Background: The implementation science literature acknowledges a need for engagement of key stakeholders when designing, delivering and evaluating implementation work. To date, the literature reports minimal or focused stakeholder engagement, where stakeholders are engaged in either barrier identification and/or barrier prioritisation. This paper begins to answer calls from the literature for the development of tools and guidance to support comprehensive stakeholder engagement in implementation research and practice. The paper describes the systematic development of the Implementation‐STakeholder Engagement Model (I‐STEM) in the context of an international, large‐scale empirical implementation study (ImpleMentAll) aimed at evaluating the effectiveness of a tailored implementation toolkit. The I‐STEM is a sensitising tool that defines key considerations and activities for undertaking stakeholder engagement activities across an implementation process. Methods: In‐depth, semistructured interviews and observations were conducted with implementers who were tailoring implementation strategies to integrate and embed internet‐based cognitive behavioural therapy (iCBT) services in 12 routine mental health care organisations in nine countries in Europe and Australia. The analytical process was informed by principles of first‐ and third‐generation Grounded Theory, including constant comparative method. Results: We conducted 55 interviews and observed 19 implementation‐related activities (e.g., team meetings and technical support calls). The final outcome of our analysis is expressed in an initial version of the I‐STEM, consisting of five interrelated concepts: engagement objectives, stakeholder mapping, engagement approaches, engagement qualities and engagement outcomes. Engagement objectives are goals that implementers plan to achieve by working with stakeholders in the implementation process. Stakeholder mapping involves identifying a range of organisations, groups or people who may be instrumental in achieving the engagement objectives. Engagement approaches define the type of work that is undertaken with stakeholders to achieve the engagement objectives. Engagement qualities define the logistics of the engagement approach. Lastly, every engagement activity may result in a range of engagement outcomes. Conclusion: The I‐STEM represents potential avenues for substantial stakeholder engagement activity across key phases of an implementation process. It provides a conceptual model for the planning, delivery, evaluation and reporting of stakeholder engagement activities. The I‐STEM is nonprescriptive and highlights the importance of a flexible, iterative approach to stakeholder engagement. It is developmental and will require application and validation across a range of implementation activities. GAMIAN‐Europe brings together a wide variety of patient representation organisations (local, regional and national) from almost all European countries. GAMIAN‐Europe was involved in pilot testing the ItFits‐toolkit and provided their views on the various aspects, including stakeholder engagement. Patients were also represented in the external advisory board providing support and advice on the design, conduct and interpretation of the wider project, including the development of the ItFits‐toolkit. Issues: Drug consumptions rooms (DCR) and supervised injecting facilities (SIF) are expanding internationally. Previous reviews have not systematically addressed evaluation methodologies.Approach: Results from systematic searches of scientific databases in English until June 2017 were coded for paper type, country and year of publication. For evaluation papers, study outcome, methodology/study design and main indicators of DCR/SIF 'exposure' were recorded.Key Findings: Two hundred and nineteen eligible peer-reviewed papers were published since 1999: the majority from Canada (n = 117 papers), Europe (n = 36) and Australia (n = 32). Fifty-six papers reported evaluation outcomes. Ecological study designs (n = 10) were used to assess the impact on overdose, public nuisance and crime; modelling techniques (n = 6) estimated impact on blood-borne diseases, overdose deaths and costs. Papers using individual-level data included four prospective cohorts (n = 28), cross-sectional surveys (n = 7) and service records (n = 5). Individual-level data were used to assess safer injecting practice, uptake into health and social services and all the other above outcomes except for impact on crime and costs. Four different indicators of DCR/SIF attendance were used to measure service 'exposure'.Implications: Research around DCRs/SIFs has used ecological, modelling, cross-sectional and cohort study designs. Further research could involve systematic inclusion of a control group of people who are eligible but do not access SIFs, validation of self-reported proportion of injections at SIFs or a stepped-wedge or a cluster trial comparing localities.Conclusions: Methodologies appropriate for DCR/SIF evaluation have been established and can be readily replicated from the existing literature. The analysis of international and collaborative publication trends in prominent behavior‐analytic journals has been a topic of interest for behavioral researchers. This paper focuses on publication trends from 1997 through 2020 in three prominent journals: Journal of the Experimental Analysis of Behavior (JEAB), Journal of Applied Behavior Analysis (JABA), and Perspectives on Behavior Science (PBS). The variable of interest was the percentage of articles published per geographical category—Australasia/East Asia, Europe, Latin America, Middle East, North America, and Africa. The results showed that 79, 96, and 87% of the published articles in JEAB, JABA, and PBS, respectively, were conducted by researchers with a North American affiliation. Furthermore, 12, 4, and 4% of the articles in JEAB, JABA, and PBS, respectively, were coauthored by at least two researchers from different geographical categories. [ABSTRACT FROM AUTHOR]

It is commonly recognized that social exclusion is a dynamic process, but it operates within well-established political, social and economic structures. A breadth of empirical data is used to explore various dimensions of social exclusion and inclusion, including Australian, Canadian and European data. While all the articles explore different aspects of social exclusion, common themes emerge. All authors note the multi-dimensional nature of social exclusion. The importance of social networks in providing information and advice to the integration of young people is also identified. Common to all articles though is the firm belief that national governments should be centrally involved in tackling social exclusion. As currently formulated, the extent of government commitment to rural development programmes is questionable. These programmes tend to be short-term with insecure and limited funding. At any rate, the capacity of local partnerships to address issues of social exclusion is restricted. The responsibility for economic, social and civic inclusion of citizens rests primarily with national governments.

The article examines several papers relating to public policy schools and programs around the world. According to the author, the paper focused on six emerging programs that had representatives present, including four that usefully supplemented the conference paper's survey of broad trends in Canada, Europe, Australia and New Zealand. In addition, the programs examined have complex relationship with the demand side of policy education. Furthermore, a table showing a descriptive overview of some of the programs discussed is offered.

