Issue Addressed: Queensland children have a higher level of developmental vulnerability compared to the Australian average. This paper reports on Healthy Kids—a capacity building strategy for the early childhood education and care (ECEC) sector targeting communities experiencing socio‐economic and child development vulnerabilities. These communities may face additional barriers when engaging and participating in health promotion models. This paper reports on the development, key components and principles of a capacity building model referred to as Healthy Kids, that strategically responds to these barriers and supports these communities. Methods: The development of the Healthy Kids model emerged through a quality improvement process that included an environmental scan, and review of existing capacity building, health promotion, and workforce development approaches. It also involved consultation and engagement with the ECEC sector. Results: Evidence indicates Healthy Kids to be an innovative health promotion model focussed on building capacity through a workforce development strategy for the ECEC sector in a way that is accessible, low cost, and sustainable. So What?: This paper shares a model for building capacity through the establishment of localised cross‐sector communities of practice across a large geographic region with a centralised coordinating hub. The hub and spoke model has facilitated community ownership to grow and be sustained over time. This model offers opportunities for partnerships, transferability, and contextualisation for those interested in contemporary health promotion, capacity building, and workforce development. The model offers an approach for those willing to step outside traditional boundaries to work across sectors and settings to achieve sustainable knowledge and skills, processes and resources that enables a collective commitment to improving health outcomes. Platform‐mediated short‐term rentals (STRs) are a relatively new addition to the housing landscape, providing accommodation on a temporal scale that falls between hotel stays and long‐term rentals (LTRs). Given the potential for STR hosts to charge significantly higher nightly rates for the same property than they might through an LTR lease enumerated in months or years, many have asked whether a 'gap' exists between the two, and how persistent this may be over space and time. In this paper, we modify and validate classical bid–rent theory to investigate spatial variation in the rent gap between STRs and LTRs over time in two distinct contexts in Queensland, Australia. We conduct statistical analyses on STR data against LTR data from local property records in Brisbane and the Sunshine Coast, identifying a significant rent gap, particularly close to high‐amenity areas. We find strong evidence of distance decay in the rent gap, with rents converging with increasing distance in most cases, indicating that STRs only 'outcompete' LTRs near high‐amenity areas, which in this analysis are the central business districts (Brisbane) and the beach (Sunshine Coast). We also find that the rent gap increases as properties get larger in size, and over time. These findings carry implications for policy aiming to balance the needs of multiple stakeholders and political agendas against planning frameworks prescribing where short‐term accommodation is best situated. Objective: A shared care model was implemented in 2006 in Queensland to facilitate paediatric oncology, haematology and palliative care patients receiving care as close to home as possible. Following initial diagnosis, care planning and treatment at the tertiary children's hospital, appropriate local care was coordinated by Regional Case Managers (RCMs) established at each of 10 Shared Care Units (SCUs). This enabled safe and quality regional care supported by a statewide network providing clinical governance and education. This paper examines learnings from 15 years of this shared care. Setting: Ten hospitals throughout Queensland facilitated a statewide model of shared care for paediatric oncology, haematology and palliative care patients, supported by a tertiary hub in Brisbane. Participants: Regional Case Managers in Shared Care Units and their supporting staff. Design Staff from SCUs were surveyed and focus group interviews conducted. Results: The paper reviews the attributes, knowledge and experience required for RCMs. Standards of care were supported through education workshops, clinical placements, chemotherapy credentialing, guidelines and standards. RCMs facilitated communication and information sharing with the tertiary centre, advocated for their cohort of patients locally and streamlined and supported the family's experience of care. Conclusion: The RCM role provided invaluable clinical leadership for the care of paediatric oncology, haematology and palliative patients across Queensland. As new treatments evolve, the expertise and coordination provided by the RCMs will be even more critical. Aim: To describe the development of social function in children with cerebral palsy (CP) classified in all levels of the Communication Function Classification System (CFCS). Method: This prospective, longitudinal population‐based cohort study recruited children with CP born in Queensland, Australia. Social functioning was measured using the Pediatric Evaluation of Disability Inventory (PEDI) social function domain at 2 years, 2 years 6 months, 3 years, 4 years, and 5 years, and the PEDI Computer Adaptive Test (PEDI‐CAT) social/cognitive domain at 8 to 12 years. Results: Seventy‐four children provided 356 observations. PEDI‐CAT social/cognitive scaled scores at 8 to 12 years were (mean [SD] n) CFCS level I, 68.6 (2.7) 45; CFCS level II, 64.0 (3.4) 10; CFCS level III, 63.5 (3.7) 4; CFCS level IV, 56.8 (5.0) 9; CFCS level V, 47.2 (5.8) 6. Scores within expected range for age (not less than 2 SD below mean) at 8 to 12 years were achieved by 35 (78%) children in CFCS level I and four (14%) in CFCS levels II to V. Forty‐nine per cent of children scored at least two standard deviations below the population mean on a proxy measure of fluid intelligence. Intellectual impairment was associated with lower PEDI‐CAT social/cognitive scaled scores in univariable analysis (β = −8.3, 95% confidence interval − 10.91 to −5.63; p < 0.001) but had a smaller effect when modelled together with CFCS. Interpretation: Social function attained by 8 to 12 years of age was strongly related to level of communication function (CFCS). The small number of children classified in CFCS levels II to V necessitates caution when viewing these individual CFCS level trajectories. Background: In Queensland, each hospital and health service (local hospital network) has its own waste reduction and recycling plan to comply with the Queensland Government's Waste Reduction and Recycling Act 2011 (Qld). The aim is to reduce both the hospital's carbon footprint and waste handling costs. Hospital environmental waste services staff do not audit pharmaceutical waste bins as this requires the presence of a registered pharmacist. Aim: Since previous published studies of healthcare waste disposal practices have not included pharmacy waste bin audits, this study aimed to investigate waste disposal behaviours in a hospital pharmacy department. Method: This sequential, two‐phase mixed methods study was conducted in a metropolitan, tertiary public hospital's pharmacy department in Queensland. Phase I involved semi‐structured interviews of hospital pharmacists and pharmacy technicians while Phase II comprised bin audits of the pharmacy department's waste streams. Results: The bin audits revealed 36.1%, 23.8%, and 4.9% of recyclable waste in the clinical waste stream for each of the three bin audits respectively. In the general waste stream, the two bin audits of this stream revealed 14.3% and 44.4%, respectively. The reasons were identified in the interviews: there were no recycling bins in the main dispensing areas and there was confusion surrounding correct disposal of original containers and non‐contaminated packaging waste. Non‐paper waste was found in the confidential (shredded) waste stream in the two bin audits of this stream (10.1% and 16.7%, respectively). Conclusion: Provision of commingled recycling bins and clean office paper waste bins in dispensing areas and education of staff on correct waste segregation processes will improve waste segregation in hospital pharmacy departments with both financial and environmental benefits for the hospital and the general population. In 2001, a study into issues of concern to assistant-in-nursing, registered and enrolled nurse members of the Queensland Nurses Union was undertaken. Approximately equal numbers of nurses from each of the aged care, acute private and acute public sectors were surveyed. Overall, 1477 nurses responded, representing a response rate of 53%. This article focuses on one aspect of the study—the perceived adequacy of support offered to new nursing graduates as they exit university and begin their transition into the workplace. In particular, responses from nurses are compared with professional level or current role designation, age, time spent in the workplace and health sector. Considerable divergence of opinion among the respondents, particularly across designation, age and years of experience, was found within the three sectors. For example, in the public and private employment sectors, older and more experienced nurses were more likely to perceive the support for new nurses as adequate compared to younger and less experienced nurses. Additionally, in the acute private sector, the more senior the nurse the more likely the perception that there was adequate preparation for new graduates entering the workforce. Rejecting nature‐culture dualism, contemporary anthropology recognises the mutually constitutive processes that create shared human and non‐human lifeworlds. Such recognition owes much to ethnographic engagement with diverse indigenous cosmologies many