Abstract

Abstract Dulaglutide is a glucagon‐like peptide‐1 (GLP‐1) receptor agonist, which stimulates insulin release via activation of GLP‐1 receptors on pancreatic beta cells. We report the case of a 45‐year‐old woman with type 2 diabetes mellitus who started dulaglutide subcutaneously and developed, a few minutes after the fourth administration, a persistent itchy reaction at the injection site, followed, after two more administrations, by a diffuse itchy delayed urticaria‐like rash, healed in 7 days after drug interruption. Skin test performed after 3 months from the event demonstrated the culprit role of the drug. Given the limited evidence in literature, there is currently no standardised protocol for testing patients sensitised to one or more glucagon‐like peptide‐1 receptor agonists. Performing skin tests, especially intradermal tests, is pivotal to confirm the aetiological role of the drug in the suspected hypersensitivity reaction, and possibly to identify an alternative to be proposed before precluding the entire drug class. To the best of our knowledge, this is the first case of systemic hypersensitivity reaction to dulaglutide presenting as a delayed urticaria‐like rash and confirmed by positive skin test.

Abstract

Abstract We present a 53‐year‐old male with nonketotic hyperglycinemia (NKH) who presented in decompensated state to our university hospital several months prior to a primary diagnosis of multifocal pneumonia accompanied by reports of seizure‐like activity, altered mental status, tremors, and fever. He was initially diagnosed with NKH in his preschool years, over 40 years previously, along with his younger sister. At that time, he had developmental and physical delays (which his sister also experienced). His health course has been relatively uneventful otherwise, as regards decompensation of his disease, and he has not been on the standard regimens of reduced dietary glycine intake along with dextromethorphan and sodium benzoate. Recent molecular confirmation of NKH was completed and both he and his sibling likely have an attenuated form of NKH mediated by the combined effects of their variants. This paper presents what we believe to be report of the oldest surviving individuals with attenuated NKH.

Abstract

Background Moderate-to-severe atopic dermatitis (AD) in the adolescence is a high burden disease, and its treatment can be very challenging due to paucity of approved systemic drugs for this age and their side-effects. Dupilumab was recently approved for treatment of adolescent AD. Objectives A multicentre, prospective, real-world study on the effectiveness and safety of dupilumab in adolescents (aged from >= 12 to <18 years) with moderate-to-severe AD was conducted. The main AD clinical phenotypes were also examined. Methods Data of adolescents with moderate-to-severe AD treated with dupilumab at label dosage for 16 weeks were collected. Treatment outcome was assessed by EASI, NRS itch, NRS sleep loss and CDLQI scores at baseline and after 16 weeks of treatment. The clinical scores were also evaluated according to clinical phenotypes. Results One hundred and thirty-nine adolescents were enrolled in the study. Flexural eczema and head and neck eczema were the most frequent clinical phenotypes, followed by hand eczema and portrait-like dermatitis. Coexistence of more than 1 phenotype was documented in 126/139 (88.5%) adolescents. Three patients (2.1%) contracted asymptomatic SARS-CoV-2 infection and 1 of the discontinued dupilumab treatment before the target treatment period. A significant improvement in EASI, NRS itch, NRS sleep loss and CDLQI was observed after 16 weeks of treatment with dupilumab. This outcome was better than that observed in clinical trials. Dupilumab resulted effective in all AD phenotypes, especially in diffuse eczema. Twenty-eight (20.1%) patients reported adverse events, conjunctivitis and flushing being the most frequent. None of patients discontinued dupilumab due to adverse event. Conclusions Dupilumab in adolescent AD showed excellent effectiveness at week 16 with consistent improvement of all clinical scores. Moreover, dupilumab showed a good safety profile also in this COVID-19 pandemic era.

