Summary: Monotherapy is the policy for management of patients with epilepsy. With increasing knowledge of the biology of epilepsy and of the modes of action of antiepileptic drugs (AEDs), this concept must be reevaluated. When monotherapy fails to control seizures, subsequent treatment should be based on “rational pharmacology,” taking into consideration the mode of action of the drugs, to provide improved efficacy with maintained tolerance and ease of administration. Introduction of vigabatrin (VGB) as a new AED calls for just such a reevaluation. VGB is an enzyme-activated irreversible inhibitor of γ-aminobutyric acid (GABA)-transaminase that increases brain and cerebrospinal (CSF) GABA concentrations in animals and humans. It has limited efficacy in the classic animal seizure screening tests, but in many clinical studies has halved the incidence of seizures in ∼50%of patients, especially those with partial epilepsies. We evaluated the efficacy of VGB in “socially integrated and active outpatients” as a likely subset to demonstrate any advantage of rational polytherapy. The criteria for this evaluation included the effects on seizure frequency, patient tolerability, and cognitive performance in a battery of psychometric tests. Fourteen of the 19 patients (73%) completing the study had >50% reduction in seizure frequency, and 10 of 19 (52%)had >70% reduction in seizure frequency. Tolerability appeared good; somnolence was the most frequent adverse event. Three patients complained of a worsening of their seizures, 1 with an increase in frequency and 2 with development of myoclonic jerks not previously reported. No deleterious effect of VGB on cognitive function was noted, and performance in late recognition tests showed significant improvement by the end of the maintenance period, which may have been due to the reduction in seizure frequency. The efficacy of VGB in this study in controlling seizures as compared with results of earlier published studies of refractory epilepsy may be related to selection of patients with more benign partial epilepsy, but does support the need for further studies in such patients to evaluate the hypothesis of rational polytherapy. RESUMEN La monoterapia es una actitud general para el control de enfermos con epilepsia. A medida que se conoce mejor la biologia de la epilepsia y los modos de accion de las medicaciones antiepilkpticas (AED) este concepto debe ser nuevamente valorado. Cuando la monoterapia falla en el control de los ataques el tratamiento subsiguiente debe basarse en una “farmacologia racional” teniendo en cuenta el modo de accion de las drogas para proporcionar una mejor eficacia, una tolerancia mantenida y una administracion sencilla. La introduccion de la vigabatrina (VGB) como una nueva AED justifica esta nueva valoracion. La VGB es un inhibidor irreversible enzimaticamente activado de la GABA-transaminasa que incrementa las concentraciones del GABA en el liquido cefalorraquideo en animales y en humanos. Tiene una eficacia limitada en los ataques de modelos animales pero, en numerosos estudios clinicos, ha reducido la incidencia de ataques en cerca del 50% de los enfermos, especialmente los que padecen epilepsias parciales. En este estudio se ha evaluado la eficacia de la VGB en “pacientes activos e integrados socialmente” que constituyen un grupo en el se pueden detectar las ventajas de una politerapia racional. Los criterios para esta valoracion incluyeron los efectos sobre la frecuencia de los ataques, la tolerancia del enfermo. Y las funciones cognitivas determinadas mediante una bateria de tests psicomktricos. Catorce de 19 enfermos (73%) que completaron el estudio demostraron una reduccion de la frecuencia de ataques superior a1 50% y 10 de 19 (52%) una reduccion de la frecuencia por encima del 70%. La tolerancia fue buena y la somnolencia fue el sintoma adverso mas frecuente. Tres pacientes se quejaron de un empeoramiento de los ataques. En uno, la frecuencia se incremento y en dos aparecieron movimientos mioclonicos que no habian ocurrido previamente. No se observaron alteraciones de las funciones cognitivas durante el tratamiento con VGB y los test de reconocimiento tardio mostraron una mejoria significativa al final del periodo de mantenimiento terapkutico. Este resultado puede ser secundario a la reduccion del ndmero de ataques. La eficacia de la VGB en el control de los ataques, cuando se compara con estudios previamente publicados en epilepsia resistentes a1 tratamiento, puede estar en relacion con la seleccidn de enfermos que padecian una epilepsia parcial mas benigna. Sin embargo este estudio apoya la necesidad de estudios subsiguientes en este tipo de enfermos para valorar la hipotesis de la “politerapia racional.” ZUSAMMENFASSUNG In der Regel wird bei Patienten mit Epilepsie eine Monotherapie angestrebt. Mit zunehmendem Wissen uber die Biologie der Epilepsie und die Modalitaten der Wirkung antiepileptischer Substanzen mu13 dieses Konzept uberdacht werden. Bleibt der Therapieerfolg bei Monotherapie aus so mu13 bei einer “rationalen Pharmakologie” der Wirkungsmechanismus der Antiepileptika in Betracht gezogen werden, um eine bessere Wirkung zu erreichen ohne Nebenwirkungen und einfacher Einnahme. Die Einfuhrung von Vigabatrin (VGB) als neues AED erfordert eine derartige neue Betrachtung. VGB ist eine Substanz die irreversibel die GAbA-Transaminase hemmt und damit im Gehirn und Liquor die GABA-Konzentration bei Tier und Mensch erhoht. Auf die klassischen Tier-Anfallsmodelle hat VGB einen begrenzten Effekt. Bei einer groβen Anzahl klinischer Studien wurde bei etwa 50% der Patienten die Anfallshaufigkeit halbiert, besonders bei Partialepilepsien. In dieser Studie wird die Wirksamkeit von VGB bu sozial integrierten und aktiven ambulznten Patienten gepruft, um den Vorteil einer rationalen Polytherapie aufzuzeigen. In die Auswertung aufgenommen sind folgende Kriterien: Einfluβ auf die Anfallshaufigkeit, Vertraglichkeit, kognitive Leistungen (psychometrischer Test). 14 der 19 Patienten (73%) bildeten die Gruppe und zeigten eine mehr als 50%ige Anfallsreduktion, bei 10 von 10 Patienten (52%) eine mehr als 70%ige Reduktion. Die Vertraglichkeit war gut bis auf Mudigkeit, die die haufigste Nebenwirkung darstellte. 3 Patienten klagten uber eine Verschlechterung der Anfalle, davon einer uber Zunahme und 2 uber zusatzliche myoklonische Zuckungen. Eine Verschlechterung kognitiver Funktionen fand sich nicht sondern eher eine Verbesserung gegen Ende der Behandlungsperiode. Dies konnte auf die verminderte Anfallszahl zuruckzufuhren sein. Die Wirksamkeit von VGB in dieser Studie bezuglich Anfallsverminderung konnte im Gegensatz zu anderen Publikationen mit der Selektion der Patienten namlich mehr beningnen Partialepilepsien zusammenhangen. Andererseits zeigt sich die Notwendigkeit fur weitere Untersuchungen um die Hypothese der rationalen Polytherapie zu untersuchen.