of which have, for millennia, upheld ideas about indivisible worlds in which all living kinds occupy a shared ontological space and non‐human species and environments are approached respectfully, with expectations of reciprocity and partnership. As many societies confront the global chaos caused by the anthropocentric prioritisation of human interests, anthropologists and indigenous communities are therefore well placed to articulate alternative models in which the non‐human domain is dealt with more equitably and inclusively. This paper is located comparatively in long‐term ethnographic research with indigenous communities in Australia, alongside the Mitchell River in North Queensland and the Brisbane River in South Queensland. It draws more specifically on involvement in legal claims for water rights by Māori iwis in New Zealand; in land claims by the Kunjen language group in Cape York; and in a recent 'sea country' case brought against a major multi‐national by the Tiwi Islanders in Australia's Northern Territory. It also makes use of a major comparative study of water beings in diverse cultural and historical contexts, and considers the central importance of water beings such as Māori taniwha and the Australian Rainbow Serpent in such legal conflicts, and in broader debates about human and non‐human rights. Like other water deities around the world, these beings personify the generative (and potentially punitive) powers of water and its co‐creative role in shaping human and non‐human lives. Rationale: Veterans living with dementia in long‐term care have complex needs, with variable manifestation of symptoms of dementia that interact with their lived experience. Best practice dementia care prioritises nonpharmacological interventions; of which few have strong evidence. Implementation of evidence is complex, with evaluation of outcomes and processes necessary. Aims and Objectives: This paper details the evaluation of implementation, at veteran and organisational level, of the Weaving Evidence into Action for Veterans with Dementia (WEAVE) programme. Methods: A Type 2 hybrid effectiveness‐implementation design was used, underpinned by the Implementation Framework for Aged Care (IFAC). Programme intervention incorporated music therapy, exercise, reminiscence therapy and/or sensory modulation, offered over a 24‐week period. Evaluation components included: (1) programme effectiveness for veterans with dementia for responsive behaviour, physical wellbeing, cognitive status, emotional state, medications and falls (at baseline, 8‐week, 16‐week and 24‐week); and (2) implementation outcomes of reach and adoption, feasibility and acceptability, fidelity (via interviews) and a preliminary cost analysis. Results: Thirty‐eight veterans participated in the 24‐week programme, with high levels of engagement in interventions of their choice. Statistically significant improvements were seen across all veteran‐level outcome measures, for functional capacity and reduced neuro‐psychiatric and depressive symptoms. Ten staff members were interviewed, highlighting co‐designed core elements were feasible and acceptable, and the momentum generated by resident and staff enthusiasm. Cost analysis included costs of programme set‐up and running the 24‐week intervention. Conclusion: Key components of programme success were the therapeutic leaders, adherence to core elements of programme design, and veterans' choice in meaningful activity. Objective: Living in regional/rural areas can impact outcomes for people with head and neck cancer (HNC). Using a comprehensive state‐wide dataset, the impact of remoteness on key service parameters and outcomes for people with HNC was examined. Methods: Retrospective quantitative analysis of routinely collected data held within the Queensland Oncology Repository. Design: Quantitative methods (descriptive statistics, multivariable logistic regression and geospatial analysis). Setting: All people diagnosed with HNC in Queensland, Australia. Participants: The impact of remoteness was examined in 1991 people (1171 metropolitan, 485 inner‐regional, 335 rural) with HNC cancer diagnosed between 2013 and 2015. Main Outcome Measures: This paper reports key demographics and tumour characteristics (age, gender, socioeconomic status, First Nations status, co‐morbidities, primary tumour site and staging), service use/uptake (treatment rates, attendance at multidisciplinary team review and timing to treatment) and post‐acute outcomes (readmission rates, causes of readmission and 2‐year survival). In addition to this, the distribution of people with HNC across QLD, distances travelled and patterns of readmission were also analysed. Results: Regression analysis revealed remoteness significantly (p < 0.001) impacted access to MDT review, receiving treatment, and time to treatment commencement, but not readmission or 2‐year survival. Reasons for readmission did not differ by remoteness, with dysphagia, nutritional inadequacies, gastrointestinal disorders and fluid imbalance indicated in the majority of readmissions. Rural people were significantly (p < 0.0001) more likely to travel to care and to readmit to a different facility than provided primary treatment. Background: Polypharmacy in haemodialysis patients can not only manifest as the continued prescribing of unnecessary medications but also has the potential to increase medication‐related hospital admissions, morbidity, and mortality. A validated deprescribing algorithm has recently been developed, specifically targeted at dialysis patients. This paper audits the experience of implementing this algorithm in Australian dialysis settings. Aim: The aim of this paper was to evaluate the usefulness and applicability of the pharmacist‐led Toronto deprescribing tool in Australian haemodialysis settings. Method: The pharmacist‐led deprescribing algorithm was implemented across two metropolitan sites and one rural site. The audit focused on five medications that could potentially be deprescribed in the target patient group (diuretics, alpha blockers, statins, proton pump inhibitors [PPIs], and quinine). Between 1 and 12 months later, a reaudit was conducted, with patients followed up to confirm if medications remained deprescribed. Results: Two hundred and eleven patients across three sites were reviewed. Application of the algorithm resulted in 168 medications deemed appropriate to deprescribe. Of these 168 medications, 56 (33%) were initially deprescribed, with 50 medications (30%) remaining deprescribed on reaudit. The deprescribing rates varied between the three different services, with initial deprescribing rates ranging from 18% to 61%. After follow‐up, deprescribing changes across target medications were fairly static, with only a small number of patients restarting either their diuretic or PPI. Conclusion: The pharmacist‐led deprescribing algorithm resulted in substantial deprescribing across the three sites. Deprescribing rates varied between the sites due to differences in the team model that the pharmacist worked within and the method of the rollout. Further studies should draw on aspects such as finding enablers to overcome prescriber and patient concerns about deprescribing and the aspects of which team models lead to higher rates of successful deprescribing. This paper reports on changes in the social‐emotional well‐being of 6‐ to 12‐year‐old children tested before the COVID‐19 pandemic and during 2020 and 2021. Well‐being was assessed using a video game that empowers children to report their own well‐being, including school attachment, social and emotional well‐being, behavioural conformity and family support. We compared well‐being over time for two groups of children in government schools in Queensland, Western Australia and Tasmania. The treatment group of 580 children were tested in 2019 (Time 1) and a second time in mid‐late 2020 and early 2021 (Time 2). The comparison group of 841 children were tested twice before the pandemic. Results showed that children in the treatment group reported significantly lower family support at Time 2 than those in the comparison group. This reduction in perceived family support was stronger for girls. In addition, children in the treatment group who reported lower levels of family support at Time 1 reported a steeper decline in family support by Time 2. Finally, boys in the treatment group reported significantly better behavioural conformity and emotional well‐being relative to girls over time. Results highlight the varied impacts of the pandemic lockdowns and the need to provide continued support to vulnerable families. [ABSTRACT FROM AUTHOR]