In the decades following the Second World War super markets and retail outlets in North America, Western Europe, and Australasia became the most important sites for the sale of food products. This dominance in food saIes was generally confined to the retail sector as super-markets tended to act as a nexus or distribution point between agri-food producers and consumers. In the 1970s this relationship began to break down as supermarkets moved up the chain of production. Supermarkets began to directly source materials from producers to use in "own" brand and generic products, which soon came to compete with the branded products of agri-food manufacturers. This paper traces the beginnings of these shifts and investigates the consequences of globally-sourced super-market goods in the context of the Australian agri-food system, with pineapples and wine as case studies The paper concludes that the increasing size and scope of supermarket buying power and the presence and growth of "own" brands suggests that capital organized through retail channels is coming to rival manufacturing capital as a significant fraction of the Australian agri-food system. Background and aims Prohibitions on producing, distributing and selling cannabis are loosening in various jurisdictions around the world. This paper describes the diversification of cannabis supply laws and discusses the challenges to and opportunities for learning from these changes. Methods We document changes in cannabis supply laws that de jure legalized cannabis production for medical and/or non-medical purposes (excluding industrial hemp) in Australasia, Europe, North America and South America. We also highlight challenges to evaluating these legal changes based on our experiences studying cannabis laws and policies in the United States. Findings As of August 2016, two countries have passed laws to legalize large-scale cannabis production for non-medical purposes at the national (Uruguay) or subnational level (four US states). At least nine other countries legally allow (or will soon allow) cannabis to be supplied for medicinal purposes. Most of the changes in cannabis supply laws have occurred since 2010. The data available in most countries are inadequate for rigorously evaluating the changes in cannabis supply laws. Conclusion The evidence base for assessing changes in cannabis supply laws remains weak. This paper examines Australia's approach to Western European integration in the 1950s. While superficially supportive of steps towards greater Western European cooperation, the Menzies government had an ambivalent attitude towards the integration process. Ministers and officials in Canberra recognised the importance of a strong and prosperous Western Europe, but the formation of a tightly knit Continental grouping also posed a number of major challenges to the makers of post-war Australian foreign policy. The Menzies government soon began to wonder whether attempts to create supranational bodies in Continental Europe accorded with Australian political, economic, and security interests. Britain and many other European countries have been important sources of settler migration to Australia for more than two hundred years. While the sources of settler migration to Australia have diversified in the last thirty years to include non-European countries, with the current emphasis on skills in Australia's migration policy, many skilled people from Europe are coming to Australia both as settlers and as temporary migrants. This paper examines Europe as a source of both permanent and temporary skilled migration to Australia in recent years. It looks at the trend in migration from European countries to Australia from the 1960s to the present, noting the changes in patterns over time, both in terms of type of migration and source countries. It then focuses on European migration to Australia since the mid-1990s when Australia's immigration policy became more skill-oriented and a temporary skilled migration visa program was implemented to simplify the process by which employers could sponsor skilled migrant workers. While Europeans are not a large group among permanent migrants, many come as temporary migrants and then decide to apply for permanent residence. Survey data are used to examine their reasons for migration and residence/return migration intentions, comparing permanent skilled migrants with temporary skilled migrants. European migrants are more likely than other migrants to indicate lifestyle reasons for migrating or coming to Australia to work and then seeking permanent residence, with differences also observed among migrants from different regions in Europe. This paper examines international patent application decisions of nine agricultural biotechnology firms from 1990–2000 in Australia, Brazil, Canada, China, the European Patent Office, Japan and South Africa. The data reveal a low frequency of international applications despite an initial United States' application, indicating very low values for patents abroad. The results indicate that invention quality plays an important role in firms' decisions to patent abroad and that a single international application is a good predictor of multiple international applications. The pharmaceutical sector is unusual amongst global regulated industries inasmuch as it is usually forbidden from advertising to the users of its products. This paper reports on research updating my 2003 review 2 of the evidence and arguments for and against the direct-to-consumer advertising (DTCA) of prescription drugs. It not only covers the two countries where significant DTCA is permitted (USA and New Zealand), but also the administrations in which there is active discussion of possibilities to ease or amend some of the current restrictions (Europe, Australia and Canada). The International Psycho-oncology Society (IPOS) has just celebrated its 30th anniversary. The growth of psychosocial oncology has been exponential, and this relatively new field is becoming a core service that focuses on prevention, reducing the burden of cancer, and enhancing the quality of life from time of diagnosis, through treatment, survivorship, and palliative care. Looking back over the past 30 years, we see that cancer care globally has evolved to a new and higher standard. Today, 'cancer care for the whole patient' is being accomplished with an evidence-based model that addresses psychosocial needs and integrates psycho-oncology into the treatment and care of patients. The President's Plenary Session in Lisbon, Portugal, highlighted the IPOS Mission of promoting global excellence in psychosocial care of people affected by cancer through our research, public policy, advocacy, and education. The internationally endorsed IPOS Standard of Quality Cancer Care, for example, clearly states the necessity of integrating the psychosocial domain into routine care, and that distress should be measured as the sixth vital sign after temperature, blood pressure, pulse, respiratory rate, and pain. The plenary paper also discussed the global progress being made in Europe, North America, and Australia in providing quality cancer care for the whole patient. Collaborative partnerships between IPOS and organizations such as the European Partnership Action Against Cancer and the World Health Organization are essential in building capacity for the delivery of high-quality psycho-oncology services in the future. The article presents several abstracts of research papers published in the July 3, 2001 issue of the journal "Sociological Ruralis." It discusses the cultural economics in the rural areas of Europe. The paper "Transnational Co-operation Between Rural Areas: Elements of a Political Economy of European Union Rural Development," by Christopher Ray considers the significance of the phenomenon in terms of the emergence of a rural development system. "Exploring the Dimensions of Culture Economies in Rural West Wales," by Moya Kneafsey, Brian Ilbery and Tim Jenkins, interrogates the nature and extent of the culture economy as a force for rural development in West Wales. The discussion draws on empirical material from research into four different product sectors, namely, specialty beef and lamb, organic fruit, vegetables and meat, specialty cheese and artisanal crafts. Of particular significance is the research paper "Governing the Boundaries of Viability Economic Expertise and the Production of the Low-Income Farm Problem in Australia," by Vaughan Higgins which discusses how economic expertise assumed prominence as key authorities in constituting previous collectivist forms of assistance as irrational, and in linking the future viability of Australian agriculture to the theories and practices of agricultural economists.

