Abstract

Background: The Beckman Coulter DxH 900 is a haematological analyser capable of counting and sizing blood cells, and obtaining a complete blood cell count (CBC). This analyses different parameters of red blood cells (RBC), platelets and white blood cells/leukocytes. Some automated CBC counters present limitations due to specimen characteristics, abnormal cells or both factors. In the presence of abnormalities, the DxH 900 has a flagging system, warning the laboratory technician that something needs to be verified. In the present work, we evaluated samples from oncologic patients, presenting a population erroneously perceived as being lymphocytes. The most common explanations for this situation are RBC resistant to lysis or serum hyperbilirubinaemia., Methods: In an attempt to solve and understand what the cause of this problem might be, we diluted our samples (1:3) and analysed the serum total bilirubin. To identify cells' abnormalities, the samples were also analysed by manual DLC counts. During the study, we also checked the different flags presented by the equipment., Results: The results evidenced that the major interference was due to RBC lysis resistance, corresponding to 94.7% of the cases, while hyperbilirubinaemia was only present in 73.4%. Besides, we determined that some samples with normal bilirubin levels also presented interference, suggesting that hyperbilirubinaemia was not the main cause of the error. The most recurrent flag observed was "High event rate"., Conclusion: The dilution solved all of the observed interferences. The results between diluted and manual counts showed a strong correlation, leading us to introduce dilution in our laboratory routine., (© 2024 The Authors. Abstract

Over the last 30 years, research into HIV has advanced the knowledge of virus genetics and the development of efficient therapeutic strategies. HIV-1 viral protein R (Vpr) is a specialized and multifunctional protein that plays important roles at multiple stages of the HIV-1 viral life cycle. This protein interacts with a number of cellular and viral proteins and with multiple activities including nuclear transport of the pre-integration complex (PIC) to the nucleus, transcriptional activation, cell cycle arrest at G2/M transition phase and induction of cell death via apoptosis. Specifically, Vpr has been shown to control many host cell functions through a variety of biological processes and by interaction with several cellular pathways. The different functions of Vpr may enhance viral replication and impair the immune system in HIV-1 infected patients. Importantly, functional defects induced by mutations in the Vpr protein correlate with slow disease progression of HIV-infected patients. Vpr is also associated with other concomitant pathologies developed by these patients, which may lead it to be considered as a potential novel therapeutic target. This review will focus on HIV-1 Vpr, mainly on the importance of its structural mutations on the progression of HIV infection, associated phenotypes and relevance for clinical pathologies. Abstract

Objectives: A randomized crossover trial was designed to analyze the impact of a short-term, isoenergetic fat-rich or carbohydrate (CHO)-rich diet on substrate oxidation rates during submaximal exercise and on performance in a 10,000-m running time trial in trained, mid- to late-pubertal boys., Methods: An incremental test was performed to determine the peak oxygen uptake (VO2peak). After 2 days on a fat-rich (24.2% ± 0.8% CHO, 60.4% ± 0.3% fat, and 15.5% ± 1.0% protein), CHO-rich (69.3% ± 1.2% CHO, 15.9% ± 2.1% fat, and 15.1% ± 1.1% protein), or habitual (56.1% ± 7.0% CHO, 27.5% ± 4.9% fat, and 16.5% ± 4.0% protein) diet, 19 trained adolescent boys (15.2 ± 1.5 years) performed a 10-minute constant run at 65% VO2peak to determine the respiratory exchange ratio (RER) during exercise and 10,000-m running on an outdoor track., Results: During the constant run, the RER and CHO contribution to energy expenditure were lower, and fat contribution higher, in the fat-rich diet than in the CHO-rich diet (P < 0.05), but the results were not different from those of the habitual diet. Performance in the 10,000-m run after consuming CHO- and fat-rich diets was similar to performance after a habitual diet (50.0 ± 7.0, 51.9 ± 8.3, and 50.9 ± 7.4 minutes, respectively), but consuming a CHO-rich diet enhanced performance compared with that after a fat-rich diet (P = 0.03)., Conclusions: These findings indicate that a CHO-rich diet provides additional benefits to 10,000-m running performance in trained adolescent boys compared with a fat-rich diet.

Abstract

Objectives: To evaluate the efficacy of the endometrial thickness measurement through transvaginal ultrasound as an orienting factor for the medical treatment of unruptured tubal pregnancy with a single dose of methotrexate in order to select the best cases for the medical treatment., Methods: A prospective study, in which the largest measurement of endometrial thickness in millimeters was evaluated, along the longitudinal uterine axis, by means of transvaginal ultrasound. A total of 38 patients meeting the inclusion criteria for single-dose methotrexate treatment [50 mg/m2 intramuscularly (i.m.)] were included in the study. We observed whether there was any difference in mean value of endometrial thickness and initial beta-human choriongonadotropin levels, between the cases that evolved successfully with the treatment, and those that failed., Results: The mean value of endometrial thickness and initial beta-human choriongonadotropin (beta-hCG) levels, for patients evolving successfully with medical treatment (28 cases), were 6.39 mm and 1936.2 mUI/ml, respectively, while the mean values for failures were 11.70 mm and 6831.3 mUI/ml. We carried out statistical analysis using the 'Student's t-test', with p < 0.05., Conclusions: The mean value of endometrial thickness, along the longitudinal uterine axis through transvaginal ultrasound, reflects hormonal action and has been demonstrated to be another important parameter in indicating the medical treatment of an unruptured tubal pregnancy.