Objectives: Wave forced impacts are known to result in cervical spine injuries (CSI) and approximately 20% of drownings in Australia occur at the beach. The most common mechanism of injury in studies examining the frequency of CSI in drowning patients is shallow water diving. The aim of the present study was to determine what proportion of CSIs occurring in bodies of water experienced a concomitant drowning injury in a location where wave forced impacts are likely to be an additional risk factor. Methods: Electronic medical records at the Sunshine Coast Hospital and Health Service EDs, Queensland Ambulance Service case records and Surf Life Saving Queensland data between 1 January 2015 and 21 April 2021 were manually linked. Outcomes recorded included victim demographics, scene information, hospital course and patient disposition. Results: Ninety-one of 574 (15.9%) CSIs occurred in a body of water with risk of drowning. However, only 4 (4.3%) had a simultaneous drowning injury, representing 0.8% (4/483) of drowning presentations. Ten (10.9%) patients reported loss of consciousness, including the four with drowning. The principal mechanism of CSI was a wave forced impact (71/91, 78%). Most injuries occurred at the beach (79/91, 86.8%). Delayed presentation was common (28/91, 31%). A history of axial loading was 100% sensitive when indicating imaging. Conclusions: The combination of CSI and drowning is uncommon. Cervical spine precautions are only required in drowning patients with signs or a history, or at high risk of, axial loading of the spine. Background: As health systems face increasing demands, non‐medical prescribing is a workforce redesign strategy adopted within some services. Despite successful implementation in other professional groups, non‐medical prescribing within speech pathology (SP) has not yet been described. Aims: To provide a descriptive account of the development and planned implementation of two SP prescribing models. Methods & Procedures: The evolution of two SP‐led prescribing models, including relevant training and credentialing, for use of (1) nystatin oral drops (100,000 units/mL); and (2) lidocaine (lignocaine) and phenylephrine nasal spray (5 mg/500 μg/spray), in the outpatient setting is detailed. Challenges to implementation are outlined. Main Contribution: The development of relevant governance structures, a research evidenced‐based project evaluation framework, and an overview of training pathways and credentialing was successfully completed. However, implementation of the models was unable to be achieved. A thorough review of the requirements and a discussion of contextual considerations that had a negative influence on the implementation of SP‐led prescribing within this specific service context is provided. Conclusions & Implications: The successful implementation of SP‐led prescribing is complex and highly context dependent. This work offers a discussion and review of the complexities of introducing a non‐medical prescribing model in an outpatient hospital setting. SP‐led prescribing has not been thoroughly investigated in the Australian context. What this study adds to existing knowledge: This study describes the development of a SP‐led prescribing process in the outpatient setting, and a thorough review and discussion of the drivers and barriers to the model's implementation. What are the potential or actual clinical implications of this work?: The successful implementation of SP‐led prescribing was identified to be complex from a legislative and operational perspective, as well as being highly context dependent. Fibropapillomatosis (FP) is a tumor‐forming disease which affects all species of marine turtle, but predominantly the green turtle (Chelonia mydas). Expression of this disease is thought to be precipitated by poor environmental conditions and often linked to anthropogenically induced environmental changes. Although FP is a globally distributed disease, targeted studies on the spatial distribution of the disease in Australia are limited. Here, we present the first comprehensive report of FP prevalence in Queensland, Australia. A retrospective analysis of 25,645 capture records for 15 sites along the Queensland coast were used to determine FP prevalence and trends in foraging green turtles. Within this data set, 791 turtles (3.1%) with FP tumors were recorded. Our analysis showed that prevalence varies between sites and years, with juvenile turtles being the most frequently affected by the disease. We found that survey method has a significant influence on the apparent FP prevalence detected at each site. That is, surveys which were explicitly FP‐targeted detected higher numbers of individual turtles with FP, and therefore generated higher prevalence rates than comprehensive population surveys. We also report the first attempt at developing water quality indices (WQIs) to compare with FP prevalence data in foraging green turtles. The WQIs were built from metrics published in a range of peer‐reviewed papers, reports, and based on expert opinion. Despite utilizing an extensive data set, a relationship between FP prevalence and WQI rankings at each site could not be quantified. The analysis was confounded by a range of limitations, including data gaps, varying temporal scales and data capture methods in the FP prevalence, and water quality data sets. This study has significant implications for management as it highlights the benefits of designing and collecting centralized data that can be integrated and used across multiple projects or programs. Background: Whilst dysphagia is a commonly reported complication of stroke, it has received relatively little attention in the literature for patients following non‐traumatic subarachnoid haemorrhage (SAH). Aims: To investigate dysphagia incidence, risk factors, clinical progression and recovery in patients following non‐traumatic SAH. Methods & Procedures: A prospective cohort study of 49 patients admitted to a tertiary neurosurgical referral unit with non‐traumatic SAH over a 12‐month period was conducted. Swallowing function was assessed by a speech–language pathologist within 72 h of medical stability and monitored throughout the acute inpatient admission. Outcomes & Results: Dysphagia incidence was 16.33% (n = 8/49). Risk factors associated with dysphagia included; Glasgow Coma Scale (GCS) score on admission, need for intensive care unit (ICU) admission, length of ICU stay, need for intubation and ventilation, and hydrocephalus. Participants with dysphagia were admitted to hospital 1.9 times longer than those without dysphagia (p < 0.05) and were more likely to be transferred to another inpatient facility for ongoing care (p < 0.05). Dysphagia remained present at hospital discharge for over half (62.5%) of participants who developed this complication. Conclusions & Implications: Care pathways for patients admitted to hospital with non‐traumatic SAH should include early screening for dysphagia risk. Further research using a larger prospective cohort is required to validate dysphagia incidence and risk factors in this patient cohort. Opiine braconids are parasitoids of the immature stages of frugivorous tephritids. The female wasp lays her eggs into the eggs or larvae of the fruit fly host, where the immature wasp develops before emerging as a next‐generation adult from the now dead host pupal case. In support of a new generation of Australian fruit fly parasitoid research, this paper comprehensively reviews what is known about the Australian fruit fly infesting opiines. Based on the most recent taxonomic revision 11 fruit fly infesting opiine species are documented to occur in Australia, but we consider as doubtful the record for Diachasmimorpha longicaudata and consider the record for Fopius illusorius to be tentative without further collections. We identify that the systematics and taxonomy of the Australian native fruit fly infesting opiines are in urgent need of further work. The history of fruit fly biological control in Australia is comprehensively reviewed, including the export of native Australian opiines for fruit fly control elsewhere in the world. Australia was actively involved in three major classical biological control programmes against fruit flies from the turn of the 1900s until the mid‐1960s. Despite the introduction of 11 opiine species, plus numerous other natural enemies, only Fopius arisanus established in eastern Australia, where in South‐east Queensland it can now cause between 30 and 40% mean parasitism. In addition to the exotic F. arisanus, the native species Diachasmimorpha kraussii and Diachasmimorpha tryoni also cause fruit fly parasitism in agriculturally important crops: both species have also been liberated widely outside of Australia for fruit fly control. Other Australian opiines have not been reared from flies infesting commercial crops and appear biologically restricted to the fruits and environs of Australian east‐coast rainforests. The biology literature for D. tryoni and D. kraussii is comprehensively reviewed, while for F. arisanus, already reviewed elsewhere, key literature only is covered. Forward looking, we consider the potential for inoculative or inundative releases of opiines in areas where they do not currently occur to be good, while conservation biological control may help to increase the impacts of parasitoids in areas where they are already established. Summary: This paper describes advances made to aerial surveys of macropods in New South Wales and Queensland. Regular aerial surveys of macropods across large areas of Australia's arid and semi‐arid rangelands were established as a response to concerns for the species' conservation. They became integral to the harvest management of the four macropod species – Eastern Grey Kangaroo (Macropus giganteus), Western Grey Kangaroo (M. fuliginosus), Red Kangaroo (Osphranter rufus) and Common Wallaroo (O. robustus). Harvest quotas are set as a proportion of population size, requiring absolute estimates of abundance. Counts were made in strip transects from fixed‐wing aircraft, but required correction for animals that are missed. In New South Wales and Queensland, counts are now made using line transect sampling, providing survey‐specific correction. Helicopters are used in more rugged and timbered areas and where Common Wallaroo can be more frequently found and harvested. Mark‐recapture distance sampling is used in more open country that can be surveyed using fixed‐wing aircraft and improves the accuracy of density estimates by accounting for animals missed on the transect line. Macropod populations are likely to be surveyed in the future by airborne imagery and by drones, but the technology needs further development. The koala, Phascolarctos cinereus, is an iconic Australian wildlife species facing a rapid decline in South‐East Queensland (SEQLD). For conservation planning, the ability to estimate the size of koala populations is crucial. Systematic surveys are the most common approach to estimate koala populations but because of their cost they are often restricted to small geographic areas and are conducted infrequently. Public interest and participation in the collection of koala sighting data is increasing in popularity, but such data are generally not used for population estimation. We modeled monthly sightings of koalas reported by members of the public from 1997 to 2013 in SEQLD by