A conference paper about the biological control of Chondrilla juncea is presented. It presents the results of the detailed ecological study being made on C. juncea and the organisms attacking it in Mediterranean Europe, as a prelude to biological control introductions into Australia. Using a line transect method at the dense Chondrilla sites between Barcelona, Spain and Nice, France, observations have shown that the plant populations decline to a greater or lesser extent according to conditions.

Brassica napus is cultivated largely as a winter or semi‐winter form in Europe and Asia, respectively, whereas spring‐sown rape types are more adapted to the climatic conditions in Canada, northern Europe, and Australia. This plant, the third most important oil crop in the world after maize and oil palm, has had a unique history among those crops that have been targets of breeding and genetic modification. Although agronomy and genetic manipulations of B. napus plants to create seed with substantially no erucic acid content in the seed oil and meal largely free from aliphatic glucosinolates were major breakthroughs, opportunities for further advances in the reduction of antinutritive compounds/enhancement of bioactive compounds, in oil and meal health safety and quality, as well as in agronomic performance and pest resistance, are still greater today than they have ever been. Understanding and application of genetic control for key agronomic traits in B. napus, such as seed oil composition and concentration as well as their environmental interactions, are the main targets, which will underpin the development of the crop for cultivation in an extensive range of environments. It is also needed to improve phytochemical content through traditional breeding programs or through bioengineering of the phenylpropanoid pathway, leading to desirable secondary metabolite accumulation. This paper is a review of modifications (in the field of biotechnology, agronomy, and processing) that have been applied to improve the nutritional value of B. napus L. Practical Applications: This paper has value for people starting their scientific work in the field related to the B. napus biotechnology and nutritional value of vegetable oils. In addition, it is a current compedium of knowledge for people working professionally in the oil industry. Moreover, this article is a cross‐section of the current achievements of genetic engineering in improving the composition of rapeseed and indicates key publications on this subject. Aim: Dietetics as a form of study predates medicine, but the profession did not emerge until the nineteenth century and is still in an emergent stage today in many parts of the world. This paper seeks to demonstrate how the impact of leaders has been critical in terms of the growth and development of the profession. Methods: A narrative review was formed from searching the published literature. Results: The published literature is silent on many aspects of dietetics. In modern times, the majority of written works have come from North America and Western Europe. No work on historical or developmental aspects of dietetics from other parts of the world could be located. Leadership within the profession is well researched, but leadership by the profession in the broader arena of national and international policy and practice is harder to identify. There are calls for curricula to keep pace with the needs of modern healthcare systems if the profession is to flourish. Conclusion: Despite all the changes and the evolution of the profession, much remains unchanged. There are recurring themes throughout the literature reviewed here, those of the breadth of dietetics, the need for evolving curricula, the need for business acumen, the need for diversity and, in more recent times, the ability to work with interdisciplinary teams. But the profession has what it takes. It has within its ranks leaders of today and tomorrow. It is up to the profession to allow those leaders to emerge and take the profession to the next level. Today, Bosnians represent one of the newly emerging and the most widely dispersed diasporic communities from the Balkans. There are large communities of Bosnians living in almost every European country, as well as throughout North America and Australia. Most were displaced during the 1992-1995 Bosnian war, in which 2.2 million people were forced to leave their homes, 1.6 million of whom looked for refuge abroad. In contrast with, and in response to, the enforced displacement, many members of the Bosnian diaspora have retained strong family and other 'informal' social ties with both Bosnians in other countries and those still living in Bosnia and Herzegovina (BiH, or Bosnia). Such ties - focused on preservation of cultural memory and performance of distinct local identities - form the basis of the global network of the Bosnian diaspora and its link with the original home (land). In this paper, I briefly outline the links and networks that constitute diaspora, and then go on to explore the extent to which recent scholarly literature is able to 'capture' the uniqueness and complexity of the Bosnian diasporic communities in Australia, the United States (U.S.) and Europe. Finally, I attempt to define the concept of 'trans-localism' and how it is (per)formed, and suggest that the predominantly 'transnational' conceptual framework within the migration studies needs to be expanded to include 'trans-local' diasporic identity formation among displaced Bosnians and similar diaspora groups. This paper provides an update of a previous review ' Red Meat in the Diet' published in the Nutrition Bulletin in 2005. An update on red meat consumption levels in the UK and other countries is provided, and a summary of the nutritional content of red meat in the diet is given. Current evidence on dietary and lifestyle factors associated with red meat consumption and the effects of red meat intake on health and chronic disease outcomes are discussed. This paper analyzes the effects of ATM surcharges on deployment and welfare, in a model where banks compete for ATM and banking services. Foreign fees, surcharges and the interchange fee are endogenously determined. We find situations in which surcharges are welfare enhancing. Under strategic fee setting, the increase in deployment caused by surcharging might compensate the surplus loss caused by the increase in prices. [ABSTRACT FROM AUTHOR]

