The HIV/AIDS pandemic is a global issue that has unequally affected several countries. Due to the complexity of this condition and the human drama it represents to those most affected by it, several fields have contributed to solving or at least alleviating this situation, and the information systems (IS) field has not been absent from these efforts. With the importance of antiretroviral therapy (ART) as a starting point, several initiatives in the IS field have focused on ways to improve the adherence and effectiveness of this therapy: mobile phone reminders (for pill intake and appointments), and mobile interfaces between patients and health workers are popular contributions. However, many of these solutions have been difficult to implement or deploy in some countries in the Global South, which are among the most affected by this pandemic. This paper presents one such case. Using a case‐study approach with an extreme‐case selection technique, the paper studies an m‐health system for HIV patients in the Kalangala region of Uganda. Using Heeks' design—reality gap model for data analysis, the paper shows that the rich interaction between social context and technology should be considered a central concern when designing or deploying such systems. Drawing on a case study from eastern Uganda, this paper describes how social and environmental factors combine to affect the sustainability of both sorghum landraces and the farmers who depend on it for food and income security. It delineates how changing regional patterns of agricultural production and consumption, institutional neglect, economic hardship, natural resource degradation and a labour supply crisis precipitated by the HIV/AIDS pandemic, may be conspiring to place sorghum landraces at risk of extinction and, thus, undermine already precarious livelihoods. The paper therefore challenges the common assumption that marginalized rural women – by virtue of having diverse varieties and species under their care – can be expected to conserve that diversity. Aim Use a Delphi Methodology to identify nursing research priorities in Uganda. Background Identifying nursing research priorities, empowering researchers, and encouraging relevant studies can advance attaining global health goals. The Uganda Nurses and Midwives Union identified the need to establish a nursing research agenda. Nurse leaders have a priority of increasing the influence of nurses in practice and policy. This study was conducted as a preliminary step in a long-term strategy to build nurses' capacity in nursing research. Methods A three-round Delphi study was conducted. The 45 study participants were nurses in practice, nurse faculty and members of the Uganda Nurses and Midwives Union. In the initial round, the participants wrote their responses during face-to-face meetings and the follow-up rounds were completed via email. Results Maternal and child morbidity and HIV/AIDS were identified as research priorities. Nurses also identified nursing practice, education and policy as key areas that nursing research could impact. Limitations Demographic characteristics such as length of time in nursing were not collected. Additionally, first round participants completed a pencil-paper survey and the follow-up rounds were done by email. Conclusions Nurse Leaders in Uganda identified areas where research efforts could have the most impact and were most relevant to nursing practice. Implications for nursing and health policy Health policy decisions have historically been made without nursing input. Nursing research can provide evidence to inform policy and, ultimately, improve population health. This paper explores the ways that young people express their agency and negotiate complex lifecourse transitions according to gender, age and inter- and intra-generational norms in sibling-headed households affected by AIDS in East Africa. Based on findings from a qualitative and participatory pilot study in Tanzania and Uganda, I examine young people's socio-spatial and temporal experiences of heading the household and caring for their siblings following their parent's/relative's death. Key dimensions of young people's caring pathways and life transitions are discussed: transitions into sibling care; the ways young people manage changing roles within the family; and the ways that young people are positioned and seek to position themselves within the community. The research reveals the relational and embodied nature of young people's life transitions over time and space. By living together independently, young people constantly reproduce and reconfigure gendered, inter- and intra-generational norms of 'the family', transgressing the boundaries of 'childhood', 'youth' and 'adulthood'. Although young people take on 'adult' responsibilities and demonstrate their competencies in 'managing their own lives', this does not necessarily translate into more equal power relations with adults in the community. The research reveals the marginal 'in-between' place that young people occupy between local and global discourses of 'childhood' and 'youth' that construct them as 'deviant'. Although young people adopt a range of strategies to resist marginalisation and harassment, I argue that constraints of poverty, unequal gender and generational power relations and the emotional impacts of sibling care, stigmatisation and exclusion can undermine their ability to exert agency and control