developing a self‐exciting spatio‐temporal point process model. This allowed us to account for characteristics that are associated with koala presence (which vary over both space and time) while accounting for detection bias in the koala sighting process and addressing spatial clustering of observations. The density of koalas varied spatially due to the heterogeneous nature of koala habitat in SEQLD, with a mean density of 0.0019 koalas per km2 over the study period. The percentage of land areas with very low densities (0–0.0005 koalas per km2) remained similar throughout the study period representing, on average, 66% of the total study area. The approach described in this paper provides a useful starting point to allow greater use to be made of incidental koala sighting data. We propose that the model presented here could be used to combine systematic koala survey data (which is spatially restricted, but more precise) with koala sighting data (which is incidental and often biased by nature, but often collected over large geographical areas). This paper describes a rock engraving site in Mithaka Country in the Channel Country, southwest Queensland, where the majority of the motifs consist of crescents or variations on crescents. This is the first rock art site to be recorded in Mithaka Country, which is in a part of Australia's sandy deserts where rock art is uncommon. Gilparrka Almira is placed within a broader social context by exploring its possible cultural connections with other sites and regions. Regional comparisons of the main motif type found that proportions decreased in all directions away from the site. Possible meanings for crescent imagery are then examined from ethnohistorical sources, indicating that crescent motifs may bear a range of "discontinuous" meanings that can be used in different contexts. It is suggested that crescent motifs may have moved/diffused across vast areas of the continent, following the north‐south Lake Eyre Basin trade network, with Mithaka Country lying at its approximate centre, and other (east‐west) trade routes, along the Dreaming tracks with which the trade routes are frequently associated. Motifs with "discontinuous" meaning ranges, like crescents, would have been particularly suitable for use in this scenario because of their ability to be readily incorporated into different social contexts. The Princess Alexandra Hospital (PAH) Gamma Knife® Centre of Queensland (GKCoQ) began operations in October of 2015 as a sub‐specialty located within a larger radiation oncology service at PAH. It is uniquely positioned as the only Leksell Gamma Knife® (LGK) treatment unit available in the public hospital system in Australia, and the first and only service in Queensland. The GKCoQ treated the 1000th patient on 23 January 2019. LGK is a non‐invasive alternative to neurosurgery which uses radioactive cobalt sources to treat a variety of intracranial conditions ranging from tumours and metastases to functional disorders. It is a platform for stereotactic radiosurgery, a highly precise form of radiotherapy utilising very high doses to the target while maximally sparing surrounding normal brain. LGK enables patient planning and treatment to be done in one day as an outpatient procedure. This paper will outline our LGK service and provide insight into the expanded role that radiation therapists have within the multidisciplinary team required to deliver radiosurgery in a timely manner. Indigenous women are disproportionately affected by the prevalence and severity of domestic and family violence (DFV) perpetrated by men against women in Australia. The impact of their experiences is further exacerbated by the level of social and cultural marginalisation affecting Indigenous communities, families and individuals along with regional, rural or remote living. The pervasive, ongoing impact of colonisation, dispossession and transgenerational trauma continues to shape the experiences of Indigenous women and their communities, including their help‐seeking decisions and opportunities, and their level of trust in and access to adequate and culturally sensitive support services. In this paper, we explore the experiences of Indigenous women affected by DFV in two regional Australian settings through face‐to‐face interviews with women (n = 13) and community stakeholders (n = 18). The findings of this study highlight the complex role of social and cultural marginalisation, spiritual (dis)connectedness and role of family as both a stressor and a protective factor. Introduction: Key elements of the Queensland Government's Tackling Alcohol Fuelled Violence (TAFV) policy included the introduction of mandatory identification (ID) scanners and reduced trading hours for licensed venues located within Safe Night Precincts (SNP). These measures raised concerns among licensed venue owners and other key stakeholders regarding the potential negative impact of the policy on business trade and nightlife. Using multiple data sources, this paper examines the impact of the TAFV policy on nightlife and trade in three Queensland SNPs: Fortitude Valley, Surfers Paradise and Cairns. Methods: Data from ID scanners (2017–2019), foot traffic counters (2017–2019) and counts of liquor licences issued (2015–2019) were used to measure the policy's impact on the number of patrons and licensed venues within SNPs. Joinpoint regression and auto‐regressive integrated moving average models were developed to examine changes in these measures over time. Results: No significant changes were observed in the number of ID scans over time, although Surfers Paradise showed a 0.13% reduction during high alcohol hours. Foot traffic counts from Fortitude Valley showed no significant impact of the policy on the number of patrons in the area during high‐alcohol hours. The number of commercial hotel licenses increased in all three regions, and commercial other‐bar licenses increased in all regions except Surfers Paradise SNP where numbers remained stable. Discussion and Conclusions: Patterns in trading and the number of patrons within SNPs were mostly stable or increasing following the implementation of the TAFV policy beyond normal fluctuations. Issue addressed: Growing evidence suggests that public parks enable physically active communities; however, little is known of the impact of council‐provided exercise facilities in outdoor green spaces, and the challenges of providing this infrastructure at a local level. This paper sought to describe some of the factors, as perceived by local government area (LGA) council representatives within Queensland, Australia, that influence community use of, as well as council provision for, weight‐bearing outdoor fitness station infrastructure in public parks. Methods: A nested mixed‐methods study was employed, comprised of a survey sent to council representatives for all 78 Queensland LGAs (Stage 1), and follow‐up interviews to elicit further insights into the provision of infrastructure in public parks (Stage 2; n = 7). Perceptions around participants' corporate role in the provision of outdoor fitness stations were the focus of analyses. Findings: A thematic analysis described themes of: Moderating public attitudes and motivation; The flow of information; Supporting an active community; Return on investment; and Safety, complianceand climate. Provision of equipment was influenced by the perception that it represented poor value for money, in terms of the desired outcome of increased physical activity, especially when compared with other types of infrastructure (particularly, walking trails); however, opinions about this lack of use outdoor exercise equipment were most often based on anecdotal evidence. Conclusions: Councils are motivated to support active communities. They would benefit from better access to information about park usage, strategies for marketing healthy living initiatives and data about the economic benefits of parks. Perinatal mental illness is a known risk to maternal–infant attachment and healthy infant development. Mothers experiencing complex mental health issues in the first year following birth are less likely to become involved in parenting programs or day stay interventions because of their mental health difficulties and perceived stigma. Currently, most perinatal day or group treatment programs only include the mother and not their infant. This paper describes "Together in Mind," a perinatal and infant mental health day program developed by the Queensland Centre for Perinatal and Infant Mental Health, targeting mothers with moderate to severe mental illness and their infants under 12 months. The service model was a 6‐week, 1 day per week psychoeducation intervention. Psychoeducational material and support were provided across each day session by an adult perinatal mental health clinician, an infant mental health clinician, and a child health nurse working in collaboration. The program was trialed across seven Hospital and Health Service sites in Queensland, Australia, during 2016–18. In total, 24 group day programs were delivered with 84 mothers and their infants. Pre and post intervention quantitative measures and a post‐program qualitative survey about participant satisfaction were collected. Statistically significant improvements in all quantitative measures showed a large to medium effect size on the: Health of the Nation Outcome Scale (HoNOS) (d = 0.82; p <.000); Depression, Anxiety and Stress Scale (DASS‐21) (d = 0.5; p <.000); Karitane Parenting Confidence Scale (KPCS) (d = 0.63; p <.000); Maternal Postnatal Attachment Scale (MPAS) (d = 0.49; p <.000), Ages and Stages Questionnaire: Social‐Emotional (6 months) (d = 0.83; p <.000). The results indicate collaboration and early intervention contributes to strengthening the emerging development of the maternal–infant relationship within the context of complex maternal mental health issues. This paper reviews the relationships between risk perception and structural measures in an Australian context in three respects: (i) opinions about authorities' ability to mitigate flood risks; (ii) the role of flood experience in shaping views on risk; and (iii) perspectives on the ways in which structural measures shape decision‐making pertaining to protective action. The main finding of this analysis is that the study participants do not suffer from the 'levee paradox'. Most take precautionary steps to guard against residual flood risk. Such actions, however, do not mean that there is a lack of trust in structural measures to reduce significant flood risk. The majority of the respondents agreed about the necessity of engineering structures to mitigate inherent flood risk. This support, though, does not extend to their management. Losses during major flooding in southeast Queensland, Australia, in 2010–11 were attributed primarily by residents to operational decisions concerning dam water releases. [ABSTRACT FROM AUTHOR]