Europe lags far behind Australia, New Zealand, Canada and the USA in terms of implementing regulatory procedures for the import and release of invertebrate biological control agents (IBCAs). A number of standards, documents and guidelines have been produced over recent years in an attempt to harmonize regulation of IBCA introduction into Europe. Despite these efforts, the number of member countries implementing any form of IBCA regulation remains low, with many industries, biological practitioners and regulators fearing that a regulatory system would render the process of approval for IBCA introduction into a country costly and time consuming. Europe’s priority is therefore to formulate a regulatory system that will be readily approved of and adopted by all member countries. In this paper we review the current regulatory processes operating in Australia, New Zealand, Canada and the USA. There is potential for Europe to benefit from the years of experience that these countries have in IBCA regulation. We therefore propose recommendations based on features of the regulatory processes in each of the four countries that work well and that could be adopted to generate a workable Europe-wide regulatory system. Floodplains are some of the most ecologically important and human‐impacted habitats throughout the world. Large efforts are underway in North America, Europe, Australia, and elsewhere to restore floodplain habitats, not only to increase fish and aquatic biota but to restore ecological diversity. As the scale, number, and complexity of floodplain restoration projects has increased, so has the need for rigorous monitoring and evaluation to demonstrate effectiveness and guide future floodplain restoration efforts. Moreover, technological advances in remote sensing, genetics, and fish marking have been evolving rapidly and there is need to update guidance on the best methods for monitoring physical and biological response to floodplain restoration. A comprehensive review of the restoration literature located 180 papers that specifically examined the effectiveness of various floodplain restoration techniques. The various methods that were historically and currently used to evaluate the physical (channel and floodplain morphology, sediment, flow, water quality [temperature and nutrients]) and biological (fish, invertebrates, and aquatic and riparian plants) effectiveness of floodplain restoration were reviewed and used to provide recommendations for future monitoring. For each major physical and biological monitoring method, we discuss their importance, how they have historically been used to evaluate floodplain restoration, newer methodologies, and limitations or advantages of different methodologies and approaches. We then discuss monitoring the effectiveness of small (<2 km in main channel length) and large (>2 km of main channel length) floodplain projects, with recommendations for various study designs, parameters, and monitoring methodologies. Background: Non‐pharmacologic interventions might be effective to reduce the incidence of delirium in pediatric intensive care units (PICU). Aim: To explore expert opinions and generate informed consensus decisions regarding the content of a non‐pharmacologic delirium bundle to manage delirium in PICU patients. Study design: A two‐round online Delphi study was conducted from February to April 2021. PICU experts (nurses, physicians, researchers, physical therapists, play specialists, and occupational therapists) located in Europe, North America, South America, Asia, and Australia participated. Results: We developed a questionnaire based on the outcomes of a comprehensive literature search in the domains: 1) cognition support; 2) sleep support; and 3) physical activity support. Under these domains, we listed 11 strategies to promote support with 61 interventions. Participants rated the feasibility of each intervention on a 9‐point Likert scale (ranging from 1 strongly disagree to 9 strongly agree). A disagreement index and panel median were calculated to determine the level of agreement among experts. In the second round, participants reassessed the revised statements and ranked the interventions in each domain in order of importance for age groups: 0–2, 3–5, and 6–18 years of age. During the first Delphi round, 53 of 74 (72%) questionnaires were completed, and in the second round 45 of 74 (61%) were completed. Five of the highest ranked interventions across the age groups were: 1) developing a daily routine, 2) adjusting light exposure according to the time of day, 3) scheduling time for sleep, 4) providing eyeglasses and hearing aids if appropriate, 5) encouraging parental presence. Conclusions: Based on expert consensus, we developed an age‐specific non‐pharmacologic delirium bundle of interventions to manage delirium in PICU patients. Oral Medicine is a young dental specialty born almost a century ago and deals with orofacial conditions not directly attributable to the most prevalent tooth pathologies such as dental caries or periodontal diseases. Presentations may reflect local disease or orofacial manifestations of more widespread pathology affecting other parts of the body. Due to its recency as a distinct discipline and to heterogeneous global settings, Oral Medicine has not yet achieved a shared scope and definition, as well as a recognized status across the globe. The current report presents survey data gathered from Oral Medicine practitioners in Europe and Australia and aimed to identify practitioner characteristics including demographics, training, clinical and research activity. As expected, Oral Medicine clinical practice commonly deals with conditions such as immune‐mediated disorders, potentially malignant disorders, oral mucosal infections and chronic pain disorders, but geographical heterogeneities are observed. The present data, representative of current clinical practice, are valuable in order to understand the evolution of Oral Medicine as a distinct discipline and should be taken into consideration in order to create or update postgraduate training curricula able to meet the needs of future practitioners and the communities they serve. The author responds to the comments made on their paper about naturopathy and critical medical anthropology (CMA). He says that he agrees with the assertion of a group of discussers that naturopathic practice in Europe and Australia resembles that in North America, but they minimize the political differences. He also argues on the comment that because CMA lacks significant political influence, it has little impact on the health of individuals and populations.

