1. Postoperative fibrinogen‐to‐albumin ratio acting as an indicator of futile recanalization in patients with successful thrombectomy

Abstract

Abstract Background Timely recognition of futile recanalization might enable a prompter response and thus improve outcomes in patients receiving successful thrombectomy. This study aims to evaluate whether postoperative fibrinogen‐to‐albumin ratio (FAR) could act as an indicator of futile recanalization. Methods This is a single‐center, retrospective analysis of patients with acute anterior circulation large‐vessel occlusion and successful thrombectomy between May 2019 and June 2022. FAR was defined as postoperative blood levels of fibrinogen divided by those of albumin, and dichotomized into high and low levels based on the Youden index. Futile recanalization was defined as patients achieving a successful recanalization with a modified Rankin Scale score of 3–6 at 90 days. Multivariable logistic regression was used to assess the association of FAR with futile recanalization. Results A total of 255 patients were enrolled, amongst which 87 patients (34.1%) had high postoperative FAR. Futile recanalization was more prevalent among patients with high FAR compared to those with low FAR (74.7% vs. 53.0%, p = .001). After adjusting for potential confounders, high postoperative FAR was found to independently correspond with the occurrence of futile recanalization (adjusted OR 2.40, 95%CI 1.18–4.87, p = .015). This association was consistently observed regardless of prior antithrombotic therapy, treatment of intravenous thrombolysis, occlusion site, time from symptom onset to groin puncture, and reperfusion status. Conclusion Our findings support high postoperative FAR serving as an indicator of futile recanalization in patients with anterior circulation large‐vessel occlusion and successful thrombectomy.

Abstract Background To evaluate the efficacy and safety of pegylated recombinant human granulocyte colony‐stimulating factor (PEG‐rhG‐CSF) in preventing neutropenia during multiple cycles of chemotherapy in patients with non–small cell lung cancer (NSCLC). Method In a multicenter, prospective, randomized trial, patients with NSCLC were randomly assigned in a 2:1 ratio to treatment group (PEG‐rhG‐CSF as primary prophylactic therapy) or control group. Patients in the control group were administered rhG‐CSF when white blood cell count was 0.05). Adverse events were reported in 47.13% of patients in the treatment group and 48.84% patients in the control group. Conclusions Primary prophylactic treatment with PEG‐rhG‐CSF could reduce the incidence of neutropenia in patients with NSCLC during multiple cycles of chemotherapy, with acceptable safety and tolerability.

Abstract

Digital rock twins are widely used to obtain hydraulic properties of porous media by simulating pore-scale fluid flow. Multifractal characteristics of pore geometry and flow velocity distribution have been discovered with two-dimensional (2D) images and three-dimensional (3D) models, whereas the dependency of results on the resolution is not well known. We investigated resolution-dependent multifractal properties of 3D twin models for a sandstone sample with originally 3 μm resolution images. 3D pore-scale water flow was simulated with the lattice Boltzmann method (LBM). As indicated by multifractal analyses, the generalized dimension spectra, the Hölder exponent spectra, and singularity spectra of the flow velocity are similar to that of the pore geometry but different in ranges and sensitivities to the change in the model resolution. Nonlinear dependencies of 2D/3D porosity, holistic/slice permeability, equivalent pore radius squared, and multifractal parameters on the resolution were discussed.

Abstract

Ionotropic γ-aminobutyric acid (GABA) receptor (GABAR) in an insect is the major inhibitory receptor and is one of the most important targets for insecticides. Due to the high spatiotemporal resolution of GABAR, the photopharmacological ligands acting on it in vertebrates but not insect have been developed.In this study, two types of photochromic ligands (PCLs) including DTFIPs (DTFIP1 and DTFIP2) and ABFIPs (p-, m-, and o-ABFIP) were synthesized by incorporating photoswitch azobenzene or dithienylethene into fipronil (FIP), which is the antagonist of insect GABAR. Their photomodulation was measured by mosquito larval behavior, and their potential action mechanism was explored by the two-electrode voltage clamp (TEVC) technique in vitro. DTFIP1 and m-ABFIP exhibited the most significant difference of insecticidal activity by about 90- and 5-fold to mosquito larvae between non-irradiated and irradiated formation, respectively, and allowed for optical control of mosquito swimming activity. TEVC assay results indicated that m-ABFIP and DTFIP1 enable optical control over the homomeric LsRDL-type GABAR, which is achieved by regulating the chloride channel of resistance to dieldrin (RDL)-type GABAR by photoisomerization.Our results suggested that PCLs synthesized from fipronil provide an alternative and precise tool for studying insect ionotropic GABARs and GABA-dependent behavior. © 2022 Society of Chemical Industry.