Summary: Protecting native biodiversity is a difficult prospect in extremely modified landscapes, especially where high‐impact exotic species are widespread. Using new data and a review of the literature, this paper comments on the use of livestock grazing to manage the invasive and highly combustible pasture grass species, Buffel Grass (Cenchrus ciliaris) and thereby help conserve fire‐sensitive Brigalow (Acacia harpophylla) vegetation in reserves in Queensland, Australia. We cite evidence that shows that grazing is a potentially useful management tool in such cases and its use can be compatible with the protection of both fire‐sensitive vegetation and other native plant species within the understorey. However, there are limitations in implementing grazing within conservation reserves including the lack of a clear understanding of the influence of grazing on biodiversity and resource condition. Importantly, we highlight secondary invasion by the exotic grass Indian Couch (Bothriochloa pertusa) as a key emerging threat that may undermine the biodiversity benefits gained by grazing in reserves. Grazing can be a useful tool for conservation management in particular scenarios, but the associated risks demand accompanying monitoring and reporting of positive and negative impacts to ensure the fundamental aim of biodiversity protection is being achieved. The specific aim of the current paper was to provide a descriptive evidence base of QoL and its specific domains by various key demographic characteristics of middle‐aged individuals. This cohort is likely to have either experienced or reached adulthood during and after large‐scale deinstitutionalisation in Australia. Methods and procedures: A cross‐sectional design was utilised, based on the QoL‐Q survey, testing domains of satisfaction; Competence/Productivity; Empowerment/Independence; and social belonging/community integration. The survey included demographic questions and was distributed to participants through disability support agencies across two states. The final sample included 291 respondents. Results: The overall QoL‐Q scores ranged from 3.0 to 29.5 (Mean = 20.4, SD, 4.1), with considerable variation in mean scores both across and within domains. The two demographic areas that showed greatest predictive value for QoL were work status and accommodation issues. The loss of agency and control in choice of co‐residents influenced perceived QoL for empowerment and independence domain of QoL‐Q. Conclusions: The findings highlight QoL issues associated with policy decisions and support programmes for middle‐aged adults. The two key recommendations arising from the project are that future planning for post‐retirement or reduced working hours needs to commence at a much younger age than currently expected, and more attention needs to focus on facilitating individual decision‐making and choice within shared accommodation options. For the Bulwandji Traditional Owners of Cape York Peninsula, Bunda Bibanji (also known as "Bare Hill") is part of bulurru – the Storytime, or the time of creation (Bottoms 1999: 1). Bunda Bibanji is the place where the culture hero, Kunandooran, turned into rock after his death by fire; it forms part of a complex cultural landscape. Bunda Bibanji also contains the largest rock art complex in the Cairns hinterland and has significant research potential for both rock art studies and understanding the occupation history of the region. Until recently, management of the site complex has focused on visitor management and its tangible scientific values, rather than its social and intangible cultural values. However, a major shift in management in recent years has aimed to enhance the Aboriginal social values of the place, potentially placing its scientific values at risk. This paper explores the tensions between managing scientific and cultural values at the Bunda Bibanji complex from both archaeological and Traditional Owner perspectives. [ABSTRACT FROM AUTHOR]

In 2010, Elders from Mapoon watched "Time Team" and saw ground‐penetrating radar (GPR) and other non‐invasive scientific methods being used to identify cultural features on archaeological sites. This led Elders to express their desire for such techniques, particularly GPR, to be brought on country to assist with documenting and protecting cultural heritage sites. This paper explores how Mapoon families have employed these techniques in archaeological settings to reconnect and reimagine specific features in their cultural landscape, as well as to explore, create and renew narratives from their interpretations of the results from the project. We also investigate the nature of Traditional Owner interactions with these technologies in archaeological settings to understand their impact on how cultural identity and value are recreated and renewed, on "caring for country" programmes, and on modernising culturally appropriate forms of engagement with ancestors' remains. We apply these findings to a discussion of how Mapoon people think about their cultural history, identity and connections to country, particularly in relation to mortuary practices and settlement patterns from pre‐contact to the "mission time". Introduction: Collaboration, choice and power‐sharing are cornerstones of practice as occupational therapists support individuals to re‐establish an occupational identity and reintegrate into the community following stroke. Yet evidence of unmet client needs suggests client‐centred care is not optimal, and little is known of client perspectives of client‐centred practice. A deeper understanding of the client experience of therapeutic relationships during adjustment following stroke, will help facilitate client‐centred practice. This paper reports on a study exploring the research question: how do stroke survivors perceive their relationship with their health care team as they adjust to life following stroke? Methods: In this constructivist grounded theory study, in‐depth interviews were conducted with six adult stroke survivors from South‐East Queensland, Australia. Transcribed interviews were analysed using a constant comparative approach, identifying emerging concepts and patterns, to construct a theoretical understanding of the experience and meaning of adjustment and reintegration back into the community, with a specific focus on perceptions of therapeutic relationships and client‐centred practice. Results: Participant perspectives of client‐centred care during their rehabilitation following stroke were revealed and implications of this for their recovery identified. Themes of being understood; out of the driver's seat; knowing what's going on; and what I need when I need it emerged. Participants described not being: involved in decision making, provided with information, or receiving services aligned to their needs. Periods of frustration, loss of hope and fluctuating motivation were also common. Conclusion: Findings shed further light on previously reported health service and discharge experiences. The insight gained into individual experiences of client‐centred care following stroke suggest ways therapists can understand and address each client's adjustment experience and the impact this has on their needs, goal‐setting, motivation, and confidence. It is well established that encounters between welfare bureaucracies and their clients have been reconfigured under neoliberalism to address the problem of "welfare dependency." Contemporary bureaucratic encounters therefore entail measures to activate clients' entrepreneurial/self‐governing capacities, and conditionality/sanctioning practices to deal with clients who behave "irresponsibly." Despite the dominance of the neoliberal model, recent research has identified a counter‐trend in the practices of housing services away from entrepreneurializing and punitive strategies and towards a more supportive approach. This paper examines this counter‐trend and its implications for neoliberal welfare governance. To do this, it presents findings from research into social housing governance in Queensland, Australia, where the neoliberal focus on welfare independence, conditionality and sanctioning has been tempered by a new supportive approach focused on assisting vulnerable clients to maintain and benefit from access to welfare/housing support. Following Larner, we argue that this shift signals the emergence of an "after neoliberal" governmental formation, wherein key features of neoliberal governmentality are replaced by, or redeployed in the service of, progressive initiatives that address neoliberalism's failings at the street level, but leave broader neoliberal policy settings undisturbed. We also challenge recent sociological accounts that construe supportive welfare practices as a function of an all‐encompassing neoliberal project, arguing instead for appreciation of the contingency of these developments and the progressive political affordances that they entail. Recent studies have demonstrated the economic benefit of exploiting partial opportunities for maintenance, which arise from the reduction of costs of doing partial maintenance