Nurses play a central role in the delivery of quality mental health services. Desired qualities of a mental health nurse, in particular therapeutic relationships, have been described in the literature, primarily reflecting the nursing paradigm. Service users' perspectives must be more fully understood to reflect contemporary mental health policy and to recognize their position at the centre of mental health service delivery and to directly influence and contribute their perspectives and experiences to mental health nursing education. A qualitative exploratory research project was undertaken to inform and enhance understanding of what service users see as the desired qualities of a mental health nurse. The project was co‐produced by service users as experts by experience, and mental health nurse academics to ensure the service user perspective was privileged. This international project conducted in Europe and Australia included a series of focus groups with service users (n = 50). Data were analysed thematically. Being with me was a major theme identified and reflected the sub‐themes: respect towards service users as persons; empathy, compassion and effective communication; understanding service users; knowledge of services; and fostering hope and believing that recovery is possible. These qualities specifically reflecting the service user perspective must be central to mental health nursing curricula to facilitate the development of holistic care and recovery‐oriented practice. These findings were utilized to directly inform development of a co‐produced mental health nursing learning module, to maximize genuine service user involvement, and to fully realize the benefits of service user led education for undergraduate nursing students. Introduction: Tom Kitwood's theoretical framework of person‐centred care is extensively and internationally referred to in healthcare services. However, despite the broad use of Kitwood's approach in clinical practice, the research examining experiences with the approach has not been mapped. Aim: To map the available literature on current empirical experiences in clinical practice of Kitwood's approach to person‐centred care for people with dementia, living in institutional settings in primary health care. Methods: A total of 154 studies concerning the experiences of people with dementia and their formal caregivers' use of Kitwood's framework of person‐centred care for residents living in institutional settings in primary health care were compiled from scientific databases. All three authors engaged in a systematic selection process, leading to nineteen articles being included in the review. Results: The results showed an overall tendency towards positive experiences gained from applying person‐centred approaches in clinical practice influenced by Kitwood's theoretical framework. Through the synthesis of data from the selected studies, six categories related to our research aim were identified 'Theoretical evaluation of Tom Kitwood's model of care', 'Models and frameworks', 'Interventions', 'Communication', 'Dementia Care Mapping' and 'Environment'. Conclusion: The present review included nineteen studies published from 1998 to 2016 and consisted mainly of peer‐reviewed scientific articles, followed by dissertations, conference posters and nonpeer‐reviewed articles. Objectives: To evaluate the global incidence of ameloblastoma and to provide a profile of ameloblastoma patients. Material and Methods: A systematic review and meta‐analysis was conducted. Searches were performed in PubMed, EMBASE, SCOPUS, and Web of Science for articles published from 1969 to 2018 for the global incidence and from 1995 to 2018 for the profile of ameloblastoma patients. Results: Seven studies on the incidence rate of ameloblastoma were included in the meta‐analysis. These studies only covered Europe, Africa, and Australia. The pooled incidence rate was 0.92 per million person‐years (95% CI: 0.57–1.49), with significant heterogeneity between studies. Forty‐two articles provided profile data of 6,446 ameloblastoma patients. Mean age was 34 years and the peak age incidence in the third decade of life. In Europe and North America, ameloblastoma mostly occurred at an older age when compared to Africa and South America. A slight male preference (53%) was found, and the mandible appeared to be the preferred site. The most common type of ameloblastoma was multicystic. The histopathologic patterns were mostly follicular and plexiform. Conclusions: This is the first study assessing the global incidence of ameloblastoma. Background: The US National Institutes of Mental Health Research Domain Criteria (RDoC) seek to stimulate research into biologically validated neuropsychological dimensions across mental illness symptoms and diagnoses. The RDoC framework comprises 39 functional constructs designed to be revised and refined, with the overall goal of improving diagnostic validity and treatments. This study aimed to reach a consensus among experts in the addiction field on the 'primary' RDoC constructs most relevant to substance and behavioural addictions. Methods: Forty‐four addiction experts were recruited from Australia, Asia, Europe and the Americas. The Delphi technique was used to determine a consensus as to the degree of importance of each construct in understanding the essential dimensions underpinning addictive behaviours. Expert opinions were canvassed online over three rounds (97% completion rate), with each consecutive round offering feedback for experts to review their opinions. Results: Seven constructs were endorsed by ≥ 80% of experts as 'primary' to the understanding of addictive behaviour: five from the Positive Valence System (reward valuation, expectancy, action selection, reward learning, habit); one from the Cognitive Control System (response selection/inhibition); and one expert‐initiated construct (compulsivity). These constructs were rated to be related differentially to stages of the addiction cycle, with some linked more closely to addiction onset and others more to chronicity. Experts agreed that these neuropsychological dimensions apply across a range of addictions. Conclusions: The study offers a novel and neuropsychologically informed theoretical framework, as well as a cogent step forward to test transdiagnostic concepts in addiction research, with direct implications for assessment, diagnosis, staging of disorder, and treatment. Background: Volume–outcome relationships for mortality following oesophagectomy have been demonstrated in Europe and the USA, but not in Australia or New Zealand. We determined whether higher volume hospitals achieve better outcomes following oesophagectomy in Australia and New Zealand. Methods: Administrative data for hospitals contributing data to the Health Roundtable were analysed. Hospitals performing oesophagectomy for cancer from July 2008 to June 2015 were grouped according to mean annual caseload: low (1–5), medium (6–11) and high (12+) volume. Univariate and multivariable analyses determined the impact of volume on 30‐day and in‐hospital mortalities, length of hospital stay and mechanical ventilation following surgery. Results: A total of 2252 patients underwent oesophagectomy in 65 hospitals. Sixty‐eight percent (n = 44) were low‐, 26% (n = 17) were medium‐ and 6% (n = 4) were high‐volume hospitals. Seven hundred and sixty‐two (34%) procedures were performed in low‐, 1042 (46%) in medium‐ and 448 (20%) in high‐volume hospitals. Overall in‐hospital mortality was 3.1% and 30‐day mortality was 2.1%. In‐hospital mortality was lowest in high‐volume hospitals; 1.6% versus 2.6% and 4.1% for