over their sexual relationships, schooling, livelihood strategies and future lifecourse transitions. Richter M.S., Mill J., Muller C.E., Kahwa E., Etowa J., Dawkins P. & Hepburn C. (2013) Nurses' engagement in AIDS policy development. International Nursing Review 60, 52-58 Background: A multidisciplinary team of 20 researchers and research users from six countries - Canada, Jamaica, Barbados, Kenya, Uganda and South Africa - are collaborating on a 5-year (2007-12) program of research and capacity building project. This program of research situates nurses as leaders in building capacity and promotes collaborative action with other health professionals and decision-makers to improve health systems for human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) nursing care . One of the projects within this program of research focused on the influence of workplace policies on nursing care for individuals and families living with HIV. Nurses are at the forefront of HIV prevention and AIDS care in these countries but have limited involvement in related policy decisions and development. In this paper, we present findings related to the barriers and facilitators for nurses' engagement in policymaking. Methods: A participatory action research design guided the program of research. Purposive sampling was used to recruit 51 nurses (unit managers, clinic and healthcare managers, and senior nurse officers) for interviews. Findings: Participants expressed the urgent need to develop policies related to AIDS care. The need to raise awareness and to 'protect' not only the workers but also the patients were critical reason to develop policies. Nurses in all of the participating countries commented on their lack of involvement in policy development. Lack of communication from the top down and lack of information sharing were mentioned as barriers to participation in policy development. Resources were often not available to implement the policy requirement. Strong support from the management team is necessary to facilitate nurses involvement in policy development. Conclusions: The findings of this study clearly express the need for nurses and all other stakeholders to mobilize nurses' involvement in policy development. Although availability of quantitative information on the extent of HIV/AIDS in Africa is improving, the socio-economic implications of the epidemic remain poorly understood. This paper explores this issue for Uganda focusing on households who received foster children between 1992 and 2000, a phenomenon that affected more than 15% of households. The authors find that addition of a foster child resulted in significant reductions of per capita consumption, income, and household investment which were more pronounced for the poor. Initial disadvantages in foster children's access to education for this group were overcome in the course of UPE implementation, while new inequalities have emerged in access to health services. Foster children's ability to access services will thus be affected by the broader policy environment as well as programs more specifically targeted at this group. [ABSTRACT FROM AUTHOR]

The article presents the author's perspective on study by S. Nakibinge and colleagues about a community engagement research on health aspects related to HIV/AIDS epidemic in Uganda. The author remarks that the study did not only focus on health care promotion and prevention, and community development linked to poverty reduction, but also counselling, and address issues of stigmatization and links this devastating disease with the broader social determinants of health and community development.

Professional counseling in Uganda has foundations in traditional cultures of its peoples, guidance offered in schools, and counseling to curb the HIV/AIDS epidemic. Currently, a definitive professional counselor profile in Uganda is being established. The Uganda Counselling Association continues the process of seeking legal authority to regulate counselor training, supervision, and practice. ABSTRACT Global health interventions to provide antiretroviral (ARV) drug treatment for people living with HIV/AIDS in developing countries have linked global and local actors in unprecedented ways. These uneven relationships have been described as creating new forms of citizenship that challenge the liberal understanding of rights and responsibilities bestowed by the state. A comparative case study based on fieldwork from South Africa and Uganda suggests different theoretical understandings of the link between technologies of AIDS treatment and relationships of belonging. Yet, ethnographic data from local clinics in both countries point to similarities that exist across AIDS interventions, and to the importance of counsellors in negotiating the rules of ARVs. Neither patients living with HIV nor the local providers of their AIDS treatment are 'bare life' subjects to be acted upon by a global development intervention. As ARV technologies are increasingly prescribed in developing country clinics, diverse social relationships are taught and negotiated as part of the pedagogy of biopolitics. The following discussion demonstrates how local counsellors and clients negotiate the rules of AIDS treatment together for mutual benefit. The article concludes that AIDS treatment creates relationships of therapeutic citizenship and clientship in ways that constrain the possibilities of citizenship and