utilizing the opportunity given. A typical example is an externally induced production stop with a duration that may not be sufficient to a complete maintenance activity. This paper combines partial opportunities and condition‐based maintenance (CBM) strategies and proposes an innovative maintenance optimization method considering time‐varying economic conditions. This scenario naturally fits a broad range of assets with a finite design life (including long‐life machinery and infrastructure), operating under variable economic conditions and usage‐intensities. The maintenance optimization problem is formulated in this study as a finite‐horizon Markov decision process, where the randomly occurring opportunities are accounted for by augmenting the time‐varying, decision‐dependent transition probabilities. A dynamic programming approach is subsequently used to obtain the optimal CBM policy, consisting of time‐varying thresholds on equipment condition and the cost of conducting maintenance during the arrived opportunity. A case study of such a maintenance policy for an induced draft fan of a sugar production system located in Queensland, Australia, is undertaken to evaluate the benefits of the proposed methodology against more traditional approaches that neglect opportunities or consider only replete opportunity durations. Issue Addressed: Evidence suggests that participation in a Chronic Disease Self-Management (CDSM) program improves the health of clients. Many factors are known to influence participation in these programs for the broader Australian population. However, less is known about why Aboriginal and Torres Strait Islanders choose to participate. This study identifies key factors that support or enable Aboriginal and Torres Strait Islanders to participate in a CDSM program in an urban setting.Methods: Twelve focus groups were undertaken with a total of 102 participants who were diagnosed with or at risk of chronic disease. These participants were recruited from the Work It Out program, a CDSM program comprising exercise and health education. The Work It Out program is specifically designed for Aboriginal and Torres Strait Islanders and delivered by an Aboriginal led and community-controlled organisation in South-East and Central Queensland. The study received ethical clearance through the Behavioural and Social Sciences Ethical Review Committee at The University of Queensland (Approval Number 2011001283).Results: Findings indicate that key features of program design based on a culturally responsive approach influences participation. The main features are as follows: providing easy access to the program; permitting flexibility in attendance; a group environment; the approach of program staff that prioritises relationship building; personalised and integrated care; communicating result regularly; and ensuring community ownership of the program.Conclusion: These findings are useful to consider when designing a health program for Aboriginal and Torres Strait Islanders. Programs which are culturally responsive and include the design features identified in this study are more likely to increase participation amongst Aboriginal and Torres Strait Islanders. Passive acoustic monitoring is gaining momentum as a viable alternative method to surveying freshwater ecosystems. As part of an emerging field, the spatio‐temporal replication levels of these sampling methods need to be standardised. However, in shallow waters, acoustic spatio‐temporal patchiness remains virtually unexplored.In this paper, we specifically investigate the spatial heterogeneity in underwater sounds observed within and between waterholes of an ephemeral river at different times of the day and how it could affect sampling in passive acoustic monitoring.We recorded in the Einasleigh River, Queensland in August 2016, using a linear transect of hydrophones mounted on frames. We recorded four times a day: at dawn, midday, dusk, and midnight. To measure different temporal and spectral attributes of the recorded sound, we investigated the mean frequency spectrum and computed acoustic indices.Both mean frequency spectrum and index analyses revealed that the site and diel activity patterns significantly influenced the sounds recorded, even for adjacent sites with similar characteristics along a single river. We found that most of the variation was due to temporal patterns, followed by between‐site differences, while within‐site differences had limited influence.This study demonstrates high spatio‐temporal acoustic variability in freshwater environments, linked to different species or species groups. Decisions about sampling design are vital to obtain adequate representation. This study thus emphasises the need to tailor spatio‐temporal settings of a sampling design to the aim of the study, the species and the habitat. [ABSTRACT FROM AUTHOR]

Background: Chronic kidney disease, Queensland (CKD.QLD) is a multidisciplinary, collaborative research platform for CKD in Queensland. Most public renal services contribute towards the CKD Registry, including Toowoomba Hospital, which is a referral hospital for Darling Downs Health serving a largely regional population in Queensland. We aim to present the profile of the CKD cohort recruited to the CKD.QLD Registry from Toowoomba Hospital, the first comprehensive report on a pre‐dialysis population from regional Australia. Methods: Study subjects were patients in the Darling Downs Health Service who consented to be included in the CKD.QLD registry from June 2011 to December 2016. Those who were on renal replacement therapy (RRT) were excluded. Patients were followed until date of RRT, death, discharge or loss to follow up or a censor date of 30th June 2017. Results: Overall 1051 subjects, representing 13% of all CKD.QLD Registry patients gave consent of whom, 42.7% were ≥70 years of age. The mean age was 63.8 ± 15.1 years (median age 67 years) with male predominance (55.4%). The majority were born in Australia (86.4%). Aboriginal and Torre Strait Islanders (A&TSI) constituted 9.6% of the cohort. The predominant CKD stages were 3b (28.9%) and 4 (27.7%). Hypertension and diabetes were noted in 91% and 44% of subjects, respectively. Diabetic nephropathy was the leading cause of CKD (26.7%) followed by renovascular disease (17.3%) and glomerulonephritis (14.8%). In 12%, the diagnosis was uncertain. Major co‐morbidities included coronary artery disease (24.7%) chronic lung disease (14.8%), cerebrovascular disease (11.6%) and peripheral vascular disease (8.9%). Non‐vascular co‐morbidities included arthritis (24.6%), gout (23.6%) and gastro‐oesophageal reflux disease (19%). The multi‐morbidity profile was differed by gender, diabetic status and age. Over a follow‐up period upto 72 months, 93 (8.8%) started RRT and 175 (16.6%) died. Of those 82% died without RRT and 18% died after RRT. Conclusion: This CKD Registry cohort from regional Queensland consisted mainly of older Caucasians with male predominance. A&TSI patients were overrepresented compared to the overall population. A significant proportion had cardio‐vascular disease and multiple co‐morbidities which differed by gender, diabetic status and age. This report provides valuable data for health services planning and delivery in regional Queensland. Child protection authorities in many countries are concerned with reducing the rates of investigations and with diverting at‐risk families from the child protection service system. In several countries, differential responses have been introduced into child protection law providing service providers with some choice between investigative or family support pathways, depending upon the level of risk posed in the circumstance. In this paper, we report on a study into a form of differential response known as Intervention with Parents' Agreement introduced in Queensland, Australia, in 2005. A unique feature of this differential response is that it occurs after an initial child protection investigation, although it does provide child protection services with options for providing supportive interventions to at‐risk families to prevent the further escalation of concerns. In this paper, we analyse practitioners' perceptions of factors that inhibit and promote implementation of the Intervention with Parents' Agreement. Drawing upon interviews with 25 practitioners, we identify factors that become important for securing participation after an initial investigation has occurred. 'Slow science' approaches to understanding the ecology, natural history and demography of species have declined over recent decades, despite the critical importance of these studies to conservation biology. With the progression of the Anthropocene, populations of invertebrates are under increasing pressure across the globe, yet few long‐term datasets exist to track potential changes or declines. Here, we present a newly developed 'slow science' study system, to understand the demography, biology and molecular ecology of a potentially threatened species of giant idiopid trapdoor spider from inland eastern Australia. This previously undescribed species in the tribe Euoplini, here newly described as Euoplos grandis Wilson & Rix sp. nov., has a highly fragmented distribution in the southern Brigalow Belt bioregion of south‐eastern Queensland, in a landscape largely cleared for cropped agriculture. The conservation significance of Idiopidae has long been recognised, and these spiders remain a flagship group for terrestrial invertebrate conservation in Australia. By studying growth rates, life spans, recruitment, natural history, fitness, gene flow, dispersal and other aspects of population and individual health, we aim gradually to uncover the population dynamics of a discrete natural population. In this paper, we summarise longitudinal data for 69 individual trapdoor spiders following an initial 18 months of study, and highlight preliminary demographic trends, biological observations and avenues for future genetic research. Ultimately, the aim of this study is