low‐ and medium‐volume hospitals (P = 0.02). Surgery in high‐volume hospitals was shorter (32 min, P = 0.001), and patients were less likely to require post‐operative ventilation (16.7% versus 25.3% and 28.0%, P < 0.001), although patients requiring ventilation in high‐volume hospitals were ventilated for longer. Conclusions: A volume–outcome relationship was demonstrated, with overall better performance in higher volume hospitals. Aim: In 2017, the European Respiratory Society task force stated that protracted bacterial bronchitis (PBB) was a distinct clinical entity and outlined the diagnostic criteria and principles for treatment. However, this guidance was based on data from Australia and the USA. This mini review evaluated the data and addressed the lack of research-based data from Europe.Methods: We supplemented the 2017 report by conducting a nonsystematic review of the literature on prolonged wet or productive cough and on PBB in children up to September 2018.Results: Our review confirmed the lack of European data on PBB. Based on the available literature, and the 2017 European guidance, PBB is a wet or productive cough that lasts for four or more weeks, with no signs or symptoms, known as specific cough pointers, which suggest an underlying illness. PBB usually recovers with appropriate antibiotics for two weeks. Failing that radiological investigations or lung function measurements are needed. If PBB occurs three times over 12 months, then computerised tomography can diagnose bronchiectasis and determine other permanent changes.Conclusion: More data are needed on PBB, especially in Europe. The implementation of PBB may help clinicians to prescribe antibiotics more effectively and reduce their inappropriate use. [ABSTRACT FROM AUTHOR]

Global drought extremes are projected to increase under future warming scenarios. However, global drought risk and pattern remain to be revealed below 1.5 and 2 °C warming levels. Here we used multi‐model simulations to estimate global drought trend based on multiple drought indicators. The frequency and duration of meteorological drought may increase much faster than surface soil moisture drought and root zone soil moisture drought under anthropogenic warming, with a probable ~36% (62%) increase in frequency and a ~15% (20%) increase in duration at 1.5 °C (2 °C) level relative to the reference period over the globe. A 5% (14%) increase of drought frequency is expected at 1.5 °C (2 °C) warming in the surface soil moisture, and a 5% (2%) increase of duration is also likely in the 0–10 cm soil layer. Unprecedented increased drought risk is anticipated at 2 °C level, with potential drought hot spots in North America, South America, southern Africa, Australia and Europe. Drought condition under future warming may be much worse in local areas. The drought frequency in the drylands is much less (>34%) than the humid areas, but the duration is much higher (>40%). The 2 °C relative to 1.5 °C warming is expected to bring higher drought risk than the 1.5 °C relative to 1 °C period with regard to the frequency. This article reports an international study of research data management (RDM) activities, services, and capabilities in higher education libraries. It presents the results of a survey covering higher education libraries in Australia, Canada, Germany, Ireland, the Netherlands, New Zealand, and the UK. The results indicate that libraries have provided leadership in RDM, particularly in advocacy and policy development. Service development is still limited, focused especially on advisory and consultancy services (such as data management planning support and data-related training), rather than technical services (such as provision of a data catalog, and curation of active data). Data curation skills development is underway in libraries, but skills and capabilities are not consistently in place and remain a concern. Other major challenges include resourcing, working with other support services, and achieving "buy in" from researchers and senior managers. Results are compared with previous studies in order to assess trends and relative maturity levels. The range of RDM activities explored in this study are positioned on a "landscape maturity model," which reflects current and planned research data services and practice in academic libraries, representing a "snapshot" of current developments and a baseline for future research. Aim: There is a paucity of data from randomized controlled treatment trials in childhood arterial ischaemic stroke. Our objectives were to identify and plan a trial through use of a Delphi consensus process.Method: The Delphi panel consisted of Australian, New Zealand, and European paediatric neurologists with interests in childhood stroke. Four rounds were conducted using a REDCap (Research Electronic Data Capture) Web-based application: the first consisted of open-ended questions; the second evaluated agreement for the most important trial; the third and fourth reached consensus on design.Results: Forty-seven out of 66 neurologists answered the first round. Eight areas of research for important and feasible trials were identified. In the second round, 43 paediatric neurologists ranked the three highest rated trials: (1) aspirin versus aspirin plus steroids in focal arteriopathy (n=31); (2) heparin versus aspirin (n=6); and (3) heparin versus aspirin versus modern anticoagulation (n=6). The third and fourth surveys reached consensus among 43 out of 44 respondents on design of the highest ranked trial, and allowed agreement on inclusion/exclusion criteria, clinical/neuroimaging data, and treatment protocols.Conclusion: The Delphi consensus process is an efficient method of identifying and planning paediatric stroke trials. Background The current study sought to examine the diagnostic overlap in DSM- IV and DSM-5 alcohol use disorder ( AUD) and determine the clinical correlates of changing diagnostic status across the 2 classification systems. Methods DSM- IV and DSM-5 definitions of AUD were compared using cross-national community survey data in 9 low-, middle-, and high-income countries. Participants were 31,367 respondents to surveys in the World Health Organization's World Mental Health Survey Initiative. The Composite International Diagnostic Interview, version 3.0, was used to derive DSM- IV and DSM-5 lifetime diagnoses of AUD. Clinical characteristics, also assessed in the surveys, included lifetime DSM- IV anxiety; mood and drug use disorders; lifetime suicidal ideation, plan, and attempt; general functional impairment; and psychological distress. Results Compared with DSM- IV AUD (12.3%, SE = 0.3%), the DSM-5 definition yielded slightly lower prevalence estimates (10.8%, SE = 0.2%). Almost one-third ( n = 802) of all DSM- IV abuse cases switched to subthreshold according to DSM-5 and one-quarter ( n = 467) of all DSM- IV diagnostic orphans switched to mild AUD according to DSM-5. New cases of DSM-5 AUD were largely similar to those who maintained their AUD across both classifications. Similarly, new DSM-5 noncases were similar to those who were subthreshold across both classifications. The exception to this was with regard to the prevalence of any lifetime drug use disorder. Conclusions In this large cross-national community sample, the prevalence of DSM-5 lifetime AUD was only slightly lower than the prevalence of DSM- IV lifetime AUD. The purpose of this research is to quantitatively compare everyday situational experience around the world. Local collaborators recruited 5,447 members of college communities in 20 countries, who provided data via a Web site in 14 languages. Using the 89 items