Abstract

Exploratory testing and fully automated testing tools represent two viable and cheap alternatives to traditional test‐case‐based approaches for graphical user interface (GUI) testing of Android apps. The former can be executed by capture and replay tools that directly translate execution scenarios registered by testers in test cases, without requiring preliminary test‐case design and advanced programming/testing skills. The latter tools are able to test Android GUIs without tester intervention. Even if these two strategies are widely employed, to the best of our knowledge, no empirical investigation has been performed to compare their performance and obtain useful insights for a project manager to establish an effective testing strategy. In this paper, we present two experiments we carried out to compare the effectiveness of exploratory testing approaches using a capture and replay tool (Robotium Recorder) against three freely available automatic testing tools (AndroidRipper, Sapienz, and Google Robo). The first experiment involved 20 computer engineering students who were asked to record testing executions, under strict temporal limits and no access to the source code. Results were slightly better than those of fully automated tools, but not in a conclusive way. In the second experiment, the same students were asked to improve the achieved testing coverage by exploiting the source code and the coverage obtained in the previous tests, without strict temporal constraints. The results of this second experiment showed that students outperformed the automated tools especially for long/complex execution scenarios. The obtained findings provide useful indications for deciding testing strategies that combine manual exploratory testing and automated testing.

Abstract

Spreadsheet applications are widely adopted by millions of end users from several application domains and provide strategic support to many business, scientific, industrial, and organizational processes. These applications are usually developed by rapid application development processes, exploiting host scripting languages allowing the basic spreadsheets to provide complex functionality, business rules, and user interfaces. Several factors complicate the comprehension of these applications because they are usually developed and maintained by end users without specific software engineering skills, grow over time, are not adequately documented, and do not present explicit separation between data, business logic, and user interface layers. This paper presents a reverse engineering tool intended to support the comprehension of Excel spreadsheet applications developed using the Visual Basic for Application programming language. The tool has been implemented as an add-in that extends the Excel working environment by providing analysis and visualization features. It is able to extract information about the elements composing the analyzed Excel spreadsheet application, the functionality it exposes through its user interface, and the dependencies among its cells. This information is provided by means of interactive views. The validity of the tool has been assessed by a qualitative case study performed with professional end users from an automotive industrial domain. Copyright © 2016 John Wiley & Sons, Ltd.

Abstract

BACKGROUND AND PURPOSE The tachykinin NK2 receptor antagonist ibodutant is under Phase III clinical investigation to treat female patients with irritable bowel syndrome. The aim of this study was to investigate the NK2 receptor-related gender specificity in a model of colitis. EXPERIMENTAL APPROACH Colitis was induced by rectal instillation of 2,4,6-trinitrobenzenesulfonic acid (TNBS, 0.5 mL, 30 mg·mL(-1) in 30% ethanol) in female and male guinea pigs. Electromyographic recording of the responses to colorectal distension (CRD) was made 3 days later. Ibodutant (0.33 , 0.65, 1.9 and 6.5 mg·kg(-1) ) was given s.c., 30 min before CRD. Release of neurokinin A and substance P from isolated mucosal and smooth muscle tissues following treatment with KCl (80 mM) or capsaicin (10 μM) was measured by EIA. Plasma pharmacokinetics of ibodutant following a single s.c. administration (0.73 or 2.1 mg·kg(-1) ) were measured over 24 h. KEY RESULTS Ibodutant did not affect abdominal contractions in control animals. After TNBS-induced colitis, ibodutant prevented the increased visceral hypersensitivity to CRD in females, at lower doses than in males. Ibodutant pharmacokinetics did not differ between females and males. Tachykinins release was greater in smooth muscle than in mucosal samples. Capsaicin-stimulated release of tachykinins from inflamed mucosal samples from females was significantly lower than in males. CONCLUSIONS AND IMPLICATIONS Ibodutant prevented abdominal nociception in a model of visceral hypersensitivity in guinea pigs with a greater efficacy in females than in males. Our results highlight a gender-related difference in colonic visceral hypersensitivity and mucosal nerve activation.