Abstract

Straightforward solutions have long been expected for the analysis of multiwell aquifer tests. In this paper, we derive series analytical solutions of steady-state groundwater flow in a rectangular-shaped aquifer with pumping/injection wells for both confined and unconfined conditions. Double Fourier Transform (DFT) technique is applied to deal with different combinations of impermeable and specified head boundaries on sides. The obtained solutions are compact and concise in mathematics and flexible in terms of well number, well locations, and pumping/injection rates. Hatoucaidang, a groundwater resource field in the Ordos Plateau, Northwestern China, is introduced as a field case study, where a multiwell aquifer test was conducted. One of the analytical solutions derived herein is used to estimate hydraulic conductivities by applying a direct calculation method and a least square estimation method regarding observed versus calculated drawdowns. By comparing with nearby single-well pumping tests, the reliability of the derived analytical solutions is proven. This study facilitates utilizing the multiwell aquifer test to analyze the general behavior of groundwater movement in aquifer systems.

Abstract

On the basis of the higher order perturbation formulas of g-factors for a 4d1 group in tetragonally compressed octahedra, the g-factors, optical absorption, local structure and their concentration dependences of pentavalent molybdenum in 40PbO-(10-x)Y2 O3 -50P2 O5 :xMoO3 (1 ≤ x ≤ 5 mol%) glass are uniformly investigated. The experimental optical absorption spectra and g factors for Mo5+ at various concentrations x are reasonably regenerated by using the reasonable exponential concentration functions of the cubic field parameter Dq, covalent factor N and relative tetragonal compression ratio ρ. The tetragonal compression ratio ρ due to the Jahn-Teller effect was found in the range of 3.8-4.2%. The decreasing trend of Dq and N and the increasing trend of ρ with concentration can be interpreted as the fact that the variations of MoO3 and Y2 O3 concentrations lead to the modulations of local structure and electron cloud distribution around Mo5+ , associated with the adjustment of the glass network. The concentration dependence of optical basicity is also analyzed for the above glass systems.

Abstract

Background This study aimed to retrospectively explore the safety and feasibility of computed tomography (CT)‐guided synchronous microwave ablation (MWA) followed by core‐needle biopsy (CNB) via a coaxial cannula for highly suspected malignant lung ground‐glass opacities (GGOs). Methods The clinical data of 66 patients (66 GGOs) treated with CT‐guided synchronous MWA followed by CNB via a coaxial cannula from January 2019 to January 2021 were included in this study. The technical success rate, curative effect, and complications were evaluated. Results Technical success rates were 100%. The pneumothorax rate was 36.4% (24/66). 72.7% (48/66) patients had the bronchopulmonary hemorrhage, 81.3% of hemorrhage was attributable to CNB. 24.2% (16/66) patients had varying degrees of pleural effusion. The pathological results were adenocarcinomas (n = 44), atypical adenomatous hyperplasia (n = 2), chronic inflammation (n = 3) and indeterminate pathological diagnosis (n = 17) with a 69.7% (46/66) positive diagnosis rate. The therapeutic response rate was 100.0% (66/66). Conclusions Synchronous MWA followed by CNB via a coaxial cannula has a satisfactory ablation effectiveness and an acceptable biopsy positive rate, which is an alternative treatment for highly suspected malignant GGOs., Synchronous microwave ablation followed by core‐needle biopsy can reduce the influence of biopsy‐induced pneumothorax and pulmonary hemorrhage on microwave ablation, which has a satisfactory therapeutic effect and an acceptable diagnostic positivity rate.