development. In Uganda, bacterial and fungal infections are prevalent especially, the sexually transmitted diseases including HIV/AIDS. Because of low socio-economic empowerment, traditional beliefs and cultural barriers, the suffering people resort to use of herbal remedies in search of cure especially for rural women who rarely discuss/disclose diseases affecting secretive body parts. The study documented 67 medicinal plants distributed among 27 families and 51 genera used to treat fungal and bacterial infections in and around Queen Elizabeth Biosphere Reserve in western Uganda. The highest numbers of species were from Families Lamiaceae (13) and Asteraceae (11). The most commonly harvested plant parts were leaves (88.1%) and roots (23.9%). The most common growth form harvested were herbs (47.8%) and most of the medicinal plants used were wild species (67.2%). The main methods of herbal drug preparation were by squeezing, boiling and pounding and were mainly orally administered. However, most of the steamed plant species were inserted in the birth canal besides oral administration. Some plants such as Allium sativum, Aloe vera and Ocimum gratissimum are topically applied on the affected body parts. This vital indigenous knowledge about healing secretive and discrete diseases among the marginalized population requires urgent ethnobotanical studies to sustain livelihoods. [ABSTRACT FROM AUTHOR]

HIV epidemics continue to spread around the world. Although most responses have emphasized medical approaches, the HIV and AIDS situation could be further improved by applying good governance principles. In analyzing national responses to HIV and AIDS from Brazil, Thailand, and Uganda, common factors consistent with democratic governance principles can be found. These factors are the strategic vision of political leaders, timely responses, strong and effective civil society participation, multisectoral mobilization, balancing prevention and treatment approaches, respecting human rights, and transparency. By building knowledge-bases, advocating policy improvement and taking strategic actions for programmatic responses, countries in South-East Asia have developed an Early Warning Rapid Response System which applies good governance principles for multisectoral actions in building HIV resilience. Engaging the construction sector to prevent HIV can ensure sustainable returns on economic investments. In mobilizing non-health sectors such as agriculture and construction, broader strategic action programmes can be devised to complement the efforts of the health sector in mitigating socio-economic crises aggravated by HIV and AIDS. The article concludes that the democratic governance approach is an essential option worth pursuing because it makes a critical contribution to reversing the relentlessly expanding trend of the HIV and AIDS pandemic. Uganda faces continual challenges as a low-income nation reliant on international donors and non-state actors. It was also one of the first countries to face a population-wide HIV epidemic, a disease that can strain state capacity to its limits. One would expect that such a combination would weaken the governance structures in a developing country; yet, if anything, the Ugandan state has emerged from its HIV crisis with its legitimacy bolstered. This article reviews the Ugandan response to HIV/AIDS, analysing the ways in which the epidemic has provided a new arena for the Ugandan state to engage with international actors. This article examines the political dimensions of Uganda''s progress in bringing a generalised HIV/AIDS epidemic under control. The article documents the history of the political processes involved in Uganda''s battle against HIV/AIDS and analyses the complexities of presidential action and the relation between action at the level of the state and that taken within societal organisations. By the mid-1980s, Uganda was experiencing a full-blown epidemic, the virulence of which was connected with social dislocation and insecurity related to economic crisis and war. Political authorities faced the same challenge as other regimes experiencing the onslaught of AIDS in Africa. The epidemiological characteristics of HIV and AIDS—transmission through heterosexual activities, with a long gestation period, affecting people in the prime of their productive life—meant that action required wide-reaching changes in sexual behaviour, and the educational activities to achieve this, as well as relatively complex systems to monitor the virus and control medical practices (blood supplies, injection practices, mitigating drug delivery). The centralist character of the Museveni regime was crucial not only to mobilising state organisations and foreign aid resources, but also to ensuring significant involvement from non-state associations and religious authorities. The Ugandan experience demonstrates that there is a tension between the requirements for systematic action that a strong public authority can deliver and the need to disseminate information requiring a degree of democratic openness. The President was able to forge a coalition behind an HIV/AIDS campaign in part because the virus largely ignored the privileges of wealth and political power. With the development of antiretroviral therapy and the access that the wealthy can gain to these drugs, this basis for the broadest possible coalition to fight HIV/AIDS may be weakened in the future.
