downtown Eastside of Vancouver, British Columbia, Canada during a 2-day pilot test in August 2019. The method included the targeted quantitative measurement of 49 drugs and an untargeted full scan to assist in identifying unknown/unexpected components., Results: During the pilot, 113 samples were submitted for analysis, with 88 (78%) containing the client expected substance. Fentanyl was detected in 45 of 59 expected fentanyl samples, and in 50 (44%) samples overall at a median concentration of 3.6% (w/w%). The synthetic precursor of fentanyl, 4-anilino-N-phenethyl-piperidine (4-ANPP), was found in 74.0% of all fentanyl samples at a median concentration of 2.2%, suggesting widespread poor manufacturing practices. Etizolam was detected in 10 submitted samples anticipated to be fentanyl at a median concentration of 2.5%. No clients submitting these samples expected etizolam or a benzodiazepine in their sample. In three instances, it was co-measured with fentanyl, and in seven cases it was detected alone., Discussion and Conclusions: The quantitative capabilities and low detection limits demonstrated by paper spray mass spectrometry offer distinct benefits over existing on-site drug checking methods and harm reduction services., (© 2021 The Authors. This paper focuses on the contradictory nature and sometimes unintended consequences of workers' efforts to defend particular communities against the ravages of capital restructuring. In the past decade, pattern collective bargaining in the highly unionized British Columbia pulp and paper industry has faced enormous strains due to intense industry restructuring. Our analysis focuses on the repercussions of actions taken by union locals in two British Columbia towns-Port Alice and Port Alberni-to try to secure the survival of their pulp and paper mills and, even in the case of Port Alice, the continued existence of the community. Our analysis resonates with recent debates surrounding worker agency as well as writing in the 1980s which addressed the often contradictory and problematic nature of workers' struggles to 'defend place'; writing largely neglected in more recent work in labour geography. Abstract. An econometric model of the Canadian pulp and paper industry, formulated specifically to simulate the effects of changes in production costs due to mandatory pollution controls, is specified and estimated. Differing structures are assumed for each of the newsprint, woodpulp export and other paper and board markets. Simulation and sensitivity tests are reported. The model consistently demonstrates low sensitivity to outside shocks, with less than unitary elasticities of major variables with respect to real income changes, unit factor requirement changes, and exchange rate variation. As part of a larger survey on cumulative effects within the Saint John River basin (Canada), a fish survey was conducted near Edmundston (NB, Canada) in the fall of 1999 using slimy sculpin (Cottus cognatus) and white sucker (Catostomus commersoni). The discharge environment receives effluent from the pulp mill, a paper mill, three sewage discharges, and tributaries receiving agricultural runoff. Sculpin collected downstream of the sewage discharges and pulp mill effluent had greater growth, condition, and liver size but no significant differences in gonad size. Stable isotope data indicated slimy sculpin did not move between sites. Female sculpin collected downstream of the paper mill showed no significant differences in length, body weight, age, condition factor, liver size, and gonad size compared to fish from reference sites. Female white sucker collected downstream of the pulp mill did not differ significantly in any measured parameter compared to reference fish. Liver sizes of white sucker from the Saint John River were outside the range considered to be indicative of uncontaminated riverine sites. In 2000, sculpin collected downstream from a poultry-processing facility had larger livers and lower condition factors, suggesting that the site is contaminated. We found no significant differences in sculpin length, weight, condition (except for males), and liver size in sculpin collected downstream from the pulp mill in October 2001. The responses of slimy sculpin and white sucker differed, perhaps in relation to differences in life history characteristics. Results from this study indicate the slimy sculpin is a suitable fish species for monitoring rivers that receive multiple industrial and municipal effluents. [ABSTRACT FROM AUTHOR]

The Canadian environmental effects monitoring (EEM) program is a regulated, cyclical, industry-funded program designed to determine whether receiving water impacts exist when a mill is in compliance with its discharge limits. The results from three cycles of the fish monitoring program (1992 to 2004) are available from over 200 surveys of fish compared between sites located upstream and downstream of pulp and paper mill effluent outfalls. Previous meta-analyses have shown a national average response pattern across cycles characterized by an increase in endpoints measuring energy storage and growth and a decrease in a reproductive endpoint, consistent with a response of nutrient enrichment in combination with some form of metabolic disruption. Although the national average pattern of effects was temporally consistent, there was some variability in the magnitude of effects among cycles. Questions were raised as to whether the intercycle variability was due to changes in effluent quality or due, at least in part, to other factors. The present study compares responses over the first three cycles, and shows that the choice of sentinel species is likely to be a major contributing factor to the variability in observed effects. Subset analyses using studies from mills that used the same sentinel species across cycles reveal fairly uniform responses and little evidence of significant improvements in overall fish health from cycles one to three. However, a meta-analysis using 1991 data collected from 10 mills before the implementation of the EEM program and data from the same mills collected during cycles one to three of the program reveal significantly reduced effects on relative liver weight and potential improvements in other endpoints.