of the Riverside Situational Q-sort (RSQ), participants described the situation they experienced the previous evening at 7:00 p.m. Correlations among the average situational profiles of each country ranged from r = .73 to r = .95; the typical situation was described as largely pleasant. Most similar were the United States/Canada; least similar were South Korea/Denmark. Japan had the most homogenous situational experience; South Korea, the least. The 15 RSQ items varying the most across countries described relatively negative aspects of situational experience; the 15 least varying items were more positive. Further analyses correlated RSQ items with national scores on six value dimensions, the Big Five traits, economic output, and population. Individualism, Neuroticism, Openness, and Gross Domestic Product yielded more significant correlations than expected by chance. Psychological research traditionally has paid more attention to the assessment of persons than of situations, a discrepancy that extends to cross-cultural psychology. Objective: Individual placement and support (IPS) has shown consistently better outcomes on competitive employment for patients with severe mental illness than traditional vocational rehabilitation. The evidence for efficacy originates from few countries, and generalization to different countries has been questioned. This has delayed implementation of IPS and led to requests for country-specific RCTs. This meta-analysis examines if evidence for IPS efficacy can be generalized between rather different countries., Methods: A systematic search was conducted according to PRISMA guidelines to identify RCTs. Overall efficacy was established by meta-analysis. The generalizability of IPS efficacy between countries was analysed by random-effects meta-regression, employing country- and date-specific contextual data obtained from the OECD and the World Bank., Results: The systematic review identified 27 RCTs. Employment rates are more than doubled in IPS compared with standard vocational rehabilitation (RR 2.07 95% CI 1.82-2.35). The efficacy of IPS was marginally moderated by strong legal protection against dismissals. It was not moderated by regulation of temporary employment, generosity of disability benefits, type of integration policies, GDP, unemployment rate or employment rate for those with low education., Conclusions: The evidence for efficacy of IPS is very strong. Aims: To assess physicians' adherence to guideline-recommended medications for the treatment of chronic heart failure (CHF) with reduced ejection fraction.Methods and Results: QUALIFY is an international prospective observational longitudinal survey of 7092 CHF outpatients recruited 1-15 months after hospitalization for heart failure from September 2013 to December 2014 in 547 centres in 36 countries. We constructed a five-class guideline adherence score for angiotensin converting enzyme inhibitors (ACEIs), beta-blockers, angiotensin receptor blockers (ARBs), mineralocorticoid receptor antagonists, and ivabradine. The adherence score was good in 67%, moderate in 25%, and poor in 8% of patients. Adherence was lower in women than men but there were differences in age (65.7 ± 12.5 years women vs. 62.2 ± 12.4 years men, P < 0.001) and the proportion of women at ≥50% target dose of beta-blockers was lower in those >67 years (median) (11% vs. 16.2%, P = 0.005). Geographic variations were observed with lower adherence scores in Central/Eastern European countries. The proportion of patients at target dose and ≥50% of target dose was low (27.9% and 63.3% for ACEIs, 14.8% and 51.8% for beta-blockers, 6.9% and 39.5% for ARBs, and 6.9% and 39.5% for ivabradine, respectively). It was also lower in patients most recently hospitalized (<6  vs. ≥6 months) except for beta-blockers.Conclusion: This international survey shows that adherence to guideline-recommended medications is relatively satisfactory but the dosage of recommended CHF medications is usually suboptimal. MRI Assessment of Ablation-Induced Scarring in Atrial Fibrillation Background There is limited knowledge on the extent and location of scarring that results from catheter ablation and its role in suppressing atrial fibrillation (AF). We examined the effect of atrial fibrosis and ablation-induced scarring on catheter ablation outcomes in AF. Methods We conducted a prospective multicenter study that enrolled 329 AF patients presenting for catheter ablation. Delayed enhancement magnetic resonance imaging (DE-MRI) of the left atrium was obtained preablation. Scarring was evaluated in 177 patients with a DE-MRI scan obtained 90 days postablation. We evaluated residual fibrosis, defined as preablation atrial fibrosis not covered by ablation scar. The primary outcome was freedom from recurrent atrial arrhythmia. Results In the analysis cohort of 177 patients, preablation fibrosis was 18.7 ± 8.7% of the atrial wall. Ablation aimed at pulmonary vein (PV) isolation was performed in 163 patients (92.1%). Ablation-induced scar averaged 10.6 ± 4.4% of the atrial wall. Scarring completely encircled all 4 PVs only in 12 patients (7.3%). Residual fibrosis was calculated at 15.8 ± 8.0%. At 325 days follow-up, 35% of patients experienced recurrent arrhythmia. Multivariable Cox proportional hazards models demonstrated that baseline atrial fibrosis (HR and 95% CIs) (1.09 [1.06-1.12], P < 0.001) and residual fibrosis (1.09 [1.05-1.13], P < 0.001) were associated with atrial arrhythmia recurrence, while PV encirclement and overall scar were not. Conclusions Catheter ablation of AF targeting PVs rarely achieves permanent encircling scar in the intended areas. Overall atrial fibrosis present at baseline and residual fibrosis uncovered by ablation scar are associated with recurrent arrhythmia. [ABSTRACT FROM AUTHOR]

Rationale, aims and objectives Given the increasing emphasis being placed on managing patients with chronic diseases within primary care, there is a need to better understand which primary care organizational attributes affect the quality of care that patients with chronic diseases receive. This study aimed to identify, summarize and compare data collection tools that describe and measure organizational attributes used within the primary care setting worldwide. Methods Systematic search and review methodology consisting of a comprehensive and exhaustive search that is based on a broad question to identify the best available evidence was employed. Results A total of 30 organizational attribute data collection tools that have been used within the primary care setting were identified. The tools varied with respect to overall focus and level of organizational detail captured, theoretical foundations, administration and completion methods, types of questions asked, and the extent to which psychometric property testing had been performed. The tools utilized within the Quality and Costs of Primary Care in Europe study and the Canadian Primary Health Care Practice- Based Surveys were the most recently developed tools. Furthermore, of the 30 tools reviewed, the Canadian Primary Health Care Practice- Based Surveys collected the most information on organizational attributes. Conclusions There is a need to collect primary care organizational attribute information at a national level to better understand factors affecting the quality of chronic disease prevention and management across a given country. Background Over the past three decades, the World Health Organization expert panel proposed cesarean delivery rate of 10-15 percent was used as a doctrine for an optimal rate of cesarean delivery despite the lack of concrete evidence. We set