to provide a baseline dataset for the conservation of Australian Idiopidae, and a guiding case study for similar taxa elsewhere in Australia. The peer workforce has increased significantly in recent years; however, structured development and support for the roles are lacking. This paper explores the role of executive and senior management understanding in the employment of peer roles. In‐depth, semi‐structured interviews and one focus group were conducted with 29 participants from a range of nongovernment and public mental health services within Queensland, Australia. Findings of this study suggest management exposure to and understanding of peer work are essential to the development of an effective peer workforce. Exposure and understanding of peer roles reportedly led to greater acceptance and commitment from management. This commitment inspired action in the form of; enhanced support including advocacy/championing, increased development of roles and influenced culture within the organization to be more accepting of peer work. There was a perception that developing an 'accepting' organizational culture supported the integrity and expansion of peer roles including designated 'peer management' positions. Development of peer management positions is suggested as an effective means of improving the impact of peer perspectives, advocating for peer work and providing ongoing and timely supervision. Recommendations include the need for training and information for management on the unique function, purpose and value of peer roles; and the development of networks, including mentoring opportunities, for organizations with limited experience to gain support and advice from those with greater experience developing peer roles. With growing concerns about the efficacy and costs of intrusive child protection interventions and increasing recognition of citizen rights to participation in governance, jurisdictions are looking to collaborative alternatives that divert families from the courts and out‐of‐home care. In Queensland (Australia), "intervention with parental agreement" (IPA) is one such response. Under IPA, the statutory child protection authority can work collaboratively with families, without a court order, to respond to children assessed as "in need of protection." In this paper, we use procedural justice theory as a lens to explore how IPA policy is enacted in practice. Procedural justice relates to the quality of treatment a person receives and the fairness of the process than an authority uses during decision‐making (Tyler,). It is associated with voluntary cooperation and compliance; key practice concerns when working to address child maltreatment via parental agreement rather than court ordered intervention. We report on a qualitative study that utilized semistructured, in‐depth interviews to capture the perceptions of 30 practitioners regarding the factors that shape the extent to which they enact IPA policy in a procedurally fair manner. Parental substance misuse has a pervasive impact on family functioning, parenting, and, ultimately, child wellbeing and development. Subsequently, linkages with informal and formal support networks are crucial for ameliorating risk. To facilitate engagement with these families prior to identification in child protection systems, it is vital to understand the factors inhibiting and promoting engagement with informal and formal support. This paper examines how different factors influence informal and formal help‐seeking by alcohol‐misusing parents with regard to parenting and family concerns. Parents reporting alcohol problems in a clinical range (n = 322) were drawn from a representative parent sample (n = 1991). Alcohol‐misusing parents reported low help‐seeking for parenting‐focused support services. The findings particularly highlight the role of age, education, socioeconomic status, ethnicity, parenting stress, empowerment and trust in support services for predicting parents' help‐seeking. Objective This paper contributes to the literature on relocation for specialist care by providing findings on specific issues impacting on rural farmers and property owners who have to travel to the metropolitan area for specialist care for a haematological malignancy. Design and setting This paper uses descriptive qualitative research based on 45 interviews with patients with haematology in Queensland. The interviews were audio-recorded, transcribed verbatim, coded and thematically analysed. Results In addition to issues of distance, farmers and rural property owners who have to relocate for specialist care must deal with problems associated with the lack of opportunity to take absence from the property because of the inescapable pressure of daily farm and property responsibilities and the high cost of, or lack of opportunity to, outsource daily maintenance. Further concerns include the cost of relocation in the context of continuing drought, serious problems sustaining the travel and time away required, and the lack of choice for some but to deal with treatment alone. Conclusion In recent years there has been considerable progress with regard to overcoming the distance barrier for rural and remote patients with cancer through innovative clinical models using technology and telemedicine. However, there has been limited uptake of such models for patients with haematology. The present findings indicate that from the perspective of rural farmers and property owners there are important reasons why the use of innovative strategies should be fostered and expanded. Objective To examine the association between residential remoteness and stage of cancer at diagnosis, treatment uptake, and survival within the Australian Indigenous population. Design Systematic review and matched retrospective cohort study. Setting Australia. Participants Systematic review: published papers that included a comparison of cancer stage at diagnosis, treatment uptake, mortality and/or survival for Indigenous people across remoteness categories were identified (n = 181). Fifteen papers (13 studies) were included in the review. Original analyses: new analyses were conducted using data from the Queensland Indigenous Cancer Study (QICS) comparing cancer stage at diagnosis, treatment uptake, and survival for Indigenous cancer patients living in rural/remote areas (n = 627, 66%) and urban areas (n = 329, 34%). Main Outcome Measures Systematic review: Papers were included if there were related to stage of disease at diagnosis, treatment, mortality and survival of cancer. Restrictions were not placed on the outcome measures reported (e.g. standardised mortality ratios versus crude mortality rates). Original analyses: Odds ratios (OR, 95%CI) were used to compare stage of disease and treatment uptake between the two remoteness groups. Treatment uptake (treated/not treated) was analysed using logistic regression analysis. Survival was analysed using Cox proportional hazards regression. The final multivariate models included stage of cancer at diagnosis and area-level socioeconomic status (SEIFA). Results Existing evidence of variation in cancer outcomes for Indigenous people in remote compared with metropolitan areas is limited. While no previous studies have reported on differences in cancer stage and treatment uptake by remoteness within the Indigenous population, the available evidence suggests Indigenous cancer patients are less likely to survive their cancer the further they live from urban centres. New analysis of QICS data indicates that Indigenous cancer patients in rural/remote Queensland were less likely to be diagnosed with localised disease and less likely to receive treatment for their cancer compared to their urban counterparts. Conclusion More research is needed to fully understand geographic differentials in cancer outcomes within the Indigenous population. The incidence of chronic illness is growing globally. As a result, there are fiscal and social implications for health delivery. Alongside the increased burden on health resources is the expectation that someone within the family will assume the responsibility of carer for those who are chronically ill. The expectation to assume the role of carer may be amplified for family members who are also nurses. Currently, there is little research that investigates the impact of nurses who are carers for family with a chronic illness. Consequently, this qualitative study, based on face‐to‐face and telephone dialogue, was conducted using unstructured interviews. A phenomenological‐hermeneutic approach utilizing the hermeneutic circle was employed to understand the meaning of the transcriptions. Hermeneutic phenomenology inspired by Heideggerian philosophy was used as a framework to clarify themes and build ways of understanding the phenomenon of ‘Being’ a nurse and a family carer. Themes identified in the analysis of the study included a personal world, a professional world and a practical world. This paper explicates the findings from examination of the personal world of ‘Being’ a nurse who is also a family carer. Analysis revealed there were three facets to this way of ‘Being’: a shared experience, a caring experience and a fraught experience. The findings of the study disclosed that participants were informed by experiences that were both enriching and conflicting. These insights can enlighten healthcare professionals to the tensions which exist for nurses who care for family so that these considerations are incorporated into individualized and effective family‐centred care. Objective This paper describes the sociodemographic and health-related characteristics of people with chronic disease attending an interprofessional student-assisted clinic in regional Queensland. Design A retrospective review of data collected during the first 10 months of operation of the clinic was conducted. Setting, participants and outcome measures Data was collected on up to 378 patients during an intake appointment at the Capricornia Allied Health Partnership ( CAHP) community-based clinic and compared with normative reference groups where available. Sociodemographic characteristics included age, gender and education level; health-related characteristics included body mass index and hospitalisations in the previous 12 months; and risk factors included prescribed medications, smoking status and general practitioner-diagnosed medical conditions. Results Patients attending the CAHP clinic had a mean number of chronic conditions of 4.9 ± 2.1 per patient, and 97% of patients had multimorbidities. A high level of socioeconomic disadvantage was found in comparison with normative comparison groups based on employment, highest level of schooling completed and the index of social disadvantage. Patients predominantly lived in inner regional areas (76.7%). The most common diagnoses of patients attending the clinic for the first time were hypertension, osteoarthritis, high cholesterol, diabetes and chronic back pain. Conclusions The CAHP clinic offers a unique student-assisted service model for interprofessional management of patients who are socioeconomically disadvantaged, have multimorbid chronic disease and live in regional areas. The description of baseline data in this paper is important to refine clinic services, to guide other chronic disease clinics and to inform future research study designs. [ABSTRACT FROM AUTHOR]