out to compile cesarean delivery rates, socioeconomic indicators, and health outcomes by countries in the last three decades to explore the optimal rates for medically necessary cesarean delivery. Methods We selected 19 countries which have readily accessible cesarean delivery and low maternal and infant mortality, including countries in North and West Europe, North America, Australia, New Zealand, and Japan. Information on cesarean delivery rate, human development index ( HDI), gross domestic products ( GDP), maternal, neonatal, and infant mortality rates of each country in the past 30 years was collected from published reports. A two-level fractional polynomial model was used to model the association between cesarean rate and mortality rates. Results Most of the countries have experienced sharp increases in cesarean delivery rates. Once cesarean delivery rate reached 10 percent, with adjustment for HDI and GDP, further increase in cesarean delivery rate had no impact on maternal, neonatal, and infant mortality rates. Conclusions Our findings corroborate the statement that a population-level cesarean section rate above 10-15 percent is hardly justified from the medical perspective. [ABSTRACT FROM AUTHOR]

Aims To determine comorbidity patterns in treatment-seeking substance use disorder ( SUD) patients with and without adult attention deficit hyperactivity disorder ( ADHD), with an emphasis on subgroups defined by ADHD subtype, taking into account differences related to gender and primary substance of abuse. Design Data were obtained from the cross-sectional International ADHD in Substance use disorder Prevalence ( IASP) study. Setting Forty-seven centres of SUD treatment in 10 countries. Participants A total of 1205 treatment-seeking SUD patients. Measurements Structured diagnostic assessments were used for all disorders: presence of ADHD was assessed with the Conners' Adult ADHD Diagnostic Interview for DSM-IV ( CAADID), the presence of antisocial personality disorder ( ASPD), major depression ( MD) and (hypo)manic episode ( HME) was assessed with the Mini International Neuropsychiatric Interview- Plus ( MINI Plus), and the presence of borderline personality disorder ( BPD) was assessed with the Structured Clinical Interview for DSM-IV Axis II ( SCID II). Findings The prevalence of DSM-IV adult ADHD in this SUD sample was 13.9%. ASPD [odds ratio ( OR) = 2.8, 95% confidence interval ( CI) = 1.8-4.2], BPD ( OR = 7.0, 95% CI = 3.1-15.6 for alcohol; OR = 3.4, 95% CI = 1.8-6.4 for drugs), MD in patients with alcohol as primary substance of abuse ( OR = 4.1, 95% CI = 2.1-7.8) and HME ( OR = 4.3, 95% CI = 2.1-8.7) were all more prevalent in ADHD+ compared with ADHD− patients ( P < 0.001). These results also indicate increased levels of BPD and MD for alcohol compared with drugs as primary substance of abuse. Comorbidity patterns differed between ADHD subtypes with increased MD in the inattentive and combined subtype ( P < 0.01), increased HME and ASPD in the hyperactive/impulsive ( P < 0.01) and combined subtypes ( P < 0.001) and increased BPD in all subtypes ( P < 0.001) compared with SUD patients without ADHD. Seventy-five per cent of ADHD patients had at least one additional comorbid disorder compared with 37% of SUD patients without ADHD. Conclusions Treatment-seeking substance use disorder patients with attention deficit hyperactivity disorder are at a very high risk for additional externalizing disorders. [ABSTRACT FROM AUTHOR]

Objectives To test whether women aged 55 and older with increasing evidence of a frailty phenotype would have greater risk of fractures, disability, and recurrent falls than women who were not frail, across geographic areas ( Australia, Europe, and North America) and age groups. Design Multinational, longitudinal, observational cohort study. Setting Global Longitudinal Study of Osteoporosis in Women ( GLOW). Participants Women (N = 48,636) aged 55 and older enrolled at sites in Australia, Europe, and North America. Measurements Components of frailty (slowness and weakness, poor endurance and exhaustion, physical activity, and unintentional weight loss) at baseline and report of fracture, disability, and recurrent falls at 1 year of follow-up were investigated. Women also reported health and demographic characteristics at baseline. Results Women younger than 75 from the United States were more likely to be prefrail and frail than those from Australia, Canada, and Europe. The distribution of frailty was similar according to region for women aged 75 and older. Odds ratios from multivariable models for frailty versus nonfrailty were 1.23 (95% confidence interval ( CI) = 1.07-1.42) for fracture, 2.29 (95% CI = 2.09-2.51) for disability, and 1.68 (95% CI = 1.54-1.83) for recurrent falls. The associations for prefrailty versus nonfrailty were weaker but still indicated statistically significantly greater risk of each outcome. Overall, associations between frailty and each outcome were similar across age and geographic region. Conclusion Greater evidence of a frailty phenotype is associated with greater risk of fracture, disability, and falls in women aged 55 and older in 10 countries, with similar patterns across age and geographic region. A buyer group is a subset of downstream firms that pool their demand for an upstream input to negotiate a better deal with suppliers. This study develops a simple model that shows how a buyer group changes market behaviour, focusing on the impact on downstream firms outside the buyer group. This impact critically depends on whether or not the buyer group and the supplier can write a contract contingent on the market price paid by outsiders. With contingent contracts, the price paid by firms outside the buyer group rises and their profits fall. If such contracts are infeasible, possibly due to legal concerns, the market price is lower after the buyer group forms. These results raise issues for antitrust authorities in Australia, Europe, the USA and elsewhere who evaluate buyer-group agreements. We extend the model to consider the situation where a supplier can commit in advance to the market price. In this situation, the bargaining power of the buyer group becomes relevant with the market price inversely related to this bargaining power. We illustrate our results with a simple example. Aim: To undertake a systematic literature review to examine the effect of food cost on diet quality and risk factors for chronic disease, specifically focusing on diet-related lifestyle diseases affecting the Australian population. Methods: A search of six databases resulted in the inclusion of one systematic review, three cohort studies, 41 cross-sectional studies and four modelling studies in this review. Results: Between 2000 and 2006, the price of healthy foods has increased more than the price of less healthy foods. Healthy Food Access Basket surveys show that a healthy diet may often be unaffordable for low- and average-income households. Diets of higher energy density were associated with lower diet cost, whereas diets of higher nutrient density and nutritional quality were associated with higher diet cost. Recent studies report an inverse association between food price and food consumption. Consequently, an increase in food cost was associated with a significant reduction in weight, waist circumference, body mass index, obesity and insulin resistance. Conclusions: Manipulation of food cost may alter food consumption and therefore risk factors for chronic disease. Further longitudinal studies investigating the impact of pricing strategies on diet quality and disease risk are needed. [ABSTRACT FROM AUTHOR]