ABSTRACT Having sufficient power to detect effect sizes of an expected magnitude is a core consideration when designing studies in which inferential statistics will be used. The main aim of this study was to investigate the statistical power in studies published in the International Journal of Mental Health Nursing. From volumes 19 (2010) and 20 (2011) of the journal, studies were analysed for their power to detect small, medium, and large effect sizes, according to Cohen's guidelines. The power of the 23 studies included in this review to detect small, medium, and large effects was 0.34, 0.79, and 0.94, respectively. In 90% of papers, no adjustments for experiment-wise error were reported. With a median of nine inferential tests per paper, the mean experiment-wise error rate was 0.51. A priori power analyses were only reported in 17% of studies. Although effect sizes for correlations and regressions were routinely reported, effect sizes for other tests (χ2-tests, t-tests, ANOVA/MANOVA) were largely absent from the papers. All types of effect sizes were infrequently interpreted. Researchers are strongly encouraged to conduct power analyses when designing studies, and to avoid scattergun approaches to data analysis (i.e. undertaking large numbers of tests in the hope of finding 'significant' results). Because reviewing effect sizes is essential for determining the clinical significance of study findings, researchers would better serve the field of mental health nursing if they reported and interpreted effect sizes. There has been limited sustained analysis of the lived reality of citizenship amongst Indigenous Australians exempted from protectionist legislation last century. Through examining the diaries of Daisy Smith, this paper explores how her household, exempted from such legislation in Queensland, experienced citizenship during the 1940s and early 1950s. The diaries show that Daisy Smith and her family exercised civil, political and social rights of citizenship, enjoyed meaningful lives and had a sense of belonging to the Australian nation. However, they also show that Daisy Smith constantly worked hard to uphold the conditions of exemption and protect her family's exempted Aboriginal status. These conditions required the Smith family to deny and relinquish all connection with their Indigenous heritage and embrace the dominant Anglo-Celtic culture or risk losing exemption. In this way, their inclusion in the Australian nation and enjoyment of citizenship rights were conditional upon them being manifestly normative Anglo-Celtic citizens. Problem The continuity of care for people with neurological conditions in a remote northwest Queensland town as services are currently only available intermittently. Design Mixed methods design using questionnaires and staff review of the program and processes. Setting Intermittent community rehabilitation service for clients with neurological conditions has been offered in Mount Isa and is supported by a similar fulltime service in Townsville. Both services use a unique client-centred, student-assisted, interprofessional model of care. Key measures for improvement Understanding participant experiences by obtaining feedback from clients, students and allied health professionals ( AHPs) regarding their experiences of using telehealth in this setting. Strategy for change Previous clients of the North West Community Rehabilitation service were offered a review assessment using telehealth by an interprofessional team. Effects of change Using telehealth enabled the client, remote AHP and students in Mount Isa to be connected to expert assistance in Townsville. Lessons learnt The findings suggest that telehealth was useful in a community rehabilitation setting to provide review services for clients. This improved continuity of care for these clients because without this telehealth assessment, the clients would have had to wait up to 12 months for the next service period in Mount Isa or travel to a major urban centre to access a similar service. Feedback from clients, students and AHPs was positive; however, some challenges were identified. Recommendations for future service delivery using telehealth are outlined in the paper. ABSTRACT This paper examines a recent dispute generated by the Queensland State Government proposal to build the Traveston Crossing Dam on the Mary River in southeast Queensland, Australia. It is particularly concerned with the ways in which interrelated issues of belonging, community identity, and social diversity were negotiated during the anti-dam campaign. As an unusual alliance of farmers, environmentalists, urban retirees, some Aboriginal people and others, it takes a view of the anti-Traveston Crossing Dam campaign as a fluid network of people and approaches the notion of community identity as the symbolic construction of similarity. Locally specific, the paper describes pertinent aspects of community politics in the context of rural socioeconomic change, and the mobilisation of heritage. With regard to local senses of belonging, it also discusses the involvement of Aboriginal people during the campaign. More broadly then, this paper attempts to make an ethnographic contribution to the study of environmental disputes and the politics of alliance in peri-urban areas of settler-descendant societies such as Australia. Objective To examine urban-rural differences and individual risk factors for a late stage of breast cancer at diagnosis in Australian women. Design Individual-level longitudinal data were linked with cancer registry data from New South Wales (New South Wales Cancer Registry linked by the Centre for Health Record Linkage (CHeReL)), Queensland (Queensland Cancer Registry) and Victoria (The Cancer Council Victoria). Setting Participants were drawn from the Australian Longitudinal Study on Women's Health 1946-1951 cohort (n = 13 715). Participants The sample included 195 women identified from the linked cancer registry data with a breast cancer diagnosis. Interventions Rural or urban residence was measured using Accessibility/Remoteness Index of Australia Plus (ARIA+). Individual characteristics and socio-demographic variables examined included survey year, menopausal status, country of birth, education and marital status. Main outcome measures A late stage of breast cancer at diagnosis was defined based on the TNM Classification of Malignant Tumours. Results A late stage of breast cancer diagnosis was observed in 36% of women residing in urban areas and 40% of women residing in rural areas. After adjusting for individual characteristics, we found that obesity was the strongest risk factor for a late stage of breast cancer at diagnosis. Conclusions Given that women are becoming increasingly obese, and that the rate of obesity is higher in the Australian rural population, this paper provides further evidence for targeting interventions for obesity, particularly in rural Australia, as a public health priority. Introduction and Aims: The lockout intervention has become embedded in Australian alcohol policy with little scientific evidence of its effectiveness in reducing violence and disorder. This paper reports an evaluation of the Queensland lockout pilot in Surfers Paradise. Patrons could not enter or re-enter licensed venues after 3 am, while patrons inside at this time could stay until close.Design and Methods: Using police and ambulance data, time series analyses examined the impact of tourism seasons and the lockout on rates of crime, violence, injury and intoxication. Additional analyses were also conducted to show spatial and temporal changes in crime over time.Results: Both police and ambulance data showed that the lockout introduction had no statistically significant impact on rates of crime, violence, head and neck injuries, and intoxication over the 2 years following lockout. Hot spot maps indicated limited spatial shift of crime within Surfers Paradise following the lockout introduction, with evidence of a temporary intensification of crime in already established hot spots. We found a moderate statistically significant change in the 24 h distribution of crime after the lockout implementation, suggesting temporal displacement of crime.Discussion: Results support the small existing body of evidence on lockouts that indicates they are largely ineffective in reducing crime and injuries in entertainment districts.Conclusion: As multi-pronged strategies that include a lockout gain in popularity, further investigation should focus on identifying the key drivers of successful interventions such as the Newcastle strategy, to better refine these interventions for replication and evaluation elsewhere.
